Lower trapezius (LT) transfer using Achilles tendon allograft or semitendinosus autograft is effective in restoring external rotation in massive irreparable posterosuperior rotator cuff tears (RCT). The purpose of this study was to evaluate if the infraspinatus fascia (IF) could be used in LT transfer to extend the LT tendon. Eight fresh-frozen whole-body cadavers were dissected using both shoulders, beach chair position. A 2.5–3 cm wide bundle of the IF was dissected, from the inferior angle of the scapula up to the insertion of the LT which was then detached subperiosteally in continuity with the fascia. The extended tendon was reinforced with sutures and brought to the center of the footprint of the supraspinatus on the humerus. We measured: (a) the distance between the insertion of the LT on the scapula and the inferior angle of the scapula, estimating the length of the IF that can be harvested, (b) the distance between the insertion of the LT on the scapula and the center of footprint of the infrastinatus on the humerus, estimating the distance needed to be covered by the graft (c) the length of the extended tendon between the center of footprint of the infrastinatus on the humerus (fixation point) and its free end, estimating the length available for fixation. The mean length of the fascia that can be harvested is 125.56 mm. The mean distance that needs to be covered by the graft is 100 mm. The extended tendon is long enough leaving 24.69 mm for fixation. The transfer of the LT extended with the IF on the footprint of the infraspinatus is feasible. It could be a viable alternative to the currently used grafts in LT transfer in irreparable posterosuperior RCT.

Background Hallux valgus is an increasingly common deformity in young female athletes that constricts their daily athletic activities and influences foot cosmesis. The aim of this study was to evaluate the outcome of modified Chevron osteotomy for hallux valgus deformity in this specific population. Methods Forty-two cases of modified Chevron osteotomies were carried out in 33 patients with mild to moderate hallux valgus deformity. Each participant was evaluated for AOFAS score, pain, range of motion, cosmetic and radiological outcome. Results Mean AOFAS score improved to 96.3 ( p p =0.138). The cosmetic result was excellent/good in 40 cases (95%). Mean metatarsophalangeal and intermetatarsal angles were decreased from 29.8° and 14.2° preoperatively to 12.2° and 8.1° postoperatively ( p p Conclusions Modified Chevron osteotomy could offer substantial correction of hallux valgus deformity in young female athletes, with excellent clinical outcome.

Introduction Dog bite wounds represent a major health problem. Despite their importance, their management and especially the role of primary closure remain controversial. In this randomised controlled trial, the outcome between primary suturing and non-closure was compared. Methods 168 consecutive patients with dog bite injuries were included in this study. The wounds were allocated randomly in two treatment approaches: Group 1, consisting of eighty-two patients, had their wound sutured, whilst Group 2, consisting of eighty-six patients, did not have their wounds sutured. All wounds were cleansed using high-pressure irrigation and povidone iodine. All patients received the same type of antibiotic treatment. Our measured outcomes included presence of infection and cosmetic appearance. Cosmetic outcome was evaluated using the Vancouver Scar Scale (VSS). Wound and patient characteristics, such as time of management, wound location and size, and patient age, were recorded and analysed for their potential role in the resulting outcome. Results The overall infection rate was 8.3%. No difference in the infection rate between primary suturing and non-suturing group was detected in the present study. The cosmetic appearance of the sutured wounds was significantly better (mean score 1.74) compared to the wounds that were left open (mean score 3.05) (p = 0.0001). The infection rate was comparable among all age groups. Wounds treated within 8 h of injury demonstrated an infection rate of 4.5%, which is lower compared to the 22.2% rate observed in wounds treated later than 8 h. The wounds located at the head and neck exhibited better results in both infection rate and cosmetic outcome. Additionally, wounds >3 cm negatively affected the cosmetic appearance of the outcome. Conclusions Primary suturing of wounds caused by dog bites resulted in similar infection rate compared to non-suturing. However, primary suturing exhibited improved cosmetic appearance. Time of management appeared to be critical, as early treatment resulted in lower infection rate and improved cosmetic appearance regardless suturing or not. Furthermore, wounds located at the head and face demonstrated better results.

Complex calcaneal defects represent a reconstructive challenge since calcaneous plays a key role in standing and gait. We report the case of a 35-year-old patient with a complex calcaneal defect due to chronic osteomyelitis after a high energy Gustillo type IIIB calcaneal fracture that was reconstructed with a free fibula-flexor hallucis longus osteomuscular flap. The fibula was osteotomized into two segments, which were used to reconstruct the bone defect, and the muscular component of the flap was used for coverage of the reconstructed calcaneal skeleton. Fifteen days later permanent skin coverage was ensured with a local random pattern rhomboid skin flap. Early and late postoperative periods were uneventful. Bone maturation was radiographically evident at a follow up of 12 weeks, and complete bone incorporation at 3 years. Full weight bearing was possible at 6 months postop. Final follow up, at 3 years postop, verified a very good functional and aesthetic outcome.

Adamantinoma is a rare, low-grade, malignant bone tumour. We report on a 46-year-old woman who had early multiple recurrences of adamantinoma of the right tibia and late metastasis to the lung and ribs 13 years after the first surgical treatment. She underwent multiple complete tumour excisions and eventually below-knee amputation and removal of the left lung and sixth to eighth ribs.

Background We present an unusual case of an aggressive and rapid progression of idiopathic bilateral avascular necrosis (AVN) of femoral head in an old woman suffering from osteoporosis.

Purpose To compare the effectiveness of buddy strapping and aluminum orthosis for treatment of proximal interphalangeal (PIP) joint hyperextension injuries. We also evaluated the effect of age on the outcome by comparing our results in adults and children. Methods One hundred twenty-one consecutive patients with a PIP joint hyperextension injury of the index, middle, ring, or little finger and without fracture were evaluated. Patients were randomly assigned into 2 groups. In the first group, treatment included buddy strapping of the injured finger to its neighbor noninjured finger for a week. In the second group, immobilization was secured with an aluminum extension blocking orthosis for a week in 15° flexion. Assessment of motion, edema, pain, and strength were performed weekly for the first month and then at 3, 6, and 12 months after injury. Results The patients treated with buddy strapping exhibited similar outcomes compared with those treated with aluminum orthoses. In patients with full recovery, buddy strapping allowed earlier recovery of motion and resolution of edema and pain compared with aluminum orthosis immobilization. Furthermore, PIP injuries appear to have better outcomes in children than in adults. Conclusions Buddy strapping is easy and effective treatment for PIP joint hyperextension injuries. We report a case of a closed subtalar dislocation without any related fractures treated with closed reduction and conservative treatment with a cast immobilization. Pure subtalar dislocation without any fractures is extremely rare and hardly reported in the literature. Such injuries are more likely to be open and associated with fractures of the surrounding foot bones such as malleoli, talus or fifth metatarsal fractures. In the examined case, closed reduction was followed by cast immobilization for 3 weeks. Six months post-injury, the patient had a full range of motion without any pain while there were no signs of residual instability or early post-traumatic osteoarthritis. Subjective clinical testing using a valid health instrument revealed an excellent outcome. We discuss in details the mechanism of such an injury and highlight the importance of prompt closed reduction and early mobilization to ensure a satisfactory long term outcome.

Isolated anterior elbow dislocation is a rare injury, and only sporadic cases have been reported in the literature. The most common mechanism of injury is a posterior force that thrusts the fl exed elbow forward. Closed reduction is usually easily achieved, but ligament tear and subsequent instability may compromise the fi nal result. We present a patient with a traumatic anterior elbow dislocation caused by an unusual mechanism of injury. Posttraumatic valgus instability was treated with repair and reconstruction of the medial collateral ligament (MCL) with a palmaris longus graft. We also reviewed the literature regarding the incidence of the injury, the frequency of associated soft tissue lesions, the available treatment options, and clinical outcomes. This study was approved by the scientifi c review board at our hospital and was conducted in accordance with the World Medical Association Declaration of Helsinki of 1964, as revised in 1983. The patient was informed that data concerning the case would be submitted for publication, and he consented.
