Background and Objective: To evaluate long-term posterior segment findings in children recovering from multisystemic inflammatory syndrome (MIS-C) associated with severe acute respiratory syndrome coronavirus 2. Patients and Methods: Our study included 22 patients who were admitted to an intensive care unit with a diagnosis of MIS-C between November 2021 and March 2022, and 25 healthy controls. The study included pediatric patients who had an eye examination an average of 12.35 ± 2.18 months after recovery from MIS-C. Detailed eye examinations and measurements of all participants were obtained retrospectively from patient files. Posterior segment parameters were measured using swept-source optical coherence tomography (OCT) and OCT-angiography (OCT-A); these parameters included peripapillary retinal nerve fiber layer (pRNFL) thickness, central macular thickness (CMT), subfoveal choroidal thickness (SCT), macular vascular densities (VD), and foveal avascular zone (FAZ) area. Results: Mean age was 9.7 ± 3.6 years in the MIS-C group and 10.6 ± 2.8 years in the healthy control group (P = 0.316). There were no statistically significant differences between the MIS-C group and the healthy control group in terms of pRNFL thickness, CMT, and SCT. However, in the MIS-C group, the macular superficial vascular plexus and deep vascular plexus showed significantly lower VD in the superior, inferior, nasal, and temporal quadrants compared to the healthy controls (P < 0.05 for all). A comparison of the superficial and deep FAZ area parameters of both groups showed no statistically significant difference (P > 0.05). Conclusions: We showed that patients who had recovered from MIS-C had retinal vascular damage at the long-term follow-up. Following up with these patients after recovery with OCT and OCT-A, which are noninvasive methods commonly used in the detailed evaluation of the posterior segment of the eye, could be beneficial for understanding the long-term effects of MIS-C on retinal microvasculature. In this study, we present a case of bilateral optic neuropathy and macular ischemia in the right eye associated with neurosarcoidosis. A 26-year-old woman presented to our clinic with complaints of bilateral blurred vision. Bilateral granulomatous anterior uveitis, vitritis, optic neuropathy, and macular ischemia were detected in the right eye in slit-lamp examination. She also reported complaints of fever, weakness, sweating, arthralgia, and headache for 2 months. She was referred to the pulmonary diseases unit of our hospital due to hilar lymphadenopathy seen in her chest x-ray, and biopsies were taken for diagnostic purposes. Histological analysis of the mediastinal lymph node biopsies revealed chronic, non-caseating, granulomatous inflammation. Furthermore, the patient was referred to a neurologist due to concomitant complaint of intense headaches. She was diagnosed with neurosarcoidosis supported by findings on cranial magnetic resonance imaging and lumbar puncture. She received a 3-day course of high-dose (1 g/day) intravenous steroid treatment (methylprednisolone) followed by a tapering dose of oral prednisone. The patient began receiving oral methotrexate 15 mg/week as a steroid-sparing agent. Significant improvement in neurological and ophthalmological symptoms occurred in the first week of treatment. In this case report, we emphasized that neurosarcoidosis should be included in the differential diagnosis of patients with both bilateral optic neuropathy and macular ischemia. Purpose: To evaluate the surgical results of augmented bimedial rectus recession in large-angle esotropia (ET). Material and Method: The records of forty-nine patients (26 male, 23 female) who underwent 6 mm and more bimedial rectus recession for large-angle deviation were reviewed. The mean follow-up period was 18.43±22.12 (between 3 and 84) months. The average of distance and near deviation amount was recorded in the postoperative 1st week and at the last visit as prism diopter (PD). Nineteen patients whose follow-up periods were longer than 1 year (mean: 40.74±20.94 months) were evaluated as a different group. Success and reoperation rates of both groups were assessed. Results: The mean age of the 49 patients was 7.04±8.77 (range from 1 to 67) years and the mean preoperative esodeviation amount was 49.8±11.9 (range from 35 to 85) PD. In the postoperative 1st week, the average deviation was 10.67±8.65 PD and success rate was found to be 61.1%, and in the last visit, the mean deviation angle was 9.82±9.24 PD and the success rate was 67.3%. Five of the cases (10.2%) needed reoperation. The mean age of the 19 patients with follow-up period longer than 1 year was 4.57±4.39 years (ranged between 1 and 17), and the mean preoperative esodeviation angle was 44.21±6.07 (ranged between 35 and 55) PD. In the group with longer follow-up period, the postoperative average deviation amount and success rate were 8.58±8.23 PD and 68.4% in the 1st week, and 8.05±8.02 PD and 73.7% in the last visit. Reoperation rate was found to be 21.05% (4 cases) in this group. Discussion: Although augmented bimedial recession technique for the correction of large-angle ET gives good results in short periods, additional surgeries may be needed after longer follow-up periods. A 23 year old woman was admitted to Dr. Lutfi Kirdar Kartal Training and Research Hospital 1.st Eye Clinic at April 2007 because of decreasing in visual acuity in her both eyes. The uncorrected visual acuity was 0.7 and corrected visual acuity was 1.0 in both eyes. During the patient's visual consultation, it was realized that theuncorrected visual acuity was 0.7 and corrected visual acuity was 1.0 in both eyes. The routine biomicroscopic evaluation revealed that anterior segment was normal but fundus examination pounted out that pigmented, well demarcated, oval lesions like pigment epithelium hypertrophy in both periphery of the retina. As it was tought that this lesions may associate with sistemic disorders, the patient was interrogated. It was learned that she had diarrhea and stomach ache for 6 months but it was unidentified. Owing to this reason the patient was consultated with internal medicine. When colonoscopy was performed, multipl polyps in the colon were demonstrated. The biopsy was taken and adenocarsinoma developed from the basement of polyposis coli was documented. Liver metastasis was identified with computarized tomography. The patients was thought to be familial adenomatous polyposis and family cross-hatching was made. Both of two brothers were diagnosed with polyposis coli and fundus examination showed the same pigmented oval lesions. The total colectomy was performed to all of the tree patients who were followed by general surgery. During the routine ophthalmologic examination, even if the visual acuity of the patient is good, the ophthalmologic examination is being made carefully, in fact the findings seeming to be not important may be helpful for early identification of serious ophthalmologic or systemic diseases. To investigate the relationship between age-related macular degeneration (AMD) and refractive error and axial length, as well as the socio-demographic characteristics and biochemical variables that may affect this relationship. Materials and Methods: A total of 196 eyes of 98 patients over 50 years of age who were diagnosed with AMD at our clinic were included in this cross-sectional study. Early and late AMD findings were categorized according to the age-related eye disease study grading scale. Objective refractive error was measured by autorefractometer, confirmed by subjective examination, and spherical equivalent was calculated. Refractive errors of -0.50 D to 0.50 D were classified as emmetropia, <-0.50 D as myopia, and >0.50 D as hyperopia. Axial length was measured by ultrasonic biometry and values ≤23.00 mm were classified as short, >23.00 and <24.00 mm as normal, and ≥24.00 mm as long axial length. Demographic, systemic, and biochemical parameters of all patients were also investigated. Results: Hypermetropic refractive error and shorter axial length were significantly more common than the other groups (p<0.01). No differences were observed between early and late stage groups in terms of refractive error and axial length. Patients with myopia had significantly lower values for total cholesterol, triglyceride, fasting blood glucose, and proportion of smokers. Rates of oral nutritional supplement use and fish consumption were significantly higher in the early AMD group. The most common comorbidity among the AMD patients in our study was essential hypertension. Conclusion: Hyperopic refractive error and shorter axial length were found to be associated with AMD. Longitudinal studies including larger patient numbers are needed to elucidate the causal and temporal relationship between hyperopic refractive error and AMD. [ABSTRACT FROM AUTHOR]
