Internal and marginal fit accuracy of an implant-supported fixed partial denture (I-FPD) along with passive fit are required for long-term clinical success. This study compared the fit accuracy of three-unit CAD/CAM-fabricated cementretained I-FPDs milled from pre-sintered cobalt-chromium (Co-Cr) and zirconia (Zr) and evaluated the effect of applying porcelain.Two implant analogs were positioned parallel at first premolar and first molar sites in an acrylic model. Their corresponding abutments were tightened with 25-Ncm torque force. Four groups were designed: two groups of I-FPD frameworks with pre-sintered Zr (ZrF) and soft millable Co-Cr alloy (SF) blocks, and two groups by applying porcelain to the Co-Cr (SP) and Zr (ZrP) frameworks (n = 4 in each group). Absolute marginal gap (AMG), marginal gap (MG), and internal discrepancies were measured. Kolmogorov-Smirnov, Levene, one-way analysis of variance (ANOVA), and Scheffe tests were used for statistical analysis using SPSS software version 22 at a significance level of .05.All measured criteria were significantly different from each other (all P values.001). The rankings from the lowest gap to the highest gap for the main criteria were as follows: MG: SFSPZrFZrP; AMG and internal discrepancies: SFSPZrPZrF. Comparing molars and premolars, molars had smaller MGs, larger internal discrepancies, and smaller AMG in ZrP but larger AMG in SP.All three-unit CAD/CAM-fabricated cement-retained I-FPDs were in the clinically acceptable range of fit accuracy. The lowest and highest misfit values were observed in the SF and ZrF groups, respectively.

Several studies have compared digital intraoral scanners and conventional impressions. The accuracy of these two methods in terms of marginal accuracy of lithium disilicate crowns is not well-established, yet. The purpose of this study was to systematically review available publications on marginal fit of single-unit, full-coverage, tooth-supported lithium disilicate restorations.Pubmed, Web of Science, Cochrane, EMBASE, and Scopus were electronically searched along with a manual search. After critical appraisal, data from selected studies were extracted and mean marginal difference with a 95% confidence interval was calculated. Meta-analysis of the collected data was conducted using STATA software.The meta-analysis revealed similar marginal gap values in intraoral scanners with conventional groups (p0.05) and in intraoral scanners with extraoral canners (p0.05).No significant difference was seen between digital and conventional impressions or intra- and extraoral scanners for marginal accuracy of lithium disilicate crowns.

Statement of problem: Intraoral scanners have been increasingly used in recent years. However, the accuracy of digital scans as it affects marginal adaptation is unclear. Purpose: The purpose of this systematic review and meta-analysis was to compare the marginal adaptation of single-unit zirconia crowns fabricated with digital scans or with conventional impressions. Material and methods: The electronic databases PubMed (MEDLINE), Cochrane Central Register of Controlled Trials (CENTRAL), Web of Science (ISI), Scopus, and EMBASE were searched and complemented by a manual search. Risks of bias were assessed by using a modified methodological index for nonrandomized studies (MINORS). Mean ±standard deviation (SD) values of marginal accuracy of studies were extracted for both methods. Mean marginal difference and 95% confidence interval (CI) were calculated to evaluate the marginal accuracy of each method. Pooled data were statistically analyzed by using a random-effect model. Results: Seventeen studies were used to perform the meta-analysis. Subgroup analysis was performed based on intraoral scanners. Statement of problem Denture stomatitis (DS) is an inflammatory oral lesion that occurs mostly in elderly denture wearers and that can negatively affect their quality of life. The conventional treatment plan for DS is topical or systemic fungicidal drug therapy, which are not without side effects. Purpose The purpose of this systematic review was to evaluate the clinical outcomes of using low-level laser therapy (LLLT) or photodynamic therapy (PDT) in the treatment of DS by reviewing high-quality published articles. Material and methods The Cochrane Library, Web of Science, Google Scholar, PubMed, ProQuest, Scopus, Embase, and Ovid databases were searched using appropriate key words. Related titles and abstracts published up to May 2017 were screened and selected based on defined inclusion criteria. The full texts of all selected articles were read, and citation lists were checked for missing references. All randomized controlled trials (RCT) were subjected to quality assessments. Results A total of 760 articles were included in the study after the initial search. Six studies were selected for assessment of eligibility, and 4 of them were sufficiently reliable in methodology and randomization to be included in the study. The applied wavelength varied from 455 nm to 830 nm. Also, the highest energy patients were exposed to was 122 J/cm2, and the lowest was 1 J/cm2. Exposure times ranged from 10 seconds to 26 minutes in the reviewed studies. Conclusions Based on the available RCTs, LLLT has a significant role in the clinical treatment of DS. However, the results of PDT were similar to those of conventional antifungal therapies.

Oral and maxillofacial surgeries might induce anxiety and pain to the patients. Sedative agents are one of the best ways for eliminating such consequences. Dexmedetomidine (DEX) is a recent sedative agent which presents higher sedative quality with greater specificity than other drugs. The aim of present paper is to evaluate the risks and benefits of administrating DEX during oral and maxillofacial surgeries by reviewing high quality released articles. Areas covered: Searches on PubMed, Scopus and Web of Science databases were completed with focus on randomized controlled trials (RCT). Related articles, from 2000 to 2015, were selected based on inclusion criteria and quality assessments factors. Full texts of the selected articles were screened and their significant information were gathered for judgments. Expert opinion: 17 RCTs on a total of 765 patients were screened. Some of the difficulties during reviewing the articles were: different pharmacokinetic and pharmacodynamics of drugs when combined with DEX, different time spots and method of monitoring, including studies on both minor and major surgeries for better data collection. Recent researches are going to focus on application of DEX for in-office procedures because of its desirable properties. Nevertheless, the analgesic and amnesic features of DEX are still questionable.

Resveratrol (RSV) is a phytoestrogen with a wide range of therapeutic effects, including antioxidant and anti-inflammatory effects. This study was performed to evaluate the protective role of RSV against osteonecrosis after tooth extraction in rats treated with bisphosphonates. Thirty-nine male Wistar rats, weighing 300–350 g, were divided into three groups (n = 13) according to the planned drug regimen: alendronate + dexamethasone (AL/DEX), alendronate + dexamethasone + RSV (AL/DEX/RSV), and no drugs (control group). The first and second molars of each rat were extracted and the extraction sites were analysed histologically 14 days later. The data collected were subjected to statistical analysis by means of Kruskal–Wallis, Mann–Whitney, χ2, and Fisher’s exact tests using IBM SPSS software at a significance level of 0.05. The incidence of osteonecrosis was significantly lower in the AL/DEX/RSV group (P = 0.001) and control group (P = 0.041) than in the AL/DEX group, while the amount of new bone formation did not differ significantly between the AL/DEX/RSV and AL/DEX groups (P > 0.05). RSV showed a protective effect by significantly reducing osteonecrosis, which could be due to its antioxidant effects.

Nowadays, lasers are widely used in many fields of medicine. Also, they can be applied at many branches of dental practice such as diagnosis, preventive procedures, restorative treatments, and endodontic therapies. Procedures like caries removal, re-mineralization, and vital pulp therapy are the most noticeable effects of laser irradiation which has gained much attention among clinicians. With controlled and appropriate wavelength, they can help stimulating dentinogenesis, controlling pulpal hemorrhage, sterilization, healing of collagenic proteins, formation of a fibrous matrix, and inducing hard tissue barrier. Nevertheless, there are many controversies in literatures regarding their effects on the quality of bonded restorations. It hampered a wide application of lasers in some aspects of restorative dentistry and requirements to identify the best way to use this technology. The aim of this mini review is to explain special characteristics of laser therapy and to introduce the possible applications of laser devices for dental purposes.

Context: Postoperative pain control has been a common challenge to clinicians in endodontics. Aims: This double-blind randomized clinical trial assessed the efficacy of clonidine added to lidocaine for postoperative pain following endodontic treatment of mandibular molars with symptomatic irreversible pulpitis (SIP). Methods: One hundred participants with lower molars experiencing SIP were recruited and randomly assigned to two groups. 1.8 mL of 2% lidocaine with either epinephrine (1:80,000) or clonidine (15 µg/mL) was administered to each group via an inferior alveolar nerve block. A Heft–Parker Visual Analog Scale was used to rate preoperative pain and at 6, 12, 24, 36, 48, and 72 h following endodontic treatment. Their postoperative analgesic consumption was recorded. Statistical Analysis Used: The analgesic efficacy was analyzed by Chi-square test, paired t-test, and repeated measures ANOVA (P < 0.05). Results: Early postoperative pain was significantly lower in the lidocaine/clonidine group than the lidocaine/epinephrine group (6 h: P = 0.038; 12 h: P = 0.031). The lidocaine/clonidine group consumed a significantly lower amount of analgesics (P = 0.048). Conclusions: The administration of clonidine added to lidocaine may reduce early postoperative pain and consumption of analgesics following endodontic treatment in lower molars with SIP.

BACKGROUNDS: Bleaching procedure can be companied before, during, or after orthodontic treatments. However, the risk of compromised bond strength of brackets to bleached enamels is in debate. This study tried to evaluate the shear bond strength (SBS) of bonded metal brackets to the previously bleached enamels. MATERIALS AND METHODS: In this in vitro study, 60 extracted, sound, human premolars were mounted vertically in cylindrical molds. The samples were randomly divided into four groups (n = 15): Control (C); at-home bleached by 20% carbamide peroxide (HB); in-office bleached by 45% carbamide peroxide (OB); and in-office bleached by 40% hydrogen peroxide activated with diode laser (L-OB). Sixty stainless steel brackets were bonded by no-primer adhesive resin (OrthoCem). Then SBS of bonded brackets was measured after 5000 thermal cycles at 5°C and 55°C. Finally, the collected data were analyzed by one-way ANOVA, and Tukey HSD tests by using SPPS software at a significant level of 0.05 (α = 0.05). RESULTS: Group C showed significantly higher SBS values (all P 0.05). CONCLUSION: The SBS of bonded orthodontic brackets were compromised after bleaching with 20% and 40% of carbamide peroxide. Diode laser activation may not eliminate the negative effect of bleaching agents on SBS of bonded orthodontic brackets, neither.

Objective White spot lesion (WSL) is recognized as the first clinical sign of enamel caries; it is a very critical phase because it can be prevented from progression to frank caries by changing the surrounding destructive environment. The present study was undertaken to systematically review the effect of resin infiltration (RI) technique on surface hardness (SH) of WSL. Methods Five electronic databases were searched with proper key words. Related titles and abstracts, up to October 2018, were screened, selected, and subjected to quality assessments. After collecting data, meta-analyses were carried out to compare the effect of RI with untreated WSL and sound enamel by using the STATA software. Results A total of 4567 articles were included in the study after initial search. Finally, 10 studies were reliable enough in methodology to be included in the study. Metadata analyses, carried out on 7 studies that compared SH of RI group with untreated samples, showed a significant increase in SH with 3.66 mean difference (95% confidence interval = 2.56‒4.77, Q value = 36.07, I2 = 83.4%). However, meta-analysis on 4 studies that compared SH of RI with sound enamel showed a significant decrease in SH with −2.35 overall mean difference (95% confidence interval = −3.91–0.98, P = .00, Q value = 31.75, I2 = 90.6%). Conclusion The RI technique can enhance SH of WSL; however, regaining the SH of RI-treated WSLs similar to sound enamel is doubtful. Application of RI is more effective than other methods, including application of fluoride, enamel pro-varnish, adhesive, and colloidal silica infiltration for enhancing SH of WSLs.

Background The bonding of fiber posts (FPs) to composite resin core buildups is a challenge due to limited penetration of resin to the polymeric matrix of FPs. This review article tries to answer this question: “What are the effects of laser surface treatment of FPs, compared to other surface roughening methods, on push-out bond strength (PBS) of FPs bonded to composite resin core buildups?” Methods Searches were run in seven electronic databases with a focus on proper key words. Related titles and abstracts, up to February 2019, were screened, selected, read and subjected to quality assessments. Results After the initial search, a total of 2635 articles were included in the study. Finally, 6 studies were reliable enough in methodology to be included. All the studies were in vitro with a total of 359 samples. Er:YAG (-0.05, 95% CI: -2.96 to 2.86; P = 0.97) and Er,Cr:YSGG (0.84, 95% CI: -0.12 to 1.81; P = 0.08) treated samples showed no significant overall mean differences in final PBS compared to the control groups. Moreover, pretreatment with Er,Cr:YSGG laser and sandblasting with 50 μm alumina showed an overall mean difference of -0.42 for PBS (95% CI: -1.23 to 0.39) with no significant differences. Conclusions Laser irradiation of FPs seems to provide no significant increase in PBS values of FPs bonded to composite resin core buildups. Effects of surface treatment of FPs with laser irradiation and sandblasting with 50 μm alumina might be similar in increasing the final PBS, either.

Dental anxiety and fear of needle injection is one of the most common problems encountered by dental practitioners, especially in the pediatric patient. In consequences, it might affect the patient's quality of life. Several methods are suggested to lower the discomfort of local anesthesia injection during dental procedures. Desensitization of injection site is one of the recommended strategies. Among chemical anesthetic topical agents that are effective but might have allergic side effects, using some nonpharmacological and safe techniques might be useful. This study aimed to overview the efficacy of using cooling techniques, mostly by ice or popsicles, warming or pH buffering of drug, and using modern devices to diminish the discomfort of local anesthesia injection during dental procedures.

Dental pain management is one of the most critical aspects of modern dentistry. Irreversible pulpitis and further root canal therapy might cause an untolerated pain to the patients. The improvements in anesthetic agents and techniques were one of the advantages of studying nerve biology and stimulation. This article tried to overview of the nerve activities in inflammatory environments or induced pain. Furthermore, the proper advises, and supplementary techniques were reviewed for better pain management of irreversible pulpitis.

Background: The entire effects of different bleaching regimens on the mechanical properties of composite resins have remained unknown. The purpose of this study was to evaluate the effects of different bleaching regimens on the flexural strength (FS) of hybrid composite resins. Materials and Methods: In this in vitro study, 80 bar-shaped specimens of hybrid composite resins were fabricated and randomly divided into four groups, 20 specimens in each group. First group (C) was considered as control. The other groups were treated by home bleaching (HB) agent, in-office bleaching (IB) agent, and the combination regimens (HIB), respectively. The FS was evaluated by three-point bending test by using a Universal Testing Machine. All data were analyzed by using Statistical Package for the Social Sciences (SPSS) software version 18, analysis of variance (ANOVA), and Turkey's post hoc statistical tests (α = 0.05). Results: The maximum mean value of FS was seen in HB group with significant differences to other groups (P < 0.05). Also, the minimum FS was observed in group HIB. Conclusion: Application of different bleaching regimens does not have any adverse effect on the FS of hybrid composite resins. However, the administration of HB regimens seemed to have lesser negative impact on the FS.

Background Temporomandibular disorders (TMDs) are musculoskeletal conditions that can inhibit the normal function of temporomandibular joints (TMJs) and affect the patient’s quality of life, negatively. Arthrocentesis (AC) is a minimally invasive surgical procedure used for treating TMDs. The aim of present paper is to evaluate the advantages of administrating corticosteroid (CS) during AC by reviewing high quality released articles. Material and Methods Searching on Cochrane Library, Web of Science, Google Scholar, PubMed, ProQuest, and Scopus databases were performed with focusing on proper key words. Related titles and abstracts, up to December 2017, were screened and selected based on inclusion criteria. The full text of all randomized controlled trials (RCTs) was extensively read and subjected to quality assessments. Results After initial search, a total of 2067 articles were included into the study. Finally, 7 studies were reliable enough in methodology and randomization to be included into the study. All of the observed studies showed improvements in jaw functions and pain relief with no statistical differences in both AC and control groups. One study reported painless maximum incisal opening in CS group than the control group. Conclusions Based on available RCTs, the AC of TMJ with CS seems to result in similar findings to other therapeutic drugs, with no significant differences. Introduction Three-dimensional Scaffold structure of synthetic biomaterials with their interconnected spaces seem to be a safe and effective option in supporting bone regeneration. The aim of this animal study was to compare the effectiveness of three different biocompatible scaffolds: bioglass (BG), demineralized bone matrix (DBM) and forstrite (FR). Materials and methods Four healthy dogs were anesthetized and the first to fourth premolars were extracted atraumatically in each quadrant. After healing, linear incision was prepared from molar to anterior segment and 4 defects in each quadrant (16 defects in each dog) were prepared. Scaffold blocks of BG, DBM and FR were resized according to size of defects and placed in the 12 defects randomly, 4 defects remained as control group. The dogs were sacrificed in 4 time intervals (15, 30, 45 and 60 days after) and the percentage of different types of regenerated bones (lamellar and woven) and connective tissue were recorded in histological process. The data were analyzed by one-way ANOVA and post hoc using SPSS software Ver. 15 at significant level of 0.05. Results In day 30th, although the amount of regenerated lamellar bone in control, DBM and BG Scaffold (22.37 ± 3.44; 21.46 ± 1.96; 21.21 ± 0.96) were near to each, the FR Scaffold provided the highest amount of lamellar (29.71 ± 7.94) and woven bone (18.28 ± 2.35). Also, FR Scaffold showed significant difference with BG (p = 0.026) and DBM Scaffolds (p = 0.032) in regenerated lamellar bone. Conclusion We recommend paying more attention to FR Scaffold as a biomaterial, but it is better to be compared with other nano biomaterials in future studies. Introduction: Local anesthesia has a paramount role in any branches of medical sciences specially dentistry. Soft tissue irritations and lips biting are adverse side-effects in some cases. This study tried out to represent a new pharmaceutical solution in returning sensations faster. Materials and Methods: Five New Zealand laboratory rabbits were anesthetized with 1 cc Ketamine and Xilosine intramuscularly in thigh site. Electrodes were attached to the palms and wrists. Then, 0.2 cc lidocain 2% was injected in forearm origin in order to block median proximal nerve in both forelegs. The nerves conduction study (NCS) was recorded before and after lidocain injection, plus 30 min and once again 1 h after calcium gloconate 10%, dextrose 50%, citric acid solution injection. The recorded data were analyzed using Wilcoxon test and using SPSS software (version 11.5) at significant level of 0.05. Results: The NCS of left and right forearms was 1.4 ± 1.35 mV and 0.48 ± 0.45 mV. According to the statistical test, there was no significant difference between two groups (P > 0.05). Conclusion: Calcium gloconate 10%, dextrose 50%, citric acid solution did not reverse the effect of anesthetic drugs. Possible reasons were pressure to the nerves, which were caused by drug volume, and low sample size.

Background: Using local anesthetic is common to control the pain through blocking the nerve reversibly in dental procedures. Gow-Gates (GG) technique has a high success rate but less common. This study aimed to compare the onset time and success rate in GG and standard technique of inferior alveolar nerve block (IANB). Material and Methods: This descriptive, single blind study was consisted of 136 patients (59 males and 77 females) who were randomly received GG or IANB for extraction of mandibular molar teeth. Comparisons between the successes of two anesthetic injection techniques were analyzed with Chi-square test. Incidence of pulpal anesthesia and soft tissue anesthesia were analyzed with Kaplan-Meier method. Mean onset times of pulpal anesthesia, soft tissue and lip numbness were analyzed with Log-Rank test. Comparisons were considered significant at P ≤0.05 by using SPSS software ver.15. Results: The incidence of pulpal anesthesia in the IANB group (canine 49.3%, premolar 60.3%) were not significantly different from the GG group (canine 41.3%, premolar 74.6%) ( P =0.200 and P =0.723). The success rate in the IANB group (80.82%) was not significantly different from the GG group (92.02%) ( P =0.123). Furthermore, onset time of lip and buccal soft tissue numbness in GG group (3.25, 4.96 minutes) was quite similar to IANB group (3.22, 4.89 minutes) (all P values >0.05). Conclusions: Although this study demonstrated higher clinical success rate for GG than IANB technique, no significant differences in success rates and onset time were observed between two techniques.

Introduction : Post and core are needed to regain retentions and functions after endodontic therapies. Also, risk of cross contamination from dental prosthesis is so high. The aim of this study was to compare dimensional changes of acrylic resin patterns (ARP) in three different storing environments. Materials and methods : conventional root canal therapy was done on one first premolar tooth and the canal filled with Guttapercha. 2/3 of the filling was expelled and 30 direct APRs were prepared by Duralay. The samples were divided into 3 groups based on storing environments: water, NaOCl 5% and air. Finally dimensional changes in coronoapical length (CAL), coronal (CD) and apical diameter (AD) of APRs were measured in 7 consecutive times (immediately after polymerization, 1, 2, 4, 8, 24, 48 hours later). All the data were analyzed by Paired T-test and Duncon test using SPSS software ver.13 at significant level of 0.05. Results : After 24 hours, the ARPs, which were stored in air, contracted 0.07, 0.06 and 0.12 mm in AD, CD and CAL; the ARPs, which were stored in water, showed 0.03, 0.06 and 0.12 mm decrease in AD, CD and CAL; But the ARPs, which were stored in NaOCl 5%, showed significant expansion in AD, CD and CAL (0.03, 0.06 and 0.10 mm) (all P values < 0.01). Conclusion : It is better not to use NaOCl for disinfecting; also the best time for storing APRs is 8 hours for water and 2 hours for air environments after setting time.

To evaluate the antiseptic properties of five different disinfectant techniques on three different peri-implantitis (PI) associated biofilms.90 implant titanium disks, with the same thickness and diameter, were prepared and randomly divided into 18 groups (n = 5) based on the microbiota strains (S. aureus, S. epidermidis and C. albicans) and using the following disinfectant techniques:soft laser therapy, photodynamic therapy (PDT), 0.12% NaOCl, 0.2% chlorhexidine, 3% H2O2, and control groups. After forming a protein layer on disk surfaces, the specimens were exposed to the microbial suspensions. After decontamination according to designated techniques, 2% Trypsin protease was administered to isolate the surviving microorganisms. Muller Hinton agar culture was used for microbiota growth. After 48-hour incubation, the standard colony forming unit (CFU) was assayed and the collected data were analyzed by Kruskal-Wallis and Mann-Whitney tests at a significance level of 0.05.The highest amount of CFU/ml values was shown by C. albicans, which was subjected to PDT (25.12 ± 30.23). The least disinfecting efficacy on S. epidermidis was demonstrated by the laser group (all P-values ≤ 0.01). Nevertheless, all of the groups exhibited significant differences with the control groups (all P-values0.01).None of the studied disinfectant techniques had the highest lethal effects on all of the tested microbiotas. Therefore, a combination of these disinfectant techniques, for instance PDT ⁺ 3% H2O2 or 0.2% chlorhexidine, is recommended.

Rapid maxillary expansion (RME) is one of the common treatments of transverse maxillary deficiency, and low-level laser therapy (LLLT) is one of the recommended solutions to enhance biological wound or bone healing. This review article aims to answer the following question: “What are the effects of LLLT, on patients who underwent surgical or non-surgical RME, in improving clinical success, wound healing, and bone regeneration?” A search in PubMed, Scopus, Web of Science, and ProQuest databases was performed, with a focus on the appropriate key words. Related articles, up to May 2017, were screened, and the full text of the randomized controlled trials (RCT) were comprehensively read and subjected to quality assessments. A total of 1804 articles were included after the initial search. Four RCTs were eligible in randomization and methodology. The applied wavelength varied from 660 to 830 nm with an output range of 40–100 mW. Also, the highest exposed energy was 420 J/cm2 and the lowest was 100 J/cm2. The exposure time differed from 20 to 84 s in each defined point in the palate. Based on the RCTs available, LLLT is better to be used at initial phase of RME, because it has some benefits in increasing the rate of bone remodeling.

Statement of problem Cement-retained implant-supported prostheses eliminate screw loosening and enhance esthetics. However, retrievability and the possibility of removing extruded excess cement (EEC) have been problematic. Purpose The purpose of this systematic review was to analyze the effects of modifying the screw access channel (SAC) on the amount of EEC and the retention of cement-retained implant-supported prostheses. Material and methods PubMed, Web of Science, Scopus, and Google Scholar databases were searched with appropriate key words. Related titles and abstracts published up to June 2017 were screened and selected on the basis of defined inclusion criteria. Full texts of all studies were read and subjected to quality assessments. After the initial search, 1521 articles were included in the study. Of these, 11 studies were subjected to critical appraisal, and 10 of them were reliable enough in methodology to be systemically reviewed. Results All the studies were in vitro and described a total of 260 specimens. According to the interpreted results, closed SACs caused lower retention with a higher amount of EEC, whereas open SACs caused the reverse. Also, as the abutment height decreased, retention decreased. Conclusions Extending the crown’s margin into the SAC, leaving the SAC open, and using internal vents in the SAC space are possible methods of modifying the SAC to gain higher retentive values. Also, the use of internal vents in the SAC system and open or partially filled SAC space reduce the amount of EEC.

Aims: Application of force to implants is helpful, especially in orthodontic-implant therapies. The aim of this study was a histomorphologic and histomorphometric evaluation of peri-implant bone after immediate orthodontic and orthopedic forces comparing them with a control group. Materials and Methods: Eighteen dental titanium implants were inserted in the premolar region of three dogs. Implants were divided into three groups: (1) group with immediate orthodontic force of 300 NC, (2) group with immediate orthopedic force of 600 NC, and (3) control group. Implants were explanted with adequate amount of surrounding bone after 3 months and bone-implant contact (BIC), amount of lamellar bone (LB), amount of woven bone (WB), amount of inflammatory connective tissue, and the rate of the movement were investigated. ANOVA, t-test, paired t–test, and Pearson's test were used to analyze the data using SPSS software version 16 at a significant level of 0.05. Results: Based on BIC, amount of lamellar and WB and the amount of inflammatory connective tissue, there was no significant difference between the three groups (300 centinewton (CN), 600–CN, and control group) (P > 0.05). In the 300-CN and 600-CN groups, the rate of movement was reported 0.41 and 0.94 with no significant differences (P = 0.38). Conclusion: 300-CN and 600-CN immediate static loads do not interfere with osseointegration phenomenon and it does not decrease the amount of BIC and LB. Implants can be moved by preserving osseointegration, and this movement is in direct relation with the amount of applied force.

Introduction: Pulp vitality and its continuous dentin prodution are essential for long-term success of direct pulp capping (DPC). The aim of present study was to evaluate the histopathological response of the canine pulp following DPC using either different dentin adhesive resins (DAR), calcium hydroxide (CH) or mineral trioxide aggregate (MTA). Methods and Materials: DPC was done on 72 dog’s teeth using 6 types of dental materials (n=12) (4 types of DAR, white MTA and CH). Therefore, six healthy dogs were anesthetized and 2 teeth from each dog were allocated to either type of mentioned DPC agents. The dental pulps were exposed mechanically by drilling in the center of class V cavities. The different types of capping materials included DARS (Clearfil S3 Bond, Optibond FL, Single Bond and Clearfil SE Bond), white MTA and CH. After 7, 21 and 63 days, two dogs were euthanized in each interval. Microscopic evaluations were done according to following criteria: intensity of inflammation, presence of necrosis and formation of hard tissue. The recorded data were analyzed by the Kruskal-Wallis, Friedman, Cochran’s and Fisher’s exact tests using SPSS software version 12 at significant level of 0.05. Results: No significant differences were found regarding necrosis among DPC materials (P>0.05). However, MTA caused higher amount of hard tissue formation after 63 days in comparison with 21 days. Conclusion: MTA provided the highest degree of hard tissue formation after 63 days. However, further studies should be performed for administering a definitive material.

Introduction:Hardness is one of the basic properties of dental materials, specially composite resins which is relevant to their polymerization. The aim of this study was to evaluate the effect of light curing distance and the color of clear Mylar strips on surface hardness of Silorane-based (SCR) and Methacrylate-based composite resins (MCR).Materials and methods:40 samples of MCRs (Filtek Z250) and SCRs (Filtek P90) were prepared in size of 5 mm×2 mm (80 samples in total). The samples divided into 8 groups (10 samples in each one) based on the color of clear Mylar strips (white or blue) and distance from light curing source (0 mm or 2 mm). All the samples cured for 40 second and stored in incubator for 24 hours in 37°C temperature. Surface hardness test was done by Vickers test machine and the collected data were analyzed by one-way ANOVA and paired T-test by using SPSS software version 13 at significant level of 0.05.Results:MCRs cured with blue Mylar strips from 0 mm distance had the highest (114.5 kg/mm2) and SCRs cured with white Mylar strips from 2 mm distance had the lowest (42.2 kg/mm2) mean of surface hardness. Also, the results of comparison among SCRs and MCRs showed significant differences among all groups (all P values Conclusion:The hardness decreased as the distance increased and the blue Mylar strips provided higher hardness than clear ones. Introduction The aim of this study was to evaluate the effects of eugenol-free temporary cement's remnants on the retentive strength of full metal crowns luted via zinc phosphate and resin cement (Maxcem) to the tooth structure. Materials and methods Forty complete standardized Ni-Cr crowns in four groups were cemented by two types of permanent cements: zinc phosphate cement and resin cement (Maxcem). In the two groups before permanent cementation of crowns, temporary acrylic crowns were cemented by eugenol-free temporary cement. Crowns¡¦ retention was evaluated by Universal testing machine. All data were analyzed by means of one-way ANOVA test in SPSS software version 11.5 (α = 0.05). Results There was no significant difference in groups with prior using eugenol-free temporary cement and groups with just using two permanents cement (p-value ≥ 0.05). Discussion The application of temporary cement before permanent cementation of full metal crowns does not have any adverse effect on retention of full metal crowns, when temporary cements are removed properly. Dental pain management is one of the most critical aspects of modern dentistry which might affect patient's quality of life. Several methods are suggested to provide a painless situation for patients. Desensitization of the oral site using topical anesthetics is one of those methods. The improvements of topical anesthetic agents are probably one of the most important advances in dental science in the past 100 years. Most of them are safe and can be applied on oral mucosa with minimal irritation and allergic reactions. At present, these agents are various with different potent and indications. Eutectic mixture of local anesthetics (EMLA) (lidocaine + prilocaine) is a commercial anesthetic agent which has got acceptance among dental clinicians. This article provides a brief review about the efficacy of EMLA as a topical anesthetic agent when used during dental procedures.

Background: Dentistry equipment are exposed to different types of pathogenic microorganisms. The aim of this study was to investigate the effect of spraying three different types of disinfectants on condensational silicones after 5 and 10 min. Materials and Methods: Totally, 66 circular samples of condensational silicone impression materials of 1 cm diameter and 2 mm thickness were contaminated by Staphylococcus aureus, Pseudomonas aeruginosa, and Candida albicans fungus. Except for control samples, all of them were disinfected with sodium hypochlorite (NaOCl) 0.525%, Deconex and Epimax by spraying method. Afterward, they kept in plastic bags with humid rolled cotton for 5 and 10 min. In order to isolate microbiotas, the samples were immersed in 2% trypsin for 1 h and diluted with normal saline in a portion of 1, 1/2, and 1/4. The trypsin suspensions were transferred to culture plates for incubation and colony-forming unit assay. The data were analyzed by Mann-Whitney test and SPSS software version 16 at a significant level of 0.05. Results: There was a meaningful difference between disinfection effects of Epimax-Deconex for all mentioned microorganisms after 5 min (P = 0.034), and between disinfection effects of NaOCl 0.525%-Epimax for S. aureus (P = 0.043) and P. aeruginosa (P = 0.046) after 5 min. Furthermore, there was a meaningful difference between disinfection effects of Epimax-Deconex (P = 0.034) and NaOCl 0.525%-Epimax (P = 0.034) for P. aeruginosa after 10 min. Conclusion: Condensational silicone can be effectively disinfected by spraying tested three disinfecting agents. More specifically, Deconex showed the best results compared to the other agents.

Background: Different studies have shown the uncertain effects of thermal cycling (TC) and mechanical load cycling (MC) on the dentin microtensile bond strength (µTBS) of composites. This study designed to investigate the effects of TC and MC on the dentin µTBS of single bond-2. Materials and Methods: Flat dentinal surface was prepared on 48 sound extracted human third molar teeth, and were bonded by single bond-2 adhesive and Z250 resin composite. The teeth were randomly divided into eight equal groups, according to the thermal/mechanical protocol. TC and MC were proceeded at 5-55°C and 90 N with 0.5 Hz. Then restorations were sectioned to shape the hour-glass form and subjected to µTBS testing at a speed of 0.5 mm/min. To evaluate the bonding failure, the specimens were observed under the scanning electron microscope. The results were statistically analyzed with analysis of variance, t-test, Tukey HSD and post-hoc by using SPSS software version 17 at a significant level of 0.05. Results: µTBS of all groups were significantly lower than the control group (P < 0.001). Adhesive failure was predominant in all groups and increased with TC and MC. Conclusions: TC and MC had an adverse effect on µTBS of the tested adhesive resin to dentin.

Background: Joint range of motion might affected by some factors like laxity and increase joint mobility. Generalized joint hypermobility and temporomandibular joint hypermobility (TMJH) are reported as risk factors for temporomandibular disorders. The aim of this study was to survey the etiological factors of TMJH and its relations to habitual status. Materials and Methods: In this cross-sectional descriptive study, 69 patients with TMJH were involved. After profiling personal information and medical history, the patients were divided into three groups based on their maximum mouth opening (MMO) as follow: (Light) MMO of 50-55 mm, (moderate): MMO between 55 and 65 mm, (severe) MMO >65 mm. For subjective observations, patients were asked to fill the prepared questionnaire. The objective evaluations conducted by a specialist. Finally, all the data subjected Chi-Square test by using SPSS software version 22 at a significant level of 0.05. Results: TMJH was more common in women (74.2%). The light group had significant differences with other groups in the discomfort of TMJ and TMJ sound (P < 0.05). Furthermore, sever group manifested highest percentage of masticatory pains, significantly (P < 0.05). Conclusion: It can be concluded that pain in TMJ would have a correlation with MMO.

Background: Long-term success of endodontic surgeries is often influenced by the type of root-end filling material (RFM). The aim of present study was to compare the marginal adaptation of two different RFM, cold ceramic (CC) and mineral trioxide aggregate (MTA), using scanning electron microscope (SEM). Materials and Methods: About 20 extracted human single-rooted teeth were collected and stored into sodium hypochlorite 5.25%. The teeth were decronated from the cemento-enamel junction to prepare 16 mm roots. The working length was measured, and 1/3 coronal of the canal was prepared by Gates-Glidden drills. Apical flaring was followed by K file size # 40-70 based on step back technique. After filling of the canals, 3 mm above the apex was cut at 90° to the long axis. Furthermore, 3 mm of the filling was removed from the apical part using the ultrasonic device. All of the prepared specimens were divided into two groups and were retro filled by MTA and CC. The roots were cut horizontally from 1 mm above the apical part, and dentin-filling material interface was observed by SEM. Finally, the collected data were analyzed by Mann-Whitney test and using SPSS software version 18 at a significant level of 0.05. Results: The mean interfacial adaptation was higher in CC group. However, no significant differences were observed by statistical test (P = 0.35). Conclusion: Both CC and MTA had similar marginal adaptation as RFM however in vivo studies are recommended for better determination.

Background: Providing an apical stop in open apex roots is one of the endodontic challenges. The aim of present study was to compare the surface hardness of both mineral trioxide aggregate (MTA) and calcium-enriched mixture (CEM) cement at different setting situations. Materials and Methods: A total of 40 freshly extracted human teeth with a single root and normal apex and no obvious caries or curvatures were selected. The teeth were sectioned horizontally from the cement enamel junction and 2 mm above the apex. Standard technique was administered for cleaning and shaping of the canals. Open apex root canal was prepared by using Peso reamer. The prepared teeth were randomly divided into four groups in which two groups were filled by MTA and the other groups were filled by CEM. White MTA and CEM cement plugs were prepared and condensed up to the apical end. In two groups, moistened paper point was placed in the canals and in the other group dried paper points was used. Vickers test was done to evaluate the microhardness and the collected data were analyzed by Kruskal-Wallis and three-way ANOVA tests using SPSS software version 18 at a significant level of 0.05. Results: The highest and lowest surface hardness was observed in CEM group at dried condition with 4 mm thickness indentation (145.10 ± 7.60 kg/mm2) and moist MTA group indented at 8 mm thickness (111.25 ± 5.37 kg/mm2). However, no significant difference was noticed (P > 0.05). Conclusion: Humidity condition might not influence the microhardness properties of both MTA and CEM cement apical plugs at different tested indentation thickness.

Connecting prostheses to the implant abutments has become a concern and achieving a satisfactory retention has been focused in cement-retention prostheses recently. Sandblasting is a method to make a roughened surface for providing more retention. The aim of this study was to compare effects of nano and micro airborne abrasive particles (ABAP) in roughening surface of implant abutments and further retention of cemented copings. Thirty Xive abutments and analogues (4.5 D GH1) were mounted vertically in self-cured acrylic blocks. Full metal Ni-Cr copings with a loop on the top were fabricated with appropriate marginal adaptation for each abutment. All samples were divided into 3 groups: first group (MPS) was sandblasted with 50 µm Al2O3 micro ABAP, second group (NSP) was sandblasted with 80 nm Al2O3 nano ABAP, and the third group (C) was assumed as control. The purpose of this literature review was to screen the valuable published articles regarding to the impacts of mini-implants on orthodontic science, briefly. The searching category was performed on the Pubmed using MeSH words such as “dental (mini) implants, orthodontic anchorage procedures, and orthodontic appliances.” After preliminary sketch, they were grouped as follow: Those evaluating (a) common appliances for providing orthodontic anchorage, (b) biomechanical details of mini-implants and their insertion, (c) clinical application of mini-implants for orthognathic treatments, (d) limitations and possible complications. In conclusion, mini-implant evolved the orthodontic treatment plans and compromised the required orthognathic surgery. Malocclusion treatment and pure orthodontic or orthopedic movements in the three-dimensions have become recently possible by using mini-implant to provide skeletal anchorage.

Traditional acupuncture has been introduced more than 2500 years ago which provides an alternative and complementary option during clinical practices. Its main mechanism is based on stimulating the nerves by altering the processes and perception of pain transmitters. It facilitates releasing natural pain relievers such as endorphins and serotonin. Its success for various dental procedures has been proved earlier. However, its effects on controlling the gag reflex seem to be overlooked. The gag reflex is recognized as a protective reaction for stopping the entrance of any foreign bodies into the oropharynx. Pronounced gag reflexes can have negative impacts on the quality of dental procedures. Many techniques have been suggested for managing this reflex and acupuncturing is one of those which seems to be overlooked recently. The aim of this paper is reviewing the published high-quality researches about the efficacy of this technique for eliminating the gag reflex during dental procedures.

Aims: Implant placement in the posterior region of the maxilla might be problematic due to poor regional bone quality. The aim of this study was to clinically and radiologically evaluate implants which were placed in the posterior region of the maxilla (with insufficient bone height) with osteotome sinus lift technique after 19-month follow-up. Materials and Methods: Twenty-four patients with posterior maxillary alveolar height ranging from 5 to 8 mm were chosen for this prospective study. After breaking of the cortical bony sinus floor, sufficient bone substitute was placed, and sinus floor was elevated. In this way, a new sinus floor was created, which was designated for further implants placement. Fifty implants were placed immediately after osteotomy sinus lift technique. The mean clinical and radiological follow-up period was 19 months (with a range of 14–24 months). Success factors such as the absence of mobility, pain, infection, and the amount of crestal bone loss were determined in this study. For data analyzing, Kruskal–Wallis and Mann–Whitney tests were used. Results: After 19 months, results showed 96% success rate. Two out of fifty implants failed due to mobility. The mean depth of implants in sinus, mean height of residual crestal bone before surgery, and the mean rate of crestal bone loss were 3.8, 7.9, and 0.71 mm, respectively. Conclusion: Osteotome sinus lift technique is a noninvasive surgical method for enhancing a desired length. Furthermore, implants insertion was successful after osteotome sinus lift technique in cases with insufficient bone height.

This clinical report describes prosthetic rehabilitation of posterior open bite relationship in a patient with several missing teeth and skeletal Class III malocclusion. Primary diagnostic esthetic evaluations were performed by mounting casts in centric relation and estimating lost vertical dimension of occlusion. Exclusive treatments were designated by applying overlay removable partial denture with external attachment systems for higher retentions.
