1. Factores de riesgo para complicaciones de hernia inguinal en pacientes operados en dos hospitales Caracas entre junio-septiembre 2023

Abstract

La cura operatoria de hernia inguinal es una cirugía común con complicaciones frecuentes. En este estudio, se explora la relación entre la cirugía de hernia inguinal y los factores de riesgo, como diabetes mellitus, hipertensión arterial, uso de esteroides, entre otros. Se analizaron 41 casos, con una tasa de complicaciones en el 39%, considerándose como complicación cualquier desviación del postoperatorio normal. Factores como la edad, el hábito tabáquico, la hipertensión y las cirugías abdominales fueron evaluados, la relación con las complicaciones no fue estadísticamente significativa. Aunque el seguimiento de 30 días no reveló recurrencias ni complicaciones graves, la tasa general de complicaciones fue mayor que en estudios internacionales, sugiriendo posible relación con los aspectos técnicos, cirugías practicadas en centros de entrenamiento o factores propios de la población. En conclusión, este trabajo establece una base valiosa para futuras investigaciones sobre la hernia inguinal en Venezuela, abogando por una expansión continua y una evaluación más detallada de los factores de riesgo y su impacto en la evolución postoperatoria.

3. A new technique to measure the dynamic fracture toughness of solids

Abstract

We propose and assess a new experimental technique to measure the fracture toughness of engineering materials and its sensitivity to strain rate. The proposed method is based on a ring expansion technique and it overcomes the limitations of current dynamic fracture tests, as it is not affected by transient stress wave propagation during loading and it results in spatially uniform remote stress and strain fields prior to fracture; the method is also suitable to achieve remote strain rates well in excess of 1000 s−1. We demonstrate the technique by measuring the plane-stress Mode I fracture toughness of PMMA specimens at remote strain rates ranging from 10−3 s−1 to 102 s−1. The experiments show an increase of the toughness of the material with increasing strain rate.

Abstract

Uterine arteriovenous malformations (AVMs) is a rare but high-risk cause of uterine bleeding. The clinical management of this condition is challenging, as the ultrasound picture can sometimes be unambiguously interpreted. Moreover, in the puerperium in which acquired AVMs are most frequently formed, it is necessary to discuss the correct management in a multidisciplinary and personalized manner. We present two cases of AVMs developing in the puerperium, both with a vaginal delivery and spontaneous and complete secondment. The symptom of onset was an episode of bright red blood loss in the puerperium, on the 14th and 21st postpartum days, respectively. Transvaginal ultrasound showed a hypervascularized lesion in the myometrium with turbulent vascular flow, confirmed by transabdominal ultrasound and angiography. To date, there are no guidelines on the management of MAVs. In our cases we opted for a conservative approach, in order to preserve the fertility of the patient. These experiences reported have the purpose of enriching a literature still sparse on the subject and in the future to be able to represent a fulcrum for official recommendations.

5. An In Vitro Assay to Quantify Effects of Micro- and Nano-Plastics on Human Gene Transcription

Abstract

In today’s age, plastic waste is a major problem for our environment. The decomposition of plastic waste causes widespread contamination in all types of ecosystems worldwide. Micro-plastics in the lower micrometer size range and especially nano-plastics can become internalized by cells and thus become a threat to human health. To investigate the effects of internalized micro- and nano-plastics on human gene transcription, we used an in vitro assay to quantify CREB (cAMP response element binding protein) mediated transcription. Here we show that CREB mediated gene expression was mainly but not exclusively induced by phosphorylation. In addition, the amount of CREB affected transcription was also studied. We were also able to show that the strong CREB mediated stimulation of transcription was diminished by micro- and nano-plastics in any chosen setting. This indicates a threat to human health via the deregulation of transcription induced by internalized micro- and nano-plastics. However, this established quantifiable in vitro transcription test system could help to screen for toxic substances and non-toxic alternatives.

Abstract

Objectives: Apical prolapse involves the upper vagina or vaginal vault after hysterectomy. Treatment is indicated for symptomatic women, and surgical approach is considered for women who failed or refused conservative therapy. We performed 10 pickups of autologous fascia, used for robotic sacrocolpopexy (RSCP). Materials and Methods: We included patients between 60 and 80 years old who showed a Pelvic Organ Prolapse Quantification (POP-q) over the second stage and with symptoms related to prolapse. Results: All of them underwent autologous fascia lata (AFL) pickup from the right leg and after to RSCP. One patient underwent also posterior colporrhaphy. The mean intraoperative time was 199.2 min (183–230 min). No intra-operatory complications were reported. POP-q assessment during follow-up showed improvements: C point gained on average 7.6 points (5–8) and mean values went from −0.6 to − 8.2 cm (−7 to −9 cm). The three women who had anterior compartment defects shows good anatomical reconstitution with a mean Aa and Ba value of − 2.83 cm (−2.5 to −3 cm) and gained 4 points (average gain: 3.5–4.5 cm). Total vaginal lenght (TVL). Conclusion: According to these data, in our experience, AFL employment showed a good anatomical result from the first to last follow-up.

7. A strategy to formulate data-driven constitutive models from random multiaxial experiments

Abstract

Abstract We present a test technique and an accompanying computational framework to obtain data-driven, surrogate constitutive models that capture the response of isotropic, elastic–plastic materials loaded in-plane stress by combined normal and shear stresses. The surrogate models are based on feed-forward neural networks (NNs) predicting the evolution of state variables over arbitrary increments of strain. The feasibility of the approach is assessed by conducting virtual experiments, i.e. Finite Element (FE) simulations of the response of a hollow, cylindrical, thin-walled test specimen to random histories of imposed axial displacement and rotation. In these simulations, the specimen’s material is modelled as an isotropic, rate-independent elastic–plastic solid obeying J2 plasticity with isotropic hardening. The virtual experiments allow assembling a training dataset for the surrogate models. The accuracy of two different surrogate models is evaluated by performing predictions of the response of the material to the application of random multiaxial strain histories. Both models are found to be effective and to have comparable accuracy.

Abstract

Laser powder bed fusion (L-PBF) of engineering thermoplastics is a well-established additive manufacturing technique. In this work, an experimental investigation of the tensile-shear behaviour of L-PBF processed polyamide-12 (PA12) is presented, with a particular focus on the concurrent effect of build orientation and rate dependency at strain rates ranging from 10−3 to 103 s−1. Results show that the deformation behaviour features significant anisotropy at both low and high strain rates, while quasi-static ultimate strength in either tension or shear shows no dependence on the build direction. As the strain rate increases, the tensile strength is characterised by apparent positive rate dependence but remains insensitive to the build orientation; however the shear strength displays significant build orientation-dependent anisotropy, highlighted by the decreasing shear strength of specimens printed in the loading direction. Post-mortem scanning electron microscopy (SEM) demonstrates torn dimples with stretched filaments in quasi-static fractography, while cleavage facets with flake structures retrieved from high-rate loading. A fibrillar structure is observed from high-rate torsional loading indicating adiabatic decohesion during rapid crack propagation. These findings draw attention to the significant anisotropy of L-PBF PA12 parts in a high-rate loading regime and provide directions to improve the current L-PBF techniques.

Abstract

This study presents a technique to develop data-driven constitutive models for the elastic-plastic response of materials, and applies this technique to the case of commercially pure titanium. The complex yield and strain hardening characteristics of this solid are captured for random non-monotonic uniaxial loading, without relying on specific theoretical descriptions. The surrogate model is obtained by supervised machine learning, relying on feed-forward neural networks trained with data obtained from random loading of titanium specimens in uniaxial stress. Uniaxial tests are conducted in strain control, applying random histories of axial strain in the range [−0.04, 0.04], to prevent the occurrence of significant damage. The corresponding stress versus strain histories are subdivided into a finite number of increments, and machine learning is applied to predict the change in stress in each increment. A suitable architecture of the data-driven model, key to obtaining accurate predictions, is presented. The predictions of the surrogate model are validated by comparing to experiments not used in the training process, and compared to those of an established theoretical model. An excellent agreement is obtained between the measurements and the predictions of the data-driven surrogate model.

12. Dynamic necking of a near α titanium alloy at high strain rates: Experiments and modelling

Abstract

The tensile behaviour of near α Ti3Al2.5 V alloy, conceived for applications in aerospace and automotive engineering, is characterized from quasi-static to high strain rates. The material is found to present noticeable strain rate sensitivity. The dynamic true strain rate in the necking cross-section reaches values up to ten times higher than the nominal strain rate. It is also observed that beyond necking the dynamic true stress-strain curves present limited rate dependence. The experimental results at different strain rates are used to determine a suitable constitutive model for finite element simulations of the dynamic tensile tests. The model predicts the experimentally macroscopic force-time response, true stress-strain response and effective strain rate evolution with good agreement.

ABSTRACT Severe functional mitral valve regurgitation should be treated in patients undergoing myocardial revascularization. When replacement is considered the best therapeutic option, preservation of the mitral subvalvular apparatus is crucial, especially in the emergency setting, because of its primary role in preserving geometry and function of left and right ventricles. Here we present a simple and quick technique, where subvalvular apparatus is preserved in toto in patients undergoing mitral valve replacement with a bioprosthesis.

Abstract

Polyneuropathies (PNP) are the most common type of disorder of the peripheral nervous system in adults. However, information on microRNA expression in PNP is lacking. Following microRNA sequencing, we compared the expression of microRNAs in the serum of patients experiencing chronic painful PNP with healthy age-matched controls. We have been able to identify four microRNAs (hsa-miR-3135b, hsa-miR-584-5p, hsa-miR-12136, and hsa-miR-550a-3p) that provide possible molecular links between degenerative processes, blood flow regulation, and signal transduction, that eventually lead to PNP. In addition, these microRNAs are discussed regarding the targeting of proteins that are involved in high blood flow/pressure and neural activity dysregulations/disbalances, presumably resulting in PNP-typical symptoms such as chronical numbness/pain. Within our study, we have identified four microRNAs that may serve as potential novel biomarkers of chronic painful PNP, and that may potentially bear therapeutic implications.

17. Transient Receptor Potential Vanilloid 1 Signaling Is Independent on Protein Kinase A Phosphorylation of Ankyrin-Rich Membrane Spanning Protein

Abstract

The sensory ion channel transient receptor potential vanilloid 1 (TRPV1) is mainly expressed in small to medium sized dorsal root ganglion neurons, which are involved in the transfer of acute noxious thermal and chemical stimuli. The Ankyrin-rich membrane spanning protein (ARMS) interaction with TRPV1 is modulated by protein kinase A (PKA) mediating sensitization. Here, we hypothesize that PKA phosphorylation sites of ARMS are crucial for the modulation of TRPV1 function, and that the phosphorylation of ARMS is facilitated by the A-kinase anchoring protein 79 (AKAP79). We used transfected HEK293 cells, immunoprecipitation, calcium flux, and patch clamp experiments to investigate potential PKA phosphorylation sites in ARMS and in ARMS-related peptides. Additionally, experiments were done to discriminate between PKA and protein kinase D (PKD) phosphorylation. We found different interaction ratios for TRPV1 and ARMS mutants lacking PKA phosphorylation sites. The degree of TRPV1 sensitization by ARMS mutants is independent on PKA phosphorylation. AKAP79 was also involved in the TRPV1/ARMS/PKA signaling complex. These data show that ARMS is a PKA substrate via AKAP79 in the TRPV1 signaling complex and that all four proteins interact physically, regulating TRPV1 sensitization in transfected HEK293 cells. To assess the physiological and/or therapeutic significance of these findings, similar investigations need to be performed in native neurons and/or in vivo.

Abstract

Helicobacter pylori (H. pylori) resistance to antibiotics has increased worldwide in recent decades, especially to clarithromycin. As a result, the World Health Organization (WHO) identified clarithromycin-resistant H. pylori as a “high priority” pathogen in 2017. As international guidelines recommend empirical therapy as first-line treatment, it is crucial to know local resistance rates and history of antibiotic use to determine the most appropriate first-line antibiotic treatment. Italy is one of the European countries with the highest prevalence of H. pylori infection and the highest percentage of antibiotic-resistant H. pylori. The aim of this review is to summarize all data on H. pylori antibiotic resistance in Italy in order to quantify the current rate and determine the most effective therapeutic approach. The study confirms an elevated level of resistance to clarithromycin, metronidazole, and levofloxacin in Italy. In addition, our results show a satisfactory eradication rate for a bismuth-based regimen when used as first- or second-line treatment. Naive patients are also successfully treated with clarithromycin-based quadruple therapies. Considering the good results of bismuth-based therapy as recovery therapy, this argues for the potential use of clarithromycin quadruple therapy as a first-line treatment.

20. The role of hysteroscopy in evaluating postmenopausal asymptomatic women with thickened endometrium

Abstract

Backgrounds and Aims: This study aims to compare hysteroscopic and histological findings in asymptomatic postmenopausal patients with thickened endometrium. Materials and Methods: A retrospective study involving case records of 295 asymptomatic postmenopausal women with a thickened endometrium >5 mm diagnosed at transvaginal ultrasound (TVS). Patients (women) underwent hysteroscopy with biopsy between 2009 and 2015, and they were followed up at National Cancer Institute of Bari and at University Hospital of Pisa. Sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) of hysteroscopy were evaluated. Results: Inclusion criteria were TVS, hysteroscopy, and endometrial biopsy. When the hysteroscopic findings were normal, a sensitivity of 100%, specificity of 98.6%, PPV of 95.2%, and NPV of 100% were achieved. For polyps and myomas, we found 100%, 98.7%, 99.5%, and 100%, respectively. In case of endometrial hyperplasia, a sensitivity of 66.7%, a specificity of 100%, a PPV of 100%, and a NPV of 98.1% were achieved. For endometrial cancer hysteroscopy, sensitivity, specificity, PPV, and NPV were 100%, 99.6%, 75%, and 100%, respectively. Conclusions: Hysteroscopy allows an accurate diagnosis in benign endometrial pathology and suspect of malignant endometrial pathology in postmenopausal women with thickened endometrium.

21. In-hospital and mid-term outcomes of patients operated on for type A acute aortic dissection complicated by postoperative malperfusion

Abstract

Aims: To evaluate the effect of postoperative malperfusion (PM) on operative mortality and on late survival in patients who underwent surgery for acute type A aortic dissection in a referred center for aortic emergency surgery. Patients and Methods: From January 2005 to September 2015, 237 patients were referred for aortic emergency surgery at our center. We examined complete data available on 214 patients (mean age 62.5±12.6 years, 156 males). At presentation, various types of preoperative malperfusion (cerebral, renal, mesenteric) were observed in 119 patients (55.6%). Arterial access for cardiopulmonary bypass was via femoral artery (n = 99), via axillary artery (n = 99), or into the ascending aorta (n = 22). Aortic repair was performed using an open technique in 124 patients (58%). Results: Fifty-five patients (25.7%) presented PM; operative mortality was 29% (62/214): 47.3% in PM patients vs. 22.6% in non-PM patients (P < 0.0001). Independent predictors were age >75 years at the time of operation (OR: 1.1, P = 0.0004) and renal PM (OR: 53.5, P = 0.0027). Five-year survival was 79±7% in PM vs. 94±3% in non-PM patients (P = 0.002). Independent predictors for reduced survival were age >75 years (OR: 375, P = 0.05) and renal PM (OR: 28.6, P = 0.01). All types of PM and the location of intimal tear distal to the ascending aorta were found as risk factors for survival in the univariate analysis only (P < 0.05). Conclusions: Surgery for acute aortic dissection is effective in reducing preoperative malperfusion by about 50%. Renal PM is associated with higher operative mortality, whereas all types of PM, in particular renal PM, negatively affected late survival. Surgical techniques, site of arterial cannulation, and more complex interventions requiring an open technique did not appear to be predictors of increased risk. Aims: A connection between psychiatric disorders (PDs) and Type A acute aortic dissection (AAD) has not been shown. The aim of this study was to define the psychological profile of patients treated for AAD, and to analyze the prevalence of PDs in their medical histories, in the immediate postoperative period, and at a mid-term follow-up. Patients and Methods: From March 2005 to October 2014, 240 consecutive patients underwent surgery for AAD. 60 patients (mean age 60+/-13 years; 43 males) underwent psychiatric consultation postoperatively, and they represent the subjects of our retrospective study. Ascending aorta +/- arch replacement was performed in 43 patients, whereas the Bentall procedure +/- arch replacement was performed in 17. Data were retrospectively analyzed. Follow-ups were completed in 59 patients (mean duration 35+/-23 months). Results: PDs were present in the medical histories of 34 patients. Postoperatively, in 28 cases, a definitive diagnosis of PD (group PD) was made in agreement with the diagnostic and statistical manual of mental disorders-IV criteria, including: Major depression (n=13), anxious-depressive syndrome (n=6), bipolar disorder Type 2 (n=4), panic attacks (n=2), paranoid schizophrenia (n=1), and anxiety (n=2). 32 patients without a definitive psychiatric diagnosis were classified as Group non-PD. Abstract

The tensile testing of a needle-punched nonwoven fabric is presented. A high-sensitivity Split-Hopkinson Tensile Bar device was specifically designed for this purpose. The strain gauge measurements were combined with high-speed photography and Digital Image Correlation to analyse the deformation micromechanisms at high strain rates. The experimental set-up allowed to determine the wave propagation velocity of the as-received nonwove fabric, the evolution of the strain field with deformation and the wave interaction inside the fabric. The deformation was accommodated by the same micromechanisms observed during quasi-static tensile testing and ballistic impact, which comprised fibre straightening, rotation and sliding. Heterogeneous strain fields were developed in the nonwoven fabric as a result of the non-linear pseudoplastic response of the fabric and the internal dissipation due to the frictional deformation micromechanisms, preventing the propagation of high magnitude strain waves into the specimen. Additionally, the output forces were analysed to determine the influence of high-strain rates in the mechanical response of the nonwoven fabric, finding an increment of the stiffness for low applied strains under dynamic loading. These findings provide the basis to develop strain-rate dependent constitutive models to predict wave propagation in needle-punched nonwoven fabrics when subjected to impact loads.

Abstract

Objectives: To evaluate the role of 18-fluoro-2-deoxyglucose positron emission tomography–computed tomography (18F-FDG-PET–CT) in the diagnosis of infectious endocarditis (IE). Methods: We retrospectively examined 27 consecutive patients who were admitted to the Infectious Diseases Department of Tor Vergata University Hospital between 2009 and 2013 with a suspicion of IE. The final IE diagnosis was defined according to the modified Duke criteria, and the microbiological and diagnostic results were collected for each patient. Results: Twenty out of 27 patients had a suspected prosthetic valve endocarditis (PVE) and seven had a suspected native valve endocarditis (NVE). Twenty-five out of 27 patients (92%) had a confirmed diagnosis of IE (18/25 PVE and 7/25 NVE); 16 had a positive echocardiography evaluation and 16 had positive 18F-FDG-PET–CT findings. Echocardiography showed a higher sensitivity as a diagnostic tool for the detection of IE compared to 18F-FDG-PET–CT (80% vs. 55%). However, a greater number of PVE had positive 18F-FDG-PET–CT results compared to those with positive echocardiography findings (11/13 vs. 9/13), and overall 89% (16/18) of confirmed PVE resulted 18F-FDG-PET–CT positive. Analyzing only the cases who underwent transoesophageal echocardiography, 18F-FDG-PET–CT showed a sensitivity of 85% in PVE (vs. 69% for echocardiography and 77% for the Duke criteria). Aim of the study. To verify the impact of the etiology of mitral valve regurgitation on a 5-year outcome after repair and concomitant coronary artery bypass grafting (CABG). Methods. Abstract

A careful management of hepatic capsular rupture, with massive hemoperitoneum which occurred 14 hours after an emergency cesarean section at 36 weeks of gestation, is meticulously reported. The grade of hepatic involvement varies from minor capsular laceration to extensive parenchymal rupture. Our management involved a combination of surgical interventions and aggressive supportive care. The patient was discharged after 53 days and 4 laparotomies and an unsuccessful attempt of superselective artery embolization. Ultrasound after 40 days from the last surgery showed uniform hepatic parenchyma free of focal lesions. Due to the rarity and the unpredictability nature of this devastating event we believe necessary to report our experience, reinforcing the importance of the postsurgery management.

Abstract

This work investigates the high strain rate behavior of AP-PLY composites. The large representative volume elements and brittle nature of this material necessitated the use of a bespoke Split-Hopkinson bar apparatus. AP-PLY and baseline laminates were subjected to tensile loading at strain rates of 30 s-1. Results were compared with quasi-static data to evaluate whether the laminate architecture introduced any strain rate dependency. In addition, the dynamic experiments were simulated using a multiscale modeling framework, providing further insights into the micromechanisms governing material behavior. The moduli of the AP-PLY composites were found to be strain rate independent, however, strengths were found to be marginally higher than those of their baseline counterparts. At high strain rates, the strain concentrations induced by the geometry of the individual tapes at through thickness undulations and tow boundaries were less significant due to reduced out-of-plane tow straightening and delamination. As a result, no reduction in AP-PLY strength in comparison to the baseline laminates was obtained. These differences in deformation micromechanisms led to an improvement of the damage tolerance when subjected to dynamic loading.

The gut microbiota plays a critical role in the modulation of host metabolism and immune response, and its impairment has been implicated in many gastrointestinal and extraintestinal diseases. Current evidence shows the well-documented role of A. muciniphila in maintaining the integrity of the intestinal barrier, modulating the host immune response, and improving several metabolic pathways, making it a key element in the pathogenesis of several human diseases. In this scenario, A. muciniphila is the most promising next-generation probiotic and one of the first microbial species suitable for specific clinical use when compared with traditional probiotics. Further studies are needed to provide more accurate insight into its mechanisms of action and to better elucidate its properties in several major areas, paving the way for a more integrated and personalized therapeutic approach that finally makes the most of our knowledge of the gut microbiota.

Abstract

Multifunctionality, lightweight and resistance to corrosion are some of the several advantages that fibre composites display with respect to other materials across a wide range of applications. In aerospace and marine industry, however, we can encounter challenges arising from certain working conditions that can significantly affect the mechanical response and integrity of composite structures. First, humidity or water exposure can induce reduction of the material strength and stiffness. Second, hot or cold temperatures that change the level of ductility in the composite. Finally, dynamics loads that may appear during service require strain rate sensitive properties to be considered. In this work, we show experimental capabilities and methodologies developed to account for all those three effects: rapidly applied loads (impact), humidity and temperatures from -55°C to 93°C. This understanding sets the basis for the development of advanced models to design better structures.

Abstract

Digital image correlation (DIC) is a widely used technique in experimental mechanics for full field measurement of displacements and strains. The subset matching based DIC requires surfaces containing a random pattern. Even though there are several techniques to create random speckle patterns, their applicability is still limited. For instance, traditional methods such as airbrush painting are not suitable in the following challenging scenarios: (i) when time available to produce the speckle pattern is limited and (ii) when dynamic loading conditions trigger peeling of the pattern. The development and application of some novel techniques to address these situations is presented in this paper. The developed techniques make use of commercially available materials such as temporary tattoo paper, adhesives and stamp kits. The presented techniques are shown to be quick, repeatable, consistent and stable even under impact loads and large deformations. Additionally, they offer the possibility to optimise and customise the speckle pattern. The speckling techniques presented in the paper are also versatile and can be quickly applied in a variety of materials.

Abstract

First evidence of a structure in the $J/\psi{\Lambda}$ invariant mass distribution is obtained from an amplitude analysis of$\Xi_b^-{\rightarrow}J/\psi{\Lambda}K^-$ decays. The observed structure is consistent with being due to a charmonium pentaquark with strangeness with a significance of $3.1\sigma$ including systematic uncertainties and look-elsewhere effect. Its mass and width are determined to be $4458.8\pm2.9^{+4.7}_{-1.1}$ MeV and $17.3\pm6.5^{+8.0}_{-5.7}$ MeV, respectively, where the quoted uncertainties are statistical and systematic. The structure is also consistent with being due to two resonances. In addition, the narrow excited $\Xi^-$ states, $\Xi(1690)^-$ and $\Xi(1820)^-$, are seen for the first time in a $\Xi^-_b$ decay, and their masses and widths are measured with improved precision. The analysis is performed using $pp$ collision data corresponding to a total integrated luminosity of 9 fb$^{-1}$, collected with the LHCb experiment at centre-of-mass energies of 7, 8 and 13 TeV.

Abstract

Over the past two decades, developments in regenerative medicine in gastroenterology have been greatly enhanced by the application of stem cells, which can self-replicate and differentiate into any somatic cell. The discovery of induced pluripotent stem cells has opened remarkable perspectives on tissue regeneration, including their use as a bridge to transplantation or as supportive therapy in patients with organ failure. The improvements in DNA manipulation and gene editing strategies have also allowed to clarify the physiopathology and to correct the phenotype of several monogenic diseases, both in vivo and in vitro. Further progress has been made with the development of three-dimensional cultures, known as organoids, which have demonstrated morphological and functional complexity comparable to that of a miniature organ. Hence, owing to its protean applications and potential benefits, cell and organoid transplantation has become a hot topic for the management of gastrointestinal diseases. In this review, we describe current knowledge on cell therapies in hepatology and pancreatology, providing insight into their future applications in regenerative medicine.

Severe functional mitral valve regurgitation should be treated in patients undergoing myocardial revascularization. When replacement is considered the best therapeutic option, preservation of the mitral subvalvular apparatus is crucial, especially in the emergency setting, because of its primary role in preserving geometry and function of left and right ventricles. Here we present a simple and quick technique, where subvalvular apparatus is preserved in toto in patients undergoing mitral valve replacement with a bioprosthesis.

Abstract

COVID-19 infection is associated with increased risk of pregnancy complications, making vaccination during pregnancy critical for mother-neonate dyads. Few data, often with an unrepresentative sample size, are available on SARS-CoV-2 vaccine-induced humoral and cell-mediated response. Here, we evaluated anti-S antibody and interferon-gamma (IFN-γ) production elicited by SARS-CoV-2 immunization in maternal and neonatal plasma. Pregnant women (n = 230) were prospectively enrolled and classified as unvaccinated (n = 103) and vaccinated (n = 127); after serological screening for previous infections, assays were performed on 126 dyads, 15 mothers and 17 newborns. Positive anti-S antibodies were found in most of the vaccinated subjects, regardless of timespan between immunization and delivery (range: 7–391 days). A total of 89 of 92 vaccinated women showed a broad response to COVID-19 immunization and highly effective placental transfer, as attested by anti-S positive rates (maternal = 96.7%, cord = 96.6%). Most of our subjects had indeterminate results in an IGRA assay, preventing a conclusive evaluation of IFN-γ production. Indeed, pregnancy-related hormonal changes may influence T-cell response with an impact on IFN-γ production. Positive pregnancy and perinatal outcomes reinforce the evidence that the anti-SARS-CoV-2 immunization is effective and well-tolerated in pregnant women and also protective for the fetus/neonate, even though it was not possible to define the related IFN-γ production and role.

Abstract

The nutritional management of acute pancreatitis (AP) patients has widely changed over time. The “pancreatic rest” was the cornerstone of the old paradigm, and nutritional support was not even included in AP management. Traditional management of AP was based on intestinal rest, with or without complete parenteral feeding. Recently, evidence-based data underlined the superiority of early oral or enteral feeding with significantly decreased multiple-organ failure, systemic infections, surgery need, and mortality rate. Despite the current recommendations, experts still debate the best route for enteral nutritional support and the best enteral formula. The aim of this work is to collect and analyze evidence over the nutritional aspects of AP management to investigate its impact. Moreover, the role of immunonutrition and probiotics in modulating inflammatory response and gut dysbiosis during AP was extensively studied. However, we have no significant data for their use in clinical practice. This is the first work to move beyond the mere opposition between the old and the new paradigm, including an analysis of several topics still under debate in order to provide a comprehensive overview of nutritional management of AP.

The aim of current study was to estimate the impact of vulvovaginal atrophy (VVA) on sexual function in a clinical population of Italian postmenopausal women. Women aged 45-75 years with at least one VVA symptom completed three questionnaires: Day-to-Day Impact of Vaginal Aging (DIVA), Female Sexual Function Index (FSFI) and Female Sexual Distress Scale revised (FSDS-R). A gynaecological examination was performed for VVA confirmation. Among the 1,066 evaluable patients, VVA was confirmed in around 90% of the sample. Abstract

In this paper, we present the dynamic response of commercially pure copper subjected to combined tension-torsion loads representative of real case impact scenarios. Experiments were conducted both quasi statically, at a strain rate equal to 10−3 s−1, and dynamically at strain rates in the region between 500 s−1 and 1000 s−1. All high rate experiments were conducted using a novel Split Hopkinson Tension-Torsion Bar instrumented with high-speed photographic equipment. The dynamic combined loading experiments demonstrate the capability of the apparatus to generate longitudinal and torsional stress waves which are synchronised upon loading of the specimen. The presented data show that dynamic equilibrium conditions and nearly steady strain rates were achieved during the experiments. Additionally, the analyses of the loading paths show that nearly proportional strain loading was attained during testing. The measured experimental results illustrate, for the first time, the failure stress locus of the material over a wide range of stress states including pure torsion, shear-dominated combined tension-shear, tension-dominated combined tension-shear and plain tension. The quasi-static and dynamic failure envelopes are herein presented in the normal stress vs shear stress space to motivate the development of accurate and effective constitutive models. To conclude, the Drucker-Prager criterion was employed to approximate the failure loci and to assess the rate sensitivity of the material. A moderate asymmetry of the uniaxial ultimate stresses in tension and compression is predicted both at quasi-static and dynamic strain rates.

Abstract

The effects of water content and initial compaction on the dynamic response of volcanic sand from Mount Etna were investigated by a series of experiments on a long Split Hopkinson Pressure Bar apparatus capable of generating stress pulses of duration exceeding one millisecond. The dynamic stress–strain characteristics were determined until large final compressive strains were achieved. An experimental protocol for the preparation of samples characterised by different initial porosity and moisture content was defined in order to reproduce, in a laboratory environment, granular volcanic aggregates representative of naturally occurring soils in different initial density and water content states. It was found that, for limited amounts of water content, the dynamic response of the investigated volcanic wet sand is more compliant than in dry conditions. Conversely, highly saturated samples exhibit a steep increase in stiffness occurring at strains when the dynamic compressive behaviour becomes dominated by the response of the nearly incompressible water. The presence of water has negligible effect on the mechanical behaviour when the samples are loaded at quasi static strain rates. The grain size distribution and morphology of samples tested in different conditions were evaluated and compared by means of edge detection analysis techniques applied to high contrast images.

Abstract

This paper studies the uniaxial and multiaxial dynamic compressive behaviour and failure of PA66-GF30 (30 wt % glass fibre reinforced polyamide 66), a typical light weight and high strength composite increasingly used in automobiles including the electric cars in applications from ambient temperature to elevated temperatures up to 90 °C. Likewise, the constitutive relation of PA66-GF30 is characterized from quasi-static to high strain rates. Constant high strain rate loading is achieved by pulse shaping technique on a bespoke split Hopkinson bar. The stress-strain relations are pressure, strain rate and temperature dependent. Effects of strain rate and temperature are found to be decoupled on the pressure sensitivity of PA66-GF30. Beyond maximum stress, micro crack has already formed in dynamically deformed PA66-GF30, which corresponds to macro strain localization monitored by high speed photography and digital image correlation techniques. The PA66-GF30 with confinement shows adiabatic shear failure, with fibres coated by severe shear matrix facets and evenly distributed filaments. This is different from the unconfined PA66-GF30 which shows fibres pull out in the fractured matrix. A modified Drucker-Prager model is proposed to describe the pressure dependent compressive strength of PA66-GF30 over various strain rates and temperatures.

Abstract

Background. To evaluate the impact of the Laparoscopic Approach to Cervical Cancer (LACC) Trial on patterns of care and surgery-related morbidity in early-stage cervical cancer. Methods. This is a retrospective, a multi-institutional study evaluating 90-day surgery-related outcomes of patients undergoing treatment for early-stage cervical cancer before (period I: 01/01/2016-06/01/2018) and after (period II: 01/01/2019-06/01/2021) the publication of the results of the LACC trial. Results. Charts of 1295 patients were evaluated: 581 (44.9%) and 714 (55.1%) before and after the publication of the LACC trial, respectively. After the publication of the LACC trial, the number of patients treated with minimally invasive radical hysterectomy decreased from 64.9% to 30.4% (p < 0.001). Overall, 90-day complications occurred in 110 (18.9%) and 119 (16.6%) patients in the period I and period II, respectively (p = 0.795). Similarly, the number of severe (grade 3 or worse) complications did not differ between the two periods (38 (6.5%) vs. 37 (5.1%); p = 0.297). Overall and severe 90-day complications were consistent between periods even evaluating stage IA (p = 0.471), IB1 (p = 0.929), and IB2 (p = 0.074), separately. Conclusions. The present investigation highlighted that in referral centers the shift from minimally invasive to open radical hysterectomy does not influence 90-day surgery-related morbidity. Abstract

Objective Coronavirus disease 2019 (COVID-19) outbreak has correlated with the disruption of screening activities and diagnostic assessments. Endometrial cancer (EC) is one of the most common gynecological malignancies and it is often detected at an early stage, because it frequently produces symptoms. Here, we aim to investigate the impact of COVID-19 outbreak on patterns of presentation and treatment of EC patients. Methods This is a retrospective study involving 54 centers in Italy. We evaluated patterns of presentation and treatment of EC patients before (period 1: March 1, 2019 to February 29, 2020) and during (period 2: April 1, 2020 to March 31, 2021) the COVID-19 outbreak. Results Medical records of 5,164 EC patients have been retrieved: 2,718 and 2,446 women treated in period 1 and period 2, respectively. Surgery was the mainstay of treatment in both periods (p=0.356).
