La linfoadenectomia cervicale e una procedura comune per il carcinoma della tiroide. Alcune delle complicanze sono congruenti con quelle della chirurgia tiroidea, in particolare lesione del nervo laringeo ricorrente e ipoparatiroidismo, nonche sanguinamento e infezione della ferita chirurgica. Complicanze specifiche della dissezione linfonodale laterocervicale sono le lesioni dei nervi accessori, frenico e ipoglosso, del tronco del plesso cervicale e ancora lesioni delle ghiandole salivari e del sistema linfatico, in particolare lesioni a carico del dotto toracico. La maggior parte di queste complicanze e molto rara, con un’incidenza inferiore all’1%. Una profonda conoscenza anatomica e un’attenta tecnica di dissezione danno un contributo decisivo alla riduzione al minimo delle complicanze.

Le complicanze della chirurgia tiroidea sono rappresentate dall’ematoma compressivo, dalle lesioni a carico del nervo laringeo inferiore e dall’ipoparatiroidismo. Negli ultimi due decenni sono entranti nel mercato i cosiddetti Energy-Based Devices, dispositivi che hanno permesso, cosi come riportato nella maggior parte dei report in letteratura, una significativa riduzione del tempo medio operatorio rispetto alle tecniche tradizionali per la realizzazione delle procedure chirurgiche anche a carico della tiroide. L’uso routinario di questi nuovi dispositivi chirurgici ha determinato un aumento delle procedure chirurgiche annue che, nei centri ad alti volumi chirurgici, ha permesso di ammortizzare l’inevitabile aumento della spesa sanitaria.

Despite the increasingly innovative techniques developed in thyroid surgery to offer patients minimally invasive and scarless interventions, conventional open procedures still account for most of the interventions performed in this field. The surgical incision length has been significantly reduced, from 6-9 cm to 3 cm, and therefore patients perceive the scar to be highly acceptable. In this technical note, we present the use of a new single retractor (APOLLO®; AFS MEDICAL GmbH, Teesdorf, Austria) for conventional open thyroidectomies with intraoperative neuromonitoring. This device offers several advantages: a) better exposure of the surgical field; b) less traction on skin flaps and neck muscles; and c) protection of the skin edges from the heat generated by energy-based devices/coagulating instruments, with consequent better healing.

The aim of this paper is to discuss the risk of recurrence in patients with differentiated thyroid cancer and emphasize the importance of risk-group stratification, early recurrence identification and application of new imaging modalities, what is the PET-CT. Moreover, follow-up of patients with thyroid carcinoma should be carried out by specialized teams throughout life. Therefore, interdisciplinary case discussions in tumor conferences may improve the use of multimodal therapy especially in patients with poorly differentiated thyroid carcinomas. After baseline follow-up, if there is a suspicion of thyroid carcinoma, early PET-CT should be used for early detection and appropriate planning. Fortunately, due to the good localization possibility, the PET-CT enables a focused surgical procedure with avoidance of an unnecessary tumor search and thereby a reduction of the risk of injury of neighboring structures which is a concern with reoperative neck surgery.

Subcutaneous implantation of thyroid tissue is a rare clinical condition that involves the head and neck region and occurs after surgery, diagnostic procedures or cervical trauma.A 90-year old woman with two skin nodules on her thyroidectomy scar was hospitalized and treated by two surgical excisions. Histologically, these lesions were two aggregates of cutaneous oncocytic thyroid cells. In the patient's clinical history there was a total thyroidectomy for multinodular goiter, performed 9 years previously and at which a well-encapsulated subcapsular oncocytic adenoma of the left lobe was also incidentally discovered. At 12 months of follow-up, the patient is showed well and her wounds healed.Subcutaneous colonization or seeding of thyroid tissue is a rare occurrence reported in the literature for both benign and malignant pathologies; among the malignant ones, the implantation of follicular carcinoma cells is the most frequent. Only in one previous case, to our knowledge, subcutaneous colonization originating from oncocytic thyroid (or Hurthle) cell neoplasms has been described.We report an unusual case of double subcutaneous implantation of oncocytic thyroid cells on the cervical scar of an elderly woman, nine years after total thyroidectomy.

Cervical lymphadenectomy is a common procedure for thyroid cancer. Some of the complications are congruent with the complications of thyroid surgery, in particular recurrent laryngeal nerve paresis and hypoparathyroidism as well as bleeding and wound infection. Specific complications of lateral cervical lymph node dissection are injuries to the accessory, phrenic and hypoglossal nerves, and the cervical plexus trunk and injuries, the salivary glands, and the lymphatic system, especially the ductus thoracicus. Most of these complications are very rare with an incidence of1%. Profound anatomical knowledge and a careful dissection technique make a decisive contribution to minimizing complications.

Graphical abstract, Highlights • Risk of RLN palsy during thyroidectomy for Graves’ disease is high. • I-IONM and C-IONM are feasible and useful during thyroidectomy for Graves’ disease. • Proportion of complete thyroidectomy was significantly higher using IONM technology., We evaluate the role of intraoperative neuromonitoring (IONM) in thyroidectomy performed for Graves’ disease (GD) with an emphasis on recurrent laryngeal nerve (RLN) management and completeness of resection. The study is a retrospective series comprising 55 thyroidectomy (control group) versus 82 procedures with intermittent IONM (I-IONM) and 72 by means of continuous IONM (C-IONM). In the control group the laryngeal nerves have been identified by visualization solely. In the I-IONM group both vagal nerve (VN) and RLN have been localized and monitored during thyroid resection. C-IONM was achieved with a vagal stimulation probe. I-IONM group had shorter operating times (P = 0.032). RLN morbidity, meaning palsy rate, was 2.7% in the C-IONM group, 3.6% in I-IONM and 5.4% in the control group (P = 0.058). The proportion of complete procedures (total or near total resections) were significantly higher using monitoring technology (P = 0.049). Persistent positive serum TBII values were found in 25 (45%), 25 (30%) and 20 (27%) patients at 12 months in the control, I-IONM and C-IONM groups respectively (P = 0,04). IONM is an effective technology in GD patients.

Introduction: Post-thyroidectomy hemorrhage is a rare but potentially life-threatening and unpredictable complication of thyroid surgery. Therefore, intraoperative bleeding control and hemostasis are crucial. However, the most efficient, cost-effective, and standardized way to achieve this is not clear. This study aimed to evaluate the outcome of total thyroidectomy (TT) and partial thyroidectomy (PT) performed using the Vivostat® hemostatic system (Vivostat A/S, Lillerød, Denmark). Methods: Patients underwent TT and PT for benign and malignant diseases (multinodular goiter, Graves’ disease, differentiated thyroid carcinoma). The primary endpoint was 1st-day postoperative drain output and bleeding that required reintervention. Secondary endpoints included surgery duration and postsurgical complications (vocal fold palsy, hypocalcemia, seroma, wound infection). Results: Between October 2020 and December 2020, 56 patients were enrolled; 69.6% female; mean age 49.5 years. The mean 24-h drain output was 40 ml. No redo surgery was needed. Seroma was present in 5.3% of cases; no permanent vocal palsy or hypocalcemia was observed. Conclusion: This study shows that the Vivostat® system is both safe and effective for hemostasis during thyroid surgery.

The unfavorable behavior of primary thyroid carcinoma (PTC) has been revealed by the hematogenous distant metastases in ~20-25% of cases with frequent localizations in lungs and bones, but infrequently in kidney. A 69-year-old male patient was admitted to Department of Human Pathology in Adulthood and Childhood ‘G. Barresi’, University Hospital G. Martino, (Messina, Italy) for an incidentally detected parenchymal mass involving the right kidney. A partial nephrectomy was done; at the post-surgical examination, a large nodular grayish mass was documented. Microscopically, a diffuse proliferation with solid/follicular pattern with some colloid-filled spaces was appreciable. An intense immunopositivity was revealed for thyroglobulin, thyroid transcription factor-1 (TTF-1), paired-box gene 8 (PAX-8) and cytokeratin 7, while CD10 and renal cell carcinoma marker were negative. A diagnosis of the metastatic thyroid follicular carcinoma localized in the kidney was made. At ultrasound examination, a hyperechoic mass extending from the left thyroid lobe to the isthmus, to which TIR4 diagnostic category according to the Italian reporting system for thyroid cytology was attributed. After thyroid surgical procedure, the final diagnosis of primitive differentiated follicular thyroid carcinoma with foci of poorly differentiated component was made.

A new device for monitoring the laryngeal nerves during thyroid surgery has been developed. NIM Vital™ (Medtronic Xomed, Inc., Jacksonville, FL, USA) incorporates (a) a new wireless design, (b) NIM NerveTrendTM (Medtronic Xomed) EMG reporting, (c) intelligent noise-reduction technology that suppresses artifacts, (d) smart troubleshooting pop-up alerts, and (e) NIM Nervassure ™ (Medtronic Xomed) for continuous monitoring. This device offers enhanced stability and flexibility for both intermittent and continuous laryngeal nerve monitoring. The new NIM NerveTrend ™ EMG reporting makes it possible to track the recurrent laryngeal nerve condition throughout a procedure, even when using intermittent nerve monitoring. During both continuous and intermittent monitoring, green, yellow and red status bars provide visual information and associated tones provide audible cues, making it easy to monitor nerve function and interpret EMG trends. This new tool for laryngeal nerve monitoring has the potential to augment nerve dissection during surgery. Measurements of long-term outcome are needed to establish their efficacy.

Introduction: Thyroid and parathyroid diseases are very common. Most of these cases are in women and may be amenable to surgery. The patient’s perception that these are not life-threatening diseases leads them to expect an excellent aesthetic result, since the surgical incision area is clearly visible. Objective: To evaluate different scarring outcomes using three different energy-based devices (Harmonic Focus®, Johnson & Johnson, New Brunswick, NJ; Thunderbeat Open Fine Jaw®, Olympus Medical, Tokyo, Japan; LigaSure Small Jaw®, Medtronic, Dublin, Ireland) and to determine the impact of post-thyroidectomy/parathyroidectomy scars on the patient’s quality of life. Methods: One hundred female patients who underwent thyroidectomy or parathyroidectomy between September 2017 and September 2019 at the Endocrine and Minimally Invasive Surgery Department of Messina University Hospital were recruited. A retrospective analysis assessed the thickness of the cervical scar via ultrasound imaging, and the patient’s degree of satisfaction through the Patient and Observer Scar Assessment Scale (POSAS) and the Body Dysmorphic Disorder Questionnaire (BDDQ). Results: The patients were divided into three groups according to the energy-device used: group A (LigaSure SJ (n=38), group B (Harmonic F, n=32) and group C (Thunderbeat OFJ, n=30). The three groups were homogeneous with respect to number of patients, age and surgical procedures. The best aesthetic result, which correlated with the lowest scar thickness, was observed in group A; these patients were more satisfied than those in the other two groups. Moreover, correlations between scar thickness and quantitative variables (such as age or BMI) were not found in any of the groups. Conclusions: Based on the data collected and our experience, the LigaSure Small Jaw® (Medtronic) seems to offer the best aesthetic outcome in patients who undergo transverse cervicotomy for thyroid and parathyroid diseases. However, further prospective studies involving a greater number of cases are needed.

Due to the direct anatomical relationship between the recurrent laryngeal nerve (RLN) and the thyroid gland, the function and anatomical integrity of the RLN is fundamentally at risk in every thyroid operation. While a RLN morbidity rate of less than 5% is achieved in specialized clinics, the morbidity rates are significantly higher in non-specialized centers. Thus, the aim is to reduce the complication rate by establishing standardized interventions. Exact knowledge of the anatomical course of the RLN, the nerve-sparing dissection technique and the supportive use of intraoperative neuro-monitoring (IONM) to identify anatomical variations are the basis for nerve-sparing surgery. We tested the new C2 Xplore® system (inomed Medizintechnik GmbH, Emmendingen, Germany) as a tool for performing intermittent and continuous laryngeal nerve monitoring during thyroid surgery. The C2 Xplore® helps to enhance surgeon-IONM interaction, and provides comprehensive digital EMG documentation with EMG quantification. EMG artifacts are removed. Image quality and EMG feedback are highly acceptable for intraoperative monitoring. The C2 Xplore® system does not have a deleterious impact on the proper function of other surgical instruments. C2 Xplore® is effective for intraoperative monitoring, optimizing RLN dissection, and supporting surgical deliberations, and for forensic use and research. A step-by-step C2 Xplore® procedure is described.

Continuous intraoperative neural monitoring (C-IONM) during thyroid surgery is a useful tool for preventing recurrent laryngeal nerve (RLN) injury. The present study aims to analyze the tensile strength tolerance of C-IONM electrodes on the vagal nerve (VN). A C-IONM wire was enclosed in a hand-held tensile testing system. The probe displacement on the VN was continuously monitored by positioning a second probe far-up/proximally in a piglet model, and an automatic periodic stimulation (APS) accessory was used. The 3-mm and 2-mm APS accessory has a mean tensile strength of 20.6 ± 10 N (range, 14.6–24.4 N) and 11.25 ± 8 N (range, 8.4–15.6 N), respectively (P = 0.002). There was no difference between bilateral VNs. The mean amplitude before and during electrode displacement was 1.835 ± 102 μV and 1.795 ± 169 μV, respectively (P = 0.45). The mean percentage of amplitude decrease on the electromyography (EMG) was 6.9 ± 2.5%, and the mean percentage of latency increase was 1.9 ± 1.5%. No significant amplitude reduction or loss of signal (LOS) was observed after > 50 probe dislocations. C-IONM probe dislocation does not cause any LOS or significant EMG alterations on the VN.

Granular cell tumor (GCT) is a rare neurogenic neoplasm originating from Schwann cells that predominantly affects women and can involve skin and mucousae. In the respiratory system it most frequently involves bronchi and larynx, while it is rare in the trachea.A 26-year old female smoker was hospitalized for a suspected hypoechoic nodule in the right thyroid lobe closely adherent to the trachea. At preoperative computed tomography tracheal lumen was totally clear. The patient underwent a total thyroidectomy with lymph node dissection and tracheal shaving. The postoperative course was complicated by an extensive subcutaneous bilatreral emphysema associated with respiratory distress appeared on the fifth day. Bronchoscopy revealed a right anterolateral subcentimeter lesion near the second tracheal ring. Histologically, the diagnosis was consistent with a tracheal GCT developing into the thyroid parenchyma. The patient was discharged on the twentieth postoperative day. At the follow-up bronchoscopy the lesion was completely healed and at the last 12 month follow-up the patient is doing well.Tracheal tumors are uncommon neoplasms accounting for about 2% of the total respiratory tree tumors. In literature we found50 papers concerning tracheal GCT and in almost all of the cases patients complained about respiratory symptoms.We report here a rare case of benign GCT of the trachea with extraluminal development, in a young patient who did not complain about preoperative respiratory symptoms, presented on ultrasound as a thyroid nodule with suspected cytology.

The application of intraoperative neural monitoring (IONM) trouble-shooting algorithms procedures in transoral endoscopic thyroidectomy vestibular approach (TOETVA) was investigated. Loss of signal (LOS) is defined as a loss of the primary electromyographic (EMG) normal biphasic waveform with reduced amplitude response to less than 100μV with a stimulation level intensity of 1–2mA. A systematic review of the IONM system at LOS was covered methodically: (i) correct endotracheal tube verification, (ii) stimulation of the recurrent laryngeal nerve (RLN) at entry point, (iii) ipsilateral or contralateral vagal nerve (VN) stimulation, and (iv) laryngeal twitch (LT). The function of 223 nerves at risk (NAR) was recorded with IONM. Twenty-seven (12%) NAR experienced a suspected LOS. LT could not be appreciated. In 15/27 (55%) cases, the application of the IONM trouble-shooting algorithm revealed upward displacement of the EMG tube (all orotracheal intubations). In 9 (4%) NAR, VN stimulation was not accomplished. In detail, there were n.5 left and n. 4 right VNs. Two VNs were ipsilateral, and 7 VNs contralateral. For EMG tube displacement, because the oral/nasal area is included in the aseptic field, it is less possible to re-check by the laryngoscope or fiberscope. A limit for applying the IONM trouble-shooting algorithm to TOETVA is determined by (a) inability to appreciate the LT, (b) difficulty in stimulating the ipsilateral and contralateral VN, and (c) remodeling EMG endotracheal tube position. A modified IONM trouble-shooting algorithm for TOETVA is proposed.

Continuous intraoperative neural monitoring (C-IONM) is a promising technology used to decrease recurrent laryngeal nerve (RLN) damage during thyroid surgery. However, C-IONM use is limited due to its challenging application. C-IONM requires dissection of the carotid sheath and placement of an electrode around the vagus nerve (VN). In our study, we simultaneously stimulated and monitored the proximal RLN trans-tracheally using surface electrodes that were positioned solely on the endotracheal tube.We described the design, implementation, and testing of a pre-prototype, combined stimulating and recording endotracheal tube (SRET) that continually delivered current from tube edge, and tested the function of the RLN at the vocal cords (VC). The SRET was tested in vivo on 10 RLNs using 5 pigs as animal models.The SRET was capable of the following, delivery of continuous trans-tracheal stimulation to the proximal RLN, continuous trans-tracheal stimulation-induced VC movement, and standard ipsilateral RLN biphasic waveforms with latency and amplitude; and recording evoked responses were recorded in the ipsilateral RLN.The pre-prototype SRET represents a possible advancement in technology because it simplified the C-IONM. Thus, the SRET provides a minimally invasive, alternative application to the C-IONM vagal nerve cuff electrodes.

L’identificazione routinaria e la visualizzazione diretta del nervo laringeo ricorrente (NLR) sono riconosciute in tutto il mondo come i gold standard nella tecnica di dissezione della tiroide. Il neuromonitoraggio intraoperatorio (IONM) puo completare l’informazione offerta dall’identificazione diretta attraverso un’analisi funzionale. L’esperienza oramai decennale in Italia con il neuromonitoraggio ha notevolmente contribuito a una piu estesa comprensione della fisiopatologia della paralisi del NLR. Il vantaggio principale dello IONM, tuttavia, e la possibilita di riconoscere una paralisi del NLR visivamente non rilevabile, identificando i potenziali meccanismi di danno e, talvolta, riducendo il rischio di un danno nervoso bilaterale. Poiche la perdita del segnale intraoperatorio puo richiedere una modifica della strategia chirurgica preoperatoriamente pianificata, le presenti raccomandazioni forniscono informazioni sulle possibilita e sui limiti dello IONM nel momento decisionale di una perdita del segnale intraoperatoria. Ci si augura, comunque, che ulteriori studi, il progressivo affinamento della tecnologia e la corretta applicazione dello IONM nella chirurgia tiroidea si evolvano in modo significativo nei prossimi anni.

La tecnica del monitoraggio intraoperatorio continuo (CIONM) del nervo laringeo ricorrente (NLR) e riconosciuta come uno strumento utile per riconoscere le lesioni nervose imminenti e interrompere la relativa manovra chirurgica per prevenire le lesioni permanenti durante la tiroidectomia. Il CIONM fornisce costantemente preziose informazioni in tempo reale, il che e molto utile durante interventi chirurgici complessi della tiroide, specialmente in contesti di anatomia insolita. Il CIONM supera la limitazione metodologica inerente al neuromonitoraggio intermittente (IINOM). L’evento elettromiografico (EMG) combinato clinicamente importante (riduzione ampiezza e aumento latenza del segnale EMG), indicativo di imminente lesione del NLR, previene la maggior parte delle lesioni legate alla trazione del NLR anatomicamente intatto consentendo la modifica della manovra chirurgica che ne e causa nell’80% dei casi. Come ulteriore estensione, il CIONM aiuta anche a identificare il recupero del nervo funzionale intraoperatorio, qualora avvenga una ripresa del valore di ampiezza ≥50% rispetto ai valori della baseline iniziale; cio consente di continuare la resezione del lato controlaterale. Il CIONM facilita un’azione correttiva precoce prima che sia stato fatto un danno permanente al NLR. Il CIONM e una tecnica recente ma in rapida evoluzione, in via di perfezionamento da vari studi incentrati sul miglioramento della sua attuazione e interpretazione, nonche sull’eliminazione degli ostacoli tecnici.

La classica incisione del collo a livello mediano ha rappresentato l’approccio predominante per rimuovere la patologia della tiroide, sia benigna che maligna, e delle ghiandole paratiroidi, ma puo provocare cicatrici cervicali antiestetiche. Un certo numero di approcci di accesso remoto alla tiroide e alle ghiandole paratiroidi sono stati descritti nel recente passato. Questi possono essere tecnicamente impegnativi, specialmente nei pazienti in sovrappeso, e richiedono grandi incisioni al di fuori del collo. Qui descriviamo l’approccio vestibolare transorale per la tiroidectomia, riportando la nostra esperienza iniziale che prevede indicazioni precise e l’utilizzo di tecniche endoscopiche. La chirurgia del collo con approccio vestibolare transorale offre l’assenza di un’incisione del collo in alternativa ad altri approcci di accesso remoto alla ghiandola tiroidea maggiormente invasivi. E una tecnica promettente per i pazienti programmati per una tiroidectomia che desiderano ottimizzare l’estetica.

The dissection of the superior thyroid gland pole is challenging when using the in TransOral Endoscopic Thyroidectomy Vestibular Approach (TOETVA) due to (a) the cranio-caudal approach, (b) cranial-caudal view, and (c) the restriction of maneuverability inside the narrow neck air pocket. In this paper and operative video guide, a series of TOETVA’s tips and tricks are presented with an emphasis on the strategies for a safe approach to the superior thyroid gland pole structures. Management of the upper thyroid pole structures includes: (a) use of a 5 mm/30°-45° endoscope; (b) retraction ports up to the limit of the lower jaw edge; (c) lateral retraction of 1/3 of the cranial strap muscles; (d) isthmectomy; (e) cutting the sternothyroid muscle cranially for 1 cm; (f) retraction of the thyroid upwards and laterally; (g) monitoring the external branch of the superior laryngeal nerve, and (h) sealing individual vessel branches. Access to the superior thyroid pole space through the TOETVA approach presents some challenges, particularly when accessing thyroid vessels or nodules located or displaced more cranially. Strategies that enhance a critical view of the superior thyroid gland structures can protect them from damage and have the potential to improve the safety of the TOETVA and decrease potential conversion rates.

Venous thromboembolism is viewed as a serious health care issue. Patients who experience venous thromboembolism often have an detoriorated quality of life after the event that may require anticoagulation. This results to the risks of spontaneous bleeding. Bleeding after thyroid surgery can present acutely and can lead to airway compromise and death if not recognized and treated appropriately. Whether prophylaxis in a low-risk patient puts the patient at a greater risk of developing a bleeding complication is not well documented. The literature is scarce regarding the prevalence of venous thromboembolism following thyroid and parathyroid surgery.

Continuous intraoperative neuromonitoring is currently the gold standard technique available to prevent recurrent laryngeal nerve injuries. It significantly reduces the complication rate compared with intermittent intraoperative neuromonitoring, and represents significant progress in thyroid surgery, particularly in cases of more difficult dissections. There are, however, some technological and interpretative limits related to the lack of standardization of continuous intraoperative neuromonitoring and the prolonged length of time employed in the surgical positioning of the probe, despite various proposed approaches to the vagal nerve. Nonetheless, this method can be considered a safe and modern approach to thyroid surgery that reduces post-surgical complications and provides useful information.

Prospective studies on the incidence, etiology, and prognosis of well-characterized patients with bleeding after thyroid surgery are lacking. Bleeding after thyroid surgery cannot be predicted or prevented even if risk factors are known in every single procedure, which enhances the im-portance of the following issues: (a) meticulous hemostasis and surgical technique; (b) coopera-tion with the anesthesiologist, i.e., controlling the Valsalva maneuver, adequate blood pressure at the end of the operation as well as at extubation phase and (c) in case of bleeding, a prompt management to guarantee a better outcome. This requires an intensive postoperative clinical monitoring of patients, ideally, in a recovery room with trained staff for at least 4-6 h. Early recognition of postoperative bleeding with immediate intervention is the key to the management of this complication.

The early identification of laryngeal nerves has been accepted as the safest technique to reduce recurrent morbidity, but the identification of the nerve alone can not ensure the presence of the function. Moreover, identification may not be easy, as in cases of reoperation, removal of central lymph nodes, thyrotoxicosis and in case of retrosternal goiter, thyroiditis or aberrant anatomy. Therefore it may be very useful to have an instrument that facilitates the safe completion of this important surgical step. For this reason, in recent years intraoperative neuromonitoring (IONM) has been proposed and applied in many centers in thyroid surgery in addition to the standard practice of visual identification of recurrent laryngeal nerve (RLN), with the primary purpose of evaluating its function and facilitating its identification and dissection. This work shows the benefits of the IONM technique, while recognizing that this technique cannot replace the fundamental elements in the performance of thyroidectomy. Moreover it describes that the introduction of IONM system, as a complement to the thyroidectomy procedure, may also have useful implications from medico-legal and training points of view.

The iatrogenic injury of the recurrent laryngeal nerve (RLN), more clinically significant than those affecting the external branch of the superior laryngeal nerve (EBSLN), constitute one of the most feared perioperative complications of thyroid surgery and parathyroid glands, in terms of impact in clinical and economic-social costs. Moreover, these events rank among the leading reasons for medicolegal litigation of surgeons because of its attendant reduction in quality of life. The average incidence of RLN paralysis, permanent and temporary, after thyroidectomy is high and stands between 2.3% and 9.8% respectively. Given the elements described above, it is essential for the surgeon to adhere to a carefully standardized intraoperative technique that minimizes the possibility of RLN injuries. Intraoperative neuromonitoring (IONM) was introduced to reduce RLN injuries and for this reason, it achieved considerable success among endocrine surgeons. However, even today it is considered an adjunct device to the direct identification of the RLN. In this perspective, IONM of RLN constitutes an important aid, since it represents a reliable tool for the evaluation of functional neural integrity. Despite the ever-increasing diffusion of the IONM, prospective randomized studies are needed for further validation. The purpose of this work is to analyze scientific evidence to show that IONM in thyroid surgery is here to stay.

The recurrent laryngeal nerve (RLN) injury represents one of the most feared complications during thyroidectomy (1). These injuries may cause a temporary or permanent palsy of the RLN and occur, more frequently, monolaterally. If only one side is involved, the palsy results in voice changes, hoarseness, or breathing trouble; in bilateral cases, instead, patients can undergo a potentially fatal airway obstruction needing an urgent tracheostomy (1). This complication, affects the quality of life of the patients, increases the charges for health care, and is the most frequent plea of lawsuits against thyroid surgeons.

The identification of laryngeal nerves during thyroidectomy procedures is considered the safest method to prevent variations in the motility of the vocal cords. However, the only visualization of the laryngeal nerves in many situations is not enough to ensure the result. In fact, anatomical wholeness does not always ensure the functional integrity. The number of endocrine surgery centers that use intraoperative neuromonitoring (IONM) is constantly increasing. Neuromonitoring techniques are currently considered safe methods; however, although infrequently, some complications and side effects related to the use of these methods have been reported. Recently, international guidelines on the use of IONM have been drafted and published to improve quality and safety.
