Abstract

BackgroundSafe and potent antiretroviral medications in child-friendly formulations are needed to treat young children living with HIV-1. We aimed to select dosing for a dispersible tablet formulation of dolutegravir that achieved pharmacokinetic exposures similar to those in adults, and was safe and well tolerated in young children.MethodsInternational Maternal Pediatric Adolescent AIDS Clinical Trial (IMPAACT) P1093 is a phase 1-2 ongoing multicentre, open-label, non-comparative study of dolutegravir. A 5 mg dispersible tablet formulation of dolutegravir was studied in children aged 4 weeks to less than 6 years old, weighing at least 3 kg, with HIV RNA of greater than 1000 copies per mL and no previous treatment with integrase strand transfer inhibitor recruited from IMPAACT clinical research sites in Africa, the Americas, and Asia. Doses were selected on the basis of intensive pharmacokinetic evaluation on days 5-10, with safety and tolerability assessed up to 48 weeks. The primary objectives of this study are to evaluate the pharmacokinetics of dolutegravir in combination with optimised background therapy and to establish the dose of dolutegravir that achieves the targeted 24-h trough concentration and 24-h area under the curve for infants, children, and adolescents with HIV-1, to establish the safety and tolerability of dolutegravir at 24 and 48 weeks, and to select a dose that achieves similar exposure to the dolutegravir 50 mg once daily dose in adults. This analysis included participants treated with the proposed dose of dolutegravir dispersible tablets in two stages for each of three age cohorts. This trial is registered at ClinicalTrials.gov (NCT01302847) and is ongoing.FindingsWe recruited 181 participants from April 20, 2011, to Feb 19, 2020; of these, 96 received dolutegravir dispersible tablets. This analysis included 73 (35, 48% female) participants who received the final proposed dose with median (range) age of 1 year (0·1 to 6·0), weight (minimum-maximum) of 8·5 kg (3·7 to 18·5), plasma HIV-1 RNA concentration of 4·2 log10 copies per mL (2·1 to 7·0), and CD4% of 24·0% (0·3 to 49·0); 64 (87·7%) were treatment-experienced. Abstract

Introduction.: The purpose of this mixed-methods study was to explore perceptions of work–life balance (WLB) of full-time faculty in professional physical therapist (PT) education programs. Review of Literature.: Studies show that faculty are at risk of diminution of WLB and subsequent burnout due to job expectations and changing educational landscapes. Limited evidence exists to describe perceived WLB of faculty in professional PT education. Subjects.: Full-time faculty in professional PT education programs (n = 239) responded to an online survey and 16 participated in focus group discussions. Methods.: The Work/Nonwork Interference and Enhancement Survey (W/NW Scale), faculty characteristic prompts, and open-ended questions were disseminated electronically to all accredited professional PT education programs within the United States. Focus groups were conducted to further explore nuances of WLB. Frequencies and percentages were calculated for survey categorical variables with means and standard deviations for continuous variables. Differences in W/NW Scale and faculty characteristics were analyzed using independent-samples Mann–Whitney U tests. Open coding and an iterative process of thematic analysis was used to analyze the qualitative data. Results.: Medians for all W/NW Scale dimensions were between 2 and 3 out of 5. W/NW Scale dimension scores differed significantly in each of the faculty characteristics of gender, defined workload, and faculty job searching (Z = −2.399 to −3.517, P ≤.016). Qualitative data yielded 3 main themes: 1) implicit and explicit job duties ; 2) leadership ; and 3) individual boundaries. Ebb and flow of WLB emerged as an overarching theme. Discussion and Conclusion.: This study offers a portrayal of the lived experience of faculty regarding WLB. Results offer useful information about faculty demographics, work-related factors, and personal responsibilities. One limitation was that cultural, community, and religious demands were not explored. Findings may facilitate improved understanding of faculty WLB perceptions and can guide leadership to optimize the balance between faculty's paid work and personal life. [ABSTRACT FROM AUTHOR]

Abstract

Objective: Developing and validating a disease-specific instrument in the Brazilian Portuguese language to assess the Health-Related Quality of Life of children with functional constipation, applied to parents/caregivers. Methods: The process of developing the questionnaire was carried out in the following steps: items generation concerning functional constipation; elaboration of the preliminary questionnaire; assessment by health professionals; identifying problems or inconsistencies by the researchers; improvement of the questions; obtaining a final questionnaire named Pediatric Functional Constipation Questionnaire-Parent Form (PedFCQuest-PR) with 26 questions divided into four domains. Responses options use a Likert scale based on the events of the last four weeks. The process of validation was an observational, cross-sectional study in a sample of 87 parents/caregivers of children from 5 to 15 years of age diagnosed with Functional constipation according to the Rome IV Criteria. The questionnaire was applied simultaneously to the Pediatric Quality of Life Inventory 4.0 (PedsQL TM 4.0) as a control. Results: The questionnaire validation included 87 parents/caregivers. The children's median age was 8.2 years, with a long time of constipation symptoms associated with fecal incontinence in approximately two-thirds. Internal consistency reliability for the Total Scale Score of PedFCQuest-PR by Coefficient Alpha of Cronbach score was 0.86. Convergent and divergent validity of PedFCQuest-PR was demonstrated by correlating the domains of both questionnaires. Conclusion: This study provides evidence that PedFCQuest-PR is a reliable instrument. The results showed a high degree of internal consistency and validity of the instrument for future applications.

Abstract

Objective: This study explores the perceptions surrounding the work of typhlologists in Colombia. Methodology: The fieldwork was conducted with three population groups: professionals in typhlology, professional blind adults, and blind youth from district educational institutions in the city of Bogota, Colombia. Semi-structured interviews were used as the primary instrument for data collection. Results: It is evident that, although national regulations highlight the importance of typhlology in inclusive education, there is no official profile stablished by the Ministry of National Education in which the field of action, characteristics and specific functions are established. In other words, there is no professionalization of the typhlologist's work. This means that any professional with a minimum of experience in relation to the educational process can accompany the visually impaired population. This could affect the quality of support provided to people with this type of disability in the country. Conclusions: It is necessary to recognize the importance of typhlology as a profession that contributes to inclusive education, to the extent that it addresses all the dimensions of visual impairment. Abstract

Introduction. The purpose of this study was to analyze and compare educational outcomes of Doctor of Physical Therapy (DPT) graduates before and during the COVID-19 pandemic. Review of Literature. Reports show increased stress, anxiety, and burnout during the COVID-19 pandemic, which may have negatively affected academic performance. Historically, academic performance is predictive of National Physical Therapy Examination (NPTE) scores. Yet, there is little evidence analyzing student outcomes during the pandemic. Subjects. Doctor of Physical Therapy graduate records (N = 1,897) were retrospectively collected from a multicenter convenience sample consisting of 5 blended programs. Records were sampled from existing "prepandemic" graduates of Fall 2018-2019 (n = 988) and "pandemic" graduates of Fall 2021-2022 (n = 909). Methods. A causal comparative and correlational study design was used. Grade point average (GPA) and NPTE scores were collected. A general linear model examined differences between groups, and a multiple linear regression examined predictors of NPTE performance. Results. Grade point average was a significant predictor of NPTE score (r² = 0.56; P < .01) for the overall sample and for the prepandemic and pandemic cohorts (r² = 0.38, P < .01; r² = 0.45, P < .01, respectively). Grade point average was not significantly different between the groups (P = .09), nor did it significantly influence differences in NPTE scores (P = .13). Pandemic graduates displayed a significantly lower NPTE pass rate (85.1%) compared with prepandemic graduates (89.7%; P < .01). Pandemic graduates experienced higher rates of academic difficulty (20%; P < .01) and had a approximately 5 times higher likelihood of not passing the NPTE. Discussion and Conclusion. This is the first study to report on DPT graduate outcomes spanning the COVID-19 pandemic. Similar to previous studies, GPA remained the most significant predictor of NPTE scores. Pandemic graduates demonstrated significantly lower NPTE scores and higher rates of academic difficulty (GPA < 3.0). Continued monitoring of NPTE performance is warranted between prepandemic, pandemic, and postpandemic cohorts across modes of program delivery. [ABSTRACT FROM AUTHOR]

29. Functional constipation in the first-year-old onset at the first visit to a specialized clinic elicits a delay in referrals, the worst symptoms, and the excessive use of rectal laxatives

Abstract

Purpose: To assess and compares sociodemographic, clinical, anthropometric, and treatment characteristics in two subgroups of children with FC: 1) with onset in the first 12 months old and 2) with onset after 13 months old. Methods: Observational, retrospective cohort study, including consecutive cases of children/adolescents for initial evaluation of constipation. Data were collected in a pre-designed standard protocol, and the parents answered all the questions during the first visit. Inclusion criteria: age between 01 month to 15 years of age, with a diagnosis of FC defined according to the Rome Criteria III and Rome IV criteria. Exclusion criteria: constipation of organic etiology; other chronic health problems such as neurological, genetic, mental/psychiatric disorders or growth and development disorders. The stool form was evaluated according to Bristol Stool Form Scale. Two experienced pediatric gastroenterologists determined the patients' final diagnoses of FC. Approval was obtained from the ethics committee. Results: Five hundred and four children were identified with constipation at their initial visit. Thirty-two were excluded based on the inclusion and exclusion criteria. The remaining 472 children (225 with onset within the first 12 months and 247 after 13 months) were included. There was a long time to first visit in those with onset in the first 12 months of life. There is no difference in BMI/age scores between the two groups. FC onset in the first 12 months has more scybalous stools, painful defecation, bloody stools, a lower proportion with bowel toilet training, and bladder control. In FC starting in the first 12 months of life, only half of the children (55%) were treated with laxatives, and an entire group used rectal treatment. Conclusion: FC is a common Functional Gastrointestinal Disorder affecting infants during the first months of life. The primary inadequate treatment and referral delay implies worsening symptoms, chronicity, and non-implementing North American Society for Pediatric Gastroenterology, Hepatology, and Nutrition guidelines.

Abstract

This paper presents a model that enables the transformation of digital signals generated by an inertial and magnetic motion capture system into kinematic information. First, the operation and data generated by the used inertial and magnetic system are described. Subsequently, the five stages of the proposed model are described, concluding with its implementation in a virtual environment to display the kinematic information. Finally, the applied tests are presented to evaluate the performance of the model through the execution of four exercises on the upper limb: flexion and extension of the elbow, and pronation and supination of the forearm. The results show a mean squared error of 3.82° in elbow flexion-extension movements and 3.46° in forearm pronation-supination movements. The results were obtained by comparing the inertial and magnetic system versus an optical motion capture system, allowing for the identification of the usability and functionality of the proposed model.

Here, we designed an electromagnetic system for practicing indoor cycling; this system can precisely simulate on the track the ascents and descents that allow the users to feel like they are training outdoors. Furthermore, this system connects to a web application that allows access from any mobile device to predefine the training routines and thus improve the user experience.

Abstract

Emotion recognition systems from physiological signals are innovative techniques that allow studying the behavior and reaction of an individual when exposed to information that may evoke emotional reactions through multimedia tools, for example, video games. This type of approach is used to identify the behavior of an individual in different fields, such as medicine, education, psychology, etc., in order to assess the effect that the content has on the individual that is interacting with it. This article shows a systematic review of articles that report studies on emotion recognition with physiological signals and video games, between January 2010 and April 2016. We searched in eight databases, and found 15 articles that met the selection criteria. With this systematic review, we found that the use of video games as emotion stimulation tools has become an innovative field of study, due to their potential to involve stories and multimedia tools that can interact directly with the person in fields like rehabilitation. We detected clear examples where video games and physiological signal measurement became an important approach in rehabilitation processes, for example, in Posttraumatic Stress Disorder (PTSD) treatments.

Abstract

This paper presents a characterization of the wireless transmission of biomechanical signals in an embedded system, where a TCP protocol is used in an IEEE 802.11 communications network (Wi-Fi). The embedded system under study, called Imocap, allows the collection, analysis and transmission of biomechanical signals in real-time for various applications, among which the analysis of the movement of the lower and upper extremities and the operation of various control systems stand out. To accomplish this, Imocap is equipped with a Wi-Fi transceiver module (ESP8266) and various input and output peripherals. The wireless communication performance of Imocap, exposed in this paper, was analyzed through different tests in miscellaneous conditions like indoors, outdoors and in the presence of interference, noise and other wireless networks. The different test protocols conducted result in the Imocap system: 1) has a maximum effective range of 45.6 m when in Access Point mode; 2) has a maximum effective range of 44.3 m when in Station mode. In indoors and under the same conditions, the Imocap system: 3) has a maximum effective range of 81.25 m2, either Access Point or Station mode. The results showed that the transmission of biomechanical information through Wi-Fi using the TCP protocol is efficient and robust, both indoors and outdoors, even in environments of radio frequency interference. The use of this protocol is emphasized since its use allows the transmission of packages to be carried out in a controlled manner, allowing the error handling and recovery. In this way, it is possible to carry out efficient and robust wireless communication through embedded and portable devices, focusing mainly on areas such as medicine, telemedicine and telerehabilitation.

Abstract

The use of videogames and motion capture systems in rehabilitation contributes to the recovery of the patient. This systematic review aimed to explore the works related to these technologies. The PRISMA method (Preferred Reporting Items for Systematic reviews and Meta-Analyses) was used to search the databases Scopus, PubMed, IEEE Xplore, and Web of Science, taking into consideration four aspects: physical rehabilitation, the use of videogames, motion capture technologies, and upper limb rehabilitation. The literature selection was limited to open access works published between 2015 and 2020, obtaining 19 articles that met the inclusion criteria. The works reported the use of inertial measurement units (37%), a Kinect sensor (48%), and other technologies (15%). It was identified that 26% used commercial products, while 74% were developed independently. Another finding was that 47% of the works focus on post-stroke motor recovery. Finally, diverse studies sought to support physical rehabilitation using motion capture systems incorporating inertial units, which offer precision and accessibility at a low cost. There is a clear need to continue generating proposals that confront the challenges of rehabilitation with technologies which offer precision and healthcare coverage, and which, additionally, integrate elements that foster the patient’s motivation and participation.

Debido al impacto de las nuevas tecnologías concretamente en los modelos sanitarios ha permitido que se pueda prestar un servicio más eficiente, eficaz, óptimo y seguro a los pacientes por parte de los profesionales de la salud en la práctica asistencial de los farmacéuticos donde resulta imprescindible la utilización de las TICs en el ámbito de la telefarmacia durante la atención farmacéutica del paciente externo; es decir que, se asocia a la dispensación a distancia con relación a las necesidades de su salud y garantizando la adecuación de los tratamientos prescritos, esta herramienta permite y potencia la realización de actividades donde se puede interaccionar con el paciente y entre sí para el desarrollo de la Telefarmacia como un parámetro fundamental para la gestión de todos los procesos estratégicos, operativos y de apoyo durante el ejercicio profesional de un Regente de Farmacia. Por lo tanto, es necesario identificar los conocimientos desarrollados sobre la telefarmacia en el contexto de la atención farmacéutica en América Latina, en la literatura científica publicada entre el 2012 a 2022 cuya metodología corresponde a una revisión integrativa con el método prisma de comprobación de requisitos, con una muestra de dos artículos científicos que cumplieron los criterios de inclusión y la evaluación de calidad, donde se tiene en cuenta el papel del farmacéutico y las necesidades individuales del paciente crónico desde el punto de vista farmacoterapéutico. Palabras claves: Telefarmacia, Atención Farmacéutica, América Latina, Paciente crónico The impact of new technologies, specifically on health models has allowed health professionals to provide a more efficient, effective, optimal and safe service to patients in the healthcare practice of pharmacists where it´s essential to use of ICTs in the ambit of telepharmacy during outpatient pharmaceutical care; that means, it is associated with remote dispensing in relation to the needs of their health and guaranteeing the adequacy of the prescribed treatments, this tool allows and promotes performance of activities where it is possible to interact with the patient and each other for the development of Telepharmacy as a fundamental parameter for management of all strategic, operational and support processes during the professional practice of a Pharmacy Manager. Therefore, it´s necessary to identify the knowledge developed on telepharmacy in the context of pharmaceutical care in Latin America, in scientific literature published between 2012 and 2022 whose methodology corresponds to an integrative review with the Prism Method of Verification of Requirements, with a sample of two scientific articles that met the inclusion criteria and quality assessment, which takes into account role of the pharmacist and individual needs of the chronic patient from the pharmacotherapeutic point of view.
