2. Evaluating and Comparing the Tensile Strength and Clinical Behavior of Monofilament Polyamide and Multifilament Silk Sutures: A Systematic Review.

Objective: This systematic review was carried out with the primary objective of verifying which suture (polyamide or silk) of two non-resorbable suture materials with different structures had better/greater tensile strength/resistance to tension, thereby presenting better mechanical behavior. The secondary objective was to verify which one had better performance. The null hypothesis was that both types of sutures had the same behavior. Methods: This systematic study followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) standards. The focused clinical question was: "In patients that underwent oral surgery treatment (P), is there significantly higher tensile strength/resistance for silk sutures (I) or for polyamide sutures (C) when comparing the outcomes (O)?". The bibliographic search was conducted on ScienceDirect, B-On, and PubMed/MedLine between March and May 2023. The following MeSH terms were defined: sutures, breaking strength, tensile strength, oral surgery, and dentistry, which were articulated and combined using Boolean operators. There were restrictions, such as articles published in Portuguese, Spanish, or English between 1 January 2018 and 3 April 2023. The quality assessment involved the use of the Joanna Briggs Institute (JBI) checklist for RCTs and the QUIN tool (Quality Assessment Tool For In Vitro Studies) for in vitro assays. Results: Ten articles were included in this review (eight in vitro studies and two RCTs). For the RCTs, there were moderate and high levels of bias, whereas in the in vitro studies, three were classified as having a high risk of bias and five as moderate risk. The results proved that suture thread with a monofilament polyamide physical structure causes a less inflammatory reaction owing to less bacterial retention and capillarity, while multifilament sutures, such as silk, have superior mechanical characteristics. Regarding hydration, the evidence demonstrated that the preservation and stability of mechanical properties lacked uniformity. Otherwise, hyaluronic acid (HA) presents a promising solution with the same characteristics and antibacterial capabilities. Conclusion: It was possible to reject the null hypothesis that both types of sutures had the same behavior and result. It was proven by the results above that sutures with a monofilament polyamide physical structure cause a less inflammatory reaction owing to less bacterial retention and capillarity. In contrast, multifilament sutures (silk) have superior mechanical characteristics. Regarding hydration using chlorohexidine in surgical sites, the evidence demonstrated in the preservation and stability of mechanical properties lacks uniformity and congruence. Objectivo : A Peri-implantitis é uma doença inflamatória que afeta os tecidos moles e duros que cercam um implante em função. A eliminação do biofilme estabelecido e a descontaminação da superfície do implante é o objetivo principal no tratamento da peri-implantite.Métodos: Algunas técnicas cirúrgicas podem ser necessárias para nos proporcionar um acesso adequado para desgranular eficazmente os tecidos inflamados, bem como para descontaminar e, se indicado, modificar a superfície do implante. Antes da utilização de técnicas cirúrgicas, a terapia não cirúrgica e o reforço da higiene bucal devem ser realizados. Resultados: Como em qualquer doença, a prevenção é a melhor forma de tratamento e a peri-implantita não é exceção. O diagnóstico precoce da doença peri-implantar pode ajudar a controlar a progressão e permite a sua resolução por meios não cirúrgicos no caso de mucosite peri-implante. Conclusão: As doenças peri-implantes são doenças importantes como resultado de sua alta prevalência e da falta de um modo padrão de terapia. Um estudo completo dos fatores de risco locais e sistêmicos do paciente é importante antes de qualquer tratamento com implante. Aim: Peri-implantitis is an inflammatory disease affecting soft and hard tissues surrounding a functional implant. The elimination of the established biofilm and decontamination of the implant surface are the main objectives in the treatment of peri-implantitis. Methods : Surgical techniques may be necessary to provide adequate access to degranulate the inflamed tissues effectively as well as to decontaminate and, if indicated, modify the implant surface. Before surgical techniques are employed, nonsurgical therapy and reinforcement of the oral hygiene should be performed. Results: As with every disease, prevention is the best treatment. Early peri-implant disease diagnosis can help in controling the progression and its resolution by non-surgical means in the case of peri-implant mucositis. Conclusion: Peri-implant diseases are important disease entities because of their high prevalence and the lack of a standard mode of therapy. Thorough study of the patient’s local and systemic risk factors is important before any implant treatment.

A Endodontia é um ramo da Medicina Dentária que tem como principal objectivo abordar patologias relacionadas na sua génese com o interior da peça dentária, vulgo sistema tridimensional de canais e camara pulpar, provocando estes, afecção pulpar ou periradicular. O tratamento em dentes permanentes imaturos, onde se encontre a polpa necrosada e/ou infectada, com inevitável interrupção do desenvolvimento radicular, torna-se um desafio para o profissional. As técnicas convencionais são eficazes na resolução quando existe perda de função da polpa dentária. Recentemente surgiram técnicas, na Medicina Regenerativa que levam à revascularização do dente, tornando-se assim uma forma promissora no tratamento de dentes imaturos com necrose pulpar ou sintomatologia irreversível. A técnica que se abordará utiliza o coágulo sanguíneo e envolve uma desinfecção canalar eficiente, seguida da promoção de um coágulo sanguíneo e do selamento coronal do canal radicular, que tem como objectivo restabelecer a funcionalidade pulpar em dentes Endodonciados. Diversos estudos sustentam que através desta técnica, o canal irá conter os elementos necessários para ocorrer a sua regeneração. No entanto, devido à imprevisibilidade do conteúdo do coágulo, são ainda necessários mais estudos de maneira a definir um protocolo adequado, bem como, para avaliação da real taxa de sucesso associada à técnica em questão. Endodontics is a branch of Dentistry that has as main goal to address related pathologies in its genesis with the interior of the tooth, the common three-dimensional system of canals and pulp chamber, provoking these, pulpal or periradicular affection. The treatment in immature permanent teeth, where the necrotic and / or infected pulp is found, with an inevitable interruption of root development, becomes a challenge for the professional. Conventional techniques are effective at resolution when there is loss of the function of the dental pulp. Recently there are techniques in Regenerative Medicine that lead to revascularization of the tooth, thus becoming a promising way to treat immature teeth with pulp necrosis or irreversible symptomatology. The technique that will be approach in this project use the blood clot and involves efficient canal disinfection followed by the promotion of a blood clot and coronal sealing of the root canal, which restore the pulp functionality in a Endodontic teeth. Several studies supports that with this technique, the channel will be contain the necessary elements to induce its regeneration. However, due to the unpredictability of the clot content, further studies are needed to define an adequate protocol, as well as to evaluate the actual success rate associated with this technique.

Há alguns anos atrás, certos actos clínicos que tinham como finalidade restabelecer a estética dentaria sendo um exemplo os implantes, eram quase impossíveis sem a altura a e espessura óssea suficiente. Hoje em dia, através de um processo de regeneração óssea podemos restabelecer condições suficientes para recuperar ou melhorar a configuração do osso e poder assim realizar a reabilitação com implantes. Existem vários tipos de regeneração tanto horizontal como vertical, neste trabalho vamos estudar e focalizar sobre a regeneração vertical visto esta ser mais difícil de alcançar e exigir uma atualização maior da parte do Médico Dentista na busca pela estabilidade do seu trabalho. Por ultimo, destacaremos as indicações, contra-indicações , vantagens e desvantagens de cada uma delas. A few years ago, certain clinical acts aimed to restoring dental aesthetics, such as implants, were almost impossible without adequate bone height and thickness. Nowadays, through a process of bone regeneration we can restore sufficient conditions to recover or improve the configuration of the bone and thus be able to carry out the rehabilitation with implants. (Antoun, H. 2013, Karouni, M. 2013, Sojod B. 2013). There are several types of regeneration both horizontal and vertical, in this work we will study / we will focus on vertical regeneration. Objectives: First, let's take a brief look at the physiological basis of bone, more precisely the mechanisms of bone remodeling, then we will talk about vertical bone regeneration and explore the different techniques available for this purpose. Finally, we will highlight the indications, contraindications, advantages and disadvantages of each of them.

Exodontias de terceiros molares são procedimentos comuns realizados por médicos dentistas, e apesar de rotineiro podem causar transtornos ou complicações trans e/ou pós operatórias. Vários fatores estão ligados às possíveis complicações, como idade e o grau de impactação do dente, em casos de terceiros molares inferiores, a sua íntima relação com o nervo alveolar inferior. Existem exames imaginológicos como radiografias e tomografias computadorizadas que podem ser utilizadas no pré-operatório a fim de auxiliar o medico dentista a estabelecer a relação entre essas estruturas, evitando futuros transtornos como a parestesia. Algumas técnicas cirúrgicas vêm sendo discutidas como métodos alternativos em casos onde há a hipótese de lesão do nervo. Este trabalho realiza uma revisão de literatura sobre o tema terceiros molares inferiores e suas relações com o nervo alveolar inferior, a fim de relacionar os riscos cirúrgicos com posicionamento do terceiro molar associando exames complementares com técnicas cirúrgicas, com o fim de prever possíveis complicações. Third molars extraction are common procedures performed by dentists, and despite frequency , it may cause trans and / or postoperative complications. Several factors are related to the possible complications, such as age and degree of impactation of the tooth, in cases of lower third molars, its intima relationship with the inferior alveolar nerve. There are imaging examinations such as x-rays and CT scans that can be used preoperatively to assist the dentist to establish the relationship between these structures, avoiding future disorders such as paresthesia. Some surgical techniques have been discussed as alternative methods in cases where there is a hypothesis of nerve damage. This paper presents a review of the literature on the third lower molar theme and its relationship with the inferior alveolar nerve, in order to relate the surgical risks with third molar positioning, associating complementary exams with surgical techniques, in order to predict possible complications.

Hoje em dia, os implantes dentários são uma parte integrante do nosso arsenal terapeutico. A implantologia, como qualquer outra disciplina cirúrgica requer delicadeza, precisão e planeamento. Embora o médico dentista possa ser experiênciado, ele será confrontado inevitavelmente com falhas. Ele deve estar lá preparado para tal e precaver o paciente dele para isto. Estas falhas implantares representam cerca de 5% dos casos mas são um desafio real para os médicos. O objetivo é de demonstrar, de uma forma muito sucinta os vários tipos para as complicações existentes em implantologia, e mais precisamente as complicações pós-operatórias e como geri-los e preveni-las. Nowadays, dental implants are an integral part of our therapeutic arsenal. Implantology, like any other surgical discipline, requires delicacy, precision and planning. Although the dentist may be experienced, he will inevitably be confronted with failures. He must be prepared for it and guard his patient for it. These implant failures represent about 5% of cases but are a real challenge for doctors. The objective is to demonstrate, in a very succinct way, the various types for the complications existing in implantology, and more precisely the postoperative complications and how to manage and prevent them.

0 conceito de plaform switching, em português, plataforma alterada, referenciado na literatura, no âmbito da implantologia, que parece permitir uma melhoria na preservação do osso peri-implantar. Dos estudos sobre esta temática surgem varias hipóteses e explicações para justificar as melhorias clinicas obtidas com a sua utilização. Este trabalho procura rever e detalhar, tendo por base uma revisão da literatura cientifica, os fundamentos biológicos que suportam as vantagens clinicas obtidas com a utilização da técnica de plataforma alterada. Uma pesquisa na base de dados "PubMed", foi efetuada considerando os artigos dos últimos 10 anos. A bibliografia obtida inicialmente foi selecionada pela leitura dos resumos e posteriormente pela leitura integral das publicações. O mecanismo pelo qual a plataforma alterada apresenta melhorias clinicas quando comparado com as técnicas convencionais poderá estar relacionado com distintas ocorrências, nomeadamente devido a uma modelação biomecânica das componentes implantares, alteração da localização do microgap, modulação do infiltrado inflamatório peri-implantar e formação de um espaço biológico horizontal. The concept of plaform switching, in Portuguese, changed platform, referenced in the literature, in the scope of implantology, which seems to allow an improvement in the preservation of the peri-implant bone. From the studies on this theme several hypotheses and explanations appear to justify the clinical improvements obtained with its use. This work seeks to review and detail, based on a review of the scientific literature, the biological foundations that support the clinical advantages obtained with the use of the altered platform technique. A search in the "PubMed" database was carried out considering articles from the last 10 years. The bibliography obtained initially was selected by the reading of the abstracts and later by the full reading of the publications. The mechanism by which the altered platform presents clinical improvements when compared to conventional techniques may be related to different occurrences, namely due to a biomechanical modeling of the implant components, alteration of microgap location, modulation of the peri-implant inflammatory infiltrate and formation of a Horizontal biological space.

A fratura de uma agulha, durante a administração anestésica, é um acontecimento raro atualmente devido ao uso de agulhas descartáveis de liga metálica, que minimiza o risco de fratura. É da responsabilidade do profissional usar as agulhas e técnicas mais adequadas ao procedimento a ser executado, com os devidos cuidados de modo a potenciar bons resultados. Objetivo: Descrever de forma sucinta, as razões pelas quais as fraturas ocorrem, bem como, quais os procedimentos, mais favoráveis ao dentista e ao utente, para a resolução dessas fraturas. Apresentar algumas técnicas anestésicas e exemplificar algumas técnicas de localização de fragmentos. Materiais e Métodos: Revisão bibliográfica tendo por base as palavras-chave delineadas e critérios de inclusão e exclusão específicos, com o objetivo de angariar o máximo de informação acerca deste tema não muito comum e cheio de possíveis complicações na Medicina Dentária. Conclusão: Numa situação extrema, quando o fragmento de uma agulha fica envolvido pelos tecidos moles, o médico dentista dispõe ao seu critério de diversas técnicas para a remoção/correção do problema. O intuito é minimizar o risco de lesão no paciente, recorrendo, sempre que possível, às mais avançadas técnicas anestésicas. Introduction: Needle fractures during local anesthesia is a rare event in modern days due to current use of disposable needles made of modern alloys that significantly reduced the incidence of needle breakage. It is the professional’s responsibility to use the most appropriate needles and techniques related to the treatment, with care, in order to minimize all risks. Objective: To show, briefly, why such breakages happen and how to proceed in order to resolve problems resulting from such events, helping patient and dentist overcome the distressful situation. To remember and talk about some local anesthesia techniques and methods to retrieve needle fragments. Conclusion: On worst case scenarios, when we have a needle fragment surround by soft tissue, there are multiple techniques to search and retrieve for the missing piece, at the discretion of the dentist to use the best searching method to solve the problem. The patient’s risk of injury and after-effects is high and all caution should be used, executing the best techniques studied and approved by many years of correct usage, avoiding needle breakage.

A construção de um sorriso saudável e estético na atualidade, tem o seu sucesso associado à sinergia de todas as especialidades na área da Medicina Dentária. Este trabalho tem como objetivo determinar quais os aspetos que contribuem para a harmonia do sorriso com base na análise dos elementos faciais. Os components faciais e musculares são particulares de cada individuo e intervém diretamente no tipo de sorriso. Fatores como a raça, a idade e o género são variáveis a ter em consideração na construção de um sorriso harmónico. O presente estudo bibliográfico, faz uma abordagem sobre a estética e harmónica do sorriso analisando os aspetos tegumentares faciais, gengivais e as proporções dentárias adequadas à contrução de um sorriso belo e saudável. A aparência do sorriso tem um grande impato na vida dos indivíduos, podendo afetar desde o convívio social até mesmo as relações profissionais. Este é um dos principais motivos pelo qual pacientes recorrem a ajuda de especialistas da área. O estudo efetuado revelou que a análise da face é fundamental no estudo do sorriso. Ela identifica as caraterísticas menos favoráveis e mais favoráveis de cada pessoa com o intúito de obter um equilíbrio entre a face e o sorriso. Making a healthy and aesthetic smile has its success linked with the sinergy of every dental specialty in Dental Medicine The objective of this work is to determin wich aspects contribute to the harmony of te smile based on the analysis of the facial elements. The facial and muscular components are specific of every individual and have a direct influence on na indivuals smile, race, age and gender are importante variables to consider when building a balanced smile. This review has na approach on esthetics and harmony of the smile analising facial soft tissue morfology, gengival, and dental proportions adequate for a healthy and beautiful smile. The effect a beatifull smile has on na individuals life can afect the social enviroment and even the work place. This i sone of the main resons why pacientes go to the dental office. In conclusion the present study revealed the facial analysis is fundamental in the study of the smile. It identifies the less favorable caracteristics in each person with the goal to obtain a balanced face and smile.

Por isso o diagnóstico precoce combinado com o tratamento adequado é a chave fundamental para o controlo da doença. Face a esta realidade os profissionais de saúde têm um papel fulcral na deteção precoce de lesões potencialmente malignas ou malignas, sendo por isso um objetivo prioritário na saúde. Para avaliação deste tipo de lesões o diagnóstico definitivo, histopatologicamente fornecido pela biópsia convencional, continua a ser o método imprescindível e de eleição para os profissionais de saúde, no entanto novas técnicas não-invasivas têm surgido como auxiliares de diagnóstico. Objetivos: Esta monografia teve como objetivo obter um conhecimento detalhado acerca do cancro oral, nomeadamente as suas principais particularidades e especificidades de forma a obter uma deteção precoce e consequentemente um prognóstico favorável. Conjuntamente, foi realizado um estudo que recolheu informações sobre o conhecimento geral dos médicos dentistas e estomatologistas portugueses relativamente ao cancro oral, a utilização de modalidades auxiliares de diagnóstico hoje em dia disponíveis, assim como a colheita de dados face ao protocolo de atuação dos clínicos aquando suspeita de lesões potencialmente malignas ou malignas. Materiais e Métodos: Este trabalho teve como base as palavras-chave delineadas e critérios de inclusão e exclusão específicos, com o objetivo de angariar e debater o máximo de informação acerca desta grande associação entre oncologia e medicina dentária, o cancro oral. De forma a dar resposta aos objetivos enunciados associou-se a esta monografia um estudo quantitativo do tipo descritivo e transversal. A recolha de dados foi realizada através de um inquérito em que participaram 324 profissionais de saúde oral. O inquérito, com a totalidade de vinte e uma perguntas, teve como principal objetivo a recolha de informação sobre o conhecimento geral do médico dentista ou estomatologista relativamente ao cancro oral. Após colheita de dados, estes foram tratados e analisados recorrendo a programas informáticos como o Microsoft Office Excel (2010) e o programa Statistical Package for the Social Sciences (IBM© SPSS© Statistics) vs. 22.0 para Windows. Resultados: A amostra selecionada demonstrou ter os conhecimentos necessários e fundamentais quando abordados sobre este tema, estando conscientemente alerta caso suspeitem de uma lesão potencialmente maligna ou maligna. Cerca de 78% dos inquiridos preferem encaminhar o paciente quando perante uma lesão suspeita. Relativamente aos indivíduos que realizam biópsia, 66.7% prefere a técnica excisional. 96% discute com colegas quando suspeita de uma lesão maligna. A maioria dos profissionais (88.9%) refere que nunca utilizou outros métodos auxiliares para além da biópsia. Dos 11.1% que admitem ter utilizado outro método auxiliar de diagnóstico, 58.8% indica o azul de toluidina como método preferencial. 27.2% dos profissionais não considera a biópsia como elemento fundamental. Discussão e Conclusão: Através da pesquisa realizada verificou-se que é fundamental um conhecimento dos sinais e sintomas e a realização de rastreio em consultas de rotina para detetar precocemente lesões potencialmente malignas ou malignas. Por consenso geral dos inquiridos, constatou-se que a história clínica, o exame clínico e a biópsia são os métodos mais eficazes e comummente utilizados a fim de reduzir a morbilidade e mortalidade associada a esta doença. Ainda assim, 27.2% dos profissionais não considera a biópsia como elemento fundamental o que revela ser preocupante. Contudo corroborou-se que a maioria dos inquiridos, mesmo aptos teoricamente, prefere encaminhar ao invés de realizar biópsia. Introduction: Currently oral cancer is the sixth most common cancer worldwide. Its high rate of morbidity and consequent mortality, is related with the detection of this disease already in advanced stages of malignancy. In this context, it ends up leading to increased complications in treatment that becomes more aggressive according to the degree of malignancy. Therefore early diagnosis along with suitable treatment is the fundamental key to control the disease. Due to this reality, health professionals have a key role in early detection of potential malignant or malignant lesions making it a priority goal in health. To evaluate this kind of lesions, the definitive histopathologic diagnosis provided by conventional biopsy, still remains the crucial method of choice for health professionals, although new non-invasive techniques have emerged as auxiliary diagnosis methods. Objective: This paper aims to obtain a detailed knowledge of oral cancer, including its main particularities and specificities in order to obtain an early detection and consequently a favorable prognosis. It has also have been conducted a study that collected information on the general knowledge of portuguese dentists and stomatologists regarding oral cancer, the use of auxiliary diagnostic tools nowadays available, as well as data collection concerning the clinical performance protocol when suspected potentially malignant or malignant lesions. Materials and Methods: This study was based on the keywords and outlined specific inclusion and exclusion criteria, in order to raise and discuss as much information about this great association between oncology and dentistry, which is oral cancer. In order to meet the stated goals, it was associated with this monography a quantitative study of a descriptive and cross-sectional type. Data collection was conducted through a survey involving 324 dental professionals. The survey, with a total of twenty-one questions, aimed to collect information on the general knowledge of dentists regarding oral cancer. After data collection, these were processed and analyzed using computer programs such as Microsoft Office Excel (2010) and the Statistical Package for Social Sciences (SPSS IBM © © Statistics) vs. 22.0 for Windows. Results: The selected sample has demonstrated the necessary and basic knowledge when approached about the subject, consciously being alert if suspected a potentially malignant or malignant lesion. About 78% of respondents prefer to refer the patient when faced with a suspected lesion. Regarding the individuals that perform biopsy, 66.7% prefer the excisional technique. 96% approaches other colleagues when suspects of a malignant lesion. Most professionals (88.9%) reported ever used other auxiliary methods in addition to biopsy. Of the 11.1% who admit to having used an auxiliary diagnostic tool, 58.8% indicates the toluidine blue as the preferred method. 27.2% of professionals do not consider the biopsy as a fundamental element. Discussion and Conclusion: Through the research made, it was found that it is essential to have knowledge of signs and symptoms and provide screening in routine consults aiming for an early detection of potentially malignant or malignant lesions. By general consensus of those surveyed, it was found that the clinical history, clinical examination and biopsy are the most effective methods and commonly used to reduce morbidity and mortality associated with this disease. Still, 27.2% of the respondents do not consider the biopsy as a fundamental element, which is concerning. However it was corroborated that the majority of respondents although theoretically able, prefer to refer instead of performing biopsy.
