Bu çalışmada, acil serviste akut pankreatit(AP) tanısı alan hastaların klinik, laboratuvar, görüntüleme ve demografik özelliklerini değerlendirmek ve bu özelliklerin AP tipine, AP'nin şiddetine ve sonlanımlarına göre farklılık gösterip göstermediğini araştırmak amaçlanmıştır. 01.10.2017-01.10.2020 tarihleri arasında hastanemiz acil servisine başvuran 18 yaş ve üzeri AP tanısı alan 221 hastanın dosyası retrospektif olarak tarandı. Hastalar demografik özelliklerine, komorbiditelerine, sigara ve alkol gibi alışkanlıklarına, başvuru semptomlarına, laboratuvar ve görüntüleme bulgularına, ödematöz veya nekrotizan şeklinde pankreatit tipine, Ranson skorlarına, servis/ yoğun bakım ihtiyaçlarına, ortalama yatış sürelerine ve taburcu/mortalite gibi sonlanım durumlarına göre değerlendirilmiştir. AP tanısı alan 221 hastanın 115'i (%52.04) kadın, 106'sı (%47.96) erkek ve genel yaş ortalamaları 60.5(±18.6) idi. Etiyolojide en sık biliyer nedenler (%59.27) izlendi. BT sonucu olan 207 hastanın %70.5'i ödematöz, %6.7'si nekrotizan pankreatit grubundaydı. Hastalığın şiddeti Ranson kriterlerine göre değerlendirildiğinde; %80'i hafif, %20'si ciddi pankreatit hastasıydı. Rekürren pankreatit yatışı olan hasta sayısı 49 (%22) olup, çoğunluk hafif pankreatit olarak saptandı. Hastaların ortalama yatış süreleri 12.4 (±9.8) gün olarak tespit edildi. Mortalite oranı %3.62 saptandı. İleri yaş, pankreatitin ciddiyeti ve nekroz bulgularının eşlik etmesinin mortaliteyi arttığı görüldü. Bu bulgular ile AP etyolojisinde en sık biliyer nedenlerin olduğunu, pankreatit şiddetinin ve mortalitenin yaş ile birlikte arttığı görüldü. Sonuç olarak AP ile alakalı daha net bilgilere sahip olmamız için, büyük hasta popülasyonlarını içeren geniş kapsamlı prospektif çalışmalara ihtiyaç vardır. Anahtar Kelimeler: Akut pankreatit, rekürren, mortalite, nekrotizan, In this study, it was aimed to evaluate the clinical, laboratory, imaging and demographic characteristics of patients diagnosed with acute pancreatitis (AP) in the emergency department and to investigate whether these features differ according to the type of AP, severity and outcome of AP. The files of 221 patients aged 18 years and older who applied to the emergency department of our hospital between 01.10.2017-01.10.2020 and diagnosed with AP were scanned retrospectively. Patients were evaluated according to their demographic characteristics, comorbidities, habits such as smoking and alcohol, admission symptoms, laboratory and imaging findings, type of edematous or necrotizing pancreatitis, Ranson scores, service/intensive care needs, average length of stay, and outcome such as discharge/mortality. Of the 221 patients diagnosed with AP, 115 (%52.04) were female, 106 (%47.96) were male, and the mean age was 60.5 (±18.6). Biliary causes (%59.27) were the most common etiology. According to the CT results of 207 patients, %70.5 were in the edematous and %6.7 necrotizing pancreatitis group. When the severity of the disease is evaluated according to the Ranson criteria; %80 of the patients had mild pancreatitis and %20 had severe pancreatitis. The number of patients hospitalized for recurrent pancreatitis was 49 (%22), and the majority were found to be mild pancreatitis. The mortality rate was %3.62. The reasons that increased mortality were determined as advanced age, severity of pancreatitis and accompanying necrosis findings. With these findings, it was observed that biliary causes were the most common in the etiology of AP, and the severity of pancreatitis and mortality increased with age. In conclusion, comprehensive prospective studies involving large patient populations are needed to have clearer information about AP. SDÜ Tıp Fakültesi Acil Servisi'ne Başvuran ve Akut Apandisit Tanısıyla Opere Edilen Hastaların Biyobelirteçleri ve Görüntüleme Yöntemlerinin Geriye Dönük Değerlendirilmesi Apandisit dünyada akut karının en yaygın nedenlerinden biri olarak kabul edilmiştir. Tanıda anamnez, fizik muayene, görüntüleme ve laboratuvar yöntemlerinden yararlanılmakla birlikte akut apandisit tanısında sorunlar devam etmektedir. Çalışmamızda akut apandisit tanısı koyarken laboratuvar bulguları ile görüntüleme yöntemlerinin duyarlılıkları karşılaştırıldı. Çalışmaya 01.06.2012 ile 01.06.2015 tarihleri arasında SDÜ Tıp Fakültesi Acil Servisine başvuran ve akut apandisit tanısı ile opere edilen 18 yaş üstü 292 hasta dahil edildi. Hastaların bilgilerine acil servis dosyalarından, ameliyat notlarından, epikrizlerden ve bilgi işlem sistemi veri tabanından ulaşıldı. Hastaların yaş, cinsiyet, WBC sayısı, nötrofil sayısı, NLO değeri, MPV sayısı, CRP düzeyleri, USG ve BT bulguları ve histopatolojik bulguları veri toplama aracı olarak bir takip formu oluşturulup bu formlara kaydedildi. Laboratuar ölçümlerinin ve görüntüleme yöntemlerinin patoloji pozitifliği ayırt ediciliğine yönelik ROC eğrileri oluşturularak, her bir ölçüme ait AUC ve kesim değeri belirlendi ve bu değerlerin patoloji ayırt edicilik performansları, sensitivite, spesifisite, +PV, -PV, +LR ve -LR kesim değerleri hesaplanarak birlikte değerlendirildi. Başvuran hastaların cinsiyet dağılımı 174 erkek, 118 kadın idi. Erkek/kadın oranı 1,6:1 olarak bulundu. Akut apandisit görülme sıklığı cinsiyete göre karşılaştırıldığında, istatistiksel olarak anlamlı fark tespit edildi. Hastaların ortalama yaşı 32,15±13,67 olarak bulundu. WBC değeri için AUC değeri 0,684 olarak hesaplandı. Akut apandisitin tanısında WBC'nin sensitivitesi %65,09, spesifitesi %62,1, pozitif tahmin değeri %92,2, negatif tahmin değeri %20,5, pozitif olabilirlik oranı (+LR) 1,71, negatif olabilirlik oranı (-LR) 0,56 ve tanısal doğruluğu %64,7 olarak saptandı. Nötrofil değeri için AUC değeri 0,698 olarak hesaplandı. Akut apandisitin tanısında nötrofilin sensitivitesi %72,15, spesifitesi %59,5, pozitif tahmin değeri %92,5, negatif tahmin değeri %23,6, pozitif olabilirlik oranı 1,78, negatif olabilirlik oranı 0,46 ve tanısal doğruluğu %70,54 olarak saptandı. Nötrofil lenfosit oranı için AUC değeri 0,647 olarak hesaplandı. Akut apandisit tanısında NLO'nun sensitivitesi %70,9, spesifitesi %51,3, pozitif tahmin değeri %90,9, negatif tahmin değeri %20,4, pozitif olabilirlik oranı 1,45, negatif olabilirlik oranı 0,56 ve tanısal doğruluğu %68,4 olarak saptandı. CRP değeri için AUC değeri 0,530 olarak hesaplandı. Akut apandisitin tanısında CRP'nin sensitivitesi %45,3, spesifitesi %48, pozitif tahmin değeri %83,9, negatif tahmin değeri %12,7, pozitif olabilirlik oranı 0,87, negatif olabilirlik oranı 1,13 ve tanısal doğruluğu %45,7 olarak bulundu. USG'nin akut apandisit için tanı değeri yüksek bulundu (AUC=0,629). Çalışmamızda ultrasonografinin sensitivitesi %82,9, spesifitesi %42,8, pozitif tahmin değeri %91,6, negatif tahmin değeri %25, pozitif olabilirlik oranı 1,44, negatif olabilirlik oranı 0,39 ve tanı doğruluğu %78,2 olarak tespit edildi. BT'nin akut apandisit için tanı değeri yüksek olduğu görüldü (AUC=0,865). Çalışmamızda abdominal BT'nin sensitivitesi %93,9, spesifitesi %78,9, pozitif tahmin değeri %96,4, negatif tahmin değeri %68,1, pozitif olabilirlik oranı 4,45, negatif olabilirlik oranı 0,07 ve tanı doğruluğu %91,8 olarak bulundu. Bu çalışmadan elde ettiğimiz verilere göre tanı koyarken laboratuar değerleri ve biyobelirteçler dikkate alınmalı, klinik şüphenin devamı halinde görüntüleme yöntemlerinden faydalanılmalıdır. Ancak akut apandisit tanısında hiçbir ek tanısal yöntemin tek başına tanı koymada yeterli olmadığını ve kullanılan bütün yöntemlerin klinisyene tanıda destek olması gerektiğini düşünmekteyiz. In the diagnosis of acute appendicitis, medical history, physical examination, imaging and laboratory methods are utilized, although problems remain. In our study, we compared the sensitivity of imaging methods with laboratory findings of acute appendicitis. The study including 292 patients over the age of 18 in that attended to the emergency department of SDU Faculty of Medicine and operated with the diagnosis of acute appendicitis between dates 01.06.2012 -01.06.2015. The informations of patients has been reached from emergency department files, surgery notes, epicrisis and information processing system database. Created a follow-up forms for data collection; the age, sex, WBC count, neutrophil count, NLO values, MPV count, CRP count, ultrasound and CT findings and histopathological findings of patients were noted to the this form. ROC curves were intended for positive distinguish of laboratory measurements and imaging methods. Determination of AUC and cutoff values for every measurement and distinguishing performances of these values were assessed with sensitivity, specifity, +PV, -PV, +LR and –LR. The gender distribution of patients were 174 male and 118 female. Male to female ratio was found 1,6: 1. Compared the incidence of acute appendicitis based on gender, statistically significant differences were detected. The average age of the patients was calculated as 32.15 ± 13.67. AUC was calculated to be 0.684 for the WBC count. WBC count had a sensivity of %65,09, a specifity of %62,1, a positive predictive value of %92,2, a negative predictive value of %20,5, a positive likelihood ratio 1,71, negative likelihood ratio 0,56 and a diagnostic accuracy of %64,7 for the diagnosis of AA. AUC was calculated to be 0.698 for the neutrophil count. Neutrophil count had a sensitivity of %72,15, a specificity of %59,5, a positive predictive value of %92,5, a negative predictive value of %23,6, a positive likelihood ratio 1,78, a negative likelihood ratio 0,46 and a diagnostic accuracy of %70,54 for the diagnosis of AA. AUC was calculated to be 0,530 for the CRP count. CRP had a sensitivity of %45,3, a specificity of %48, a positive predictive value of %83,9, a negative predictive value of %12,7, a positive likelihood ratio 0,87, a negative likelihood ratio 1,13 and a diagnostic accuracy of %45,7 for the diagnosis of AA. USG was significantly higher diagnostic value for appendicitis (AUC=0,629). Ultrasound had a sensitivity of %82,9, a specificity of %42,8, a positive predictive value of %91,6, a negative predictive value of %25, a positive likelihood ratio 1,44, a negative likelihood ratio 0,39 and a diagnostic accuracy of %78,2 for the diagnosis of AA. CT was significantly higher diagnostic value for appendicitis (AUC=0,865). CT had a sensitivity of %93,9, a specificity of %78,9, a positive predictive value of %96,4, a negative predictive value of %68,1, a positive likelihood ratio 4,45, a negative likelihood ratio 0,07 and a diagnostic accuracy of %91,8 for the diagnosis of AA. According to the data we obtained from this study when making a diagnosis laboratory values and biomarker should be considered and should benefit from imaging methods case of continuation of clinical suspicion. We believe that all the method should be used in tests required to support clinicians in diagnosis. However, no additional diagnostic methods in the diagnosis of acute appendicitis is not enough by itself. Akut miyokard infarktüsü (AMI), dünya genelinde yüksek ölüm oranlarına sahiptir. Ancak erken tanı ve tedavi ile prognozun yüksek oranda düzelmesi sağlanabilmektedir. Bu nedenle AMI ve akut koroner sendrom (AKS) acil serviste erken tanınması önemlidir. HEART skoru acil serviste göğüs ağrılarını değerlendirmek için geliştirilmiş, yüksek riskli hastaları olduğu kadar düşük riskli hastaları da ayırt edebilen, bir skorlama sistemidir. AKS gibi hayatı tehdit eden durumlarda periferik kan akımında azalmaya neden olur. Bu azalma dolaşım halindeki kan ile durgun halindeki kanın oranı olan perfüzyon indeksi (PI) ile gösterilir. Bu çalışmanın amacı göğüs ağrısı ile acil servise başvuran hastaların nedenlerini tesbit etmekte PI ve PVI nın HEART Skoru ile karşılaştırarak kullanılabileceğini araştırmaktır. Hastanemiz acil servise 01.04.2019-01.10.2019 tarihleri arasında göğüs ağrısı ile başvuran 154 hasta çalışmaya dahil edildi. Hastalar cinsiyet, yaş, göğüs ağrısı tipi, bilinen kronik hastalıkları ve sigara kullanım durumuna göre gruplandırıldı ve başvuru anındaki vital bulguları, PI değeri, troponin değeri, acil servis ve hastaneye yatanların sonlanımları (taburculuk, eksitus) çalışma formuna işlendi. 154 hastanın 58 i kadın hasta 96 tanesi erkek hastaydı. Hastaların 55'inin tipik göğüs ağrısı (13'ü kadın, 42'si erkek), 99'unın atipik göğüs ağrısı (48'ü kadın, 51'i erkek) mevcuttu. 154 hastanın 88'i acil servisten taburcu olurken 66 tanesi hastaneye yatırıldı. Yatan 66 hastanın 3 tanesi hastane içinde eksitus olurken, 63 hasta servisten taburcu edildi. HEART Skoru ile PI değerleri arasında istatistiksel olarak anlamlı fark saptanmıştır (p=0,003). HEART Skoru ile PVI değerleri karşılaştırıldığında istatistiksel olarak anlamlı fark saptanmamıştır (p=0,756). However, early diagnosis and treatment can reform the prognosis. For this reason, early diagnosis in the emergency department is very important for AMI and acute coronary syndrome (ACS). The HEART score was developed to evaluate chest pain in the emergency department. However, it is a scoring system that can distinguish among high-risk patients as well as low-risk patients. Life-threatening conditions such as ACS cause a decrease in peripheral blood flow. This decrease is indicated by the perfusion index (PI), that is the ratio of circulating blood to stagnant blood. The aim of this study was to determine the causes of patients applying to the emergency department with chest pain. In addition, we aimed to investigate whether PI and PVI can be compared with HEART Score. 154 patients admitted to our emergency department with chest pain between 01.04.2019 and 01.10.2019 were included in the study. Patients were grouped according to gender, age, type of chest pain, known chronic diseases and smoking status. The vital signs, PI value, troponin value, emergency department and hospitalization outcomes (discharge, death) were recorded in the study form. Of the 154 patients, 58 were female and 96 were male. 55 patients had typical chest pain (13 female, 42 male) and 99 had atypical chest pain (48 female, 51 male). 88 out of 154 patients were discharged from the emergency department and 66 were hospitalized. Of the 66 hospitalized patients, 3 died in the hospital and 63 patients were discharged from the hospital. There was a statistically significant difference between HEART score and PI values (p = 0.003). There was no statistically significant difference between HEART score and PVI values (p = 0.756). Çalışmamızın amacı hastanemiz acil servisine başvuran düşme öyküsü olan travma hastalarının demografik özelliklerini tespit ederek alınabilecek önlemleri belirlemek. Süleyman Demirel Üniversitesi Tıp Fakültesi Hastanesi'ne Ocak 2016-Mayıs 2017 tarihleri arasında başvuran 1433 düşme vakası retrospektif olarak değerlendirilerek olguların demografik özellikleri ortaya konuldu. Her olgu için cinsiyet, yaş, düşme sebebi, düşme yeri, etkilenen vücut bölgesi, düşülen ay, taburculuk -yatış durumu parametrelerine bakıldı. Bu vakaların 836 (%58,3)'sı erkek, 597 (%41,7)'si bayan olarak tespit edildi. Olgular 1-100 yaş aralığında olup yaş ortalaması 30,7(25.4) yıl olarak hesaplandı. Yaş gruplarına dağılım incelendiğinde en fazla başvuru 559 (%39) ile 0-18 yaş grubunda olduğu görüldü. 1433 hastanın 1126 (%78,6)'sı acil servisten taburcu edilirken, 307 (%21,4)'sinin kliniğe yatırıldığı saptandı ve yatırılan hastaların yaş ortalaması 43,57(29,4) yıl ile daha yüksek bulundu. Yaş grupları arasında en fazla yatış oranının 115(%45,3) hasta ile 60 yaş üzerinde olduğu görüldü. Çalışma hastalarının düşme sebeplerine bakıldığında 1389 (%96,9) kişinin dikkatsizlik, tedbirsizlik sebebiyle normal düşme ile, 44 (%3,1) kişinin iş kazası sebebiyle başvurduğu görülmüştür. Yatış oranları karşılaştırıldığında ise 19(%43,2) kişi ile iş kazalarının daha yüksek oranda yatırıldığı görülmüştür. Olguların düşme yerleri incelendiğinde 834 (%58,2) kişi ile aynı seviyeden düşmeler en fazla hasta sayısına sahip grubu oluştururken düşme yerlerine göre yatış oranı en yüksek olan %55,6 oran ile 1 metre üzerinden düşen grup oldu. Vakaların aylara göre dağılımı incelendiğinde en fazla hasta sayısı temmuz-ekim aylarında görüldü. Düşme sonucu etkilenen vücut bölümlerine bakıldığında 599 (%41,8)'kişi ile en fazla yaralanma extremite yaralanması iken 433 (%30,2)'kişi ile kafa travmaları 2.sırada yer aldı. Yatış oranlarına baktığımızda %52,6 yatış oranıyla multitravma hastalarının yatış oranı en yüksek olarak hesaplandı. Sonuç olarak, travmalar içerisinde önemli bir yere sahip olan düşme vakalarının azaltılmasında toplumun bu konuda bilinçlendirilmesi ve gerekli eğitimin verilmesinin önemi vurgulandı. We resourched retrospectively the demographic properties of 1433 patients who came Süleyman Demirel University Medical Faculty between january 2016 and may 2017 and determined demographic characteristics. For each case, gender, age, cause of fall, place of fall, affected body region, fallen months, discharge, admission parameters were examined. Of these cases, 836 (58.3%) were male and 597 (41.7%) were female. The cases are in the 1-100 age range and the average age is calculated as 30.7 (25.4) years. When the distribution of age groups was examined, it was seen that the highest number of applicants was 0-18 age group with 559 (39%) patient. Of the 1433 patients, 1126 (78.6%) were discharged from the emergency department and 307 (21.4%) were admitted to the clinic, and the mean age of the patients admitted to clinic was 43.57 (29.4) years. The average age of the patients admitted to the clinic was higher. The highest rate of admission among the age groups was found to be over 60 years with 115 (45.3%) patients. 96.9% (n:1389) of ceses were unexpected accidental falls, 3.1% (n:44) were workplace accidents. When the rates of hospitalization were compared, it was seen that 19 (43.2%) persons were higher rate hospitalization because of workplace fall. When the falling places of the cases were examined, it was found that 834 (58.2%) persons had the highest number of falls from the same level. The highest rate of hospitalization was the group fell over 1 meter with 55.6%. When the distribution of the cases according to the month was examined, the maximum number of patients was seen in July-October. When the affected parts of the body were examined, 599 (41.8%) were the most injured extremities and 433 (30.2%) were the head traumas. When we looked at the rates of hospitalization, the rate of hospitalization for multitrauma patients was calculated to be the highest with a 52.6% admission rate. In conclusion, the emphasis was placed on raising awareness and training of the community in reducing the number of falls that have a significant impact on trauma. Genellikle baş ağrısının özellikleri ve eşlik eden semptomlar gözönüne alınarak konulur. Kesin bir tanı yönetim yoktur. Laboratuvar tetkikleri ve görüntüleme yöntemleri sıklıkla diğer ciddi baş ağrısı nedenlerini dışlamak için kullanılır. Genellikle nörolojik muayene normal saptanır. İnsan vücudu herhangi bir stres faktörü ile karşılaştığında hipotalamopitüiter-adrenal (HPA) aks stimüle olarak adrenal stres hormonu olan kortizolün plazma konsantrasyonlarında artış meydana gelir. AVP (arjinin vazopressin) en önemli hipotalamik stres hormonlarından birisidir ve birçok stres faktörü tarafından uyarılmaktadır. Ancak pulsatil salgılandığı, anstabil bir molekül olduğu ve plazmadan hızlıca uzaklaştırıldığı için AVP seviyelerinin ölçülmesi zor bir durumdur. AVP daha büyük bir prekürsör peptid olan preprovasopressin'den copeptin ile birlikte ve eşit miktarda sagılanır. Copeptin plazma konsantrasyonları ile AVP üretimi yakın benzerlik tespit edilmiş ve copeptinin kortizole göre endojen stres seviyesini daha iyi yansıttığı gösterilmiştir. Copeptin seviyeleri ile hastalık ciddiyeti ve klinik sonlanım arasında pozitif ilişki varlığından dolayı hastalıklarda copeptinin prognostik bir marker olarak kullanılabileceği ileri sürülmüştür. Buradan yola çıkarak kopeptin düzeyi migren tanısının konmasında ve tedavisi için umut verici yeni stratejiler oluşturabilir. Bu çalışmanın amacı migrenli hastalarla sağlıklı kontrol grubu arasında serum bazal copeptin düzeylerinde farklılık olup olmadığı, varsa bu farklılığın hastalığın klinik görünümü ile ilişkisi ve migren hastalarında akut atak döneminde ve ataksız dönemde sapmalar gösterip göstermediği incelenmek; böylelikle copeptin migren hastalarında tanı ve tedavide rol oynayabilecek bir madde olup olmadığını belirlemektir. Bu sekilde gerek yeni tedavi stratejilerinin geliştirilmesine katkıda bulunmak ve gerekse migren ayırıcı tanısı ve prognozunun tayinine yardımcı olabilecek biyolojik bir belirteç bulmak amaçlanmıştır. Çalışma için sözel bilgilendirme sonrası 87 hasta yazılı onam vererek atak dönemi için venöz kan örneği verdi. İlk başvurularından en az 7 gün sonra telefon ile aranan atak dönemi için kan veren hastalardan 53 tanesi farklı zamanlarda ataksız dönemde tekrar venöz kan örneği verdi. Cinsiyet ve yaş olarak benzer 52 sağlıklı kontrol hastasının venöz kan örneği alındı. Çalışmaya toplam 103 hasta dahil edildi. Biyokimya tüpüne alınıp, örneklerin serumu ayrıştırıldıktan sonra ELİSA yöntemi ile copeptin düzeyi incelenmiştir. Migrenli hastaların yaş ve cinsiyet demografik özellikler kaydedildi. Kontrol grubu ile ataksız dönemde alınan copeptin ortalama değerleri birbirine oldukça yakın bulundu ve aradaki fark anlamlı değildi (p=0,984). Kontrol grubunda copeptin ortalaması 608,67±950,69 (medyan 359) pg/mL iken atak grubunda 689,27±730,35 (medyan 423) pg/mL bulundu. Benzer şekilde atak döneminde ölçülen copeptin değeri ile kontrol grubu arasında da anlamlı fark saptanmadı (p=0,069). Fakat ataklı migren grubuna ait ölçülen copeptin ortalaması ataksız dönemde ölçülen değerler göre anlamlı düzeyde yüksek bulundu (p=0,026). Copeptin ölçümlerinin migre hastalarını ayırt etmede diagnostik değeri olup olmadığının anlaşılması için ROC analizi yapıldı. Çalışmada ataklı ve ataksız migren dönemlerini ayırt etmede copeptin düzeylerinin düşük oranda ayırıcı tanı değerine sahip olduğu tespit edildi. Bu sonuçlar copeptinin sadece baş ağrısı hastalarında değil, diğer ağrılı durumlarda da çalışılmaya değecek bir peptid olduğunu ve bilgiler toplandıkça patofizyolojik rolünün daha iyi değerlendirilebileceğini düşündürmektedir. Migraine is usually diagnosed by taking cognizance of the headache features and the associated symptoms. There is no definitive diagnostic method. Laboratory tests and imaging methods are often used to exclude other causes of serious headache. Generally, neurological examination is normal. When the human body is compared with any stress factor, hypothalamopituitary-adrenal axis stimulates, the plasma concentrations of cortisol, which is the adrenal stress hormone, increase. AVP is one of the most important hypothalamic stress hormones and is stimulated by many stress factors. However, it is difficult to measure AVP levels because it is a unstabil molecule that is pulsatile secreted and rapidly removed from plasma. AVP and copeptin are both co-administered and equally secreted from a larger precursor peptide, preprovasopressin. Copeptin plasma concentrations were closely related to AVP production and copeptin has been shown to better reflect endogenous stress levels relative to cortisone. Due to the positive relationship between copeptin levels and disease severity and clinical outcome it has been suggested that copeptin may be used as a prognostic marker in diseases. From there, copeptin levels can create promising new strategies for diagnosing migraine and for treatment This study aims to examine if there is serum basal copeptin level difference between migraine patients and healthy control group, if this difference is related to the clinical appearance of the disease and whether migraine patients show deviations in the acute episode and in the absence episode and thus to determine whether copeptin is a substance, playing a role in the diagnosis and treatment of migraine. In this way, it is aimed to contribute to the development of new treatment strategies and to find a biological marker that can help to determine the differential diagnosis and prognosis of migraine. After verbal information, 87 patients gave a written consent and a venous blood sample for the episode. At least 7 days after the first application, 53 of the patients who gave blood for the wanted episode by phone gave repeat venous blood sample at different times.Venous blood samples of 52 healthy control patients with similar characteristics as gender and age were taken. A total of 103 patients were included in the study. After the samples were taken into biochemistry tube and the serum of the samples were separated, copeptin level was investigated by ELISA method. Age and gender demographic characteristics of patients with migraine were recorded. Mean values of copeptin taken during the attack-free period with control group were found very close to each other and the difference was not significant(p=0,984). The mean of copeptin in the control group was 608.67 ± 950.69 (median 359) pg / mL and in the attack group 689.27 ± 730.35 (median 423) pg / mL was found. Similarly, no significant difference was found between the control group and copeptin level measured during the attack period (p=0,069). Howeverthe mean of copeptin in the attacked migraine group was found to be significantly higher than in the non-attacked period (p=0,026). ROC analysis was performed to determine whether the copeptin measurements have a diagnostic value in distinguishing migraine patients. In the study, it was determined that copeptin levels had a low diagnostic value in differentiating migraine episodes with and without attack. These results suggest that copeptin is a peptide that is worth working not only in headache patients but also in other painful conditions, and that the pathophysiological role can be better evaluated as the information is collected. However, copeptin is still in infancy and more research is needed to prove its clinical usefulness. Süleyman Demirel Üniversitesi Acil Servisine Başvuran Kronik Obstruktif Akciğer Hastalığı (KOAH) olan hastaların geriye dönük değerlendirilmesi KOAH genellikle zararlı partikül veya gazlara ciddi maruziyetin neden olduğu havayolu ve/veya alveoler anormalliklere bağlı kalıcı hava akımı kısıtlanması ve solunumsal semptomlarla karakterize, yaygın, önlenebilir ve tedavi edilebilir bir hastalıktır. KOAH kronik bir hastalık olması yanında ataklarla seyrettiği dönemde acil servis başvurularına ve hastane yatışlarına neden olmaktadır. Bu çalışmanın amacı KOAH nedeniyle acil servise başvuran hastaların demografik özellikleri, vital bulguları, laboratuvar değerlerinin retrospektif olarak incelenerek hastaneye yatış ve mortalite üzerine etkisi olan parametreleri değerlendirmektir. 01.03.2016-01.03.2019 tarihleri arasında hastanemiz acil servisine başvuran 18 yaş ve üzeri KOAH'lı 444 hastanın dosyası retrospektif olarak tarandı. Hastaların demografik özellikleri, vital bulguları, hemogram, CRP, kan gazı değerleri kaydedildi. NLO, PLO, MLO değerleri sistem üzerinde hesaplandı. Toplam 444 hasta değerlendirildi. Hastalardan 188'inde (%47) yatış, 207'sinde (%42) taburculuk kararı verilmiş idi. Yatış kararı verilen hastalarda taburcu edilenlere göre nabız, SaO2, WBC, nötrofil sayısı, lenfosit sayısı, CRP, NLO, MLO, PLO, kan gazı değerlerinde istatiksel olarak anlamlı fark izlendi. Yatış kararı verilmesinde CRP ve pO2 değerlerinin prediktif etkisinin olduğu gözlendi. Hastaların 49'u (%11) ex olmuş idi. NLO ve laktat değerlerinin mortaliteyi tahmin etmede kullanılabileceği gözlendi. KOAH ile acil servise başvuran hastalarda hem kolay ulaşılabilir hem de hızlı sonuç veren hemogram, CRP, kan gazı parametrelerinin kullanılması klinisyene yatış kararı verilmesi ve hastaların prognozunu öngörme açısından yardımcı olmaktadır. In addition to being a chronic disease, COPD causes emergency services admittings and hospitalizations during the attacks. The aim of this study is to evaluate the demographic characteristics, vital signs, laboratory values of patients admitted to the emergency department due to COPD, to evaluate the parameters that affect hospitalization and mortality by retrospectively. The files of 444 patients with COPD aged 18 years or older who applied to the emergency department of our hospital between 01.02.2016-28.02.2019 were reviewed retrospectively. Demographic features, vital signs, hemogram, CRP, blood gas values of the patients were recorded. NLO, PLO, MLO values were calculated on the system. A total of 444 patients were evaluated. It was decided to hospitalize in 188 (%47) and discharge in 207 (%42). There was a statistically significant difference in pulse, SaO2, WBC, neutrophil count, lymphocyte count, CRP, NLO, MLO, PLO, blood gas values compared to the patients discharged. CRP and pO2 values had a predictive effect on admission. 49 (%11) of the patients were ex. It was observed that NLO and lactate values can be used to predict mortality. The use of hemogram, CRP, blood gas parameters, which are both easily accessible and fast, in patients presenting to the emergency department with COPD, helps the clinician to decide on hospitalization and predict the prognosis of the patients. Çalışmamızın amacı hastanemiz acil servisine başvuran intihar girişiminde bulunan hastalarının demografik özelliklerini ve laboratuvar özelliklerini tespit ederek alınabilecek önlemleri belirlemek. Süleyman Demirel Üniversitesi Tıp Fakültesi 20.04.2015 ile 20.04.2018 tarihleri arasında Süleyman Demirel Üniversitesi Hastanesi Acil Servisi'ne intihar girişimi sebebi ile başvuran 295 hastanın ve 197 kontrol grubunun demografik özellikleri ve tam kan sayımı parametreleri geriye dönük olarak ortaya konuldu. Her olgu için cinsiyet, yaş aralıkları, girişimde kullanılan yöntemler, medeni durum, eğitim durumu, meslek özellikleri, psikiyatrik ve diğer tıbbi hastalık özgeçmişi, kullandığı ilaçlar, önceki girişimi ve ailede girişim öyküsü, başvurunun saat-ay-mevsim zamanlarına göre ayrımı, taburculuk-yatış-exitus durumu ve tam kan sayımı parametrelerine bakıldı. Bu vakaların 198'si (%67,1) kadın 97'si ( %32,9) erkek olarak tespit edildi. Olgular 13-83 yaş aralığında olup yaş ortalamaları 26,5 (+-13,4) olarak hesaplandı. Yaş gruplarına göre dağılım incelendiğinde en sık 15-19 yaş aralığında başvuru olduğu görüldü. Vakaların 273'si (%92) şiddet içermeyen 23'ü (%8) şiddet içeren yöntemleri tercih ettiği görüldü. Şiddet içermeyen yöntemleri tercih eden hastaların 258 tanesinin ilaç içerek intihar girişiminde bulunduğu görüldü. Şiddet içeren yöntemleri erkek hastaların daha çok tercih ettiği görüldü. Vakaların aylara ve mevsimlere göre dağılımına bakıldığında en çok 78 (%26,4) vakanın kış mevsiminde en az ise 68 (%23) vakanın sonbaharda geldiği görüldü. 34 vaka ile en fazla Aralık ayında en az 17 vaka ile Ekim ayında başvurunun olduğu görüldü. Olguların başvuru saatlerinde ise 129 (%43,7) hastanın 18:00-24:00 arasında başvurduğu görüldü. Hastaneye başvuru sürelerinde ise 114 (%38,6) hastanın 1-3 saat aralığında hastaneye ulaştığı görüldü. Hastaların sosyo-demografik özelliklerinde ise %73,2 olgunun meslek sahibi olmadığı (işsiz-öğrenci) olduğu hesaplandı. %68 hastanın evli olmadığı ( bekar-dul) görüldü. Vakaların 134 (%45,3) tanesinin halen aktif eğitim hayatına devam ettiği bunların çoğunluğunun üniversite öğrencisi olduğu görüldü. Hastaların eğitim durumlarında ise en çok %14,9 ile lise mezunlarının oluşturduğu hesaplandı. Çalışmamızdaki vakaların 39 tanesinin ikinci veya fazla girişimiyle acil servise başvurduğu görüldü. Çoklu girişimlerde kadın cinsiyetin daha çok ön planda olduğu görüldü. Olguların 114(38,6) tanesinde önceden psikiyatrik hastalık olduğu görüldü. Bu hastaların 105 tanesi aktif olarak psikiyatrik endikasyonlarla ilaç kullandığı görüldü. İlaç alımı ile intihar girişiminde bulunan ve psikiyatrik hastalık öz geçmişi olan hastaların girişimde bulunurken sıkça kendi reçeteli ilaçlarını kullandığı görüldü. Vakaların %38'inin sigara %20'sinin alkol %2,4 ünün madde kullanmakta olduğu görüldü Hastaların 44 (%14,9) tanesi tedavi reddi yaptığı görüldü. 199 (67,5) yoğun bakım 29 ( %9,8) servis yatışı yapıldı. 10 hasta (%3,4) merkezimizde exitus oldu. 19 (%6,4) hasta çeşitli sebeplerle sevk edildi Olguların 262 tanesi GKS skoru 15 olarak acil servise giriş yaparken 11 tanesi GKS skoru 3 olarak acile giriş yaptı. GKS skoru 3 olarak başvuran 11 hastanın 6 tanesi exitus olduğu görüldü. 219 hastaya psikiyatri konsültasyonu istendi. Vakaların tam kan sayımı parametreleri incelendiğinde %20,3 vakanın anemik olduğu görüldü. Şiddet içeren yöntemleri kullanan hastaların arasında anemik vakalarla çok karşılaşıldı. Hastaların nötrofil sayılarının kontrol grubuna göre yüksek saptandı. Vakaların kontrol grubu ile karşılaştırıldığında NMR, NLR ve MLR 'nin daha yüksek olduğu hesaplandı. Sonuç olarak bölgemizde belirgin şekilde artış gösteren intihar girişimi vakalarının azaltılmasında risk faktörlerinin tekrar değerlendirilip toplum bilinçlendirmesi yapılmasının ve intihar sayısında azalma sağlanabilmesi için topluma psikososyal desteğin verilmesinin önemi vurgulandı., The aim of our study was to determine the demographic characteristics and laboratory features of patients who attempted suicide in the emergency department of our hospital. The demographic characteristics and complete blood count parameters of 295 patients and 197 control group who applied to Suleyman Demirel University Hospital Emergency Department due to suicide attempt between 20.04.2015 and 20.04.2018 were evaluated retrospectively. Gender, age ranges, methods used in the intervention, marital status, educational status, occupational traits, psychiatric and other medical illness history, medications used, previous intervention and family history hospitalization-exitus status and complete blood count parameters were examined. Of these cases, 198 (67.1%) were female and 97 (32.9%) were male. The mean age was 26.5 (+ -13.4) years. When the distribution according to age groups was examined, it was observed that most of the patients were in the 15-19 age group. It was observed that 273 (92%) of the cases preferred non-violent methods (23%). It was observed that 258 of the patients who preferred non-violent methods attempted suicide by taking drugs. It was observed that male patients preferred more violent methods. According to the distribution of cases according to months and seasons, 78 (26.4%) cases were seen in the winter and 68 (23%) cases were in the autumn. In October, there were 34 cases with at least 17 cases. 129 (43.7%) patients applied between 18: 00-24: 00 hours in the admission hours. 114 (38.6%) patients were admitted to the hospital within 1-3 hours. In the socio-demographic characteristics of the patients, it was calculated that 73.2% of the patients did not have a profession (unemployed-students). 68% of the patients were unmarried (single-widow). 134 (45.3%) of the cases were still active in education and the majority of them were university students. In the educational status of patients, it was calculated that high school graduates formed with 14.9%. It was seen that 39 of the cases in our study applied to the emergency department with the second or more attempts. It was seen that the female gender was more prominent in multiple interventions. It was seen that 114 (38.6%) of the cases had a previous psychiatric disorder. It was seen that 105 of these patients used drugs with active psychiatric indications. Patients who attempted suicide with drug intake and who had a history of psychiatric illness frequently used their own prescription drugs when attempting. It was seen that 38% of the cases used cigarette and 20% alcohol and 2,4% used substance. 44 (14.9%) of the patients were found to be refused treatment. 199 (67.5) intensive care services were performed in 29 (9.8%) patients. 10 patients (3.4%) died in our center. 19 (6.4%) patients were referred for various reasons While 262 of them had GCS score of 15, they entered the emergency room and 11 of them had GCS score and 3 of them entered the emergency room. Out of 11 patients who presented with a GCS score of 3, 6 were found to be exitus. 219 patients were asked for psychiatric consultation. When complete blood count parameters were examined, 20.3% of the cases were anemic. Anemic cases were encountered among the patients using violent methods. Neutrophil counts of the patients were higher than the control group. It was calculated that the cases had higher NMR, NLR and MLR compared to the control group.
