Denoising reconstruction techniques can introduce nonlinear properties into computed tomography (CT) systems. These nonlinear algorithms introduce distortion which affects the assessment of the resolution of the system. The purpose of the present study was to decouple and investigate amplitude modulation and waveform distortion in reconstruction algorithms in CT. The methodology developed by Wells, J. R. and Dobbins, J. T. III [Frequency response and distortion properties of nonlinear image processing algorithms and the importance of imaging context. Med. Phys. 40, 091906 (2013)] was adapted to CT reconstruction algorithms. The CT simulating program ASTRA Toolbox© for MATLAB™ was used for the reconstruction of the sinusoidal wave functions. Filtered back projection and the simultaneous iterative reconstruction technique were investigated with simple nonlinear mechanisms: a median filter and a non-negative constraint, respectively. The native reconstruction algorithms were not free from nonlinear waveform distortion, however, none of the metrics showed any dependence on the contrast-to-noise ratio (CNR). Furthermore, the algorithms including nonlinear mechanisms showed a clear and specific CNR dependence, indicating the necessity for distortion analysis in nonlinear CT reconstruction.

This study compares dual-energy computed tomography (DECT) images of a phantom including different material inserts and with additional lateral titanium or stainless steel inserts, simulating bilateral hip prostheses. Dual-source (DS) and fast kV-switching (FKS) DECT with/without metal artefact reduction (MAR) were compared with regards to virtually monoenergetic CT number accuracy and the depiction of different materials. Streak artefacts were observed between the metal inserts that were more severe with steel compared to titanium inserts. The artefact severity and CT number accuracy depended on the photon energy (keV) for both DECT techniques. While MAR generally increased the CT number accuracy and material depiction within the streak artefacts, it sometimes decreased the accuracy outside the streak artefacts for both DS and FKS. FKS depicted the metal inserts more accurately than DS with regards to both CT numbers and external diameter.

Background and purpose: Metal artefacts in computed tomography (CT) images impairs structure delineation. These artefacts can potentially be reduced with dual-energy CT (DECT) with or without using metal artefact reduction (MAR). The purpose was to investigate how structure delineation in DECT with or without MAR and single-energy CT (SECT) images were affected by metals. Materials and methods: A phantom with known irregular structures was developed. Reference structures were determined from a low-noise scan without metal. Bilateral hip prostheses were simulated with steel or titanium inserts. The phantom was scanned with SECT and fast-kV switching DECT with optional MAR. Four radiation oncologists delineated the structures in two phantom set-ups. Delineated structures were evaluated with Dice similarity coefficient (DSC) and Hausdorff distance relative to the reference structures. Results: With titanium inserts, more structures were detected for non-MAR DECT compared to SECT while the same or less were detected with steel inserts. MAR improved delineation in DECT images. For steel inserts, three structures in the region of artefacts, were delineated by at least two oncologists with MAR-DECT compared to none with non-MAR DECT or SECT. The highest values of DSC for MAR-DECT were 0.69, 0.81 and 0.77 for those structures. Conclusions: Delineation was improved with non-MAR DECT compared to SECT, especially for titanium inserts. A larger improvement was seen with the use of MAR for both steel and titanium inserts. The improvement was dependent on the location of the structure relative to the inserts, and the structure contrast relative to the background. The purpose of this study was to investigate the effect of different combinations of convolution kernel and the level of Adaptive Statistical iterative Reconstruction (ASiR™) on diagnostic image quality as well as visualisation of anatomical structures in paediatric abdominal computed tomography (CT) examinations. Thirty-five paediatric patients with abdominal pain with non-specified pathology undergoing abdominal CT were included in the study. Transaxial stacks of 5-mm-thick images were retrospectively reconstructed at various ASiR levels, in combination with three convolution kernels. Four paediatric radiologists rated the diagnostic image quality and the delineation of six anatomical structures in a blinded randomised visual grading study. Image quality at a given ASiR level was found to be dependent on the kernel, and a more edge-enhancing kernel benefitted from a higher ASiR level. An ASiR level of 70 % together with the Soft™ or Standard™ kernel was suggested to be the optimal combination for paediatric abdominal CT examinations.

The purpose of this study was to investigate the effect of adaptive statistical iterative reconstruction (ASiR) on the visualisation of anatomical structures and diagnostic image quality in paediatric cerebral computed tomography (CT) examinations. Forty paediatric patients undergoing routine cerebral CT were included in the study. The raw data from CT scans were reconstructed into stacks of 5 mm thick axial images at various levels of ASiR. Three paediatric radiologists rated six questions related to the visualisation of anatomical structures and one question on diagnostic image quality, in a blinded randomised visual grading study. The evaluated anatomical structures demonstrated enhanced visibility with increasing level of ASiR, apart from the cerebrospinal fluid space around the brain. In this study, 60 % ASiR was found to be the optimal level of ASiR for paediatric cerebral CT examinations. This shows that the commonly used 30 % ASiR may not always be the optimal level.

Aims To evaluate subjective image quality for two diagnostic tasks, periapical diagnosis and implant planning, for cone beam computed tomography (CBCT) using different exposure parameters and fields of view (FOVs). Materials and methods Examinations were performed in posterior part of the jaws on a skull phantom with 3D Accuitomo (FOV 3cm×4cm) and 3D Accuitomo FPD (FOVs 4cm×4cm and 6cm×6cm). All combinations of 60, 65, 70, 75, 80kV and 2, 4, 6, 8, 10mA with a rotation of 180° and 360° were used. Dose-area product (DAP) value was determined for each combination. The images were presented, displaying the object in axial, cross-sectional and sagittal views, without scanning data in a random order for each FOV and jaw. Seven observers assessed image quality on a six-point rating scale. Results Intra-observer agreement was good ( κ w =0.76) and inter-observer agreement moderate ( κ w =0.52). Stepwise logistic regression showed kV, mA and diagnostic task to be the most important variables. Periapical diagnosis, regardless jaw, required higher exposure parameters compared to implant planning. Implant planning in the lower jaw required higher exposure parameters compared to upper jaw. Overall ranking of FOVs gave 4cm×4cm, 6cm×6cm followed by 3cm×4cm. Conclusions This study has shown that exposure parameters should be adjusted according to diagnostic task. For this particular CBCT brand a rotation of 180° gave good subjective image quality, hence a substantial dose reduction can be achieved without loss of diagnostic information.

The purpose was to demonstrate a non-parametric statistical method that can identify and explain the components of observer disagreement in terms of systematic disagreement as well as additional individual variability, in visual grading studies. As an example, the method was applied to a study where the effect of reduced tube current on diagnostic image quality in paediatric cerebral multidetector CT (MDCT) images was investigated. Quantum noise, representing dose reductions equivalent to steps of 20 mA, was artificially added to the raw data of 25 retrospectively selected paediatric cerebral MDCT examinations. Three radiologists, blindly and randomly, assessed the resulting images from two different levels of the brain with regard to the reproduction of high- and low-contrast structures and overall image quality. Images from three patients were assessed twice for the analysis of intra-observer disagreement. The intra-observer disagreement in test-retest assessments could mainly be explained by a systematic change towards lower image quality the second time the image was reviewed. The inter-observer comparisons showed that the paediatric radiologist was more critical of the overall image quality, while the neuroradiologists were more critical of the reproduction of the basal ganglia. Differences between the radiologists regarding the extent to which they used the whole classification scale were also found. The statistical method used was able to identify and separately measure a presence of bias apart from additional individual variability within and between the radiologists which is, at the time of writing, not attainable by any other statistical approach suitable for paired, ordinal data.

The aim of this study was to evaluate the possibility of reducing the radiation dose to paediatric patients undergoing computed tomography (CT) brain examination by using image-enhancing software. Artificial noise was added to the raw data collected from 20 patients aged between 1 and 10 y to simulate tube current reductions of 20, 40 and 60 mA. All images were created in duplicate; one set of images remained unprocessed whereas the other was processed with image-enhancing software. Three paediatric radiologists assessed the image quality based on their ability to visualise the high- and low-contrast structures and their overall impression of the diagnostic value of the image. For patients aged 6-10 y, it was found that dose reductions from 27 mGy (CTDI(vol)) to 23 mGy (15 %) in the upper brain and from 32 to 28 mGy (13 %) in the lower brain were possible for standard diagnostic CT examinations when using the image-enhancing filter. For patients 1-5 y, the results for standard diagnostics in the upper brain were inconclusive, for the lower brain no dose reductions were found possible.

The purpose of the present work was to describe a method of using an imaging plate from a computed radiography system to determine the computed tomography (CT) dose profile (the tritium method) and to compare this method with point-dose measurements using a solid-state detector (CT Dose Profiler; RTI Electronics, Mölndal, Sweden) and the indirect method of comparing the air kerma-length product (P(KL)) at different beam collimations. The three methods were used to determine the full width at half maximum (FWHM) of the dose profile of a multi-slice CT at different nominal beam collimations. For all beam collimations, the obtained deviation between the tritium method and the CT Dose Profiler was smaller than 0.1 mm. The maximum relative error was 2 %. For the P(KL) method, the deviation from the CT Dose Profiler was between 0.2 and 0.4 mm, resulting in a relative error larger than 10 % for the smallest beam collimation even after normalisation to a known FWHM. In conclusion, the proposed method of using an imaging plate to determine the FWHM of the CT dose profile has a high accuracy and shows good agreement with the more advanced method of point-dose measurements using a solid-state detector.

The aim of this study was to investigate the effect of tube current on diagnostic image quality in paediatric cerebral multidetector CT (MDCT) images in order to identify the minimum radiation dose required to reproduce acceptable levels of different diagnostic image qualities. Original digital scanning data (raw data) were selected retrospectively from routine MDCT brain examinations of 25 paediatric patients. All examinations had been performed using axial scanning on an eight-slice MDCT (LightSpeed Ultra, GE Healthcare). Their ages ranged from newborn to 15 years. Quantum noise was added artificially to the raw data representing dose reductions equivalent to steps of 20 mA. Patient identification information was removed. Three experienced radiologists blindly and randomly assessed the resulting images from two different levels of the brain with regard to reproduction of structures and overall image quality. Final data were evaluated using the non-parametric statistical approach of inter-scale concordance. The minimum value of tube current-time product (mAs) required to reproduce an image of sufficient diagnostic quality was established in relation to the age of the patient. The corresponding CT dose index values by volume (CTDI(vol) (mGy)) were also established. In conclusion, acceptable reproduction of low-contrast structures was possible at CTDI(vol) values down to 20 mGy (patients 1-5 years old). For acceptable reproduction of high-contrast structures, CTDI(vol) values down to 10 mGy were considered possible (patients 1-5 years old). The original image quality for patients under 6 months of age (15 mGy) was found to be inadequate for acceptable reproduction of low-contrast structures.

This study evaluates two methods for calculating effective dose, CT dose index (CTDI) and dose-area product (DAP) for a cone beam CT (CBCT) device: 3D Accuitomo at field size 30x40 mm and 3D Accuitomo FPD at field sizes 40x40 mm and 60x60 mm. Furthermore, the effective dose of three commonly used examinations in dental radiology was determined.CTDI(100) measurements were performed in a CT head dose phantom with a pencil ionization chamber connected to an electrometer. The rotation centre was placed in the centre of the phantom and also, to simulate a patient examination, in the upper left cuspid region. The DAP value was determined with a plane-parallel transmission ionization chamber connected to an electrometer. A conversion factor of 0.08 mSv per Gy cm(2) was used to determine the effective dose from DAP values. Based on data from 90 patient examinations, DAP and effective dose were determined.CTDI(100) measurements showed an asymmetric dose distribution in the phantom when simulating a patient examination. Hence a correct value of CTDI(w) could not be calculated. The DAP value increased with higher tube current and tube voltage values. The DAP value was also proportional to the field size. The effective dose was found to be 11-77 microSv for the specific examinations.DAP measurement was found to be the best method for determining effective dose for the Accuitomo. Determination of specific conversion factors in dental radiology must, however, be further developed.

The diagnostic image quality of 75 paediatric abdominal computed tomography (CT) examinations reconstructed with two different iterative reconstruction (IR) algorithms-adaptive statistical IR (ASiR™) and model-based IR (Veo™)-was compared. Axial and coronal images were reconstructed with 70 % ASiR with the Soft™ convolution kernel and with the Veo algorithm. The thickness of the reconstructed images was 2.5 or 5 mm depending on the scanning protocol used. Four radiologists graded the delineation of six abdominal structures and the diagnostic usefulness of the image quality. The Veo reconstruction significantly improved the visibility of most of the structures compared with ASiR in all subgroups of images. For coronal images, the Veo reconstruction resulted in significantly improved ratings of the diagnostic use of the image quality compared with the ASiR reconstruction. This was not seen for the axial images. The greatest improvement using Veo reconstruction was observed for the 2.5 mm coronal slices.

In 2007, the International Commission on Radiological Protection (ICRP) updated the methodology for determining the effective dose, and the salivary glands were assigned tissue-specific weighting factors for the first time. The aims of this study were to determine the absorbed dose to the organs and to calculate, applying the ICRP publication 103 tissue-weighting factors, the effective doses delivered during digital intraoral and panoramic radiography. Methods: Thermoluminescent dosemeter measurements were performed on an anthropomorphic head and neck phantom. The organ-absorbed doses were measured at 30 locations, representing different radiosensitive organs in the head and neck, and the effective dose was calculated according to the ICRP recommendations. Results: The salivary glands and the oral mucosa received the highest absorbed doses from both intraoral and panoramic radiography. The effective dose from a fullmouth intraoral examination was 15mSv and for panoramic radiography, the effective dose was in the range of 19-75 mSv, depending on the panoramic equipment used. Conclusion: The effective dose from a full-mouth intraoral examination is lower and that frompanoramic radiography is higher than previously reported. Clinicians should be aware of the higher effective dose delivered during panoramic radiography and the risk-benefit profile of this technique must be assessed for the individual patient. Advances in knowledge: The effective dose of radiation from panoramic radiography is higher than previously reported and there is large variability in the delivered radiation dosage among the different types of equipment used.

The aim of this study was to compare the image quality of storage phosphor plates with that in screen-film radiograms in mammography. Two anode/filter combinations were also compared – Mo/Mo and W/Rh. S Storage phosphor plates, generation IIIN (Fuji, Tokyo, Japan) and a conventional screen-film system (Kodak, Rochester, N. Y.) were evaluated using two mammographic units. One unit had a 0.6-mm focal spot, an anode/filter combination of Mo/Mo and no grid (AMo); the other had a 0.3-mm focal spot, a grid, and two possible combinations of anode/filter Mo/Mo (BMo) and W/Rh (BW). Simulated tumours and microcalcifications were randomly positioned in an anthropomorphic breast phantom (RMI model 165, no. 210–009, Radiation Measurements Inc., Middleton, Wisconsin). The image quality was evaluated using a modified version of receiver operating characteristics analysis. Five observers evaluated 300 films and 300 hard copy images each. Radiation doses were also determined. The image quality of the conventional screen-film images was significantly better than that for the storage phosphor plate mammograms. The BMo system rated best, for the detection of both tumours and microcalcifications, although it was not significantly different from the BW system. Systems BMo and BW rated significantly better than the AMo system for both image receptors studied. The mean absorbed dose was twice as high for the BMo system as for the AMo and BW systems for both conventional and digital technique. The mammograms produced with the screen-film combination gave a significantly better detectability than the storage phosphor plates used in this study. Substantial dose reduction could be achieved using an anode/filter combination of W/Rh instead of Mo/Mo with no significant loss of information in the images.

Background Measurements of attenuation (in Hounsfield units [HU]) and contrast wash-out are widely used to characterize adrenal lesions as benign or indeterminate/malignant at computed tomography (CT). Clinical experience suggests that such measurements of adrenal lesions may vary over time and between observers, making evaluation difficult. Purpose To investigate the change over time of adrenal lesion size, attenuation, and contrast wash-out at CT, to determine inter-observer variability, and to analyze other factors underlying the variability. Material and Methods In a cohort of patients, with or without malignant disease, undergoing CT, adrenal lesions were prospectively analyzed. Lesions with growth >20% or >5 mm over 6 months were excluded. Non-enhanced attenuation and contrast medium wash-out over 2-year follow-up were analyzed. An inter-observer analysis with five observers and a phantom study of eight different CT scanners were performed to assess measurement variability. Results Mean adrenal lesion non-enhanced attenuation values decreased by 0.5 HU/year during follow-up. Using 10 HU or 40% relative wash-out as threshold values for benign versus indeterminate lesions, 27 (20%) and 39 (29%) of 136 lesions, respectively, would be reclassified at some occasion during follow-up. In the observer analysis 37 of 40 lesions demonstrated agreement between all observers, using established threshold values. The phantom study showed an intra-scanner variability of 1–3 HU, but an inter-scanner variability of up to 8 HU for water. Conclusion The clinically widespread use of specific attenuation threshold values for characterizing adrenal lesions must be used with great caution, considering that multiple factors, related to patient, equipment, scanning technique, and observer influence the outcome.

A wide variety of X-ray equipment is used today in dental radiology, including intra-oral, orthopantomographic, cephalometric, cone-beam computed tomography (CBCT) and computed tomography (CT). This raises the question of how the radiation risks resulting from different kinds of examinations should be compared. The risk to the patient is usually expressed in terms of effective dose. However, it is difficult to determine its reliability, and it is difficult to make comparisons, especially when different modalities are used. The classification of the new CBCT units is also problematic as they are sometimes classified as CT units. This will lead to problems in choosing the best dosimetric method, especially when the examination geometry resembles more on an ordinary orthopantomographic examination, as the axis of rotation is not at the centre of the patient, and small radiation field sizes are used. The purpose of this study was to present different methods for the estimation of the effective dose from the equipment currently used in dental radiology, and to discuss their limitations. The methods are compared based on commonly used measurable and computable dose quantities, and their reliability in the estimation of the effective dose.

The purpose of the present work was to investigate the reliability of subjective assessments of the low-contrast visibility in constancy control of computed tomography (CT). Axial CT images of a low-contrast phantom were acquired on an 8-slice multi-detector CT scanner at nine tube current settings ranging from 75 to 440 mA. Five medical physicists assessed the visibility of the low-contrast details in two sessions. In the first session, containing 54 images, the visibility was rated on an absolute scale by determining the number of visible details in each contrast group in each image. In the second session, 180 image pairs were presented to the observers with the task of determining if the two images had been acquired under identical conditions or not. In the absolute session, both the intra- and inter-observer variabilities were high. In the relative session, the variability was smaller, but an exposure difference of 50 % was needed for all observers to correctly identify a change in all cases. In conclusion, the present study indicates that subjective assessments of the low-contrast visibility in constancy control of CT are not reliable.

Different types of X-ray equipment are used in dental radiology, such as intra-oral, panoramic, cephalometric, cone-beam computed tomography (CBCT) and multi-slice computed tomography (MSCT) units. Digital receptors have replaced film and screen-film systems and other technical developments have been made. The radiation doses arising from different types of examination are sparsely documented and often expressed in different radiation quantities. In order to allow the comparison of radiation doses using conventional techniques, i.e. intra-oral, panoramic and cephalometric units, with those obtained using, CBCT or MSCT techniques, the same units of dose must be used. Dose determination should be straightforward and reproducible, and data should be stored for each image and clinical examination. It is suggested here that air kerma-area product (PKA) values be used to monitor the radiation doses used in all types of dental examinations including CBCT and MSCT. However, for the CBCT and MSCT techniques, the estimation of dose must be more thoroughly investigated. The values recorded can be used to determine diagnostic standard doses and to set diagnostic reference levels for each type of clinical examination and equipment used. It should also be possible to use these values for the estimation and documentation of organ or effective doses. Background: High-resolution computed tomography is the image procedure of choice in the evaluation of interstitial lung disease. Multidetector-row computed tomography provides the possibility of simultaneous reconstruction of thin and thick slices from the same raw data, acquired from one single series. Thus, it may be tempting to exclude the step-and-shoot series. Purpose: To compare high-resolution computed tomography (HRCT step-and-shoot) from single-slice CT (SSCT) and 16-channel multidetector CT (MDCT) in terms of visibility and motion artifacts, and to investigate whether thin images reconstructed from helical MDCT are equal to or better than conventional HRCT by SSCT in terms of visibility and motion artifacts. Material and Methods: 20 patients underwent HRCT step-and-shoot by SSCT (SSCT step-and-shoot) and MDCT (MDCT step-and-shoot), and a helical MDCT acquisition (MDCT helical). Images from four anatomical levels were analyzed in random order regarding visibility and motion artifacts. Results: Visibility using MDCT step-and-shoot was significantly better than or equal to SSCT step-and-shoot for segmental bronchi and fissures, but not for subsegmental bronchi. For MDCT helical, visibility was equal to or better than SSCT step-and-shoot for segmental bronchi, but not for fissures and subsegmental bronchi. Concerning motion artifacts, MDCT step-and-shoot and MDCT helical were significantly better than or equal to SSCT step-and-shoot. Conclusion: The image quality (accounting for motion artifacts and visibility) of SSCT step-and-shoot and MDCT step-and-shoot is comparable. The visibility of anatomic structures in images from MDCT helical is inferior to HRCT step-and-shoot.

There is a need for tools that in a simple way can be used for the evaluation of image quality related to clinical requirements in mammography. The aim of this work was to adjust the present European image quality criteria to be relevant also for digital mammography images, and to use as simple and as few criteria as possible. A pilot evaluation of the new set of criteria was made with mammograms of 28 women from a General Electric Senographe 2000D full-field digital mammography system. One breast was exposed using the standard automatic exposure mode, the other using about half of that absorbed dose. Three experienced radiologists evaluated the images using visual grading analysis technique. The results indicate that the new quality criteria can be used for the evaluation of image quality related to clinical requirements in digital mammography in a simple way. The results also suggest that absorbed doses for the mammography system used may be substantially reduced.

The European Commission (EC) quality criteria for screen-film mammography are used as a tool to assess image quality. A new set of criteria was developed and initially tested in a previous study. In the present study, these criteria are further evaluated using screen-film mammograms that have been digitised, manipulated to simulate different image quality levels and reprinted on film. Expert radiologists have evaluated these manipulated images using both the original (EC) and the new criteria. A comparison of three different simulated dose levels reveals that the new criteria yield a larger separation of image criteria scores than the old ones. These results indicate that the new set of image quality criteria has a higher discriminative power than the old set and thus seems to be more suitable for evaluation of image quality in mammography.

To evaluate how anode-filter combinations influence image quality in and mean glandular dose to breasts of different thicknesses and compositions.Mammograms were obtained with a molybdenum (Mo) anode and a Mo filter at 26 kVp, a Mo anode and a rhodium (Rh) filter at 27 kVp, or a tungsten (W) anode and a Rh filter at 26 kVp in 965 women. One anode-filter-tube voltage combination was used in the right breast and another in the left. The mean glandular dose to each breast was calculated.Image contrast was highest in the Mo-Mo mammograms. However, depiction of the glandular tissue, pectoral muscle, and skin and subcutis was significantly (P.001) better with the Mo-Rh and the W-Rh than with the Mo-Mo combination. The average mean absorbed doses to the glandular tissue for W-Rh and Mo-Rh were 50% and 75%, respectively, of that for Mo-Mo.Breast thickness is the most important parameter in selection of an anode-filter-tube voltage combination. Compared with Mo-Mo, both Mo-Rh and W-Rh gave good image quality of the mammary gland and a considerably lower absorbed dose. Mo-Rh-27 kVp is recommended for breast thicknesses of 60 mm or less; W-Rh-26 kVp, for breast thicknesses of greater than 60 mm.
