Abstract

Close to the Roche radius of a white dwarf (WD), an asteroid on a circular orbit sheds material that then adopts a very similar orbit. Observations of the resulting debris show a periodic behavior and changes in flux on short timescales, implying ongoing dynamical activity. Additional encounters from other minor planets may then yield co-orbital rings of debris at different inclinations. The structure, dynamics, and lifetime of these debris discs remains highly uncertain, but is important for understanding WD planetary systems. We aim to identify and quantify the locations of co-orbitals in WD-asteroid-dust particle 3-body systems by exploring the influence of 1:1 resonant periodic orbits. We begin this exploration with co-planar and inclined orbits in the circular restricted 3-body problem (CRTBP) and model the dynamical evolution of these exosystems over observable timescales. The mass ratio parameter for this class of systems ($~2\times 10^{-11}$) is one of the lowest ever explored in this dynamical configuration. We computed the periodic orbits, deduced their linear stability, and suitably seeded the dynamical stability maps. We carried out a limited suite of N-body simulations to provide direct comparisons with the maps. We derive novel results for this extreme mass ratio in the CRTBP, including new unstable 3D families. We illustrate through the maps and N-body simulations where dust can exist in a stable configuration over observable timescales across a wide expanse of parameter space in the absence of strong external forces. Over a timescale of 10 yr, the maximum orbital period deviations of stable debris due to the co-orbital perturbations of the asteroid is about a few seconds. Unstable debris in a close encounter with the asteroid typically deviates from the co-orbital configuration by more than about 20 km and is on a near-circular orbit with an eccentricity lower than ~0.01., Comment: Accepted for publication in A&A

4. The Moderating Role of Preadolescents Humour Styles in the Relationship of Victimization with Internalized and Externalized Symptoms

Abstract

Although the direct effects of victimization on internalizing and externalizing symptoms among children have been well documented, not much is known about the buffering effect of humour styles. The aim of this study was to examine the moderating role of humour styles in the relationship between victimization and internalizing/externalizing symptoms among 155 Greek students (10-12 yrs), using an anonymous self-report questionnaire. Victimization was positively correlated to both internalized and externalized symptoms, while boys scored higher on self-defeating and aggressive humour. Finally, moderation analysis revealed that the effect of victimization on internalized symptoms was reduced at higher levels of aggressive humour. These findings provide additional evidence for the effect of different humour styles in the school context. As the beneficial impact of aggressive humour on victims may be short-termed, prevention efforts could help students learn how to avoid malign humour and use benign styles to cope with stressful situations.

Abstract

Summary: Background: Endotype classification becomes the cornerstone of understanding sepsis pathogenesis. Macrophage activation-like syndrome (MALS) and immunoparalysis are the best recognized major endotypes, so far. Interferon-gamma (IFNγ) action on tissue macrophages stimulates the release of the cytotoxic chemokine CXCL9. It was investigated if this mechanism may be an independent sepsis endotype. Methods: In this cohort study, 14 patient cohorts from Greece, Germany and Italy were studied. The cohorts were 2:1 randomly split into discovery and validation sets. Sepsis was defined by the Sepsis-3 definitions and blood was sampled the first 24 h from meeting the Sepsis-3 definitions. Concentrations of IFNγ, CXCL9, IP-10 (IFNγ induced protein-10), soluble CD163 and ferritin were measured. The endotype of IFNγ-driven sepsis (IDS) was defined in the discovery set as the combination of a) blood IFNγ above a specified cut-off associated with the minimal risk for immunoparalysis (defined as ≥8000 HLA-DR receptors on CD45/CD14-monoytes); and b) increase of CXCL9. Results were compared to the validation set. Findings: 5503 patients were studied; 3670 in the discovery set and 1833 in the validation set. IDS was defined as IFNγ more than 3 pg/ml and CXCL9 more than 2200 pg/ml. The frequency of IDS in the discovery set was 19.9% (732 patients; 95% confidence intervals-CIs 18.7–21.3%) and in the validation set 20.0% (366 patients; 95% CIs 18.2–21.9%). Soluble CD163, a marker of macrophage activation, was greater in IDS and IDS had features distinct from MALS. The mortality in IDS patients was 43.0% (315 patients; 95% CIs 39.5–46.6%) in the discovery set and 40.4% in the validation set (148 patients; 95% CIs 35.5–45.5%) (p = 0.44 compared to patients of the discovery set). IDS was an independent risk factor for death in the presence of other endotypes, severity scores and organ dysfunctions of the multivariate model [hazard ratio 1.71 (95% CIs 1.45–2.01) in the discovery set and 1.70 (95% CIs 1.34–2.16) in the validation set]. Decreases of IFNγ and CXCL9 blood levels within the first 72 h were associated with better outcome. Interpretation: IDS is a new sepsis endotype independently associated with unfavorable outcome. Abstract

Background:. Οnline education has become widespread, but there has been little evaluation of its impact in terms of knowledge gain and acceptance. This article presents an assessment of a webinar on spasticity and botulinum toxin injection. The seminar aimed at health professionals working with patients with spasticity Methods/Results:. It was conducted for 2 days; on the first day, a thorough analysis of spasticity was performed, whereas the second day focused on treatment, particularly botulinum toxin injections. The evaluation was performed using 2 questionnaires: the first rated the knowledge gain (questions prepared from the expert panel; evaluation was performed before the beginning and after the completion, plus a clinical case to justify the treatment), and the second rated acceptance (with a 5-point Likert scale. The results showed significant improvement (P < 0.001) in knowledge gain and positive acceptance by the participants (from max 100% to a min 69.81% agreement). Conclusions:. An online workshop on spasticity management is not only acceptable but also has a positive impact on knowledge.

Abstract

Objective:. Aging is associated with an increased burden of multimorbidity and disease-related functional loss and disability, widely impacting patients and health care systems. Frailty is a major factor in age-related disability and is an important target for rehabilitation interventions, considering that is a reversible condition. Methods:. A working group of members of the International Society of Physical Medicine and Rehabilitation, responding to the World Health Organization (WHO) 2030 call for action to strengthen rehabilitation, was established to assess the quality and implementability of the existing guidelines for the rehabilitation of frailty. Guidelines were retrieved using a systematic search on Pubmed, Scopus, and Web of Science and from the reference lists of screened articles. The included guidelines were evaluated using the AGREE II to assess their quality and using the AGREE-REX to assess their clinical credibility and implementability. Guidelines with a score >4 in the AGREE II item evaluating the overall quality of the guideline were considered for endorsement. Finally, 9 external reviewers evaluated the applicability of each recommendation from the endorsed guidelines, providing comments about the barriers and facilitators for their implementation in their country. Results:. Ten guidelines were retrieved and evaluated by the working group, of which 4 guidelines, that is, the WHO Guidelines on Integrated Care for Older People, the FOCUS guidelines, the Asia-Pacific Clinical Practice Guidelines for the Management of Frailty, and the ICFSR International Clinical Practice Guidelines for Identification and Management of Frailty, were considered for endorsement. All these guidelines were rated as of adequate quality and implementability. Conclusion:. The WHO Guidelines on Integrated Care for Older People (24), the ICFSR International Clinical Practice Guidelines for Identification and Management of Frailty (15), the FOCUS guidelines (25), and the Asia-Pacific Clinical Practice Guidelines (14) for the Management of Frailty have the best quality and applicability of the existing guidelines on the management of frailty, we suggest that should be employed to define the standards of care for patients with frailty. There are barriers to their implementation, as stated by our experts, to take into account, and some of them are country or region-specific. Screening for frailty, exercise, nutrition, pharmacological management, social and psychological support, management of incontinence, and overall comprehensive clinical management are the best tools to face frailty.

Abstract

There is a growing shift from the use of conventional pharmaceutical oral care products to the use of herbal extracts and traditional remedies in dental caries prevention. This is attributed to the potential environmental and health implications of contemporary oral products. This comprehensive review aims at the analysis of plant-derived compounds as preventive modalities in dental caries research. It focuses on data collected from 2019 until recently, trying to emphasize current trends in this topic. The research findings suggest that several plant-derived compounds, either aqueous or ethanolic, exhibit notable antibacterial effects against Streptococcus mutans and other bacteria related to dental caries, with some extracts demonstrating an efficacy comparable to that of chlorhexidine. Furthermore, in vivo studies using plant-derived compounds incorporated in food derivatives, such as lollipops, have shown promising results by significantly reducing Streptococcus mutans in high-risk caries children. In vitro studies on plant-derived compounds have revealed bactericidal and bacteriostatic activity against S. mutans, suggesting their potential use as dental caries preventive agents. Medicinal plants, plant-derived phytochemicals, essential oils, and other food compounds have exhibited promising antimicrobial activity against oral pathogens, either by their anti-adhesion activity, the inhibition of extracellular microbial enzymes, or their direct action on microbial species and acid production. However, further research is needed to assess their antimicrobial activity and to evaluate the cytotoxicity and safety profiles of these plant-derived compounds before their widespread clinical use can be recommended.

Abstract

Abstract Introduction EXO-CD24 are exosomes genetically manipulated to over-express Cluster of Differentiation (CD) 24. It consists of two breakthrough technologies: CD24, the drug, as a novel immunomodulator that is smarter than steroids without any side effects, and exosomes as the ideal natural drug carrier. Methods A randomized, single blind, dose-finding phase IIb trial in hospitalized patients with mild to moderate Coronavirus disease 2019 (COVID-19) related Acute Respiratory Distress Syndrome (ARDS) was carried out in two medical centers in Athens. Patients received either 109 or 1010 exosome particles of EXO-CD24, daily, for five consecutive days and monitored for 28 days. Efficacy was assessed at day 7 among 91 patients who underwent randomization. The outcome was also compared in a post-hoc analysis with an income control group (n = 202) that fit the inclusion and exclusion criteria. Abstract

OBJECTIVES:. It is suggested that sepsis may be classified into four clinical phenotypes, using an algorithm employing 29 admission parameters. We applied a simplified phenotyping algorithm among patients with bacterial sepsis and severe COVID-19 and assessed characteristics and outcomes of the derived phenotypes. DESIGN:. Retrospective analysis of data from prospective clinical studies. SETTING:. Greek ICUs and Internal Medicine departments. PATIENTS AND INTERVENTIONS:. We analyzed 1498 patients, 620 with bacterial sepsis and 878 with severe COVID-19. We implemented a six-parameter algorithm (creatinine, lactate, aspartate transaminase, bilirubin, C-reactive protein, and international normalized ratio) to classify patients with bacterial sepsis intro previously defined phenotypes. Patients with severe COVID-19, included in two open-label immunotherapy trials were subsequently classified. Heterogeneity of treatment effect of anakinra was assessed. The primary outcome was 28-day mortality. MEASUREMENTS AND MAIN RESULTS:. The algorithm validated the presence of the four phenotypes across the cohort of bacterial sepsis and the individual studies included in this cohort. Phenotype α represented younger patients with low risk of death, β was associated with high comorbidity burden, and δ with the highest mortality. Phenotype assignment was independently associated with outcome, even after adjustment for Charlson Comorbidity Index. Phenotype distribution and outcomes in severe COVID-19 followed a similar pattern. CONCLUSIONS:. A simplified algorithm successfully identified previously derived phenotypes of bacterial sepsis, which were predictive of outcome. This classification may apply to patients with severe COVID-19 with prognostic implications.

Abstract

IntroductionCrohn’s disease (CD) is characterized by chronic inflammation and intestinal fibrosis leading to lifelong complications. However, the disease pathogenesis remains elusive, and the therapeutic options are limited. Here, we investigated the interaction between neutrophils and intestinal fibroblasts in the development of CD immunofibrosis, a disease mechanism predisposing to inflammatory and fibrotic complications.MethodsPeripheral neutrophils, enriched neutrophil extracellular traps (eNETs), serum, primary intestinal fibroblasts (PIFs) and intestinal biopsies from CD, ulcerative colitis (UC) patients, and healthy individuals (HI), were studied. Transcriptome analysis of neutrophils, multi-cytokine profiling and cell-based functional assays at mRNA/protein level were performed.ResultsCompared to UC, PIFs from CD patients, independently to the presence of strictures, displayed a distinct pro-fibrotic phenotype characterized by negative Krüppellike Factor-2 (KLF2) and increased cellular communication network factor-2 (CCN2) expression leading to collagen production. In both UC and CD, PIFs-derived IL-8 acted as a culprit chemoattractant for neutrophils in the intestine, where CD neutrophils were accumulated close to fibrotic lesions. Functionally, only CD neutrophils via eNETs induced a CD-like phenotype in HI PIFs, suggesting their fibrotic plasticity. High IFNa in serum and IFΝ-responsive signature in peripheral neutrophils were observed in CD, distinguishing it from UC. Moreover, CD serum stimulated the release of fibrogenic eNETs from neutrophils in an IFNa-dependent manner, suggesting the priming role of IFNa in circulating neutrophils. Inhibition of eNETs or JAK signaling in neutrophils or PIFs prevented the neutrophil-mediated fibrotic effect on PIFs. Furthermore, both serum IFNa levels and mRNA levels of key IFN signaling components in neutrophils were wellcorrelated with CD severity.ConclusionsThis study reveals the important role of the IFNa/neutrophil/fibroblast axis in CD immunofibrosis, suggesting candidate biomarkers and putative therapeutic targets.

This study focuses on senior dental students of the Department of Dentistry at the National and Kapodistrian University of Athens, Greece, aiming to scrutinize their use of SWOT analysis and assess its application as a tool for evaluating entrepreneurial goals and making career decisions in dental entrepreneurship. The research sample comprises 116 senior dental students (N1) in the final undergraduate year of their dental education, with data collection accomplished through the administration of an e-questionnaire during the obligatory course of “Organization and management of dental practices” in December 2023. The data extracted from the SWOT analysis encompass internal and external factors, gender distinctions, and outcomes derived from Stepwise Binary Logistic Regression concerning predictor markers. The results from the SWOT analysis of 114 valid questionnaires (N2), revealed that participants identified communication skills (50%) and organization skills (49.10%) as their primary strengths, followed by favorable personal traits contributing to goal success (36%). Weaknesses predominantly centered around emotional and personal traits like anxiety (41.20%) and other characteristics, alongside practical challenges such as lack of initial capital (24.60%). Main opportunities included collaboration with experienced dentists (33.30%) and access to training programs (27.20%), while economic instability in Greece (77.20%) and the saturated dentist profession (26.30%) were perceived as significant threats. Gender differences were notable, with female dentists more likely to report organization skills as a strength and anxiety as a weakness. Values such as industriousness, persistence, and ethics were commonly shared, with actions focusing on training programs (57.9%) and gaining experience with experienced dentists (29.8%). Cluster analysis identified two subgroups, with one emphasizing utilizing all available options (n = 49) and the other prioritizing on gaining professional skills and experience (n = 65). Logistic regression indicated that participants valuing industriousness were less likely to explore all available options, while those recognizing personal traits were more likely to do so. The study’s outcomes highlight key predictor factors linked to a proactive orientation in career decision-making among senior dental students. These insights offer valuable implications for educational institutions and career counselors.

Abstract

Space missions have discovered a large number of exoplanets evolving in (or close to) mean-motion resonances (MMRs) and resonant chains. Often, the published data exhibit very high uncertainties due to the observational limitations that introduce chaos into the evolution of the system on especially shorter or longer timescales. We propose a study of the dynamics of such systems by exploring particular regions in phase space. We exemplify our method by studying the long-term orbital stability of the three-planet system Kepler-51 and either favor or constrain its data. It is a dual process which breaks down in two steps: the computation of the families of periodic orbits in the 1:2:3 resonant chain and the visualization of the phase space through maps of dynamical stability. We present novel results for the general four-body problem. Stable periodic orbits were found only in the low-eccentricity regime. We demonstrate three possible scenarios safeguarding Kepler-51, each followed by constraints. Firstly, there are the 2/1 and 3/2 two-body MMRs, in which $e_b<0.02$, such that these two-body MMRs last for extended time spans. Secondly, there is the 1:2:3 three-body Laplace-like resonance, in which $e_c<0.016$ and $e_d<0.006$ are necessary for such a chain to be viable. Thirdly, there is the combination comprising the 1/1 secondary resonance inside the 2/1 MMR for the inner pair of planets and an apsidal difference oscillation for the outer pair of planets in which the observational eccentricities, $e_b$ and $e_c$, are favored as long as $e_d\approx 0$. Abstract

Summary: Background: The COVID-19 pandemic heightened risks for individuals with hematological malignancies due to compromised immune systems, leading to more severe outcomes and increased mortality. While interventions like vaccines, targeted antivirals, and monoclonal antibodies have been effective for the general population, their benefits for these patients may not be as pronounced. Methods: The EPICOVIDEHA registry (National Clinical Trials Identifier, NCT04733729) gathers COVID-19 data from hematological malignancy patients since the pandemic's start worldwide. It spans various global locations, allowing comprehensive analysis over the first three years (2020–2022). Findings: The EPICOVIDEHA registry collected data from January 2020 to December 2022, involving 8767 COVID-19 cases in hematological malignancy patients from 152 centers across 41 countries, with 42% being female. Over this period, there was a significant reduction in critical infections and an overall decrease in mortality from 29% to 4%. However, hospitalization, particularly in the ICU, remained associated with higher mortality rates. Factors contributing to increased mortality included age, multiple comorbidities, active malignancy at COVID-19 onset, pulmonary symptoms, and hospitalization. On the positive side, vaccination with one to two doses or three or more doses, as well as encountering COVID-19 in 2022, were associated with improved survival. Interpretation: Patients with hematological malignancies still face elevated risks, despite reductions in critical infections and overall mortality rates over time. Hospitalization, especially in ICUs, remains a significant concern. The study underscores the importance of vaccination and the timing of COVID-19 exposure in 2022 for enhanced survival in this patient group. Ongoing monitoring and targeted interventions are essential to support this vulnerable population, emphasizing the critical role of timely diagnosis and prompt treatment in preventing severe COVID-19 cases. Abstract

Previous works on the divergence of first-order mean-motion resonances (MMRs) have studied in detail the extent of the pericentric and apocentric libration zones of adjacent first-order MMRs, highlighting possible bridges between them in the low-eccentricity circular restricted three-body problem. Here, we describe the previous results in the context of periodic orbits and show that the so-called circular family of periodic orbits is the path that can drive the passage between neighbouring resonances under dissipative effects. We illustrate that the circular family can bridge first and higher order resonances while its gaps at first-order MMRs can serve as boundaries that stop transitions between resonances. In particular, for the Sun-asteroid-Jupiter problem, we show that, during the migration of Jupiter in the protoplanetary disc, a system initially evolving below the apocentric branch of a first-order MMR follows the circular family and can either be captured into the pericentric branch of an adjacent first-order MMR if the orbital migration is rapid or in a higher order MMR in case of slow migration. Radial transport via the circular family can be extended to many small body and planetary system configurations undergoing dissipative effects (e.g., tidal dissipation, solar mass-loss and gas drag)., Comment: Accepted for publication in MNRAS

Abstract

Objectives: Elderly patients with hematologic malignancies face the highest risk of severe COVID-19 outcomes. The infection's impact on different age groups remains unstudied in detail. Methods: We analyzed elderly patients (age groups: 65-70, 71-75, 76-80, and >80 years old) with hematologic malignancies included in the EPICOVIDEHA registry between January 2020 and July 2022. Univariable and multivariable Cox regression models were conducted to identify factors influencing death in COVID-19 patients with hematological malignancy. Results: The study included data from 3,603 elderly patients (aged 65 or older) with hematological malignancy, with a majority being male (58.1%) and a significant proportion having comorbidities. The patients were divided into four age groups, and the analysis assessed COVID-19 outcomes, vaccination status, and other variables in relation to age and pandemic waves. The 90-day survival rate for patients with COVID-19 was 71.2%, with significant differences between groups. The pandemic waves had varying impacts, with the first wave affecting patients over 80 years old, the second being more severe in 65-70, and the third being the least severe in all age groups. Factors contributing to 90-day mortality included age, comorbidities, lymphopenia, active malignancy, acute leukemia, less than three vaccine doses, severe COVID-19, and using only corticosteroids as treatment. Conclusion: These data underscore the heterogeneity of elderly hematological patients, highlight the different impacts of COVID-19 waves and the pivotal importance of vaccination, and may help in planning future healthcare efforts.

Abstract

BackgroundPassive immunotherapy using hyperimmune intravenous immunoglobulin (hIVIG) to SARS-CoV-2, derived from recovered donors, is a potential rapidly available, specific therapy for an outbreak infection such as SARS-CoV-2. Findings from randomised clinical trials of hIVIG for the treatment of COVID-19 are limited.MethodsIn this international randomised, double-blind, placebo-controlled trial, hospitalised patients with COVID-19 who had been symptomatic for up to 12 days and did not have acute end-organ failure were randomly assigned (1:1) to receive either hIVIG or an equivalent volume of saline as placebo, in addition to remdesivir, when not contraindicated, and other standard clinical care. Randomisation was stratified by site pharmacy; schedules were prepared using a mass-weighted urn design. Infusions were prepared and masked by trial pharmacists; all other investigators, research staff, and trial participants were masked to group allocation. Follow-up was for 28 days. The primary outcome was measured at day 7 by a seven-category ordinal endpoint that considered pulmonary status and extrapulmonary complications and ranged from no limiting symptoms to death. Deaths and adverse events, including organ failure and serious infections, were used to define composite safety outcomes at days 7 and 28. Prespecified subgroup analyses were carried out for efficacy and safety outcomes by duration of symptoms, the presence of anti-spike neutralising antibodies, and other baseline factors. Analyses were done on a modified intention-to-treat (mITT) population, which included all randomly assigned participants who met eligibility criteria and received all or part of the assigned study product infusion. This study is registered with ClinicalTrials.gov, NCT04546581.FindingsFrom Oct 8, 2020, to Feb 10, 2021, 593 participants (n=301 hIVIG, n=292 placebo) were enrolled at 63 sites in 11 countries; 579 patients were included in the mITT analysis. Compared with placebo, the hIVIG group did not have significantly greater odds of a more favourable outcome at day 7; the adjusted OR was 1·06 (95% CI 0·77-1·45; p=0·72). Infusions were well tolerated, although infusion reactions were more common in the hIVIG group (18·6% vs 9·5% for placebo; p=0·002). The percentage with the composite safety outcome at day 7 was similar for the hIVIG (24%) and placebo groups (25%; OR 0·98, 95% CI 0·66-1·46; p=0·91). The ORs for the day 7 ordinal outcome did not vary for subgroups considered, but there was evidence of heterogeneity of the treatment effect for the day 7 composite safety outcome: risk was greater for hIVIG compared with placebo for patients who were antibody positive (OR 2·21, 95% CI 1·14-4·29); for patients who were antibody negative, the OR was 0·51 (0·29-0·90; pinteraction=0·001).InterpretationWhen administered with standard of care including remdesivir, SARS-CoV-2 hIVIG did not demonstrate efficacy among patients hospitalised with COVID-19 without end-organ failure. Abstract

The aim of the study was to evaluate changes in the degree of C=C conversion (DC%), chemical structure, optical properties and roughness of one-shade composites before/after photoaging. Τhe one-shade materials tested were Charisma Topaz One (CHT), Clearfil Majesty ES-2 Universal (MES), Essentia Universal (ESU) and Omnichroma (OMN), with G-aenial Anterior (CNA) serving as control. Specimens (2 mm thickness) were prepared and tested for DC% and chemical structure (ATR–FTIR spectroscopy), optical properties (L*a*b*–ΔΕ, translucency parameter–TP, opalescence parameter–OP, contrast ratio–CR and total transmittance–TT by UV–Vis spectroscopy) and roughness (Sa, Sz, Sdr, Sds and Sc by optical profilometry) before and after photoaging (Xe-arc weatherometer). Significant differences were found in DC% between top–bottom surfaces (ESU, OMN before; ESU, CNA after). Photoaging improved DC%, reduced ester peaks implying photodegradation, reduced L* (CHT, OMN, CNA), a* (CHT, CNA), b* (OMN, CNA), TP (all, except for MES), OP (only MES), CR (only MES, but an increase in CNA) and TT (CHT, OMN). OMN, CNA and MES demonstrated ΔΕ > 3.3. Photoaging significantly increased all roughness parameters in all materials, except for MES (Sz, Sdr, Sc) and OMN (Sdr). Although listed in the same group, significant differences were found in one-shade composites before and after photoaging. Several products were strongly affected by photoaging, demonstrating evidence of photodegradation, an increased roughness and color changes exceeding the clinically acceptable levels.

Abstract

Primary cytomegalovirus infection during pregnancy has a high risk of vertical transmission, with severe fetal sequelae mainly associated with first-trimester infections. We conducted a retrospective analysis of 200 IU/kg cytomegalovirus-specific hyperimmune globulin (HIG), used in first-trimester maternal primary infections for congenital infection prevention. The primary outcome was vertical transmission, defined as neonatal viruria or positive amniocentesis if pregnancy was discontinued. HIG, initially administered monthly and since 2019 biweekly, was discontinued in negative amniocentesis cases. Women declining amniocentesis and positive amniocentesis cases with normal sonography were offered monthly HIG until delivery as a treatment strategy. The total transmission rate was 29.9% (32/107; 10 pregnancy terminations with positive amniocentesis, 18 completed pregnancies with positive amniocentesis and 4 declining amniocentesis). Maternal viremia was the only factor associated with fetal transmission (OR 4.62, 95% CI 1.55–13.74). The transmission rate was not significantly different whether HIG was started during the first or second trimester (28.2% vs. 33.3%; p = 0.58), or between monthly and biweekly subgroups (25.7% vs. 37.8%, p = 0.193). Pre-treatment maternal viremia could inform decisions as a predictor of congenital infection.

Abstract

Introduction: Currently approved SARS-CoV-2 vaccines have been proven effective in protecting against severe COVID-19; however, they show variable efficacy against symptomatic infection and disease transmission. We studied the breakthrough COVID-19 infection (BTI) after booster vaccination against SARS-CoV-2 in people living with HIV (PWH). Methods: This was a retrospective, single-center, descriptive cohort study involving PWH, who were followed in the HIV Clinic of “Attikon” University Hospital in Athens, Greece. A BTI was defined as a case of laboratory-confirmed COVID-19 occurring at least 14 days after the third (booster) vaccine dose. Results: We studied 733 PWH [males: 89%, mean age: 45.2 ± 11.3 years, mean BMI: 26.1 ± 4.1, HIV stage at diagnosis (CDC classification): A/B/C = 80/9/11%, MSM: 72.6%] with well-controlled HIV infection. At least one comorbidity was recorded in 54% of cases. A history of ≥1 vaccination was reported by 90%, with 75% having been vaccinated with ≥3 vaccines. Four hundred and two (55%) PWH had a history of COVID-19 and 302 (41.2%) had a BTI, with only 15 (3.7%) needing hospitalization. Only one patient was admitted to the ICU, and no death was reported. Regarding BTI after booster dose, increased age (OR = 0.97, 95% CI: 0.96–0.99, per 1-year increase), and COVID-19 infection prior to booster dose (OR = 0.38, 95% CI: 0.21–0.68) were associated with a lower likelihood for BTI, whereas higher BMI (OR = 1.04, 95% CI: 1.01–1.08) and MSM as a mode of HIV transmission were associated with increased risk (OR = 2.59, 95% CI: 1.47–4.56). The incidence rate of total COVID-19 and BTI followed the epidemic curve of the general population, with the highest incidence recorded in June 2022. Conclusions: A significant proportion of PWH with well-controlled HIV infection experienced a BTI, with the majority of them having mild infection. These data, which include the period of Omicron variant predominance, confirm the importance of vaccination in the protection against severe COVID-19.

Summary: Dysregulated cholesterol metabolism has been linked to neurodegeneration. We previously found that free, non-esterified, 7α,(25R)26-dihydroxycholesterol (7α,26-diHC), was significantly elevated in the cerebrospinal fluid of patients with Parkinson’s disease (PD). In this study we investigated the role of 7α,26-diHC in midbrain dopamine (mDA) neuron development and survival. We report that 7α,26-diHC induces apoptosis and reduces the number of mDA neurons in hESC-derived cultures and in mouse progenitor cultures. Voriconazole, an oxysterol 7α-hydroxylase (CYP7B1) inhibitor, increases the number of mDA neurons and prevents the loss of mDA neurons induced by 7α,26-diHC. These effects are specific since neither 7α,26-diHC nor voriconazole alter the number of Islet1+ oculomotor neurons. Furthermore, our results suggest that elevated 24(S),25-epoxycholesterol, which has been shown to promote mDA neurogenesis, may be partially responsible for the effect of voriconazole on mDA neurons. These findings suggest that voriconazole, and/or other azole CYP7B1 inhibitors may have implications in PD therapy development.

Abstract

Many extrasolar systems possessing planets in mean-motion resonance or resonant chain have been discovered to date. The transit method coupled with transit timing variation analysis provides an insight into the physical and orbital parameters of the systems, but suffers from observational limitations. When a (near-)resonant planetary system resides in the dynamical neighbourhood of a stable periodic orbit, its long-term stability, and thus survival, can be guaranteed. We use the intrinsic property of the periodic orbits, namely their linear horizontal and vertical stability, to validate or further constrain the orbital elements of detected two-planet systems. We computed the families of periodic orbits in the general three-body problem for several two-planet Kepler and K2 systems. The dynamical neighbourhood of the systems is unveiled with maps of dynamical stability. Additional validations or constraints on the orbital elements of K2-21, K2-24, Kepler-9, and (non-coplanar) Kepler-108 near-resonant systems were achieved. While a mean-motion resonance locking protects the long-term evolution of the systems K2-21 and K2-24, such a resonant evolution is not possible for the Kepler-9 system, whose stability is maintained through an apsidal anti-alignment. For the Kepler-108 system, we find that the stability of its mutually inclined planets could be justified either solely by a mean-motion resonance, or in tandem with an inclination-type resonance. Going forward, dynamical analyses based on periodic orbits could yield better constrained orbital elements of near-resonant extrasolar systems when performed in parallel to the fitting of the observational data., Comment: Accepted for publication in A&A

46. Holding True or Caving In? Academics' Values, Emotions, and Behaviors in Response to Higher Education Reforms

Abstract

Higher education (HE) in many countries has been characterized by increased marketization, external accountability, and managerialism. This article examines how academics feel about and respond to HE reforms in Cyprus, a country whose HE sector is heavily commercialized and affected by austerity measures. We analyzed interviews with twenty-three Cypriot academics in four universities, who had been working in business schools from 3 to 29 years. Interviewees described experiences of being an academic in Cyprus, highlighting contextual factors that triggered emotional events in their day-to-day lives. Integrating previous literature, we present six different behavioral responses to the events they described, including compliance, resistance, and flight. We explain how academics chose different responses at different times based on their interpretations of value congruence/incongruence, their felt emotions, and the need to comply with emotional display rules. Considering these elements together highlighted the emotional labor associated with various behavioral responses. The study contributes theoretically by showing how values, perceived emotional demands within particular events, and emotions influence behavior, including different types of emotional labor. We suggest that further research on academics' responses to HE reforms should focus on particular events and on particular contested academic values, such as autonomy or collegiality.

Abstract

Typically, background noise of different types and levels is present during the measurement of the impulse response in spaces. The two methods that are, in practice, most frequently used in the measurement of the impulse response, are the exponential sine sweep (ESS), and the maximum length sequence (MLS). This study’s objective was to estimate the impact of background noise (white noise, tonal noise) on the acoustic parameters (T30, EDT, C80, and D50) for ESS and MLS measurements, by introducing artificial background noise, employing an external sound source. For this purpose, measurements were performed with varying levels of external noise (in steps of 2 dB), and the effect was assessed, using the relative error compared to measurements without artificial background noise. According to the findings for white noise (as background noise), in the case of T30 and EDT, the difference between the two methods, as well as the relative error, for the initial levels of added background noise, was small. However, for higher levels of added background noise, there was a sharp increase in the relative error, which was greater for the ESS method, both for T30 and EDT. Regarding C80 and D50, while initially the differences between the ESS and MLS methods were small, cumulatively, as the background noise increased, the relative error increased for both methods, with the ESS method showing the largest error. In the case of tonal noise (as background noise), the results were consistent with those observed in the case of white noise. The study’s findings contribute to a better understanding of the ESS and MLS methods, and suggest the expected relative error of acoustic parameters when various types and levels of background noise are present. Additionally, the study suggests, based on background noise and level, the optimum method to conduct impulse response measurements.

Human immunodeficiency virus (HIV) infection represents a major cardiovascular risk factor, and the cumulative cardiovascular disease (CVD) burden among aging people living with HIV (PLWH) constitutes a leading cause of morbidity and mortality. To date, CVD risk assessment in PLWH remains challenging. Therefore, it is necessary to evaluate and stratify the cardiovascular risk in PLWH with appropriate screening and risk assessment tools and protocols to correctly identify which patients are at a higher risk for CVD and will benefit most from prevention measures and timely management. This review aims to accumulate the current evidence on the association between HIV infection and CVD, as well as the risk factors contributing to CVD in PLWH. Furthermore, considering the need for cardiovascular risk assessment in daily clinical practice, the purpose of this review is also to report the current practices and novel perspectives in cardiovascular risk assessment of PLWH and provide further insights into the development and implementation of appropriate CVD risk stratification and treatment strategies, particularly in countries with high HIV burden and limited resources.

Abstract

Coral mortality is a global phenomenon of increasing magnitude, correlated with climate change. Prolonged marine heatwaves have particularly affected the north Aegean Sea in summer 2021, threatening shallow-water stony corals, such as Balanophyllia europaea and Cladocora caespitosa. To assess their population status, ten coastal, rocky-bottom stations dispersed in Natura 2000 sites of Chalkidiki (north Aegean) were surveyed using non-destructive techniques in autumn 2021. At each station, corals’ abundance was estimated in situ, by counting the number of B. europaea polyps within randomly placed 50 × 50 cm quadrats, and the number of C. caespitosa colonies along three replicate belt transects 1 × 10 m. The status of corals was qualitatively assigned as healthy, bleached (partially or complete), or in necrosis (partial or complete). B. europaea was found in 80% of stations; in total, 58.17% of the coral specimens were affected by necrosis. C. caespitosa was found in 30% of stations; in total, 27.49% of the coral colonies were partially bleached and 11.32% in necrosis. Another nine sessile invertebrates (sponges, bivalves, and ascidians) were observed in necrosis. These results highlight the need to establish monitoring programs on vulnerable sessile invertebrate populations along the Aegean Sea to assess climate change impacts.

50. Co-constructing effective collective intelligence networks in rare diseases: a mixed method approach to identify the parameters that matter for patients, professionals and policy-makers, piloted in Cyprus

Abstract Background Rare diseases are a particular field of public health that is characterized by scattered, often insufficient knowledge and infrastructure. The scarcity of specialized knowledge often forces clinicians and patients to an incomplete picture of the diseases and their associated risks. Effective person-centred networks appear promising for solving such real world and life-defining problems by purposely sourcing expert knowledge that is geographically-dispersed. The design and implementation of the RARE-e-CONNECT network technology is described. The project was funded to create collaborative spaces for the development of international partnerships in Cyprus’ healthcare, promoting the dissemination of expert knowledge on rare diseases while saving resources through teleconsultation. Parameters that matter for patients, providers and policy-makers through the RARE-e-CONNECT experience were evaluated through a participatory mixed-method approach, consisting of (1) a needs assessment survey with 27 patients/families and 26 healthcare professionals at the two referral hospitals for the diagnosis and management of rare diseases in Cyprus; (2) interviews with 40 patients, families and patient representatives, as well as 37 clinicians and laboratory scientists, including national ERN coordinators/members; (3) activity metrics from 210 healthcare professionals and 251 patients/families/patient representatives who participated on the platform at the time of the research. Results Our results indicate usage and intention by both healthcare professionals and patients/families to openly provide decentralized specialized information for raising suspicion amongst clinicians to facilitate the necessary referrals, as well as peer to peer psychosocial support to help cope with the everyday challenges of living with the disease. User behavior was largely affected by the prevailing social norm favoring individual practice, as well as missing policies for telemedicine and shared care. This article discusses how telehealth is inextricably linked to social, cultural, organizational, technological and policy factors affecting uptake. Conclusions We argue that collective intelligence tools need to be formally considered and work hand in hand with national and European policies/regulatory frameworks to promote proactiveness amongst the healthcare community with regard to the timely diagnosis of rare diseases and the facilitation of patients’ pathway to specialists. Collaborative channels between countries need to be established to source collective intelligence on complex cases and save resources through teleconsultation/telementoring.
