En los últimos años se han producido importantes avances tecnológicos, y en especial en el mundo de las tecnologías de la información y la comunicación (TIC), que están transformando a pasos acelerados las distintas profesiones vinculadas con la intervención social. Partimos de la hipótesis de que los trabajadores sociales presentan un alto grado de aceptación tecnológica porque detectan importantes ventajas en el uso de las tecnologías de teletrabajo para su práctica profesional. Por ello se propuso como objetivo principal: Comprobar el grado de aceptación tecnológica de los trabajadores sociales a nivel internacional. Y un objetivo secundario: analizar cómo se establece la vinculación entre la práctica profesional y el uso de las nuevas posibilidades tecnológicas en los trabajadores sociales. A nivel metodológico se aplicó un cuestionario especializado de 27 variables (Alfa de Cronbach α = .950) y la escala Technology Acceptance Model (TAM), a una muestra de 1144 trabajadores sociales de 13 países. A nivel conclusivo, se detecta un alto grado de aceptación tecnológica; una identificación clara entre la práctica profesional, el uso de los avances tecnológicos, y su conexión con los estándares propuestos por la NASW; y una identificación clara de las principales ventajas del uso de estas tecnologías para el Trabajo Social. Todos estos elementos guardan una estrecha relación con la extensión del Teletrabajo Social, y con el e-Social Work

Introduction: Metanephric adenoma (MA) is an uncommon benign tumor accounting for 0.2–0.7% of adult renal epithelial neoplasms. The clinical course is often indolent, but diagnosis should not be delayed since clinical symptoms (hematuria, fever, palpable abdominal mass, and flank pain) may be non-specific and overlap with those of a malign renal neoplasm. We report on 4 cases of AM, for which morphological and mutational analysis were performed. Material and Methods: Immunohistochemical staining was performed on sections cut from paraffin blocks to assess expression of WT1, vimentin, racemase, CK7, CD10 and RCC. Testing for the BRAF gene mutation V600 was carried out using real-time PCR (Cobas® 4800). Results: In all four cases, tumors were visible as well-circumscribed, non encapsulated masses located in the renal cortex and extending towards the medulla. At immunohistochemical examination, tumor cells stained negative for CK7, CD10 and RCC and positive for both WT1 (nuclear, intense) and vimentin (cytoplasmic, intense, and diffuse). Molecular analysis revealed the BRAF gene mutation V600E in three cases and wild-type BRAF in the fourth. Conclusions: BRAF molecular mutation analysis may aid diagnosis in cases with atypical histological features, especially in small incisional biopsies when reassessment of surgical treatment may be considered.

Numerous cells with very large and irregular nuclei (“monster” cells) have not hitherto been reported in desmoplastic melanoma (DM). Their prognostic significance in melanomas is a matter of debate, although some authors have associated them with more aggressive tumor behavior. We report a mixed DM on the scalp of an 88-year-old woman imitating an atypical fibroxanthoma. Tumor cells stained positive for SOX10, S100, and cyclin D1; BRAF mutation status was negative, and fluorescence in situ hybridization analysis showed copy number gains in 11q13 (cyclin D1) and 6p25 (RREB1), and loss in 6q23 (MYB). Cyclin D1 amplification is associated with poor prognosis in melanoma.

Purpose The mitotic count (MC), number of mitosis per unit area, is a very important parameter frequently used for classification and grading of some tumors. Traditionally, the MC has been expressed in terms of number of mitoses per high power field. The size of the field of view can vary greatly among different microscopes. In order to avoid under or overestimation of mitotic count, a conversion needs to be made. Methods A simple formula based on a simple rule of three has been devised to standardize the mitotic count to the reference area by multiplying the number of mitotic figures by a correction factor which has been calculated for the most frequently used microscopes and various common tumors. Results and conclusions We propose this simple method, which involves only a single multiplication, to standardize the mitotic count to the reference area.

Sedation is necessary in the management of critically ill patients, both to alleviate suffering and to cure patients with diseases that require admission to the intensive care unit. Such sedation should be appropriate to the patient needs at each timepoint during clinical evolution, and neither too low (undersedation) nor too high (oversedation). Adequate sedation influences patient comfort, safety, survival, subsequent quality of life, bed rotation of critical care units and costs. Undersedation is detected and quickly corrected. In contrast, oversedation is silent and difficult to prevent in the absence of management guidelines, collective awareness and teamwork. The Zero Oversedation Project of the Sedation, Analgesia and Delirium Working Group of the Spanish Society of Intensive and Critical Care Medicine and Coronary Units aims to offer a practical teaching and collective awareness tool for ensuring patient comfort, safety and management with a view to optimizing the clinical outcomes and minimizing the deleterious effects of excessive sedation. The tool is based on a package of measures that include monitoring pain, analgesia, agitation, sedation, delirium and neuromuscular block, keeping patients pain-free, performing dynamic sedation according to clinical objectives, agreeing upon the multidisciplinary protocol to be followed, and avoiding deep sedation where not clinically indicated.

Background: Pain continues to be an underdiagnosed problem. Objective tools are needed for its assessment. The objective of this study was to determine the diagnostic performance, validity and reliability of the pupillary dilation reflex (PDR) against the Behavioural Pain Scale (BPS) to assess pain in patients under light-moderate sedation.Methods: A study of diagnostic tests using PDR versus BPS as a reference test was performed. The patients were recruited from the Intensive Care Units of the Araba University Hospital and were consecutively admitted. They were older than 18 years, under intravenous analgosedation, mechanically ventilated and had a BPS score of three and a Richmond Agitation and Sedation Scale (RASS) score between -1 and -4. The responses to a non-painful (NP) stimulus, 10 mA, 20 mA, 30 mA and 40 mA stimuli, and was assessed with the BPS and PDR. PDR measurements were performed with an Algican® pupilometer. Pain was considered to be present at BPS≥4. The receiver operating curve (ROC) was plotted, and the area under the curve (AUC) was calculated. We identified the cut-off points showing the highest sensitivity and specificity. Diagnostic performance was studied based on the Youden index, negative predictive value (NPV), positive predictive value (PPV), accuracy, positive likelihood ratio (PLR), and negative likelihood ratio (NPC) of each of them. They are presented with their 95% confidence intervals (CI). Results: Thirty-one patients were included and 183 measurements were performed. 49 (27%) measurements showed a painful response according to the BPS. We obtained an AUC of 0.885 (95% CI 0.830-0.940). The PDR value with the best diagnostic efficacy was 11.5%, which had a sensitivity of 89.8% (95% CI 78.2-95.6) and a specificity of 78.4% (95% CI 70.6-84.5) with an accuracy of 81.4 (75.2-86.4). The agreement between BPS and PDR had a kappa index of 0.6. Conclusions: Pupillometry could be a valid alternative for identifying pain in analgosedated critical patients. Dermatofibrosarcoma protuberans is a low-grade sarcoma typically originating in the dermis but with local invasion of subcutaneous cell and muscle tissue. We report a case of perianal dermatofibrosarcoma protuberans in a 41-year-old male complaining of anal pain and constipation. To date, only two cases of perianal dermatofibrosarcoma protuberans have been reported. The unusual location hinders surgical treatment. The characteristic translocation t(17;22)(q22;q13) leading to the formation of COL1A1/PDGFβ fusion transcripts is not only of diagnostic value but also enables an alternative imatinib-based therapy in inoperable or metastatic cases. The pertinent literature is reviewed.

One of the most severe effects of coronavirus disease 2019 (COVID-19) is lung disorders such as acute respiratory distress syndrome. In the absence of effective treatments, it is necessary to search for new therapies and therapeutic targets. Platelets play a fundamental role in respiratory disorders resulting from viral infections, being the first line of defense against viruses and essential in maintaining lung function. The direct application of platelet lysate (PL) obtained from the platelet-rich plasma of healthy donors could help in the improvement of the patient due its anti-inflammatory, immunomodulatory, antifibrotic, and repairing effects. This work evaluates PL nebulization by analyzing its levels of growth factors and its biological activity on lung fibroblast cell cultures, besides describing a scientific basis for its use in this kind of pathology. The data of the work suggest that the molecular levels and biological activity of the PL are maintained after nebulization. Airway administration would allow acting directly on the lung tissue modulating inflammation and stimulating reparative processes on key structures such as the alveolocapillary barrier, improving the disease and sequels. The protocol developed in this work is a first step for the study of nebulized PL both in animal experimentation and in clinical trials.

Introduction: Commonly accepted standardized sets of outcomes to be measured in clinical trials are still limited. In accordance, outcome measures vary widely between clinical trials even within defined hematological malignancy (HM) entities. Definition of a core outcome set (COS), which represents a standardized agreed set of outcomes that should be measured and reported in all trials for the respective disease of interest may improve this situation. Furthermore, respective COS should address the need of all stakeholders, i.e. not only the views of physicians and industry running the trials, but also the interests of patients and regulators. To perform a task like this, HARMONY - Healthcare Alliance for Resourceful Medicine Offensive against Neoplasms in Hematology - a private-public partnership established in January 2017, including 53 partners and 43 associated partners in 18 different European countries and also 6 patient organizations, poses an optimal platform to define COS for HMs. Methods: To define COS for 5 selected HMs, it was decided to use the Delphi survey method provided by COMET initiative. Since HARMONY has members of several important stakeholder groups (clinicians, industry, health authorities and patient groups) it was decided to include all stakeholders to participate in the Delphi surveys. A pilot study was implemented for the COS for acute myeloid leukemia (AML) based on which additional Delphi surveys were developed for myelodysplastic syndrome (MDS), non-Hodgkin lymphoma (NHL), multiple myeloma (MM), and chronic lymphocytic leukemia (CLL). As starting point preliminary outcome lists were generated based on published reports and available guidelines. Surveys were performed with representatives from each stakeholder group to agree within a pre-defined and iterative process on a COS for each HM. Conditions and criteria were defined in study protocols. Each outcome was rated into 3 categories (1-3 "not important", 4-6 "important but not critical" and 7-9 "critical"). A "consensus-in criterion" was defined if 70 % or more respondents scored the outcome as critically important (7-9) and 15% or fewer rated the outcome as limited important (1-3). To make sure that the patients' voice was heard within the consensus process, a special "patient-important criterion" had been implemented during data analysis. Outcomes ranked with a median of 7 or higher in the patient group were highlighted, showing these are really important for patients. According to a bottom-up-approach, an overarching COS was then created based on the individual survey results. Results: For the Delphi surveys a total of 365 individuals participated including 126 clinicians (35%), 46 EFPIA members (13%), 177 patients/patient advocates (48%) and 16 regulators (4%). While there was a large overlap of outcomes among HMs, there were also many disease specific outcomes such as leukemia-free survival (LFS) for AML, very good partial response (VGPR) for MM to name only few. In addition, there were sometimes major differences in the assessment of individual stakeholders within an outcome, e.g. between clinicians and patients. Finally, the general COS applicable to all HMs included core outcomes that met the consensus-in criterion for all HMs. Conclusion: Using Delphi surveys to define specific COS for HMs revealed meaningful results. Based on the bottom-up-approach not only disease specific HM COS could be defined, but also an overarching COS applicable to all HMs. This overarching COS will subsequently not only allow to compare results more easily within a distinct HM subgroup but also results across different HMs. To our knowledge, this is the first multidisciplinary approach to define COS across four different stakeholder groups. These COS should now be a starting point to further refine COS and to apply them within future clinical trials, thereby reducing inconsistencies and bias in outcome-reporting. Results of COS based clinical trials will simplify the development of novel clinical recommendations, which will improve future patient management and clinical patient care in the real-world setting. Nowadays they are a weight amount of mourning and grief studies and definitions, most of them related to death and focused in family and friends of the deceased. However just a few authors have research and define the mourning process the professionals (nurses, social workers, psychologist, …) who has worked directly with the deceased, have to get throw. That kind of mourning has been calling disenfranchised grief. With the aim to study that phenomenon, we have explored the burnout symptom and disenfranchised grief among 90 nursing home workers in Spain, using a questionnaire composed by The Maslach Burnout Inventory; Grief Support in Health Care Scale; Disenfranchised Grief making an univariate, bivariate, and multivariate data analyses were conducted using SPSS statistical software version 22. Having as a result the association between these three scales and some demographic variables.

Background: To date, 750 000 patients with COVID-19 worldwide have required mechanical ventilation and thus are at high risk of acute brain dysfunction (coma and delirium). We aimed to investigate the prevalence of delirium and coma, and risk factors for delirium in critically ill patients with COVID-19, to aid the development of strategies to mitigate delirium and associated sequelae. Methods: This multicentre cohort study included 69 adult intensive care units (ICUs), across 14 countries. We included all patients (aged ≥18 years) admitted to participating ICUs with severe acute respiratory syndrome coronavirus 2 infection before April 28, 2020. Patients who were moribund or had life-support measures withdrawn within 24 h of ICU admission, prisoners, patients with pre-existing mental illness, neurodegenerative disorders, congenital or acquired brain damage, hepatic coma, drug overdose, suicide attempt, or those who were blind or deaf were excluded. We collected de-identified data from electronic health records on patient demographics, delirium and coma assessments, and management strategies for a 21-day period. Additional data on ventilator support, ICU length of stay, and vital status was collected for a 28-day period. The primary outcome was to determine the prevalence of delirium and coma and to investigate any associated risk factors associated with development of delirium the next day. We also investigated predictors of number of days alive without delirium or coma. These outcomes were investigated using multivariable regression. Findings: Between Jan 20 and April 28, 2020, 4530 patients with COVID-19 were admitted to 69 ICUs, of whom 2088 patients were included in the study cohort. The median age of patients was 64 years (IQR 54 to 71) with a median Simplified Acute Physiology Score (SAPS) II of 40·0 (30·0 to 53·0). 1397 (66·9%) of 2088 patients were invasively mechanically ventilated on the day of ICU admission and 1827 (87·5%) were invasively mechanical ventilated at some point during hospitalisation. Infusion with sedatives while on mechanical ventilation was common: 1337 (64·0%) of 2088 patients were given benzodiazepines for a median of 7·0 days (4·0 to 12·0) and 1481 (70·9%) were given propofol for a median of 7·0 days (4·0 to 11·0). Median Richmond Agitation–Sedation Scale score while on invasive mechanical ventilation was –4 (–5 to –3). 1704 (81·6%) of 2088 patients were comatose for a median of 10·0 days (6·0 to 15·0) and 1147 (54·9%) were delirious for a median of 3·0 days (2·0 to 6·0). Mechanical ventilation, use of restraints, and benzodiazepine, opioid, and vasopressor infusions, and antipsychotics were each associated with a higher risk of delirium the next day (all p≤0·04), whereas family visitation (in person or virtual) was associated with a lower risk of delirium (p

This chapter talk about the management of sexual diversity in senior citizen centres. It is centred on elderly’s opinion about homosexuality and its goal to see if homophobia exists in senior citizen’s centres between the residents. It is a part of an extender research about homophobia. We have decided to use the quantitative technique of the questionnaire, it has been created with Homophobia Scale (HS) [1], developed at the University of Georgia and some demographic variables, used in a sample of 23 nursey home residents as our research method. We have make the data correlations using the SPSS program, concluding the existence of homophobia in nursing homes.

The aim of this chapter is to introduce biological and empirical bases for integrating immune mechanisms in the study of how prenatal maternal stress and anxiety may shape child health outcomes. We begin with a general discussion of immune mechanisms and what may be particular about them in the perinatal period for the mother. We then briefly review how immune mechanisms have been considered in the research on prenatal stress and child development, with a particular focus on the developmental programming hypothesis. The limited but growing literature on prenatal maternal stress and anxiety on child immune and immune-related health outcomes is considered and critiqued for what they imply about mechanisms and for how these findings pose new opportunities and challenges for understanding other child outcomes, such as neurodevelopment and metabolic functioning. Throughout, we attend to methodological considerations and highlight some of the more important unresolved matters that require further study.

Purpose Community transmission of SARS-CoV-2 was detected in Spain in February 2020, with 216% intensive care unit (ICU) capacity expanded in Vitoria by March 18th, 2020. Methods We identified patients from the two public hospitals in Vitoria who were admitted to ICU with confirmed infection by SARS-CoV-2. Data reported here were available in April 6th, 2020. Mortality was assessed in those who completed 15-days of ICU stay. Results We identified 48 patients (27 males) with confirmed SARS-CoV-2. Median [interquartile range (IQR)] age of patients was 63 [51–75] years. Symptoms began a median of 7 [5–12] days before ICU admission. The most common comorbidities identified were obesity (48%), arterial hypertension (44%) and chronic lung disease (37%). All patients were admitted by hypoxemic respiratory failure and none received non-invasive mechanical ventilation. Forty-five (94%) underwent intubation, 3 (6%) high flow nasal therapy (HFNT), 1 (2%) extracorporeal membrane oxygenation (ECMO) and 22 (46%) required prone position. After 15 days, 14/45 (31%) intubated patients died (13% within one week), 10/45 (22%) were extubated, and 21/45 (47%) underwent mechanical ventilation. Six patients had documented super-infection. Procalcitonin plasma above 0.5 μg/L was associated with 16% vs. 19% (p = 0.78) risk of death after 7 days. Conclusion This early experience with SARS-CoV-2 in Spain suggests that a strategy of right oxygenation avoiding non-invasive mechanical ventilation was life-saving. Seven-day mortality in SARS-CoV-2 requiring intubation was lower than 15%, with 80% of patients still requiring mechanical ventilation. After 15 days of ICU admission, half of patients remained intubated, whereas one third died.

The study was motivated by the need to understand the high number of deaths caused by COVID-19 in the global pandemic declared since December 2019, and how it impacted differently in European countries. The hypothesis was that less investment in the public health system, the number of doctors per inhabitant and the number of hospital beds available to the population led to a higher number of deaths after the arrival of COVID-19 in each country studied. The objective was to analyze the relationship between the number of deaths from COVID-19 in the global pandemic declared since December 2019 and health policies and investment in European countries.A research study was conducted in which a total of six variables were analyzed with official and contrasted data: public health expenditure per capita; doctors per 1,000 inhabitants; number of beds per 1,000 people; deaths from COVID-19 per million inhabitants; number of tests to detect COVID-19 per 1,000 inhabitants; and GINI Coefficient to measure the degree of social inequality in each country. It was carried out in 30 European countries. Frequency and correlation analyses were carried out (Pearson).Five countries were found, which gave values above 300 deaths per million (data from April 27, 2020): United Kingdom; (305.39), France (350.16), Italy (440.67), Spain (495.99) and Belgium (612.1). Precisely, in the countries that recorded the most deaths (United Kingdom, France, Italy, Spain and Belgium) on April 27, we did not find high values of TEST performance. In our analysis, we found that the lower the investment of public spending in health (per capita), the higher the number of deaths per COVID-19 per million inhabitants, the lower the coverage of hospital beds, and the lower the number of doctors. Finally, we found that the lower the expenditure on public health, the higher the GINI coefficient (thus greater social inequality).A negative effect in terms of deaths was detected when investment in public health was lower; the higher number of deaths from COVID-19 was correlated (p0.005) with greater social inequality (GINI coefficient) and with lower investment in public health (p0.001); this had an impact on the lower number of available beds and low physician coverage per 1,000 inhabitants.El estudio se fundamentó en la necesidad de entender el elevado número de fallecimientos por COVID-19 en la pandemia mundial declarada desde Diciembre de 2019, y cómo golpeó de forma distinta en los países de Europa. La hipótesis planteada fue que una menor inversión en el sistema de sanidad pública, el número de médicos por habitante y el número de camas hospitalarias disponibles para la población provocaron un mayor número de fallecidos tras la llegada de la COVID-19 a cada país estudiado. El objetivo fue analizar la relación entre el número de fallecimientos por COVID-19 en la pandemia mundial declarada desde diciembre de 2019 y las políticas e inversión sanitarias en los países de Europa.Se realizó un estudio de investigación en el que se analizaron un total de seis variables con datos oficiales y contrastados: gasto público en salud per cápita; médicos por cada 1.000 habitantes; número de camas por cada 1.000 personas; muertes por COVID-19 por cada millón de habitantes; número de test para detectar COVID-19 por cada 1.000 habitantes; y Coeficiente GINI para medir el grado de desigualdad social en cada país. Precisamente, en los países que más muertes registraron (Reino Unido, Francia, Italia, España y Bélgica) a fecha de 27 de abril, no encontramos valores elevados de realización de TEST. En nuestros análisis, obtuvimos que a menos inversión de gasto público en salud (per cápita), se daba un mayor número de muertes por COVID-19 por cada millón de habitantes, una menor cobertura en camas hospitalarias, y un menor número de doctores. Finalmente, comprobamos que a menor fue el gasto en salud pública, más alto era el coeficiente GINI (por tanto mayor desigualdad social).Se detecta un efecto negativo en término de muertes cuando la inversión en salud pública es menor. El mayor número de muertes por COVID-19 está correlacionado (p0,005) con una mayor desigualdad social (coeficiente GINI) y con una menor inversión en salud pública (p0,001). Esto ha incidido en el menor número de camas disponibles y una baja cobertura de médicos por cada 1.000 habitantes.

Laboratory corrosion and scaling testing of metallic materials exposed in high temperature and pressure environments generally involves complex, multi-instrument measurement setups. Here, we present a setup including an autoclave that is instrumented for in-situ electrochemical testing and that contains a ZrO2-based solid-state pH electrode and devices for temperature control and solution stirring. We show results highlighting the importance of adequate pre-calibration of the pH measurement, due to the hysteresis depending on temperature sweep. Additionally, we illustrate how interfacing the autoclave and the electrochemical cell to measuring and controlling instruments, using different data communication interfaces, can create ground loops. These ground loop interferences can introduce significant errors in the measurement, such as a potential shift of >100 mV. In complex, multi-instrument setups, a complete understanding of ground loops may often be difficult. Thus, we recommend systematic checks to identify the ground loops and we propose measures to avoid them. ISSN:0263-2241 ISSN:1873-412X

Study Objectives: Sleep disturbances are commonly reported by cancer survivors. Systematic light exposure using bright light has been used to improve sleep in other populations. In this secondary data analysis, the effect of morning administration of bright light on sleep and sleep quality was examined in a mixed group of fatigued cancer survivors. Methods: Forty-four cancer survivors screened for cancer-related fatigue were randomized to either a bright white light or a comparison dim red light condition. Participants were instructed to use a light box every morning for 30 minutes for 4 weeks. Wrist actigraphy and the Pittsburgh Sleep Quality Index were administered at 4 time points: prior to light treatment (baseline), 2 weeks into the intervention, during the last week of the intervention, and 3 weeks postintervention. Thirty-seven participants completed the end-of-intervention assessment. Results: Repeated-measures linear mixed models indicated a statistically significant time × treatment group interaction effect with sleep efficiency improving more in the bright light condition over time compared with the dim light condition (F 3, 42 = 5.55; P = .003) with a large effect size (partial η 2 = 0.28). By the end of the intervention and 3 weeks postintervention, mean sleep efficiency in the bright light group was in the normal range. Medium to large effect sizes were also seen in sleep quality, total sleep time, and wake after sleep onset for participants favoring the bright light condition. Conclusions: The results suggest that systematic bright light exposure in the morning may have beneficial effects on sleep in fatigued cancer survivors. Larger scale efficacy trials are warranted.
