4. A Virtual Clinic for Diabetes Self-Management: Pilot Study

Abstract

Background Internet-based interventions to assist in diabetes management have the potential to provide patients with the information and support they need to become effective self-managers. Objective To assess the feasibility, acceptability, and effectiveness of an Internet-based virtual clinic designed to facilitate self-management in patients who used insulin pumps to manage their diabetes. Methods For a period of 6 months, 17 patients joined the virtual clinic. The system allowed patients to communicate with health professionals, interact with peers and access information. HbA1c, quality of life, and self-efficacy were monitored at baseline and after 6 months. Questionnaires and qualitative interviews examined patient experiences. Results Participants found the virtual clinic easy to use and positively rated its design. Peer support was the most valued aspect and the discussion boards the most used component. All participants highly rated the virtual clinic in terms of improving communication with peers, but few agreed it had improved communication with health care professionals. No significant improvements in physiological and psychological measurements were found. Regarding HbA1c measurements, there was no significant difference found between the pre- and post-test results (P = .53). Mean ADDQoL scores at baseline were -2.1 (SD 1.1, range -3.4 to -0.5) compared to -2.0 (SD 1.2, range, -4.6 to -0.4) post-test (n = 12), (P = .62). Surprisingly, patients’ confidence in their ability to perform self-care tasks was found to be significantly reduced from baseline to follow up (P = .045). Conclusions An Internet-based system to aid the management of diabetes appears feasible and well accepted by patients. The pilot study did not identify evidence of an impact on improving quality of life or self-efficacy in patients who used insulin pump therapy.

Abstract

Background The development of the Internet has created new opportunities for health care provision, including its use as a tool to aid the self-management of chronic conditions. We studied stakeholder reactions to an Internet-based “virtual clinic,” which would allow people with diabetes to communicate with their health care providers, find information about their condition, and share information and support with other users. Objective The aim of the study was to present the results of a detailed consultation with a variety of stakeholder groups in order to identify what they regard as the desirable, important, and feasible characteristics of an Internet-based intervention to aid diabetes self-management. Methods Three focus groups were conducted with 12 people with type 1 diabetes who used insulin pumps. Participants were recruited through a local diabetes clinic. One-on-one interviews were conducted with 5 health care professionals from the same clinic (2 doctors, 2 nurses, 1 dietitian) and with 1 representative of an insulin pump company. We gathered patient consensus via email on the important and useful features of Internet-based systems used for other chronic conditions (asthma, epilepsy, myalgic encephalopathy, mental health problems). A workshop to gather expert consensus on the use of information technology to improve the care of young people with diabetes was organized. Results Stakeholder groups identified the following important characteristics of an Internet-based virtual clinic: being grounded on personal needs rather than only providing general information; having the facility to communicate with, and learn from, peers; providing information on the latest developments and news in diabetes; being quick and easy to use. This paper discusses these characteristics in light of a review of the relevant literature. The development of a virtual clinic for diabetes that embodies these principles, and that is based on self-efficacy theory, is described. Conclusions Involvement of stakeholders is vital early in the development of a complex intervention. Stakeholders have clear and relevant views on what a virtual clinic system should provide, and these views can be captured and synthesized with relative ease. This work has led to the design of a system that is able to meet user needs and is currently being evaluated in a pilot study.

20. Impact of the COVID-19 pandemic on hospital episodes for falls and fractures associated with new-onset disability and frailty in England: a national cohort study.

Abstract

Background Older people with frailty are at risk of harm from immobility or isolation, yet data about how COVID-19 lockdowns affected them are limited. Falls and fractures are easily measurable adverse outcomes correlated with frailty. We investigated whether English hospital admission rates for falls and fractures varied from the expected trajectory during the COVID-19 pandemic, and how these varied by frailty status. Methods NHS England Hospital Episode Statistics Admitted Patient Care data were analysed for observed versus predicted outcome rates for 24 January 2020 to 31 December 2021. An auto-regressive integrated moving average time-series model was trained using falls and fracture incidence data from 2013 to 2018 and validated using data from 2019. Models included national and age-, sex- and region-stratified forecasts. Outcome measures were hospital admissions for falls, fractures, and falls and fractures combined. Frailty was defined using the Hospital Frailty Risk Score. Results 144,148,915 pre-pandemic hospital admissions were compared with 42,267,318 admissions after pandemic onset. For the whole population, falls and fracture rates were below predicted for the first period of national lockdown, followed by a rapid return to rates close to predicted. Thereafter, rates followed expected trends. For people living with frailty, however, falls and fractures increased above expected rates during periods of national lockdown and remained elevated throughout the study period. Effects of frailty were independent of age. Conclusions People living with frailty experienced increased fall and fracture rates above expected during and following periods of national lockdown. Abstract

The COVID-19 pandemic created an unprecedented shock to capture fisheries and aquaculture sectors. This scoping review of 670 articles explored the spatial, temporal, and thematic coverage of this event. The search period was January 1, 2020 to February 7, 2022. Articles were mainly peer-reviewed journals (88%) with the remainder from the gray literature (12%). Studies were performed at the global (21%), multi-country (11%), national (45%), and sub-national (23%) levels. Most studies involved primary or secondary data collection (71%) and the remainder were either review articles (17%) or commentaries (12%). Among the studies using primary and secondary data, nearly half (49%) were performed in the first five months of the pandemic (March to July 2020), and 84% within the first year of the pandemic (March 2020–February 2021). There were many studies in South and Southeast Asia, Southern Europe, China, and the United States, and fewer studies in other regions. The pandemic created challenges and opportunities, with heterogeneity in impacts among industrial and small-scale sectors, production methods, geographies, value chains, and by gender. Cumulative impacts from climate change and conflicts contributed to pandemic-related hardships. Increased unemployment and inflation led to rising food insecurity for small-scale producers, fishworkers, and low-income households. Aquatic food intake was more severely affected than other food groups during lockdowns, and it decreased more in low- and middle-income countries than in high-income countries. Responses were diverse, reflecting the capacity and resources of a country, but in general there was unprecedented public support to sustain the private sector (i.e., income support, tax relief, subsidies). As this study focused mainly on the first year of the pandemic, future work is needed to identify which groups exited the pandemic stronger or weaker, what factors enabled some populations to bounce-back, how the crisis affected value chains, and the effectiveness of interventions. Abstract

INTRODUCTION: While screening uptake is variable, many individuals feel they 'ought' to participate in screening programmes to aid the detection of conditions amenable to early treatment. Those not taking part in screening are often presented as either hindered by practical or social barriers or personally at fault. Why some people choose not to participate receives less consideration. METHODS: We explored screening nonparticipation by examining the accounts of participants who chose not to participate in screening offered by a national research trial of atrial fibrillation (AF) screening in England (SAFER: Screening for Atrial Fibrillation with ECG to Reduce stroke). AF is a heart arrhythmia that increases in prevalence with age and increases the risk of stroke. Systematic screening for AF is not a nationally adopted programme within the United Kingdom; it provides a unique opportunity to explore screening nonparticipation outside of the norms and values attached to existing population-based screening programmes. We interviewed people aged over 65 (n = 50) who declined an invitation from SAFER and analysed their accounts thematically. RESULTS: Beyond practical reasons for nonparticipation, interviewees challenged the utility of identifying and managing AF earlier. Many questioned the benefits of screening at their age. The trial's presentation of the screening as research made it feel voluntary-something they could legitimately decline. CONCLUSION: Nonparticipants were not resistant to engaging in health-promoting behaviours, uninformed about screening or unsupportive of its potential benefits. Instead, their consideration of the perceived necessity, legitimacy and utility of this screening shaped their decision not to take part. PATIENT OR PUBLIC CONTRIBUTION: The SAFER programme is guided by four patient and carer representatives. The representatives are embedded within the team (e.g., one is a co-applicant, another sits on the programme steering committee) and by participating in regular meetings advise on all aspects of the design, management and delivery of the programme, including engaging with interpreting and disseminating the findings. For the qualitative workstream, we established a supplementary patient and public involvement group with whom we regularly consult about research design questions.

Abstract

Introduction: Neonatal care is complex, involving multiple people and technologies within a community of care. When preterm babies are cared for far from home and/or transferred between units, the whole community of care (and particularly parent participation) is disrupted. Although previous studies have captured subjective experiences of parents, there has been little research exploring the material practices undertaken by parents as a consequence of place‐of‐care decisions, or the social organisation of those practices. Methods: As part of a wider study exploring optimal place‐of‐care, semistructured interviews were conducted between July 2018 and October 2019 with 48 parents (36 families) with one or more preterm babies (born at 27–31 weeks gestation) cared for in a neonatal unit in the last 12 months. Findings: We highlight parents' labour‐intensive and stressful work to: (1) parent in the neonatal care community (an oversight role that goes beyond contemporary notions of 'involvement'); (2) create continuity amid place‐of‐care disruptions; and (3) adapt to the managerial logics of neonatal care settings. Our analysis focuses on the work generated by managerial systems that organise place‐of‐care decision‐making and other efficiency‐focused practices. Parents are absorbed into negotiating institutional systems and diverted from routine parenting activities. Conclusion: Those involved in the organisation and management of neonatal care should take account of how managerial systems impact parents' workload, ability to participate in their baby's community of care and, ultimately, on the wellbeing and development of babies and their families. Patient or Public Contribution: The OPTI‐PREM study embedded parents' experiences of neonatal care into the research, through a discrete workstream that employed qualitative methodology to capture parents' experiences—as reported in this paper. Abstract

This study is an empirical exploration of Foucault's theoretical ideas on resistance, through a case study of cervical cancer screening and women's responses to the official discourse surrounding it. In England, this form of screening is organised through a national programme and consistently achieves coverage of over 80%. Given this high attendance it may appear that any resistance is negligible. However, this thesis argues that such a focus on attendance, or behaviour, is misguided and that, by focusing attention on the level at which the official discourse on screening is interpreted, understood and made sense of by individual women, it is possible to identify instances of thought and talk based resistance. Using qualitative interviews with a sample structured to include a range of ethnic backgrounds and ages, the thesis identifies three key forms of resistance. Firstly, women may resist the general subject position suggested within the official discourse and make sense of screening in ways that are meaningful to them as individuals. Secondly, many women resist the general 'at risk' status suggested and negotiate their own position drawing on a range of risk factors that do not always fit well with those medically recognised. Thirdly, in making sense of the information they receive, women frequently attempt to create a rational framework of knowledge and understanding which can lead to them interpreting issues such as risk factors or disease development in different ways. Based upon these, the thesis argues for conceptualising power and resistance in terms of a complex network of possibilities with multiple points of potential difference or divergence that can lead to individuals adopting very different subject positions. Although the majority of resistance detailed is thought and talk based, this is nevertheless important as it provides the means for challenges to the official discourse and constitutes a necessary prerequisite for further behavioural resistance.

Introduction Health and social care research ultimately aims to improve outcomes through the provision of insights and evidence. Innovations such as toolkits, modelsor frameworks are developed through applied health and social care research, with the intention of subsequently implementing these into practice in an effort to bring about improvements of some kind.Implementation challenges can arise from poor choice of strategies or lack of alignment to local contexts.Researchhasidentified the importance of involving and engaging patients, health professionals, policymakers, and other stakeholders in the design and delivery of the underpinning research, and in informing subsequent implementation. However, how and why co-production influences the sustainability of innovations in health and social careis unclear. Inclusion criteria This scoping review will include articlesrelated to the role of co-production in the sustainability of innovation in applied health and social care researchpublished in peer reviewed journals.The review will be limited to articles reporting applied health and social care research conducted in the United Kingdom. Methods Scopus, Web of Science, CINAHL, PubMed, and MEDLINE will be searched for studies. Titles and abstracts will be screened by two independent reviewers for assessment against the inclusion criteria, followed by a full-text review and data extraction. Data will be extracted using a data extraction tool developed by the reviewers, and presented in tabular format, accompanied by a narrative summary describing how the results relate to the review objective and question.

Abstract

OBJECTIVES: There is insufficient evidence to support national screening programmes for atrial fibrillation (AF). Nevertheless, some practitioners, policy-makers and special interest groups have encouraged introduction of opportunistic screening in primary care in order to reduce the incidence of stroke through earlier detection and treatment of AF. The attitudes of the public towards AF screening are unknown. We aimed to explore why AF screening participants took part in the screening. DESIGN: Semistructured longitudinal interview study of participant engagement in the SAFER study (Screening for Atrial Fibrillation with ECG to Reduce stroke). We undertook initial interviews face to face, with up to two follow-up telephone interviews during the screening process. We thematically analysed and synthesised these data to understand shared views of screening participation. SETTING: 5 primary care practices in the East of England, UK. PARTICIPANTS: 23 people taking part in the SAFER study first feasibility phase. RESULTS: Participants were supportive of screening for AF, explaining their participation in screening as a 'good thing to do'. Participants suggested screening could facilitate earlier diagnosis, more effective treatment, and a better future outcome, despite most being unfamiliar with AF. Participating in AF screening helped attenuate participants' concerns about stroke and demonstrated their commitment to self-care and being a 'good patient'. Participants felt that the screening test was non-invasive, and they were unlikely to have AF; they therefore considered engaging in AF screening was low risk, with few perceived harms. CONCLUSIONS: Participants assessed the SAFER AF screening programme to be a legitimate, relevant and safe screening opportunity, and complied obediently with what they perceived to be a recommendation to take part. Their unreserved acceptance of screening benefit and lack of awareness of potential harms suggests that uptake would be high but reinforces the importance of ensuring participants receive balanced information about AF screening initiatives.
