Abstract

Background and aims: Stroke has long-term consequences for the functional performance of daily activities. Evaluating the impact of stroke on health and wellbeing is essential when designing stroke-specific programs supporting home and community participation. This study describes the impact of stroke on quality of life as reported by stroke survivors. It also examined the effect of different personal and clinical characteristics on the quality of life among adults with stroke. Materials and Methods: This cross-sectional study encompassed a sample of 64 adults with stroke (mean age 58.9 years) who were receiving rehabilitation services after stroke onset. Outcome measures included the Stroke Impact Scale (SIS) and its subdomains to assess the self-perceived disability following a stroke. The data analysis plan included MANOVA, followed by ANOVA of main effects and post-hoc analyses. Results: Jordanian individuals with stroke had a mean SIS total score of 59.9 (SD ±15.4, range 25.8–94) indicating moderate quality of life following stroke. Furthermore, the domains of hand function, strength, activities of daily living, participation, and emotions received lower mean SIS scores. Participants with a higher number of previous strokes had significantly lower emotional subdomain scores on the SIS (p=0.001). Participants with aphasia had significantly lower scores in the memory (p< 0.001) and communication subdomains (p=0.0001). Conclusion: Individuals with stroke reported moderate levels of disability after stroke onset. Aphasia and an increased number of previous strokes were associated with greater challenges in quality of life post-stroke. [ABSTRACT FROM AUTHOR]

Abstract

Objectives: Visceral leishmaniasis (VL) is a deadly disease endemic in all countries around the Mediterranean Sea. In Palestine, VL is transmitted to humans from infected dogs by a sandfly bite. The aim of this study was to investigate the epidemiology and spatiotemporal pattern of VL in Palestine within a period of 27 years (1990–2017). Methods: A long-term descriptive epidemiological study on human VL was conducted based on patient's profiles to calculated disease prevalence, geographical, spatiotemporal and seasonal distribution, distribution by age and gender, diagnosis, treatment, and treatment outcome. Results: A total of 343 patients were reported, the average annual incidence rate was 0.73 case/100 000 population. Most cases came from the western parts of the West Bank. The number of reported males was 162 (51%), and patient’s age ranged from 4 months to 16 years (average 2.5 years), of which 93.3% were ≤5 years old. The annual incidence rate increased between 1993–1999, peaked in 1995, and dropped as from 2002. The future projections of VL indicate that the endemic foci in most governorates are projected to disappear in the future and only the very northwest of the West Bank will be at risk of VL. Conclusions: Visceral leishmaniasis continues to be endemic in the West Bank but not the Gaza Strip. Pentavalent antimonial sodium stibogluconate continues to be the first line treatment and physicians are recommended to consider liposomal Amphotericin B (AmBisome) for refractory patients only. Abstract

Background Cutaneous leishmaniasis (CL) is a vector-borne parasitic diseases of public health importance that is prevalent in the West Bank but not in the Gaza Strip. The disease caused by parasitic protozoans from the genus Leishmania and it is transmitted by infected phlebotomine sand flies. The aim of our study is to investigate the eco-epidemiological parameters and spatiotemporal projections of CL in Palestine over a 30-years period from 1990 through 2020 and to explore future projections until 2060. Methodology/Principal findings This long-term descriptive epidemiological study includes investigation of demographic characteristics of reported patients by the Palestinian Ministry of Health (PMoH). Moreover, we explored spatiotemporal distribution of CL including future projection based on climate change scenarios. The number of CL patients reported during this period was 5855 cases, and the average annual incidence rate (AAIR) was 18.5 cases/105 population. The male to female ratio was 1.25:1. Patients-age ranged from 2 months to 89 years (mean = 22.5, std 18.67, and the median was 18 years). More than 65% of the cases came from three governates in the West Bank; Jenin 29% (1617 cases), Jericho 25% (1403), and Tubas 12% (658) with no cases reported in the Gaza Strip. Seasonal occurrence of CL starts to increase in December and peaked during March and April of the following year. Current distribution of CL indicate that Jericho, Tubas, Jenin and Nablus have the most suitable climatic settings for the sandfly vectors. Future projections until 2060 suggest an increasing incidence from northwest of Jenin down to the southwest of Ramallah, disappearance of the foci in Jericho and Tubas throughout the Jordan Vally, and possible emergence of new foci in Gaza Strip. Conclusions/Significance The future projection of CL in Palestine until 2060 show a tendency of increasing incidence in the north western parts of the West Bank, disappearance from Jericho and Tubas throughout the Jordan Vally, and emergence of new CL endemic foci in the Gaza Strip. These results should be considered to implement effective control and surveillance systems to counteract spatial expansion of CL vectors.

Abstract

Background: The use of cosmetic products is growing in dominance in the Arab population, making it essential to measure its effects on users. The production of cosmetics has been largely driven by consumerism and a bid to keep abreast with the latest trends in the beauty industry with less attention on how the users' quality of life (QoL) is affected. Aims: This study aims to investigate the effect of cosmetic products on users' quality of life in eight Arab countries. Methods: A cross‐sectional study was carried out using an online data collection approach. A validated and specialist instrument tool called BeautyQoL, which consists of five domains and a total of 52 questions, was distributed to a sample of 2219 cosmetic users. Descriptive and inferential statistical analysis was done using SPSS® version 26.0. Results: The mean age of participants was 34 ± 11.25 years, and more women were represented in the sample (71%) than men. The majority of respondents had oily skin type (39.6%) and tan skin tone (30.4%). QoL through cosmetic use is computed with a mean score of 51 out of 100. The users' mean score satisfaction from cosmetic use is centred on attractiveness (56.1), followed by self‐confidence (51.8). Cosmetics have a statistically significant effect on participants who are young adults, women, single, and employed with high income. As the respondents' skin tone deepens from very fair to dark, the mean score for each domain significantly increases, whereas when skin type changes from very oily to dry, the mean score for each domain decreases. Conclusion: The effect of cosmetics on the users' QoL is limited, contrary to the narrative commonly portrayed in cosmetics' advertisements. Therefore, the use of cosmetics among the Arab population should be from an informed perspective of their specific needs instead of conforming to the viral trends pedaled by influencers and bloggers on social media, which might be irrelevant for them. Abstract

Measures to limit the spread of COVID-19 have disrupted the lives of countless millions of individuals worldwide. To explore the experiences of occupational disruption and changing social roles among Jordanians during COVID-19 lockdown in 2020. The specific aim was to explore the changing occupations, routines, roles and coping strategies. This qualitative study used a digital ethnographic approach. Data collection included online observations of Jordanian's posts of comments, photos and videos on six public Facebook pages during the COVID-19 lockdown for the initial 3-week period. In addition, 150 Jordanians participated in an online interview answering four open-ended questions to achieve triangulation. Qualitative analyses included open coding of the photos, videos and text as well as the interviews independently and blindly by the research team. Four themes were identified: changing daily occupations; changing family roles; missing pre-lockdown life and coping strategies under lockdown. The COVID-19 lockdown restrictions caused direct, significant and immediate changes to Jordanians' occupations, routines and roles; enforcing new obligatory occupations and eliminating desired ones. Knowledge from this study highlight the kinds of strategies and supports during and after lockdowns that need to be considered in future planning of occupational therapy practice in Jordan. Abstract

Background: The gallstones are common health problem across the world with huge financial burden on health authorities. Obesity and insulin resistance are associated with risk of gallstones disease (GSD). The aim of this study was to assess the prevalence of metabolic syndrome (MetS) and diabetes and associated risk factors in Sudanese patients with gallstones. Methods: A prospective cross-sectional study, enrolled patients with gallstones attending Ibn Sina Specialized Teaching Hospital for gastrointestinal and hepatobiliary diseases. A structured questionnaire was applied, anthropometric measures were taken, and blood tested for HbA1c, fasting glucose and lipid profile. Data was analysed using SPSS version 23. Results: A total number of 151 participants were recruited in the study, 71 of them were ultrasound confirmed GSD patients, and the other 80 were controls without GSD over a period of six months. The prevalence of the MetS and diabetes was 30% and 23.9% respectively. Borderline diabetes was 16.9% and overweight and obesity constituted more than half of the sample 59.6%. Using Chi-Square test, a statistically significant association was found between MetS and HDL, TG, LDL level, waist circumference and blood pressure (BP). Absolute predictors and the risk factors for gallstone disease were waist circumference, age, HbA1c and LDL. Conclusions: The prevalence of MetS and diabetes among gallstone patients was 30% and 23.9% respectively. Absolute predictors and the risk factors for gallstone disease were waist circumference, age, HbA1c and LDL.

Abstract

Abstract Background Canine visceral leishmaniasis (CVL) is caused by Leishmania infantum in all Mediterranean countries. The Leishmania parasite is transmitted by the bite of a corresponding sand fly vector and primarily maintained in nature by wild and domestic reservoirs, including dogs, foxes and jackals. Infected dogs are the primary reservoir host in endemic regions and are the most significant risk disposing humans to infection. The present study aimed at assessing the prevalence of infection with Leishmania and identification of Leishmania infantum in domestic dogs in the West Bank, Palestine. Methods The infection rate among domestic dogs collected from seven districts in the Palestinian West Bank was investigated by examination of parasites in culture from the buffy coat using serological and molecular methods; based on ELISA, internal transcribed spacer 1 (ITS1) and cysteine protease (CPB) PCR. Results Out of 215 dogs examined for Leishmania, 36 (16.7%) were positive in at least one method. Twenty three animals (11.5%) were positive for Leishmania DNA, whereas, ELISA and culture revealed 16 (7.5%), and 4 (1.5%) respectively. CPB-PCR on one of three culture-positive isolates revealed Leishmania infantum as the causative agent for Leishmania infection in dogs. Conclusions Our study showed that canine leishmania infection is prevalent with varying degrees in all the seven studied districts in Palestine despite the absence of human VL cases in 4 of these districts. The causative agent was confirmed to be Leishmania infantum.

Abstract Background Phlebotomus papatasi is a natural vector of Leishmania major, which causes cutaneous leishmaniasis in many countries. Simple sequence repeats (SSRs), or microsatellites, are common in eukaryotic genomes and are short, repeated nucleotide sequence elements arrayed in tandem and flanked by non-repetitive regions. The enrichment methods used previously for finding new microsatellite loci in sand flies remain laborious and time consuming; in silico mining, which includes retrieval and screening of microsatellites from large amounts of sequence data from sequence data bases using microsatellite search tools can yield many new candidate markers. Results Simple sequence repeats (SSRs) were characterized in P. papatasi expressed sequence tags (ESTs) derived from a public database, National Center for Biotechnology Information (NCBI). A total of 42,784 sequences were mined, and 1,499 SSRs were identified with a frequency of 3.5% and an average density of 15.55 kb per SSR. Dinucleotide motifs were the most common SSRs, accounting for 67% followed by tri-, tetra-, and penta-nucleotide repeats, accounting for 31.1%, 1.5%, and 0.1%, respectively. The length of microsatellites varied from 5 to 16 repeats. Dinucleotide types; AG and CT have the highest frequency. Dinucleotide SSR-ESTs are relatively biased toward an excess of (AX)n repeats and a low GC base content. Forty primer pairs were designed based on motif lengths for further experimental validation. Conclusion The first large-scale survey of SSRs derived from P. papatasi is presented; dinucleotide SSRs identified are more frequent than other types. EST data mining is an effective strategy to identify functional microsatellites in P. papatasi.

Abstract

The Baharyia formation is a common reservoir in the Western Desert of Egypt. It is characterized as a heterogeneous reservoir with low sand quality. It is comprised of fine-grained sandstone, thin, laminated, sand-poor parasequences with shale interbeds. The heterogeneity and low permeability of the Upper Baharyia reservoirs are the primary challenges to maintaining economic well productivity. The interest in developing low permeability reservoirs stems from favorable economics attributed to advancements in horizontal well drilling and hydraulic fracturing technology, offering methods to increase production by increasing the contact area of the producing interval. Subsequently, it became apparent that wellbore contact alone was not always sufficient for providing production increases expected, thus requiring multistage hydraulic fracturing (MSHF) stimulation treatments to achieve production targets. Primary well production analysis revealed that the cumulative production from the horizontal well discussed was enhanced from 37 to 70% of recoverable reserve and the recovery factor was doubled. From a production analogy standpoint, these resulted in reduced drilling of three vertical wells and had direct economic benefits by reducing the installed artificial lift strings, related expensive artificial lift equipment repairs, and the number of necessary workovers. This paper takes a multidisciplinary approach to help understand productivity enhancement of low permeability reservoirs in the Western Desert of Egypt, through a detailed analysis of well performance and successful implementation of MSHF in horizontal wells to maximize drainage volume around the well. It is intended to serve as guidelines to help operators facing similar challenges.

47. Successful Field Application of Aqueous-Based Formation Consolidation Treatment Implemented in Nile Delta, Egypt

Abstract

Sand production can be challenging for many operators in unconsolidated formations. The production of formation sand and fines can cause erosion damage to both surface and downhole equipment, resulting in major well intervention and sand disposal costs. Conventional resin consolidation systems have been successfully used to prevent solids production in short, homogenous intervals. However, resin consolidation attempts in longer intervals have resulted in erratic success rates, which could be attributed to a lack of complete and uniform treatment of the entire interval length. A recently developed aqueous-based resin (ABR) system has helped provide effective and efficient treatment of significantly longer intervals, has improved health, safety, security, environmental (HSSE) compatibility, and can help improve various operational considerations. This paper discusses results of laboratory testing and the subsequent successful treatment using the ABR system within a well located in Nile Delta, which was the first attempt of its kind in Egypt. Sieve analysis revealed unsorted formation sand, which usually requires a gravel pack completion, rather than a standalone screen; however, because of limitations with respect to wellbore diameter (5 in.), it was difficult to complete the well using conventional gravel pack completion methods. Additionally, the well had a history of unsuccessful treatment using a non-resin product, which modifies the surface potential charges of the sand grains and reduces the repelling forces between them. The well was still producing sand after that treatment; thus, the decision was made to treat the interval using the ABR system. Thereafter, hydrocarbon production, at a rate of 5.5 MMscf/D, was monitored and sand production from the well decreased by more than 95%, with no impairment to hydrocarbon production.
