1. An external validation of the Kidney Donor Risk Index in the UK transplant population in the presence of semi-competing events

Abstract: Abstract Background Transplantation represents the optimal treatment for many patients with end-stage kidney disease. When a donor kidney is available to a waitlisted patient, clinicians responsible for the care of the potential recipient must make the decision to accept or decline the offer based upon complex and variable information about the donor, the recipient and the transplant process. A clinical prediction model may be able to support clinicians in their decision-making. The Kidney Donor Risk Index (KDRI) was developed in the United States to predict graft failure following kidney transplantation. The survival process following transplantation consists of semi-competing events where death precludes graft failure, but not vice-versa. Methods We externally validated the KDRI in the UK kidney transplant population and assessed whether validation under a semi-competing risks framework impacted predictive performance. Additionally, we explored whether the KDRI requires updating. We included 20,035 adult recipients of first, deceased donor, single, kidney-only transplants between January 1, 2004, and December 31, 2018, collected by the UK Transplant Registry and held by NHS Blood and Transplant. The outcomes of interest were 1- and 5-year graft failure following transplantation. In light of the semi-competing events, recipient death was handled in two ways: censoring patients at the time of death and modelling death as a competing event. Cox proportional hazard models were used to validate the KDRI when censoring graft failure by death, and cause-specific Cox models were used to account for death as a competing event. Results The KDRI underestimated event probabilities for those at higher risk of graft failure. For 5-year graft failure, discrimination was poorer in the semi-competing risks model (0.625, 95% CI 0.611 to 0.640;0.611, 95% CI 0.597 to 0.625), but predictions were more accurate (Brier score 0.117, 95% CI 0.112 to 0.121; 0.114, 95% CI 0.109 to 0.118). Calibration plots were similar regardless of whether the death was modelled as a competing event or not. Updating the KDRI worsened calibration, but marginally improved discrimination. Conclusions Predictive performance for 1-year graft failure was similar between death-censored and competing event graft failure, but differences appeared when predicting 5-year graft failure. The updated index did not have superior performance and we conclude that updating the KDRI in the present form is not required.

Abstract: Fibromuscular dysplasia (FMD) is a non-atherosclerotic, non-inflammatory disorder of the arterial wall muscular layer which can lead to arterial stenosis, occlusion, and dissection. Clinical presentations of FMD vary depending on the arterial territories involved, often leading to diagnostic challenges. This case report describes an exceptionally unusual presentation of FMD, not previously described, affecting a previously fit and well 37-year-old female presenting with bilateral renal infarction, sequential vertebral artery dissections, mesenteric ischaemia, and the requirement for continued renal replacement. This report highlights how unusual presentations of FMD can mask the underlying diagnosis. Early consideration of FMD in a differential diagnosis can guide an effective management strategy, including appropriate imaging and multi-speciality involvement.

Abstract: The identification of bright quasars at z ≳ 6 enables detailed studies of supermassive black holes, massive galaxies, structure formation, and the state of the intergalactic medium within the first billion years after the Big Bang. We present the spectroscopic confirmation of 55 quasars at redshifts 5.6 < z < 6.5 and UV magnitudes −24.5 < M _1450 < −28.5 identified in the optical Pan-STARRS1 and near-IR VIKING surveys (48 and 7, respectively). Five of these quasars have independently been discovered in other studies. The quasar sample shows an extensive range of physical properties, including 17 objects with weak emission lines, 10 broad absorption line quasars, and 5 objects with strong radio emission (radio-loud quasars). There are also a few notable sources in the sample, including a blazar candidate at z = 6.23, a likely gravitationally lensed quasar at z = 6.41, and a z = 5.84 quasar in the outskirts of the nearby ( D ∼ 3 Mpc) spiral galaxy M81. The blazar candidate remains undetected in NOEMA observations of the [C ii] and underlying emission, implying a star formation rate

Abstract: Background The effects of empagliflozin in patients with chronic kidney disease who are at risk for disease progression are not well understood. The EMPA-KIDNEY trial was designed to assess the effects of treatment with empagliflozin in a broad range of such patients. Methods We enrolled patients with chronic kidney disease who had an estimated glomerular filtration rate (eGFR) of at least 20 but less than 45 ml per minute per 1.73 m2 of body-surface area, or who had an eGFR of at least 45 but less than 90 ml per minute per 1.73 m2 with a urinary albumin-to-creatinine ratio (with albumin measured in milligrams and creatinine measured in grams) of at least 200. Patients were randomly assigned to receive empagliflozin (10 mg once daily) or matching placebo. The primary outcome was a composite of progression of kidney disease (defined as end-stage kidney disease, a sustained decrease in eGFR to Results A total of 6609 patients underwent randomization. During a median of 2.0 years of follow-up, progression of kidney disease or death from cardiovascular causes occurred in 432 of 3304 patients (13.1%) in the empagliflozin group and in 558 of 3305 patients (16.9%) in the placebo group (hazard ratio, 0.72; 95% confidence interval [CI], 0.64 to 0.82; PConclusions Among a wide range of patients with chronic kidney disease who were at risk for disease progression, empagliflozin therapy led to a lower risk of progression of kidney disease or death from cardiovascular causes than placebo. Abstract: Introduction: Treatment of anemia in dialysis patients has been associated with increased risk of vascular access thrombosis (VAT). Proactive IV irOn Therapy in hemodiALysis Patients (PIVOTAL) was a clinical trial of proactive compared with reactive i.v. iron therapy in patients requiring hemodialysis. We analyzed the trial data to determine whether randomized treatment arm, alongside other clinical and laboratory variables, independently associated with VAT.Methods: In PIVOTAL, 2141 adult patients were randomized. The type of vascular access (arteriovenous fistula [AVF], arteriovenous graft [AVG], or central venous catheter [CVC]) was recorded at baseline and every month after randomization. The associations between clinical and laboratory data and first VAT were evaluated in a multivariate analysis.Results: A total of 480 (22.4%) participants experienced VAT in a median of 2.1 years of follow-up. In multivariable analyses, treatment arm (proactive vs. reactive) was not an independent predictor of VAT (hazard ratio [HR] 1.13, P = 0.18). Diabetic kidney disease (HR 1.45, P < 0.001), AVG use (HR 2.29, P < 0.001), digoxin use (HR 2.48, P < 0.001), diuretic use (HR 1.25, P = 0.02), female sex (HR 1.33, P = 0.002), and previous/current smoker (HR 1.47, P = 0.004) were independently associated with a higher risk of VAT. Angiotensin receptor blocker (ARB) use (HR 0.66, P = 0.01) was independently associated with a lower risk of VAT.Conclusion: In PIVOTAL, VAT occurred in nearly 1 quarter of participants in a median of just >2 years. In this post hoc analysis, randomization to proactive i.v. iron treatment arms did not increase the risk of VAT.

Abstract: A urinoma is an unusual complication following renal transplant biopsy that can easily be missed or mistaken for a hematoma. In addition to trauma to the renal collecting system, a degree of urinary tract obstruction is required for urine to leak into the surrounding tissues and form a urinoma, which can in turn cause pressure on surrounding structures. This case report describes a patient who developed ipsilateral leg swelling several months after a renal transplant biopsy. Imaging demonstrated a perirenal transplant fluid collection, which biochemical analysis confirmed to be urine. This was successfully managed with percutaneous nephrostomy and antegrade ureteric stent insertion. The fluid collection persisted as a seroma however, and the patient proceeded to have peritoneal fenestration and marsupialization surgery. To our knowledge, this is the first reported case of urinoma complicating a renal transplant biopsy. This case highlights a diagnosis that can be easily missed and is therefore a potential pitfall for clinicians.

Abstract: It is generally accepted that Roman administrators, arriving in Egypt in the aftermath of Augustus'annexation of the province, confiscated en masse the land and other property belonging to the temples of Egypt—estimated at as much as one-third of the country. It is further accepted that this confiscation doomed the temples by removing their economic support and making them subservient to the Roman state, and that this in turn led to the collapse of Egyptian religion. In Confiscation or Coexistence: Egyptian Temples in the Age of Augustus, author Andrew Connor takes direct issue with both claims. The interpretative consensus developed after the publication of a handful of key documents—P.Tebt. 2.302 especially, alongside BGU 4.1198 and 1200, and P.Berl.Leihg. 1.5. Connor offers a fundamentally revised interpretation of these texts, building from a fresh examination of the papyri themselves. The book frames the interpretation in a wider discussion of Roman interactions with Egyptian religion, including material from inside and outside Egypt, and locates the development of an interpretative consensus in early 20th-century scholarship within the wider context of empire and colonization at the time. In doing so, Connor explores these papyri through their historical, intellectual, and linguistic contexts, alongside a number of other important texts bearing on the relationship between the temples and the Roman state.

14. Advancing the Clinical Potential of Carbon Nanotube-enabled stationary 3D Mammography

Abstract: Scope and purpose. 3D imaging has revolutionized medicine. Digital breast tomosynthesis (DBT), also recognized as 3D mammography, is a relatively recent example. stationary DBT (sDBT) is an experimental technology in which the single moving x-ray source of conventional DBT has been replaced by a fixed array of carbon nanotube (CNT)-enabled sources. Given the potential for a higher spatial and temporal resolution compared to commercially-available, moving-source DBT devices, it was hypothesized that sDBT would provide a valuable tool for breast imaging. As such, the purpose of this work was to explore the clinical potential of sDBT. To accomplish this purpose, three broad Aims were set forth: (1) study the challenges of scatter and artifact with sDBT, (2) assess the performance of sDBT relative to standard mammographic screening approaches, and (3) develop a synthetic mammography capability for sDBT. Throughout the work, developing image processing approaches to maximize the diagnostic value of the information presented to readers remained a specific goal. Data sources and methodology. Sitting at the intersection of development and clinical application, this work involved both basic experimentation and human study. Quantitative measures of image quality as well as reader preference and accuracy were used to assess the performance of sDBT. These studies imaged breast-mimicking phantoms, lumpectomy specimens, and human subjects on IRB-approved study protocols, often using standard 2D and conventional 3D mammography for reference. Key findings. Characterizing scatter and artifact allowed the development of new processing approaches to improve image quality. Additionally, comparing the performance of sDBT to standard breast imaging technologies helped identify opportunities for improvement through processing. This line of research culminated in the incorporation of a synthetic mammography capability into sDBT, yielding images that have the potential to improve the diagnostic value of sDBT. Implications. This work advanced the evolution of CNT-enabled sDBT toward a viable clinical tool by incorporating key image processing functionality and characterizing the performance of sDBT relative to standard breast imaging techniques. The findings confirmed the clinical utility of sDBT while also suggesting promising paths for future research and development with this unique approach to breast imaging.

Abstract: Luminex single antigen bead (SAB) assays have become an essential tool in monitoring the status of antibody to the Human Leucocyte Antigen (HLA) of patients both before and after transplantation. In addition SAB data is used to aid risk stratification to assess immunological risk of humoral rejection in solid organ transplantation (CTAG/BTAG guidelines) [1]. Increasingly laboratories are reporting false negative results at high antibody titre due to a prozone effect. Here we report a case study where the prozone effect led to a false negative antibody result that could have resulted in adverse outcome. We describe a method to reliably remove the prozone effect through heat inactivation and the addition of Ethylenediaminetetraacetic acid (EDTA) to the Luminex wash buffer.

Abstract: Gitelman syndrome is caused by inactivating mutations of the gene that encodes the renal sodium/chloride cotransporter (NCC; encoded by SLC12A3), resulting in hypokalemia, hypomagnesemia, hypocalciuria, and metabolic alkalosis. Renal salt wasting commonly provokes mild hypotension. The paucity of previous kidney transplants from donors with known tubulopathies suggests that such conditions may be considered contraindications to donation. A 76-year-old man received a live unrelated kidney transplant from a donor with known Gitelman syndrome secondary to a pathogenic mutation of SLC12A3. Immediate graft function preceded the emergence of the Gitelman syndrome biochemical phenotype and blood pressure subsequently improved. The recipient developed unexpected hyponatremia. Potential causes are discussed, including the possibility that it paralleled the physiologic changes seen in the high-volume state of thiazide-induced hyponatremia. Transplanted kidneys are subject to nephrotoxicity from the use of calcineurin inhibitors. Acquired Gitelman syndrome may confer a potential long-term advantage to the recipient through both improved blood pressure control and protection against the calcineurin inhibitor–induced side-effect profile caused by NCC overactivation. Both the donor and recipient remain well. In conclusion, Gitelman syndrome need not preclude kidney donation and transference of the phenotype may have benefits for the recipient.

Abstract: A 1991 reappraisal of property in Atlanta and Fulton County, Georgia, corrected systemic inequalities in property taxation that had subsidized affluent whites during a boom period. Many of those homeowners responded to reassessment by initiating a major tax revolt. The tax revolt failed to stop reassessment but won lasting victory in the political arena. Tax revolt politicians successfully linked the economic grievances of taxpayers to resentments against black political leaders and the poor, replaced older, moderate suburban politics with confrontation, and sped partisan realignment and Republican ascendancy in the state. Historical perspective on the politics of the Fulton County tax revolt supports the conclusion that metropolitics were essential to the development of “color-blind” or “laissez-faire” forms of racial ideology that became the dominant mode of American racial discourse in the post–Civil Rights era.

Abstract: The Supreme Court’s five-to-four decision in Citizens United v. Federal Election Commission overturned provisions of the 2002 Bipartisan Campaign Reform Act (commonly known as McCain-Feingold) that had barred corporations from spending money on independent (i.e., not formally affiliated with a candidate) political communication. 1 Justice Anthony Kennedy wrote for a five-to-four majority that the Federal Election Commission (FEC) could not constitutionally allow individuals to speak through paid political advocacy while prohibiting corporations from doing so. Critics of the decision, beginning with the dissenting justices and including President Barack Obama, have accused the court of partisanship and encouraging corporations to dominate electoral politics. The phrase “post–Citizens United” has come to signify a new, money-driven political order in which the voices and influence of ordinary citizens are suppressed. In this article, I argue that the implications of the decision and therefore the changes needed to create a more democratic society have been widely misunderstood. A more productive understanding of Citizens United must view the case in its entirety as a sociolegal event—including winning, losing, and unmade arguments and the premises underlying them. Abstract: Background The use of generic formulations of immunosuppressive drugs in place of brand name drugs offers considerable cost savings. Brand name tacrolimus (Prograf®) came off patent in April 2008. However, published evidence supporting therapeutic equivalence of generic formulations of tacrolimus in solid organ transplantation is lacking. The South West Transplant Centre switched from administering Prograf® to a generic formulation (Adoport®) for de novo transplant recipients in November 2010. This study sought to compare the clinical outcomes of renal transplant recipients administered Prograf® with those receiving Adoport®. Methods Data regarding patient characteristics and clinical outcomes were collected retrospectively for all patients undergoing renal transplantation at the South West Transplant Centre between 8 November 2009 and 8 November 2011 to whom tacrolimus was prescribed. Results A total of 48 patients received Prograf® and 51 received Adoport®. At 6 months, no statistically significant differences were identified in the rates of patient survival, graft survival, acute allograft rejection, delayed graft function, calcineurin inhibitor toxicity or cytomegalovirus infection occurring within the two groups. Conclusions This is the first study to compare the clinical outcomes of patients receiving Adoport® with those receiving brand name tacrolimus. We report comparable clinical outcomes at 6 months in patients receiving either Prograf® or Adoport® from the time of renal transplantation. These early outcome data therefore support the use of Adoport® in place of Prograf® as a potential cost-saving measure.

Abstract: Emergent public choice theory and innovations in suburban local government worked together to create and justify greater inequality among metropolitan places in postwar Los Angeles County. This article examines public choice theory and suburban home rule as mutually dependent components of suburbanization. Theorists praised postwar metropolitan fragmentation, challenging a prior consensus for metropolitan political and economic integration. Local governments under the “Lakewood Plan” obtained public services by contract from Los Angeles County at covertly subsidized prices, making government in new suburbs cost effective at the expense of older municipalities. The governments deployed symbols of suburban “home rule” and public choice principles of efficiency to defend their privileges. Movements to incorporate minority-dominated cities in Watts and East Los Angeles and for the secession of the San Fernando Valley from Los Angeles reflected the dominance of the practice and ideology of localism in California metropolitics.

Abstract: Oral bowel-cleansing preparations are used before colonic surgery and endoscopic and radiological assessment of the intestine to minimise faecal contamination. In February 2009, the UK National Patient Safety Agency issued a Rapid Response Report highlighting the potential risk of harm associated with the use of these preparations and instructing local NHS Trusts to implement safeguards to reduce this risk. This guidance has been prepared to help NHS Trusts to respond to these concerns, as the risk of complications is influenced by both individual patient risk factors and the choice of bowel preparation, for which definitive guidance was not previously available. This document provides an outline of the different available oral bowel-cleansing agents and the complications that may arise. This is followed by recommendations for their use in different patient groups and circumstances. The recommendations are based on consensus between the authors, each of whom circulated drafts to members of their specialist society. The evidence for these recommendations has been assessed using the modified GRADE system. The recommendations cover the choice, administration and complications (relative and absolute) of the different oral bowel-cleansing agents, with specific guidance provided for different patient groups.

This was a component analysis study adhering to PAS2050. Emissions factors were applied to data that were collected for building energy use, travel and procurement. Thrice weekly in-center HD has a carbon footprint of 3.8 ton CO2 Eq per patient per year. The majority of emissions arise within the medical equipment (37%), energy use (21%), and patient travel (20%) sectors. The carbon footprint of providing home HD varies with the regime. For standard machines: 4 times weekly (4 days, 4.5 hours), 4.3 ton CO2 Eq; 5 times weekly (5 days, 4 hours), 5.1 ton CO2 Eq; short daily (6 days, 2 hours), 5.2 ton CO2 Eq; nocturnal (3 nightly, 7 hours), 3.9 ton CO2 Eq; and nocturnal (6 nightly, 7 hours), 7.2 ton CO2 Eq. For NxStage equipment: short daily (5.5 days, 3 hours), 1.8 t CO2 Eq; 6 nightly nocturnal (2.1 ton CO2 Eq). The carbon footprint of HD is influenced more by the frequency of treatments than by their duration. However, the very provision of healthcare itself has a significant environmental impact, and the contribution of kidney care to the carbon footprint of the NHS is likely to be disproportionately high. Furthermore, the current economic climate will ensure that healthcare systems face unprecedented reductions in resources (or, at the very least, diminished expansion in the face of ongoing increases in demand). Improvements in the way that renal services use resources will address both issues. This survey was designed to identify a baseline for sustainability in kidney care, to support a clinical transformation to lower carbon kidney care by identifying fruitful areas for attention, and to act as an educational tool. The survey identified measures for improvement across the different areas of the provision of kidney care, including building energy use, patient and staff transport, water use and the consumption and procurement of resources. The results of the survey, which was completed by 58 adult and paediatric renal units across England, Scotland, and Wales, are reported here and potential changes are discussed.

Action must be taken to reduce this impact. Water is a precious and finite natural resource. Vast quantities of high-grade water are required to provide haemodialysis. The reverse osmosis systems used in the purification process reject approximately two-thirds of the water presented to them. Therefore, around 250 litres of 'reject water' result from the production of the dialysate required for one treatment. This good quality reject water is lost-to-drain in the vast majority of centres worldwide. Simple methodologies exist to recycle this water for alternative purposes. We describe here a case study of the only UK renal service we know to have implemented such water-saving methodologies. We outline the benefits in terms of financial and environmental savings.

40. Toward greener dialysis: a case study to illustrate and encourage the salvage of reject water

Abstract: Climate change is now considered to be a major global public health concern. However, the very provision of health care itself has a significant impact upon the environment. Action must be taken to reduce this impact. Water is a precious and finite natural resource. Vast quantities of high-grade water are required to provide haemodialysis. The reverse osmosis systems used in the purification process reject approximately two-thirds of the water presented to them. Therefore, around 250 litres of 'reject water' result from the production of the dialysate required for one treatment. This good quality reject water is lost-to-drain in the vast majority of centres worldwide. Simple methodologies exist to recycle this water for alternative purposes. We describe here a case study of the only UK renal service we know to have implemented such water-saving methodologies. We outline the benefits in terms of financial and environmental savings.

Abstract: A 43-year-old man with no cardiac history presented with chest pain followed by cardiac arrest. He was successfully defibrillated and underwent primary percutaneous coronary angioplasty to a culprit coronary artery lesion. He later re-presented with a diffuse urticarial rash and lip swelling, reporting that these symptoms had been present for 4 weeks before his cardiac arrest and voicing concern that a further cardiac arrest may be imminent. A diagnosis of post-viral or idiopathic autoimmune urticaria and angioedema was made. Given the absence of cardiac symptoms before the development of the rash, it was hypothesised that coronary artery spasm precipitated by histamine release due to his dermatological condition contributed to his myocardial infarction and cardiac arrest. The final diagnosis was therefore cardiac arrest secondary to type II Kounis syndrome, resulting from idiopathic autoimmune or post-viral urticaria and angioedema.

Abstract: Most patients with early chronic kidney disease are managed in primary care. These patients have an increased risk of cardiovascular disease; the lower the estimated glomerular filtration rate, the higher the risk.1 Patients with stages 1 to 4 of chronic kidney disease are more likely to die from cardiovascular disease than to require renal replacement therapy, and cardiovascular disease is the most common cause of death among dialysis and transplant patients.2 However, the causal pathway is unclear; conventional cardiovascular risk factors (including dyslipidaemia) may also cause kidney disease progression, but kidney disease may also cause cardiovascular disease through pathways unrelated to dyslipidaemia. The benefits of lipid lowering treatment in primary and secondary prevention of cardiovascular disease in people with preserved renal function are well established.3 4 Recent guidance from the National Institute for Health and Clinical Excellence (NICE) recommends treatment with statins for secondary prevention of cardiovascular disease in all patients with chronic kidney disease, irrespective of the lipid profile, although patients with chronic kidney disease remain less likely to be prescribed statins after a cardiovascular event, despite the side effects being similar to those of placebo.5 6 7 However, the role of statins in primary prevention among patients with chronic kidney disease remains uncertain. Abstract: Successful management of the idiopathic inflammatory myopathies requires an early and accurate diagnosis. The muscle biopsy remains the definitive test. However, false-negative biopsy results are common, as the disease is typically patchy in distribution. The advent of short tau inversion recovery sequences now allows rapid magnetic resonance imaging of the whole body to be performed, enabling identification of the muscles most suitable for biopsy. It also provides further diagnostic information through the form and anatomic distribution of the pathology. We report 2 cases illustrating the advantages of whole-body short tau inversion recovery magnetic resonance imaging before muscle biopsy. We encourage clinicians to use the technique in this context.

Abstract: Multicentric carpal-tarsal osteolysis is a rare skeletal disorder characterized by osteolysis of the metacarpal, carpal, and tarsal bones and leading to crippling joint deformities. Progressive nephropathy occurs in more than half the cases. All previously reported series with renal biopsies showed only end-stage renal disease on histological examination because of the late presentation to nephrologists. Accurate diagnosis of the underlying renal pathological state therefore has not been possible. We report the first case in which early and sequential renal biopsies were performed. These show the renal lesion to be focal and segmental glomerulosclerosis, which was treated successfully with cyclosporine A.

50. Makah IndianTribe – U.S. Coast Guard Memorandum of Agreement A Model for Meaningful Tribal-Federal Engagement

Abstract: On 12 April 2013, the Makah Tribal Council Chairman and the District Commander of the 13th Coast Guard District (D13) signed into effect the “Memorandum of Agreement between the United States Coast Guard and the Makah Indian Tribe Regarding Interoperability and Coordination”. The purpose of this “Makah-USCG MOA” is to establish terms by which the two parties will coordinate and collaborate in the fulfillment of their mutual trust responsibility. The MOA's focus is to enhance consultation, improve leveraging of resources within each party's authorities, and improve collective all-hazards prevention and response posture in the Makah Treaty Area. Such an agreement between a federal agency and an Indian tribe is authorized by Executive Order 13175 - Consultation and Coordination with Indian Tribal Governments, signed in 2000 and is consistent with the 2011 Department of Homeland Security Tribal Consultation Policy. This groundbreaking document represents a 20 year relationship between key members of the Makah Tribe and the oil spill response members of CG D13 and Sector Puget Sound, and the trust developed over that period while working together to mutually support each party's oil spill prevention, preparedness and response readiness, as well as the desire to help and support each other by harmonizing efforts. With recent legislation such as the 2010 Coast Guard Authorization act, authorizing resources to engage tribes into training, exercises and other spill response preparedness activities particularly in the Northwest, it became clear there was a need to memorialize the strong supportive working relationship between the Makah and the CG, and provide some structure to our planning and development efforts. The MOA lays out a minimum of semi-annual meetings during which the two parties discuss overarching items of mutual interest and determine a work list of concrete, achievable goals to complete within the next 6 month period. An initial work list of common interest issues and responsibilities are outlined in the MOA, and fall into three general categories: Prevention; Preparedness and Response. The MOA also addresses the importance of Communication with the goal of enhancing mutual situational awareness via improved communication protocols between the Coast Guard and the Makah Tribe.
