The contamination rate of 212 corneas, obtained by enucleation from April 2014 to January 2015 in a single eye bank, was assessed retrospectively according to age, sex, cause of death, systemic antibiotic use, hospitalization time, ICU time, mechanical ventilation (MV), death to enucleation interval (DEI), enucleation to processing interval (EPI), and corneal epithelial exposure grading. The relative risk (RR) and adjusted RR with a 95% confidence interval were calculated using IBM-SPSS 20.0. The contamination rate was 35.6% (n = 75). On multivariate analysis, ICU stay of 4 days or longer and enucleation to processing interval (EPI) greater than 7.4 h (RR 1.58, CI 0.96–2.60, P = 0.06) were associated with donor cornea contamination. Purpose In 2012, four patients with multiple asymptomatic, indolent, unilateral, choroidal lesions were described. We suspected benign-behaving lymphocytes infiltrating the choroid. This article expands the number of patients and duration of follow-up and speculates further on the etiology. Although histopathologic confirmation of these lesions is still unknown, the natural course of these patients is excellent and should be distinguished from aggressive choroidal lymphoma. Methods To qualify for the study, the patients had to meet the following criteria: 1) Patients collected had asymptomatic choroidal infiltrates as demonstrated in the figures; 2) absence of vitreous cells; 3) no evidence of concomitant systemic malignancy; 4) no systemic inflammatory diseases, including sarcoidosis; 5) no birdshot chorioretinopathy; 6) no conjunctival or orbital lesions; and 7) advanced multimodal imaging and clinical follow-up were performed. Results There were 11 eyes of 11 patients seen. Follow-up ranged from 4 months to 12 years and 1 month (mean 50.2 months; median 24 months). Systemic workup was unrevealing. No patients in this cohort developed systemic, conjunctival, orbital, or vitreoretinal lymphoma or inflammatory disease. No patients developed symptoms or vision loss. Conclusion This entity is an indolent choroidal infiltrative disease. It resembles some cases of choroidal lymphoma and may represent an indolent lymphocytic infiltrate.

Vision impairment and blindness have been significantly associated with high medical care expenditures, decrease in health utility, and loss or reduction of productivity. The objective of this study was to assess the humanistic and economic burden of blindness in a Brazilian sample from a societal perspective. Cross-sectional, observational, and multicenter study enrolling individuals with blindness (defined as the best corrected visual acuity less than 6/60 in the better-seeing eye) caused by retinal disorders. Data collection was performed between December 2012 and December 2014 through face-to-face interview using a structured questionnaire and three standardized patient-reported outcomes instruments. Direct costs were estimated by multiplying the amount of resources used (12-month recall period) by the corresponding unit cost. Productivity losses were measured using the human capital method. All data were collected in Brazilian real (BRL) and converted to United States dollar (USD), using the exchange rate of 1 USD = 3.0415 BRL (May 7, 2015). A total of 146 subjects from 17 research sites were included with a mean age of 68 (SD = 14.8) years and equal gender distribution. Blindness negatively affected both general and vision-specific health-related quality of life. One-half of patients presented some level of anxiety and depression; of these, about 50% with moderate or severe symptoms. Around one-third of subjects (34.2%) reported at least one fall in the previous 12 months due to vision impairment; of these subjects, 14% reported fractures. Emergency room visits and hospitalization were reported by around 25% and 5% of subjects, respectively. The short-term costs (annual costs) of severe vision impairment or blindness for the studied subjects was USD 128,389.09 (USD 879.37 per person). Total medical direct costs summed USD 116,182.00 (USD 795.77 per person), 61.7% of which was due to outpatient visits (with physicians and other healthcare professionals). The long-term costs (lifetime productivity loss) totalized USD 1,962,599.50 (USD 13,442.47 per person). This study demonstrated that blindness imposes both humanistic and economic burden for individuals and for Brazilian society. It also pointed out that there is room to improve blindness management, especially for the poorest people, including health education for individuals, availability of services, and reduction of barriers to patients’ access to healthcare assistance. Hansen’s disease (HD) belongs to the group of neglected diseases and can cause physical deformities and disabilities, in addition to leading to social discrimination. Ocular involvement in HD is estimated at 70–75% worldwide. About 10–50% suffer from severe ocular symptoms and loss of vision occurs in approximately 5% of cases. Ocular changes may persist or worsen even after patients are considered cured and it is necessary to better understand these conditions in order to determine the need for additional public policies. The objective of this study was to identify the prevalence of ocular involvement in patients with HD at two specialist referral centers for treatment of the disease. A cross-sectional study was conducted with ophthalmological evaluations of patients with HD from June 2017 to June 2018. Diagnostic ocular findings, corrected visual acuity, and refractive error were described. Findings were correlated with patients’ clinical and epidemiological variables. A total of 86 patients were evaluated, with a mean age of 50.1 years, predominantly males (59.3%), and with multibacillary HD (92%). The prevalence of ophthalmologic changes was 100% and the most common were dysfunction of the Meibomian glands (89.5%) and dry eye syndrome (81.4%). Cataracts were observed in 22 patients (25.6%), but best corrected visual acuity was normal or near normal in 84 patients (97.7%) and there were no cases of bilateral blindness. Patients with some degree of physical disability had more ophthalmological alterations, involving both the ocular adnexa (p = 0.03) and the ocular globe (p = 0.04). Ocular involvement is common in patients with Hansen’s disease, reinforcing the importance of ophthalmologic examination in the evaluation and follow-up of these patients., Author summary Leprosy–although use of this term is not recommended because of the negative stigma attached to it–also known as Hansen’s disease is one of the oldest diseases in the world. In modern times, we have identified the agent that causes it, we know how to diagnose it, and we know how to treat it. Nevertheless, thousands of people are still diagnosed late and even though they receive the treatment recommended, they suffer from sequelae that can be highly incapacitating. When discussing its clinical presentation, what springs to mind, even among the public, are the notorious, whitened, discolored patches of skin with sensory dysfunction, but ocular involvement caused by the Mycobacterium leprae bacteria is often ignored by patients and even health professionals. Few studies have been published worldwide reporting the true incidence of eye damage caused by Hansen’s disease and identifying which patients need special care for the eyes. It is extremely important to raise awareness and increase knowledge about these conditions, so that public policies on treatment and prevention can be established. This study found that 100% of the sample of patients with Hansen’s disease had some type of abnormality of the eyes or adnexa and that patients over the age of 50 and those with some degree of physical incapacity secondary to Hansen’s disease were at greatest risk. Another important finding was the high rate of Meibomian gland dysfunction (89.5%) and dry eye syndrome.

Naringenin is a biologically active analgesic, anti-inflammatory, and antioxidant flavonoid. Naringenin targets in inflammation-induced articular pain remain poorly explored. The present study investigated the cellular and molecular mechanisms involved in the analgesic/anti-inflammatory effects of naringenin in zymosan-induced arthritis. Mice were pre-treated orally with naringenin (16.7–150 mg/kg), followed by intra-articular injection of zymosan. Articular mechanical hyperalgesia and oedema, leucocyte recruitment to synovial cavity, histopathology, expression/production of pro- and anti-inflammatory mediators and NFκB activation, inflammasome component expression, and oxidative stress were evaluated. Naringenin inhibited articular pain and oedema in a dose-dependent manner. The dose of 50 mg/kg inhibited leucocyte recruitment, histopathological alterations, NFκB activation, and NFκB-dependent pro-inflammatory cytokines (TNF-α, IL-1β, and IL-33), and preproET-1 mRNA expression, but increased anti-inflammatory IL-10. Naringenin also inhibited inflammasome upregulation (reduced Nlrp3, ASC, caspase-1, and pro-IL-1β mRNA expression) and oxidative stress (reduced gp91phox mRNA expression and superoxide anion production, increased GSH levels, induced Nrf2 protein in CD45+ hematopoietic recruited cells, and induced Nrf2 and HO-1 mRNA expression). Naringenin presents analgesic and anti-inflammatory effects in zymosan-induced arthritis by targeting its main physiopathological mechanisms. These data highlight this flavonoid as an interesting therapeutic compound to treat joint inflammation, deserving additional pre-clinical and clinical studies.

AimTo study the macular structure and vasculature in consecutive nanophthalmic eyes using optical coherence tomography angiography.MethodsThis is a prospective, multicentre, cross-sectional study of patients with nanophthalmos (one or both eyes). The superficial and deep foveal avascular zones (FAZ) were measured both manually and with the machine’s built-in automated measurement tool. Correlations between best corrected visual acuity (BCVA), central macular thickness (CMT) and subfoveolar choroidal thickness (SFCT) were calculated.ResultsSixty-five eyes of 35 subjects (16 men and 19 women) with a mean age of 37.4 years were analysed. The mean±SD of refractive error was 14.3±3.2 dioptres, axial length was 16.4±1.6 mm, CMT was 410.2±128.3 µm and SFCT was 450.1±108.3 µm. FAZ was unmeasurable small size in both the superficial and deep capillary plexus in all eyes, along with tortuosity of the superficial foveal capillaries and large vessels. Foveal folds were present in 29 eyes. Disc drusen was detected in 27 eyes and was absent in 31 eyes, while fundus autofluorescence was positive in 17 and negative in 24 eyes. BCVA varied from 20/20 to 20/800, with a mean of 20/76. Using Spearman’s correlation, logarithm of the minimum angle of resolution BCVA correlated negatively with axial length (r=−0.30; p=0.015).ConclusionsFAZ attenuation, capillary tortuosity, foveal folds and thickened subfoveal choroid characterise the nanophthalmic macula. These findings may result from a redundant retina and the absence of apoptotic foveolar retraction because of developmental arrest of the optic vesicle after closure of the embryonic fissure.

BACKGROUND AND OBJECTIVE: To evaluate the efficacy of combined bevacizumab-triamcinolone intravitreal injection in the treatment of diabetic macular edema (DME) compared to monotherapy. PATIENTS AND METHODS: At eight clinical sites, 111 patients with DME were randomly assigned to receive an intravitreal injection of bevacizumab (Avastin; Genentech, South San Francisco, CA), triamcinolone (Ophthalmos Pharmaceutical Industry, São Paulo-SP, Brazil), or their combination. The primary outcome was visual acuity (VA) at 6 months' follow-up. RESULTS: The average number of injections was 3.2 in the bevacizumab group, 2.4 in the combined group, and 2.1 in the triamcinolone group. All groups presented with improvements in VA ( P < .001); however, no differences between groups were observed ( P = .436). Mean reduction in central retinal thickness was statistically different only between the triamcinolone and bevacizumab groups ( P < .015). CONCLUSION: Mono- or combination therapy was effective for DME treatment. No synergistic effects were observed; however, triamcinolone alone or a drug combination may reduce the number of injections required when compared to bevacizumab alone. To report clinical characteristics and treatment outcomes from the largest case series of choroidal neovascularization secondary to central serous chorioretinopathy.Retrospective analysis of 46 eyes of 43 consecutive subjects. Collected data included demographic details, history of presenting illness, clinical examination details including visual acuity at presentation and follow-up with imaging and treatment details. Main outcome measures were the proportion of eyes that had improved (3 or more lines), stable (within ±1 line), or decreased (3 or more lines) vision at the final visit as compared with baseline examination. Secondary efficacy outcomes included change in visual acuity at final follow-up, number of injections, treatment-free interval, and adverse events.Mean age was 57.56 years (range 29-79 years). Mean follow-up duration was 38.3 months ± 58.9 months. More than 3 lines of improvement in 12 eyes (26%), vision was stable (within ±1 line) in 19 eyes (41.3%), and3 lines of loss was noted in 6 eyes (13%). Mean change in the number of lines was 1.16 ± 3.74. Mean number of anti-vascular endothelial growth factor injections during the follow-up was 4.45 ± 4.1. The longest treatment-free interval was 8.9 months ± 7.5 months. There were no adverse events noted.Anti-vascular endothelial growth factor therapy as a primary therapy for choroidal neovascularization secondary to central serous chorioretinopathy is safe and efficacious, without any serious adverse events.

The effects and mechanisms of the citrus flavonoid naringenin were investigated in a mouse model of gout arthritis induced by intra-articular injection of monosodium urate crystals (MSU). Mice were treated with naringenin (16.7–150 mg/kg, per oral) 1 h before MSU (100 µg/kg, i.a.) stimulus. MSU induced joint pain and edema, which were inhibited by naringenin at the dose of 150 mg/kg, without inducing liver and kidney toxicity. Naringenin inhibited MSU-induced histopathological changes, leukocyte recruitment, cytokine production, oxidative stress, NFκB activation, NLRP3 inflammasome components mRNA expression, and IL-1β maturation in cultured macrophages. The present data support the therapeutic applicability of naringenin in the treatment of gout arthritis.

Purpose To investigate the effect of naringenin eye drops in corneal neovascularization induced by alkali (1 N NaOH) burn in mice. Methods Corneal neovascularization in the right eye of male Swiss mice was induced by alkali. Treatment with naringenin eye drops (0.08-80 μg; 8 μL of 0.01-10 g/L solution) or vehicle (saline) started 2 days before corneal neovascularization was induced and was performed twice a day. Mice were treated up until the time animals were euthanized and cornea tissue was collected for testing, which was 2, 4, and 6 hours after alkali stimulus for cytokine and antioxidant capacity measurements, and 3 and/or 7 days after alkali stimulus for the assessment of corneal epithelial thickness and neovascularization, neutrophil, and macrophage recruitment, and vascular endothelial growth factor (Vegf), platelet-derived growth factor (Pdgf), matrix metalloproteinase-14 (Mmp14), and pigment epithelium-derived factor (Pedf) mRNA expression. Results Naringenin eye drops inhibited alkali burn-induced neutrophil (myeloperoxidase activity and recruitment of Lysm-GFP+ cells) and macrophage (N-acetyl-β-D glucosaminidase activity) recruitment into the eye, decrease in epithelial thickness, and neovascularization in the cornea. Further, naringenin inhibited alkali-induced cytokine (IL-1β and IL-6) production, Vegf, Pdgf, and Mmp14 mRNA expression, and the reduction of ferric reducing antioxidant power and Azinobis-(3-Ethylbenzothiazoline 6-Sulfonic acid) radical scavenging capacity as well as increased the reduced glutathione and protein-bound sulfhydryl groups levels. Conclusions Collectively, these results indicate that naringenin eye drops are protective in alkali-induced corneal burn by inhibiting leukocyte recruitment, the proangiogenic factor expression, inflammatory cytokine production, and loss of antioxidant defenses.

Purpose Central serous chorioretinopathy (CSCR) is characterized by macular detachment due to thickened choroid, mostly affecting young men under perceived stress. Although most previous studies on CSCR have been retrospective and have focused on a single facet of the patient's personality, we conducted a prospective, intercontinental, controlled study to analyze the multifaceted personality profile in CSCR. Design Prospective, cross-sectional, case-control study. Participants Subjects with CSCR from 6 university-based eye clinics consented to participate in a questionnaire. Controls without retinal disease were recruited from the same clinics. Methods The interview consisted of a 60-item questionnaire. Recruitment of participants was from January 2015 to February 2016. Controls were matched for age, gender, and race. Statistical analyses were performed using univariate and multivariate analyses. Main Outcome Measures The main parameters registered were presence of stress, daily number of cups caffeine intake, and personality traits (Type A; obsessive–compulsive; aggressive). Results A total of 83 consecutive patients with CSCR (mean age, 45.9 years; male, 80.7%) and 83 controls (mean age, 46.0 years; male, 80.7%) were analyzed for 60 variables. Multivariate analysis revealed a strong association with obsessive–compulsive behavior (P = 0.001), caffeine intake (P = 0.002), Type A personality (P = 0.002), continuous stress (P = 0.001), and premature ejaculation (P = 0.001). Conclusions This study sheds light on the unique psychologic functioning of patients with CSCR: preoccupied, inflexible, perfectionist (obsessive–compulsive tendency), competitive, ambitious, impatient, high achiever (Type A personality), and under continuous stress. In addition, caffeine abuse and premature ejaculation were linked to CSCR.

OBJETIVO: Avaliar o conhecimento de estudantes de Medicina da Universidade Estadual de Londrina (UEL), em cirurgia refrativa, assim como analisar o percentual de estudantes que são portadores de ametropias, seus métodos de correção e seu interesse ou não na realização do procedimento cirúrgico. MÉTODOS: Realizou-se um levantamento através de questionário autoaplicável, previamente testado, entre 154 estudantes do primeiro ao quarto ano de Medicina da Universidade Estadual de Londrina entre setembro e novembro de 2011. RESULTADOS: Foi relatado que 70,8% dos estudantes possuíam algum tipo de erro de refração, sendo a miopia o erro mais prevalente, com 72,5% dos estudantes amétropes apresentando-a, associada ou não a outros erros de refração. Os óculos foram o método de correção visual referido como o mais utilizado, por 80% dos pesquisados. Quanto à cirurgia refrativa, 85,7% dos estudantes já haviam ouvido falar a respeito, porém, apenas 42,9% sabiam como o procedimento é realizado, sendo o oftamologista a principal fonte de informação sobre o tema, para 23,5% dos alunos. Apenas 43,2% dos alunos têm interesse na realização da cirurgia, e apenas 3 (1,9%) estudantes já foram submetidos ao procedimento. CONCLUSÃO: Apesar da importância da cirurgia refrativa na Oftalmologia verificou-se baixo conhecimento acerca do tema entre os estudantes, o que afeta o interesse dos mesmos em serem submetidos ao procedimento. Observou-se também uma taxa relativamente alta de falsa expectativa quanto ao seu resultado, principalmente entre os estudantes que querem ser submetidos ao procedimento, provavelmente pelas fontes pouco confiáveis alegadas pelos estudantes. Considerando o fato de que se tratam de futuros médicos, fica clara a necessidade de maiores esclarecimentos sobre o tema na graduação. OBJECTIVE: Evaluate the knowledge of medical students from Universidade Estadual de Londrina (UEL), about refractive surgery, as well as analyzing the percentage of students that presents refractive errors, their correction methods and their interest (or not) in the surgical procedure. METHODS: We conducted a survey using self-evaluation questionnaire (previously tested) among 154 medical students from first to fourth year, between september and november 2011. RESULTS: It was reported that 70.8% of students had some type of refractive error, and myopia was the most prevalent,with 72.5% of students with refractive erros presenting it, with or without other refractive errors associated. The glasses were the most used method of visual correction . About refractive surgery, 85.7% of students had already heard about, but only 42.9% knew how the procedure is performed, and the ophthalmologist was the main source of information on the subject, to 23.5% of students. Only 43.2% of students have an interest in surgery, and only 3 (1.9%) students have been undergoing the procedure. CONCLUSION: There has been little information and knowledge about refractive surgery among medical students,which affects their interest in undergoing the procedure, which maybe due the fact that many of them do not have the most adequate source of information to obtain knowledge about the subject.

OBJETIVO: O objetivo deste trabalho é investigar a expressão do fator de crescimento vascular endotelial (VEGF) na coroide e esclera, utilizando um modelo experimental de hipercolesterolemia. MÉTODO: Coelhos New Zealand foram organizados em dois grupos: O grupo dieta normal (GN), composto por 8 coelhos (8 olhos), recebeu ração padrão para coelhos, durante 4 semanas; e o grupo hipercolesterolêmico (GH), composto por 13 coelhos (13 olhos), recebeu dieta rica em colesterol a 1% por 8 semanas. Foi realizada a dosagem sérica de colesterol total, triglicerídeos, HDL colesterol, glicemia de jejum no início do experimento e no momento da eutanásia. Ao final da 8ª semana para o GH e 4ª semana para o GN foi realizada a eutanásia dos animais e os olhos foram submetidos à análise imuno-histoquímica com os anticorpos RAM-11 e VEGFR-1. Purpose To study the outcomes of management of rhegmatogenous retinal detachment in eyes with chorioretinal colobomas. Methods A retrospective review of 119 patients (119 eyes) with chorioretinal colobomas who underwent surgical repair for rhegmatogenous retinal detachment was performed. Data were collected on the site of the retinal break, type of surgery, anatomical success, and complications. Results The most common location of the primary retinal break was the intercalary membrane in 58.8% of eyes. The most common surgical intervention was vitrectomy with endolaser and silicone oil tamponade (77.3% of eyes). Final anatomical success was achieved in 87.4% of eyes. Anatomical success was significantly higher in eyes that received long-acting tamponade (P = 0.006). Cryotherapy was significantly associated with failure of primary vitrectomy (P = 0.028). Placement of an encircling band did not affect anatomical outcomes (P = 0.75). Most of the eyes (60%) with recurrent retinal detachment after primary vitrectomy had a primary break within the normal retina. Conclusion The optimal option for managing retinal detachment in eyes with chorioretinal colobomas is pars plana vitrectomy with long-acting tamponade (silicone oil or octafluoropropane) and retinopexy to the edge of the coloboma and the primary breaks. Cryotherapy is associated with poor anatomical outcomes. An encircling band does not seem to affect the final anatomical outcome.

Purpose: To evaluate and compare the effects of topical application and subconjunctival injection of bevacizumab on corneal neovascularization (CNV) in rabbits' eyes after chemical burning of the cornea. Methods: The animals were randomly distributed into four groups of five animals. In one group, the drug was instilled, while in another, it was administered by subconjunctival injection. The two procedures using bevacizumab were compared with instillation and subconjunctival injection of saline solution (S). Neovascularization was evaluated according to the size of the invasion area of new blood vessels and through computerized analysis of this area. The data were analyzed using the Kruskal-Wallis test followed by Dunn's test for two-by-two comparison of the groups, to assess the external examination of CNV. OBJECTIVES: The objective of this study was to evaluate the relationship between the treatment of Helicobacter pylori gastric infection and changes in best-corrected visual acuity and macular detachment in patients with chronic central serous chorioretinopathy. METHODS: Seventeen patients diagnosed with central serous chorioretinopathy were examined for gastric infection with Helicobacter pylori using the urease test and gastric biopsy. Helicobacter pylory-positive patients were treated with the appropriate medication. The response to therapy was monitored by evaluating the best-corrected visual acuity and optical coherence tomography. The data were analyzed using Student's t-test before and after treatment. RESULTS: Fourteen patients (15 eyes) aged 30-56 years (mean 43.4 ± 8.3 years) were positive for Helicobacter pylori. Most of the positive patients had gastric symptoms (78.5%); one had bilateral central serous chorioretinopathy. The mean baseline best-corrected visual acuity was 20/98 (logMAR = 0.53 ± 0.28). Three months after starting treatment with antibiotics, the serous detachment had resolved in 14 of 15 eyes, but two cases required laser treatment. The follow-up period ranged from 6 to 27 months. The mean final best-corrected visual acuity differed significantly from baseline. CONCLUSION: Our findings suggest that Helicobacter pylori infection may be present in many chronic central serous chorioretinopathy patients and that treatment for the infection may have a favorable effect on the outcome of chronic central serous chorioretinopathy. Due to the possibility of the spontaneous regression of chronic central serous chorioretinopathy and the high prevalence of the infection in the general population, prospective and masked clinical trials are necessary to confirm that treatment for Helicobacter pylori infection may benefit patients with chronic central serous chorioretinopathy.

PURPOSE: To report the findings of optical coherence tomography (OCT) in three cases of diffuse unilateral subacute neuroretinitis (DUSN). METHODS: Three young patients with confirmed diagnosis of diffuse unilateral subacute neuroretinitis were followed-up using Stratus® OCT. RESULTS: Optical coherence tomography findings included retinal nerve fiber layer atrophy and focal edema where the worm was initially lodged. In two patients we could identify the worm by optical coherence tomography as an area of hiper-reflectivity. The worms were laser photocoagulated and post-treatment optical coherence tomography scanning revealed improvement of edema, nerve fiber layer thinning and a retinal hyper-reflectivity where laser had been applied. CONCLUSION: The main finding reported here is the potential of optical coherence tomography in detecting the presence of retinal nerve fiber layer atrophy and focal retinal edema in areas affected by the worm. Optical coherence tomography can be used to distinguish diffuse unilateral subacute neuroretinitis from other mimicking diseases like punctate outer retinitis, when there are no retinal fiber layer atrophy.OBJETIVO: Demonstrar os achados da tomografia de coerência óptica em três casos de neurorretinite subaguda difusa unilateral (DUSN). MÉTODOS: Os pacientes com diagnóstico confirmado de neurorretinite subaguda difusa unilateral realizaram seguimento pré e pós-tratamento por meio da tomografia de coerência óptica, Stratus® OCT. RESULTADOS: Os achados marcantes da tomografia de coerência óptica foram a atrofia das camadas de fibras nervosas da retina e edema da retina localizado em áreas nas quais a larva esteve. Em dois pacientes pôde-se localizar a larva no espaço sub-retiniano por meio da tomografia de coerência óptica, que se traduziu por pequena área densa (hiperrefletividade). As larvas foram fotocoaguladas a laser e os achados da tomografia de coerência óptica após o tratamento demonstrou melhora do edema, afinamento das camadas de fibras nervosas e hiperrefletividade no local da aplicação do laser. CONCLUSÃO: Os principais achados na tomografia de coerência óptica foram a atrofia difusa das camadas de fibras nervosas e o edema localizado nas áreas afetadas pela larva. A tomografia de coerência óptica pode ser usado para diferenciar neurorretinite subaguda difusa unilateral de doenças que a simulam, como retinite punteada externa, que não manifesta alterações das camadas de fibras nervosas.

Todos submetidos a modelo de trauma com dispase associada à diatermia da retina para indução de membranas de PVR. Separados randomicamente para receberem hiperecina (10 µM em 0,1 ml) ou solução salina (0,1 ml) como placebo. Avaliados clinicamente no sétimo, décimo quarto, vigésimo primeiro e vigésimo oitavo dias de pós-operatório com oftalmoscopia indireta e retinografia colorida digitalizada. O grau de PVR foi classificado em estágios (de 0 a 7) segundo Hida e colaboradores. RESULTADOS: A formação de membranas esteve presente em 79% dos olhos, sendo 100% nos olhos do grupo placebo e 60% nos olhos do grupo tratamento (hiperecina). A comparação entre as médias dos estágios de PVR entre os grupos mostrou diferença estatisticamente significativa, com valor p=0,0321 pelo teste Wilcoxon. CONCLUSÕES: O modelo de trauma com uso de dispase e diatermia da retina produz membranas vitreorretinianas. PURPOSE: To determine the prevalence of early and late-stage age-related macular degeneration (ARMD) and its association with risk factors such as age, gender, smoking, body mass index, hypertension and diabetes history, cataracts and pseudophakia. Design: Population-based cross-sectional study in an elderly Japanese-Brazilian population from Londrina (Paraná, Brazil). METHODS: The study included 483 (80.5%) of the 600 registered members of a local association for Japanese immigrants and their descendants, aged 60 years and up. The presence of early and late-stage age-related macular degeneration was determined using the standard protocol and the international classification system. RESULTS: The mean age of the study subjects was 71 years. The overall prevalence of age-related macular degeneration was 15.1% (CI 95%; 12-18.7). The prevalence of early-stage age-related macular degeneration was 13.8% (CI 95%; 10.9-17.3), geographic atrophy was present in 0.4% and neovascular age-related macular degeneration in 0.8%. Age-related macular degeneration was significantly (p=0.004) and linearly (p=0.001) associated with age. CONCLUSION: Our study population displays a prevalence of early and late-stage age-related macular degeneration and component lesions similar to those of other Western countries, and data suggest a higher prevalence than that reported for populations in Japan. Since the age-related macular degeneration prevalence tends to rise as the population ages, studies identifying risk factors and exploring prevention methods are becoming increasingly important.OBJETIVO: Identificar a prevalência da degeneração macular relacionada à idade (DMRI) inicial e tardia na população de japoneses e descendentes e verificar a associação com os fatores de risco: idade, sexo, hábito de fumar, índice de massa corpórea, história de hipertensão e diabetes, catarata e pseudofacia. MÉTODOS: Realizado um estudo transversal na população de japoneses e descendentes, acima de 60 anos, residentes na cidade de Londrina (PR) - Brasil. Quatrocentos e oitenta e três (80,5%) das 600 pessoas registradas foram submetidas ao exame oftalmológico completo. A presença de degeneração macular relacionada à idade foi determinada seguindo um protocolo padrão e classificação internacional, no período de setembro de 2002 a julho de 2003. RESULTADOS: A média de idade foi de 71 anos (60-92 anos). A prevalência da degeneração macular relacionada à idade foi de 15,1% (intervalo de confiança (I.C. 95%: 12-18,7), sendo na fase inicial em 13,8% (I.C. 95%: 10,9-17,3) e na fase tardia (atrofia geográfica 0,4% e membrana neovascular sub-retiniana 0,8%) em 1,3%. A degeneração macular relacionada à idade foi associada à idade (p=0,004) e apresentou tendência linear (p=0,001). Não foi observada associação entre a DMRI e os outros fatores de risco analisados. CONCLUSÃO: A prevalência da degeneração macular relacionada à idade neste estudo foi semelhante aos dos países do Ocidente, e é possível que ela seja maior do que a da população do Japão. Os dados comprovam a importância da degeneração macular relacionada à idade nessa população de japoneses e descendentes, e mais estudos são necessários para identificar os fatores de riscos e os métodos de prevenção.

METHODS: Interventional case series. The authors developed a non-randomized clinical trial protocol to investigate the clinical evolution of diffuse unilateral subacute neuroretinitis cases treated with albendazole. According to protocol criteria up to now, six patients were selected that will be described separately. RESULTS: Of the six studied patients, four presented the worm. All six patients treated with the antiparasitic drug showed improvement of visual acuity and of chorioretinal scars. During the weeks of treatment, evidence of worm inactivation was documented for the four patients with visible worms. No adverse drug side effects were observed. CONCLUSIONS: The antiparasitic drug albendazole seems to be beneficial and safe in patients with diffuse unilateral subacute neuroretinitis. More studies are necessary to evaluate the effectiveness of albendazole in the treatment of diffuse unilateral subacute neuroretinitis.

MÉTODOS: Estudo prospectivo e comparativo entre um grupo de 70 pacientes que foi submetido à avaliação angiográfica com retinógrafo convencional, injetando-se 5 ml de fluoresceína sódica a 10% (grupo controle), e um grupo de 70 pacientes que foi submetido à avaliação angiográfica com retinógrafo digital injetando-se 2 ml de fluoresceína a 10% (grupo estudo). Pressão arterial, freqüência cardíaca e oximetria foram avaliadas antes e após a injeção de contraste. Reações orgânicas, relacionadas ao exame, foram notificadas. A qualidade das fotografias e os custos foram comparados entre as duas técnicas. RESULTADOS: Observou-se que os pacientes do grupo controle apresentaram maior aumento da pressão arterial sistólica e diastólica. Freqüência cardíaca, oximetria e reações adversas não demonstraram diferenças estatisticamente significativas entre os dois grupos. Quanto à qualidade das fotografias foi notado melhor desempenho no grupo controle. Quanto aos custos observou-se que o exame realizado no grupo estudo proporcionou economia de aproximadamente 54,8% por exame em relação ao grupo controle. CONCLUSÃO: A realização do exame com menor dosagem de fluoresceína, utilizando equipamento digital com máquina de fotocópia a laser, proporcionou maior estabilidade da pressão arterial sistólica e diastólica, porém não exerceu influência sobre a freqüência cardíaca, oximetria e reações adversas como náusea, vômito, síncope e urticária. A qualidade das fotografias pode ser considerada inferior, porém possibilitou a realização de diagnóstico e orientação terapêutica para quem executou o exame. Economicamente apresentou geração de lucro de 66,26% contra 25,81% do equipamento convencional. METHODS: Prospective and comparative study carried out in a group of 70 patients, who underwent an angiographic evaluation with a conventional retinographer injecting 5 ml sodium fluorescein at 10% (control group) as well as a group of 70 patients who underwent an angiographic evaluation with a digital retinographer injecting 2 ml fluorescein at 10% (study group). Arterial pressure, heart rate and oximetry were assessed prior to and after the dye injection. Organic reactions related to the examination were reported. Photograph quality as well as cost between the two techniques were compared. RESULTS: Control group patients showed a greater increase in systolic and diastolic arterial pressure. Heart rate, oximetry measurement and adverse reactions did not show any significant statistical differences between both groups. As for the quality of photographs, a better performance was noticed in the control group. As for the cost, the examination carried out in the study group required lower cost and thus saved around 54.8% per examination in relation to the control group. CONCLUSION: The examination carried out with a lower dose of fluorescein using digital equipment along with a laser photocopier provided greater stability in the systolic and diastolic arterial pressure. However, it did not have any influence on heart rate, oximetry or adverse reactions such as nausea, vomiting, syncope and rashes. The quality of photographs was poor although they enabled diagnosis as well as therapy follow-up for those who carried out the examination. Moreover, economically the above procedure represented a gain of 66.26%, against 25.81% in relation to the conventional equipment.

MÉTODOS: 24 pacientes com alteração da visão após participarem de ritual religioso, foram avaliados quanto à acuidade visual inicial e final, após 6 meses, refração, biomicroscopia de mácula, retinografia, tela de Amsler e sensibilidade ao contraste. RESULTADOS: 24 casos foram relatados. Destes, 21 (87,5%) eram mulheres e 3 (12,5%) eram homens. A idade variou de 18 a 46 anos. Houve acometimento de 43 olhos. A lesão foi bilateral em 19 (79,2%) pacientes e unilateral em 5 (20,8%). A acuidade visual inicial variou de 20/20 a 20/70 e a final foi de 20/25, ou melhor, em 40 (93%) olhos. Destes, 33 (76,7%) olhos alcançaram 20/20. Apenas 2 (4,6%) olhos ficaram com alteração da sensibilidade ao contraste e 3 (6,9%) ficaram com escotoma persistente. CONCLUSÃO: Este é o maior relato de casos de retinopatia solar após ritual religioso. A retinopatia solar apresenta evolução bastante favorável. No entanto, a acuidade visual não apresenta recuperação completa da visão. Os pacientes podem ficar com escotoma e metamorfopsia residual. Data of the initial and final visit after 6 months of follow-up are presented and include best corrected visual acuity, refraction, color fundus picture, macular biomicroscopy, Amsler's screen and contrast sensibility. RESULTS: A total of 24 cases was reported. Of these, 21 (87.5%) were females and 3 (12.5%) males. Age varied from 18 to 46 years. A total of 43 eyes were damaged. The lesion was bilateral in 19 (79.2%) patients and in 5 (20.8%) it was unilateral. The initial visual acuity ranged from 20/20 to 20/70, and the final visual acuity was 20/25 or better in 40 (93%) eyes. Of these, 33 (76.7%) eyes reached 20/20 or the last follow-up visit. Only 2 (4.6%) eyes presented alteration in the sensibility to contrast and 3 (6.9%) remained with persistent scotoma. CONCLUSION: This is the largest series of solar retinopathy after religious ritual. The cases herein presented showed a quite favorable evolution. However recovery of visual acuity is not complete, once scotoma and residual metamorphopsia persisted in a few patients.

Purpose: The aim of this study was to report a case of a 52-year-old man with a rare fungal endophthalmitis after penetrating ocular trauma with a fish hook. Results: The patient was submitted to wound repair and hook extraction. Three weeks after the trauma, a cataract extraction with phacoemulsification and in-the-bag intraocular lens implantation was performed. After the development of endophthalmitis, vitrectomy and lens explantation with positive culture for Curvularia lunata were carried out. The infection was controlled with the administration of systemic itraconazole and intraocular voriconazole. Conclusions: The use of intravitreal voriconazole injection is a viable option in cases of Curvularia fungal endophthalmitis.

PURPOSE We report a case of rhegmatogenous retinal detachment immediately (first postoperative day) after the refractive surgery of laser in situ keratomileusis (LASIK). CASE REPORT A 50-year-old man underwent bilateral LASIK; on the first postoperative day, he presented with decreased visual acuity due to retinal detachment with vitreous hemorrhage in the left eye. There were two horseshoe tears posterior to the equator and one peripheral operculated hole, all temporally located. Four months later, he presented with vitreous hemorrhage and a horseshoe tear temporal posterior to the equator in the right eye. RESULTS Surgical treatment was done, and the retina reattached immediately after surgery and remained stable thereafter in both eyes. Six months after surgery, visual acuity was 20/30 in the left eye and 20/40 in the right eye. CONCLUSION Although the cause/effect relationship between LASIK and retinal detachment has not yet been established, occurrence of immediate postoperative retinal detachment may represent a strong temporal suggestion of its association.

The series comprised ten consecutive patients with a confirmed diagnosis of DUSN, ie, seven patients with late-stage disease and three with early-stage disease, who were assessed by SD-OCT for mean macular, retinal nerve fiber layer, and choroidal thickness using enhanced depth imaging software.Results: Comparing the affected eye with the healthy fellow eye, significant diffuse atrophy of the retinal layers with a decrease in mean macular (P=0.004) and retinal nerve fiber layer (P=0.002) thickness was found in all cases. Os autores apresentam um caso de neurorretinite subaguda difusa unilateral (D.U.S.N) confirmada com a identificação da larva em uma criança de 6 anos associada a quadro de larva migrans cutânea, bem como a análise de outros sete casos de D.U.S.N. e sua associação sorológica à toxocaríase e antecedentes de larva migrans cutânea. Este é o primeiro caso descrito na literatura de D.U.S.N. concomitante a larva migrans cutânea na fase ativa. The autors present a confirmed case of diffuse unilateral subacute neuroretinitis (D.U.S.N.) with identification of the worm in a 6-year-old child with symptoms of cutaneous larva migrans, as well as analysis of 7 other cases of D.U.S.N. and association with cutaneous larva migrans or positive sorology for Toxocara canis. This is the first described case in the literature of the association between D.U.S.N. and cutaneous larva migrans in its active form. This study aimed to evaluate the expression of the inflammatory cytokines TNF-α and IL-6 in the sclera and choroid of hypercholesterolemic rabbits.Twenty-one New Zealand male albino rabbits were divided into two groups: NG and HG. The NG group was fed a standard rabbit diet and the HG group was fed a cholesterol-enriched diet (1%). The serum total cholesterol, triglyceride, HDL cholesterol, and fasting blood glucose levels were determined at the beginning of the experiment and on the day of euthanasia. Euthanasia of animals in the NG and HG groups was performed at the end of the 4th and 8th week, respectively. The eyes were analyzed immunohistochemically using TNF-α and IL-6 antibodies.At the time of euthanasia, the HG group showed a significant increase in total cholesterol and triglyceride when compared with the NG group (p0.001). When compared with the NG group, there was a significant increase in the expression of TNF-α (p0.001) and IL-6 (p=0.002) in the choroid and sclera of animals in the HG group.This study demonstrates that the hypercholesterolemic diet induces expression of TNF-α and IL-6 in the choroid and sclera of rabbits.

Demonstrate that the blockade of angiotensin II AT-1 receptors, through the systemic administration of olmesartan, can reduce the MCP-1 expression and the resulting macrophage accumulation in the choroid and sclera of hypercholesterolemic rabbits.Thirty-two New Zealand rabbits were divided into 3 groups: group I (GI) was fed a standard rabbit diet; group II (GII) was fed a hypercholesterolemic diet; and group III (GIII) was fed a hypercholesterolemic diet plus olmesartan. Serum levels of total cholesterol, triglyceride, HDL cholesterol, and blood glucose were determined in fasting rabbits at the beginning of the experiment and on the day of euthanasia. The choroid and sclera were submitted to morphometric analysis as well as immunohistochemical analysis with MCP-1 and RAM-11 (macrophage marker) antibodies.No abnormality was detected in GI. Group II and III had significant increases in choroid-sclera complex thicknesses when compared with group I (P0.001). GII showed a significant increase in immunoreactivity for MCP-1 in relation to GI (P=0.001) and GIII (P=0.004). GII showed a significant increase in immunoreactivity for RAM-11 of the choroid-sclera complex in relation to GI (P0.001) and GIII (P=0.034). A significant increase in immunoreactivity for RAM-11 was observed in GIII in relation to GI (P=0.008).Olmesartan reduced the MCP-1 expression and the resultant macrophage accumulation in the choroid-sclera complex of hypercholesterolemic rabbits.

Total serum cholesterol, triglyceride, HDL cholesterol and fasting blood glucose exams were performed at the initiation of the experiment and at the euthanasia time. After hypercholesterolemic group 8th week and NG 4th week, animals were euthanized and their eyes underwent immunohistochemical analysis with the RAM-11 and VEGFR-1).The diet has induced a significant increase in total cholesterol and triglyceride levels in HG when compared with NG (p0.001). There was a significant increase in the RAM-11 and VEGFR-1 expressions in hypercholesterolemic group choroid and sclera in relation to NG (p0,001).This study has revealed that the hypercholesterolemic diet in rabbits induces an increase in the macrophage concentration and immunoreactivity to VEGFR-1 in the choroid and sclera, resembling human age-related macular degeneration (ARMD).

To report a case of gyrate atrophy of the choroid and retina associated with retinal detachment. Hyperornithinemia confirmed the diagnosis of gyrate atrophy. Pars plana vitrectomy with silicone oil infusion was performed with good anatomical results, despite the persistence of low visual acuity. Retinal detachment is a rare complication of gyrate atrophy and can be managed with pars plana vitrectomy and silicone oil. We discuss the possible mechanisms that led to low visual acuity.

OBJETIVO: O objetivo deste trabalho é demonstrar experimentalmente que a dieta rica em colesterol provoca aumento da expressão da MCP-1 na coroide e esclera. MÉTODO: Coelhos New Zealand foram organizados em dois grupos: GN (grupo dieta normal), composto por 8 coelhos (8 olhos), recebeu ração padrão para coelhos, durante 4 semanas; GH (grupo hipercolesterolêmico), composto por 13 coelhos (13 olhos), recebeu dieta rica em colesterol a 1% por 8 semanas. Foi realizada a dosagem sérica de colesterol total, triglicerídeos, HDL colesterol, glicemia de jejum no início do experimento e no momento da eutanásia. Ao final da 8ª semana para o GH e 4ª semana para o GN foi realizada a eutanásia dos animais e os olhos foram submetidos à análise imuno-histoquímica com o anticorpo anti-MCP-1. ResumoA toxoplasmose congênita pode causar aborto e danos neurológicos e/ou oculares ao feto. Desde que a Áustria e a França estabeleceram a triagem pré-natal, a prevalência da toxoplasmose caiu de 50% para 35% e de 84% para 44%, respectivamente. Outros países, como o Reino Unido, adotam práticas educativas para reduzir o risco de infecção em gestantes soronegativas. No Brasil, a triagem pré-natal é realizada nos estados do Mato Grosso do Sul e Minas Gerais e nas cidades de Curitiba e Porto Alegre. Em Londrina, Paraná, foi implantado o “Programa de Vigilância da Toxoplasmose Adquirida na Gestação e Congênita”, que se baseia na triagem sorológica, com orientação sobre as medidas de prevenção e monitoramento sorológico trimestral nas gestantes inicialmente soronegativas, além do acompanhamento das gestantes e crianças com infecção aguda e notificação dos casos. Nos primeiros quatro anos da implantação, a avaliação do programa demonstrou uma redução de 63% no número de gestantes e de 42% no número de crianças encaminhadas aos serviços de referência, resultando na liberação de vagas para o atendimento de pacientes com outras doenças. Quanto aos medicamentos, houve redução de 62% no consumo de ácido folínico e de 67% de sulfadiazina. Além disso, a definição dos protocolos resultou na padronização do atendimento e segurança para a tomada de decisões por parte dos médicos. Portanto, como existem diversos protocolos individualizados nos diversos serviços e regiões, o estabelecimento de uma conduta ideal e consensual, com respaldo técnico, implicará na adoção de medidas que, certamente, ocasionarão economia aos cofres públicos, com a diminuição da toxoplasmose congênita.SummaryCongenital toxoplasmosis can cause miscarriage and neurological and/or eye damage to the fetus. Since Austria and France established the prenatal screening, the prevalence of toxoplasmosis has declined from 50% to 35% and 84% to 44%, respectively. Other countries, such as the United Kingdom, have educational practices to reduce the risk of infection in seronegative pregnant women. In Brazil, prenatal screening is carried out in the states of Mato Grosso do Sul and Minas Gerais and the cities of Curitiba and Porto Alegre. In Londrina, state of Parana, the “Health Surveillance Program for Toxoplasmosis Acquired during Pregnancy and Congenital Toxoplasmosis” was established, which is based on serological screening, advising on prevention measures and quarterly serological monitoring in pregnant women that are initially seronegative, in addition to the monitoring of pregnant women and children with acute infection and case notification. In the first four years of implementation, the program evaluation showed a 63% reduction in the number of pregnant women and 42% in the number of children referred to reference services, resulting in the opening of vacancies for the care of patients with other diseases. As for medications, there was a 62% reduction in consumption of folic acid and 67% of sulfadiazine. Moreover, the definition of the protocols resulted in the standardization of care and safety for the decision-making by physicians. Therefore, as there are several protocols individualized in various departments and regions, the establishment of an ideal, consensual conduct with technical support, will result in implementing measures that will certainly save public resources, with the decrease in congenital toxoplasmosis.

PURPOSE: To report the findings of optical coherence tomography (OCT) in three cases of diffuse unilateral subacute neuroretinitis (DUSN). METHODS: Three young patients with confirmed diagnosis of diffuse unilateral subacute neuroretinitis were followed-up using Stratus® OCT. RESULTS: Optical coherence tomography findings included retinal nerve fiber layer atrophy and focal edema where the worm was initially lodged. In two patients we could identify the worm by optical coherence tomography as an area of hiper-reflectivity. The worms were laser photocoagulated and post-treatment optical coherence tomography scanning revealed improvement of edema, nerve fiber layer thinning and a retinal hyper-reflectivity where laser had been applied. CONCLUSION: The main finding reported here is the potential of optical coherence tomography in detecting the presence of retinal nerve fiber layer atrophy and focal retinal edema in areas affected by the worm. Optical coherence tomography can be used to distinguish diffuse unilateral subacute neuroretinitis from other mimicking diseases like punctate outer retinitis, when there are no retinal fiber layer atrophy.

PURPOSE: To determine the prevalence of early and late-stage age-related macular degeneration (ARMD) and its association with risk factors such as age, gender, smoking, body mass index, hypertension and diabetes history, cataracts and pseudophakia. Design: Population-based cross-sectional study in an elderly Japanese-Brazilian population from Londrina (Parana, Brazil). METHODS: The study included 483 (80.5%) of the 600 registered members of a local association for Japanese immigrants and their descendants, aged 60 years and up. The presence of early and late-stage age-related macular degeneration was determined using the standard protocol and the international classification system. RESULTS: The mean age of the study subjects was 71 years. The overall prevalence of age-related macular degeneration was 15.1% (CI 95%; 12-18.7). The prevalence of early-stage age-related macular degeneration was 13.8% (CI 95%; 10.9-17.3), geographic atrophy was present in 0.4% and neovascular age-related macular degeneration in 0.8%. Age-related macular degeneration was significantly (p=0.004) and linearly (p=0.001) associated with age. CONCLUSION: Our study population displays a prevalence of early and late-stage age-related macular degeneration and component lesions similar to those of other Western countries, and data suggest a higher prevalence than that reported for populations in Japan. Since the age-related macular degeneration prevalence tends to rise as the population ages, studies identifying risk factors and exploring prevention methods are becoming increasingly important.
