Abstract

Abstract Alzheimer's disease (AD) is considered to be the most typical form of dementia that provokes irreversible cognitive impairment. Along with cognitive impairment, circadian rhythm dysfunction is a fundamental factor in aggravating AD. A link among circadian rhythms, sleep, and AD has been well‐documented. The etiopathogenesis of circadian system disruptions and AD serves some general characteristics that also open up the possibility of viewing them as a mutually reliant path. In this review, we have focused on different factors that are related to circadian rhythm dysfunction. The various pathogenic factors, such as amyloid‐beta, neurofibrillary tangles, oxidative stress, neuroinflammation, and circadian rhythm dysfunction may all contribute to AD. In this review, we also tried to focus on melatonin which is produced from the pineal gland and can be used to treat circadian dysfunction in AD. Aside from amyloid beta, tau pathology may have a notable influence on sleep. Conclusively, the center of this review is primarily based on the principal mechanistic complexities associated with circadian rhythm disruption, sleep deprivation, and AD, and it also emphasizes the potential therapeutic strategies to treat and prevent the progression of AD.

Abstract

We aimed to further explore the relationship between heterocyclic molecules and their associated biological activities for acetylcholinesterase (AChE) and butyrylcholinesterase (BChE) enzymes. A dataset of 36 heterocycles was used to predict the activity of AChE and BChE inhibitors (the pIC50 values ranged from 7.84 to 12.49). A quantitative structure-activity relationship (QSAR) study was generated with the help of four different models (BMA, MNLR, MLR, and ANN). Four of the models were statistically acceptable based on both internal and external validation. The descriptors used in the models were similar to the X-ray structures of the target-ligand complexes, which made it possible to predict the pIC50 for AChE and BChE enzymes. Five selected molecules (compounds 6 (C 21 H 21 F 3 N 4 O), compound 7 (C 22 H 23 F 3 N 4 O), and compound 8 (C 22 H 23 F 3 N 4 O 2 ) belong to the oxadiazole derivative group; compound 16 (C 17 H 13 ClN 2 O 3 ) is classified into the chemical structures of different N, O, and S-based heterocycle groups; and compound 25 (C 19 H 17 NO 2 ) pertains to the pyrimidine derivative group) possessed high pIC50 values for AChE and BChE enzymes (pIC50 values for AChE and BChE ranged from 9.01 to 10.32). The range of docking scores between the AChE and BChE receptors and their respective candidates was from -8.1 to -9.2 kcal/mol. The pharmacokinetics, biological activities, and physicochemical properties of five selected compounds supported their ability to protect against AD because they are not toxic, have a cholinergic effect, can cross the blood-brain barrier, and are well absorbed by the gastrointestinal tract.Communicated by Ramaswamy H. Abstract

The Ethics Subcommittee of AMDA–The Society for Post-Acute and Long-Term Care Medicine ("The Society") presents arguments for and against Stopping Eating and Drinking by Advance Directives (SED by AD). SED by AD is a type of advance directive in which a proxy is instructed to stop offering food and fluids to a person when they reach a certain stage of dementia. Although most conversations regarding SED by AD focus on patient autonomy and the right to determine one's care, we propose that the ethical principle of justice—the obligation to treat all individuals equally regardless of race, gender, and physical or cognitive ability—is the decisive principle in this controversy. We also suggest that implementing SED by AD can violate a physician's obligation to beneficence and nonmaleficence. On the other hand, we identify with the families of our patients who see the refusal to follow an advance directive as an injustice of the highest order. In the end, The Society is convinced that no choice can be made here without practicing an injustice: if one refuses to implement SED by AD, one violates the autonomy of the person who drew up the advance directive. If, on the other hand, one refuses food and fluid to a resident who still accepts food, one risks practicing an injustice against that person as they are now. Recognizing that we have the greatest responsibility to our patients as they present to us in the residential setting, The Society recommends against implementing SED by AD in residents who still accept food and fluids, implementing instead, a policy of comfort feeding for those with advanced dementia. Abstract

Transgenic Drosophila melanogaster lines modeling the main neuromorphological markers of Alzheimer's disease were used to study the neuroprotective properties of nicotinic acid. Thirty days old flies treated with nicotinic acid showed better survival rate and reduced neurodegeneration.

Abstract

International audience; Most research has mainly focused on the decline of the subjective experience in Alzheimer's disease (AD). However, few attempts have been made to evaluate whether subjective experience may be maintained in AD. In this narrative review, we attempt to provide a positive view, according to which patients with AD can enjoy, to some extent, subjective experience during memory retrieval. Memory and expression difficulties (e.g., aphasia) limit the ability of patients with AD to describe their memories, resulting in a little specificity of reported memories. However, according to the "authentic subjective experience" view, we propose in this study that the ability to mentally relive these memories could be preserved in the patients. By proposing the authentic subjective experience view, we attempt to provide an alternative view to the general consideration that the patients suffer a diminished subjective experience. This view can contribute to a larger clinical framework that gives a positive meaning to the subjective experience of patients with AD. Furthermore, several clinical and empirical implications can be drawn from the authentic subjective experience view, including the possibility to evaluate behavioral correlates of the subjective experience in AD.

Abstract

Objective To evaluate the performance of a Random Forest (RF) classifier on Transcranial Magnetic Stimulation (TMS) measures in patients with Mild Cognitive Impairment (MCI). Methods We applied a RF classifier on TMS measures obtained from a multicenter cohort of patients with MCI, including MCI-Alzheimer’s Disease (MCI-AD), MCI-frontotemporal dementia (MCI-FTD), MCI-dementia with Lewy bodies (MCI-DLB), and healthy controls (HC). All patients underwent TMS assessment at recruitment (index test), with application of reference clinical criteria, to predict different neurodegenerative disorders. The primary outcome measures were the classification accuracy, precision, recall and F1-score of TMS in differentiating each disorder. Results 160 participants were included, namely 64 patients diagnosed as MCI-AD, 28 as MCI-FTD, 14 as MCI-DLB, and 47 as healthy controls (HC). A series of 3 binary classifiers was employed, and the prediction model exhibited high classification accuracy (ranging from 0.72 to 0.86), high precision (0.72–0.90), high recall (0.75–0.98), and high F1-scores (0.78–0.92), in differentiating each neurodegenerative disorder. By computing a new classifier, trained and validated on the current cohort of MCI patients, classification indices showed even higher accuracy (ranging from 0.83 to 0.93), precision (0.87–0.89), recall (0.83–1.00), and F1-scores (0.85–0.94). Conclusions TMS may be considered a useful additional screening tool to be used in clinical practice in the prodromal stages of neurodegenerative dementias.

Abstract

Because of the complex cascade of molecular events that can occur in the brain of an Alzheimer''s disease (AD) patient, the therapy of this neurodegenerative disease seems more likely to be achieved by multifunctional drugs. Herein, a new series of dual-targeting ligands have been developed and in vitro bioevaluated. Their architecture is based on conjugating the acetylcholinesterase inhibition and anti-oxidant properties in one molecular entity. Specifically, a series of naturally occurring phenolic acids with recognized anti-oxidant properties (derivatives of caffeic acid, rosmarinic acid, and trolox) have been conjugated with choline to account for the recognition by acetylcholinesterase (AChE). The synthesized hybrid compounds evidenced AChE inhibitory capacity of micromolar range (rationalized by molecular modeling studies) and good antioxidant properties. Their effects on human neuroblastoma cells, previously treated with beta-amyloid peptides and 1-methyl-4-phenylpyridinium ion neurotoxins (to simulate AD and Parkinson''s disease, respectively), also demonstrated a considerable capacity for protection against the cytotoxicity of these stressors. Current interest in amyloid fibrils stems from their involvement in neurodegenerative and other diseases and from their role as an alternative structural state for many peptides and proteins. Solid-state nuclear magnetic resonance (NMR) methods have the unique capability of providing detailed structural constraints for amyloid fibrils, sufficient for the development of full molecular models. In this article, recent progress in the application of solid-state NMR to fibrils associated with Alzheimer's disease, prion fibrils, and related systems is reviewed, along with relevant developments in solid-state NMR techniques and technology. Abstract

Objectives: Estrogen is known to prominently benefit neuronal syndromes and neurodegenerative diseases. Ginsenoside Rg1, an active ingredient found in a Chinese plant, ginseng root, was previously demonstrated to exert estrogen-like activity. This study was performed to assess the neuroprotective effect of ginsenoside Rg1 against apoptosis induced by ββ-amyloid protein 25--35 (Aββ25--35) in primary cultured rat hippocampal neuronal cells as well as in the underlying mechanisms. Methods: We first measured cell viability and lactate dehydrogenase (LDH) release from primary cultured rat hippocampal neurons. After that, the inhibition effects of ginsenoside Rg1 on neuronal cell apoptosis were evaluated with flow cytometric analysis. Furthermore, western blot analysis was used for detecting the expression of apoptosis-related proteins Bcl-2, Bax, and active caspase 3. Results: The results show that ginsenoside Rg1 could increase neuronal viability and reduce LDH release; rescue cell apoptosis induced by Aββ25--35; decrease the expression of caspase 3, increase the ratio of Bcl-2/Bax at the protein levels compared with the cells only treated with Aββ25--35. Conclusions: Taken together, our results indicate that the apoptosis induced by Aββ25--35 could be reversed by ginsenoside Rg1. Furthermore, this neuroprotective effect is probably mediated by up-regulating the ratio of Bcl-2/Bax that activates caspase 3. [ABSTRACT FROM AUTHOR]

Abstract

Background The natural history and disease mechanisms of Alzheimer’s disease and related disorders (ADRD) are still poorly understood. Very few resources are available to scrutinise patients as early as needed and to use integrative approaches combining standardised, repeated clinical investigations and cutting-edge biomarker measurements. Methods In the nationwide French MEMENTO cohort study, participants were recruited in memory clinics and screened for either isolated subjective cognitive complaints (SCCs) or mild cognitive impairment (MCI; defined as test performance 1.5 SD below age, sex and education-level norms) while not demented (Clinical Dementia Rating [CDR]

Abstract

To investigate the characteristics of the caregiving experience according to age at onset of dementia to adapt support programs.Fifty-seven spouse caregivers of persons with early-onset dementia (PEOD) and 93 spouse caregivers of persons with late-onset dementia (PLOD) participated. The characteristics of the caregiving experience were assessed using questionnaires. We compared the two groups according to age at onset of the disease using a multivariate test, Pillai's Trace test.The analysis showed that there were similarities and differences between the two groups of spouse caregivers. All spouse caregivers were confident in their caregiving role and fairly well prepared for future needs and reported mild depressive and anxious symptoms. However, they lacked informal support, had low confidence in requesting respite care and reported effects on their health. Compared to spouse caregivers of PLOD, spouse caregivers of PEOD had more severe perceptions of the cognitive disorders of persons with dementia (PWD) and had a better sense of preparedness and knowledge of services. Spouse caregivers of PLOD were more confident in their ability to control disturbing thoughts.The results suggest that programs should provide information on support networks to improve preparedness for spouse caregivers of PLOD as well as emphasizing positive coping strategies for caregivers of PEOD to maintain good-quality relationships with PWD, which influences the perception of the symptoms. For both groups, family relationships should be considered.

Wir schlagen daher die aktometrische Darstellung von motorischer Aktivität, wie sie etwa aus der Schlafforschung bekannt ist, vor. In dieser Dissertation wurde zunächst die Reliabilität und konkurrente Validität der Messung von motorischer Unruhe bei Demenz mittels eines aktometrischen Gerätes untersucht. Die Ergebnisse zeigen, dass die aktometrische Messung von Verhaltensstörungen eine valide und reliable Methode auch bei schwerer Demenz darstellt. Im zweiten Teils dieser Arbeit wurde unter quasiexperimenteller Kontrolle von Modulatoren der circadianen Rhythmizität Unterschiede der circadianen Rhythmik zwischen vaskulärer Demenz und Demenz vom Alzheimer-Typ untersucht. Dabei konnte gezeigt werden, dass es bei Patienten mit Demenz vom Alzheimer-Typ und bei Patienten mit vaskulärer Demenz in Abhängigkeit von der motorischen AktivitŠt zu einer charakteristischen Phasenverschiebung der circadianen Rhythmik kommt. Dieser Zusammenhang ist jedoch bei Alzheimer-Demenz, bei der von einer neurobiologischen Störung des suprachiasmatischen Nukleus ausgegangen werden kann, stärker ausgeprägt. Dieses Befundmuster unterstützt theoretische Modelle der circadianen Rhythmik und ihrer Störung bei Demenz und legt nahe, dass der suprachiasmatischen Nukleus bei Demenz vom Alzheimer-Typ eine zentrale Rolle bei der circadianen Regulation des Verhaltens einnimmt. Behavioural signs and symptoms in dementia are of central clinical significance. There are a number of standardised rating scales available for the assessment of motor phenomena in dementia. However, there is no objective method of assessing these symptoms. In addition, the reliability of these scales, especially when used by caregivers from within families, has been questioned. In order to overcome this flaw, we propose the use of an actometric device for assessing behaviour motor symptoms in dementia. In the first part of this dissertation we assessed the reliability and the concurrent validity of an actometric device against two behavioural scales. Results show satisfactory validity and good reliability of this method. In the second part of this dissertation, we reanalysed data from our validation study, investigating whether the pattern of circadian rhythm disturbances is different in patients suffering from Alzheimer disease and patients suffering from vascular dementia, controlling experimentally for the severity of behaviour disturbances. With regards to circadian motor activity, we found increased nocturnal activity and fragmentation of diurnal rhythm in both groups. In patients showing an equal severity of behaviour disturbances, the phase-delay of the rest-activity rhythm was delayed in patients with Alzheimer disease as compared to patients with vascular dementia. These findings suggest that, in Alzheimer disease, structural changes in the SCN might induce disturbance in the circadian pacemaker, leading to a phase shift in the circadian rhythm. The differential pattern of rhythm disturbance found in this study could be indicative of different processes involved in sleep disorders in the dementias.
