Femoroacetabular impingement (FAI) is an anatomical hip condition caused by malformations on femoral head and acetabular rim resulting in abnormal contact across the joint. FAI can cause the labral, cartilaginous, and tissue damage that leads to early osteoarthritis. FAI can be divided into three groups: cam (bump on femoral head- neck junction), pincer (acetabular over coverage), or mixed (most common) by the characteristic morphological changes of the bony structures. The exact ethology of FAI is still unclear, mostly considered as idiopathic. Cam lesions demonstrate a near 3:1 male predominance and are more often seen in the younger population. Pincer is typically seen in middle-aged women. A plain radiography of the pelvis and hips is considered as the primary imaging modality for diagnosing FAI, which can be used to quantify the severity. MRI and direct MRI arthrography allow assessment of concomitant labral and chondral injuries. Conservative treatment is typically considered first-line treatment for mild to moderate FAI syndrome, but usually not to successful. However, the outcomes following postoperative surgical intervention have demonstrated excellent results. The most common surgical treatment option for FAI is done arthroscopically, other procedures such as a reverse periacetabular osteotomy or surgical dislocation of the hip are rarely indicated. Background: The early detection of ovarian cancer is presently not effective, and it is crucial to establish biomarkers for the early diagnosis of ovarian cancer to improve the survival of patients. Materials and methods: The aim of this study was to investigate the role of thymidine kinase 1 (TK1) in combination with CA 125 or HE4 to serve as a potential diagnostic biomarkers for ovarian cancer. In this study, a set of 198 serum samples consisting of 134 ovarian tumor patients and 64 healthy age-matched controls were analyzed. The TK1 protein levels in serum samples were determined using the AroCell TK 210 ELISA. Results: A combination of TK1 protein with CA 125 or HE4 showed better performance than either of them alone in the differentiation of early stage ovarian cancer from the healthy control group, but also a significantly better performance than the ROMA index. However, this was not observed using a TK1 activity test in combination with the other markers. Furthermore, the combination of TK1 protein and CA 125 or HE4 could differentiate early stage disease (stage I, II) more efficiently from advanced-stage (stage III, IV) disease (p < 0.0001). Cemented total hip endoprosthesis Link Lubinus SP II has been used for decades with very good results in arthroplasty registries, but surgeon-stratified reports of endoprosthetic survival are very rare. The aim of the presented single hospital cohort analysis of this implant was to determine Link Lubinus SP II survival rates 10/15/20/25/30 years after the primary implantation and to find out whether endoprosthesis survival depended on patients' age, gender, operated side, implanted femoral head diameter, and the operating surgeon.The study included 2943 consecutive primary Link Lubinus SP II hip endoprostheses implanted at the University Medical Centre Ljubljana, Department of Orthopaedic Surgery (Ljubljana, Slovenia) between January 1, 1985, and December 31, 2018. Kaplan-Meier analysis and Cox regression were performed after minimum two and maximum 30 years of follow-up.At 10/15/20/25/30 years after implantation, the estimated cumulative proportion of revision-free surviving Link Lubinus SP II total hip endoprostheses was 94/90/88/87/87% and the cumulative proportion with unremoved endoprosthetic components was 96/93/90/89/89%, respectively. Higher patient's age at operation was associated with lower risk of subsequent implant removal (hazard ratio 0.97 for each additional year of age; 95% confidence interval 0.95-0.99; p = 0.00), while the patient's gender, the implanted femoral head diameter, and the operating surgeon had no significant impact on implant survival.The study presents the largest published Link Lubinus SP II total hip arthroplasty cohort from a single non-developmental hospital with 26,981 component-years of observation. The findings highlight excellent outcomes of this implant in the elderly population, regardless of performance variability between surgeons.

We sought to compare VeraCept (VC175), a novel nitinol intrauterine contraceptive (IUC) with 175 square-mm of copper surface area, to a copper T380S IUC.We enrolled parous women into a randomized subject-blinded comparison of VC175 and a copper T380S in a 2:1 fashion at a single clinic. The primary outcomes were total adverse events and continuation at 12 months. We also examined pain on insertion, ease of placement, expulsion, tolerability and pregnancy. Subjective ratings were on a 5-point Likert scale (0, no pain to 5, worst pain). We followed subjects through 24-month follow-up.We enrolled 300 women with 199 randomized to VC175 and 101 to the T380S. Insertion was successful in 198 subjects for VC175 and 100 for the T380S. Mean age was 25 years (range 18, 41), and median parity was 2 (range 1, 8), with 39% having only had Cesarean deliveries. No subjects developed clinical infection or reported serious adverse events. In the VC175 and T380S groups, mean pain at insertion was 1.4 and 2.4, respectively (p.01). At the 12-month primary endpoint for VC175 and T380S, respectively, continuation was 84% and 68% (p.002) with expulsions in 5.0% and 12.0% (p.05) and removal for pain/bleeding in 3.5% and 17.0% (p.01). At the 24-month visit for VC175 and T380S, respectively, continuation was 77% and 62% (p.02 by log-rank). One ectopic pregnancy was identified at the 12-month follow-up in a VC175 user. No other pregnancies were diagnosed. With 297.3 and 132.4 woman-years, pregnancy rates were 0.3 and 0.0 per 100 woman-years for VC175 and T380S, respectively.VC175 resulted in less pain at insertion, fewer expulsions and higher total continuation than the T380S, with similar contraceptive efficacy.VC175 is a promising new intrauterine copper contraceptive on a nitinol frame that warrants further clinical trials.

MSCT showed thickened wall of ileum with a sharp foreign object inside and some free fluid in the right lower quadrant. In modern gynecology an intrauterine device (IUD) with levonorgestrel is often used as a method of contraception. The levonorgestrel-releasing intrauterine system is small and T-shaped. In Slovenia, only a gynecologist may insert it. CASES: We present 2 clinical cases in which, despite strong evidence that no perforation had occurred during insertion, the IUD was found outside the uterus. If the IUD threads are not visible or the IUD cannot be located in the uterine cavity, an X-ray of the abdomen must be performed. If the IUD is found in the abdominal cavity outside the uterus, removal by laparoscopy is carried out.Given the large number of inserted IUDs, the complications associated with the levonorgestrel-releasing intrauterine system are quite rare, and therefore it remains one of the most widely used contraceptive methods.
