Parathyroid adenoma is the most common cause of primary hyperparathyroidism. Giant parathyroid adenoma is considered a rare subtype of parathyroid adenomas that weigh more than 3.5 grams and cause a small number of adenoma related hyperparathyroidism. We present a case of 68-year-old asymptomatic male patient who had hypercalcemia and markedly high parathyroid hormone level on routine blood testing. Radiological investigations demonstrated a large mediastinal mass that showed peripheral high uptake on sestamibi scan. Histopathology confirmed the diagnosis of giant parathyroid adenoma. Giant parathyroid adenomas can have an atypical radiological appearance and present as a mediastinal mass that resembles other mediastinal masses, making it important to include in the list of possible diagnoses.

Purpose: To evaluate the ability of acoustic radiation force impulse (ARFI) elastography in differentiating benign from malignant etiologies of splenomegaly based on differences in splenic stiffness. Materials and Methods: Between September 2020 and November 2022, we evaluated 40 patients with splenomegaly—defined by a splenic long axis greater than 13 cm and/or a short axis greater than 6 cm, without visible focal or infiltrative mass lesions—using abdominal ultrasound at our university hospital. Each patient also underwent a standardized ARFI elastographic assessment of the enlarged spleen, with data collected prospectively. We then retrospectively analyzed the cases with confirmed etiologies of splenomegaly from their final medical reports. Mean ARFI velocities (MAV) were compared across patients with splenomegaly due to malignant infiltration (MIS) from hematological malignancy, congestive splenomegaly (CS) due to portal or splenic vein congestion/occlusion, and immune-related splenomegaly (IRS) associated with systemic infectious or autoimmune diseases. Results: Among the 40 patients with splenomegaly, 21 (52.5%) were diagnosed with malignant infiltrative splenomegaly (MIS), 11 (27.5%) with congestive splenomegaly (CS), and 8 (20%) with immune-related splenomegaly (IRS). The mean ARFI velocities (MAV) for the MIS, CS, and IRS groups were 3.25 ± 0.68 m/s, 3.52 ± 0.47 m/s, and 2.84 ± 0.92 m/s, respectively. No significant differences were observed in splenic stiffness (MAV) among these groups. Conclusions: Differentiating between benign and malignant etiologies of splenomegaly based on stiffness differences observed in ARFI elastography is not feasible. Larger prospective studies are necessary to validate these findings.

Analysis was conducted in SPSS with p < 0.05 as the significance threshold. Results: In our study of 500 patients (mean age 46.1 years, 61.5% female), key gender differences emerged. Females had a higher prevalence of low back pain (61.9% vs. 38.1%, p = 0.024) and hip pain (100%, p = 0.008), as well as greater anxiety about disease progression (62.2% vs. 37.8%, p = 0.045) and fear of disability (64.2% vs. 35.8%, p = 0.048). Females also reported lower mental health (p = 0.036), sleep quality (p = 0.044), and overall quality of life (p = 0.019). In contrast, males showed higher workload (54.4% vs. 45.6%, p = 0.020), more work-related injuries (82.8%, p < 0.001), and greater disability (p = 0.024) with lower functional status (p = 0.041). These findings underscore significant gender-specific needs in MSK care. Conclusion: Our study reveals notable gender-based differences in musculoskeletal complaints and treatment experiences in a Jordanian tertiary setting. Females reported higher rates of low back and hip pain, more frequent referrals, and lower quality of life, while males experienced greater occupational strain, work-related injuries, and disability. This study examines the impact of animated graphics versus text-based learning formats on students' anxiety, engagement, comprehension, and academic performance in an object-oriented programming (OOP) course. Sixty university students were divided into two groups: one using animated graphics and the other text-based media. Results revealed that students in the group using animated graphics had significantly lower levels of anxiety and higher levels of engagement, confidence, and ease of comprehension than those in the group using text-based material. Positive correlations were observed between engagement and comprehension, as well as between confidence and performance, in the animated graphics group. These results highlight the potential of animated graphics to enhance learning experiences by reducing anxiety and improving academic performance in technical subjects such as OOP. The lessons learned from this study can guide educators in integrating multimedia tools to optimize student learning and promote student well-being. [ABSTRACT FROM AUTHOR]

Simple Summary: This study investigated how pulmonary metastases in the lungs can be assessed using a special imaging technique called contrast-enhanced ultrasound (CEUS). We analyzed data from 54 patients with confirmed cancer spread to the lungs. Our goal was to identify patterns of blood flow in the lung tumors and to compare these with the findings from tissue samples stained to highlight blood vessels. The results showed that most lung tumors had a blood supply from the bronchial arteries, with a few showing a supply from the pulmonary arteries. Additionally, most tumors showed a rapid reduction in contrast enhancement. This research helps improve the understanding of blood flow in lung tumors and could aid in developing better diagnostics strategies. Purpose: Description of the perfusion of pulmonary metastasis by contrast-enhanced ultrasound (CEUS) and their correlation with vascularization patterns represented by immunohistochemical CD34 endothelial staining. Patients and methods: The data of 54 patients with histologic proven peripheral pulmonary metastasis, investigated between 2004 and 2023 by CEUS. These CEUS parameters were evaluated: time to enhancement (TE), categorized as early pulmonary-arterial (PA) or delayed bronchial-arterial (BA) patterns; extent of enhancement (EE), either marked or reduced; homogeneity of enhancement (HE), homogeneous or inhomogeneous; and decrease of enhancement (DE), rapid washout (<120 s) or late washout (≥120 s). Additionally, tissue samples in 45 cases (83.3%) were stained with CD34 antibody for immunohistochemical analysis. Results: In total, 4 lesions (7.4 %) exhibited PA enhancement, and 50 lesions (92.6%) demonstrated BA enhancement. Furthermore, 37 lesions (68.5%) showed marked enhancement, while 17 lesions (31.5%) exhibited reduced enhancement. The enhancement was homogeneous in 28 lesions (51.86%) and inhomogeneous in 26 lesions (48.14%). Additionally, 53 lesions (98.1%) displayed a rapid washout. A chaotic vascular pattern indicative of a bronchial arterial blood supply was identified in all cases (45/45, 100%), including all 4 lesions with PA enhancement. Conclusion: Pulmonary metastases in CEUS predominantly reveal bronchial arterial enhancement and a rapid washout. Regarding EE and HE, pulmonary metastases show heterogeneous perfusion patterns. A PA enhancement in CEUS does not exclude BA neoangiogenesis. [ABSTRACT FROM AUTHOR]

Purpose To describe the clinical awareness and acceptance of ultrasound-diagnosed acute epiploic appendagitis (EA) and their importance to avoid unnecessary therapeutic and imaging measures. Patients and Methods The data were obtained of n=54 patients with acute, localized, peritonitic pain and EA diagnosed by B-mode ultrasound and contrast-enhanced ultrasound examination from November 2003 to September 2020. All examinations were performed by a German Society for Ultrasound in Medicine (DEGUM) Level III qualified examiner. Based on documentation by the treating physicians, the clinical awareness and acceptance of EA diagnosis was determined in all patients and compared between subgroups diagnosed before 2013 and from 2013 onwards. In 2013, a local educational training program regarding the diagnosis of and therapy for EA was initiated for physicians. Results In all patients, EA was sonographically diagnosed by a DEGUM level III qualified examiner. At enrollment, EA was mentioned as a suspected clinical diagnosis in n=1/54 (1.9%) patient. Furthermore, in n=39/54 (72.2%) cases, the EA was documented and accepted by the treating physicians at the time of patient discharge as the final clinical diagnosis. The clinical acceptance was significantly higher from 2013 onwards compared with before 2013 (p0.05). Conclusion In our retrospective study, we showed that awareness and acceptance of the disease EA are low. Low diagnostic acceptance of EA by the clinician leads to unnecessary therapeutic and imaging measures and is a general problem related to rare diseases in the healthcare system.

Background: Tinnitus is a common complaint that carries a significant impact on life. However, there is paucity of data regarding this condition in Jordan. Our study is the first study to demonstrate the prevalence of tinnitus in Jordan and its characteristics. Methods: A sample of 1060 participant's representative of the Jordanian population was invited to answer a questionnaire. The questionnaire comprised of 15 questions about tinnitus and its characteristics. The data was collected over a year (2016-2017). Results: The prevalence of tinnitus was 28.8% with 72.8% reporting non-pulsatile tinnitus, 95.5% reporting intermittent tinnitus and 57.4% had bilateral tinnitus. There was no significant difference found between males and females (p value: 0.786). There was significant correlation between tinnitus, vertigo, and dizziness (p value < 0.0001). Conclusion: Tinnitus is a common condition with high prevalence in Jordan denoting the significance of diagnosis and offering treatment especially for debilitating cases. [ABSTRACT FROM AUTHOR]
