- Abstract
Objectives: To assess the effectiveness of bictegravir/emtricitabine/tenofovir alafenamide (BIC/FTC/TAF) among people poorly represented in clinical trials and potentially at higher risk of suboptimal response to ART. Methods: Observational cohort study on persons with HIV (PWH) enrolled in ICONA who started BIC/FTC/TAF as initial therapy or as switching regimen while virologically suppressed. Primary endpoint was time to treatment failure (TF): new AIDS/death or virological failure (VF) or discontinuation for toxicity/failure. Secondary endpoints were time to treatment discontinuation for toxicity (TDT) and to VF. Groups of interest were those aged >50 years, female sex, and advanced HIV disease at first ART start. Probability of the events overall and according to groups and adjusted HR for every endpoint were calculated by Kaplan–Meier curves and Cox regression models. Results: Nine hundred and thirty-three ART-naive and 1655 ART-experienced PWH initiated BIC/FTC/TAF. Over a median follow-up of 69.8 weeks, 89 (9.6%) PWH at their first regimen experienced TF. PWH aged >50 years had 1.83-fold (95% CI: 1.19–2.83) higher risk of TF; PWH with advanced HIV disease had 2.21-fold (95% CI: 1.53–3.82) higher risk; there were no differences in TF according to sex. Over a median follow-up of 146.3 weeks, 109 (6.6%) out of 1655 switching PWH experienced TF; no differences were found in the risk of TF, TDT and VF according to groups of interest. Conclusions: Overall, BIC/FTC/TAF is well tolerated and virologically effective in the real-world scenario for ART-naive and -experienced PWH. Older ART-naive PWH and those with advanced HIV disease may respond less well as the burden of diseases might compromise treatment efficacy.

- Abstract
Background: Mpox virus (MPXV) has recently spread outside of sub-Saharan Africa. This large multicentre study was conducted in Lombardy, the most densely populated Italian region accounting for more than 40% of Italian cases. The present study aims to: i) evaluate the presence and the shedding duration of MPXV DNA in different body compartments correlating the MPXV viability with the time to onset of symptoms; ii) provide evidence of MPXV persistence in different body compartment as a source of infection and iii) characterize the MPXV evolution by whole genome sequencing (WGS) during the outbreak occurred in Italy. Material and methods: The study included 353 patients with a laboratory-confirmed diagnosis of MPXV infection screened in several clinical specimens in the period May 24th - September 1st, 2022. Viral isolation was attempted from different biological matrices and complete genome sequencing was performed for 61 MPXV strains. Results: MPXV DNA detection was more frequent in the skin (94.4%) with the longest median time of viral clearance (16 days). The actively-replicating virus in cell culture was obtained for 123/377 (32.6%) samples with a significant higher viral quantity on isolation positive samples (20 vs 31, p < 0.001). The phylogenetic analysis highlighted the high genetic identity of the MPXV strains collected, both globally and within the Lombardy region. Conclusion: Skin lesion is gold standard material and the high viral load and the actively-replicating virus observed in genital sites confirms that sexual contact plays a key role in the viral transmission.

- Abstract
Objectives: To compare the long-term risk of treatment failure of dolutegravir-based ART in men and women in a real-life setting. Patients and methods: Persons living with HIV (PLWH) from the ICONA cohort were included if they had started dolutegravir in a two- or three-drug regimen as ART-naive or as virologically controlled ART-experienced. The primary endpoint was time to treatment failure (virological/clinical failure or dolutegravir discontinuation). Secondary endpoints were: time to dolutegravir discontinuation due to toxicity and to neuropsychiatric adverse events; and time to virological failure. Cox regression analyses focused on differences in outcomes by sex. Results: A total of 2304 PLWH (15% women) initiated dolutegravir-based therapy from ART-naive, and 1916 (19.8% women) while experienced. After a median follow-up of 2.2 (IQR: 0.9-3.9) years in ART-naive and 2.4 (IQR: 1.1-4.3) years in experienced, the 4-year cumulative probability of treatment failure was 33% (95% CI 30.5-35.1) and 20% (95% CI 17.8-22.3), respectively. In the multivariable analyses, in ART-naive the risk of treatment failure was higher for women, but not different after excluding women discontinuing dolutegravir for pregnancy concerns. We also observed a higher risk of discontinuation for toxicity in women (ART-naives: Adjusted Hazard Ratio (AHR): 1.56%; 95% CI: 1.03-2.37; ART-experienced: AHR: 1.53%; 95% CI: 1.01-2.32), although the absolute 4-year probability was low: 7.7% (95% CI 6.5-9.2) in ART-naive and 8.3% (95% CI 6.9-9.9) in experienced. Conclusions: In our cohort of PLWH treated with dolutegravir-based regimens and followed up for up to 4 years, we observed a low risk of treatment failure and no evidence for a difference by sex, after excluding discontinuation due to pregnancy concerns. However, we observed a higher risk of dolutegravir discontinuation for toxicity in women.

The pseudorapidity density and multiplicity distribution of charged particles produced in proton-proton collisions at the LHC, at a centre-of-mass energy √s= 7 TeV, were measured in the central pseudorapidity region {pipe}η{pipe} < 1. Comparisons are made with previous measurements at √s = 0.9 TeV and 2.36 TeV. At √s = 7 TeV, for events with at least one charged particle in {pipe}η{pipe} < 1, we obtain dNch/dη = 6.01 ± 0.01(stat.)+0.20-0.12(syst.). This corresponds to an increase of 57.6% ± 0.4%(stat.)+3.6-1.8%(syst.) relative to collisions at 0.9 TeV, significantly higher than calculations from commonly used models. The multiplicity distribution at 7 TeV is described fairly well by the negative binomial distribution. © 2010 CERN for the benefit of the ALICE collaboration.

On 23rd November 2009, during the early commissioning of the CERN Large Hadron Collider (LHC), two counter-rotating proton bunches were circulated for the first time concurrently in the machine, at the LHC injection energy of 450 GeV per beam. Although the proton intensity was very low, with only one pilot bunch per beam, and no systematic attempt was made to optimize the collision optics, all LHC experiments reported a number of collision candidates. In the ALICE experiment, the collision region was centred very well in both the longitudinal and transverse directions and 284 events were recorded in coincidence with the two passing proton bunches. The events were immediately reconstructed and analyzed both online and offline. We have used these events to measure the pseudorapidity density of charged primary particles in the central region. - Abstract
Chronic liver disease induced by hepatitis D virus (HDV) infection is associated with autoimmune manifestations. The present study investigated the prevalence of autoantibodies (auto-ab) and their association with clinical and virologic features in patients with chronic HDV infection. We studied 494 patients with chronic HDV infection from the HDV Describe study cohort. Non-organ-specific auto-ab (e.g., anti-nuclear auto-ab [ANA], anti-smooth muscle auto-ab [ASMA], anti-mitochondrial auto-ab [AMA], and anti-liver-kidney microsomal auto-ab [LKM]) were tested in diluted serum samples (1:80) by indirect immunofluorescence (IIF) on rat kidney/stomach/liver tissue slides (Euroimmun, Germany). Selected serum samples were analysed by immunoblot (IB) (Euroimmun, Germany) to detect antigen-specific auto-ab. The prevalence of ANA, ASMA, AMA, and LKM by IIF was 9.9% (n=49), 12.3% (n=61), 2.0% (n=10), and 1.8% (n=9), respectively. Among LKM+ patients, only one patient was LKM-1+ at IB. A high proportion of patients had anti-brush border auto-ab (n=99; 20.0%); 9 patients were anti-parietal cells auto-ab+, 1 was anti-reticulin auto-ab+, and 1 was anti-lysosome auto-ab+. ASMA+ was associated with liver cirrhosis (OR=2.00, 1.08–3.70) and HDV RNA >3 Log IU/mL (OR=1.91, 1.03–3.52), whereas no association was observed between ASMA+ and ALT >40 U/L (OR=1.12, 0.65–1.93). At multivariate analysis, ASMA+ resulted significantly associated with liver cirrhosis (OR=1.89, 1.01–3.51) independently from HDV RNA (OR=1.57, 1.01–2.42) and ALT (OR=1.22, 0.79–1.87). The assessment of circulating auto-ab by IIF may represent a valuable and inexpensive tool for identifying patients with high HDV replication and advanced liver disease. - Abstract
Migratory flows are reconstituting the HDV reservoir in Italy, displacing the infection in Italians. To compare the features of contemporary Chronic Hepatitis D (CHD) in native Italians (It) and foreign-born (Fb) patients. Consecutive HBsAg carriers positive for antibodies to HDV (anti-HD) referred to 32 Italian Centers, were prospectively enrolled from 08/2022 to 07/2024. Sera were centralized in Torino for virologic assessment. Overall, 432 of 515 (83.9%) patients were HDV-RNA-positive (4.39, IQR 1.30–5.82 Log IU/mL; 99.0% HDV genotype-1). HDV-RNA levels correlated with ALT (r s =0.58, 0.51–0.63) and liver stiffness (r s =0.28, 0.19–0.37). The 317 It (61%) were older than the 198 Fb (39%) (median age 60, IQR 55–65 vs 46, IQR 39–54 years; p<0.001) with more advanced liver disease (cirrhosis: 223/317 [70.3%] vs 100/198 [50.5%]; p<0.001). Most (70.3%) of the ageing It had a cirrhosis acquired long ago. However, 29.7% (94/317) had no cirrhosis: their features denoted an active and viremic CHD in 44.7% (42/94) and an inactive/minimal disease with low viremia (<3 Log) in 55.3% (52/94), presumably acquired as a slow infection decades before. Despite the younger age, Fb already exhibited a 50.5% rate (100/198) of symptom-free compensated cirrhosis (median albumin: 4.1, IQR 3.8–4.4 g/dL); in the other 45.5% (98/198), 50 (51.0%) had an active CHD and 48 (49.0%) an inactive/mild infection with low viremia (<3 Log). In Italy, the current scenario of chronic HDV infection is more heterogeneous than expected, changing the perspective of CHD as a most severe disease; about a quarter of It and Fb had a lingering less virulent CHD, and half of Fb presented with recently acquired cirrhosis, which was compatible with a stable clinical condition. - Abstract
Discontinuation of antimicrobial stewardship programs (ASPs) and increased antibiotic use were described during SARS-CoV-2 pandemic. In order to measure COVID-19 impact on ASPs in a setting of high multidrug resistance organisms (MDRO) prevalence, a qualitative survey was designed. In July 2021, eighteen ID Units were asked to answer a questionnaire about their hospital characteristics, ASPs implementation status before the pandemic and impact of SARS-CoV-2 pandemic on ASPs after the 1st and 2nd pandemic waves in Italy. Nine ID centres (50%) reported a reduction of ASPs and in 7 cases (38.9%) these were suspended. After the early pandemic waves, the proportion of centres that restarted their ASPs was higher among the ID centres where antimicrobial stewardship was formally identified as a priority objective (9/11, 82%, vs 2/7, 28%). SARS-CoV-2 pandemic had a severe impact in ASPs in a region highly affected by COVID-19 and antimicrobial resistance but weaknesses related to the pre-existent ASPs might have played a role.

Objectives We aimed to investigate and compare the efficacy and safety of two dolutegravir-based two-drug regimens: dolutegravir + lamivudine versus dolutegravir + rilpivirine. Methods We analysed a cohort of people living with HIV (PLWHIV) switching to dolutegravir + lamivudine or dolutegravir + rilpivirine. We excluded from the analysis PLWHIV with no available pre-switch genotypic test or with a known resistance mutation to one of the study drugs. We evaluated incidence of virological failure (VF) and treatment discontinuation (TD), as well as changes in immunological and metabolic parameters. Results We enrolled 592 PLWHIV: 306 in the lamivudine group and 286 in the rilpivirine group. We observed nine VFs in the lamivudine group [1.4 VF per 100 patient-years of follow-up (PYFU)] and four VFs in the rilpivirine group (0.6 VF per 100 PYFU). Subsequent genotypic analysis showed no acquired resistance-associated mutations in those experiencing VF. Estimated probability of maintaining virological suppression at 144 and 240 weeks were 96.6% and 92.7%, respectively, in the lamivudine group and 98.7% and 98.7%, respectively, in the rilpivirine group (log-rank P = 0.172). The estimated probability of maintaining study regimen at Week 240 was 82.3% in the lamivudine group and 85.9% in the rilpivirine group (log-rank P = 0.018). We observed a significant improvement in CD4+ cell count at Week 240 in the lamivudine group (P = 0.012); in the rilpivirine group we registered a significant increase in CD4/CD8 ratio (P = 0.014). Conclusions Both analysed strategies are effective and safe as switch strategies in clinical practice, with a low incidence of VF and a favourable immunological recovery, even in the long term.

High concentrations of ivermectin demonstrated antiviral activity against SARS-CoV-2 in vitro. The aim of this study was to assess the safety and efficacy of high-dose ivermectin in reducing viral load in individuals with early SARS-CoV-2 infection. This was a randomised, double-blind, multicentre, phase II, dose-finding, proof-of-concept clinical trial. Participants were adults recently diagnosed with asymptomatic/oligosymptomatic SARS-CoV-2 infection. Exclusion criteria were: pregnant or lactating women; CNS disease; dialysis; severe medical condition with prognosis 10viral load reduction was 2.9 (1.6) in arm C, 2.5 (2.2) in arm B and 2.0 (2.1) in arm A, with no significant differences (P = 0.099 and 0.122 for C vs. A and B vs. A, respectively). High-dose ivermectin was safe but did not show efficacy to reduce viral load. For anonymization purpose, the dates of recruitment have been shifted of an unspecifiednumber of days. However, timeframe between follow up visits remains fixed.

A severe outbreak of coronavirus disease 2019 (COVID-19) emerged in China in December 2019, and spread so rapidly that more than 200,000 cases have so far been reported worldwide; on January 30, 2020, the WHO declared it the sixth public health emergency of international concern. The two previously reported coronavirus epidemics (severe acute respiratory syndrome [SARS] and Middle East respiratory syndrome [MERS]) share similar pathogenetic, epidemiological and clinical features as COVID-19. As little is currently known about SARS-CoV-2, it is likely that lessons learned from these major epidemics can be applied to the new pandemic, including the use of novel immunosuppressive drugs.

- Abstract
Background: A growing body of observational evidence supports the value of ceftazidime-avibactam (CAZ-AVI) in managing infections caused by carbapenem-resistant Enterobacteriaceae. Methods: We retrospectively analyzed observational data on use and outcomes of CAZ-AVI therapy for infections caused by Klebsiella pneumoniae carbapenemase-producing K. pneumoniae (KPC-Kp) strains. Multivariate regression analysis was used to identify variables independently associated with 30-day mortality. Results were adjusted for propensity score for receipt of CAZ-AVI combination regimens versus CAZ-AVI monotherapy. Results: The cohort comprised 577 adults with bloodstream infections (n = 391) or nonbacteremic infections involving mainly the urinary tract, lower respiratory tract, and intra-abdominal structures. All received treatment with CAZ-AVI alone (n = 165) or with ≥1 other active antimicrobials (n = 412). The all-cause mortality rate 30 days after infection onset was 25% (146/577). There was no significant difference in mortality between patients managed with CAZ-AVI alone and those treated with combination regimens (26.1% vs 25.0%, P =. 79). In multivariate analysis, mortality was positively associated with presence at infection onset of septic shock (P =. 002), neutropenia (P <. 001), or an INCREMENT score ≥8 (P =. 01); with lower respiratory tract infection (LRTI) (P =. 04); and with CAZ-AVI dose adjustment for renal function (P =. 01). Mortality was negatively associated with CAZ-AVI administration by prolonged infusion (P =. 006). All associations remained significant after propensity score adjustment. Conclusions: CAZ-AVI is an important option for treating serious KPC-Kp infections, even when used alone. Further study is needed to explore the drug's seemingly more limited efficacy in LRTIs and potential survival benefits of prolonging CAZ-AVI infusions to ≥3 hours.

This paper explains the design and the construction of the prototype of an end-ladder card for the silicon drift detector of a high-energy physics experiment at CERN. The end-ladder performs data reduction, compression, and packing for a data acquisition chain. In particular, the end-ladder card includes a chip whose main function is to execute these tasks. Besides the chip's functions, the end-ladder card interfaces with the front-end electronics and processes and transmits the data set received from the front end through an optical channel to the counting room. The paper also describes this chip and the ongoing tests on its production. In addition, the paper shows details of the end-ladder prototype card's design and of the relative tests that are going to be carried out in the near future.
