Introduction and aims: The primary aim was to explore the epidemiologic trend of pediatric inflammatory bowel disease in Latin America, and the secondary aims were to obtain an overview of the diagnostic/therapeutic focus of the members of the LASPGHAN and examine the relation of case frequency to year, during the study period. Materials and methods: Latin American pediatric gastroenterologists participated in an online survey, conducted through the SurveyMonkey platform, that investigated the yearly frequency of new inflammatory bowel disease patients within the time frame of 2005–2016, their disease variety, the gastrointestinal segments affected, and the diagnostic and treatment methods utilized. The correlation of new case frequency with each study year was evaluated. Results: A total of 607 patients were studied. The diagnoses were ulcerative colitis in 475 (78.3%) cases, Crohn’s disease in 104 (17.1%), and inflammatory bowel disease D unclassified in 28 (4.6%). The trend in ulcerative colitis was a lineal increase in the frequency of new cases related to each study year, with a significant correlation coefficient. Pancolitis was found in 67.6% of the patients. The diagnostic methods included clinical data, endoscopy, and biopsies in more than 99% of the cases, and imaging studies were indicated selectively. Drug regimens were limited to 5-aminosalicylic acid derivatives, azathioprine, 6-mercaptopurine, infliximab, and adalimumab. Conclusions: Pediatric inflammatory bowel disease in Latin America appears to have increased during the years included in the study period, with a predominance of moderate or severe ulcerative colitis. That lineal trend suggests the predictive likelihood of a gradual increase in the coming years, with possible epidemiologic and clinical implications. Resumen: Introducción y objetivo: La obesidad se asocia con el hígado graso no alcohólico (HGNA) en niños. El objetivo fue explorar la asociación de factores dietéticos y sociodemográficos con el HGNA en niños y adolescentes obesos. Material y métodos: Treinta y tres pacientes obesos entre seis a 16 años fueron incluidos en este estudio transversal analítico. El diagnóstico de obesidad se realizó con puntuación z del índice de masa corporal > 2 DE. El HGNA se estimó con ultrasonido hepático, aminotransferasas séricas y elastografía. Las variables sociodemográficas se evaluaron con cuestionarios validados. La dieta se estimó mediante dos encuestas dietéticas por recordatorio de 24 horas enfocadas a la cantidad de energía, azúcares simples, ácidos grasos poliinsaturados (PUFAs) y antioxidantes. Resultados: En 13 pacientes (39.4%), se identificó esteatosis hepática por ultrasonido abdominal; 54.2% tuvieron una concentración sérica de alanina aminotransferasa por arriba de la referencia. El nivel educativo superior de ambos padres, el mayor gasto familiar mensual en alimentos y el mayor nivel socioeconómico se asociaron con HGNA. En el grupo total, el consumo de azúcares simples y grasas saturadas fue excesivo y el consumo de vitamina E y PUFAS estuvo por debajo del parámetro. Conclusiones: Se identificó HGNA en un tercio de los casos. Se demostró asociación entre HGNA y variables sociodemográficas. Ambos grupos tuvieron una ingesta aumentada de azúcares simples y disminuida de PUFAs. La ocurrencia selectiva de HGNA se podría relacionar con predisposición genética demostrada en población mexicana. A higher educational level in both parents, greater monthly food expenditure, and a higher socioeconomic level were associated with NAFLD. Overall, simple sugar and saturated fat consumption was above the recommended daily intake, whereas vitamin E and PUFA consumption was below those parameters. Conclusions: NAFLD was identified in one-third of the cases. There was an association between NAFLD and sociodemographic variables. Both groups had an increased intake of simple sugars and a reduced intake of PUFAs. The selective occurrence of NAFLD could be related to a genetic predisposition that has been demonstrated in a Mexican population.

The objective of this study was to validate and measure the internal reliability of the Baby and Child Eating Behavior Questionnaires for Toddlers (BEBQ-Mex and CEBQ-T-Mex), that evaluate appetitive trait (ATs). Mothers recruited from a public hospital in Guadalajara, Mexico, completed the BEBQ-Mex or CEBQ-T-Mex along with information on sociodemographic characteristics. Internal reliability of the BEBQ-Mex was sufficient for Food Responsiveness (FR) (Cronbach α = 0.82), while Enjoyment of Food (EF) and Satiety Responsiveness (SR) showed poor reliability (α = 0.56) and Slowness in Eating (SE) had unacceptable reliability (a = 0.36). All reliability values for the CEBQ-T-Mex were acceptable (>0.70), except for SE (α = 0.64). Confirmatory factor analysis (CFA) revealed an adequate model fit for the BEBQ-Mex, except the SE subscale. CFA for the CEBQ-T-Mex confirmed the six-factor structure. Mothers of a low sociodemographic background were unable to recognize their infants’ ATs; the BEBQ-Mex partly met the criteria for validity and reliability. Mothers from similar sociodemographic characteristics were more able to recognize the ATs of their toddlers than their infants; the CEBQ-T-Mex was found to be a valid and reliable tool. Findings support the need to help mothers’ ability to recognize their infants’ ATs, which have been previously associated with weight and growth.

We aimed to conduct an updated review on the pathophysiology, diagnosis, and nutritional intervention of CCLD and secondary malnutrition in infants. Protein-energy malnutrition, impaired linear growth, fat-soluble vitamin deficiencies, and hepatic osteodystrophy can occur in up to 80% of cases. The proposed pathophysiological mechanisms include insufficient energy intake, lipid- and fat-soluble vitamin malabsorption, increased energy expenditure, altered intermediate metabolism, hormonal dysregulation, and systemic inflammation. The current approach to diagnosis is the identification of the deviation of growth parameters, body composition, and serum concentration of micronutrients, which determines the type and magnitude of malnutrition. Currently, liver transplantation is the best therapeutic alternative for the reversal of nutritional impairment. Early and effective portoenteroanatomosis can extend survival in patients with biliary atresia. Medical and dietary interventions in some storage and metabolic diseases can improve liver damage and thus the nutritional status. A proportion of patients with biliary atresia have fat-soluble vitamin deficiencies despite receiving these vitamins in a water-soluble form. With aggressive enteral nutrition, it may be possible to increase fat stores and preserve muscle mass and growth. The nutritional issues identified in the pre- and post-transplantation stages include muscle mass loss, bone demineralization, growth retardation, and obesity, which seems to correspond to the natural history of CCLD. Due to the implications for the growth and development of infants with CCLD with this complex malnutrition syndrome, innovative projects are required, such as the generation of prediction and risk models, biomarkers of growth and body composition, and effective strategies for nutritional prevention and intervention.

The polyamine content of human breast milk, which is the first exogenous source of polyamines for the newborn, can be affected by several factors associated with the mother, the infant, or breastfeeding itself. The aim of this study was to evaluate the influence of different breastfeeding factors on the polyamines found in human milk. For this study, a cohort of 83 mothers was considered for up to 4 months, and a subgroup of 33 mothers were followed during the first six months of breastfeeding. Two breast milk samples were collected at each sampling point (foremilk and hindmilk) and the polyamine content was determined by UHPLC-FL. Polyamine levels varied considerably between the mothers and tended to decrease over time. Putrescine was the minor polyamine, whereas spermidine and spermine contents were very similar. The concentrations of the three polyamines were significantly higher in hindmilk than foremilk (p &lt, 0.001). Spermidine and spermine levels decreased significantly through the lactation progress (p &lt, 0.05). Finally, slightly higher levels of polyamines were observed in the milk of mothers providing partial, rather than full, breastfeeding, although the differences were not significant. The polyamine content in human milk was found to change during a single feed (foremilk versus hindmilk) and as lactation progressed, mainly in response to the specific circumstances of the newborn.

ObjectiveWe aimed to study the correlation between neck circumference (NC) and anthropometric adiposity indicators, and to determine cut-off points of NC for both sexes to identify elevated central adiposity in schoolchildren in western Mexico.DesignCross-sectional study.SettingRural settings in western México.ParticipantsChildren from a convenience sample of six schools in Acatlán, Jalisco, Mexico (n1802).ResultsNC showed a strong positive correlation with all anthropometric adiposity indicators in both sexes, which were notably higher in boys regardless of age. Noteworthy, waist circumference displayed the highest significant correlation when analysed by both age and sex. As age increased, NC cut-off points to identify elevated central adiposity ranged from 25·7 to 30·1 cm for girls and from 27·5 to 31·7 cm for boys.ConclusionsNC could be used as a simple, inexpensive and non-invasive indicator for central obesity assessment in Mexican schoolchildren.

Aim: To identify and discuss the efficacy of dietary interventions, antioxidant supplementation, physical activity, and nutritional and psychologic counseling in the treatment of children and adolescents with non alcoholic fatty liver disease associated with obesity. Materials and methods: A scoping review of studies on nutritional and educational interventions and physical activity in pediatric patients with non alcoholic fatty liver disease was conducted. A search for randomized clinical trials or quasi-experimental studies published up to December 2017 was carried out, utilizing seven databases (Medline, EBSCO, OVID, Science Direct, JSTOR, Wiley, and Biblioteca Digital UDG). Results: From a total of 751 articles, 729 were excluded due to the criteria of age, design, language, diagnostic method, and outcome variables. The analysis included 22 articles. The most frequently used intervention variables were diet and physical activity. The interventions had different durations, but most were carried out for one year. Some authors employed ascorbic acid, vitamin E, or omega-3 fatty acid supplementation. There were varying degrees of improvement in the variables analyzed in the majority of the studies, such as a decrease in ALT levels, a reduced frequency of steatosis determined through imaging studies, and a decrease in body mass index. Conclusions: The dietary interventions, omega-3 fatty acid supplementation, physical activity, and nutritional and psychologic counseling were identified as efficacious measures in the treatment of non alcoholic fatty liver disease associated with obesity in children and adolescents, according to biochemical or imaging study indicators, within the time frame of the intervention. El lactante de 12 a 24 meses es un foco de atención debido a que se encuentra en un periodo de edad de gran vulnerabilidad a la mala nutrición y/o al establecimiento de malos hábitos de alimentación; en consecuencia, es crucial identificar las prácticas de alimentación de los infantes durante los primeros 24 meses de edad. En este estudio, transversal analítico, se reportan las prácticas de lactancia materna y alimentación complementaria de 211 lactantes, de 15 a 24 meses de edad, separados por género. Se obtuvieron las características del lactante, las prácticas de lactancia materna y de alimentación complementaria, el consumo de bebidas azucaradas y de alimentos ultraprocesados. Se utilizaron las pruebas t de Student no pareada, chi cuadrada y se estimó la razón de momios (IC95%). Las madres prolongaron más la lactancia materna en varones. En 99% de los lactantes se observó una ingestión excesiva de alimentos ultraprocesados, y los mismos se consumían con mayor frecuencia (seis veces/semana). La frecuencia de consumo de bebidas azucaradas fue de cinco veces por semana en ambos géneros. La cantidad de bebidas azucaradas consumidas al día fue significativamente mayor en varones y tenían cuatro veces mayor probabilidad de consumir las mismas [RM= 4.9 (IC95% 1.22, 19.5), p=0.02] y mayor probabilidad de consumir jugos procesados [RM=1.94 (IC 95% 0.92, 4.09), p=0.078] que las niñas. En la población estudiada se observó un consumo excesivo de alimentos ultra-procesados y bebidas azucaradas, especialmente entre los varones. Infants aged 12 to 24 months are a focus of attention because they are in an age period of great vulnerability to poor nutrition and/or the establishment of bad eating habits; consequently, it is crucial to identify infant feeding practices during the first 24 months of age. In this analytical, cross-sectional study, breastfeeding and complementary feeding practices of 211 infants, aged 15 to 24 months, separated by gender, are reported. Infant characteristics, breastfeeding and complementary feeding practices, consumption of sugary drinks and ultra-processed foods were obtained. The unpaired Student t-tests, chi-square were used and the odds ratio (95% CI) was estimated. Mothers prolonged breastfeeding in boys. Excessive ingestion of ultra-processed foods was observed in 99% of infants, and they were consumed more frequently (six times / week). The frequency of consumption of sugary drinks was five times a week in both genders. The amount of sugary drinks consumed per day was, not only significantly higher in boys, but also they were four times more likely to consume this kind of beverages [RM = 4.9 (95% CI 1.22, 19.5), p = 0.02], as well as processed juices [RM = 1.94 (95% CI 0.92, 4.09), p = 0.078] than girls. In this studied population excessive consumption of ultra-processed and sugary drinks was observed, especially among males.

The main objective was to explore the relationship between the microbiota of human milk and adiposity in Mexican mothers during the first lactation stage. Methods: Seventy lactating women were included. Adiposity by anthropometric measurements and by bioelectric impedance was obtained. The donation of human milk was requested, from which bacterial DNA was extracted and qPCR of the 16S region was performed. The Mann–Whitney U test, Spearman and Pearson correlations, and multiple linear regressions models were also calculated. Results: The median percentage of Bacteroidetes had a direct and significant correlation with normal adiposity, current BMI, waist circumference, and body fat percentage. The correlation with current BMI became significantly inverse in women with BMI ≥ 25. In women with normal BMI, the percentage of Actinobacteria showed a direct and significant correlation with current BMI, waist circumference, and percentage of body fat. Multiple linear regressions showed that pre-pregnancy BMI was the variable with the highest predictive value with the Bacteroidetes phyla in normal BMI and in BMI ≥ 25. Conclusions: the adiposity of the woman before pregnancy and during lactation would have an important effect on the abundance of Bacteroidetes and Actinobacteria in human milk.

Neck circumference was studied for the first time in a pediatric population in 2010. Since then, various countries have proposed cutoff values to identify overweight, obesity, and metabolic syndrome. However, no reference values have been established for the Mexican child population. The aim of this study is to provide percentile reference values for the neck circumference of Mexican schoolchildren. Only normal-weight schoolchildren aged 6–11 years were included. Percentiles and growth charts were constructed based on the “Generalized Additive Model for Location, Scale and Shape” (GAMLSS). A total of 1059 schoolchildren (52.9% female) was evaluated. Weight, height, and BMI values were higher for males, however, this difference was not statistically significant. The 50th percentile for females was 24.6 cm at six years old and 28.25 cm at 11 years old, and for males, it was 25.75 cm and 28.76 cm, respectively. Both males and females displayed a pronounced increase in neck circumference between 10 and 11 years of age. The greatest variability was found in the 11-year-old group, with an increase of 5.5 cm for males and 5.4 cm for females. This study presents the first reference values for neck circumference for a Mexican child population.

OBJECTIVE To compare and correlate anthropometric measurements of the left arm versus the right arm and to establish their relation to laterality, sex, and nutritional status in children and adolescents from 6 to 12 years of age. METHODS A cross-sectional study included 605 children and adolescents (285 males; 320 females), with a middle socioeconomic status, that attended an urban elementary school. Anthropometric measurements were taken in both arms, along with height and weight, to assess nutritional status. Self-reported hand preference was recorded. RESULTS A total of 49.3% were females and 93% of the subjects were right-handed (n = 563). The mean comparison of right arm and left arm measurements and areas, according to laterality, showed differences in arm fat measurements, with higher values in the right arm. However, the mean difference in triceps skinfold and upper arm measurements between the right arm and left arm was smaller than the technical error of measurement. Significant differences were observed between arm measurements and areas. Upper arm and total upper arm areas of both arms had the highest regression coefficients, with an adjusted R2 of .98, whereas the arm fat index had the lowest (adjusted R2 of .88). Adjusting the previous differences between the right arm and left arm by age, sex, BMI, and laterality produced similar results. CONCLUSIONS Statistical but not clinically significant differences were found between some measurements and areas of both arms, discretely modified in models adjusted for age and BMI, so the measurements can be made on either arm.

Objetivo: presentar los resultados de una encuesta aplicada a pediatras gastroenterologos de Latinoamerica para conocer como realizan el abordaje diagnostico y terapeutico de pancreatitis. Material y metodos: estudio transversal, encuesta exploratoria, multicentrica. Resultados: el 73,6 % definio los tres tipos de pancreatitis de acuerdo con el grupo INSPPIRE. La enfermedad biliar, trauma abdominal e ingesta de medicamentos fueron la etiologia mas frecuente. El 80 % fueron pancreatitis leve. El 73 % utilizo soporte nutricional; la nutricion enteral fue la mas empleada. Conclusion: es el primer estudio exploratorio que permite tener una estimacion del abordaje diagnostico y terapeutico de la pancreatitis por PG en Latinoamerica.

IIntroduction: maternal employment after childbirth is associated with abandonment of breastfeeding; however, lactation rooms in the workplace increase the practice of breastfeeding. Objective: to compare the frequency and duration of breastfeeding among working mothers based on the presence of lactation rooms in their workplaces. Methods: we included mothers from different institutions whose infants were between 6 and 35 months (n = 158), and an ad hoc questionnaire was applied to assess breastfeeding, exclusive breastfeeding (EBF), partial breastfeeding (PBF), and use of human milk substitutes (HMS). Two groups were compared: working mothers with a lactation room at their workplace (n = 76) versus working mothers without this resource (n = 82). Results: breastfeeding duration (7.5 vs. 5.0 months, p0.001) and EBF (3.0 vs. 1.2 months, p = 0.005) were higher in mothers who had a lactation room. HMS use was shorter in mothers who had a lactation room (2.5 vs. 10.0 months, p = 0.001). There were more working mothers who breastfed for more than six months (75.0 % vs. 48.8 %) [OR = 3.15 (95 % CI, 1.60-6.19), p = 0.001] and 12 months (31.6 % vs. 14.6 %) [OR = 2.69 (95 % CI, 1.23-5.87), p = 0.014] when lactation rooms were available in their workplaces. Conclusion: the presence of a lactation room in the workplace was associated with a higher frequency and duration of breastfeeding.RESUMEN Introducción: el empleo materno después del parto se asocia con el abandono de la lactancia materna, mientras que las salas de lactancia en el lugar de trabajo aumentan la práctica de la lactancia. Objetivo: comparar la frecuencia y duración de la lactancia materna entre madres trabajadoras en función de la presencia o no de salas de lactancia en sus lugares de trabajo. Métodos: incluimos madres de diferentes instituciones cuyos bebés tenían entre 6 y 35 meses (n = 158) y se aplicó un cuestionario ad hoc para evaluar la lactancia materna, la lactancia materna exclusiva (LME), la lactancia materna parcial (LMP) y el uso de sucedáneos de la leche humana (SLH). Se compararon dos grupos: madres trabajadoras con una sala de lactancia en su lugar de trabajo (n = 76) y madres trabajadoras sin este recurso (n = 82). Resultados: la duración de la lactancia (7,5 vs. 5,0 meses, p0,001) y LME (3,0 vs. 1,2 meses, p = 0,005) fueron mayores en las madres que tenían sala de lactancia. El uso de SLH fue más corto en las madres que tenían sala de lactancia (2,5 vs. 10,0 meses, p = 0,001). Hubo más madres trabajadoras que amamantaron más de seis meses (75,0 % vs. 48,8 %) [OR = 3,15 (IC 95 %: 1,60-6,19), p = 0,001] y 12 meses (31,6 % vs. 14,6 %) [OR = 2,69 (IC 95 %: 1,23-5,87), p = 0,014] cuando había salas de lactancia disponibles en sus lugares de trabajo. People who live with diabetes consume sucralose to control their blood glucose, but there is a controversy about this topic. To evaluate the relationship between sucralose consumption and serum concentration of glycosylated hemoglobin in people with Type 2 Diabetes Mellitus without complications. Cross-sectional study. Universe of 27 214 people with Type 2 Diabetes Mellitus without complications, users of a primary care unit from the Instituto Mexicano del Seguro Social in the state of Jalisco, Mexico. Simple probabilistic sample, n = 194 (p = 0,05). Propositive sampling. Selection criteria: adults of any gender and education level who agreed to participate. Variables: sociodemographic, anthropometric, clinical and dietary. Data collection instruments: Sociodemographic questionnaire, Tanita Fitscan© 585 scale, Tanita Fitscan© HR-200 stadiometer, Body Flex© tape-measure, Slim Guide© plicometer, Afinion© AS100 analyzer, and Frequency of Food Consumption Questionnaire. Information sources: clinical files and Mexican System of Equivalent Foods. Analysis: descriptive and inferential statistics (p ≤ 0,05). 194 people. Mean age 60,23 ± 11,16, interval 28-93 years. 56,2% females and 43,8% males. Difference between glycosilated hemoglobin means: sucralose consumers 7,5% ± 1,7%, no sucralose consumers 8,1% ± 2,1% (p < 0,01). Association force “sucralose consumption/high glycosilated hemoglobin concentration” OR = 1,42 (CI95% 0,63, 3,21). Lineal correlation “quarterly sucralose consumption/serum concentration of glycosylated hemoglobin” ρ = -0,754 (R2 = 0,0057, p = 0,333). This results were partially consistent to the pre-existing literature. Studies with representative stratified samples and control of dietary variables are required for better results.

Introduction Probiotics are frequently prescribed in clinical practice. Their efficacy in treating gastrointestinal disorders is supported by a significant number of clinical trials. However, the correct prescription of these agents is hampered due to a lack of knowledge of the scientific evidence and to the different presentations and microbial compositions of the probiotics that are currently available. Aim To provide the clinician with a consensus review of probiotics and recommendations for their use in gastroenterology. Materials and methods Controlled clinical trials, meta-analyses, and systematic reviews published up to 2015 were selected, using the MESH terms: probiotics, gastrointestinal diseases, humans, adults, AND children. The Delphi method was employed. Eighteen gastroenterologists treating adult patients and 14 pediatric gastroenterologists formulated statements that were voted on until agreement of >70% was reached. The level of evidence based on the GRADE system was evaluated for each statement. Results and conclusions Eleven statements on the general concepts of probiotics and 27 statements on the use of probiotics in gastrointestinal diseases in both adults and children were formulated. The consensus group recommends the use of probiotics under the following clinical conditions: the prevention of diarrhea associated with antibiotics, the treatment of acute infectious diarrhea, the prevention of Clostridium difficile infection and necrotizing enterocolitis, the reduction of adverse events from Helicobacter pylori eradication therapy, relief from irritable bowel syndrome symptoms, the treatment of functional constipation in the adult, and the induction and maintenance of remission in patients with ulcerative colitis (UC) and pouchitis, and the treatment of covert and overt hepatic encephalopathy.

Resumen Introduccion El uso de los probioticos es comun en la practica clinica. Existe un numero significativo de estudios que apoyan la eficacia de los probioticos en algunos trastornos digestivos. Sin embargo, el desconocimiento de la evidencia cientifica y las diferentes presentaciones y composiciones microbianas de los probioticos disponibles dificultan su prescripcion. Objetivo Proveer al clinico de una revision consensuada sobre los probioticos y recomendaciones de su uso en gastroenterologia. Material y metodos Se seleccionaron los ensayos clinicos controlados, metaanalisis y revisiones sistematicas publicados hasta 2015, usando los terminos MESH: probiotics, gastrointestinal diseases, humans, adults and children. Se utilizo la metodologia Delphi. Diecisiete gastroenterologos de adultos y 12 de ninos elaboraron enunciados los cuales fueron votados hasta obtener acuerdo > 70%. Para cada enunciado se evaluo el nivel de evidencia basado en el sistema GRADE. Resultados y conclusiones Se generaron 11 enunciados sobre conceptos generales de probioticos y 27 enunciados sobre uso de probioticos en enfermedades gastrointestinales tanto en ninos como en adultos. El grupo de consenso recomienda el uso de probioticos en las siguientes condiciones clinicas: prevencion de la diarrea asociada a antibioticos, tratamiento de la diarrea aguda infecciosa, prevencion de infeccion por Clostridium difficile y enterocolitis necrosante, para disminuir los eventos adversos de la terapia de erradicacion del Helicobacter pylori, el alivio de los sintomas del sindrome de intestino irritable, en el estrenimiento funcional del adulto, para inducir y mantener la remision en pacientes con colitis ulcerosa cronica idiopatica y pouchitis, y en la encefalopatia hepatica oculta y manifiesta.

It has been accepted that satiety‐ and appetite‐stimulating hormones play a role in the regulation of food intake and body composition during and after the lactation stage. Therefore, the purpose was to demonstrate that serum appetite‐regulating hormones in infants differ according to anthropometric indicators and type of feeding. In a nonrandom cohort study, 169 mother–newborn dyads whose pregnancy and birth were attended at the Hospital Civil de Guadalajara were enrolled. According to the type of feeding, infants were classified as full breastfeeding (FBF), partial breastfeeding (PBF), and infants receiving human milk substitutes (HMS). Serum concentrations of ghrelin (pg/ml), leptin (ng/ml), peptide YY (pg/ml), and glucagon‐like peptide‐1 (GLP‐1) (pM) were measured. Anthropometric measurements including weight, length, cephalic, arm circumference, tricipital, and subscapular skinfolds were obtained. Weight/age, weight/height, height/age, and BMI Z‐score indexes were estimated. We performed one‐way ANOVA, unpaired Student's t test, post hoc Tukey test, and Pearson correlation tests. The ANOVA comparison of the three feeding types showed significant differences in most anthropometric indicators (z‐scores), especially between infants receiving FBF versus HMS and particularly on indicators of adiposity; no differences were observed in length and cephalic circumference z‐scores at 8th and 16th weeks. Further, significant correlations were found between most of the adiposity indicators with ghrelin, leptin, and GLP‐1, especially in infants who received FBF. There were differences in anthropometric and body composition parameters among infants receiving FBF, PBF, and HMS. There were significant correlations between body composition indicators with ghrelin, leptin, and GLP‐1 mainly in infants receiving FBF., This was a comparative and correlational analysis of a nonrandom cohort study of 169 infant/mother dyads. We measured appetite‐regulating hormones and performed anthropometry related to growth and body composition; the analysis was carried out according to the type of feeding. We found differences in body composition according to the type of feeding and correlations of anthropometric indicators with ghrelin, leptin, and GLP‐1, especially in infants who received full breastfeeding.

espanolObjetivo: se han observado diferencias entre ninas y varones en la prevalencia de desnutricion en diferentes partes del mundo, lo que apunta a la discriminacion contra las ninas, incluso con respecto a la lactancia materna completa. El objetivo fue mostrar que la lactancia completa es menos comun en las ninas en una poblacion de estratos socioeconomicos medio-bajo y bajo. Metodos: se realizo un analisis transversal de una muestra de 170 diadas madre-lactante segun el tipo de alimentacion (74 de lactancia completa, 57 de lactancia parcial y 39 sucedaneos de la leche humana) en el Nuevo Hospital Civil de Guadalajara. Variables dependientes segun el tipo de alimentacion: lactancia completa (exclusiva y/o predominante), lactancia parcial y sucedaneos de leche humana. Variables independientes: datos demograficos, escolaridad, ocupacion de madres y/o padres e ingresos familiares. Se utilizaron pruebas de Kruskal-Wallis, Mann-Whitney U y Chi-cuadrado y razon de momios. Resultados: la probabilidad de lactancia materna completa fue 3,8 veces menor en ninas que en varones. De manera no significativa, la probabilidad de lactancia completa fue menor que la de lactancia parcial y la frecuencia de lactancia completa fue menor que la combinacion de lactancia parcial y sucedaneos de leche humana en ninas. La frecuencia de lactancia completa y lactancia parcial fue menor que los sucedaneos de leche humana y la lactancia parcial fue menos frecuente que los sucedaneos de leche humana en ninas. Conclusion: hay un caracter diferenciado en el privilegio de ofrecer lactancia materna completa. Es cuatro veces menor en las ninas que en varones. EnglishObjective: differences have been shown between males and females in terms of the prevalence of malnutrition in different parts of the world, which point to discrimination against females, including with respect to full breastfeeding. Therefore, the objective was to show that exclusive breastfeeding is less common for females in a population of medium-low and low socioeconomic strata. Methods: this was a cross-sectional analysis of a sample of 170 mother-infant dyads according to type of feeding (74 full breastfeeding, 57 partial breastfeeding and 39 human milk substitutes) at the Nuevo Hospital Civil de Guadalajara. Dependent variables according to type of feeding: full breastfeeding (exclusive and/or predominant), partial breastfeeding, and human milk substitutes. Independent variables: demographic data, schooling, occupation of mothers and/or parents, and family income. Kruskal-Wallis, Mann-Whitney U and Chi-square tests and odds ratio were used. Results: the probability of full breastfeeding was 3.8 times lower in females than in males. In a non-significant way, the likelihood of full breastfeeding was lower than that of partial breastfeeding, and full breastfeeding was lower than the combination of partial breastfeeding and human milk substitutes in females. Full breastfeeding and partial breastfeeding were lower than human milk substitutes, and partial breastfeeding was lower than human milk substitutes in females. Conclusion: there is a differentiated character in the privilege of full breastfeeding; it is four times lower in females than in males.

Introduction: children with cerebral palsy (CP) have multiple risk factors for low bone mineral density or osteoporosis. Objective: to explore the association between bone mineral density (BMD) and biochemical and hormonal indicators of bone metabolism in children with quadriplegic cerebral palsy (CP). Methods: a cross-sectional analytical study included 59 participants from six to 18 years of age with quadriplegic CP. Serum concentrations of calcium, phosphorus, 25OHD metabolite, parathyroid hormone (PTH), alkaline phosphatase, and thyroid hormones were determined using standardized methods. The BMD measurement was obtained from the lumbar spine expressed in g/cm2 and Z-score. Unpaired Student's t-test, Chi-square test, odds ratio and Pearson's correlation were performed. Results: participants with CP and malnutrition had lower serum concentrations of calcium, phosphorus and alkaline phosphatase. Those who had low BMD showed lower serum concentrations of calcium, phosphorus and alkaline phosphatase. Most participants with low and normal BMD had vitamin D deficiency (27.1% and 10%) and insufficiency (35.4% and 30%), respectively. There was a significant correlation between BMD and serum concentrations of calcium, phosphorus, alkaline phosphatase, vitamin D and thyroid-stimulating hormone (TSH). There were no differences in the biochemical and hormonal indicators by level of gross motor function, use of anticonvulsants and oral versus enteral feeding method. Conclusion: malnutrition and alteration of vitamin D nutritional status were associated with low BMD and alterations of biochemical indicators of bone metabolism in pediatric patients with quadriplegic CP. The relationship between BMD and biochemical indicators of bone metabolism in children with quadriplegic CP was also demonstrated.Introducción: los niños con parálisis cerebral (PC) presentan múltiples factores de riesgo de densidad mineral ósea baja u osteoporosis. Objetivo: explorar la asociación entre la baja densidad mineral ósea (DMO) e indicadores bioquímicos y hormonales del metabolismo óseo en niños con PC cuadripléjica. Métodos: un estudio transversal analítico incluyó a 59 participantes de entre seis y 18 años de edad con PC cuadripléjica. Las concentraciones séricas de calcio, fósforo, metabolito 25OHD, hormona paratiroidea (PTH), fosfatasa alcalina y hormonas tiroideas se determinaron utilizando métodos estandarizados. La medición de DMO se obtuvo de la columna lumbar expresada en g/cm2 y puntaje Z. Se realizaron pruebas t de Student no pareada, Chi-cuadrado, razón de momios y correlación de Pearson. Resultados: los participantes con PC y desnutrición tenían concentraciones séricas más bajas de calcio, fósforo y fosfatasa alcalina. Los participantes con DMO baja tuvieron menor concentración sérica de calcio, fósforo y fosfatasa alcalina. Los participantes con DMO baja y normal tenían deficiencia de vitamina D (27.1% y 10%) e insuficiencia (35.4% y 30%), respectivamente. Hubo una correlación significativa entre DMO y las concentraciones séricas de calcio, fósforo, fosfatasa alcalina, vitamina D y hormona estimulante de la tiroides. Conclusión: la desnutrición y la alteración del estado nutricio de la vitamina D se asociaron con DMO baja y alteraciones de los indicadores bioquímicos del metabolismo óseo. Se demostró una asociación entre DMO e indicadores bioquímicos y hormonales del metabolismo óseo en niños con PC cuadripléjica.

Introduction: the purpose of this study was to evaluate the relationship of arm anthropometric indicators with direct indicators of nutritional status in hospitalized pediatric patients. Methods: an analytical cross-sectional study with 760 patients hospitalized in the Pediatric Division of the Nuevo Hospital Civil de Guadalajara during 2014 was used. The anthropometric indices were weight/length, weight/height, weight/age, length/age, height/age, head circumference/age and body mass index (BMI)/age. The arm indicators were mid-upper arm circumference (MUAC), total arm area (TAA), arm muscle area (AMA), arm fat area (AFA) and fat percentage (FP). The ANOVA, Kruskall-Wallis, Mann-Whitney U and Pearson's correlation tests and also odds ratios were used to identify the probability of nutritional status impairment. Results: the prevalence of acute and chronic malnutrition was higher in infants (31% and 30%, respectively). With arm areas (TAA, AMA, AFA), the risk of deficit (≤ -2DE) was higher in infants and early preschoolers (p0.001). The correlation between the anthropometric indexes and the arm areas was direct and significant (p0.001). The BMI variability was explained in 68% by the AMA, AFA, and FP (p0.001); the variability of the height/age index was also explained in 34% by the AMA and AFA (p0.001). Conclusion: it is possible to diagnose both a chronic and acute deficit using the indirect indicators of the arm, while the body mass index only reflects an acute deficit. Therefore, arm areas would be more useful indicators in the assessment of nutritional status and the diagnosis of chronic-acute malnutrition in hospitalized pediatric patients.Introducción: el objetivo de este estudio fue evaluar la relación de los indicadores antropométricos de brazo con los indicadores directos del estado de nutrición en pacientes pediátricos hospitalizados. Métodos: se utilizó un estudio transversal analítico con 760 pacientes ingresados en la División de Pediatría del Nuevo Hospital Civil de Guadalajara durante 2014. Los índices antropométricos fueron peso/longitud, peso/altura, peso/edad, longitud/edad, altura/edad, circunferencia cefálica e IMC. Los indicadores del brazo fueron circunferencia media del brazo (CMB), área total del brazo (ATB), área muscular del brazo (AMB), área grasa del brazo (AGB) y porcentaje de grasa. Se utilizaron las pruebas de ANOVA, Kruskall-Wallis, U de Mann-Whitney, correlación de Pearson y razón de momios para identificar la probabilidad de deterioro del estado nutricional. Resultados: la prevalencia de desnutrición aguda y crónica fue mayor en lactantes (31% y 30%, respectivamente). Con las áreas del brazo (ATB, AMB, AFA), el riesgo de déficit (≤ -2 DE) fue mayor en lactantes y preescolares tempranos (p0,001). La correlación entre los índices antropométricos y las áreas del brazo fue directa y significativa (p0,001). La variabilidad del IMC fue explicada en un 68% por AMB, AGB y porcentaje de grasa (p0,001); la variabilidad del índice de talla/edad también fue explicada en un 34% por AMB y AGB (p0,001). Conclusión: es posible diagnosticar el déficit crónico y agudo utilizando los indicadores indirectos del brazo, mientras que el IMC solo refleja un déficit agudo. Las áreas de brazo serían indicadores más útiles en el diagnóstico de desnutrición crónica-aguda en pacientes pediátricos hospitalizados.

espanolObjetivo: correlacionar indicadores de crecimiento y del estado nutricional con la concentracion serica de interleucina 6 (IL-6) y proteina C-reactiva ultrasensible (PCR) en ninos con enfermedad renal cronica (ERC). Metodos: estudio transversal analitico de 29 ninos y adolescentes de 3 a 16 anos de edad con ERC, estadios 3 o 4, en dos hospitales generales de tercer nivel. Las variables dependientes fueron indicadores antropometricos de crecimiento y del estado nutricional y la composicion corporal por impedancia bioelectrica. Las variables independientes fueron IL-6 y PCR. Este estudio fue revisado y aprobado por los Comites de Etica y de Investigacion de ambos hospitales. Resultados: la talla para la edad (T/E), el indice de masa corporal, el pliegue cutaneo subescapular, el area de grasa del brazo y la masa magra obtuvieron una correlacion negativa con la IL-6. La T/E obtuvo una correlacion negativa con la PCR. La IL-6 explico el 15% y 35% de la varianza de la T/E y de los indicadores del estado nutricional. La CRP predijo el 22% de la T/E. Una quinta parte de los pacientes tenia desnutricion aguda y una tercera parte desmedro. El compartimento corporal mas afectado fue el muscular. Conclusion: la IL-6 y la PCR en ninos y adolescentes con ERC en etapa predialitica explicaron una quinta y una tercera parte de la varianza de los indicadores de desnutricion aguda y cronica, respectivamente. La frecuencia de la desnutricion aguda y el desmedro fueron clinicamente significativos. El deficit de masa muscular fue un componente central de la desnutricion. EnglishObjective: secondary malnutrition and systemic inflammation may impair growth and body composition in children and adolescents with chronic kidney disease (CKD). This association has been scantily studied, particularly in pre-dialytic stages. Our aim was to correlate growth and nutritional status indicators with the serum concentration of interleukine 6 (IL-6) and ultrasensitive C-reactive protein (CRP) in children with CKD. Methods: this was a prospective cross-sectional study in 29 children and adolescents aged 3-16 years with CKD, stages 3 or 4, in two third-level general hospitals. The outcome variables were height for age, body mass index, arm anthropometric indicators, plus lean mass/fat percentage by bioelectrical impedance. The independent variables were IL-6 and CRP. This study was reviewed and approved by the Health Research and Ethics Committees of both hospitals. Results: height for age, body mass index, subscapular skinfold, arm fat area, and lean mass had a significant negative correlation with IL-6. The height-for-age z-score had a negative correlation with CRP. IL-6 explained 15% to 35% of the variance in height for age and nutritional status indicators. CRP predicted 22% of height for age. One fifth of the patients had acute malnutrition, and one third were stunted. Muscle was the most affected compartment. Conclusion: IL-6 and CRP in children and adolescents with CKD in the pre-dialytic stage predicted one fifth and one third of the variance in acute and chronic malnutrition indicators. The frequency of acute malnutrition and impaired growth was clinically significant. Muscular mass deficit was a central component of malnutrition.

espanolIntroduccion: los ninos con paralisis cerebral (PC) presentan multiples factores de riesgo de densidad mineral osea baja u osteoporosis. Objetivo: explorar la asociacion entre la baja densidad mineral osea (DMO) e indicadores bioquimicos y hormonales del metabolismo oseo en ninos con PC cuadriplejica. Metodos: un estudio transversal analitico incluyo a 59 participantes de entre seis y 18 anos de edad con PC cuadriplejica. Las concentraciones sericas de calcio, fosforo, metabolito 25OHD, hormona paratiroidea (PTH), fosfatasa alcalina y hormonas tiroideas se determinaron utilizando metodos estandarizados. La medicion de DMO se obtuvo de la columna lumbar expresada en g/cm2 y puntaje Z. Se realizaron pruebas t de Student no pareada, Chi-cuadrado, razon de momios y correlacion de Pearson. Resultados: los participantes con PC y desnutricion tenian concentraciones sericas mas bajas de calcio, fosforo y fosfatasa alcalina. Los participantes con DMO baja tuvieron menor concentracion serica de calcio, fosforo y fosfatasa alcalina. Los participantes con DMO baja y normal tenian deficiencia de vitamina D (27.1% y 10%) e insuficiencia (35.4% y 30%), respectivamente. Hubo una correlacion significativa entre DMO y las concentraciones sericas de calcio, fosforo, fosfatasa alcalina, vitamina D y hormona estimulante de la tiroides. Conclusion: la desnutricion y la alteracion del estado nutricio de la vitamina D se asociaron con DMO baja y alteraciones de los indicadores bioquimicos del metabolismo oseo. Se demostro una asociacion entre DMO e indicadores bioquimicos y hormonales del metabolismo oseo en ninos con PC cuadriplejica. EnglishIntroduction: children with cerebral palsy (CP) have multiple risk factors for low bone mineral density or osteoporosis. Objective: to explore the association between bone mineral density (BMD) and biochemical and hormonal indicators of bone metabolism in children with quadriplegic cerebral palsy (CP). Methods: a cross-sectional analytical study included 59 participants from six to 18 years of age with quadriplegic CP. Serum concentrations of calcium, phosphorus, 25OHD metabolite, parathyroid hormone (PTH), alkaline phosphatase, and thyroid hormones were determined using standardized methods. The BMD measurement was obtained from the lumbar spine expressed in g/cm2 and Z-score. Unpaired Student’s t-test, Chi-square test, odds ratio and Pearson’s correlation were performed. Results: participants with CP and malnutrition had lower serum concentrations of calcium, phosphorus and alkaline phosphatase. Those who had low BMD showed lower serum concentrations of calcium, phosphorus and alkaline phosphatase. Most participants with low and normal BMD had vitamin D deficiency (27.1% and 10%) and insufficiency (35.4% and 30%), respectively. There was a significant correlation between BMD and serum concentrations of calcium, phosphorus, alkaline phosphatase, vitamin D and thyroid-stimulating hormone (TSH). There were no differences in the biochemical and hormonal indicators by level of gross motor function, use of anticonvulsants and oral versus enteral feeding method. Conclusion: malnutrition and alteration of vitamin D nutritional status were associated with low BMD and alterations of biochemical indicators of bone metabolism in pediatric patients with quadriplegic CP. The relationship between BMD and biochemical indicators of bone metabolism in children with quadriplegic CP was also demonstrated.

espanolObjetivo: evaluar la relacion entre la concentracion de suero y la leche materna y la concentracion de grelina y leptina en leche materna en madres lactantes segun el tipo de alimentacion. Metodos: diseno de cohorte realizado en 131 diadas madre-lactante que ingresaron en una sala de puerperio fisiologico. Variables independientes: tipo de alimentacion, lactancia materna completa (LMC, 56,5%) y lactancia materna parcial (LMP, 43,5%). Variables dependientes: concentracion serica de grelina total (pg/ml) y leptina (ng/ml), leche humana pre-tetada y pos-tetada a las ocho y 16 semanas. Se utilizaron pruebas no pareadas t de Student, U de Mann-Whitney, correlacion de Pearson, coeficiente de determinacion y regresion linear. Resultados: la concentracion de grelina y leptina en leche humana pre-tetada fue mayor que en leche humana pos-tetada en ambos grupos a las ocho y 16 semanas. La concentracion de grelina y leptina fue mayor en suero que en leche humana en ambos grupos; estos valores mostraron una correlacion lineal directa y significativa con la excepcion de la grelina en el grupo de LMC. La concentracion serica de leptina en las madres explico el 32% de la varianza de su concentracion en leche humana en LMC y el 13% en madres en LMP. Conclusion: el gradiente de leche humana pre-tetada/pos-tetada sugiere un mecanismo de regulacion e ingestion de leche. La concentracion de grelina y leptina fue mayor en suero que en leche humana y los coeficientes de determinacion sugieren una transferencia de suero-leche, ademas de una regulacion de la sintesis por la glandula mamaria, el tejido adiposo u otros organos. EnglishObjective: we assessed the relationship between serum and human foremilk and hindmilk concentrations of ghrelin and leptin in nursing mothers according to the type of feeding. Methods: this cohort design was carried out on 131 mother-newborn dyads admitted to a physiological puerperium ward. The independent variables were the type of feeding, full breastfeeding (FBF, 56.5%) and partial breastfeeding (PBF, 43.5%). The dependent variables were the concentration of total ghrelin (pg/ml) and leptin (ng/ml) in serum, foremilk and hindmilk at eight and 16 weeks. Fasting blood samples were obtained from the nursing mothers at four months for serum assays. Unpaired Student’s t-test, Mann-Whitney U test, Pearson’s correlation tests, coefficient of determination and linear regression were used. Results: the concentration of ghrelin and leptin in hindmilk was higher than that of foremilk in both groups at eight and 16 weeks. The concentration of ghrelin and leptin was higher in serum than in foremilk in both groups. These values showed a direct and significant linear correlation with the exception of ghrelin in the FBF group. The serum concentration of leptin in mothers explained 32% of the variance of its concentration in foremilk in the FBF and 13% in the PBF groups. Conclusion: the hindmilk/foremilk gradient suggests an intake regulating mechanism during the fed. The concentration of ghrelin and leptin was higher in the serum than in foremilk and its correlation and determination coefficients could suggest plasma-milk transfer in addition to synthesis regulation by the mammary gland, adipose tissue or other organs.

Satiety and appetite‐stimulating hormones play a role in the regulation of food intake. Breastfed infants may have a different profile of serum appetite‐regulating hormones than formula‐fed infants. We propose to demonstrate that the serum concentration of appetite regulatory hormones differs according to the type of feeding and that there is a correlation between the serum concentrations of these hormones in mothers and in infants at 4 months of age. In a cross‐sectional analysis, 167 mother–newborn dyads at the Hospital Civil de Guadalajara were enrolled: 74 full breastfeeding (FBF), 56 partial breastfeeding (PBF), and 37 receiving human milk substitutes (HMS). Serum levels of ghrelin (pg/ml), leptin (ng/ml), peptide YY (pg/ml), and glucagon‐like peptide‐1 (GLP‐1) (pM) were measured. We performed one‐way analysis of variance, unpaired Student t test, post hoc Tukey test, and Pearson correlation. The total sample at 16 weeks postpartum included 167 dyads. The mean age was 16 ± 1 weeks. The concentrations of GLP‐1 (pM) and peptide YY (pg/ml) were higher in the FBF group (42.6 and 442.9) than in the HMS group (35.2 and 401.9), respectively, p = 0.046 and p = 0.056. And, the FBF group had higher correlation coefficients of ghrelin (r = 0.411 vs. 0.165), GLP‐1 (r = 0.576 vs. 0.407), and peptide YY (r = 0.218 vs. 0.067), respectively, than the HMS group. The concentrations of GLP‐1 and peptide YY were higher in the FBF group when compared with the HMS group. Mother–infant dyads fed by FBF had more significant direct correlations of appetite‐regulating hormones than those who received HMS.

The aim of this study was to compare the effectiveness of oral (PO) versus enteral nutrition (EN) medium-chain triglyceride (MCT) containing-formula to prevent malnutrition and growth impairment in infants with biliary atresia (BA) waiting for a liver transplant.A total of 15 infants, 3 to 9 months old with BA were included. They were randomly assigned to either PO or EN. For 12 weeks, both groups received an MCT formula fortified with glucose polymers and corn oil to reach a caloric density between 0.8 and 1 kcal/mL. The formula given to the PO group was administered ad libitum and that given via EN was infused through a nasogastric tube to reach 140% of the energy intake recommended by the Dietary Recommended Intake guidelines. Protein intake was adjusted to 4 to 5 g/kg present weight. Outcome variables were growth and nutritional status evaluated periodically by anthropometric indicators. Biochemical and hematological variables were evaluated through the study.Baseline clinical, nutritional, biochemical, and hematological variables showed no differences between the study groups. Baseline length/age was-2 SD in 10 of the 15 patients; in the PO group, it fell-3 SD, whereas in the EN group, it remained stable. Head circumference z score dropped 0.6 SD in the PO group, whereas in the EN group it remained stable. Triceps skinfold values improved in the infants taking EN, P0.001. The frequency of adverse effects--respiratory infection and diarrhea--was higher in the EN group. No biochemical or hematological differences were observed between the study groups throughout the study.A 12-week EN trial with an MCT-fortified formula prevented malnutrition and growth impairment in infants with BA waiting for a liver transplant.

Objective: the purpose was to evaluate the nutritional status in hospitalized children according to their socio-demographic variables, underlying pathology and based on hospital length of stay. Methods: seven-hundred and sixty-three hospitalized patients of the Civil Hospital of Guadalajara Dr. Juan I. Menchaca were included in a cross-sectional study. The following anthropometric indices were taken: weight/length, weight/height, weight/age, length/age, height/age, head circumference/age and BMI/age. Data of socio-demographic variables and underlying pathology were also obtained. Student's t-test, Chi-squared test, ANOVA with post-hoc tests and logistic regression were used to identify the likelihood of risk of malnutrition. Results: subjects were divided according to their hospital length of stay in lower than seven days and seven days or more. At the time of admission, children with stay of seven days or more showed a greater deficit in the height/age index (OR = 1.77 [95% CI 1.16-2.7], p = 0.007), head circumference (OR = 2.68 [95% CI 1.5-4.7], p ≤ 0.001) and BMI (OR = 2.9 [95% CI 1.9-4.4], p ≤ 0.001). Males had higher risk of moderate/severe malnutrition (OR = 1.87 [95% CI 1.22-2.85], p = 0.003) and higher deficit in head circumference (OR = 1.89 [95% CI 1.06-3.37], p = 0.029). Infants with gastrointestinal and respiratory problems had higher frequency of malnutrition at the time of admission than did those with other pathologies (p < 0.001). Conclusion: the pathological disease at time of admission and longer hospital length of stay increased the likelihood of malnutrition in male infants and preschool-aged children. There were demographic and educational risk factors in the home environment that could influence acute and chronic malnutrition.

This study demonstrated the relationship of low bone mineral density (BMD) with the degree of motor impairment, method of feeding, anthropometric indicators, and malnutrition in children with quadriplegic cerebral palsy (CP). The control of these factors could optimize adequate bone mineralization, avoid the risk of osteoporosis, and would improve the quality of life. The purpose of the study is to explore the relationship between low BMD and nutritional status in children with quadriplegic CP. A cross-sectional analytical study included 59 participants aged 6 to 18 years with quadriplegic CP. Weight and height were obtained with alternative measurements, and weight/age, height/age, and BMI/age indexes were estimated. The BMD measurement obtained from the lumbar spine was expressed in grams per square centimeter and Z score (Z). Unpaired Student’s t tests, chi-square tests, odds ratios, Pearson’s correlations, and linear regressions were performed. The mean of BMD Z score was lower in adolescents than in school-aged children (p = 0.002). Patients with low BMD were at the most affected levels of the Gross Motor Function Classification System (GMFCS). Participants at level V of the GMFCS were more likely to have low BMD than levels III and IV [odds ratio (OR) = 5.8 (confidence interval [CI] 95% 1.4, 24.8), p = 0.010]. There was a higher probability of low BMD in tube-feeding patients [OR = 8.6 (CI 95% 1.0, 73.4), p = 0.023]. The probability of low BMD was higher in malnourished children with weight/age and BMI indices [OR = 11.4 (1.3, 94), p = 0.009] and [OR = 9.4 (CI 95% 1.1, 79.7), p = 0.017], respectively. There was a significant relationship between low BMD, degree of motor impairment, method of feeding, and malnutrition. Optimizing these factors could reduce the risk of osteopenia and osteoporosis and attain a significant improvement of quality of life in children with quadriplegic CP.

The aim of this paper was to estimate the adiposity trend in children from a public elementary school in Guadalajara, Mexico, between 2007 and 2011.Comparative study of two samples taken transversely in 1432 schoolchildren aged 6-12 years. Z-score of BMI and anthropometric indicators of arm adiposity (triceps skinfold, fat area and arm fat index) were calculated. Reference standards of the 2007-WHO and Frisancho were used. Analysis: Chi-square and Student t test.Adiposity decreased significantly in children with normal weight (arm fat area Z-score -0.3 SD) overweight and obesity (arm fat area and index -0.4 and -0.3 z-scores respectively). An increase in the prevalence of subjects with normal weight and thinness and a decrease of overweight (4.6, -2.0 and -2.9 percentage points respectively) was observed. Obesity prevalence remained unchanged.Decrease of adiposity in overweight and obese children, reduction in the prevalence of thinness and overweight and the increase in the proportion of subjects with normal weight was observed during the study period. These data probably reflect the nutritional transition in Mexico.Introducción: el objetivo de este estudio fue estimar la tendencia de la adiposidad de alumnos de una escuela primaria pública entre 2007 y 2011. Métodos: estudio comparativo de dos muestras obtenidas transversalmente en 1432 escolares de 6 a 12 años de edad. Se calculó la puntuación z del índice de masa corporal y de indicadores antropométricos de adiposidad (pliegue cutáneo tricipital, área grasa e índice de grasa del brazo). Se utilizaron los patrones de referencia de la OMS-2007 y de Frisancho. El análisis se llevó a cabo con Chi cuadrada y t de Student. Resultados: la adiposidad disminuyó significativamente tanto en niños con peso normal (z área grasa del brazo -0.3) como en los que tenían sobrepeso y obesidad (z área grasa del brazo -0.4 y -0.3, respectivamente). Se observó un incremento en la prevalencia de niños con peso normal (4.6 puntos porcentuales), así como disminución de casos con delgadez y sobrepeso (-2.0 y -2.9 puntos porcentuales, respectivamente). La prevalencia de obesidad no se modificó. Conclusiones: se identificó disminución de la adiposidad en niños con sobrepeso y obesidad, disminución en la prevalencia de delgadez y sobrepeso, así como aumento en la proporción de sujetos con peso normal. Estas observaciones probablemente reflejan la transición nutricia en México.

Introduction: Caustic ingestion (CI) in children and adolescents may lead to esophageal burns, esophageal stenosis and secondary dysphagia. These complications may limit the normal feeding process leading to malnutrition and growth impairment. Aims: Our aim was to evaluate the nutritional status and its association with dysphagia and esophageal stenosis in children with CI. Methods: Sixty-two patients with caustic ingestion treated at a pediatric referral hospital were included in this cross-sectional study. Independent variables were dysphagia/normal swallowing and esophageal stenosis/normal barium-swallow. The dependent variables were growth and nutritional status evaluated by anthropometry. Analysis: c2 test, OR, 95% CI, kappa test and Student’s t-test. Results: The average age at the time of CI was 39.7 months; 38.7% of the patients were girls. Endoscopy performed upon admission revealed erosive esophagitis (II-b, III-a, and III-b) in 46 (77.8%) of the patients, dysphagia in twenty-four (38.7%) and esophageal stenosis in forty (64.5%). Both complications occurred simultaneously in 20 children (32.3%, kappa = 0.3, p = 0.014).The z-score of height-for-age was below -2 SD in five children (8.1%). The z score of body mass index (BMI) was-2 SD in three children (4.8%) and it was above +1 SD in 24.2%. The z-score means of the arm anthropometric indicators of fat stores and muscle mass in both the dysphagia and esophageal stenosis groups were located in the negative area of the z-score curve and their values differed significantly from the z-scores of the non-dysphagia and non-stenosis groups. Conclusions: The proportion of erosive esophagitis, esophageal stenosis and dysphagia was high.
