2. Perceptions and experiences of directly observed treatment in tuberculosis: insights from a mixed-methods cross-sectional study.

Objective: The demanding nature and psychosocial burdens of directly observed treatment (DOT) have opened a path to alternative strategies such as video-observed therapy (VOT), which offers comparable treatment outcomes and patient satisfaction while potentially saving time and reducing costs. The objective of this study was to evaluate the perceptions and experiences of patients and health care professionals regarding DOT and other treatment strategies implemented in Portugal. Methods: Patients with a confirmed diagnosis of tuberculosis, treated at the Vila Nova de Gaia Outpatient Tuberculosis Centre in the last two years, were asked to complete a brief questionnaire, as were health care professionals working in the northern region of Portugal. Differences were analysed with chi-square tests, complemented by thematic analysis. Results: A total of 62 individuals completed the questionnaire: 29 health care professionals and 33 patients. There were significant differences between the two groups in their views regarding the impact of DOT on treatment outcomes, with health care professionals perceiving a higher degree of negative effects and patients expressing greater satisfaction. Long travel distances, transportation issues and high costs were some of the challenges mentioned by the patients. Significant differences were also found regarding the role DOT plays in ensuring treatment adherence, with patients emphasising personal responsibility and its importance in preventing loss to follow-up and strengthening relationships with health care professionals. Dose dispensing was favoured for its convenience in specific situations, and VOT was generally preferred to reduce constant travelling. Both parties raised some concerns. Conclusions: Existing discrepancies suggest a misalignment between patient experiences and health care provider perceptions, underscoring the need for enhanced communication and a more nuanced understanding of patient perspectives when designing and implementing different tuberculosis treatment adherence strategies. The presented article articulates the theme of the presence of religious beliefs in the central area of Rio de Janeiro, in the beginning of XXth century, with contemporary methods of digital mapping, from the primary sources related with the chronists views that experienced this dimension of the city, in the eve of the great urban transformations brought by Modernity. From the chronicles of João do Rio and Luiz Edmundo, we seek to visualize the location of places and characters present in their reports, revealing the coexistence of various religious beliefs far beyond the churches which formed the most basic and visible matrix of religion in urban space. In the end, comparisons with nowadays city reveal the transformations of its form and uses that deeply modified its character in the period of a century. Keywords: Mapping, Religion, Rio de Janeiro, Society., O artigo apresentado articula o tema da presença das crenças religiosas no centro da cidade do Rio de Janeiro, no início do século XX, com métodos contemporâneos de mapeamento digital, a partir das fontes primárias relacionadas com a visão de cronistas que vivenciaram esta dimensão da cidade, às vésperas das grandes transformações urbanas trazidas pela Modernidade. Partindo dos textos de João do Rio e Luiz Edmundo, busca-se visualizar a localização dos lugares e personagens presentes em seus relatos, revelando a coexistência das diversas crenças religiosas para além das igrejas que formavam a matriz básica e mais visível da religião no espaço urbano. Ao final, comparações com a cidade atual nos revelam as transformações da sua forma e dos seus usos que, no período de um século, modificaram profundamente o caráter do centro do Rio de Janeiro. Objectives Osteoarthritis (OA) is a chronic degenerative musculoskeletal disease that causes articular damage and chronic pain, with a prevalence of up to 50% in individuals >60 years of age. Patients suffering from chronic painful conditions, including OA, also frequently report anxiety or depression. A systematic review and meta-analysis were performed to assess the correlation between pain severity and depressive and anxious symptomatology in OA patients. Methods A systematic search was conducted using four databases (PubMed, Medline, Scopus, and Web of Science) from inception up to 14 January 2020. We included original articles evaluating pain severity and anxiety and/or depression severity in OA-diagnosed patients. Detailed data were extracted from each study, including patients' characteristics and pain, anxiety, and depression severity. When available, the Pearson correlation coefficient between pain and depression severity and pain and anxiety severity was collected, and a meta-analysis of random effects was applied. Results This systematic review included 121 studies, with a total of 38 085 participants. The mean age was 64.3 years old, and the subjects were predominantly female (63%). The most-used scale to evaluate pain severity was the Western Ontario and the McMaster Universities Osteoarthritis Index, while for anxiety and depression, the Hospital Anxiety and Depression Scale was the most used. The meta-analysis showed a moderate positive correlation between pain severity and both anxious (r = 0.31, P <0.001) and depressive symptomatology (r = 0.36, P <0.001). Conclusion Our results demonstrate a significant correlation between pain and depression/anxiety severity in OA patients, highlighting the need for its routine evaluation by clinicians. Tese de doutoramento em Ciências da Saúde, Acute pain is a protective mechanism that triggers escape behaviours in order to avoid a source of injury. However, when pain becomes constant and prolonged, such as in arthritis, it highly affects the quality of life. Moreover, arthritic pain is often accompanied by other disorders, such as mood alterations like anxiety and depression, which are usually overlooked when treating arthritic patients. Although the mechanisms underlying the comorbid development of anxiety and depression during chronic pain are not yet understood, it is clear that brain regions and neurotransmitter pathways involved in pain processing and modulation are also implicated in the modulation of mood and emotions. Among other neurotransmitters, galanin (GAL) appears to be an important player in both pain and emotions, with GAL infusion into the brain ventricles eliciting depressive-like behaviours and activation of the amygdala, a central area of emotional and pain processing, and the dorsomedial nucleus of the hypothalamus (DMH), a region that recently emerged as a hypothalamic region involved in anxiety response and capable of mediate behavioural hyperalgesia (increased nociception). The DMH pronociceptive action was correlated with alterations in the activity of rostral ventromedial medulla (RVM) On- and Off- pain modulatory cells. The RVM is part of the main pain inhibitory system although its facilitatory role has also been demonstrated. In addition to the RVM, the medullary dorsal reticular nucleus (DRt) is also a brainstem area associated to pronociception, with its neurones responding almost exclusively to noxious stimulation. The kaolin/carrageenan (K/C) model of monoarthritis was used to assess the effect of prolonged arthritic-like pain in the knee upon nociception as well as upon the comorbid development of emotional-like disorders. Four weeks after saline (SHAM animals) or K/C (ARTH animals) injection into the right knee, animals were evaluated by the pressure application test directly in the injected knee to confirm the arthritic pain state. The open field and elevated plus maze tests were used to assess locomotion and anxiety-like behaviour; and the sucrose preference and forced swimming tests to evaluate depression-like behaviours. In order to study a potential therapeutic effect of amitriptyline, a tricyclic antidepressant, in the reversion of pain- and emotional-related behaviours in ARTH animals, a three-week period of amitriptyline treatment was performed. The contribution of GAL receptors for the DMH-mediated behavioural hyperalgesia and associated pronociception, in SHAM and ARTH animals, was evaluated using heat noxious stimulation and electrophysiological recordings of RVM and spinal dorsal horn neuronal activities when pharmacologically manipulating the DMH with agonist and antagonists of GAL receptors. The role of the DRt as a potential brainstem mediator and the serotonin subtype-3 receptors (5HT3R) as potential spinal cord mediators of the descending GAL/DMH pronociceptive effect was assessed by administering GAL in the DMH with simultaneous blockage of the DRt and 5HT3R in the spinal cord, respectively, and measuring the behavioural response of SHAM and ARTH animals in the Hargreaves test. Our results demonstrated that prolonged arthritis in the knee induces pain and comorbid development of anxiety- and depressive-like symptoms, which can be at least partially reversed by the treatment with amitriptyline. In addition, the general activation of the DMH by glutamate resulted in behavioural hyperalgesia only in SHAM animals with concomitant alteration in the activities of RVM On- and Off-like cells. Interestingly, a GAL-dependent DMH pronociceptive pathway, independent of RVM On- and Off-like cells, is not only functional in SHAM but also in ARTH animals, as the administration of GAL in the DMH decreased the paw-withdrawal latency of the animals, an effect that was mimicked by the infusion of a GAL receptor 1 agonist. The reversion of the pronociceptive effect of GAL in the DMH was observed when inhibiting the DRt with lidocaine or by blocking spinal 5HT3R, although the behavioural hyperalgesia elicited by DRt activation was not eliminated by blocking these spinal receptors. Importantly, not only the DRt-dependent but also the spinal 5HT3R-dependent pathways are tonically activated in ARTH animals. Finally, preliminary results showed that RVM Neutral-cells could be the brainstem mediators of the GAL-DMH nociceptive facilitatory pathway that acts through the activation of spinal 5HT3R. In conclusion, GAL receptors in the DMH appear to play a key role in nociceptive facilitation during chronic arthritis, with these receptors having a potential role in the integration of emotional and nociceptive information. Further studies should evaluate the role of GAL in the DMH upon the development of anxiety and depression-like pathologies; and to clarify upstream nuclei that could be activating this pathway, such as the amygdala and the bed nucleus of the stria terminalis, for a better understanding of the mechanisms underlying chronic pain and associated emotional disorders., A dor aguda é um mecanismo protetor que desencadeia comportamento de escape no sentido de evitar danos tecidulares. No entanto, quando esta se torna constante e prolongada, como no caso da artrite, tem um enorme impacto na qualidade de vida. Além disso, a dor artrítica é geralmente acompanhada por outras patologias, como alterações emocinais (ansiedade e depressão), que são normalmente sub-avaliadas quando se trata pacientes com artrite. Embora os mecanismos responsáveis pelo desenvolvimento de ansiedade e depressão durante dor crónica não estejam clarificados, a existência de regiões encefálicas e neurotransmissores envolvidos no processamento e modulação da dor que estão também implicados na modulação do humor e das emoções é amplamente conhecida. Entre outros neurotransmissores, a galanina (GAL) parece ser um importante interveniente tanto na dor como nas emoções, sendo que a injecção de GAL nos ventrículos cerebrais promove um comportamentos do tipo depressivo e ativa a amigdala, uma área importante no processamento das emoções e da dor, e o núcleo dorsomedial do hipotálamo (DMH), uma região que está envolvida na resposta à ansiedade e na mediação de hiperalgesia comportamental (aumenta a nociceção). A ação pronocicetiva do DMH foi correlacionada com alterações na atividade das células-“On” e –“Off” do bolbo rostral ventromedial (RVM). O RVM é parte do principal sistema de inibição descendente da dor, embora já tenha sido demonstrado o seu papel na facilitação da nociceção. Para além do RVM, também o núcleo dorsal reticular (DRt), no tronco cerebral, está associado a pronociceção, contendo neurónios que respondem preferencialmente a estímulos nóxicos. O modelo de monoartrite induzido por caolina e carragenina (K/C) foi usado para estudar o efeito de dor artrítica prolongada no joelho na nociceção, assim como no desenvolvimento de patologias do tipo emocional. Quatro semanas após a injeção de soro fisiológico (animais SHAM) ou K/C (animais ARTH) no joelho direito, os animais foram avaliados no teste pressure application measurement diretamente sobre o joelho injetado para confirmar o estado de dor artrítica. Os testes open field e elevated plus maze foram usados para avaliar a locomoção e o comportamentos do tipo ansioso; e os testes sucrose preference e forced swimming para avaliar o comportamento do tipo depressivo. No sentido de estudar um potencial efeito terapêutico da amitriptilina, um antidepressivo tricíclico, na reversão dos comportamentos dolorosos e emocionais em animais ARTH, foi realizado um tratamento durante três semanas. A contribuição dos recetores de GAL na hiperalgesia e pronociceção mediadas pelo DMH, em animais SHAM e ARTH, foi avaliada usando estimulação nóxica quente e gravações eletrofisiológicas da atividade neuronal do RVM e do corno dorsal da medula espinhal, enquanto se manipulou farmacologicamente o DMH com agonistas e antagonistas dos recetores de GAL. O papel do DRt como um potencial mediador encefálico e dos recetores de serotonina do tipo-3 (5HT3R) como potenciais mediadores espinhais do efeito pronocicetivo descendente da GAL no DMH foi avaliado através da administração de GAL no DMH com o bloqueio simultâneo do DRt e dos 5HT3R na medula espinhal, respetivamente, e medindo a resposta comportamental dos animais SHAM e ARTH no teste de Hargreaves. Os resultados demonstram que a artrite prolongada no joelho induz dor e o desenvolvimento de sintomas de ansiedade e depressão que podem ser parcialmente revertidos pelo tratamento com amitriptilina. Além disso, uma ativação geral do DMH com glutamato resulta em hiperalgesia comportamental apenas em animais SHAM com alterações concomitantes na atividade das células do tipo “On” e “Off” do RVM. Interessante foi o facto da via pronociceptiva do DMH dependente de GAL, que é independente das células do tipo “On” e “Off” do RVM, se encontrar funcional não só em animais SHAM mas também em animais ARTH, uma vez que a administração de GAL no DMH diminuiu a latência de flexão da pata dos animais, um efeito que foi mimetizado pela injeção de um agonista dos recetores de GAL do tipo-1. A reversão do efeito pronocicetivo da GAL no DMH foi observada quando se inibiu o DRt com lidocaina ou bloqueou os 5HT3R espinhais, embora a hiperalgesia comportamental induzida pela ativação do DRt não tenha sido eliminada pelo bloqueio destes recetores espinhais. Importante é o facto de, tanto a via dependente do DRt como a via dependente dos 5HT3R espinais se encontrarem tonicamente ativas nos animais ARTH. Finalmente, resultados preliminares mostram que as células “Neutras” do RVM podem ser os mediadores supraspinhais da via facilitadora induzida pela GAL no DMH que atua pela ativação dos 5HT3R na medula espinal. Em conclusão, os recetores de GAL no DMH parecem ser importantes na facilitação da nociceção durante artrite crónica, com estes recetores a terem um potencial papel na integração da informação emocional e nocicetiva. Dissertação de mestrado em Ciências da Saúde, Chronic neuropathic pain affects 7-10% of the population and is often accompanied by comorbid emotional impairments that adversely affect nociceptive symptomatology. This condition greatly reduces the quality of life of the patients, impairing physical, cognitive, emotional and social functioning which, consequently, interferes with treatment. Importantly, although chronic pain and emotional disorders are more prevalent in women, only a few publications focus on female animals. While the chronic constriction injury (CCI) model has been shown to mimic human neuropathies regarding mechanical/thermal hyperalgesia, allodynia and spontaneous pain in both sexes, data on CCI-induced emotional impairments on female rats remains scarce. In this work, young adult rats (Wistar Han) were divided into five groups: males with CCI (CCIM), gonadally intact females (SHAM/SHAM), ovariectomized females (SHAM/OVX), ovariectomized females with CCI (CCI/OVX) and gonadally intact females with CCI (CCI/SHAM). Nociceptive testing began before CCI surgery and was performed weekly throughout 5 weeks after which the anxiodepressive-like behaviour was also assessed. In the postoperative period, CCI animals displayed visible gait abnormalities, limping and guarding the affected hind paw, and the development of mechanical and cold allodynia from week 1 onwards. CCI developed mechanical allodynia, with CCI/OVX animals displaying symptoms prior to CCI/SHAM females. While no differences were found between CCI and SHAM animals concerning anxiety- and depressive-like behaviours, ovariectomy induced anhedonic-like behaviour, regardless of CCI surgery. Single cell electrophysiological data from the rostral ventromedial medulla (RVM), an area mediating nociceptive processing, suggest an increase in descending pronociception after chronic neuropathy. These results demonstrate the existence of sex differences on the comorbidity between neuropathic pain on the CCI model, modulated by RVM descending control., A dor crónica neuropática afeta 7-10% da população e é frequentemente acompanhada por distúrbios emocionais, nomeadamente ansiedade e depressão. Esta patologia reduz significativamente a qualidade de vida dos pacientes, afetando as suas capacidades físicas, cognitivas, emocionais e sociais e, consequentemente, interferindo com o tratamento. No entanto, apesar de a dor crónica e dos distúrbios emocionais que lhe estão associados serem mais prevalentes em mulheres, poucos trabalhos são realizados em animais fêmea. Não obstante o modelo de lesão por constrição crónica (CCI) mimetizar neuropatias humanas em termos de hiperalgesia mecânica e térmica, alodinia e dor espontânea em ambos os sexos, são escassos os dados sobre distúrbios emocionais em ratos fêmea. Neste trabalho, ratos jovens (Wistar Han) foram divididos em cinco grupos: machos com CCI (CCIM), fêmeas gonadicamente intactas (SHAM/SHAM), fêmeas ovariectomizadas (SHAM/OVX), fêmeas gonadicamente intactas com CCI (CCI/SHAM) e fêmeas ovariectomizadas com CCI (CCI/OVX). A avaliação nocicetiva iniciou-se na semana anterior à cirurgia de CCI, perdurando 5 semanas, após as quais o comportamento anxiodepressivo foi também avaliado. No período pós-operatório, os animais com CCI apresentaram alterações na marcha, e o desenvolvimento de alodinia mecânica e térmica ao frio. Todas as fêmeas CCI desenvolveram alodinia mecânica e este comportamento instalou-se mais cedo nas CCI/OVX comparativamente às CCI/SHAM. A indução de CCI em fêmeas não levou ao desenvolvimento de comportamentos ansiosos e depressivos, embora a ovariectomia tenha induzido um comportamento tipo-anedónico, independentemente de CCI. As gravações eletrofisiológicas single cell na medula rostral ventromedial (RVM), área mediadora do processamento nociceptivo, sugerem um aumento da pronociceção descendente após neuropatia crónica. Estes resultados demonstram a existência de diferenças entre sexos na comorbidade entre a dor neuropática e distúrbios emocionais no modelo de CCI, modulada pelo controlo descendente da RVM., Este trabalho foi realizado no Instituto de Investigação em Ciências da Vida e da Saúde (ICVS) da Universidade do Minho. 28. Comorbidity between experimental osteoarthritis and mood disorders in the rat : investigating the role of supraspinal galanin

Dissertação de mestrado em Ciências da Saúde, Arthritis and depression are pathologies highly prevalent in society with great impact not only on National Health Systems but also on the quality of life of patients. Several studies have shown that anxiety and depression are common comorbidities of chronic pain patients while anxious and/or depressed individuals also display altered perception of pain. This interplay has been suggested to result from the fact that these pathologies share common modulatory pathways. Indeed, acute changes in the neurochemistry of brain areas, such as the amygdala (AMY), have been shown to transiently alter both mood and nociception. In chronic inflammatory pain, galanin (GAL) and its receptors have been recently proposed as potential mediators of pain-depression comorbidity as the expression of this neuropeptide is greatly increased not only in areas mediating emotions (AMY), but also in areas mediating nociception, such as the dorsomedial nucleus of the hypothalamus (DMH). Additionally, some studies using animal models of depression, have also demonstrated that the differential availability/activation of galanin receptors could induce a depressive profile in these animals. In the present work, we propose to (i) evaluate how GAL in the DMH influences anxiety- and/or depressive-like behaviour; (ii) study the role of the rostral ventromedial medulla (RVM) as a mediator of GAL effects and (iii) investigate which supraspinal areas might be involved in relaying GAL effects through the quantification of c-Fos expression. Wistar Han adult male rats were divided into two experimental groups: animals with experimental osteoarthritis (ARTH) and control animals (SHAM). In a first set of animals, emotional and nociceptive behaviours were assessed after GAL administration in the DMH. In a second set, c-Fos expression in caudal brain areas that mediate nociception was also quantified after GAL in the DMH and peripheral noxious stimulation of the right knee joint. Our results showed ARTH animals display a depressive phenotype concomitant with alterations in serotoninergic tone, a pathway mediated by GAL. Descending facilitation from the DMH after GAL microinjection appears to be mediated both by the RVM and the dorsal reticular nucleus (DRt)., A artrite e a depressão são patologias muito prevalentes na sociedade com um grande impacto não só sobre os Sistemas Nacionais de Saúde mas também sobre a qualidade de vida dos pacientes. Vários estudos demonstraram que a ansiedade e a depressão são comorbidades comuns em pacientes que sofrem de dor crónica, enquanto indivíduos ansiosos e/ou deprimidos também apresentam alterações na perceção da dor. Considera-se que esta interação entre alterações emocionais e dor crónica resulta da partilha de vias moduladoras comuns entre as duas patologias. De facto, já se demonstrou que alterações agudas na neuroquímica de áreas cerebrais, como a amígdala (AMY), alteram tanto o humor como a perceção dolorosa. Na dor inflamatória crónica, a galanina (GAL) e os seus receptores foram recentemente propostos como potenciais mediadores da comorbidade dor-depressão, uma vez que a expressão deste neuropeptídeo está aumentada não apenas em áreas que medeiam as emoções (AMY), mas também em áreas que medeiam a nociceção, como o núcleo dorsomedial do hipotálamo (DMH). Paralelamente, alguns estudos realizados em modelos animais de depressão, também demonstraram que diferenças na disponibilidade/activação dos recetores de galanina promoviam um fenótipo depressivo nestes animais. Neste trabalho, propusemo-nos a (i) avaliar o efeito da GAL no DMH sobre o comportamento emocional e nocicetivo; (ii) estudar o papel da medula rostral ventromedial (RVM) como mediadora do efeito facilitador da GAL, e (iii) estudar quais as áreas supraespinhais que potencialmente medeiam a ação da GAL através da quantificação da expressão de c-Fos. Os animais, ratos machos Wistar Han adultos, foram divididos em dois grupos experimentais: animais osteoartríticos (ARTH) e animais controlo (SHAM), nos quais se avaliou o comportamento emocional e nocicetivo após a administração de GAL no DMH. Num segundo grupo avaliou-se a expressão de c-Fos em áreas cerebrais caudais após a administração de GAL no DMH e a aplicação de estímulos nóxicos periféricos. Os nossos resultados mostraram que a osteoartrite promove um fenótipo depressivo nos animais associado a alterações nas vias serotoninérgicas, as quais se sabe serem mediadas pela GAL. Por fim, verificou-se que a facilitação descendente após a ativação do DMH pela GAL é mediada pelo RVM e pelo núcleo reticular dorsal (DRt).

29. Obstructive Sleep Apnea Syndrome in Heavy Truck Drivers: A Portuguese Prospective Cohort Study.

Introduction: Heavy truck drivers with untreated obstructive sleep apnea (OSA) are at higher risk of driving accidents. This study aims to estimate the prevalence of OSA and to identify the most frequent symptoms and comorbidities in heavy truck drivers., Methods: This cohort study included the employees of a Portuguese transport company between 2019 and 2022. A home sleep apnea test (HSAT) was performed on all patients. SPSS® was used for statistical analysis, and a p-value lower than 0.05 was considered statistically significant., Results: A total of 86 truck drivers were included, with a mean age of 48.02 years (min. 24, max 66) and a mean body mass index (BMI) of 30.14±4.4 kg/m². After performing an HSAT, it was found that 77.9% of drivers (n=67) had OSA, with a mean apnea-hypopnea index (AHI) of 16.72±14.69 events/hour. Concerning diagnosed patients, 44.78% (n=30) had mild, 31.32% (n=23) moderate, and 20.89% (n=14) severe OSA. Obesity, hypertension, and dyslipidemia had a statistically significant association. There were no statistically significant differences between patients with and without type II diabetes mellitus. The presence of nighttime and daytime symptoms had a statistically significant correlation with OSA diagnosis. Despite only eight patients reporting a high score on the Epworth Sleepiness Scale (ESS), 14 patients reported previous episodes of falling asleep while driving, which might be associated with the non-valorization of daytime sleepiness in these patients. The patients who reported previous episodes of falling asleep while driving were older and had higher BMI, higher ESS, and higher AHI., Conclusions: In the evaluated truck drivers, the prevalence of OSA was very high (77.9%), which reinforces the importance of screening for this pathology since, when left untreated, it is a major risk factor for exercising their profession safely., Competing Interests: Ana Macedo reports equipment or supplies and statistical analysis were provided by Linde Healthcare and a relationship with Linde Healthcare that includes non-financial support. Abstract: Osteoarthritis (OA), the most common joint disorder worldwide, is characterized by progressive degeneration of articular and periarticular structures, leading to physical and emotional impairments that greatly affect the quality of life of patients. Unfortunately, no therapy has been able to halt the progression of the disease. Owing to the complexity of OA, most animal models are only able to mimic a specific stage or feature of the human disorder. In this work, we demonstrate the intraarticular injection of kaolin or carrageenan leads to the progressive degeneration of the rat's knee joint, accompanied by mechanical hyperalgesia and allodynia, gait impairments (reduced contact area of the affected limb), and radiological and histopathological findings concomitant with the development of human grade 4 OA. In addition, animals also display emotional impairments 4 weeks after induction, namely, anxious and depressive-like behaviour, important and common comorbidities of human OA patients. Overall, prolonging kaolin or carrageenan-induced monoarthritis mimics several important physical and psychological features of human OA in both male and female rodents and could be further applied in long-term studies of OA-associated chronic pain., (Copyright © 2023 International Association for the Study of Pain.)

- Abstract
Introduction: Chronic postoperative pain is the most frequent late complication of inguinal hernia repair surgery. The aim of this study is to evaluate the incidence of chronic post-hernioplasty pain in outpatient care at Centro Hospitalar do Porto, describe it, analyse its relation with other variables defined in the literature and study its functional interference., Material and Methods: We performed a retrospective cohort study between February and May 2016, using a structured telephone interview composed of questions from the authors and sections of published questionnaires, two of which are validated for the Portuguese language and culture. We included men who underwent ambulatory inguinal hernioplasty, by laparotomy or laparoscopy, at Centro Hospitalar do Porto, between January 2011 and October 2015., Results: In a final sample of 829 surgeries, the incidence of chronic post-hernioplasty pain was 24.0% [confidence interval: 21.2 - 27.1]. The development of chronic post-hernioplasty pain was higher in patients with pre-surgical pain and younger age and was related with the presence of pain during the first month after surgery. No relationship was found between surgical technique and the development of chronic post-hernioplasty pain. Of the individuals with chronic pain, 65.0% mentioned moderate-severe 'pain on the average' and 37.7% presented descriptors suggestive of neuropathic pain. The only parameter evaluated with which chronic post-hernioplasty pain 'did not interfere completely' was sleep., Discussion: The prevalence found for chronic posthernioplasty pain with significant functional interference is in line with data retrieved from literature. The predictive potential of pre-surgical pain and young age for the development of chronic posthernioplasty pain is also in agreement with previous studies. Limitations were found to this study given its retrospective nature., Conclusion: The high prevalence of chronic post-hernioplasty pain raises the urgent need for raising awareness regarding this issue among health care professionals. The main areas for improvement are diagnosis, follow-up and treatment of pain.
