Abstract

Bu yazıda antenatal hidronefroz nedeniyle takip edilen ve pyeloplastiden sonra nöroblastom tanısı alan 8 aylık bir erkek çocuğun klinik özellikleri bildirilmiştir. Postnatal 3. ayda gerçekleştirilen sol pyeloplastinin erken postoperatif döneminde belirgin hemoglobin, hematokrit düşüşü olması nedeniyle çekilen tomografide tümör veya kolleksiyon lehine bulgu izlenmedi. Fakat postoperatif 4. aydaki kontrol ultrasonografisinde sol sürrenalde kitle görüldü. Kan ve idrar testleri nöroblastom lehine sonuçlandı. Bunların üzerine yapılan eksplorasyonda sol sürrenaldeki kitle eksize edildi ve iyi histopatolojik özelliklere sahip, evre 1 nöroblastom tanısı konmuş oldu. Bu vakada olduğu gibi insidental olarak veya tarama programları ile tanısı konan düşük riskli tümörler sadece cerrahi müdahale ile tedavi edilebilir

Abstract

Çocukluk çağında akut üriner retansiyon oldukça ender görülen bir durumdur. Bu çalışmada, mesane divertikülüne bağlı olarak akut üriner retansiyon gelişen olgunun klinik özellikleri değerlendirilmiştir. Antenatal ultrasonografide iki taraflı hidroüreteronefroz saptanan fetusta, ektazinin ilerleyen haftalarda değişmediği görüldü. Postnatal dönemde tekrarlayan ultrasonografiler iki taraflı derece 4 hidroüreteronefrozun devam ettiğini gösterdi. İşeme sistouretrografisinde, iki taraflı derece 5 vezikoüreteral reflü saptaması üzerine, antibiyotik profilaksisi başlandı. Statik böbrek sintigrafisinde sağ ve sol böbreğin göreceli işlevleri sırası ile %21 ve %79 olarak tespit edildi. Ayrıca sintigrafik olarak skar dokusu izlenmedi. Profilaksinin 7. ayında, hasta idrar yapamama yakınması ile acil polikliniğe başvurdu. Kateterize edilen hastanın ultrasonografisinde mesane tabanını kaplayan ve tabandan posteroinferiora uzanım göstererek üretraya bası yapan iki adet divertikül izlendi. Miksiyonel sistoüretrografide mesane tabanının sol tarafında daha büyük olduğu görülen divertikülün üretrayı arkadan komprese ettiği anlaşıldı. İki taraflı yüksek dereceli reflünün devam ettiği de dikkat çekti. Üretroskopisi normal olan hastanın sistoskopisinde ise mesane tabanının her iki yanında yerleşim gösteren ve içlerine üreter orifislerinin drene olduğu iki adet dev divertiküler oluşum izlendi. Ekstravezikal ve intravezikal yaklaşımla iki taraflı divertikülektomi ve iki taraflı transtrigonal üreteral reimplantasyon yapıldı. Ameliyat sonrası işeme sistoüretrografisinde vezikoüreteral reflü veya postmiksiyonel artık idrar lehine bulgu izlenmedi. Acute urinary retention is a very rare condition in pediatric population. Bladder diverticulum is usually not considered initially in the initial differential diagnosis. Herein we report a case of acute urinary retention due to bladder diverticulum. Our patient had antenatally diagnosed bilateral hydroureteronephrosis. Postnatal voiding cystourethrography demonstrated bilateral grade 5 vesicoureteral reflux. We decided to follow the patient on antimicrobial prophylaxis. After 1 year of follow-up, patient presented to the emergency department with failure to urinate. Ultrasonography and voiding cystourethrography demonstrated 2 giant bladder diverticula originating from bladder base and causing urethral obstruction. Bilateral diverticulectomy and bilateral transtrigonal ureteral reimplantation was performed. Voiding cystourethrography, which was carried out 6 months after the operation, demonstrated an urethra with normal caliber and no signs of vesicoureteral reflux or post-void residual urine Although encountered very rarely, bladder diverticula should always be considered in the differential diagnosis of acute urinary retention in pediatric population.

43. Fetal urinoma and prenatal hydronephrosis: how is renal function affected?

Abstract

Objective: In our study, the functional prognosis of kidneys with prenatal urinomas were investigated., Material and Methods: Between 2006 and 2010, fetal urinomas were detected in 19 fetuses using prenatal ultrasonography (US), and the medical records were reviewed retrospectively. Of the 19 cases, the follow-up data were available for 10 fetuses. The gestational age at diagnosis, prognosis of urinomas, clinical course and renal functions were recorded. Postnatal renal functions were assessed with renal scintigraphy., Results: Unilateral urinomas and increased parenchyma echogenicity in the ipsilateral kidney were detected in all of the fetuses. Of the 10 fetuses with follow-up data, the option of termination was offered in 6 cases of anhydramnios, including 3 cases with signs of infravesical obstruction (a possible posterior urethral valve (PUV) and poor prognostic factors and 3 cases with unilateral hydronephrosis and increased echogenicity in the contralateral kidney. Only one family agreed the termination. The other 5 fetuses died during the early postnatal period. The average postnatal follow-up period in the 4 surviving fetuses was 22.5 months (8-38 months). One patient with a PUV underwent ablation surgery during the early postnatal period. In the postnatal period, none of the 4 kidneys that were ipsilateral to the urinoma were functional on scintigraphic evaluation. The urinomas disappeared in 3 cases. Nephrectomy was performed in one case due to recurrent urinary tract infections., Conclusion: In our study, no function was detected in the ipsilateral kidney of surviving patients with urinomas. Upper urinary tract dilatation accompanied by a urinoma is a poor prognostic factor for renal function.

Abstract

Transfusion of platelet concentrates remains the first-line therapy for Glanzmann thrombasthenia in case of bleeding or preparation for surgery. However, development of antibodies to platelet glycoprotein (Gp) IIb/IIIa complex or human leukocyte antigens (HLA) is frequent and the main cause of platelet refractoriness. Recombinant activated factor VII (rFVIIa) is a potent alternative for patients with Glanzmann thrombasthenia with anti-platelet antibodies. We describe a case of Glanzmann thrombasthenia with alloantibodies to platelet Gp IIb/IIIa complex who underwent a successful pyelolithotomy operation under the coverage of recombinant activated factor VIIa and platelet transfusions.
