Abstract

Aim To evaluate the impact of antiplatelet therapy (APT)on the incidence of hepatocellular carcinoma (HCC) and mortality following its treatment. Methods A systematic literature search was performed using PubMed and Cochrane Central Register of Controlled Trials Databases. Two HCC clinical settings were explored: (i) incidence, and (ii) death after any HCC treatment. Odds ratios (OR) and 95% confidence intervals (95%CI) were calculated to compare the pooled data between patients who received or did not receive APT. Results A total of 20 studies were identified, of whom 15 focused on HCC incidence, including 2,685,009 patients, and five on post-treatment death, including 3281 patients. APT was associated with an overall reduced risk of HCC incidence (OR: 0.63; 95%CI = 0.51-0.79; p < 0.001) as well as of post-treatment mortality (OR: 0.54; 95%CI = 0.35-0.83; p = 0.006). Conclusions Current data suggest that APT correlated with higher HCC incidence and poor overall survival following tumour treatment.

Abstract

Growing interest is recently being focused on the role played by the platelets in favoring hepatocellular cancer (HCC) growth and dissemination. The present review reports in detail both the experimental and clinical evidence published on this topic. Several growth factors and angiogenic molecules specifically secreted by platelets are directly connected with tumor progression and neo-angiogenesis. Among them, we can list the platelet-derived growth factor, the vascular endothelial growth factor, the endothelial growth factor, and serotonin. Platelets are also involved in tumor spread, favoring endothelium permeabilization and tumor cells' extravasation and survival in the bloodstream. From the bench to the clinics, all of these aspects were also investigated in clinical series, showing an evident correlation between platelet count and size of HCC, tumor biological behavior, metastatic spread, and overall survival rates. Moreover, a better understanding of the mechanisms involved in the platelet-tumor axis represents a paramount aspect for optimizing both current tumor treatment and development of new therapeutic strategies against HCC. Abstract

Prognostic assessment of patients with hepatocellular carcinoma (HCC) at the time of diagnosis remains controversial and becomes even more complex at the time of restaging when new variables need to be considered. The aim of the current study was to evaluate the prognostic utility of restaging patients before proceeding with additional therapies for HCC. Two independent Italian prospective databases were used to identify 1,196 (training cohort) and 648 (validation cohort) consecutive patients with HCC treated over the same study period (2008-2015) who had complete restaging before decisions about additional therapies. The performance of the Italian Liver Cancer (ITA.LI.CA) prognostic score at restaging was compared with that of the Barcelona Clinic Liver Cancer, Hong Kong Liver Cancer, and Cancer of the Liver Italian Program systems. A multivariable Cox survival analysis was performed to identify baseline, restaging, or dynamic variables that were able to improve the predictive performance of the prognostic systems. At restaging, 35.3% of patients maintained stable disease; most patients were either down-staged by treatment (27.2%) or had disease progression (37.5%). The ITA.LI.CA scoring system at restaging demonstrated the best prognostic performance in both the training and validation cohorts (c-index 0.707 and 0.722, respectively) among all systems examined. On multivariable analysis, several variables improved the prognostic ability of the ITA.LI.CA score at restaging, including progressive disease after the first treatment, Model for End-Stage Liver Disease at restaging, and choice of nonsurgical treatment as additional therapy. A new ITA.LI.CA restaging model was created that demonstrated high discriminative power in both the training and validation cohorts (c-index 0.753 and 0.745, respectively). Conclusion: Although the ITA.LI.CA score demonstrated the best prognostic performance at restaging, other variables should be considered to improve the prognostic assessment of patients at the time of deciding additional therapies for HCC.

Sono introdotti i contributi al convegno, i cui atti sono raccolti nel volume, presentati e discussi nella sessione dedicata alla leale collaborazione fra istituzioni dell’Unione Europea e istituzioni regionali e locali. I lavori sono ricondotti a uno schema che li collega uno con l’altro e con l’architettura generale del convegno (il secondo organizzato dalla rivista Diritti regionali). Sono infine anticipati alcuni interrogativi che la lettura dei contributi ha suscitato.

40. A STRANGE CASE OF PULMONARY HYPERTENSION

Abstract

PH is a haemodinamic and pathophysiological state that can be found in multiple clinical conditions.Chronic hemolytic anemia as sickle cell disease (SCD) is complicated by PH with 30 % of prevalence and worse prognosis.PH in SCD is multifactorial including vasoconstriction, proliferation, obstructive remodelling of the pulmonary vessel wall; inflammation and thrombosis in situ play a role in the pulmonary vascular disease.We have observed a 44–yr old woman affected by sickle cell who developed dyspnoea (WHO–FC III) without chest pain or fever. Transthoracic ecocardiography revealed moderate–severe tricuspid with a gradient of 70 mmHg with an assumed right atrial pressure di 20 mmHg estimated by inferior vena cava, right atrial and ventricular dilatation, reduced TAPSE 16 mm, Ryan index 0.8 with normal size and ejection fraction of left ventricular.Electrocardiogram provided right axial deviation.Computed angiography was normal and ventilation /perfusion lung scan showed an elevated probability for CTEPH. BNP was 921 pg /ml and lung diffusion capacity for carbon monoxide was slight decreased .Prophylactic transfusion during crisis to decrease the frequency of these and new direct anticoagulants were started. After two weeks the patient was in WHO–FC II and transtoracic echocardiography showed a slight tricuspidal regurgitation with a pressure gradient of 40 mmHg with TAPSE of 20 mm and Ryan index was normal.In the new guidelines SCD is moved into Group 5 and our patient was not subjected to specific therapy for pulmonary hypertension. DOAC has showed a reasonable alternative to VKAs for VTE.
