14. How Do Parents Communicate with Their Infants? : The Function of Parental Proxy Talk in Pre-Verbal Communication

Abstract

How do parents communicate with their infants before the infants learn totalk? It has been observed that the parents use Parental Proxy Talk (PPT)as if the speech came from the infants’ own voice. In other words, PPT reflectstheir expectations of what the infants were thinking and feeling. Thepresent study of PPT explored how PPT functions from birth to 15 monthsof age, and how PPT contributes to communication with pre-verbal infants.The results showed that there are three periods in the developmentof the use of PPT; (1) a gradual increase between 0 and 3 months, (2) apeak period from 6-9 months, and (3) a period of decreasing use of PPTfrom 12-15 months. The study also showed that PPT functions to supportnot only the pre-verbal infants but also parents themselves, e.g., in parents’emotion regulation.

Of these, the 999 cases that could be tested for alcohol within 48 h of death were examined.Results: The alcohol-positive and alcohol-negative groups included 179 (17.9%) and 820 (82.1%) cases, respectively. In terms of medical issues, compared with the alcohol-negative group, men in the positive group were mostly in their 40s to 60s, whereas women’s age peaked in the 70s. The causes of death included many blunt injuries in men, though there was no difference in women. Underlying diseases were more frequent in men than women, with many of them having heart or liver disease, and the percentage of cases with mental disorders was 16.8% in the alcohol-positive group. In 15 cases of the alcohol-positive group, phenothiazine and barbituric acids were detected in 53.3% and 46.7% of cases, respectively. The incidence of traffic accidents and homicides was higher in the alcohol-positive group than in the alcohol-negative group.Conclusion: Measures to address social issues based on risk factors for alcohol and deaths are required.Keywords: forensic autopsy, alcohol

Abstract

P#345;edkládaná studie zkoumala aktuální stav a p#345;í#269;iny smrtelných pracovních úraz#367; ze zdravotn#283;-sociálního hlediska. Retrospektivn#283; jsme hodnotili p#345;ípady smrtelných pracovních úraz#367; mezi lety 2007 a 2016 v Ósace, která je druhým nejlidnat#283;j#353;ím m#283;stem v Japonsku, z forenzních posmrtných dat 1689 zem#345;elých. Z t#283;chto p#345;ípad#367; jsme analyzovali celkem 57 smrtelných pr#367;myslových nehod. Ob#283;#357;mi byli typicky mu#382;i v relativn#283; pokro#269;ilém v#283;ku pracující generace, tém#283;#345; polovina p#345;ípad#367; (p#345;esn#283;ji 28) byli mu#382;i mezi 50 a 70 lety#382;ivota. Nej#269;ast#283;j#353;ím typem nehody byl pád z vý#353;e (24 p#345;ípad#367;) a nej#269;ast#283;j#353;ím místem, kde k nehodám docházelo, bylo staveni#353;t#283; (28 p#345;ípad#367;). V#283;t#353;ina nehod (29 p#345;ípad#367;) se odehrála na pracovi#353;ti s malým pracovním týmem. V 60 % p#345;ípad#367; ob#283;#357; na sob#283; nem#283;la ochranou helmu a ve 34 p#345;ípadech (60 %) byla nehoda p#345;ipisována vlastní nedbalosti. Nehody se nej#269;ast#283;ji stávaly v úterý. Nejb#283;#382;n#283;j#353;í doba, kdy k nehodám docházelo, byla kolem 10. a 11. hodiny dopolední a kolem 1. a 2. hodiny odpolední. V p#345;ibli#382;n#283; 60 % p#345;ípad#367;#382;alude#269;ní obsah zahrnoval i zbytky potravy, co#382; nazna#269;uje,#382;e k nehodám typicky docházelo po jídle. V p#345;edkládané studii jsme zjistili,#382;e pracovní nehody, z nich#382; nej#269;ast#283;j#353;í byl pád z vý#353;e, vykazují tendenci se vyskytovat na pracovi#353;ti s malým pracovním týmem. Pouze pár ob#283;tí m#283;lo na sob#283; ochranou helmu. Tato zji#353;t#283;ní nazna#269;ují,#382;e na pracovi#353;tích by m#283;ly být d#367;kladn#283; zavád#283;ny systémy monitorování a #345;ízení bezpe#269;nosti a #382;e pracovníci by m#283;li p#345;ed i po jídle dbát zvý#353;ené opatrnosti.

24. Toxicological and Biochemical Analyses of an Autopsy Case Involving Oral Overdose of Multiple Antidiabetic and Antihypertensive Drugs

Oral antidiabetics can cause fatal hypoglycemia; although they can be chemically identified and quantified, biochemical investigations are important for assessing the biological consequences of an overdose. Such cases of overdose involving oral antidiabetics may involve other drugs for treating lifestyle-related diseases, particularly antihypertensives. Here, we report a toxicological and biochemical investigation of drugs and biochemical profiles in a fatal overdose involving multiple oral antidiabetics and antihypertensives. A 55-year-old woman died about 2 days after the ingestion of around 110 tablets of antidiabetics and antihypertensives that had been prescribed for her husband. A forensic autopsy and histological analysis demonstrated no evident pathology as the cause of death. A toxicological analysis suggested hypoglycemia and an overdose of antihypertensives as well as the retention of antidiabetics and diuretics in the pericardial fluid. A relatively low pericardial amlodipine concentration was observed, which may have been the result of its long half-life (slower distribution and reduction rate) and/or possible affinity with the myocardium. In addition, a biochemical analysis indicated hypoglycemia, without increased serum insulin and C-peptide, but with increased glucagon levels, as the possible influence of glibenclamide overdose. These observations suggest the usefulness of a combination of toxicological and biochemical analyses in postmortem investigations involving a fatal overdose of such drugs.

Abstract

妊婦が胎動をどのような感覚としてとらえているか。本研究では,胎動そのものの変化に対して,妊婦がどのような感受性を示すかを検討する。妊婦の胎動への感受性の指標として,妊娠中に記録された胎動日記において,胎動を表現するために用いられたオノマトペに着目した。妊婦38 名から得られた胎動日記1032 を分析した結果,妊娠期には,実にさまざまなオノマトペが用いられていることがわかった。オノマトペに用いる音が豊富であり,語基の変形だけでなく,臨時のオノマトペの使用も認められた。また,胎動を表すオノマトペと胎児への意味づけとの関連について週齢変化を検討した結果,3 つの時期に整理することができた。第一期(～ 28 週)は胎動のオノマトペが多様性を帯び,第三期(35 ～ 40 週)に向けて,オノマトペの多様性より,胎児への意味づけの多様性が増すように変化することを見いだした。最後に,母子の身体接触としての胎動という観点から考察した。, How sense does a pregnant woman have at fetal movement? The present study examined how pregnant women (N=38) described fetal movement in order to consider women's perceptions of relationships with their fetuses. Especially, it focused on how onomatopoeia they use to express their sense at fetal movement. Participants kept a pregnancy diary about fetal movement. Analysis of the 1032 diary entries found out that pregnant women used very different kinds of idiomatic onomatopoeia, and also produced temporary onomatopoeia. Further, it also found out that in each of three periods (e.g., before 28th week, 29th – 34th week, and 35th – 40th week), pregnant women used different onomatopoeia for fetal movement. 32. Evaluation of the Relationships between Intestinal Regional Lymph Nodes and Immune Responses in Viral Infections in Children.

Abstract

Viral infections increase the risk of developing allergies in childhood, and disruption of mucosal homeostasis is presumed to be involved. However, no study has reported a role for viral infections in such disruption. In this study, we clarified the mechanism of immunoglobulin A (IgA) overproduction in viral infections. Autopsies were performed on 33 pediatric cases, IgA and interferon (IFN)β levels were measured, and histopathological and immunohistochemical examinations were conducted. Furthermore, we cultured human cells and measured IFNβ and IgA levels to examine the effect of viral infections on IgA production. Blood IgA levels in viral infections were higher than in bacterial infections. Moreover, IFNβ levels in most viral cases were below the detection limit. Cell culture revealed increased IgA in gastrointestinal lymph nodes, especially in Peyer's patches, due to enhanced IFNβ after viral stimulation. Conversely, respiratory regional lymph nodes showed enhanced IgA with no marked change in IFNβ. Overproduction of IgA, identified as an aberration of the immune system and resulting from excessive viral infection-induced IFNβ was observed in the intestinal regional lymph nodes, particularly in Peyer's patches. Further, increased IgA without elevated IFNβ in the respiratory system suggested the possibility of a different mechanism from the gastrointestinal system. [ABSTRACT FROM AUTHOR]

33. Detection of Borrelia duttonii, a Tick-Borne Relapsing Fever Agent in Central Tanzania, Within Ticks by Flagellin Gene-Based Nested Polymerase Chain Reaction

Abstract

Argasid ticks collected in the site near Mvumi Hospital, Dodoma, Tanzania, were subjected to flagellin gene-based nested polymerase chain reaction (PCR) amplification for examination of borrelial infections. Eight of 13 ticks gave a strong 350-bp signal; three had a weak signal at the same size, and the rest were negative. Sequence determination of eight of the positive samples resulted in three types of flagellin gene sequences. The first type of sequence (shown by three individuals) was identical to that of Borrelia duttonii strain Ly. The second type of sequence from PCR products from four individual ticks had only one base substitution without amino acid alteration in deduced protein sequence. The third type of sequence was different from that of any other Old World relapsing fever borreliae, and the tick was thought to be infected with an unknown Borrelia species. Ticks were also examined to determine the nucleotide sequence of the mitochondrial 16S rRNA gene. The partial sequence of ˜470 bases was aligned for comparison with previously published sequences to identify the species. The sequences of 13 individual ticks were all identical, and the sequence similarity analysis revealed the ticks should be classified as members of the Ornithodoros porcinus species. The PCR method described in this report appears to be a reliable tool for the detection of borreliae and epidemiological study of tick-borne relapsing fever.
