Introduction: Dupilumab is a fully human monoclonal antibody that blocks interleukin-4 and interleukin-13, key drivers of type 2 helper T-cell (Th2)-mediated inflammatory response. It was the first biologic treatment approved for adult patients with inadequately-controlled moderate-to-severe atopic dermatitis (AD). Continuous collection of daily data practice is important in order to evaluate the real effectiveness and safety of dupilumab treatment. Methods: In this observational cohort study, we prospectively included all adult patients with moderate to severe AD treated with dupilumab in our portuguese dermatology center from July 2019 to April 2020. Baseline clinical data was initially collected and treatment effectiveness and safety were assessed after 16 weeks. Results: Twenty-five patients were included. All patients had been previously treated with systemic immunosuppressants. The estimated mean Eczema Area and Severity Index Score (EASI) decreased from 27.8 at baseline to 8.8 at week 16 (+/- 4 weeks). A ΔEASI 75 response was achieved by 58.3% of patients (ΔEASI 90 - 29.1%). Conjunctivitis was the main reported side-effect, affecting 20.8% of patients. Discussion: Our study showed a significant EASI reduction during the first 16-weeks of dupilumab treatment in adult patients with AD. Despite its overall safety, daily-practice data tend to report a higher risk of conjunctivitis than previously expected and we hence recommend that patients should be specifically informed about this possible side-effect.

The purpose of this study is to report the clinical features and outcomes of ocular surface toxicity following depatuxizumab mafoditin (ABT-414) therapy for unresectable glioblastoma. Ocular signs and symptoms of three patients treated with ABT-414 during a phase III trial for glioblastoma multiforme were evaluated. Both eyes of all patients were damaged during the week after the first infusion of the ABT-414 molecule. In all patients, mild-to-moderate keratitis could be ascertained, along with decreased visual acuity and blurred vision, as well as foreign-body sensation and redness. Symptoms and visual acuity improved 4 weeks. In conclusion, ABT-414 therapy may cause transient ocular surface toxicity. The initiation of artificial tears and lubricant ointment was enough to control the ocular surface signs and symptoms. A multidisciplinary approach, complete ophthalmologic monitorization, and elaboration of protocols are required to adequately manage these patients. RESUMO Nosso objetivo é relatar as características clínicas e os resultados da toxicidade na superfície ocular após a terapia com depatuxizumabe mafodotina (ABT-414) para glioblastoma irressecável. Os sinais e sintomas oculares de três pacientes que foram tratados com ABT-414 durante um estudo de fase III para glioblastoma multiforme foram avaliados. Ambos os olhos de todos os pacientes foram danificados durante a semana após a primeira infusão da molécula ABT-414. Em todos os pacientes, uma ceratite de leve a moderada pode ser verificada, juntamente com uma diminuição da acuidade visual e visão turva, bem como sensação de corpo estranho e vermelhidão. Os sintomas e a acuidade visual melhoraram em um período de 4 semanas. Em conclusão, a terapia com ABT-414 pode causar toxicidade transitória na superfície ocular. A iniciação com lágrimas artificiais e pomada lubrificante foi suficiente para controlar os sinais e sintomas na superfície ocular. Uma abordagem multidisciplinar, com acompanhamento oftalmológico completo e a elaboração de protocolos são necessários para o manejo adequado desses pacientes.

RESUMO Introdução: O dupilumab é um anticorpo monoclonal totalmente humano que bloqueia a interleucina-4 e a interleucina-13, elementos chave da resposta inflamatória mediada por células T auxiliares tipo 2 (Th2). Foi o primeiro tratamento biológico aprovado para doentes adultos com dermatite atópica (DA) moderada a grave, inadequadamente controlada. A colheita contínua de dados de prática clínica diária é importante para avaliar a real eficácia e segurança do tratamento com dupilumab. Métodos: Neste estudo de coorte observacional, foram incluídos prospectivamente todos os doentes adultos com DA moderada a grave tratados com dupilumab no Centro Hospitalar Lisboa Central de julho de 2019 a abril de 2020. Os dados clínicos iniciais foram recolhidos e a eficácia e segurança do tratamento foram avaliadas após 16 semanas. Resultados: Vinte e cinco doentes foram incluídos e todos tinham sido previamente tratados com imunossupressores sistêmicos. O EASI médio inicial diminuiu de 27,8 no início do estudo para 8,8 à semana 16 (+/- 4 semanas). Uma resposta ΔEASI 75 foi alcançada em 58,3% dos doentes (ΔEASI 90 - 29,1%). A conjuntivite foi o principal efeito adver so registado, afetando 20,8% dos pacientes. Discussão: O nosso estudo mostrou uma redução significativa do EASI durante as primeiras 16 semanas de tratamento com dupilumab em pacientes adultos com DA. Apesar de sua segurança geral, os dados da prática diária tendem a relatar um risco maior de conjuntivite do que o esperado anteriormente e, por isso, recomendamos que os doentes sejam informados especificamente sobre esse possível efeito adverso.

Apresenta-se o caso de uma adolescente de 17 anos, com urticária ao frio desde os 10, com reações imediatas diárias e de gravidade variável após contato mínimo com desencadeantes frios, apesar de terapêutica com anti-histamínico H1 e H2 e antagonista dos recetores dos leucotrienos. A doença tinha grande impacto na qualidade de vida, tendo ocorrido dois eventos sistémicos ameaçadores de vida. Iniciou tratamento com omalizumab, verificando-se melhoria após a primeira administração, e após a quinta sem necessidade de outra terapêutica. Cumpriu oito doses mensais de 300 mg de omalizumab, sem efeitos adversos, ficando assintomática. A doente foi reavaliada aos quatro e 10 meses após suspensão do tratamento, apresentando apenas prurido ligeiro com a prática de exercício físico vigoroso, que melhora com uma toma de anti-histamínico H1., Portuguese Journal of Pediatrics, Vol. 48 No. 3 (2017)

Bevacizumab, a monoclonal anti-vascular endothelial growth factor antibody, has been suggested as a potential healing therapeutic following glaucoma surgery. Here, we aimed to improve the bioavailability of bevacizumab when used as an adjunct therapy to non-penetrating deep sclerectomy (DS) by using a bevacizumab-methylcellulose mixture (BMM). Ten previously non-operated eyes in ten patients diagnosed with primary open angle glaucoma underwent DS with a subconjunctival injection of 0.3 ml of BMM (bevacizumab 3.75 mg incorporated into 4% methylcellulose) at the surgical site. Bevacizumab release was evaluated in vitro using size-exclusion high performance liquid chromatography (HPLC). Intraocular pressure (IOP), bleb morphology, corneal endothelial cell count (CECC), and complications were evaluated at 6 months after surgery. Using HPLC, bevacizumab was detected in BMM for up to 72 h. Moreover, all surgical blebs remained expanded with hyaline material during the first week.
