Purpose: Visual and motility disorders have been reported in cases of syndromic and non-syndromic craniosynostosis. However, to the best of our knowledge, no regional or local studies have addressed the prevalence of ocular manifestations in craniosynostosis among Middle Eastern populations. The purpose of this study was to describe the frequency and nature of ophthalmic abnormalities in Arab children with craniosynostosis and to evaluate these findings pre- and post-craniofacial surgery., Materials and Methods: A prospective cohort study of 31 children with craniosynostosis. Demographic information was gathered along with genetic results and ophthalmic evaluations, including cycloplegic refraction., Results: Age at presentation to the ophthalmology service ranged from 14 months to 8 years, with a median of 3 years. Sixteen patients were female (52%), and 24 patients had undergone a cycloplegic refraction. Of these, 20.8% had significant refractive errors; of those with refractive errors, 40% were myopic and 60% were hyperopic. Eight patients (25.8%) had papilledema, with significant improvement after craniofacial surgery. Five patients (16%) had visual acuity≤20/30 in at least one eye at baseline with improvements in follow-up visits., Conclusion: Craniosynostosis has a significant impact on visual function in the Arab population. Potentially correctable cases of visual impairment, such as those caused by amblyogenic refractive error and papilledema, are commonly encountered; therefore, timely referral and regular ophthalmic evaluation are recommended for these patients., (Copyright © 2024 Elsevier Masson SAS. Purpose: This article aims to describe the causes of ophthalmological disqualification from the military services detected during specialist consultations conducted at Army Training Hospitals., Methods: This observational, cross-sectional, multicenter study retrospectively included individuals deemed as "unfit for military service" due to eye diseases identified during the specialist consultation conducted at 3 ATHs between January 2020 and December 2021. The data collected included age, medical and surgical history, reasons for ophthalmological disqualification, uncorrected distance visual acuity, best corrected distance visual acuity and cycloplegic refraction., Results: Over this period, 133 subjects (98 men and 35 women) were included. Thirty-eight candidates (28.6%) were declared unfit due to a refractive error beyond the required limits, including 30 myopic subjects in excess of -10 diopters (D) and 8 hypermetropic subjects over +8 D. Twenty-five candidates (18.8%) were unfit under the age of 21 years due to corneal refractive surgery performed before the required age. Four subjects (3.0%) were unfit due to phakic intraocular lenses. Degenerative conditions were observed in 23 subjects (17.3%), including 21 patients with severe keratoconus. Other causes of incapacity were linked to oculo-orbital trauma in 11 subjects (8.3%), moderate or severe amblyopia in 7 patients (5.3%), congenital causes in 7 subjects (5.3%), inflammatory or infectious diseases in 7 candidates (5.3%), hereditary causes in 6 subjects (4.5%) and undetermined visual dysfunctions in 4 subjects (3.0%)., Conclusion: The three main causes of ophthalmological disqualification were high ametropia, refractive surgery performed before the required age and keratoconus., (Copyright © 2024 Elsevier Masson SAS. L'amblyopie est un trouble du développement qui intéresse l'ensemble du système visuel et du cortex visuel au-delà du cortex extra-strié. Le principal retentissement fonctionnel est une altération de l'acuité visuelle de l'œil dominé. Mais, tous les paramètres de la fonction visuelle sont perturbés puisqu'il peut être mis en évidence un déficit de la sensibilité aux contrastes ou de la vision stéréoscopique et une hyperacuité visuelle Vernier au niveau de l'œil « sain ». Ce trouble du développement ne peut s'installer qu'au cours de la période critique du développement cérébral, période de maturation et de plus grande plasticité cérébrale, et plus volontiers lors du pic de celle-ci, avant l'âge de 3 ans. Cette période est également celle au cours de laquelle la rééducation est la plus efficace. L'amblyopie est due soit à une privation visuelle, soit à un strabisme soit enfin à une anisométropie. Elle peut également s'associer à des troubles organiques lors de formes dites « mixtes ». L'amblyopie est définie par une différence d'acuité visuelle d'au moins une ligne d'acuité visuelle LogMAR. Hormis lorsqu'il existe un strabisme franc, ce déficit visuel reste souvent méconnu chez un enfant qui ne se plaint pas et voit bien d'un œil. C'est dire l'importance du dépistage, cherchant toutes les situations à risques telles qu'une différence de réfraction entre les deux yeux ou des facteurs héréditaires. Dans tous les cas, la confirmation de sa présence est indispensable avant la mise en route de la rééducation. Cette mise en évidence repose sur la mesure de la réfraction sous cycloplégie, le bilan ophtalmologique et orthoptique. L'acuité visuelle ou l'acuité de résolution peuvent être mesurées, avec des contraintes liées à l'âge. La rééducation nécessite pour être efficace la coopération active des parents. Elle repose ensuite sur la prescription de la correction optique totale déterminée sous cycloplégie qui peut parfois suffire à corriger cette amblyopie. La rééducation en elle-même est basée sur l'occlusion ou la pénalisation de l'œil sain permettant à l'œil dominé de développer sa vision et doit être complétée par un traitement d'entretien jusqu'à la fin de la période critique. En son absence, il existe un risque de récidive. Amblyopia is a developmental disorder that affects the entire visual system and the visual cortex beyond the extra-striated cortex. The main functional manifestation is an impairment of visual acuity in one eye called "the dominated eye". However, all the parameters of visual function are disturbed since it can be demonstrated a deficit in contrast sensitivity or stereoscopic vision. In addition, a Vernier visual hyperacuity is frequently observed in the "healthy" eye. This developmental disorder can only occur during the critical period of brain development, a period of maturation and greater brain plasticity, and more readily at its peak, before the age of 3. Rehabilitation is also most effective during this period. Amblyopia is due either to visual deprivation, or to strabismus or finally to anisometropia. It can also be associated with organic disorders in so-called "mixed" forms. Amblyopia is defined as a difference in visual acuity of at least one LogMAR visual acuity line. Except when there is a manifest strabismus, this visual deficit is often unrecognised in a child who does not complain and sees well with one eye. This shows the importance of screening, looking for all risky situations, such as a difference in refraction between the two eyes or hereditary factors. In all cases, confirmation of its presence is essential before starting rehabilitation. This demonstration is based on the measurement of refraction under cycloplegia, the ophthalmological and orthoptic assessment. Visual acuity or grating acuity can be measured by specific method according to the age of the child, with age-related difficulties. In order to be effective, rehabilitation requires the active cooperation of parents. It is then based on the prescription of total optical correction determined under cycloplegia which may sometimes be sufficient to correct this amblyopia. The rehabilitation itself is based on the occlusion or penalisation of the healthy eye allowing the dominated eye to develop its vision and must be supplemented by maintenance treatment until the end of the critical period. Problemstellung Diese Bachelorarbeit beschäftigt sich mit den orthoptischen Untersuchungsmethoden und dem weiteren Vorgehen bei Verdacht auf eine zerebrale visuelle Formwahrnehmungsstörung im Kindesalter. Die zentrale Fragestellung lautet, ob durch die Anwendung von LEA Symbolen bei der Visusprüfung der Verdacht auf eine visuelles Formwahrnehmungsstörung bei Kindern festgestellt werden kann und wie bei Schwierigkeiten oder Defiziten weiter untersucht wird. Methoden Damit diese Forschungsfrage beantwortet werden kann, wird eine umfangreiche Literaturrecherche durchgeführt. Es wird nach Studien und Reviews in den verschiedenen Datenbanken wie PubMed gesucht. Ergebnisse In den Ergebnissen wird auf die LEA Symboltests eingegangen und wie Verdacht auf eine zerebrale visuelle Formerkennungsstörung gestellt werden kann. Es wird auch beschrieben wie als Orthoptist*in bei Verdacht auf Defizite weiter vorgegangen werden soll. Diskussion Im Kapitel Diskussion werden die Ergebnisse kritisch betrachtet, verglichen und diskutiert. Die orthoptischen Untersuchungsmethoden und das Vorgehen bei zerebraler visueller Formwahrnehmungsstörung werden wissenschaftlich diskutiert, und es werden umfassende Konsequenzen und Möglichkeiten für die berufliche Praxis aufgezeigt. Problem statement This bachelor's thesis focuses on orthoptic examination methods and the subsequent steps in cases of suspected cerebral visual shape perception disorder in children. The central research question is whether the use of LEA symbols during visual acuity testing can help identify visual shape perception deficits in children and determine further investigation strategies when difficulties or deficits are observed. Methods To answer this research question, an extensive literature review was conducted, searching for relevant studies and reviews in databases such as PubMed. Results The results section discusses the LEA symbol tests and how they can be used to identify suspected cerebral visual shape recognition disorders. It also describes the recommended approach for orthoptists when further investigating deficits. Discussion In the discussion chapter, the results are critically examined, compared, and discussed. The orthoptic examination methods and approaches to cerebral visual perceptual disorders are scientifically debated, and comprehensive implications and possibilities for professional practice are presented.

21. Therapiemanagement der okulären Symptomatik bei einem Patienten mit Verdacht auf Marfan-Syndrom : Ein Fallbericht

Einleitung: Diese Bachelorarbeit beschäftigt sich mit dem Fall eines jungen Patienten mit Verdacht auf ein Marfan-Syndrom, bei dem eine linsenbedingte hochgradige Refraktionsanomalie und eine daraus resultierende Amblyopie, vorhanden sind. Das Marfan-Syndrom gehört zu den häufigsten autosomal-dominanten systemischen Bindegewebserkrankungen, welche mit kardiovaskulären Veränderungen, Skelett- und Augenanomalien assoziiert ist und sich durch okuläre Erscheinungen, wie Linsenverlagerungen und linsenbedingte Refraktionsanomalien charakterisiert. Das Ziel dieser Arbeit ist, die orthoptische Relevanz bei der Behandlung der okulären Manifestation von Patient*innen mit Marfan-Syndrom darzustellen, da dies bisher in Fachliteratur kaum beschrieben wird. Methodik: Die angewandten Untersuchungsmethoden bei dem analysierten Patientenfall waren regelmäßige orthoptische und ophthalmologische Untersuchungen, die unter anderem eine Refraktionsbestimmung, Visusprüfung, Pentacam-Untersuchung und Begutachtung des vorderen und hinteren Augenabschnitts beinhalten. Ergebnisse: Die Untersuchungen ergaben einen hochgradigen Astigmatismus, eine Myopie und eine mittelgradige Amblyopie beidseits. Des Weiteren zeigte sich in der ophthalmologischen Begutachtung eine Lentodonesis und eine Subluxatio lentis nach temporal oben. Diskussion: Die analysierten angewandten Untersuchungsmethoden bzw. Therapiemaßnahmen und der Vergleich zu aktueller Literaturrecherche zeigen die Wichtigkeit einer raschen ophthalmologischen, sowie orthoptischen Intervention, da diese zur Prävention einer irreversiblen Amblyopie dienen. Die empfohlenen effektiven Behandlungsmaßnahmen sind einerseits eine Lensektomie gefolgt von der Implantation einer aphaken irisfixierten Intraokularlinse und andererseits eine Aphakiekorrektur. Introduction: This bachelor thesis deals with the case of a young patient with suspected Marfan-syndrome, in which a lenticular high-grade refractive anomaly and resulting amblyopia, are present. Marfan-syndrome is one of the most common autosomal-dominant systemic connective tissue diseases associated with cardiovascular changes, skeletal, and ocular abnormalities. It is characterised by ocular manifestations such as lens displacement and lens-related refractive anomalies. This bachelor thesis aims to present the orthoptic relevance in the management of ocular manifestations of patients with Marfan-syndrome, as this has been poorly described in the literature. Methodology: The examination methods, applied in the analysed patient case are regular orthoptic and ophthalmologic examinations, including refraction, visual acuity, Pentacam- examination, and assessment of the anterior and posterior segment. Results: The orthoptic examination revealed a high degree of astigmatism, myopia, and moderate amblyopia in both eyes. Furthermore, the ophthalmologic evaluation showed a lentodonesis and a superotemporal subluxation of the lens. Discussion: The analysed examination methods, therapeutic measures and the comparison with current literature research show the importance of a quick ophthalmological and orthoptic intervention, as these serve to prevent irreversible amblyopia. The recommended effective treatment measures are lensectomy, implantation of an aphakic iris fixated intraocular lens and aphakia therapy.

Zusätzlich kommt es zu einer Auseinandersetzung hinsichtlich der therapeutisch am sinnvollsten Behandlungsmethode im Rahmen einer Amblyopiediagnose. Zur Beantwortung der Forschungsfragen wird eine systematische Literaturrecherche verwendet, um für die Thematik relevante Literatur aus Fachdatenbanken wie PubMed bzw. ScienceDirect ausfindig zu machen. Anhand der vorliegenden Ergebnisse lässt sich erkennen, dass die Dauer der Foveation ausschlaggebend ist bei der Beurteilung der Visusentwicklung. Bei einem latenten bzw. manifest latenten Nystagmus (LN/MLN) lässt die Recherche bisher nur Vermutungen über einen möglichen Einfluss zu. Der Spasmus nutans (SN) hingegen übt aufgrund gewisser Kompensationsmechanismen wie dem charakteristischen Kopfnicken, keinerlei Einfluss auf den Visus aus. Das Krankheitsbild birgt lediglich bei stark ausgeprägter Asymmetrie ein Riskio für die Entwicklung einer Amblyopie. Im Hinblick auf die Wahl der optimalen Amblyopietherapie ist zu betonen, dass ein Nystagmus weder eine Okklusion noch eine Penalisation völlig ausschließt. Die letztendliche Entscheidung sollte stets anhand diverser Faktoren, wie etwa dem Alter, der Fixation, dem Schweregrad der Amblyopie oder der vorliegenden Compliance getroffen werden. This bachelor thesis deals with the three most common forms of benign infantile nystagmus and their influence on the development of visual acuity. In addition, it discusses the success of therapy in the context of an amblyopia diagnosis. In order to answer the research questions, a systematic literature search is used to find literature relevant to the topic from specialist databases such as PubMed and ScienceDirect. The present results show that the duration of foveation is decisive in the assessment of visual acuity development. In the case of latent or manifest latent nystagmus (LN/MLN), the research so far only allows assumptions about a possible influence. Spasmus nutans (SN), on the other hand, has no influence on visual acuity due to certain compensatory mechanisms such as the characteristic nodding of the head. This clinical picture only bears a risk for the development of amblyopia in cases of severe asymmetry. Regarding the choice of optimal amblyopia therapy, it should be emphasised that nystagmus does not completely exclude occlusion or penalisation. The final decision should always be made based on various factors such as age, fixation, severity of amblyopia or compliance.

23. Persistance de la membrane pupillaire bilatérale et forte myopie

Après la naissance, des vestiges de la membrane pupillaire qui constituait l'apport vasculaire pour le cristallin peuvent persister. Ces vestiges restent accolés à la collerette irienne pouvant causer une amblyopie par privatisation en obstruant l'aire pupillaire. Nous allons rapporter le cas d'un enfant de 4 ans, de parents consanguins et qui présente une persistance de la membrane pupillaire bilatérale associée à une forte myopie. Son examen a montré des vestiges de la membrane pupillaire bilatérale, un sphincter irien intact et au fond d'éil une atrophie chorio rétinienne diffuse. L'acuité visuelle était difficile à apprécier, la réfraction par contre à montrer l'existence d'une forte myopie OD -10.75 (-3.25 A 29) OG -10 (-0.75 A 180). La gestion de la persistance de la membrane pupillaire peut se faire par l'utilisation d'agents mydriatiques, par excision chirurgicale ou par destruction au laser. Pour notre cas nous avons opté pour l'utilisation d'agents mydriatiques avec prescription de correction optique totale et traitement de l'amblyopie.

To synthesize and appraise economic evaluations of vision screening to detect vision impairment in children.Literature searches were conducted on seven electronic databases, grey literature, and websites of agencies conducting health technology assessments. Studies were included if they (1) were full, comparative economic evaluations that used cost-utility, cost-benefit, cost-effectiveness, cost-consequence, or cost-analysis methods; (2) described screening services designed to detect amblyopia, strabismus, or uncorrected refractive errors in children under 6 years of age; and (3) published after 1994. High-quality studies were synthesized descriptively. Currencies were reported in 2019 Canadian dollars. Quality was assessed with the Pediatric Quality Appraisal Questionnaire (PQAQ).Vision screening services were conducted by paid staff, volunteers, or health care professionals in schools or clinics. Thirteen studies were published from five countries: China (n = 1), United States (n = 4), United Kingdom (n = 1), Canada (n = 1), and Germany (n = 6). Analytical techniques included cost-utility/cost-effectiveness combination (n = 2), cost-effectiveness analysis (n = 7), cost-utility analysis (n = 1), cost-benefit analysis (n = 1), cost-consequence analysis (n = 1), and cost analysis (n = 1). Incremental cost-effectiveness ratios ranged from C$1,056 to C$151,274 per additional case detected/prevented and from C$9,429 to C$30,254,703 per additional QALY gained, depending on the type of screening service and comparator. Six studies were determined to be of high quality.Vision screening to detect amblyopia for young children may be cost-effective compared with no screening if amblyopia reduced quality of life. Studies varied significantly in the type of screening services and comparators used. Methodological limitations were common. Future studies would be aided immensely by prospective studies on the impact of amblyopia on the health-related quality of life of young children and guidelines on the effective conduct of vision screening.RéSUMé: OBJECTIF: Synthétiser et évaluer des évaluations économiques de dépistages visuels visant à détecter la déficience visuelle chez les enfants. MéTHODE: Nous avons interrogé sept bases de données électroniques, la littérature grise et les sites Web d’organismes effectuant des évaluations des technologies de la santé. Nous avons inclus les études correspondant aux critères suivants : (1) évaluations économiques comparatives exhaustives utilisant l’analyse coûts-utilité, coûts-bénéfices, coûts-efficacité ou coûts-conséquences ou l’analyse des coûts; (2) décrivant des services de dépistage visant à détecter l’amblyopie, le strabisme ou les anomalies de la réfraction non corrigées chez les enfants de moins de six ans; et (3) publiées après 1994. Nous avons fait la synthèse descriptive des études de haute qualité. Les devises ont été converties en dollars canadiens de 2019. Nous avons évalué la qualité des études à l’aide de l’outil PQAQ (Pediatric Quality Appraisal Questionnaire). RéSULTATS: Les services de dépistage visuel étaient offerts par du personnel rémunéré, des bénévoles ou des professionnels de santé dans des écoles ou des cliniques. Treize études ont été publiées dans cinq pays : Chine (n = 1), États-Unis (n = 4), Royaume-Uni (n = 1), Canada (n = 1) et Allemagne (n = 6). Les techniques d’analyse employées étaient la combinaison analyse coûts-utilité/analyse coûts-efficacité (n = 2), l’analyse coûts-efficacité (n = 7), l’analyse coûts-utilité (n = 1), l’analyse coûts-avantages (n = 1), l’analyse coûts-conséquences (n = 1) et l’analyse des coûts (n = 1). Les rapports coût-efficacité différentiels s’échelonnaient entre 1 056 $ CA et 151 274 $ CA par cas supplémentaire détecté/prévenu et entre 9 429 $ CA et 30 254 703 $ CA par année de vie pondérée par la qualité (AVPQ) supplémentaire gagnée, selon le type de service de dépistage et le comparateur. Six études ont été jugées être de haute qualité. CONCLUSION: Comparativement à l’absence de dépistage, les dépistages visuels pour détecter l’amblyopie chez les jeunes enfants peuvent être efficaces par rapport à leur coût lorsque l’amblyopie réduit la qualité de vie. L’occlusion de l’œil qui voit le mieux comme méthode pour traiter l’amblyopie est depuis longtemps une norme de soins et il existe une différence entre les praticiens sur l’utilisation soit de l’occlusion à temps plein, soit de l’occlusion à temps partiel. Le but de ce travail était donc de comparer ces deux méthodes de traitement en termes d’efficacité, de temps de récupération de l’acuité visuelle de l’œil amblyope, mais aussi en termes d’observance. Pour ce faire, nous avons passé en revue les différentes études effectuées sur le sujet et élaboré un sondage à destination des orthoptistes du monde entier afin de recueillir leur avis sur le traitement d’occlusion et ses différentes méthodes pour l’amblyopie. Il a été largement démontré par les études et les articles sur l’amblyopie que la thérapie d’occlusion, qu’elle soit partielle ou totale, était un traitement efficace pour « venir à bout » de l’amblyopie, mais qu’il était également difficile à mettre en place du fait de ses problèmes d’observance et d’acceptabilité. Malgré les innombrables études prouvant que l’occlusion à temps partiel est aussi efficace, voire meilleure, que l’occlusion à temps plein pour l’amblyopie légère à modérée, de nombreux orthoptistes préconisent encore une thérapie d’occlusion à temps plein. L’analyse des réponses au sondage nous a permis de conclure qu’il n’y a pas vraiment de règle à respecter concernant le traitement de l’amblyopie et que chaque prise en charge est différente.

L’OCT Angiographique est une méthode non invasive et sans contact permettant la visualisation 3D du réseau vasculaire rétinien et choroïdien. Il est utilisé dans le suivi de pathologies entraînant des répercussions sur la rétine telles que la DMLA, l’OMC ou la rétinopathie diabétique accédant aux images d’œdèmes, d’atrophies, de néo-vascularisations ou encore d’occlusions. Il pourrait paraître intéressant d’utiliser cette technique pour étudier un potentiel changement de la structure rétinienne dans l’amblyopie. Cette pathologie connue comme étant une asymétrie de développement entre les deux yeux, est plus connue par sa dysfonction fonctionnelle que sa dysfonction anatomique, cette dernière étant explorée depuis peu. Pour se faire, l’étude de la modification de différentes données semble indispensable à effectuer à plusieurs reprises au cours du traitement d’occlusion pendant la période de maturation cérébrale pour mettre en évidence une évolution. Tout changement représenterait un argument de plus dans la prise en charge précoce de l’amblyopie. En effet, s’il y a une rectification anatomique qui doit s’effectuer pour que l’isoacuité soit retrouvée, alors cela ne peut se passer que pendant la période sensible durant laquelle toutes les structures sont plus facilement modulables. Cette période sensible s’étendrait jusqu’à l’âge de 13 ans (cf étude Milleret) (1).

Summary Purpose To evaluate the Spot Vision Screener (SVS) compared with the orthoptic examination for detection of amblyopia risk factors in preschools. Methods This prospective study included children with a visual screening organized by the department of “le” (PMI) in Cote d’Or (Burgundy, France), between June 2017 and April 2018. All children were evaluated with the SVS followed by a clinical orthoptic examination. Results with the SVS were compared with those obtained by clinical orthoptic examination. Results A total of 1236 subjects were included in the study from 100 preschools. The mean age of the children was 3.6 ± 0.7 years, and 627 were female (50.7%). The orthoptic examination detected 308 (24.9%) children with subnormal visual acuity for age in one eye or both. In children with a history of prematurity, the orthoptic examination was more frequently abnormal (P = 0.002), which was not seen with the SVS (P = 0.050). The SVS screened 20 (1.6%) children with strabismus, while 40 (3.2%) were detected by orthoptic examination. At the end of the screening, the SVS detected 182 (14.7%) suspect patients while 311 (25.1%) suspect patients were detected after the orthoptic examination. Comparing SVS with orthoptic examination, agreement was fair (κ = 0.4). Conclusion The SVS can be a useful device for visual screening, but agreement with the orthoptic examination was only fair. The Spot vision screener should be used in conjunction with a clinical orthoptic examination.

31. Bilateral LIM2-associated cataract with preoperative nystagmus. A case report

Einleitung: Eine junge Patientin wurde im Alter von drei Jahren vom niedergelassenen Augenfacharzt mit Verdacht auf eine bilaterale kongenitale Katarakt an das Kepler Universitätsklinikum geschickt. Dort erhielt die Patientin die Diagnose LIM2-assoziierte Katarakt. Des Weiteren bemerkte man auch einen horizontalen Nystagmus und einen schlechten Visus. Methoden: Bei jedem Besuch in der Sehschule wurde sowohl ein orthoptischer als auch ein ophthalmologischer Befund erhoben. In Fällen wie diesen ist die Zusammenarbeit der Orthoptist*innen und Ophthalmolog*innen für die Diagnosestellung von großer Bedeutung. Bei der Patientin wurde eine Staroperation durchgeführt, die von zahlreichen ausführlichen Untersuchungen gefolgt war. Ergebnisse: Die orthoptische und vor allem ophthalmologische Untersuchung bestätigten die vom Augenfacharzt gestellte Diagnose der bilateralen kongenitalen Katarakt. Daher wurde eine Staroperation eingeleitet. Der Nystagmus, der präoperativ bestand, verschwand nach der Operation. Des Weiteren kam es postoperativ zu einer Verbesserung der Sehstärke. Diskussion: Dieses Kapitel behandelt das Krankheitsbild der bilateralen kongenitalen Katarakt genauer. Des Weiteren werden verschiedene Artikel und Studien miteinander und mit dem Patientenfall verglichen. Im Fall der Patientin stellt sich vor allem die Frage, weshalb der präoperative Nystagmus nach der Operation nicht mehr vorhanden war. Zusammenfassung: In diesem Kapitel werden die Hauptaussagen dieser Arbeit zusammengefasst. Introduction: A young female patient at the age of three years was sent to the Kepler Universitätsklinikum after her ophthalmologist suspected her to have bilateral congenital cataract. There, the patient was diagnosed with LIM2-associated cataract. Furthermore, the patient had a horizontal nystagmus and a bad visual acuity. Methods: At every check-up, an orthoptic examination, as well as an ophthalmologic one, was performed. In cases like these, a collaboration of the ophthalmologists and orthoptists is crucial for the diagnosis. The patient underwent cataract surgery, which was followed by many extensive examinations. Results: The orthoptic and especially ophthalmologic examination confirmed the diagnosis bilateral congenital cataract. Therefore, cataract surgery was initiated. Postoperatively, the nystagmus which was existent before the surgery was performed, disappeared. Furthermore, the visual acuity improved postoperatively. Discussion: In this chapter, the clinical picture of the bilateral congenital cataract is explained in detail. Furthermore, different works of literature are compared with each other and the patient case. In her case the question arises as to why the preoperative nystagmus disappeared after the surgery. Summary: This chapter summarizes the main points of this case report.

32. Esotropia with eccentric fixation, anisometropia and severe amblyopia A case report

Problemstellung In dieser Arbeit wird der Fall einer jungen Patientin mit Strabismus convergens, Anisometropie, exzentrischer Fixation und hochgradiger Amblyopie links behandelt. Eine Mischform aus Supressions- und Deprivationsamblyopie, in Kombination mit einer exzentrischen Fixation, war bei der Patientin für einen Visus von nur 0,003 verantwortlich. Methoden Die Diagnose wurde aufgrund der Anamnese, der Prüfung von Sehschärfe und Augenstellung, sowie einer Schielwinkelmessung, Fixationsprüfung und Refraktionsbestimmung gestellt. Als initiale Therapie wurde eine faziale Okklusion des rechten Auges angeordnet. Ergebnisse Wegen fehlender Compliance der Patientin wurde keine ausreichende Visusverbesserung erreicht. Daraufhin wurde eine Kontaktlinse mit +25 Dioptrien für das rechte Auge verordnet. Die beabsichtigte Überkorrektur hatte eine Vernebelung des Sehvermögens des rechten Auges und eine Förderung des sehschwachen linken Auges zum Zweck. Im Zeitraum von 15 Monaten konnte durch diese Behandlung links ein Visus von 0,8 in der Nähe und 0,63 in der Ferne erzielt werden. Diskussion Dieser Patientenfall zeigt, dass eine Überkorrektur des besseren Auges mit einer Kontaktlinse eine erfolgversprechende alternative Methode zur fazialen Okklusion bietet. Insbesondere, wenn, wie bei diesem Fall, eine mangelnde Compliance für die faziale Okklusion einen Therapieerfolg zur Verbesserung des Sehvermögens verhindert. Abstract Objective In this paper the case of a young female patient with strabismus convergens, anisometropia, eccentric fixation and severe amblyopia in the left eye is discussed. A combination of a suppression and a deprivation amblyopia together with an eccentric fixation had resulted in a visual acuity of 0.003 in the patient's left eye. Methods The diagnosis was performed on the basis of the patient's medical history, the testing of the visual acuity and the eye position, as well as of the measurements of the squint angle, the testing of the fixation, and the determination of the refraction. Facial occlusion, also called "patching", of the right eye was ordered as an initial therapy. Results However, due to the patient's lack of compliance, a sufficient improvement of the visual acuity with an occlusion therapy could not be achieved. Consequently, a contact lens with +25 dioptres was fitted on the right eye. The purpose of the intended overcorrection was the penalization of the right eye in order to benefit the amblyopic left eye. Over a period of one and a half years, the treatment resulted in an improvement of the visual acuity of the left eye to 0.63. Discussion This patient case showed that an overcorrection of the better eye with a contact lens proved to represent a practicable alternative method to an occlusion therapy in the presence of a severe amblyopia especially when a lack of compliance for patching prevents an improvement of the visual acuity.

Purpose: To compare the macular and papillary parameters on optical coherence tomography (OCT) between the amblyopic eye and the healthy eye in subjects with unilateral strabismic or anisometropic amblyopia., Patients and Methods: This is a cross-sectional and comparative study carried out over two years, from April 1, 2019, to March 31, 2021. We included patients aged over 5years, followed for unilateral amblyopia, free of any neurological and/or ocular pathology. The evaluation of the macular and papillary parameters in the amblyopic and healthy eyes was performed with Swept-Source Optical Coherence Tomography (OCT-SS). The parameters were compared according to age group and type of amblyopia., Results: We collected 50 patients, 29 children, and 21 adults, with a mean age of 19.8years. Amblyopia was secondary to anisometropia in 40 patients and strabismus in 10 patients. Analysis of macular tomographic parameters found no significant difference between amblyopic eyes and healthy eyes for mean macular thickness (P=0.86), central macular thickness (P=0.86), or mean macular volume (P=0.54). The mean retinal peripapillary fiber thickness (RNFL) was 107.48μm in the amblyopic eye and 103.8μm in the healthy eye, with a statistically significant difference (P<0.001). This significant thickening of the RNFL in amblyopic eyes was present in adults (P<0.001), children (P<0.001), anisometropic (P<0.001), and strabismic amblyopia (P=0.024). Analysis of the other optic nerve head parameters revealed no significant difference between amblyopic eyes and healthy eyes in terms of optic disc surface area (P=0.89), neuro-retinal annulus surface area (P=0.47), vertical (P=0.98) or horizontal (P=0.74) cup to disc ratio., Conclusion: Amblyopia is accompanied by thickening of the peripapillary retinal fibers without macular repercussions. Résumé Les malformations palpébrales de l’enfant peuvent être isolées ou associées à des syndromes malformatifs craniofaciaux. Leur évaluation est surtout clinique, les examens complémentaires sont réalisés pour faire le bilan des malformations craniofaciales associées. Un bilan génétique doit être demandé systématiquement en cas de lésions extrapalpébrales associées ou de malformations palpébrales complexes. Tandis que certaines malformations isolées ne nécessitent qu’une simple surveillance, d’autres ont besoin d’un traitement adapté. Celui-ci devra être réalisé à un âge précis. Il prend en compte la croissance de l’enfant, des structures anatomiques et les conséquences de la malformation sur l’œil et la vision (degré d’urgence). Ces anomalies s’accompagnent de deux risques principaux : l’irritation cornéenne par inocclusion palpébrale et l’amblyopie par obstruction de l’axe visuel, par anisométropie ou par strabisme. Summary Palpebral malformations can be isolated or associated with a craniofacial disorder. Their assessment is based on clinical examination; additional investigations are mainly done to characterize craniofacial syndromes. In case of extrapalpebral lesions or complex craniofacial pathology, genetic testing must be performed. Some isolated malformations will only need a simple follow-up; others must have specific treatment, undertaken following a precise timing, taking into account child and anatomical structures’ growth and the possible consequences of the malformation on the eye and child's sight (degree of urgency). When dealing with these malformations, there are two main risks to be taken into consideration: corneal irritation due to lagophtalmos and amblyopia owing to visual axis obstruction, anisometropia or strabismus. Résumé But Nous rapportons à travers un cas d’hémangiome capillaire palpébral et orbitaire géant les résultats de l’usage de bêtabloquants par voie orale dans la prise en charge de cette pathologie. Observation Il s’agit d’une fillette de 2 mois qui présente un hémangiome palpébral et orbitaire volumineux de l’œil gauche à l’origine d’une obstruction de l’axe visuel et d’une amblyopie. Un traitement combiné bêtabloquant par voie orale et corticoïde a permis la régression de l’hémangiome et le dégagement de l’axe visuel. Discussion et conclusions L’hémangiome capillaire est la tumeur vasculaire la plus fréquente chez l’enfant qui évolue généralement vers la régression spontanée. Le traitement n’est pas systématique au cours de cette pathologie. Cependant, la survenue de complications à type d’amblyopie, de strabisme, d’exposition cornéenne, de compression du nerf optique ou des malformations de l’orbite osseuse impose une prise en charge rigoureuse et adaptée. Les bêtabloquants par voie orale ont prouvé leur efficacité dans le traitement de ces formes sévères et étendues avec des effets indésirables rarement notés. Summary Purpose We discuss, through a case of a voluminous cutaneous and orbital capillary hemangioma, results of using oral beta-blockers in the treatment of this pathology. Observation We report the case of a 2-month-old girl who had a large cutaneous and orbital capillary hemangioma of the left eye causing an obstruction of the visual axis and amblyopia. Beta-blocker treatment combined with oral corticosteroid allowed hemangioma regression and liberation of the visual axis. Discussion and conclusions Capillary hemangioma is the most common vascular tumor in children that usually progresses to spontaneous regression; that is why the treatment is not obligatory in this disease. However, when complications such as amblyopia, strabismus, corneal exposure, optic nerve compression or malformations of the bony orbit occur, a rigorous management becomes compulsory. Oral beta-blockers proved their effectiveness in the treatment of these severe and extended forms with rare side effects. [ABSTRACT FROM AUTHOR]

Le ptosis congénital constitue la malposition palpébrale la plus fréquente de l'enfant. Le but de ce travail est de rapporter l'expérience de notre service dans la prise en charge de cette affection. Cette étude analyse les données épidémiologiques, cliniques et thérapeutiques des patients opérés pour ptosis congénital dans notre service entre Janvier 2005 et Décembre 2012. 44 patients (48 yeux) ont été opérés. La médiane d'âge était de 10 ans et une prédominance masculine a été retrouvée. Le ptosis était unilatéral et isolé dans 40 cas (90,90 %), bilatéral et associé à un syndrome de blépharophimosis dans 3 cas et à un syndrome de fibrose congénitale des muscles oculomoteurs dans un cas. Une amblyopie a été notée dans 11 cas (25%). La résection du muscle releveur de la paupière supérieure était la technique chirurgicale la plus utilisée (81,81%). Les résultats postopératoires étaient bons dans 33 cas (75%).

Within the framework of the German pediatric screening examinations, the checks include visual functions. There is no ophthalmological screening examination in childhood in Germany. This study investigated whether participation in the pediatric screening examinations U8 (at the end of the fourth year of life) and U9 (at the beginning of the sixth year of life) is associated with the results of visual acuity, which are compiled at the school entry examinations (SEU).This study evaluated data of the SEU for school the enrollment years 2009/2010-2014/2015 of the State of Rhineland-Palatinate. In these years visual acuity was assessed using the Rodenstock visual testing device (E-hooks; Rodenstock intruments GmbH, Ottobrunn, Deutschland) wearing glasses if present. The association between participation in the U8 and U9 screening examinations and the presence of unilateral and bilateral visual acuity0.7 was investigated using multiple logistic regression adjusted for important disturbance variables.Data from 189,704 children (91,041 girls, 98,663 boys) from 35 out of 36 districts were included. A visual acuity0.7 was measured in 8416 (4.4%) children and in both eyes in 4345 (2.3%) children. The participation rates in the U8 and U9 were 93.9% and 93.3%, respectively. There was a negative association between participation in the U8 and U9 and a unilateral or bilateral SEU visual acuity0.7 (adjusted odds ratio, OR 0.68, 95% confidence interval, CI 0.61-0.75; p 0.01, N = 124,467/adjusted OR 0.57, 95% CI 0.51-0.65; p 0.01, N = 121,496).The proportion of children with visual acuity0.7 at the SEU was high. Children who were examined in the U8 and U9 had a better chance for a good visual acuity in the school entry examination.HINTERGRUND: Im Rahmen der kinderärztlichen Vorsorgeuntersuchungen U1 bis U9 von der Geburt bis zum Beginn des sechsten Lebensjahres wird unter anderem das Sehvermögen getestet – eine augenärztliche Vorsorgeuntersuchung im Kindesalter gibt es in Deutschland nicht. Kurzfassung Einleitung: In dieser Arbeit wird auf den Fall eines fünfjährigen Kindes mit seitenungleicher Hyperopie, einem vollakkommodativen Strabismus convergens rechts und einer Amblyopie rechts, aufgrund von Anisometropie und Astigmatismus, eingegangen. Methoden: Es wurde ein, dem Patientenalter entsprechender, orthoptischer Status inklusive Fixationskontrolle, Skiaskopie in Cycloplegie und Funduskontrolle durchgeführt. Ergebnisse: Anhand der, in der Anamnese, erfassten Informationen und der Ergebnisse der orthoptischen und ophthalmologischen Untersuchungen konnte die Diagnose einer Amblyopie rechts gestellt werden. Die Ursache dafür liegt in der hyperopen Anisometropie und dem anfangs nicht gänzlich ausgeglichenen, vollakkommodativen Strabismus convergens rechts. Diskussion und Zusammenfassung: In diesem Kapitel wird auf das Krankheitsbild und die Pathogenese der Amblyopie im Hinblick auf Anisometropie eingegangen und genau beschrieben. Die vorhandenen Behandlungsmethoden werden diskutiert und miteinander verglichen. Ebenfalls wird die methodische Vorgehensweise bei der Untersuchung kritisch betrachtet. Am Ende werden nochmal die wichtigsten Punkte der Arbeit zusammengefasst und gesammelt dargestellt. Abstract Introduction This bachelor-thesis takes a closer look at the case of a five-year-old child with bilateral hyperopia, accommodative esotropia of the right eye, anisometropia and amblyopia due to higher hyperopia and astigmatism of the right eye. Methods A complete orthoptic status befitting the patient’s age was carried out, including fixation testing, cycloplegic retinoscopy and ophthalmoscopy. Results A right-sided amblyopia was diagnosed, according to the orthoptic and ophthalmologic examination results and the information gathered during anamnesis. The anisometric hyperopia and the accommodative esotropia could be identified as the reason. Discussion and Summary This chapter discusses the pathology and pathogenesis of amblyopia with regards to anisometropia. Possible methods of therapy are presented, discussed and compared and the methodical procedure is critically examined. The closing chapter summarizes the main aspects of this thesis.

The topic of this bachelor thesis are the current options and limits within the area of reeducation of eyesight abilities of pre-school children with binocular vision disorders in ordinary kindergartens and in classes organised based on the § 16 par. 9 School Law for children with a visual impairment or in kindergartens organised based on the § 16 par. 9 School Law for children with a visual impairment. The thesis is divided into two parts - a theoretical one and a practical one. The theoretical part introduces the characteristics of a pre-school child. Further, it deals with the definition of visual functions and options of their cure. It briefly specifies the development of eyesight. In the following chapter, binocular vision disorders are defined, such as strabismus and amblyopia. A considerable part of the bachelor thesis is devoted to the consequences of binocular vision disorders and to the therapeutical methods of correcting these disorders. The options of pre-school education for children with binocular vision disroders are described. Further on, the theoretical part deals with the demarcation of the form of special pedagogical care for children with binocular vision disorders and the conditions of visual reeducation in these children in kindergarten. Attention is also focused on the description of a special pedagogy centre for people with visual impairment. The objective of the practical part of the bachelor thesis was to compare the current options and limits within the area of reeducation of visual abilities in pre-school children with binocular vision disorders in usual kindergartens and in classes organised based on the §16 par. 9 School Law for children with a visual impairment or in kindergartens organised based on the §16 par. 9 School Law for children with a visual impairment. To fulfill the objective, qualitative research was chosen. To collect data, the method of three questionnaires was chosen, where the first questionnaire was directed at kindergarten teachers, the second questionnaire was directed at kindergarten directors, head teachers, and kindergarten director representatives, and the third questionnaire was directed at ortoptists.
