Objetivos: O objetivo do presente estudo foi testar o papel mediador do medo de receber compaixão dos outros e dos sentimentos de segurança e proximidade, em contexto social, na associação entre vergonha externa e o bem-estar psicológico dos adolescentes. Foi hipotetizado que adolescentes com elevados níveis de vergonha externa apresentem uma diminuição nos níveis de bem-estar psicológico, através do aumento do medo de receber compaixão e da diminuição dos sentimentos de segurança e proximidade em contexto social. Métodos: Participaram neste estudo 361 adolescentes portugueses de ambos os sexos (43,8% rapazes; 56,2% raparigas), com idade entre os 12 e os 18 anos, os quais completaram medidas de autorresposta relativamente a sentimentos de vergonha, medo da compaixão, sentimentos de segurança e de proximidade aos outros, e bem-estar percebido dos adolescentes. Os dados foram explorados através de estatísticas descritivas e correlacionais, e o modelo teórico proposto foi testado através de análises path. Resultados: Os dados pareceram demonstrar que, em ambos os sexos, a vergonha externa está associada a um decréscimo do bem-estar psicológico dos adolescentes, e que este efeito é mediado pelo aumento do medo de receber compaixão dos outros e pela diminuição dos sentimentos de segurança e conexão em contexto social. Os resultados indicaram que o modelo testado apresenta um adequado ajustamento aos dados, explicando 15,0% da variância do medo de receber compaixão dos outros, 37,0% da variância dos sentimentos de segurança e proximidade aos outros e 46,0% da variância do bem-estar psicológico dos adolescentes. Conclusões: Este estudo parece contribuir para uma compreensão mais aprofundada acerca da importância do papel dos sentimentos de inferioridade, do medo de receber compaixão dos outros, e dos sentimentos de segurança e proximidade, no contexto social, no bem-estar psicológico dos adolescentes.

RESUMO Objetivo: Descrever um caso de sífilis congênita com diagnóstico tardio e identificar as oportunidades perdidas nas diversas fases/níveis da atenção à saúde, que retardaram a realização do diagnóstico. Descrição do caso: Menino, 34 dias de vida, encaminhado da Unidade Básica de Saúde a um hospital terciário por apresentar aumento do volume abdominal e icterícia progressiva há 2 semanas, hipocolia fecal, hepatoesplenomegalia, anemia, plaquetopenia e elevação de enzimas hepáticas. Ao exame físico, apresentava lesões eritemato-descamativas nas mãos e nos pés e exantema macular em região inguinal, presença de ascite, fígado palpável a 5 cm do rebordo costal direito e baço palpável a 3 cm do rebordo costal esquerdo. Sorologia do lactente: CMIA (quimioluminescência de micropartículas) reagente, VDRL (Venereal Diseases Research Laboratory) 1:1024 e TPHA (Treponema pallidum Hemaglutination) reagente. Sorologia materna: CMIA e TPHA reagentes, VDRL 1:256. Radiografia de ossos longos mostrava periostite simétrica; levantamento periosteal; e bandas metafisárias lucentes em fêmures, úmeros, ulnas e tíbias. Após tratamento com penicilina cristalina, apresentou melhora clínica e laboratorial, recebendo alta no 18º dia de internação. Comentários: Este caso mostra que ainda ocorre diagnóstico tardio de sífilis congênita por falhas nas estratégias de prevenção dessa doença, tanto na atenção básica quanto nos níveis secundário e terciário. A aplicação das intervenções preconizadas pelo Ministério da Saúde e a identificação das situações em que ocorrem falhas na sua execução são importantes para a avaliação da assistência de rotina em todos os níveis de atenção e nas diversas unidades responsáveis pelo cuidado do recém-nascido e do lactente jovem.

Diferentes processos de regulação emocional parecem estar envolvidos na relação entre a avaliação negativa da imagem corporal e a psicopatologia alimentar. O presente estudo pretendeu clarificar, numa amostra de 369 mulheres, os efeitos distintos da fusão cognitiva e do apreço pela imagem corporal na relação entre a discrepância sentida entre o corpo real e idealizado e o comportamento alimentar perturbado, quando controlado o efeito do IMC. Os resultados indicaram que mulheres que percecionam o seu corpo como significativamente discrepante do padrão valorizado na sociedade atual têm uma maior tendência para apresentar comportamentos alimentares perturbados. No entanto, esta relação é parcialmente explicada por maiores níveis de fusão cognitiva em relação à aparência física e por um menor apreço pela própria imagem corporal. Estes resultados constituem um contributo significativo ao sublinharem a relevância da promoção de desfusão cognitiva e de atitudes autocompassivas enquanto estratégias de regulação emocional protetoras da psicopatologia alimentar.

6. Early memories of warmth and safeness and eating psychopathology: The mediating role of social safeness and body appreciation

Research on human development and functioning has highlighted the importance of early emotional and relational experiences. Particularly, an association between the absence of early positive memories and the presence of disordered eating has been evidenced by recent investigations. However, the study of the mechanisms underlying this association remains unclear. Using a sample of 490 women, the hypothesis that early positive memories are negatively associated with disordered eating via social safeness and a positive relationship with one’s body image was tested via path analysis. The tested model explained 51% of eating psychopathology’s variance, and revealed a good adjustment to the empirical data. Additionally, social safeness and body appreciation were revealed as mediators of the impact of early warm and safe memories on disordered eating, when controlling for the effect of BMI. These findings suggest the importance of targeting social safeness and acceptance, especially when defining potentially effective programmes for the prevention of eating disorders.

Gastric cancer is one of the most frequent malignant tumors in the world. The majority of patients are diagnosed with metastatic gastric cancer, which has a low survival rate. These data reinforce the importance of studying the anticancer activity of new molecules with the potential to suppress gastric cancer metastasis. Curcumin is a well-studied compound that has demonstrated anti-metastatic effects. Here we investigated if CH-5, a curcumin derivative compound, has anti-metastatic properties in the human gastric cancer cell line HGC-27. Firstly, we found that CH-5 decreased viability and induced apoptosis in HGC-27 cells in a dose-dependent manner. Additionally, CH-5 suppressed the migration and invasion of HGC-27 cells by downregulating the expression and collagenase activity of matrix metalloproteinase 2 in a dose-dependent manner. In conclusion, CH-5 showed anticancer activities, including the induction of apoptosis, and the suppression of migration and invasion in HGC-27 cells, suggesting that CH-5 can be a lead molecule for the development of anti-metastatic drugs for gastric cancer therapy.

Objectives: To evaluate outcomes of oral food challenge (OFC) test to assess tolerance in infants with non-IgE-mediated cow's milk allergy (CMA) with gastrointestinal manifestations and explore clinical data predictive of these outcomes. Methods: Single-center retrospective study including infants (age < 12 months) who were referred for CMA between 2000 and 2018 and underwent OFC on follow-up. A univariate logistic regression test was performed to evaluate variables associated with the outcomes of the follow-up OFC test. Results: Eighty-two patients were included, 50% were male. Eighteen patients had a positive OFC test (22%). Most patients had presented with hematochezia (77%). The median age of symptom onset was 30 days. Two-thirds of the patients were on appropriate infant formula (extensively hydrolyzed or amino acid-based formula), exclusively or in association with breastfeeding. The median time on an elimination diet before the OFC test was 8 months (Q1 6 - Q3 11 months). All cases with positive follow-up OFC tests (n = 18) had been exposed to cow's milkbased formula before the first clinical manifestation of CMA. Five out of eight cases with Food Protein-Induced Enterocolitis Syndrome (FPIES) had positive OFC tests. Exposure to cow's milkbased formula before diagnosis, a history of other food allergies, hematochezia and diarrhea were predictors of a positive OFC test. Conclusions: In infants with non-IgE-mediated CMPA with gastrointestinal manifestations, the use of cow's milk-based formula, a history of other food allergies, and hematochezia and diarrhea upon initial presentation were associated factors for the later achievement of tolerance. We report a severe case of rhinothecal cryptococcoma in a 13-year-old female African Grey Parrot (Psittacus erithacus). The bird was born and bred in captivity in Portugal. The beak was deformed and showed several round soft masses, and microscopic examination revealed the presence of cells suggestive for Cryptococcus. Phenotypic and molecular analyses identified the obtained yeast culture as C. bacillisporus (AFLP5/VGIII). By phylogenetic analysis the parrot strain clustered with clinical C. bacillisporus strains from Mexico.

Some studies have demonstrated the link between the recall of early affiliative experiences and psychopathological symptoms in adolescence. However, literature seems to be scarce in relation to the association between these early memories and adolescent well-being, as well as in relation to the processes and mechanisms that may mediate this relationship. Therefore, the aim of the current study was to explore the association between early affiliative memories and psychological well-being, and whether this relationship is mediated by the self-conscious emotion of shame and by self-critical attitudes. Furthermore, differences in sex were explored. In this cross-sectional study, 310 adolescents from Portuguese schools (136 boys and 174 girls), aged between 13 and 17, completed self-report measures. Path analysis results suggested that, when controlling the effect of age, the recall of early affiliative experiences with family figures had a direct impact on adolescent psychological well-being, and an indirect effect mediated by increased levels of external shame and self-critical attitudes. This model revealed a good fit between the hypothesized mediational model and the data, explaining 43% of the variance of adolescent psychological well-being. These findings highlight the relevance of assessing and working with maladaptive emotion regulation processes and mechanisms, especially in adolescents with a scarce recall of early affiliative experiences with close figures and, additionally, emphasize the importance of the development of prevention and intervention programs focused on compassion and acceptance approaches for adolescents.

Feelings of shame and social comparison focused on physical appearance have been identified as important risk factors for the engagement in disordered eating behaviours. Further, recent studies have emphasized the role of body-image psychological (in)flexibility in the association between several risk factors and eating psychopathology. The current study intended to explore, in two different path models, the effects of external shame, physical appearance-related social comparison, and body image inflexibility on the explanation of eating psychopathology severity. This study follows a cross-sectional design and was conducted in a sample of 776 emerging-adult women, aged between 18 and 28, who completed an online battery of self-report measures. Path analyses were conducted using a structural equation modeling. Model 1’s results showed that external shame and unfavourable social comparison based on physical appearance directly accounts for 26% of the variance in disordered eating. In turn, when body image inflexibility is introduced as a mediator in the relationship presented in Model 1, variance in disordered eating increases to 60% (Model 2). These results suggest that both external shame and unfavourable physical appearance-related social comparison have an impact on disordered eating behaviours. However, when body image inflexibility is introduced, the mentioned relationships change and reveal that psychological inflexibility is a significant mediator of these associations. These findings seem to demonstrate that body image inflexibility is a key process for the engagement in disordered eating, so the promotion of psychological flexibility focused on one’s body is crucial for the prevention and treatment of disordered eating behaviours and attitudes. 18. The roles of self-compassion and psychological flexibility in the psychological well-being of adolescent girls

Research has suggested that the perception of life satisfaction and quality of life tends to decrease during adolescence, and points out that adolescent girls tend to report lower levels of psychological well-being when compared with adolescent boys. Furthermore, studies have highlighted the role of affiliation indicators in the psychological well-being of adolescent girls. However, the study of the mechanisms that mediate this relationship remains scarce. Thus, the current study intends to explore the roles of self-compassion attitudes and psychological flexibility in association between early affiliative memories and current feelings of social safeness and the psychological well-being of girls, through path analysis. The study was conducted with 221 female adolescents, between 12 and 18 years of age, who completed self-report measures. The path model accounted for 22%, 17%, 18% and 52% of the variances of current feelings of social safeness, self-compassion attitudes, psychological flexibility and psychological well-being of adolescent girls, respectively, and revealed a very good fit. Results showed that early affiliative memories and the experience of feeling safe in current social relationships were positively related to psychological well-being of girls through higher levels of self-compassion and psychological flexibility. These findings suggest that not only affiliation indicators are important for the psychological well-being of adolescent girls but also the presence of specific adaptive emotion regulation processes. Objective In accordance to ACT theory, psychological inflexibility may influence the well-known link between learned helplessness and depression symptomatology. This exploratory preliminary study aims to analyse whether psychological flexibility moderates the relationship between these variables. Methods A community sample of 84 Portuguese participants (30 men and 54 women), with a mean age of 33.98 (SD = 11.05), completed the LHS, CompACT, and DASS-21. The moderating effect of psychological flexibility on the relationship between learned helplessness and depression symptomatology was calculated using the PROCESS computation macro. Results The interaction term between learned helplessness and psychological flexibility was significant (b = −0.01, SE = 0.00, p Discussion Higher psychological flexibility seems to be protective for depression symptomatology, in particular for those individuals who experience higher levels of learned helplessness. Individuals who are most likely to struggle with learned helplessness are potentially those who could benefit the most of an intervention targeting psychological flexibility as a way to ameliorate depressive symptomatology. Future studies with larger and clinical samples are required to confirm these preliminary findings.

An extensive body of research has showed the impact of major life events on depressive symptoms. However, the underlying psychological processes that contribute to this association are still unclear. The present study aims to explore the mediating role of external shame and fears of receiving compassion on the associations of the number of major life events and their appraisals by the individual with depressive symptoms. Participants were 400 Portuguese-speaking adults from the general population (121 men and 279 women) with ages between 18 and 65 years, that completed self-report measures. Correlation analysis showed significant and positive associations between the number of major life events that occurred in the previous year and the positive and negative appraisal of such events, external shame, fears of receiving compassion, and depressive symptoms. Path analysis revealed that external shame and fears of receiving compassion fully mediated the effects of number of life events on depressive symptoms. Furthermore, results showed a direct effect of negative appraisal of major life events on depressive symptoms. This study sheds light on the psychological processes that may underlie the relationship between the cumulative number of major life events and depressive symptoms. Specifically, it suggests that the occurrence of major life events can impact on depressive symptoms when associated with feelings of shame and fears of receiving compassion from others.

Recent literature has documented the relationship between fears of compassion and disordered eating attitudes and behaviours. However, research on the processes underlying this association is still in the early stages. As such, this study tested a mediator model where insecure striving and inflexible eating (i.e. psychological inflexibility focused on eating) were hypothesized to mediate the impact of fears of compassion on the severity of disordered eating, while controlling for the effects of body mass index and age. The study’s sample comprised 310 Portuguese women from the general population, aged between 18 and 65 years, who completed an online survey. Path analysis results revealed that the impact of fear of compassion for self and for others on disordered eating was fully mediated by insecure striving and inflexible eating, whereas the impact of fear of compassion from others was only partly mediated by these processes. The tested model accounted for 48% of disordered eating’s variance and presented excellent fit indices. These results suggest that fears of experiencing compassion may preclude the experience of social safeness, hindering the activation of social mentalities other than rank-focused mentality. Maladaptive competitive strategies may ensue (insecure striving), such as inflexible eating, which is linked to the development of disordered eating. In terms of clinical implications, these findings stress the need to effectively assess and address fears of compassion, as they not only seem to be involved in the development of disordered eating, but also have been reported to be significant predictors of poor treatment outcomes. O desenvolvimento e o uso de materiais bioativos que permitem a manutenção dos tecidos dentários são os pilares da abordagem minimamente invasiva. No entanto, a durabilidade da união adesiva a dentina ainda não é completamente satisfatória, principalmente em dentina afetada por cárie (DAC). Assim, o objetivo foi avaliar a influência da incorporação direta do vidro bioativo (BAG) a 10% em um sistema adesivo autocondicionante de dois passos, para remineralização de dentina hígida e DAC. Terceiros molares hígidos foram selecionados e metade dos dentes foi submetido à ciclagem de pH para simular DAC. Os grupos foram divididos da seguinte forma: BAG+DH; BAG+DAC; DH; DAC. Foi avaliado resistência de união, pelo teste de microtração (n=8) após 24 horas e 6 meses de armazenamento. The need for a transnational validation is imperative at the stage of development of the CompACT, a self-report measure of psychological flexibility. This study aimed to translate, validate and test the factor structure of the Portuguese version of the CompACT and to conduct a measurement invariance analysis comparing the scale's performance in Portuguese and UK samples. Results from an Exploratory Factor Analysis demonstrated that the Portuguese version of the CompACT statistically performed better without 5 items from the Openness to Experience subscale. The 18-item Portuguese-adapted CompACT presented significant correlations in the expected directions and with the expected magnitudes with AAQ-II, CFQ-7, MAAS, CAQ-8, and DASS-21. Partial metric invariance was demonstrated between the Portuguese-adapted 18-item CompACT and the original CompACT in a UK sample. The non-correspondence between responses to these versions may be due to differences between the Portuguese and British cultures. This study contributes with the adaptation of the original CompACT to the Portuguese language and with the refinement of this instrument to an 18-item measure of psychological flexibility, that appears to be adequate for use in Portuguese samples. The lack of complete metric invariance of the CompACT found across the Portuguese and UK samples highlights the importance of psychometrically analyzing psychological instruments before use in cultural contexts distinct from the one targeted in the measure's original validation study.

The need to examine the associations between emotion regulation and mental health and well-being among adolescents is increasingly recognized. Adolescence is a critical time characterized by increased vulnerability towards emotional struggles and difficulties, especially related with body image and eating behaviours. Thus, it seems particularly important to analyse the processes and mechanisms underlying the relationships between several risk factors (such as the lack of early affiliative memories) and body and eating-related difficulties, in this specific developmental phase. The current study intended to examine whether early affiliative memories are significantly associated with body and eating-related difficulties (i.e., body image shame and eating psychopathology severity), and whether this association is mediated by lower feelings of social safeness and increased levels of fears of receiving compassion from others. This cross-sectional study was conducted with 231 adolescent girls from the community, aged between 12 and 18, who completed self-report measures. Data were examined through descriptive and correlational statistics, and the adequacy of the model was performed via path analysis. Path analysis results revealed that fears of receiving compassion from others and low feelings of social safeness mediated the relationship between early affiliative memories and body and eating-related difficulties. The path model accounted for the variances in the following way: 27% of feelings of social safeness; 23% of fears of receiving compassion from others; 28% of body-image shame; and 54% of eating psychopathology severity, revealing a very good fit. These findings emphasize the relevance of assessing and working on potential processes underlying the adoption of disordered body and eating attitudes and behaviours, not only but especially in female adolescents with scarce recall of early affiliative experiences with close ones.

O teste de provocação oral é o método padrão ouro para o seu diagnóstico, auxiliando também na determinação da aquisição de tolerância. Como a Secretaria de Saúde do estado de São Paulo fornece gratuitamente fórmulas especiais para crianças com APLV, a acurácia diagnóstica e o manejo adequado desta condição são relevantes para a política de gastos em saúde pública. Objetivos: Avaliar a realização do teste de provocação oral em crianças com APLV em hospital universitário, com foco na sua segurança e efetividade. Calcular o impacto econômico do teste de provocação oral nos gastos de saúde pública, pela redução do uso de fórmulas especiais. Metodologia: Avaliação retrospectiva de 76 crianças com hipótese diagnóstica de APLV submetidas a teste de provocação oral aberto para leite de vaca, em hospital universitário, entre janeiro/2016 e junho/2018. Resultados: Mediana de Idade (em meses) no surgimento dos sintomas: 2,0 [0,8-4,4]. Mediana de idade ao teste (em anos): 2,00 [0,8-5,0]. Meninos: 41/76, meninas: 35/76. Durante dieta de exclusão: uso de fórmula especial: 59/76, 12/76 mantiveram aleitamento materno, 5/76 não utilizaram fórmula especial nem leite materno. Mecanismo de alergia: não IgE mediado 52/76; IgE mediado 20/76, misto: 4/76. Finalidade do teste de provocação: 14/76 para confirmar diagnóstico e 62/76 para avaliar aquisição de tolerância natural. Resultado teste de provocação oral: Negativo em 58/76. Positivo 18/76; destes, uma criança apresentou anafilaxia, tratada imediatamente, sem complicações. Evolução após teste de provocação oral: Para os testes de provocação oral negativos, liberado leite com proteína intacta para 58 crianças. Dentre as crianças que usaram fórmula especial, os testes realizados no período citado, permitiram a economia de R$ 259.337,09 para o SUS-SP. Conclusões: O teste de provocação oral para leite de vaca realizado em ambiente hospitalar universitário mostrou-se seguro e efetivo. The oral food challenge is the gold standard method for the diagnosis of CMA. It also helps to determine the natural acquisition of tolerance. As the São Paulo State Health Service provides special infant formulas for children with CMA, the diagnostic accuracy and proper management of this condition are relevant for public health expenditures. Objectives: To evaluate the performance of oral food challenge in children with CMA in a university hospital, focusing on its safety and effectiveness. To calculate the economic impact of oral food challenge on public health expenditures by reducing the use of special formulas. Methodology: Retrospective analysis of 76 children with a diagnostic hypothesis of CMA, submitted to open oral food challenge for cow's milk at a university hospital, from January 2016 to June 2018. Results: Median Age (in months) at the onset of symptoms: 2.0 [0.8-4.4]. Median age at the test (in years): 2.00 [0.8-5.0]. Boys: 41/76, girls: 35/76. Use of special formula during exclusion diet: 59/76. 12/76 maintained breastfeeding, 5/76 didn¿t use special formula or breast milk. Of the oral food challenges performed, 58/76 were negative and 18/76 were positive. One child had anaphylaxis, receiving treatment, with symptom control. No. of bitstreams: 0 Previous issue date: 2020-12-01 The skin prick test is used to diagnose patients' sensitization to antigens through a mediated IgE response. It is a practical and quick exam, but its diagnosis depends on instruments for measuring the allergic response and observer's interpretation. The conventional method for inferring about the allergic reaction is performed from the dimensions of the wheals, which are measured using a ruler or a caliper. To make this diagnosis less dependent on human interpretation, the present study proposes two alternative methods to infer about the allergic reaction: computational determination of the wheal area and a study of the temperature variation of the patient's skin in the puncture region. For this purpose, prick test using histamine was performed on 20 patients randomly selected. The areas were determined by the conventional method using the dimensions of the wheals measured with a digital caliper 30 min after the puncture. The wheal areas were also determined by a Python algorithm using photographs of the puncture region obtained with a smartphone. A variable named circularity deviation was also determined for each analyzed wheal. The temperature variation was monitored using an infrared temperature sensor, which collected temperature data for 30 min. All results were statistically compared or correlated. The results showed that the computational method to infer the wheal areas did not differ significantly from the areas determined by the conventional method (p-value = 0.07585). Temperature monitoring revealed that there was a consistent temperature increase in the first minutes after the puncture, followed by stabilization, so that the data could be adjusted by a logistic equation (R2 = 0.96). This adjustment showed that the optimal time to measure the temperature is 800 s after the puncture, when the temperature stabilization occurs. The results have also shown that this temperature stabilization has a significant positive correlation with wheal area (p-value = 0.0015). Thus, we concluded that the proposed computational method is more accurate to infer the wheal area when compared to the traditional method, and that the temperature may be used as an alternative parameter to infer about the allergic reaction. Literature has emphasized the significant role of social acceptance and connectedness in well-being and the benefits of cultivating a positive body image in the prevention and treatment of body and eating-related difficulties. The current study aims to examine whether strategies of self-reassurance and body-image appreciation mediate the association of feelings of social safeness and acceptance with the “core” dimensions of body and eating-related psychopathology (restraint, eating concern, weight concern, and shape concern), while controlling the effects of body mass index (BMI). Participants were 309 Portuguese women, aged between 18 and 50 years that completed self-report measures. Results from the performed path analysis revealed that self-reassurance and body-image appreciation mediated the impact of feelings of social safeness and acceptance on eating psychopathology-related dimensions. Results seem to suggest that women who perceive the self as acceptable and desirable tend to adopt self-reassurance strategies. These strategies in turn seem to predict a protective and caring relationship with one’s own body and decreased severity of eating psychopathology symptoms. This path model explained 22%, 27%, 51%, and 47% of restraint, eating concern, weight concern, and shape concern, respectively, and showed an excellent model fit. Our findings appear to offer significant insights in the field of body and eating-related psychopathology and seem to support the pertinence of creating intervention programs for women from the general community with body and eating-related difficulties that encourage the adoption of self-reassurance strategies and promote a positive and respectful relationship with one’s own body image. Feelings of social safeness and connectedness have been associated with adaptive emotion regulation processes and well-being indicators. Further, literature has demonstrated that interpersonal experiences play an important role in the etiology and maintenance of body and eating psychopathology. However, the study of the role of social variables and emotion regulation processes in the engagement in inflexible eating rules and eating psychopathology is still in its early stages. The current study aims to fill some gaps within the literature and explore the mediator role of body appreciation and inflexible eating rules in the link between social safeness and disordered eating. Participants were 253 women, aged between 18 and 50 years old, who completed a series of online self-report measures. Results from the tested path analysis model showed that social safeness holds a significant effect on eating psychopathology, through the mechanisms of body appreciation and inflexible eating rules. Also, results suggested that women who present higher levels of social safeness tend to present a more positive and respectful attitude towards their body and decreased adoption of inflexible eating rules, which seem to explain lower levels of disordered eating behaviours. These findings seem to present empirical support for the development of intervention programs that promote a positive, affectionate and healthy relationship with one's body image, in order to prevent the inflexible adherence to eating rules and disordered eating behaviours.

Shame has been for long associated with the development and maintenance of body image and eating-related difficulties. However, the mechanisms underlying this association remain unclear. Therefore, the current study sought to examine the mechanisms of self-judgment and fears of receiving compassion from others in the association between external shame and disordered eating, while controlling for body mass index (BMI). Participants in this study were 400 women from the general population, aged between 18 and 55 years old. Correlation analyses revealed significant and positive relationships between external shame, self-judgment, fears of receiving compassion from others and eating psychopathology. A path analysis confirmed that, when controlling for the effect of BMI, external shame has a direct effect on disordered eating severity, and also an indirect effect, mediated by higher levels of self-judgment and increased fears of receiving others' kindness and compassion. Results showed the plausibility of the tested model which explained 36% of the variance of disordered eating. These findings seem to support that women who perceive that others view them negatively tend to be defensive and engage in maladaptive emotion regulation strategies (such as harsh critical attitudes towards the self and being resistant to others' compassion), which may trigger maladaptive eating attitudes and behaviours. The current research appears to be an innovative study in the field of body image and eating-related psychopathology and seems to represent a new avenue for future research and for the development of intervention programs.

Adolescence is considered a transitional stage characterized by several physical, psychological and social changes. During this period, there is an increased propensity for the emergence of emotional difficulties, especially those related to body image and eating attitudes and behaviours. In particular, shame has been pointed out as a key risk factor for body image and eating-related difficulties. However, research seems to be scarce regarding this association in adolescent samples. Therefore, the current study aimed to explore whether psychological inflexibility impacts the association between general feelings of shame with body image-related shame and eating psychopathology severity. The study was conducted with 226 adolescent girls from the community, aged between 12 and 18, who completed self-report measures. Path analysis indicated that higher levels of external shame were associated with more externalized and internalized body image shame and also with greater engagement in disordered eating behaviours. Furthermore, the model demonstrated that these relationships were mediated by increased levels of psychological inflexibility. The tested model accounted for a total of 30% and 22% of externalized and internalized body image variances, respectively, and 51% of the variance of disordered eating behaviours. These findings appear to suggest that in adolescent girls, body shame and disordered eating may emerge as defensive and maladaptive strategies to cope with general feelings of inferiority, particularly in the presence of psychological inflexibility. Also, this study highlights the importance of assessing and working underlying maladaptive processes in this association, mainly in adolescents with high levels of shame.

Studies on body image-related disturbances have recently embraced a fresh and innovative construct: body appreciation. Body appreciation, an aspect of positive body image, defines as the detention of a balanced, affectionate and health-conscious relationship with one's own body's features. Its exploration is considered to be essential to the success of upcoming prevention and intervention programs in the area of body image and eating disorders. The role of shame, although widely studied in negative body image and eating psychopathology, is yet unknown regarding body appreciation. In this line, a main goal of this study was to explore the effect of external shame on body appreciation in a sample of Portuguese young adults. Also, taking into account the similarities between body appreciation and self-compassion, and since self-compassion is known as a powerful tool against the impact of shame on body image-related disturbances, this study also intended to test the role of self-compassion in the relationship between external shame and body appreciation. Results revealed body mass index and external shame as negative correlates, and self-compassion as a positive correlate of body appreciation. Also, path analysis showed that external shame holds a significant effect on body appreciation, both directly and indirectly. Specifically, this analysis revealed that self-compassion acts as a mediator between shame and body appreciation. Present findings seem to support the pertinence of programs to promote an affectionate and healthy relationship with one's own body image, which may benefit from the target of shame and the inclusion of self-compassion practices.

Early affiliative experiences play an important role in social and emotional development. Several authors have suggested that early positive experiences may be recorded as memories of warmth and soothing. However, the relationship between such memories and current feelings of social safeness and connectedness remains scarcely studied. The current study examined the association between recall of early positive affiliative experiences (with family and peers) and women's social safeness. External shame and fear of others' compassion were hypothesized to be mediators in these associations. The sample was recruited from October 2016 to February 2016 and included 400 women from the Portuguese population, aged between 18 and 55 years. The path model explained 52% of the variance of social safeness. Early positive affiliative memories (with family and peers) were associated with a lower tendency to fear receiving others' compassion and to feel ashamed, which seemed to be associated with an increased sense of social safeness. These findings offer pertinent insights for future studies in and interventions for women´s well-being and mental health by highlighting the importance of addressing shame and fears of compassion when working with women who tend to perceive their social world as unsafe and others' compassion as threatening.

RESUMO Objetivo: Descrever um caso de sífilis congênita com diagnóstico tardio e identificar as oportunidades perdidas nas diversas fases/níveis da atenção à saúde, que retardaram a realização do diagnóstico. Descrição do caso: Menino, 34 dias de vida, encaminhado da Unidade Básica de Saúde a um hospital terciário por apresentar aumento do volume abdominal e icterícia progressiva há 2 semanas, hipocolia fecal, hepatoesplenomegalia, anemia, plaquetopenia e elevação de enzimas hepáticas. Ao exame físico, apresentava lesões eritemato-descamativas nas mãos e nos pés e exantema macular em região inguinal, presença de ascite, fígado palpável a 5 cm do rebordo costal direito e baço palpável a 3 cm do rebordo costal esquerdo. Sorologia do lactente: CMIA (quimioluminescência de micropartículas) reagente, VDRL (Venereal Diseases Research Laboratory) 1:1024 e TPHA (Treponema pallidum Hemaglutination) reagente. Sorologia materna: CMIA e TPHA reagentes, VDRL 1:256. Radiografia de ossos longos mostrava periostite simétrica; levantamento periosteal; e bandas metafisárias lucentes em fêmures, úmeros, ulnas e tíbias. Após tratamento com penicilina cristalina, apresentou melhora clínica e laboratorial, recebendo alta no 18º dia de internação. Comentários: Este caso mostra que ainda ocorre diagnóstico tardio de sífilis congênita por falhas nas estratégias de prevenção dessa doença, tanto na atenção básica quanto nos níveis secundário e terciário. A aplicação das intervenções preconizadas pelo Ministério da Saúde e a identificação das situações em que ocorrem falhas na sua execução são importantes para a avaliação da assistência de rotina em todos os níveis de atenção e nas diversas unidades responsáveis pelo cuidado do recém-nascido e do lactente jovem. At physical examination, the infant presented with erythematous-exfoliative lesions on the palms and soles, macular rash in the inguinal region, ascitis, palpable liver 5 cm below the right costal margin and a palpable spleen 3 cm from the left costal margin. Infant serology: reactive CMIA (chemiluminescent microparticle immunoassay), VDRL (Venereal Diseases Research Laboratory) 1:1024 and reactive TPHA (Treponema pallidum Hemagglutination). Maternal serology: reactive CMIA and TPHA, VDRL 1:256. Radiography of the long bones showed symmetric periostitis, periosteal thickening, and lucent bands in the femur, humerus, ulna and tibia. After treatment with crystalline penicillin, the infant showed clinical and laboratory improvement, receiving hospital discharge at the 18th hospitalization day. Comments: This case shows that congenital syphilis is occasionally diagnosed late as a result of failed strategies to prevent this disease, both in the basic and secondary/tertiary levels of care. The application of interventions recommended by the Ministry of Health and identification of the situation in which there is ineffective implementation of these measures are important to assess routine care in all levels of healthcare and diverse units responsible for newborn and infant health care.

The relationship between self-compassion and well-being and health (e.g. a lower proneness for eating-related disturbances) is well stressed in the literature. However, the specific contribution of self-compassionate attributes, actions, and body compassion remains scarcely studied. The main aim of the present study was to examine whether the link between self-compassionate attributes and disordered eating attitudes and behaviours is mediated by self-compassionate actions and body compassion, in a sample of 299 Portuguese women from the general population. The tested model explained 44% of eating psychopathology's variance and presented excellent fit indices. The most interesting contribution of this study was the suggestion that the ability to act in accordance with self-compassionate attributes is associated with higher levels of body compassion, that is, an attitude of appreciation, acceptance, warmth toward body-related thoughts, perceptions and feelings, which reflects in a lower susceptibility to adopt disordered eating attitudes and behaviours. These results seem to offer an important contribution for research and clinical practice by supporting the importance of including strategies to develop self-compassionate skills and body compassion competencies in prevention and treatment programs in the area of eating psychopathology.Level of evidence Level III, evidence obtained from a well-designed cohort.

The majority of patients are diagnosed with metastatic gastric cancer, which has a low survival rate. These data reinforce the importance of studying the anticancer activity of new molecules with the potential to suppress gastric cancer metastasis. Curcumin is a well-studied compound that has demonstrated anti-metastatic effects. Here we investigated if CH-5, a curcumin derivative compound, has anti-metastatic properties in the human gastric cancer cell line HGC-27. Firstly, we found that CH-5 decreased viability and induced apoptosis in HGC-27 cells in a dose-dependent manner. Additionally, CH-5 suppressed the migration and invasion of HGC-27 cells by downregulating the expression and collagenase activity of matrix metalloproteinase 2 in a dose-dependent manner. In conclusion, CH-5 showed anticancer activities, including the induction of apoptosis, and the suppression of migration and invasion in HGC-27 cells, suggesting that CH-5 can be a lead molecule for the development of anti-metastatic drugs for gastric cancer therapy.
