SARS-CoV-2 é um vírus zoonótico que está causando enormes danos à saúde pública e à economia mundial. No momento em que o surto de Covid-19 foi declarado, não havia medicamentos antivirais específicos disponíveis e as drogas mais promissoras foram desenvolvidas originalmente para outros propósitos, como a hidroxicloroquina. Vários pesquisadores propõem o desenvolvimento de agentes antivirais de ação ampla como uma primeira frente de combate a vírus emergentes, e peptídeos são bons candidatos devido à amplitude de seus alvos moleculares, especificidade de suas interações e baixa toxicidade [1]. Já foram descritos peptídeos com atividade desestabilizadora de partículas virais, inibidores do processo de internalização viral por meio da interferência com proteínas fusionais, peptídeos ligantes a glicoproteínas virais e com propriedades imunomodulatórias.

Dissertação de mestrado integrado em Psicologia, A escrita sobre acontecimentos emocionalmente significativos tem sido associada a benefícios na saúde e bem-estar psicológico. Estudos apontam a escrita sobre experiências positivas como benéfica, direcionando o indivíduo para um desenvolvimento saudável. Este estudo pretende analisar qual o impacto do paradigma original de Pennebaker e uma variação deste centrada nos recursos do indivíduo, no bem-estar de uma amostra saudável. Participaram neste estudo 50 indivíduos, com idades entre 17 e 41 anos. Para avaliar o bem-estar e sofrimento foram aplicados o Outcome Questionnaire 10.2 (OQ-10.2), a Positive and Negative Affect Schedule (PANAS) e a Escala de Satisfação com a Vida (ESCV). Para os hábitos de escrita e perceção da tarefa foram usadas questões construídas para o efeito. Nos participantes que escreveram sobre experiências difíceis houve uma diminuição do afeto negativo (p= .019), enquanto os participantes que escreveram sobre recursos apresentaram um aumento da satisfação com a vida (p= .034). O impacto foi maioritariamente cognitivo e está associado à promoção de emoções positivas. Apesar de os efeitos dos paradigmas terem sido ligeiros, mostraram que ambos produzem algum impacto positivo, sugerindo que a Escrita Expressiva apresenta um ligeiro impacto no bem-estar. Contudo estes resultados devem interpretar-se com reserva e novas investigações são necessárias., Writing about significant emotionally events has been linked to health and psychological well-being benefits. Studies point out that writing about positive experiences is beneficial, directing the individual towards healthy development. This study aims to analyze the impact of the original Pennebaker paradigm and a variation of this centered on the individual's resources, on the well-being of a healthy sample. Fifty participants have taken part of in this study, aged between 17 and 41 years. To assess well-being and suffering, the Outcome Questionnaire 10.2 (OQ-10.2), the Positive and Negative Affect Schedule (PANAS) and the Life Satisfaction Scale (ESCV) were applied. For writing habits and task perception, questions built for that purpose were used. In the participants who wrote about difficult experiences, there was a decrease in negative affect (p = .019), while the participants who wrote about resources showed an increase in life satisfaction (p = .034). The impact was mostly cognitive and is associated with the promotion of positive emotions. Although the effects of the paradigms were slight, they showed that both produce some positive impact, suggesting that Expressive Writing has a slight impact on well-being. However, these results must be interpreted with reserve and further investigations are necessary.

Background: Colorectal cancer (CRC) is the first cause of death by cancer in Portugal and mortality has been increasing in the last 30 years. Materials and Methods: During a raising awareness campaign performed by our Gastroenterology Department, in Setúbal, Portugal, an anonymous written questionnaire was developed and presented in order to evaluate the population’s knowledge and attitude regarding CRC screening. Results: The following results were reported: 140 persons; mean age 54.6 years; 61.4% women; 22.1% had a family history of colorectal cancer. The main risk factors mentioned by the respondents were family history of CRC, previous history of intestinal polyps, and intestinal infection. Screening was considered useful by all respondents. About 60% of the respondents had been counselled, at least, on one screening technique, mainly by their general practitioner: colonoscopy in 31, fecal occult blood test in 44, and flexible sigmoidoscopy in 9. Most of the respondents had had the appropriate screening test, according to their age and family history. Fourteen of the respondents had not undergone the recommended screening. Their answers showed that this was due to fear of pain/ discovering a disease as well as embarrassment. Conclusion: Although the majority of the respondents were aware of the importance of CRC screening, results show that there are still several misconceptions about risk factors, fear, and reluctance concerning the screening techniques. Awareness actions are useful to clarify possible questions and inform the population in order to increase compliance with screening. Introdução: O cancro colorretal (CCR) é a primeira causa de morte por cancro em Portugal, com uma mortalidade que tem aumentado nos últimos 30 anos. Materiais e Métodos: Durante uma acção de sensibilização levada a cabo por profissionais de saúde do Serviço de Gastrenterologia, em Setúbal, Portugal, foi realizado um inquérito escrito, anónimo, no sentido de avaliar os conhecimentos e atitudes da população face ao CCR. Resultados: 140 indivíduos; idade média 54.6 anos; 61.4% mulheres; 22.1% tinham pelo menos 1 familiar com CCR. Os factores de risco para CCR mais referidos foram a presença de pólipos e a história familiar. A infecção intestinal foi considerada como um factor de risco para CCR por 74% dos inquiridos, sendo que o rastreio foi considerado útil por todos. Tinha sido recomendado pelo menos um exame de rastreio a cerca de 60% dos participantes, a maioria por indicação do médico assistente de medicina geral e familiar: colonoscopia total em 31, pesquisa de sangue oculto nas fezes em 44 e rectossigmoidoscopia em 9. Atendendo à idade e história familiar, a maioria efectuou um rastreio apropriado. Catorze pessoas não efectuaram o rastreio aconselhado; os motivos mais apontados foram o medo da dor/descobrir uma doença e vergonha. Conclusão: Embora a maioria dos inquiridos estivesse alerta para a importância do rastreio do CCR, persistem muitas ideias erradas em relação a factores de risco, receios e reticências quanto à realização dos exames de rastreio. Acções de sensibilização são úteis para clarificar dúvidas e informar a população no sentido de aumentar a adesão ao rastreio.

Dissertação de mestrado em Relações Internacionais, As sucessivas crises económicas e alteração da nova ordem mundial que vem sendo implementada desde o início do século XXI obrigaram a uma reestruturação das políticas do Estado em matéria de Cooperação Internacional para o Desenvolvimento. A articulação com o setor privado e suas empresas, com poderio financeiro e cada vez mais envolvido com a responsabilização pública/social dos seus empreendimentos, tem sido apontada por vários organismos internacionais como uma estratégia para dar novo impulso a esta ferramenta de política externa. Numa análise à realidade portuguesa, a presente dissertação conclui que em Portugal, apesar de reconhecidos os benefícios desta aproximação por ambas as partes, os programas implementados neste sentido por parte do setor público respondem a imposições internacionais e ainda não se traduzem em propostas concretas a um setor privado com uma crescente consciência social e manifesta disponibilidade para colaborar com os objetivos nacionais de cooperação internacional para o desenvolvimento., The successive economic crises and changes in the new world order that have been implemented since the beginning of the 21st century have forced a restructuring of the State's policies on International Cooperation for Development. The articulation of the private sector and its companies, with financial power and increasingly involved in the public / social accountability of its enterprises, has been pointed out by several international organizations as a strategy to give new impetus to this foreign policy tool. In an analysis of the Portuguese reality, the present dissertation concludes that in Portugal, while recognizing the benefits of this approach by both parts, the programs implemented in this direction by the public sector respond to the international impositions and still not translating into concrete proposals for a national private sector with a growing social awareness and able to cooperate with the national objectives of international cooperation for development.

17. Predictors for incomplete response to ursodeoxycholic acid in primary biliary cholangitis. Data from a national registry of liver disease

- Abstract
The current standard of treatment in primary biliary cholangitis (PBC) is ursodeoxycholic acid (UDCA), although a considerable proportion of patients show incomplete response resulting in disease progression. This study aimed to assess the prevalence of incomplete response to UDCA and determine associated patients' characteristics. Patients with PBC as main diagnosis were included from a national multicentric patient registry—Liver.pt. Main endpoints included incomplete response to UDCA treatment according to Barcelona, Paris I and Paris II criteria, Globe and UK PBC scores and the association between baseline characteristics and incomplete response according to Paris II criteria. A total of 434 PBC patients were identified, with a mean age of 55 years and 89.2% females. Nearly half of patients were asymptomatic at diagnosis and 93.2% had positive anti‐mitochondrial antibodies. Almost all patients (95.6%) had been prescribed at least one drug for PBC treatment. At the last follow‐up visit, 93.3% were under treatment of which 99.8% received UDCA. Incomplete response to UDCA was observed in 30.7%, 35.3%, 53.7% and 36.4% of patients according to Barcelona, Paris I, Paris II criteria and Globe score, respectively. After adjusting for age and sex, and accordingly to Paris II criteria, the risk for incomplete biochemical response was 25% higher for patients with cirrhosis at diagnosis (odds ratio [OR] = 1.25; 95% confidence interval [95%CI]: 1.02–1.54; p= 0.033) and 35% (95%CI:1.06–1.72; p= 0.016) and 5% (OR = 1.05; 95%CI:1.01–1.10; p= 0.013) for those with elevated gamma‐glutamyl transferase (GGT) and alkaline phosphatase (ALP). A considerable proportion of patients showed incomplete biochemical response to UDCA treatment according to Paris II criteria. Cirrhosis, elevated GGT and ALP at diagnosis were identified as associated risk factors for incomplete response. Early identification of patients at risk of incomplete response could improve treatment care and guide clinical decision to a more careful patient monitorization. 18. Metrite pós-parto em bovinos de leite: um estudo de caso

Dissertação de Mestrado Integrado em Medicina Veterinária Os processos inflamatórios pós-parto com sede no útero, como a metrite, são um dos problemas mais frequentemente diagnosticados em bovinos de leite. Estão associadas a perdas económicas importantes, não só relacionadas com uma diminuição do desempenho reprodutivo, mas também com o seu efeito negativo na produção de leite. Como outras patologias do pós-parto na vaca leiteira, a metrite pós-parto (MPP), ou metrite puerperal, possui uma etiologia multifatorial. Ao longo do tempo têm sido identificados alguns fatores como sendo predisponentes desta afeção, apesar de haver alguma controvérsia sobre a sua associação à etiopatogenia da doença. Neste estudo pretendeu-se analisar a associação da MPP com a ocorrência prévia de Retenção Placentária (RP) e a presença de valores elevados de corpos cetónicos (CC) no momento do diagnóstico da metrite, além de outros parâmetros como a idade, paridade, momento de diagnóstico, condição corporal e tipo de parto (eutócico ou distócico). Foram incluídas neste trabalho um total de 42 vacas, sendo 19 as fêmeas diagnosticadas com metrite pós-parto. Incluiu-se no estudo fêmeas primíparas (n= 5) e multíparas (n= 37), com idades compreendidas entre os 3 e os 9 anos (idade média de 4 ±1,2 anos). As vacas eram provenientes de 5 explorações localizadas na Região Litoral Norte de Portugal, com maneio geral similar entre si. O diagnóstico de metrite foi realizado até aos 38 dias após o parto, após consulta solicitada pelo proprietário. O diagnóstico de metrite foi emitido após exame clínico e reprodutivo da vaca com base na existência de uma quebra na produção leiteira, diminuição na ingestão de matéria seca, e a presença de um odor pútrido ou de corrimento vaginal anómalo. As fêmeas do grupo controlo foram selecionadas dentro das mesmas explorações, entre animais que se encontravam no mesmo momento do pós-parto, tinham sensivelmente a mesma idade/paridade e um nível de produção de leite semelhante. Para os dois grupos foram registados os dados básicos do animal, bem como recolhida informação sobre a facilidade do parto (que foi classificado em eutócico/distócico) e a existência de retenção placentária (sim/não; RP) no último parto. A recolha de uma amostra de sangue para determinação de corpos cetónicos apenas foi possível em 21 animais (11 no grupo controlo e 10 no grupo metrite). A maior parte dos diagnósticos de metrite foram realizados nas duas primeiras semanas após o parto, tendo sido mais frequente em fêmeas com 2 e 3 partos (36,8% e 31,5% respetivamente) e menos comum nas fêmeas primíparas ou com quatro ou mais partos (15,8% em ambos os casos). A condição corporal dos animais incluídos no estudo era relativamente uniforme e dentro dos limites desejados no peri-parto. Em relação ao tipo de parto, foram registados 21% e 17% de partos distócicos respetivamente para os grupos metrite e controlo, não sendo estas diferenças estatisticamente significativas. A medição dos CC em ambos os grupos revelou valores médios semelhantes (P=0,704), embora os valores fossem numericamente mais baixos nas vacas controlo (0,84± 0,40 mmol/L) do que no grupo metrite (1,11± 0,91 mmol/L). A ocorrência prévia de retenção placentária foi registada em 14 (73,3%) e 5 (21,7%) animais nos grupos com diagnóstico de metrite e controlo, respetivamente. Não se encontraram influências significativas da idade, paridade, condição corporal, tipo de parto e presença de cetose no momento de diagnóstico sobre a ocorrência de MPP nos animais incluídos no estudo. No entanto, encontrou-se uma associação positiva entre a MPP e a ocorrência prévia de RP. A razão de probabilidades revelou que vacas com retenção placentária no último parto têm 10 vezes mais probabilidade de virem a apresentar um diagnóstico de MPP. Dados os encargos económicos que acompanham a ocorrência da MPP e o comprometimento do bem-estar das fêmeas afetadas, é importante a sensibilização do proprietário para a necessidade de programas preventivos que sejam eficazes no controlo da patologia e na monitorização mais apertada de animais que sofreram de complicações no peri-parto, como RP. A nutrição assume um papel determinante nesta prevenção, assim como a sinalização precoce das vacas em risco. The inflammatory conditions of the postpartum uterus, such as metritis, are frequently diagnosed in dairy cattle. They are associated with serious economic losses, related not only with a decrease of the reproductive performance, but also with its adverse effect on milk production. Like in other postpartum conditions of dairy cows, postpartum metritis (PPM) presents a multifactorial etiology. Over time, some factors have been identified as predisposing to this disease, even though there is some controversy about its association with the pathogenesis of the disease. This study intent to analyze the association between PPM with retained placenta (RP) in the last calving and the presence of high levels of ketone bodies (CC) at the time of diagnosis of metritis, besides other parameters such as age, parity, body condition and type of delivery (eutocic or dystocic). Forthy-two cows were enrolled in this study, 19 of them having a diagnosis of PPM. It included either primiparous (n = 5) and multiparous (n = 37) females, within an age range between 3 and 9 years old (age average of 4 ±1,2 years). The cows belonged to 5 farms of similar management characteristics, from the North Coast of Portugal. Metritis diagnose was establish up to 38 days post-calving, following a call from the owner or caretaker that signalized the sick animal. The diagnosis of metritis was raised after the physical and reproductive examination of a cow based on the following clinical signs: inapetence, raised temperature, drop in milk production, decreased dry matter intake and the presence of a putrid odor, or abnormal vaginal discharge. Females used in the control group were selected within the same farm, between animals at the same postpartum time, similar same age/parity and similar milk production. For both groups, beside the basic animal data it was also recorded information on the calving easiness (scored as eutocic/distocic) and the occurrence of retained placenta (yes/no) on the last calving. The blood collection for measurement of ketone bodies was possible only once and limited to 21 animals (11 in the control group and 10 in the metritis group). The diagnosis of metritis was establish more often in the first two weeks after parturition, in females in the 2nd or 3rd calving (36.8% and 31.5% respectively) and it was less common in primiparous females or in cows with four or more calving (15.8% in both cases). The body condition score of the animals was relatively uniform and within the limits generally foreseen in the peripartum. Distocia was present in 21% and 17% of cows in the metritis and control groups, but the differences were not significant. Even though the values of ketone bodies were numerically lower for cows in the control group (0.84 ± 0.40 mmol / L) than the ones in the metritis group (1.11 ± 0.91 mmol / L), the groups did not differ (P = 0.704),. The occurrence of retained placenta in the previous calving was recorded in 14 (73.3%) and 5 (21.7%) animals for the groups metritis and control, respectively. The occurrence of PPM was not influenced by age, parity, and body condition score, type of delivery and presence of ketose at the time of diagnosis. However, a positive association between MPP and the prior occurrence of RP was found in this study. The odds ratio showed that cows with retained placenta in their last parturition were 10 times more likely to present a positive diagnosis for MPP. Given the major economic constraints associated to the occurrence of MPP and the negative impact on the affected cow´s welfare, it is important to alert the owner for the need to establish effective prevention programs to control the disease and also to put animals developing peri-parturient diseases under a closer monitorization. Nutrition plays a key role in this prevention, as well as an early detection of cows at risk.

Celiac disease is the most important cause of intestinal villous atrophy. Seronegative intestinal villous atrophy, including those that are nonresponsive to a gluten-free diet, is a diagnostic challenge. In these cases, before establishing the diagnosis of seronegative celiac disease, alternative etiologies of atrophic enteropathy should be considered. Recently, a new clinical entity responsible for seronegative villous atrophy was described—olmesartan-induced sprue-like enteropathy. Herein, we report two uncommon cases of atrophic enteropathy in patients with arterial hypertension under olmesartan, who presented with severe chronic diarrhea and significant involuntary weight loss. Further investigation revealed intestinal villous atrophy and intraepithelial lymphocytosis. Celiac disease and other causes of villous atrophy were ruled out. Drug-induced enteropathy was suspected and clinical improvement and histologic recovery were verified after olmesartan withdrawal. These cases highlight the importance for clinicians to maintain a high index of suspicion for olmesartan as a precipitant of sprue-like enteropathy.

Seu objetivo geral foi analisar as referidas contribuições acerca do tema, com base nas obras de Freud. O método utilizado foi o qualitativo descritivo, delineado como pesquisa bibliográfica. Para que o objetivo fosse atingido, percorreram-se, historicamente, obras de Freud, desde suas primeiras teorias sobre a sexualidade infantil até a postulação dos caminhos que a criança deve traçar para tornar-se mulher. Foram também pesquisados e discutidos alguns sucessores e comentadores de Freud, que contribuíram na compreensão da feminilidade. As teorizações psicanalíticas apresentam diversos impasses, confirmando que ainda há muito que se estudar sobre a mulher e os caminhos trilhados no seu percurso rumo à feminilidade.

29. Induction of premature senescence in human fibroblasts through endoplasmic reticulum stress: protection by phenolic diterpenes

This is however associated with a higher incidence of age-related chronic disorders, such as neurodegenerative and cardiovascular diseases, diabetes and cancer. Therefore, new approaches to promote a healthy ageing are needed. The accumulation of damaged and misfolded proteins are present during the ageing process and known to be associated with endoplasmic reticulum (ER) stress and to the progression of many age-related diseases. Based on this, an in vitro method to induce premature senescence in normal human fibroblasts through induction of ER stress was developed in the present study in order to test potential anti-ageing compounds that acts by dealing with protein damage and associated ER stress. Tunicamycin (TUN) was successfully used to induce ER stress in normal diploid human skin fibroblasts since it triggered the unfolded protein response (UPR) signaling, as shown by the up-regulation of BiP, ATF6- α, CHOP and spliced XBP1. Treatment of fibroblasts with TUN significantly inhibited cell growth without inducing cell death (even by apoptosis), and it affected cell morphology in a way that resembled a senescent phenotype. Induction of premature senescence was confirmed by the appearance of several markers of cellular senescence, such as induction of G1 arrest, presence of SA β-Gal positive cells, increased cells’ green autofluorescence and expression of p16. Finally, this model was used to test the potential protective effect of carnosic acid - a natural phenolic diterpene. Although preliminary, the results suggested us that carnosic acid is able to prevent against the TUN-induced premature senescence corroborating the anti-ageing potential of this compound shown by previous results from the group. This work contributed to the development of a new model of induction of premature senescence in human fibroblasts, which can then be used to test compounds applicable for nutritional ageing interventions., O envelhecimento da população está a acontecer de uma forma crescente devido ao aumento da esperança média de vida e por razões sociais. Contudo, este fenómeno está associado ao aumento da incidência de doenças crónicas relacionadas com o envelhecimento, tais como doenças neurodegenerativas e cardiovasculares, diabetes e cancro. Portanto, novas abordagens são necessárias de modo a promover um envelhecimento saudável. A acumulação de proteínas danificadas está presente durante o processo de envelhecimento e é reconhecido a sua associação ao stress do retículo endoplasmático (ER) e com a progressão de muitas doenças relacionadas com o envelhecimento. Deste modo, no presente estudo foi desenvolvido um método in vitro de indução de senescência prematura em fibroblastos humanos normais através da indução de stress do ER, de modo a testar compostos com potencial anti-envelhecimento que atuam por modulação dos níveis de proteínas danificadas e o associado stress do ER. A tunicamicina (TUN) foi usada com sucesso em fibroblastos diplóides normais isolados de pele humana para indução de stress do ER, uma vez que ativou a unfolded protein response (UPR) demonstrado pelo aumento observado da expressão de BiP, ATF6-α, CHOP e spliced XBP1. O tratamento dos fibroblastos com TUN inibiu significativamente o crescimento celular sem, porém, induzir morte celular (incluindo apoptose), mas afetou significativamente a morfologia das células de tal modo que mimetizava um fenótipo de senescência. A indução de senescência prematura foi confirmada através da presença de diversos marcadores de senescência celular, tais como a indução da paragem do ciclo celular em G1, presença de células marcadas com β-Gal, aumento da autofluorescência verde e da expressão de p16. Finalmente, este modelo foi usado para testar os potencias efeitos protetores do ácido carnósico - um diterpeno fenólico. Embora preliminares, os resultados sugerem que este composto previne contra a senescência prematura induzida pela TUN, corroborando assim o seu potencial efeito anti-envelhecimento anteriormente demonstrado pelo grupo. Este trabalho contribuiu para o desenvolvimento de um novo modelo de indução de senescência prematura em fibroblastos humanos, o qual pode ser usado para testar compostos aplicáveis em intervenções nutricionais para o envelhecimento.

30. A importância da educação sexual em meio escolar

Em Portugal existe legislação referente à educação sexual em meio escolar desde 1984. A mais recente é de agosto de 2009. Este estudo pretende perceber se esta lei está a ser cumprida num Agrupamento de Escolas do norte do país e quais as principais dúvidas, relacionadas com a sexualidade, que os adolescentes apresentam. Para isso foi aplicado um questionário a 274 alunos (46.4% de rapazes e 53.6% de raparigas), entre os 10 e os 20 anos de idade (média = 13.7%). O estudo permitiu concluir que a maioria (75.4%) concorda com aulas de educação sexual nas escolas, no entanto, do total da amostra, 59.0% nunca tiveram aulas de educação sexual. Também se percebeu que grande parte não fala nunca destes assuntos com os pais (34.1%). As dúvidas relacionadas com a temática são variadas (desde as mudanças na adolescência, às doenças sexualmente transmissíveis ou aos métodos contracetivos, passando pela relação que estes jovens estabelecem com os pais e amigos), pelo que se torna necessária e urgente a aplicação da atual legislação sobre a educação sexual nas escolas do nosso país. In Portugal there is legislation regarding sex education in schools since 1984. The latest is from August 2009. This study aims to understand whether this law is being accomplished in a Group of Schools from the north of the country and what are the main questions that adolescents have as far as sexuality is concerned. For this purpose a questionnaire was given to 274 students (46.4% were boys and 53.6% were girls) between the ages of 10 and 20 (average = 13.7%). The study concluded that the majority (75.4%) agrees with sex education in schools, however, from the total sample 59.0% have never had sex education classes. It was also noticed that most never speak about these matters with their parents (34.1%). Doubts related to the topic are varied; it is therefore necessary and urgent the implementation of current legislation on sex education in the schools of our country.

INTRODUÇÃO: O envolvimento anal em doentes com Pênfigo Vulgar tem sido raramente reportado. Apresentamos o caso de uma doente de 76 anos com diagnóstico de Pênfigo Vulgar muco-cutâneo, sob corticoterapia sistémica, que inicia quadro proctológico caracterizado por hematoquézias e proctalgia associado a obstipação e dificuldade defecatória. A rectosigmoidoscopia flexível não mostrou alterações, mas o exame proctológico revelou coroa de mariscas com múltiplas erosões, compatível com Pênfigo Vulgar anal. Após corticoterapia local e normalização do trânsito intestinal, verificou-se cicatrização das lesões anais. INTRODUCTION: Anal involvement in patients with Pemphigus Vulgaris has rarely been reported. We report a case of a 76-year-old woman with muco-cutaneous Pemphigus Vulgaris on systemic corticotherapy who started with hematochezia and anal pain associated with constipation and difficult defecation. Flexible sigmoidoscopy was normal but proctologic examination revealed skin tags with multiple erosions. After local corticotherapy and normalization of bowel movements, healing of anal lesions was seen.

32. Estudo da bioactividade de péptidos - proteínas para a regeneração de tecidos

Dissertação de mestrado em Micro-Nano Tecnologias, O principal objectivo da medicina regenerativa e engenharia de tecidos é restaurar a função de tecidos danificados. Por esta razão, o desenvolvimento de biomateriais nestas áreas pode contemplar a incorporação de proteínas para mimetizar comportamentos endógenos. Alguns dos péptidos/proteínas bioactivas que têm sido utilizados para facilitar a reparação e reconstrução duma variedade de tecidos, evitando a formação de cicatrizes, são as matriquinas e a eritropoetina (EPO). As matriquinas podem ser obtidas a partir da matriz extracelular (MEC), através da descelularização de tecidos ou órgãos, sendo que neste trabalho será utilizada a MEC do urotélio da bexiga de porco (UBM). As matriquinas têm demonstrado efeitos quimiotáticos e mitogénicos em células progenitoras/estaminais. Também a EPO para além da sua actividade na eritropoiese, revelou possuir actividade quimiotática para este tipo de células. Como tem sido demonstrada a participação de células estaminais na remodelação dos tecidos in vivo, torna-se interessante examinar a resposta das células indiferenciadas aos péptidos derivados da UBM. Assim, o objectivo deste trabalho recai sobre a avaliação da actividade quimiotática e mitogénica de matriquinas nas células progenitoras/estaminais através de estudos in vitro e da avaliação da reparação de feridas in vivo. Foi ainda testada a incorporação da EPO num nanogel de dextrino, uma vez que um sistema de libertação controlada da proteína pode trazer vantagens para posteriores aplicações in vivo. Os resultados obtidos, embora preliminares, não confirmam o efeito na proliferação das células mesenquimais, ao contrário do esperado e do descrito na literatura. Por outro lado, nos ensaios de migração in vitro obtiveram-se resultados promissores, assim como nos estudos, in vivo, usando o modelo de feridas. Embora macroscopicamente não fossem visíveis diferenças nas feridas, a avaliação histológica sugere que nos animas tratados com o hidrolisado da UBM parece haver uma regeneração mais eficaz da pele lesada. No estudo de incorporação da EPO, os resultados sugerem que a proteína foi incorporada no nanogel de dextrino, sendo posteriormente libertada na presença de soro. Os estudos efectuados reforçam a ideia de que as matriquinas promovem a migração de células estaminais in vitro e que o recrutamento, in vivo, deste tipo de células para o local da lesão poderá ser fundamental para a regeneração dos tecidos, evitando a formação de cicatriz. A incorporação da EPO no nanogel dextrino abre novas possibilidades de estudo para o desenvolvimento de um novo método terapêutico na medicina regenerativa., The main goal of regenerative medicine and tissue engineering is to restore the function of damaged tissues. For this reason, the development of biomaterials for these applications can include the incorporation of proteins to mimic endogenous behaviors. Some of the bioactive peptides/proteins that have been used to facilitate the repair and reconstruction of a variety of tissues, preventing scar formation, are the matrikines and the erythropoietin (EPO). The matrikines are obtained from the extracellular matrix (ECM) through decellularization of tissues or organs and, in this work, the ECM used was from pig’s urinary bladder (UBM). The matrikines have shown chemotactic and mitogenic effects on progenitor/stem cells, as well as, EPO protein that beside its function on the red blood cells formation, it also showed chemotactic activity for MSCs. Since the participation of stem cells has been demonstrated in tissue remodeling in vivo, it is interesting to evaluate the response of undifferentiated cells to UBM’s peptides. Thus, the aim of this work falls on the evaluation of the mitogenic and chemotactic activity of matrikines in progenitor/stem cells through in vitro studies and assessment of wound repair in vivo. The incorporation of an EPO into a dextrin nanogel was also tested, in order to develop a protein delivery system, which can provide benefits for future in vivo applications. Preliminary the results do not confirm the effect on the proliferation of MSCs described in the literature. Nevertheless, promising results were obtained in the in vitro migration assays, as well as, in the in vivo wound regeneration studies. Although macroscopic differences were not detected, the histological evaluation suggests that the wounds of animals treated with UBM hydrolysates showed a more effective regeneration of damaged skin. Concerning of the EPO incorporation assays, the results suggest that EPO has been efficiently incorporated into dextrin nanogel, and its release was achieved in the presence of serum. These studies support the idea that matrikines are important factors in stem cells migration in vitro, and that the recruitment of this type of cells in vivo to the injury site could promote the tissue regeneration, preventing scar formation. The use dextrin nanogel as delivery system for EPO opens new possibilities to the development of a new therapeutic method in regenerative medicine.

34. Isolated Jejunal Crohn's Disease: a challenging diagnosis.

A 27-year-old Nepalese male presented with recurrent abdominal pain accompanied by a lower stool consistency over the past 2 years. These episodes occurred several times a year, lasting 1 to 2 weeks, and resolved spontaneously, after adjustment of diet and/or medication for symptomatic control (e.g., antispasmodics, probiotics). Over the last year, the patient had undergone an extensive diagnostic investigation, which revealed no alterations in the laboratory workup, abdominal scan, esophagogastroduodenoscopy, and colonoscopy, including biopsies of the duodenum, and colon, so the symptoms have been attributed to irritable bowel syndrome. However, the symptoms had become more frequent, so the patient was referred to our gastroenterology department. We repeated and extended the work-up. Laboratory investigations showed an elevated erythrocyte sedimentation rate and faecal calprotectin. The remaining laboratory as well an extensive stool workup for infection were unremarkable. Esophagogastroduodenoscopy and ileocolonoscopy were normal. Small bowel capsule endoscopy revealed jejunal mucosa with lymphangiectasias, pseudopolypoids formations and superficial longitudinal ulcers, these findings were corroborated by the double-balloon enteroscopy, and biopsies showed marked architectural distortion, chronic inflammatory infiltrate, and an epithelioid granuloma. The clinical, endoscopic, biochemical, and histological findings were consistent with isolated jejunal Crohn's disease. The patient started adalimumab with complete remission after one year. We present this case given its exuberant endoscopic findings and due to the difficulty in making the diagnosis due to its rarity, location, and unspecific presentation.

- Abstract
Lichen planus is a rare, idiopathic disease that usually involves the skin and mucosae. Oral lesions occur in two thirds of cases and may occur without skin involvement. Esophageal lichen planus occur more frequently in middle-age women, it is frequently asymptomatic but may cause odynophagia and dysphagia. Esophageal lichen planus has been associated with squamous cell carcinoma. The most effective treatment is systemic corticotherapy, but relapse is expected in 85% with steroid withdrawal. We present the case of esophageal lichen planus in a 50-year-old man.

36. A comparative study of two methods of colonic cleansing before colonoscopy.

Background/aims: good colonic preparation is essential to perform a complete and safe colonoscopy. The preparation used in the Endoscopy Unit was adopted after testing it against the classic 4-liter polyethylene glicol (PEG) preparation (Klean-Prep). Recently a new 3-liter PEG preparation was commercialised in Portugal, Endo-Falk, which appeared to be simpler and effective. The present study compared the preparation we usually use (a combination of 4 tablets of bisacodyl with 2-liter PEG and enemas) with this new preparation., Methodology: we enrolled 305 patients in 2 groups. Group 1 was given our usual preparation, as described above and group 2 was given the new 3-liter preparation, Endo-Falk. All patients were ambulatory, and only patients undergoing total colonoscopies were included. Patients with benign or malignant stenosis and patients who had previously undergone colorectal resection were excluded. Preparation was graded as good when there were no stools and little liquid in the colon, fair when there were stools or liquid that could be aspirated and did not obscure the mucosa and bad when stools or excess liquid prevented an adequate view of the surface or precluded polipectomy., Results: The present study included 164 male and 141 female patients, with a mean age of 61.3 years (no difference between the demographic data). In group 1 the preparation was good in 57.5%, fair in 28.5% and bad in 14% of patients and in group 2 the preparation was good in 33.1%, fair in 48.9% and bad in 18% (p < 0.001).
