Introduction Advising is happening across the medical education continuum, within non-medical graduate education programs, and is central to the advancement of said learners. This suggests that advising should play a role in graduate health progressions education (HPE) programs. Materials and Methods To explore advising curricula among HPE programs, we conducted a website review of all published HPE programs on the Foundation for Advancement of International Medical Education and Research’s website. Results We recognized the lack of information published on advisory roles in graduate HPE programs. This prompted a literature review, which revealed a similar gap. Conclusions Advising serves to benefit a student, advisor, and program thus carrying importance and need for discussion. This article is intended to kick-start a scholarly discussion about advising within graduate HPE programs.

The robots automatically retarget their optical fibers every 10–20 minutes, each to a precision of several microns, with a reconfiguration time of fewer than 2 minutes. Over the next 5 yr, they will enable the newly constructed Dark Energy Spectroscopic Instrument (DESI) to measure the spectra of 35 million galaxies and quasars. DESI will produce the largest 3D map of the universe to date and measure the expansion history of the cosmos. In addition to the 5020 robotic positioners and optical fibers, DESI's Focal Plane System includes six guide cameras, four wave front cameras, 123 fiducial point sources, and a metrology camera mounted at the primary mirror. The system also includes associated structural, thermal, and electrical systems. In all, it contains over 675,000 individual parts. We discuss the design, construction, quality control, and integration of all these components. We include a summary of the key requirements, the review and acceptance process, on-sky validations of requirements, and lessons learned for future multiobject, fiber-fed spectrographs. © 2022. Objectives Burnout and occupational stress are frequently experienced by healthcare professionals (HCPs). Mindfulness-based stress reduction (MBSR) has been found to improve the psychological health outcomes of HCPs. To date, systematic reviews and meta-analyses have primarily focused upon empirical investigations into the reduction of stress amongst HCPs using MBSR and are limited to empirical studies published before December 2019. This systematic review aimed to update the current evidence base and broaden our understanding of the effectiveness of MBSR on improving the psychological functioning of HCPs. Methods Three electronic databases (Medline, Psych Info and Web of Science) were searched without time frame restrictions. Quantitative studies included randomised controlled trials, clinical controlled trials, pre-post designs and studies with up to a 12-month follow-up period. All studies included in the review employed a MBSR programme, standardised measures of psychological functioning and qualified HCPs as participants. Results Using PRISMA guidelines thirty studies were included in the review. The reviewed literature suggested that MBSR was effective in reducing HCPs experiences of anxiety, depression and stress. MBSR was also found to be effective in increasing HCP levels of mindfulness and self-compassion. However, MBSR did not appear as effective in reducing burnout or improving resilience amongst HCPs. Abbreviated MBSR programmes were found to be as effective as the traditional 8-week MBSR programmes. Conclusions MBSR is an effective intervention which can help improve the psychological functioning of HCPs. Recommendations include improving the overall quality of the studies by employing more robust controlled designs with randomisation, increased sample sizes with heterogeneous samples, and making active comparisons between interventions used.

ObjectivesTo explore reasons for the lack of uptake of ‘Better Care Better Value (BCBV)’ prescribing indicators for renin–angiotensin–aldosterone system (RAAS) inhibitors and identify learning lessons to inform the implementation of future prescribing policies.DesignIn-depth, semistructured interviews to explore: general practitioners’ (GPs) experiences in prescribing RAAS, perceptions of the BCBV policy and potential barriers to policy implementation and suggestions for improving future policy implementation. Interviews were audio recorded, transcribed verbatim and analysed thematically, then mapped onto behavioural change frameworks (the Capability, Opportunity, Motivation and Behaviour) model and Behaviour Change Wheel (BCW)).SettingPrimary care setting in EnglandParticipantsInterviews were conducted with 16 GPs recruited from a purposive sample of 91 GP practices in three English counties.ResultsFour factors/barriers, related mainly to GPs’ psychological capability and reflective motivation, emerged as the possible barriers for the BCBV’s lack of uptake, including: lack of the policy awareness, negative attitude to the policy, lack of incentives and GPs’ reluctance to switch patients from angiotensin receptor blockers (ARBs) to ACE inhibitors (ACEIs). The participating GPs proposed interventions to improve future BCBV implementation and they were related to six intervention/policy functions of the BCW, addressing the four identified barriers: education/communication (increase GPs’ awareness) and environmental restructuring/regulations (provide GPs with reminding alerts); incentivisation/fiscal (provide GPs with financial incentives); enablement/guidelines-regulations (provide GPs with benchmarking against peers) and enablement/regulations and education/guidelines (facilitate switching from ARBs to ACEIs).ConclusionsThe main reason underpinning the low uptake of the BCBV indicator appears to be lack of a proactive implementation strategy. This case study demonstrated that passively disseminating policy without an effective implementation strategy results in low uptake. Furthermore, multifaceted implementation strategies are necessary to influence complex clinical decision making in a time-limited environment, such as prescribing behaviours. These findings suggest that effective policy implementation requires the application of a systematic comprehensive behaviours change framework.

The Dark Energy Spectroscopic Instrument (DESI) is a new instrument currently under construction for the Mayall 4-m telescope at Kitt Peak National Observatory. It will consist of a wide-field optical corrector with a 3.2 degree diameter field of view, a focal plane with 5,000 robotically controlled fiber positioners and 10 fiber-fed broad-band spectrographs. The DESI Instrument Control System (ICS) coordinates fiber positioner operations, interfaces to the Mayall telescope control system, monitors operating conditions, reads out the 30 spectrograph CCDs and provides observer support and data quality monitoring. In this article, we summarize the ICS design, review the current status of the project and present results from a multi-stage test plan that was developed to ensure the system is fully operational by the time the instrument arrives at the observatory in 2019.

The success of an economic sanctions episode, as viewed from the perspective of the sanctioning country, has two parts: the extent to which the foreign policy outcome sought by the sender country was, in fact, achieved; and the contribution made by the sanctions. Sanctions in most of the post-World War period had a distinctly unilateral flavor, often with a Cold War twist. The inevitable decline of American postwar hegemony has substantially reduced the utility of unilateral US economic sanctions. The civil war in the former Yugoslav republics vividly illustrates the difficulties in organizing effective multilateral cooperation. The comparison of the Iraq and Yugoslav cases supports the general conclusion that, especially when the goal is ambitious, sanctions should be imposed quickly and comprehensively. To maximize the chances for a successful outcome, the leaders of the sanctioning coalition should also try to even out the distribution of the costs, providing assistance to the hardest hit.

We describe the first public data release of the Dark Energy Survey, DES DR1, consisting of reduced single epoch images, coadded images, coadded source catalogs, and associated products and services assembled over the first three years of DES science operations. DES DR1 is based on optical/near-infrared imaging from 345 distinct nights (August 2013 to February 2016) by the Dark Energy Camera mounted on the 4-m Blanco telescope at Cerro Tololo Inter-American Observatory in Chile. We release data from the DES wide-area survey covering ~5,000 sq. deg. of the southern Galactic cap in five broad photometric bands, grizY. DES DR1 has a median delivered point-spread function of g = 1.12, r = 0.96, i = 0.88, z = 0.84, and Y = 0.90 arcsec FWHM, a photometric precision of < 1% in all bands, and an astrometric precision of 151 mas. The median coadded catalog depth for a 1.95" diameter aperture at S/N = 10 is g = 24.33, r = 24.08, i = 23.44, z = 22.69, and Y = 21.44 mag. DES DR1 includes nearly 400M distinct astronomical objects detected in ~10,000 coadd tiles of size 0.534 sq. deg. produced from ~39,000 individual exposures. Benchmark galaxy and stellar samples contain ~310M and ~ 80M objects, respectively, following a basic object quality selection. These data are accessible through a range of interfaces, including query web clients, image cutout servers, jupyter notebooks, and an interactive coadd image visualization tool. The Dark Energy Spectroscopic Instrument (DESI) is under construction to measure the expansion history of the universe using the baryon acoustic oscillations technique. The spectra of 35 million galaxies and quasars over 14,000 square degrees will be measured during a 5-year survey. A new prime focus corrector for the Mayall telescope at Kitt Peak National Observatory will deliver light to 5,000 individually targeted fiber-fed robotic positioners. The fibers in turn feed ten broadband multi-object spectrographs. We describe the ProtoDESI experiment, that was installed and commissioned on the 4-m Mayall telescope from August 14 to September 30, 2016. ProtoDESI was an on-sky technology demonstration with the goal to reduce technical risks associated with aligning optical fibers with targets using robotic fiber positioners and maintaining the stability required to operate DESI. The ProtoDESI prime focus instrument, consisting of three fiber positioners, illuminated fiducials, and a guide camera, was installed behind the existing Mosaic corrector on the Mayall telescope. A Fiber View Camera was mounted in the Cassegrain cage of the telescope and provided feedback metrology for positioning the fibers. ProtoDESI also provided a platform for early integration of hardware with the DESI Instrument Control System that controls the subsystems, provides communication with the Telescope Control System, and collects instrument telemetry data. Lacking a spectrograph, ProtoDESI monitored the output of the fibers using a Fiber Photometry Camera mounted on the prime focus instrument. We introduced facial electromyography as a tool for predicting advantaged group members' engagement in antidiscrimination action on behalf of a disadvantaged outgroup. Heterosexual men's corrugator supercilii (brow “frowning” muscles) activity while viewing videos of male–male and male–female couples interacting was measured. Corrugator (negative affect) response to male–male versus male–female targets, but not self-reported attitudes toward gay men, predicted number of flyers calling for action to reduce antigay violence and discrimination that participants privately took to distribute. Our discreet behavioral measure mirrored real-life collective action possibilities such as voting against laws prohibiting same-sex marriage in the privacy of one's voting booth.
