The effects of staff shortage on the economy include wage growth, improved productivity, inflation control, and development of new markets. The positive and negative aspects of this phenomenon are analyzed, taking into account its impact on macroeconomic stability and prospects for economic growth. Aim. To study the causes and consequences of staff shortage for indicators such as inflation and GDP. Based on the data obtained, predict the value of some macroeconomic indicators by 2024. Methods. Comparative and retrospective analysis of statistical data with the identification of certain patterns, compilation of a modified Philips equation for forecasting macroeconomic indicators. Results. The authors have collected and evaluated statistical data on the unemployment rate, found out the causes and average rate of change in inflation before and after quarantine measures.

Introduction. IBS is a functional bowel disorder that has a significant impact on patients and society, especially in terms of quality of life and medical costs.Pathogenesis. It is believed that the pathogenesis of IBS consists of several mechanisms: the syndrome of intersection of functional disorders (gut-brain), stress, visceral hypersensitivity and changes in motor skills.Visceral hypersensitivity. Changes in visceral sensitivity in IBS are characterized by central abnormalities in areas of the cerebral cortex. Motility impairment in IBS manifests itself as abnormal myoelectric activity in the colon, resulting in repetitive contractions of the small intestine and colon, which appear to cause pain.Intestinal microflora. FODMAPs are found in high amounts in some fruits, artificial sweeteners, legumes, and green vegetables and are poorly absorbed by all people. FODMAPs have enzymatic and osmotic effects that may contribute to the onset of symptoms in some patients.The principles of IBS therapy. Treatment for IBS should be based on the type and severity of symptoms. For the treatment of IBS, drugs of various pharmacological groups are used, depending on the prevailing symptoms. These include opioid receptor agonists, bile acid sequestrants, guanylate cyclase agonists, chlorine channel activators, as well as antibiotics, probiotics, antidepressants, 5-HT3 receptor antagonists, and antispasmodics.Myotropic antispasmodics. Drugs with antispasmodic activity are used to treat functional and organic diseases of the gastrointestinal tract as a basic therapy or «on demand». Mebeverine quickly and effectively relieves spasm, pain and the entire complex of intestinal symptoms, in addition, the drug reduces visceral hypersensitivity due to a local anesthetic effect. The drug has a high safety profile and has a number of advantages over drugs of the same pharmacological group.Conclusion. Myotropic antispasmodics have been shown to be highly effective in the treatment of IBS. Mebeverine occupies a special place among myotropic antispasmodics. Its combined action provides a pronounced antispasmodic activity along with a high safety profile.

Atopic dermatitis is one of the most common inflammatory diseases. This disease is characterized by severe itching, recurrent eczematous lesions and has a heterogeneous clinical picture. AD occurs in all countries, in both sexes, in different age groups. There has been a steady increase in the incidence of AD detection over the past three decades. The prevalence of AD symptoms in various regions of the Russian Federation (RF) ranged from 6.2 to 15.5%. There is no generally accepted classification of AD, the working classification of AD reflects the age dynamics, clinical and morphological forms, severity and stages of the course of the disease. Treatment of AD should be patient-centered and should include prevention of individual trigger factors, restoration of the skin barrier, and a stepwise and gradual approach to reduce inflammation based on the severity of the disease. mGCS are the first-line treatment for exacerbations of AD. Topical calcineurin inhibitors are tacrolimus and pimecrolimus. Pimecrolimus is indicated for the treatment of mild to moderate AD, and tacrolimus is indicated for moderate to severe AD. Tacrolimus and pimecrolimus are characterized by low systemic absorption, they do not cause skin atrophy and do not affect the function of the hypothalamic-pituitary-adrenal system. Systemic corticosteroids should be used only in exceptional cases for short-term treatment of an exacerbation or when starting another systemic therapy. The therapeutic value of 1st generation antihistamines lies in their sedative properties by normalizing nighttime sleep and reducing the intensity of itching. Inhaled antibacterial drugs have been used since the 1940s with greater or lesser effectiveness, due to the possibility of targeted delivery of drugs directly to the infection site at concentrations higher than MICs. High local concentrations of antibacterial agents expand the possibilities of treating infections caused by multiresistant strains and reduces antibiotic resistance in the population. The inhaled delivery method is characterized by a high level of safety due to the absence of systemic toxic effects, which reduces the risk of pseudomembranous colitis and other complications of antibiotic therapy. Thiamphenicol glycinate acetylcysteinate is a combined drug that includes thiamphenicol and N-acetylcysteine, which causes its antibacterial and mucolytic activity. The results of clinical studies have demonstrated the effectiveness of TGA in the treatment of acute and chronic bronchitis, chronic obstructive pulmonary disease, community-acquired pneumonia, laryngotracheitis, rhinosinusitis and other infections of the upper and lower respiratory tract. In pediatrics, the drug is used to treat bronchitis and pneumonia, especially those that cannot be treated with other antibacterial drugs, bronchiolitis, whooping cough, and cystic fibrosis. The article presents data on the effectiveness and safety of containing TGA. A wide spectrum of antibacterial actions, the ability to form high local concentrations of antibacterial and mucolytic components in the focus of infection, a low risk of serious side effects in both adults and children are noted. The topical use of the combination drug has proven to be effective not only in bringing about a pronounced clinical improvement, but also in reducing the frequency of use of systemic antibiotic therapy. The efficacy of thiamphenicol glycinate acetylcysteinate is not inferior to macrolide antibiotics.

Intestinal microflora in terms of the integral body of knowledge about its importance for the human body is now considered as an additional “virtual” organ, producing many biologically active compounds that affect the various properties and functions of the macroorganism: fermentation of non-digestible food polysaccharides, maintenance of the optimal values of the intra-lumen pH and “intestinal barrier”, the activity of the immune system, vitamin status, energy homeostasis, angiogenesis. Saccharomyces boulardii is not a representative of the natural microflora of the human intestine, but, used as a probiotic, interacts with it, restoring the balance of short-chain fatty acids, reducing the levels of bacterial toxins, acting as a surrogate normal microflora until it is restored and replaced by natural representatives. S. boulardii has immunostimulating and anti-inflammatory activity. The results of clinical studies have demonstrated the effectiveness of S. boulardii in correcting pathological conditions associated with Helicobacter pylori, Clostridium difficile, Candida albicans, Salmonella typhimurum, Yersinia enterocolitium, protozoa Entamoeba and in treating irritable bowel syndrome and antibiotic-associated diarrhea. The efficacy of a drug containing S. boulardii has been demonstrated in the article by clinical studies and a clinical example. This drug has proven to be a highly effective and safe probiotic agent for the prevention and treatment of a number of diseases of the gastrointestinal tract with physiologically optimized pharmacodynamic effect that simulates the protective properties of normal human intestinal microflora. When using a drug containing S. boulardii, both adults and children do not have serious side effects, which increases the possibility of its safe use in clinical practice.

The practice-oriented approach of the workshop and the value of the campus have made the Winter University a platform for interesting discussions and innovative proposals.

Aim: to analyze the pool of potential donors in the Moscow region from among the corpses received for forensic medical examination, the effectiveness of their use, the assessment of the participation of the forensic service in transplantation coordination.Materials and methods. The potential donors from among the dead persons in 2017 from various injuries were analyzed, the conclusions of forensic experts in cases of the removal of internal organs from the corpse were studied.Results. In 2017, 417 forensic examinations of corpses with open and closed isolated traumatic brain injuries received as a result of various injuries were conducted. 328 corpses of males and 89 females, dominated by the age group of 61 and above – 170 (40.7%). The period of hospitalization 2–7 days – 173 (41.8%). In addition, 76 cases of no traumatic hemorrhages in the brain and under the matter of the brain caused by diseases were studied, the age was more than 60 years – 41 (53.9%) prevailed. In 29 cases, internal organs were taken for transplantation. Such corpses were investigated in 15 forensic departments out of 47 (31.9%), which are part of 8 departments of the Ministry of health of the Moscow region out of 15. Among donors, the prevalence of men 27 (93%) over women 2 (7%) in the 31–60 age group is 19 (65.5%). 17 were diagnosed with brain death (58.6%) and 12 asystolic donors (41.4%). Multiorgan removal was carried out in 13 cases (44.8%), in 15 cases kidneys were removal (51.7%), 1 case of refusal of the removal after visual inspection of the extracted organ.Conclusion. The Analysis allows to estimation a possible pool of potential donors in the Moscow region, to correlate with the volume of removal organs, which indicates insufficient efficiency of their use. Insufficiently active participation of medical institutions of Administrations of transplantological coordination in the territory of the Moscow region correlates with participation of medico legal experts in this coordination. In the conclusions of the experts found no evidence of traumatic damage to the organ and the obstruction of the expert when giving answers to the questions of the investigatory organizations.

The nautical system’s conditions in high latitudes (in the area of Talakan and Igarka airfields) assisted by GLONASS, GPS and combined system GLONASS/GPS have been examined through semirealistic simulation. Nautical session’s conditions are characterized by horizontal (HDOP) and vertical (VDOP) geometrical factors’ values in specified areas of terrain. Researches using the firmware complex within the satellite navigation systems signal simulator CH-3803M and the aeronautical receiver CH-4312, interlinked by bus ARINC-429 with personal сomputer have been con-ducted. Particularities of using the simulator and receiver CH-4312 for carrying out semirealistic experiments have been shown. By comparing these results with the results of realistic experiments the validity of semirealistic modelling has been estimated. The accuracy of receiver’s CH-4312 position determination when operated by GLONASS, GPS and combined mode for Talakan and Igarka areas have been examined.

Based on the application of hardware-software complex NI GPS Simulation Toolkit and aviation receiver CH-4312 proposed a method of constructing the fields of GPS precision according to the RAIM receiver (horizontal and vertical geometrical factor system) data. By comparing with experimental data the reliability of the obtained results is assessed and limitations on the conditions of use of the complex for investigation of the characteristics of the GPS were found. Built fields precision GPS for two areas of airspace, located in mid-and high latitudes were constructed.
