Abstract

AIM: The aim of this paper is to describe the implementation and evaluation of the Growing Strong Brains® (GSB) toolkit in a remote Aboriginal community in Western Australia (WA) over a 2-year period, 2018-2019. BACKGROUND: Ngala, a community service organisation in WA, developed the GSB toolkit in 2014, a culturally appropriate and interactive resource to build knowledge of early childhood development within Aboriginal communities. This was in response to evidence that a higher percentage of children in Aboriginal communities were developmentally vulnerable compared to the rest of the population. The GSB toolkit promotes awareness and understanding of early brain development pre-birth and in the early years of a child's life. METHODS: The project was underpinned by participatory action research (PAR). Reflective PAR review cycles (n = 5) monitored local community engagement, navigated challenges and utilised community strengths. Fifty-nine local service providers attended a 2-day formal training. Data were collected by using various methods throughout the project, including feedback following training, focus groups, surveys, one-on-one interviews using yarning techniques and reflective feedback from the Project Lead. FINDINGS: Establishing local Aboriginal project staff was pivotal to the success of the project. When delivering services for and with Aboriginal people, it is essential that cultural competence, safety and decision-making is carried through from planning to implementation and evaluation, and involves genuine, respectful and authentic relationships. Sufficient time allocation directed towards building relationships with other service providers and local community members needs to be considered and built into future projects.The Growing Strong Brains® project is embedded within the local community, and anticipated implementation outcomes were achieved. A study was conducted to evaluate the efficacy of Xpert MTB/RIF, an automated molecular detection test for myobacterium tuberculosis (MTB) and resistance to rifampin (RIF) in patients with suspected drug-sensitive tuberculosis. Results indicated that the test was highly effective in sensitive detection of tuberculosis and RIF resistance.

A randomized, controlled trial is conducted to test the efficacy and safety of using diarylquinoline TMC207, which inhibits mycobacterial ATP synthase as a mechanism of antituberculosis action, in the treatment of multidrug-resistant tuberculosis. Results reveal that TMC207 is safe and effective, thus validating ATP synthase as a target for antituberculosis therapy.

Abstract

Tidal wetlands provide critically important ecosystem services such as storm surge and flood attenuation, pollution retention and transformation, and carbon sequestration. The ability of tidal wetlands to maintain surface elevation under accelerated sea level rise is critical for their persistence. Saltwater intrusion can further threaten tidal freshwater marshes by decreasing primary production and organic matter accumulation as well as cause shifts in microbial pathways, leading to increases in organic matter decomposition and an overall decrease in marsh elevation. The objectives of this research were to examine accretion dynamics across the estuarine gradient of the Nanticoke River, a major tributary of the Chesapeake Bay, and determine the relative contribution of organic and inorganic matter to accretion in the marshes; determine the accumulation rates of C, N, and P across the estuarine gradient; and examine the effects of sulfate intrusion on biogeochemical transformations and marsh surface elevation in tidal freshwater marsh soil. Results of the collective studies suggest that the mechanisms controlling accretion dynamics and nutrient accumulation are complex and are likely driven by site-specific factors rather than estuary-wide factors. Accretion rates and nutrient accumulation rates were highly variable across the estuarine gradient, but were largely dependent on both organic matter accumulation and inorganic sedimentation. Only 8 out of the 15 subsites had accretion rates higher than relative sea level rise for the area, with the lowest rates of accretion found in the oligohaline marshes. Organic matter accumulation is especially important in marshes with low mineral sediment supply, particularly mid-estuarine oligohaline marshes, but may not be enough to help keep these marshes above relative sea level. The tidal marshes along the Nanticoke River removed approximately 15% and 9% of the total N and P load entering the system, but their ability to continue to remove nutrients may be compromised due to rising sea levels. Shifts in microbial pathways and increases in organic matter decomposition due to saltwater intrusion further threaten the ability of these marshes to keep pace with sea level rise, potentially resulting in the loss of an extremely valuable ecosystem.
