The increasing pace of advances in cardiology throughout the last few decades has fundamentally altered the natural course of heart patients. In the last few years, available therapies have been revolutionized completely by new transcatheter therapeutic approaches, novel ventricular assist devices, and new drugs. Developments in the electrical treatment of arrhythmias have been so fast over the last few years that access to available, state-of-the-art knowledge has become a major challenge. The proceedings of scientific meetings are therefore most helpful in this regard. Therapy in cardiology has advanced enormously in recent years. This has resulted in the organization of many meetings and the publication of numerous books dedicated to examining the latest aspects of, for instance, pharmacolog­ ic, electrical and surgical treatment. However, only a few of these meetings and publications have attempted to present an overall review of all advances that have taken place in the field of therapy. In the last years the Spanish Society of Cardiology has shown a great interest in the continuous medical education in cardiology. The Society has organized various meetings and has published several books on the above-mentioned topics. Consequently, the Society has decided to publish this book, an update of the therapeutics in cardiology, of which a Spanish version is also available. World renowned experts and outstanding Spanish cardiologists were invited to review all aspects of therapy in cardiology. Ensuring the safety of new medical products remains a major challenge for the pharmaceutical industry. Cardiac safety, particularly drug-induced heart rhythm abnormalities, remains an important cause of pipeline attrition and has resulted in countless major product recalls or label changes. The last of an ongoing series of Cardiostim monographs all devoted to cardiac pacing, was published four years ago. Since then, cardiac res- chronization for the treatment of heart failure has undergone spectacular progress and has revolutionized device therapy. Many patients have benefited from ventricular resynchronization often combined with an implantable cardioverter-defibrillator. More implantations are likely in the future as the indications continue to evolve with more attention being paid to the primary prevention of heart failure in selected patients and the treatment of earlier stages of left ventricular dysfunction. Thus, we felt it was time for the current Cardiostim monograph to focus exclusively on cardiac resynchronization so as to review the remarkable technologic and clinical advances in the field. We are grateful to the contributors who worked so hard to complete their manuscripts on time and their diligence in presenting new concepts and technical details in an easily understandable fashion. The number of ICD patients is increasing sharply, yet the number of electrophysiologists—physicians who specialize in the electrical system of the heart—is not increasing as dramatically. In this book, well-known physicians, Bocker, Eckardt and Breithardt have put together a succinct and focused book that compliments the CATA Series well. Implantation of defibrillators has evolved dramatically since its introduction by Mirowski in 1980. Technological improvements in devices and leads included a gradual reduction in the size of the device, the introduction of the endocardial approach in 1988, the biphasic waveform and antitachycardia pacing in 1991, pectoral implantation in 1995, inclusion of DDD pacing in 1996 and the delivery of atrial therapies in 1998. Since the first implantation, a huge body of information on the impact of implantable cardioverter defibrillators (ICD) on prognosis has become available, first as observational studies and later as prospective randomized trials. At the present time, there is a large evidence base from the several ICD trials, although it was not always certain that such a large body of ICD evidence would accumulate.

Resynchronization and Defibrillation for Heart Failure: A Practical Approach is one of the first texts to provide caregivers with information about the background and application of this new and rapidly expanding discipline. Its four authors are cardiologists who have expertise in device management, electrophysiology, and heart failure management.
