1. Why We Support Some Original Ideas but Reject Others: An Application of Signaling Theory

Abstract

Although organizations say new ideas are desirable, investing in original products rather than the "tried and true" can be unsettling for decision-makers. This discomfort may be due, in part, to uncertainty surrounding whether a new idea will prove successful. As such, the originality of a creative idea can be paradoxically viewed both as an asset, driving up appeal, as well as a detrimental risk. To help unpack these relationships, we propose that how an original idea is proposed (i.e., pitched) plays a central role in the support it receives. Guided by signaling theory we examined how pitch quality relates to funding for ideas with varying degrees of originality. In a field sample of 245 Kickstarter crowdfunding campaigns, pitch quality for ideas high in originality predicted investment in such ideas. In a follow-up experimental study, we created high- and low-quality pitches for two products, also varying product originality and product quality. Results highlight the importance of high-quality pitches for highly original ideas that are associated with reductions in uncertainty. Supplemental analyses suggest that high-quality pitches improve enthusiasm for a product as well, highlighting a potential second path of support influence.

6. Improving Asthma Control Test completion rates across a multi-site pediatric pulmonary clinic network: a quality improvement initiative.

Abstract

Objective: Assessing asthma control is an essential part of the outpatient management of children with asthma and can be performed through validated questionnaires such as the Asthma Control Test (ACT). Systematic approaches to incorporating the ACT in outpatient visits are often lacking, contributing to inconsistent completion rates. We conducted a quality improvement initiative to increase the proportion of visits where the ACT is completed for children with asthma in our multi-site pediatric pulmonary clinic network. Methods: We developed an intervention of sending the ACT questionnaire to patients and caregivers through the electronic patient portal to complete prior to their visits. This strategy was first piloted at one clinic beginning in July 2020 and then expanded to 5 other clinics in the network in October 2020. Our outcome measure was average monthly proportion of visits with a completed ACT, tracked using statistical process control charts. The process measure was method of ACT completion tracked using run charts. Results: At the pilot clinic, average monthly completion rate rose within 3 months of the intervention from 27% to 72% and was sustained more than 22 months. Completion across all clinics increased from 57% pre-intervention to 76% post-intervention. Importantly, the intervention did not rely on clinic staff to administer the questionnaire and did not interfere with existing clinic flow. Conclusion: An intervention of delivering the ACT electronically to patients and caregivers for completion prior to visits led to a rapid and sustained improvement in ACT completion rates across a large, pediatric pulmonary clinic network. Abstract

ObjectivePlexiform neurofibromas (PNs) are complex, benign nerve sheath tumors that occur in approximately 25%-50% of individuals with neurofibromatosis type 1 (NF1). PNs that cause airway compromise or pulmonary dysfunction are uncommon but clinically important. Because improvement in sleep quality or airway function represents direct clinical benefit, measures of sleep and pulmonary function may be more meaningful than tumor size as endpoints in therapeutic clinical trials targeting airway PN.MethodsThe Response Evaluation in Neurofibromatosis and Schwannomatosis functional outcomes group reviewed currently available endpoints for sleep and pulmonary outcomes and developed consensus recommendations for response evaluation in NF clinical trials.ResultsFor patients with airway PNs, polysomnography, impulse oscillometry, and spirometry should be performed to identify abnormal function that will be targeted by the agent under clinical investigation. The functional group endorsed the use of the apnea hypopnea index (AHI) as the primary sleep endpoint, and pulmonary resistance at 10 Hz (R10) or forced expiratory volume in 1 or 0.75 seconds (FEV1 or FEV0.75) as primary pulmonary endpoints. The group defined minimum changes in AHI, R10, and FEV1 or FEV0.75 for response criteria. Secondary sleep outcomes include desaturation and hypercapnia during sleep and arousal index. Secondary pulmonary outcomes include pulmonary resistance and reactance measurements at 5, 10, and 20 Hz; forced vital capacity; peak expiratory flow; and forced expiratory flows.ConclusionsThese recommended sleep and pulmonary evaluations are intended to provide researchers with a standardized set of clinically meaningful endpoints for response evaluation in trials of NF1-related airway PNs.

Abstract

Based on the insights derived from practical trials conducted with carriers operating in London, this article proposes a last-mile delivery model that involves both driving and walking. A particularly innovative aspect of the model is the shared use of a third-party portering service between the carriers as an enabler to collaborative provision, and shared micro-consolidation points for temporary storage of goods. The model gives rise to a complex routing problem, for which a tabu search algorithm is developed. The algorithm is able to suggest vehicle routes that consolidate parcels in shared locations and porter paths that deliver items from various companies, reducing the overall delivery cost. We applied this algorithm to a manifest dataset obtained from two carriers operating in London. The results from the model suggested that sharing a third party portering service for the last-mile delivery element between the rival carriers in the case study reduced overall transport costs by up to 14%.
