2. Antihypertensive medication versus health promotion for improving metabolic syndrome in preventing cardiovascular events: a success rate-oriented simulation study

Abstract

Abstract Background In practice, it is difficult to compare the effectiveness of traditional antihypertensive treatment with that of health promotion in reducing incidence rate of cardiovascular disease (IRCVD, events/year). This simulation study compared the effectiveness of two approaches to reducing IRCVD in a sample population: a traditional approach, in which high-risk patients are treated with conventional antihypertensive medications, and a population-based approach, in which subjects participate in a health promotion program. Methods We constructed a simulation model for a sample population of middle-aged Japanese men whose systolic blood pressure (SBP) levels are normally distributed (130 ± 20 mm Hg). The principal assumption was that IRCVD increases exponentially according to SBP. The population IRCVD was calculated as the product of the distribution of SBP multiplied by IRCVD at each SBP. The cumulative IRCVD was calculated by the definite integral from the lowest to the highest SBP of IRCVD at each SBP level. The success rates were calculated according to SBP and metabolic risk profiles in the two approaches, respectively. Results The reduction in IRCVD was twice as large for antihypertensive medications as it was for health promotion in several situations. For example, if adherence to antihypertensive treatment occurred at a realistic level, the decrease in IRCVD was estimated at 9.99 × 10-4. In contrast, even if the health program was promoted optimistically, the decrease in IRCVD was estimated at 4.69 × 10-4. Conclusions The success rate-oriented simulation suggests that prescribing antihypertensive medications is superior to promoting the health promotion program in reducing IRCVD in virtual middle-aged Japanese men.

Abstract

Japanese patients with very high‐risk cutaneous squamous cell carcinomas (cSCCs), based on the National Comprehensive Cancer Network guidelines, have been reported to display a higher cumulative incidence of relapse and disease‐specific death (DSD) than those with high‐risk cSCC. Therefore, prognosis prediction is crucial for Japanese patients with very high‐risk cSCCs. Herein, we aimed to evaluate the prognostic prediction ability of our novel Japanese Risk Factor Scoring Systems (JARF scoring) in a Japanese cohort of cSSC patients. Data of 424 Japanese patients with resectable very high‐risk cSCCs were analysed. We compared the prognostic ability of the following three staging systems: Brigham and Women's Hospital (BWH) tumour staging, number of NCCN very high‐risk factors, and JARF scoring, including recurrent tumour, high‐risk histological features, deep tumour invasion and lymphatic or vascular involvement as risk factors. The prognostic ability of these staging systems was evaluated according to the cumulative incidence of local recurrence (LR), regional lymph node metastasis (RLNM), DSD, and overall survival (OS). When BWH staging was used, high T stage led to significantly poor outcomes only in the cumulative incidence of RLNM (p = 0.01). The presence of very high‐risk NCCN factors led to significantly poor outcomes in terms of RLNM (p = 0.03) and OS (p = 0.02). Meanwhile, a high number of risk factors in the JARF scoring system clearly led to poor outcomes in terms of LR (p = 0.01), RLNM (p < 0.01), DSD (p = 0.03), and OS (p < 0.01). Abstract

Objectives: To evaluate whether laser‐cut covered self‐expandable metallic stents (LC‐CSEMSs) can be the first choice for patients with unresectable distal malignant biliary obstruction (DMBO). Methods: Patients with unresectable DMBO who received LC‐CSEMS (September 2014‐December 2020) or braided CSEMS (B‐CSEMS) (July 2013‐December 2020) placement for biliary drainage were enrolled. Outcomes, including endoscopic CSEMS removal, of LC‐CSEMSs and B‐CSEMSs were compared. Results: Overall, 124 patients received LC‐CSEMSs placement, and 121 B‐CSEMSs. Technical success, clinical success, and procedure‐related adverse event rates with LC‐CSEMSs were 100%, 96.8%, and 6.5%, respectively, and 100%, 95.9%, and 5.8%, respectively, for B‐CSEMSs, with no significant difference (P >.99,.75, and >.99, respectively). The recurrent biliary obstruction (RBO) rates with LC‐CSEMSs and B‐CSEMSs were 9.7% (12/124) and 13.2% (16/121), respectively (P =.43). The median time to RBO with LC‐CSEMSs and B‐CSEMSs was 198 (interquartile range [IQR], 124‐244) days and 191 (IQR, 106‐271) days (P =.41). The endoscopic removal success rates of LC‐CSEMSs and B‐CSEMSs were 88.9% (8/9) and 90% (9/10) (P >.99), although there were a small number of cases. Conclusions: The non‐inferiority of LC‐CSEMSs was proven. LC‐CSEMSs can be considered as the first choice for patients with unresectable DMBO. [ABSTRACT FROM AUTHOR]

Abstract

Background and Aims: Endoscopic retrograde cholangiopancreatography (ERCP) in patients with surgically altered anatomy is still challenging. Usefulness of single‐balloon enteroscopy (SBE)‐assisted ERCP has been increasingly reported. Short SBE is considered beneficial with a 152‐cm working length and a 3.2‐mm working channel. This has increased the variety of devices that can be used during ERCP procedures. The aim of this pooled analysis was to evaluate the efficacy of SBE‐assisted ERCP in patients with surgically altered anatomy and elucidate the current status. Methods: This systematic review only involved biliary interventions which excluded pancreatic cases. Studies involving SBE‐assisted ERCP in patients with Roux‐en‐Y gastrectomy, hepaticojejunostomy with Roux‐en‐Y, pancreaticoduodenectomy (Whipple or Child procedure), or Billroth II gastrectomy were analyzed. Enteroscopy, biliary cannulation, and procedural success were assessed by pooling data in a random‐effect model, according to the degree of heterogeneity, to obtain a proportion with 95% confidence interval (CI). The outcomes observed for conventional and short SBE cases were also reported. Results: Overall, 1227 SBE‐assisted ERCP procedures from 21 studies were included. The pooled enteroscopy, biliary cannulation, and procedural success rates were 86.6% (95% CI, 82.4–90.3%), 90% (95% CI, 87.1–92.5%), and 75.8% (95% CI, 71.0–80.3%), respectively. Adverse events occurred in 6.6% (95% CI, 5.3–8.2%) of the procedures. Although good outcomes were reported for short SBE‐assisted ERCP, these should not be directly compared to the outcomes observed for conventional SBE, as they assume different backgrounds and include confounding variables. Conclusions: Single‐balloon enteroscopy‐assisted ERCP in patients with surgically altered anatomy on biliary interventions is effective. [ABSTRACT FROM AUTHOR]
