Scop. Studierea mortalității bolnavilor de tuberculoză prin boli asociate și alte cauze. Materiale și metode. A fost efectuată o analiză comparativă, retrospectivă a 222 cazuri de deces prin tuberculoză, boli asociate și alte cauze în mun. Chișinău în două perioade de timp: perioada I (2001-2003) - 157 și perioada II (2018- 2020) - 65 cazuri. Rezultate. În perioada II comparativ cu perioada I a avut loc o deplasare a vârstei spre 65+ (peste 1/3 cazuri); apro- ximativ ½ cazuri de decese au fost din cazurile de retratament; a predominat forma infiltrativă de tuberculoză; a crescut patologia oncologică; a scăzut numărul bolilor hepatice, au apărut cazuri de deces după ulcerul gastric perforant și COVID-19. În ambele perioade bolnavii au decedat și de alte cauze: În perioada I - septicemie - 2; accdente rutiere -15 , îngheț - 8 și suicide - 2 cazuri; În perioada II- peritonite -2, înec - 2, îngheț - 2, supradozarea drogurilor- 2 și suicid - 1 caz. Concluzie. Cele mai frecvente boli asociate care au contribuit la decesul bolnavilor de tuberculoză au fost: patologia oncologică, bolile sistemului cardiac, ictusul cerebral.

Background: In the Republic of Moldova almost 5% of the cases with tuberculosis are diagnosed annually among diabetic patients. The aim of this study was to assess the impact of diabetes mellitus on the evolution and anti-tuberculosis treatment effectiveness in a prospective study. Material and methods: A prospective, longitudinal and case-control study, which included a total number of 252 patients diagnosed with pulmonary tuberculosis and distributed in a study group, consisting of 93 patients diagnosed with diabetes mellitus and a control group, consisting of 159 patients without glycemic disorders, was performed. Results: This study identified that one half of the group with diabetes was detected by active screening and one third received anti-tuberculous treatment before actual episode. A similar rate of diabetic and non-diabetic patients was microbiologically positive, as well confirmed with drug-resistance. The anti-tuberculous treatment effectiveness was lower in diabetic patients, the death rate and the low treatment outcome (lost to follow-up and failed) were higher than in non-diabetic patients. The main causes of unfavorable evolution were: glycemic disorders (hyperglycemia), diabetes complications and the history of the anti-tuberculous treatment in the anamnesis. Conclusions: The individualized approach and a tight follow-up should be performed regularly in all patients with glycemic disorders and tuberculosis for the improvement of the disease outcome.

Background: The surge demand and decreased availability of the health workers determined the designation of the Clinical Municipal Hospital of Phthisiopneumology (CMHP) into a hospital for the case-management of COVID-19 patients during the period of December 2020 – April 2021 for the treatment of the mild cases. The aim of the study was to assess the main peculiarities of the patients with COVID-19 through a cross-sectional study developed during the period February-April 2021 in the CMHP. Material and methods: A cross-sectional, analytical and prospective study was realized, which included 145 patients with COVID-19 hospitalized in the CMHP during February-April 2021. The inclusion criteria were: patient older 18 years, COVID-19 infection diagnosed by the pulmonologist with the positive polymerase chain reaction result for SARS-CoV-2 (COVID-19) and signed informed consent. Results: The peculiarities of patients with COVID-19 were dominated by the female sex, age over 60 years, residence in the urban districts of Chisinau and the infection acquired in the Republic of Moldova. Common clinical manifestations were: high temperature, profuse sweats, asthenia and headaches, productive cough, dyspnea, loss of smell or taste, and sore throat. The typical radiological findings such ground-glass pattern or consolidation with bilateral multifocal involvement were most frequent. Were cured most of the patients and the death rate was low. Conclusions: Peculiarities and clinical manifestations of the patients admitted in CMHP were typical for the COVID-19 infection. These findings did not show the real outcome of the standard case-management of the COVID-19 due to the restricted admission criteria.

Детский туберкулез имеет первичный генез - является следствием первичного контакта организма ребенка с источником инфекции. В большинстве случаев у детей инфекция остается латентной и только при неблагоприятных условиях перерастает в свою активную фазу. Иммунный статус является решающим фактором в этом процессе. В этой статье мы хотим проанализировать функциональную активность иммунокомпетентных клеток детского организма в процессе противостояния Mycobacterium tuberculosis.

Au fost studiate datele a 112 bolnavi, care au decedat prin tuberculoză pe parcursul a 3 ani (2018-2020) în Spitalul Municipal de Ftiziopneumologie din Chişinău. Bolnavii decedaţi au fost repartizaţi în două loturi: lotul 1 — 73(65,9%) din cazurile noi de tuberculoză pulmonară şi lotul 2 — 39(34,8%) cazuri din re-tratament. S-a constatat, că 55(75,5%) pacienţi din lotul 1 şi 22(56,4%) — din lotul 2 au decedat prin progresarea TB şi complicaţiile ei. Bolnavii au decedat prin alte boli în 18 (24,7%) din lotul 1 şi în 17(45,6%) cazuri — din lotul 2. În ambele loturi a predominat vârsta între 41 şi mai mult de 60 ani, persoanele neangajate, solitare, cu forme grave de tuberculoză (generalizată, pneumonie cazeoasă), fără loc de trai — 36(32,1%), cu co-infecţia TB/HIV — 26(23,21%), alcoolism — 58(51,8%) decedaţi. Din staţionarele somatice în stare gravă au fost transferaţi — 107(95,5%) pacienţii şi în 2/3 cazuri decesul a survenit în prima lună după internare

Objectives. The national policy allows tuberculosis patients to benefit from 3 types of treatment options: 1. community directly observed treated when the treatment is delivered close to the patient’s home, which is most extended. 2. directly observed treatment administrated in hospitals. 3. video-observed treatment. The purpose of the study was to assess the peculiarities of patients treated with video-observed compared with directly observed and the treatment effectiveness. Material and methods. A prospective, case-control study, conducted in the 2020 year, which included 276 patients, from which 78 were treated remotely and included in the video-observed group and 198 cases were monitored by a trained health care worker when the patient swallow every dose, and included in the directly observed group. Results. The remote treatment can be provided if the patient has an electronic device, wideband internet, have no contraindications, as deep social vulnerability, and mental impairment as a consequence of psychic diseases, alcohol abuse, and drug use. The complexity of the patient’s risk factors endangers the treatment outcome, which did not differ significantly compared with patients treated with directly-observed treatment. Conclusions. Video-observed treatment can be implemented at the national level in the actual epidemiological conditions of the Republic of Moldova if supporting measures for patients will be performed.

Pacienții cu maladii psihice și tulburările mentale și comportamentale au un risc sporit de îmbolnăvire de tuberculoză.Scopul studiului a constat în identificarea particularităţilor și a factorilor de risc de îmbolnăvire de tuberculoză la paciențiicu tulburări mentale şi de comportament. Material şi metodologie: a fost realizat un studiu de tip selectiv, descriptiv, analiticşi retrospectiv în care au fost incluși 71 bolnavi de tuberculoză diagnosticați cu tulburări mentale şi de comportament înregistrați în perioada 2015-2019. Rezultate studiului au stabilit că particularitățile pacienților au constituit sexul masculin (58(81,7%) cazuri), vîrsta între 35 și 64 de ani (48 (67,9%) cazuri). Factorii de risc de îmbolnăvire de tuberculoza au constituitstatutul socio-economic redus (67 (85,9%) cazuri), condițiile paupere de trai – 58 (81,7%) cazuri, contactul tuberculos – 12(16,9%) și patologiile asociate, inclusiv infecția HIV – 11 (15,5%) cazuri. Deși localizarea pulmonară a predominat la 61(85,9%) cazuri, confirmarea microbiologică s-a stabilit doar la 12 (16,9%) cazuri. Succesul terapeutic s-a constatat doar la37 (52,1%) cazuri și decesul a survenit la 20 (28,1%). Concluzionăm, că managementul standardizat al pacienților cu tulbuări mentale și comportament constituie o provocare pentru finalizarea cu succes a tratamentului antituberculos a majorităţiicovîrşitoare a cazurilor, iar individualizarea conform necesităților pacientului va îmbunătăți rezultatul terapeutic.

Scopul studiului a constat în identificarea particularităţilor clinico-microbiologice pacienților diagnosticați cu tuberculoză. Material şi metodologie: a fost realizat un studiu analitic şi retrospectiv, ce a inclus 380 cazuri noi de tuberculozădiagnosticate în anul 2020. Rezultate: particularitățile pacienților au constituit sexul masculin, vîrsta între 25 și 44 de ani,statutul socio-economic redus, contactul tuberculos și patologiile asociate. Baciliferi au fost 17,4%, GeneXpert MTB/Rifpozitivi - 21,6%, TB RR/MDR s-a confirmată la 33 (8,7%) cazuri. Au finalizat cu succes tratamentul 71,1%, au decedat12,1% și alte rezultate nesatisfăcătoare la 10,8%. Concluzionăm, că deși indicatorii epidemiologici s-au redus particularitățile pacienților confirmă gravitatea situației epidemiologice în Chișinău.

Au fost supuse analizei comparative datele a 164 bolnavi decedați din retratament prin progresarea tuberculozei înstaționarele de profil în două perioade: I perioadă – 120 (73,2%) în anii 2001-2003 (perioada introducerii strategii DOTS)și a II-a perioadă – 44 (26,8%) bolnavi decedați în anii 2018-2020. S-a constatat că în perioada II comparativ cu perioadaI au decedat mai mulți bolnavi cu vârsta de peste 55 ani, a crescut de 6 ori numărul decedaților cu co-infecția HIV, eșeculterapeutic de 7 ori. A crescut formele infiltrativă și generalizată de tuberculoză și a scăzut forma fibro-cavitara. Peste ½din bolnavii decedați au avut tuberculoză rezistentă, inclusiv XDR (13,6%)

Obiective. Controlul tuberculozei reprezintă o prioritate a sistemului de sănătate a Republicii Moldova, iar Programul Național de Control al Tuberculozei a inclus obiectivele strategiei „End TB”. Scopul studiului a fost evaluarea impactului factorilor de risc asupra incidenței tuberculozei în municipiul Chișinău. Material și metode. A fost realizat un studiu retrospectiv, descriptiv, care a evaluat particularitățile a 380 pacienți, cazuri noi de tuberculoză, diagnosticați în perioada anului 2020, în municipiul Chișinău. Rezultate. Din grupul factorilor socio-economici a predominat vulnerabilitatea socială la două treimi din pacienți și nivelul foarte redus de educație școlară la fiecare al zecelea. Din factorii de risc epidemiologici au predominat pacienții dificil de accesat (persoanele fără loc de trai și persoanele migrante), cât și contacții cu bolnavi de tuberculoză. Din categoria factorilor medico-biologici au predominat pacienții co-infectați HIV și cu deprinderi nocive (alcoolism și narcomanie). Pozitivi la examenele microbiologice au fost fiecare al cincilea, cu destrucții parenchimatoase în ambii plămâni fiecare al doilea și cu ambii plămâni afectați fiecare al treilea. Tratamentul a fost administrat standardizat, etapizat, pentru tipul de caz nou și s-a soldat cu succes terapeutic în 71,1%, cu deces la 12,1% cazuri, iar pierderea din supraveghere sau eșecul terapeutic la 10,8% din cazuri. Concluzii. Au demonstrat impactul asupra incidenței tuberculozei în municipiul Chișinău factorii de risc socio-economici, grupurile pacienților dificil de accesat, prezența deprinderilor nocive, care s-au reflectat asupra ponderii înalte a formelor destructive și rezultatului terapeutic suboptimal. Se recomandă: a fortifica depistarea activă, luând în considerație structura grupurilor cu risc sporit de îmbolnăvire ce predomină în municipiul Chișinău; a utiliza pe larg metodele centrate pe pacient și implementarea tehnologiilor videoasistate pentru asigurarea nivelului înalt al complianței terapeutice.

Background: It is known that the main barriers in the anti-tuberculosis treatment delivery are social, economic, educational and psychological issues. According to the estimations the Republic of Moldova (RM) remains a high risk zone showing an inadequate concern regarding social determinants that represent the risk factors for achieving high treatment outcome. Tuberculosis is concentrated in areas with high density of the population, poor environmental and sanitation conditions: poverty, food insecurity, low living conditions. Material and methods: A retrospective selective, descriptive study of socioeconomic, epidemiological peculiarities, case-management, diagnosis and microbiological characteristics of 693 patients with tuberculosis registered in Chisinau in 2016 was performed. Results: Despite the fact that criteria for selection of patients for video-assisted anti-tuberculosis treatment (VOT) were defined, a range of risk factors can endanger treatment performing, such as: deep social economic vulnerability, comorbidities associated or not with psychic impairment, disease related characteristics, such as extensiveness, severity, duration of the tuberculosis evolution, positive microbiological state and multi-drug resistance are conditions which can exclude the ambulatory treatment and VOT. The low treatment outcome shows the limited potential of VOT to improve the epidemiological indices due to the complexity of patient’s risk factors. Conclusions: VOT can be implemented in the management of tuberculosis patients in the actual epidemiological state of the RM, if a complex of patients supporting measures are performed.

Tuberculoza poate evolua cu complicaţii severe cu risc letal. Scopul studiului a constat în evaluarea complicaţiilor tuberculozei şi rezultatului terapeutic la copii afectaţi cu vârsta între 0 şi18 ani. Material și metode: s-a efectuat o cercetare retrospectivă, descriptivă, selectivă, care a inclus 40 copii bolnavi de tuberculoză complicată. Rezultate: pleurezia a fost complicaţia predominantă. Tuberculoza localizată pulmonar s-a complicat cel mai frecvent printr-o singură complicaţie. Băieţii şi copiii din grupul de vârstă între 15 şi 18 ani au fost mai afectaţi de tuberculoza complicată. Contactul tuberculos s-a constatat la fiecare al doilea pacient. Testul tuberculinic a contribuit la stabilirea infecţiei tuberculoase la majoritatea copiilor, însă examenele microbiologice au confirmat diagnosticul etiologic doar la fiecare al cincilea copil. Durata prelungită a tratamentului a fost determinată de persistenţa complicaţiilor şi spectrului de rezistenţă la preparatele antituberculoase. Succesul terapeutic a fost diminuat de ponderea mare a pacienţilor care continuau tratamentul.

Scopul studiului a fost analiza particularităților clinice, paraclinice și a factorilor de risc a bolnavilor decedați prin tuberculoza pulmonară. A fost efectuat un studiu retrospectiv și descriptiv, în baza datelor a 92 bolnavi decedați prin tuberculoză în Spitalul Municipal de Ftiziopneumologie din Chișinău, în perioada anilor 2016-2018. Prin progresarea tuberculozei au decedat 40 (43,5%) bolnavi cazuri noi și 23 (25,0%) din retratamente, iar 29 (31,8%) bolnavi de tuberculoza au decedat prin alte boli. Din spitalele somatice au fost transferați 45,6% bolnavii, de la domiciliu prin intermediul serviciului de urgența în stare gravă au fost internați 39,1% pacienți și numai 15,3% – îndreptați de medicul ftiziopneumolog. Au predominat formele grave de tuberculoza (pneumonia cazeoasă și tuberculoza generalizată). Co-infecția TB/HIV a constituit 19,6%. Fără loc stabil de trai au fost 27,2% pacienți decedați. La decedații prin alte boli cauzele principale de deces au fost cancerul de diferite localizări, pneumoniile comunitare, ciroza hepatică.

Material and methods: A retrospective selective, descriptive study of socioeconomic, epidemiological peculiarities, case-management, diagnosis and microbiological characteristics of 694 patients with tuberculosis registered in Chisinau in 2016 was performed. Among them 581, had an urban residency and 112 rural residency. Results: Residents from rural population and young persons in urban areas were most affected. Socioeconomic vulnerability predominated in both subpopulations; however, the gravity was more represented in the urban group. Lower level of education and tuberculosis contacts were more dominating in the rural group. Comorbidities, HIV infection were more frequently identified in the urban group, but destructive forms – in the rural patients. Low treatment outcomes were more frequently established in the rural group. Conclusions: Risk factors for tuberculosis in urban subpopulation were: unemployment, lack of health insurance, homelessness, comorbidities, HIV infection. In rural population prevailed the following risk factors: low school education and tuberculosis contact.

Background: The aim of the study was the assessment of the impact of the molecular genetic Xpert MTB/Rif assay on the diagnostic delay and the treatment effectiveness in pulmonary tuberculosis, according to the results of the microbiological and molecular-genetic methods, in order to optimize the management of the drug resistant tuberculosis. Material and methods: A retrospective selective study, which included 226 patients with multidrug-resistant pulmonary tuberculosis (study sample) and 78 patients with drug-susceptible pulmonary tuberculosis (control group), was performed. Patients were divided into 2 subgroups following the results of the molecular-genetic and bacteriological investigations. The study sample was divided in two groups: the 1st group included 85 patients enrolled in the treatment for drug resistant tuberculosis, according to the results of the Xpert MTB test and the 2nd group included 141 patients enrolled in the treatment DOTS plus, according to the results of the conventional cultures. Results: The impact of the molecular genetic GeneXpert MTB/Rif assay was demonstrated on early diagnosis, adequate treatment according to the drug susceptibility to rifampicin, shorter duration of the treatment, low rate of the side events, optimal treatment compliance and higher treatment effectiveness. The conventional microbiological methods contributed to the late diagnosis of the patients, high rate of the severe and complicated tuberculosis, long duration of the treatment which were reflected on the low treatment effectiveness. However the conventional culture methods allowed the treatment individualization according to the drug susceptibility tests. Conclusions: Target screening of the population with risks and compulsory investigation by GeneXpert MTB/Rif assay will diminish the rate of severe late detected forms of pulmonary tuberculosis and adapt the treatment according to the drug resistance to rifampicin.

Background: One of the most important among risk factors for active tuberculosis development represents diabetes mellitus. The aim of the study was the assessment of the predictive factors for pulmonary tuberculosis in Xpert MBT/Rif resistant assay patients with diabetes mellitus. Material and methods: A retrospective, selective, descriptive and case-control study was performed. Were enrolled 119 pulmonary drug resistant tuberculosis patients, diagnosed and hospitalized in the Municipal Clinical Hospital of Phthysiopneumology of Chisinau city in the period of 01.01.2013- 01.01.2015. The patients were distributed in 2 groups: the 1st group – 34 MDR-TB patients with diabetes mellitus and the 2nd – 85 MDR-TB patients. Investigations were performed according to the National Clinical Protocol – 123. Results: The biological characteristics of the pulmonary MDR-TB patients with diabetes mellitus were old age with associated diseases, which contributed to a lower treatment outcome. For MDR-TB groups were common social-economic vulnerability, late detection as symptomatic cases and a high treatment success rate. Conclusions: Patients with MDR-TB and diabetes mellitus need an individualized approach for an early TB detection and prompt initiation of the adequate treatment regimen according of the susceptibility testing results.
