Background: Linezolid, bedaquiline, and newer fluoroquinolones are currently placed as priority Group A drugs for the treatment of drug-resistant tuberculosis. The number of reported linezolid-resistant clinical strains is still low, and the correlation of molecular determinants with phenotype is not perfect. Methods: We determined the linezolid MICs for clinical isolates from the Moscow region and identified mutations in rplC and rrl genes. Results: All 16 linezolid-resistant isolates had previously reported mutations in the rplC or rrl loci, and 13 of them bore a RplC C154R substitution. Detection of this substitution in a heteroresistant state was not successful, probably, due to the more stable DNA secondary structure of the mutated fragment, which precludes its amplification in mixes with the wild-type DNA. Strains with an rplC mutation had higher linezolid MIC compared to isolates with rrl mutations. Conclusions: Linezolid resistance mostly emerged during treatment with the latest regimen. Three primary cases with linezolid resistance question the possible transmission of totally drug-resistant tuberculosis in the Moscow region, which demands further investigation.

Late blight is one of the most important potato diseases. To minimize yield losses, various protective measures are used including fungicide application. Active use of fungicides results in a contamination of the environment. Therefore, crop protection strategies optimizing the number of treatments are of great interest. Using information about late blight development in an experimental potato field recorded over 30 seasons, a simulator to forecast yield losses caused by the disease was developed based on the number of 5-d periods favorable for reinfection of plants during a vegetation season. The simulator was successfully verified using independent data on the disease development from nine unprotected potato fields in the Netherlands and Germany. The average difference between the calculated and real yield losses did not exceed 5%. Using the simulator and weather data for a period of 2007–2017, yield losses were calculated for several areas of the Bryansk, Tambov, and Orenburg Regions of Russia. The results revealed differences in disease development between these regions and may be used to develop recommendations for a frequency of fungicide applications according to the regional risk of epidemics, leading to a significant reduction in fungicide use.

VNIIFBlight decision support system is intended to identify weather conditions favourable or unfavourable for the late blight outbreaks on potato. To provide an improved choice of fungicide treatment dates, we investigated the use of a standard five-day weather forecast based disease management decision support system VNIIFBlight as well as the use of information about the late blight resistance level of the cultivars used. The efficiency of this decision support system was compared with a seven-day routine program of fungicide treatments in the course of four-year field trials in Moscow region and six-year "virtual" trials in four regions of Russia, three regions of Lithuania, and single regions of Canada, Ukraine, Poland, and the Netherlands. According to the results of field experiments, in the case of a high infection pressure (2004 and 2008), yield losses caused by late blight in the untreated control made up 62% and 39%, respectively, whereas the losses for both compared schemes of protection did not exceed 8.2% for both years. In the case of weak disease development (2005 and 2007), for all studied treatments, including the untreated control, yield losses did not exceed 3%. In all years, the use of the VNIIFBlight decision support system significantly reduced the number of fungicidal treatments as compared to the routine scheme of protection; this reduction in 2004, 2005, 2007 and 2008 made up 62, 50, 50 and 17 %, respectively. According to the results of "virtual" trials, the use of the VNIIFBlight decision support system reduced the number of fungicide treatments of susceptible and resistant potato cultivars by 50% and 60% on average, respectively, as compared with the routine program.

The World Health Organization (WHO) recommends that countries implement pharmacovigilance and collect information on active drug safety monitoring (aDSM) and management of adverse events.The aim of this prospective study was to evaluate the frequency and severity of adverse events to anti-tuberculosis (TB) drugs in a cohort of consecutive TB patients treated with new (i.e. bedaquiline, delamanid) and repurposed (i.e. clofazimine, linezolid) drugs, based on the WHO aDSM project. Adverse events were collected prospectively after attribution to a specific drug together with demographic, bacteriological, radiological and clinical information at diagnosis and during therapy. This interim analysis included patients who completed or were still on treatment at time of data collection.Globally, 45 centres from 26 countries/regions reported 658 patients (68.7% male, 4.4% HIV co-infected) treated as follows: 87.7% with bedaquiline, 18.4% with delamanid (6.1% with both), 81.5% with linezolid and 32.4% with clofazimine. Overall, 504 adverse event episodes were reported: 447 (88.7%) were classified as minor (grade 1–2) and 57 (11.3%) as serious (grade 3–5). The majority of the 57 serious adverse events reported by 55 patients (51 out of 57, 89.5%) ultimately resolved. Among patients reporting serious adverse events, some drugs held responsible were discontinued: bedaquiline in 0.35% (two out of 577), delamanid in 0.8% (one out of 121), linezolid in 1.9% (10 out of 536) and clofazimine in 1.4% (three out of 213) of patients. Serious adverse events were reported in 6.9% (nine out of 131) of patients treated with amikacin, 0.4% (one out of 221) with ethionamide/prothionamide, 2.8% (15 out of 536) with linezolid and 1.8% (eight out of 498) with cycloserine/terizidone.The aDSM study provided valuable information, but implementation needs scaling-up to support patient-centred care.

Gene Expression Atlas (http://www.ebi.ac.uk/gxa) is an added-value database providing information about gene expression in different cell types, organism parts, developmental stages, disease states, sample treatments and other biological/experimental conditions. The content of this database derives from curation, re-annotation and statistical analysis of selected data from the ArrayExpress Archive and the European Nucleotide Archive. A simple interface allows the user to query for differential gene expression either by gene names or attributes or by biological conditions, e.g. diseases, organism parts or cell types. Since our previous report we made 20 monthly releases and, as of Release 11.08 (August 2011), the database supports 19 species, which contains expression data measured for 19 014 biological conditions in 136 551 assays from 5598 independent studies.

Large studies on bedaquiline used to treat multidrug-resistant (MDR-) and extensively drug-resistant tuberculosis (XDR-TB) are lacking. This study aimed to evaluate the safety and effectiveness of bedaquiline-containing regimens in a large, retrospective, observational study conducted in 25 centres and 15 countries in five continents.428 culture-confirmed MDR-TB cases were analysed (61.5% male; 22.1% HIV-positive, 45.6% XDR-TB). MDR-TB cases were admitted to hospital for a median (interquartile range (IQR)) 179 (92–280) days and exposed to bedaquiline for 168 (86–180) days. Treatment regimens included, among others, linezolid, moxifloxacin, clofazimine and carbapenems (82.0%, 58.4%, 52.6% and 15.3% of cases, respectively).Sputum smear and culture conversion rates in MDR-TB cases were 63.6% and 30.1%, respectively at 30 days, 81.1% and 56.7%, respectively at 60 days; 85.5% and 80.5%, respectively at 90 days and 88.7% and 91.2%, respectively at the end of treatment. The median (IQR) time to smear and culture conversion was 34 (30–60) days and 60 (33–90) days. Out of 247 culture-confirmed MDR-TB cases completing treatment, 71.3% achieved success (62.4% cured; 8.9% completed treatment), 13.4% died, 7.3% defaulted and 7.7% failed. Bedaquiline was interrupted due to adverse events in 5.8% of cases. A single case died, having electrocardiographic abnormalities that were probably non-bedaquiline related.Bedaquiline-containing regimens achieved high conversion and success rates under different nonexperimental conditions.
