Purpose: To retrospectively evaluate the clinical-functional outcomes, healing rates, complications, and surgical time in patients treated with superior capsular reconstruction (SCR) using long head of the biceps autograft (LHB) and Achilles allograft (AA)., Materials and Methods: This retrospective study included 24 patients with irreparable rotator cuff tears of the supraspinatus and infraspinatus, treated with SCR. Two treatment groups were formed: one with 12 cases using AA and another with 12 cases using LHB. All patients were followed for a minimum of 2 years., Results: The SSV was 73.7±25.3 vs. 86.0±8.7 (p=0.26), the Constant score was 76.8±20.1 vs. 83.7±4.6 (p=0.12), and the VAS was 1.6±2.3 vs. 1.7±0.5 (p=0.9) for the LHB and AA groups, respectively. Tendon healing was 66.7% in AA and 100% in LHB (p=0.001). Complications were 50% in AA and 0% in LHB. The average surgical time was 127.7±37.6minutes for AA and 84.3±14.3min for LHB (p=0.01)., Conclusions: SCR with LHB showed better results in terms of tendon healing, fewer complications, and reduced surgical time compared to the use of AA., (Copyright © 2024. Introduction: Shoulder Injuries Related to Vaccine Administration (SIRVA), describes those cases of shoulder severe post-inoculation complications, including pain and prolonged disability. Most of the reported cases have been secondary to influenza vaccination. This study retrospectively describes a series of 18 patients following SARS-CoV-2 inoculation and compares the findings with those previously reported for other vaccines., Materials and Methods: Inclusion criteria was onset of symptoms within 48 h after injection, symptoms duration of at least seven days, and restricted range of motion in absence of symptoms prior to vaccination. Average age was 59.4 years old (38-76), and 72.2% were women., Results: In many cases (58%) the initial diagnosis was not clear, which lead to incorrect treatment. The most common pathological finding was subacromial-subdeltoid bursitis (66.6%). All patients who received depot corticosteroids followed by a gentle rehabilitation program showed strong clinical improvement but did not completely resolve the symptoms at 7.2 months average final follow-up. Surgical intervention was necessary in one of the patients due to the persistence of symptoms despite conservative treatment., Conclusions: Shoulder injury related to vaccine administration is rare, but when present, its torpid evolution makes it difficult to treat. We have found in our case series a similar pattern to that already described for other vaccines. Seroepidemiological studies help identify the indirect distribution of diseases, detecting serological markers of immunity and demonstrating undiagnosed infections in the general population. The objectives were to estimate the seroprevalence of SARS-CoV-2 antibodies in Córdoba, Argentina, from December 2020 to January 2021 and to identify factors associated with the virus' contagiousness. A population-based cross-sectional observational study was performed in 3,225 individuals over two years of age living in Córdoba city, selected by multiple-stage random sampling proportional to distribution by gender, age group, and socioeconomic status in the city's population. Clinical characteristics, anthropometry, and comorbidities were collected by interview. Qualitative serological testing was performed for detection of SARS-CoV-2 nucleocapsid IgG antibodies (ARCHITECT, Abbott). SARS-CoV-2 seroprevalence was estimated for the total population and by age group, sex, socioeconomic status, and presence of target diseases. Prevalence ratios (PR) were estimated using a log-binomial regression model. SARS-CoV-2 seropositivity was 16.68% (95%CI: 15.41-18.01). Age 2 to 18 years, living in neighborhoods with low socioeconomic status, and obesity increased the odds of seropositivity (PR = 1.50; 95%CI: 1.10-2.04, PR = 1.91; 95%CI: 1.34-2.67 and PR = 1.39; 95%CI: 1.04-1.85). The results indicate that the city of Córdoba displays differential attributes that increase the likelihood of a positive SARS-CoV-2 antibody test. This allows targeting epidemiological surveillance strategies to reduce the spread of the virus.

HTLV-1 proviral load (pVL) in peripheral blood mononuclear cell (PBMCs) is proposed as a marker of disease progression but its role still remains controversial. The aim of this study was to evaluate the levels of HTLV-1 pVL in symptomatic patients and asymptomatic HTLV-1 carriers. In this cross-sectional study the pVL was measured by Real Time PCR in 102 asymptomatic carriers and 22 symptomatic patients (5ATLL, 15 TSP and 2 uveitis). We observed that the HTLV-1 pVL was significantly higher in symptomatic patients (median = 4.99 log10 HTLV-1 copies /106 PBMCs) compared to asymptomatic HTLV-1 carriers (median = 4.38 log10 HTLV-1 copies /106 PBMCs; p = 0.0030). A wide variation on the HTLV-1 pVL levels among asymptomatic HTLV-1 carriers was observed with some pVL as high as those observed in symptomatic patients. The asymptomatic HTLV-1 carriers were divided according to the place of birth and the highest levels of pVL were detected among patients from endemics areas from the North of Argentina. Our results reinforce the usefulness of the proviral load would be a prognostic marker of HTLV-1 disease progression. Vertical transmission is the main route of HIV infection in childhood. Because of the persistence of maternal HIV antibodies, virologic assays that directly detect HIV are required to diagnose HIV infection in infants younger than 18 months of age. The sensitivity of HIV RNA/DNA assays increases as the child becomes older. These tests have specificity values greater than 95%. The aim of this study was to evaluate the performance of the COBAS Taqman HIV-1 Test, v1.0 assay (Roche) and its concordance with a Multiplex Nested-PCR. Of 341 samples processed, 15 were positive and 326 negative by both methods. Sensitivity and specificity overall values for the viral load assay were 88.2% and 100%, respectively. Our results indicate that the COBAS Taqman assay evaluated could be used as an alternative method to diagnose HIV congenital infection., (Copyright © 2014 Asociación Argentina de Microbiología. Background: Different RET oncogene mutations have been found to be associated with inherited medullary thyroid carcinoma (MTC) in the context of three different syndromes including multiple endocrine neoplasia types 2A (MEN 2A) and 2B (MEN 2B) and familial medullary thyroid carcinoma (FMTC). These mutations have been recorded in different populations, but to date there is no corresponding study in Mexican families. Our purpose was identification of RET mutations in Mexican families with inherited or sporadic MTC (SMTC) and search for RET protein expression as prognostic marker in MTC tumors., Methods: Nine unrelated families with MTC corresponding either to two MEN 2A, three MEN 2B, or four SMTC were studied. Screening of exons 10, 11, and 13-16 of RET oncogene in DNA from circulating lymphocytes and tumor samples were analyzed. Immuno- staining for RET was performed in the corresponding tumor., Results: Germline 918 ATG-->ACG RET mutation was present in three unrelated MEN 2B individuals and corresponding somatic mutation in one individual with SMTC; 634 TGC-->TTC RET mutation was detected in two related patients in an MEN 2A family and the 634 TGC-->TAC RET mutation was detected in 12 related individuals from a second MEN 2A family. RET protein expression was detected in all MTC tumors showing different staining intensity., Conclusions: RET mutations found in Mexican patients with MTC are similar to those previously reported in several MTC families worldwide. This indicates that RET mutations are highly conserved and that MTC etiology does not depend to a great extent on environmental factors or ethnic differences. Detection of RET protein in MTC tissue sections is not useful as prognostic marker.

To study the effects of LT4 dose on thyroid hormone serum levels, a prospective, longitudinal and comparative study was designed, including 56 term eutrophic 1-89 day-old infants with congenital hypothyroidism (CH) detected by neonatal screening. Patients were divided into four groups according to delayed or normal bone age at birth, and athyreosis or ectopic thyroid. All received an initial dose of 50 micrograms/day (12.9-13.7 micrograms/kg/day) LT4 and were followed bimonthly (first year) and quarterly (second year) with thyroid profile and bone age determinations at 6, 12 and 24 months. At diagnosis, hormone levels were higher in cases of ectopia than in athyreosis (p < 0.001), and T4 was lower in children with delayed than in normal bone age at birth (p < 0.05). During treatment, all groups were clinically euthyroid despite T4 and FT4 serum levels higher than the upper normal limit (p < 0.0.001), though T3 and FT3 were within the normal limit (p > 0.05). TSH normalized within 8 weeks. Bone age accelerated at 2 years in eight children of the bone age delayed group. No patient had craniosynostosis.

Type I diabetes is an autoimmune and a polygenic disease, in which MHC-class II genes contribute to 48% of the disease. The aim of the present study, is to provide a guideline to understanding the molecular association of these genes, through the immunogenetic analysis of 3 Latin american mestizo populations. We included 606 individuals, 349 patients with DMDI and 257 healthy controls coming from 3 geographical areas: Mexico City, Mexico; Caracas, Venezuela and Medellin, Colombia. The results clearly indicate that in mestizo groups, the diabetogenic haplotypes are from mediterranean ancestry, while protection is due to Amerindian genes. It was demonstrated that the relevant sequences for IDDM expression are located to DRB1 and DQB1 loci with a minimal contribution of DQA1 residues. The sequences determining peptide recognition and the induction of TH1 cells mediating the cellular autoimmune response are in positions DRB1-57 and 74 (an aspartic acid and a glutamic acid respectively, confer protection), modulated by D-57 in the DQ, 8 chain. These data show that DRB1-DQB1 haplotypes are central for IDDM expression and open new pathways for the disease management.

MHC class II genes play an important role in the autoimmune destruction of the pancreatic b-cell occurring in IDDM. The genetic pattern of the disease was investigated in Mexican Mestizos. The serological findings of HLA antigens showed a significant association of DR3, DR4, DQ2 and DQ8 and a protective effect of DR11, DR15, DQ5, DQ6 and DQ7. With these results, DNA analysis of HLA-DRB1, B3, B4, DQA1, DQB1, DPA1, DPB1 genes was performed using PCR with allele specific oligotyping. Among the patients, 92.78 carry DQA1 alleles that have ARG in position 52 of DQa chain, and 78.2% are ASP- in DQ5-57. The RR for homozygotes is 32.8 and 5.6, respectively. The main haplotype involved is DRB1*0405, DQA1*0301, DQB1*0302. Thus, DQa and DQb form a relevant recognition site for the "diabetogenic peptidett which induces the autoimmune destruction. Positions 57 and 74 of DRB1 locus contribute highly to the expression and severity of IDDM in Mestizos and other ethnic groups, but not in Caucasians or Blacks.

Biosynthetic growth hormone extracted from mammalian cells was used in eight children with growth hormone deficiency, in search for a clinical safety study. Each child received 0.6 U/kg/week, subcutaneously, during a period of six months, and was evaluated monthly for clinical, anthropometric and laboratory modifications induced by the drug in a physiological or pathological way. The growth velocity increased from less that 4 cm/year to 11.2 +/- 1.08 cm/year, the bone age maintained according to the chronological age, and the anthropometric evaluation showed an harmonious and physiological growth. There was not any undesirable nor secondary changes except transient hypothyroidism in two patients, and unmasked hypophyseal hypothyroidism in another two. We concluded that biosynthetic growth hormone extracted from mammalian cells is a safety drug, and that patients with growth hormone deficiency may be treated with it in order to improve their final height expectancy.

Ten growth hormone deficient patients (GHDP) (six girls & four boys) with chronological age range six-15 years old, received recombinant human growth hormone (GH) (0.6 UI/kg/week) in three subcutaneous injections per week over 14 months. A complete physical examination was performed monthly; thyroid hormones concentrations every two months, as well as, bone age every four months were determined. Growth velocity was higher in the first six months' treatment (12.73 +/- 1.65 cm/yr vs 10.6 +/- 1.67 cm/yr; previous growth velocity 3.76 +/- 2.4 cm/yr). The variables with better prognosis were: lower previous growth velocity, lower chronological age (< 12 ys), lower bone age (< 8 ys), higher BMI than ideal; and higher acid alkaline phosphate during treatment. Two of ours patients developed biochemical hypothyroidism during the treatment period, and they needed hormonal substitutive therapy. With GH treatment a harmonious growth is obtain with bone age progress according chronological age. The height of GHDP treated with GH do not approaches the height range of their parents, probably due the time of treatment. In the follow-up of all patients treated with GH is important to realize periodical laboratories screenings looking for another hypothalamic and/or hypophyseal deficiencies.
