Abstract

Background: Breastfeeding is crucial for child health and development, yet its practices and their implications on children's nutritional status vary. Aim & Objectives: This study aimed to explore the determinants of breastfeeding practices and their effects on the nutritional status of children in an Indian population. Methodology: This study is conducted in four villages of Udaipur district, Rajasthan, among 120 mother-child pairs. The data is collected on anthropometric measurements, socioeconomic factors, and child health parameters. Results: We found that only 37.5% of children were exclusively breastfed. Significant associations were observed with birth month (P=<0.001) and nearly significant associations with mode of delivery (P=0.06) and undernutrition (P=0.07). Conclusion: These findings highlight the need for larger-scale research to further investigate these associations and enhance breastfeeding practices to improve child health outcomes. Abstract

Since there is no current international consensus on the optimal approach for pain management in acute pancreatitis (AP), analgesic practices may vary across different healthcare settings. This study explored global disparities in analgesic use, in particular opioids, during admission and at discharge in hospitalised AP patients. This was a post hoc analysis of the prospective PAINAP database, which included all admissions for AP between April and June 2022 with a 1‐month follow‐up. Demographic details, analgesic use, and clinical outcomes were recorded during admission and at discharge. Odds ratios (ORs) for opioid use during admission and at discharge were identified using multivariable regression analyses. Amongst the 1864 patients (52% males, median age 56 (interquartile range, 41–71)) across three different continents, simple analgesics were predominantly used as the primary analgesic (70%). Opioid use during admission was lowest in European centres (67%). Admission in Asian (OR, 2.53 (95% confidence interval (CI), 1.59–4.04), p< 0.001), and Australian (OR, 5.81 (95% CI, 3.19–10.56), p< 0.001) centres was associated with opioid administration during admission compared with European centres. Increased pain severity, longer pre‐admission pain duration, organ failure, and longer length of admission increased opioid use during admission. At discharge, Asian (OR, 2.01 (95% CI, 1.40–2.88), p< 0.001) and Australian (OR, 1.91 (95% CI, 1.28–2.85), p= 0.002) centres were associated with opioid prescription compared with European centres. Increased pain severity, longer pre‐admission pain duration, acute necrotic collections, and walled‐off necrosis also increased the likelihood of opioid prescription at discharge. There are substantial intercontinental differences in opioid use for AP pain. Accordingly, there is a need for international guidelines on pain management in AP.

Introduction: The hospital environment is recognized as one of the most stressful workplaces. The impacts of the hospital environment on the health-related quality of life of nurses necessitate the consideration of their viewpoints in the field of design and maintenance of the hospital environment. Accordingly, this study aimed to identify environmental factors affecting the promotion of nurses' health-related quality of life. Methods: In this study, 10 nurses working in a selected hospital were chosen through purposeful sampling and participated in the narrative analysis. The required data were collected through narratives from semi-structured interviews with participants. The content of all narratives was evaluated by the thematic analysis. Results: The environmental factors affecting the nurses' health-related quality of life were classified into four main themes: physical environment”, “functional support of the environment”, “social support of the environment”, and “psychological support of the environment. Besides, the themes related to the physical environment were classified under space configurations and air quality. Furthermore, functional support was placed under ergonomics. Communications and territory were found as subsets of the theme of social support, while interior design, tranquility, light quality, air quality, and the sense of control were classified under the main theme of psychological support. The frequency of themes repeated in the narratives indicated that psychological support and the physical environment have the highest impact on the health-related quality of life of nurses. Conclusion: According to the findings of the study, policymakers, managers, and designers are recommended to try to understand nurses' perceptions of positive workplace components, and to introduce necessary environmental actions and interventions to promote the health-related quality of life of nursing staffs.

14. Lifestyle patterns and their nutritional, socio-demographic and psychological determinants in a community-based study: A mixed approach of latent class and factor analyses.

Abstract

BackgroundLifestyle risk factors, such as unhealthy diet, physical inactivity or tobacco smoking can have detrimental effects on health and well-being. Therefore, it is important to examine multiple lifestyle risk factors instead of single ones. Cluster analysis allows the combination of single health behaviors in order to recognize distinguished behavior patterns. This study aimed to evaluate lifestyle patterns of general adult population in northwest of Iran with particular focus on dietary patterns, physical activity, and smoking status.MethodsThe current cross-sectional study consists of 525 adults aged 18-64 years from East-Azarbaijan Iran. Latent class analysis (LCA) was applied to recognize patterns of lifestyle behaviors with ingredients of diet, physical activity, and smoking status. Dietary intake was assessed using a validated food frequency questionnaire and dietary patterns were derived using factor analysis. Biochemical parameters including fasting blood sugar (FBS), serum lipids, liver enzyme and serum 25(OH)-D3 were measured with commercial ELIZA kits.ResultsMean ages of participants were 42.90 ± 11.89 years. Using principal component analysis (PCA) three major dietary patterns were extracted including traditional dietary pattern (e.g. nuts and dry fruits), unhealthy dietary pattern (e.g. fast foods, refined grains) and the healthy dietary patterns (e.g. fruits, vegetables). Using LCA, three classes of lifestyles pattern were identified: 1st class was characterized by a healthy dietary pattern, moderate physical activity, and low probability of smoking. 2nd class was characterized by a traditional dietary pattern, low level of physical activity and low probability of smoking and 3rd class was characterized by a unhealthy dietary pattern, low level of physical activity and low probability of smoking and further analysis found that there were significant differences in body mass index (BMI), Waist-to-hip ratio (WHR), FBS, Hemoglobin (Hb), education levels and anxiety status between classes (P ConclusionThis study attempts to classify Iranian adults by their own health behavior. Healthcare professionals should be aware of associations between different lifestyle risk factors and health promotion strategies should further focus on multiple behaviors at the same time. In our country, more studies about the adult population are needed to support the observed findings of our study and therefore allow for a certain generalization of the observations.

Abstract

Background: There is limited information on the relationship between family psychological functions and perceived social support with progress in cigarette smoking stages in adolescents. This study was aimed to determine the relationship between family psychological function and perceived social support through a transition in different stages of cigarette smoking in adolescents of Tabriz, Iran. Methods: In this study, 4,216 students (14–19 years old) from high schools were selected by multistage cluster sampling method. Initially, we used valid and reliable questionnaires for demographic characteristics, risk factors, Iranian family psychological function, and perceived social support. The smoking status questionnaire was administered twice, with an interval of 6 months. Data were analyzed using univariate and multivariate logistic regression analysis. Results: Overall, 3,968 students with the mean (standard deviation) age of 15.96 (0.75) years completed the questionnaire in the first phase of the study. The results showed that lower levels of family psychosocial function (P < 0.001) and perceived social support (P < 0.001) in the univariate state were significantly associated with progress in cigarette smoking stages. By controlling the potential confounding factors, the weak and moderate family psychological function compared to the strong function increased the chance of progress in the cigarette smoking stages by almost 11 and 4 times, respectively (P < 0.001). The low and moderate level of perceived social support compared to the high level increased the chance of progress in cigarette smoking stages by almost 7 and 2 times, respectively (P < 0.001). Conclusions: It is suggested to perform an organized, precise, and operational planning for strengthening the family psychological functions and providing an appropriate social support condition among adolescents to prevent the tendency toward cigarette smoking and its more advanced phases.

This is an exploratory and analytical study of the phenomenon of community organisations of the Bangladeshi immigrants who settled in the inner city area of East London. The primary aim of the study is to explore and explain the community organisations, analyse the organisational processes that took place in their goal attainment efforts and ascertain their contributions in meeting community needs. A case study approach has been adopted in this research and seven different community organisations, each representing a particular category of organisation, have been chosen out of over 100 identified groups/ organisations operating in the area. The broad categories of the organisations, with a much overlapping in their activities, represented in the study are: a welfare association, a community centre, a youth organisation, a community school, a women's association, an educational pressure group, and a cultural organisation. Six of the seven organisations have been chosen out of some 97 identified groups operating within the area of the inner London Borough of Tower Hamlets. This has the largest Bangladeshi community ;n Britain (22.9 per cent of the Borough population, according to the 1991 Census). The seventh organisation has been chosen out of half a dozen groups operating ;n the outer London Borough of Waltham Forest which has only a small Bangladeshi community (0.9 per cent of the Borough population). Detailed case histories of the seven selected organisations have been compiled under the headings of: origin, objectives, activities, resources, membership, leadership, decision making, tactics and strategies adopted, achievements and failures. These case histories have been discussed and analysed to explain what organisational processes took place and what benefit and/or services resulted from the activities of the organisations and what community needs, if any. have been met.

Abstract

Introduction: Intravenous fosphenytoin is widely used for acute exacerbation of trigeminal neuralgia, however, few studies have investigated this treatment. We aimed to examine the efficacy and side effects of initial intravenous fosphenytoin plus oral tapering of phenytoin for exacerbation of trigeminal neuralgia. Methods: Consecutive patients with primary trigeminal neuralgia were included in this prospective observational 90-days follow-up study. Data were collected using standardized interviews before, at 24 hours, day 7, 30 and 90 post loading dose. The primary outcome was the proportion of responders defined as a 50% reduction in pain intensity 24 hours post loading dose. Results: We included 15 patients. Nine patients (60%) were responders. Pain intensity 24 hours post loading dose was reduced by 5.00 points on the numerical rating scale (p < 0.001), and at day 7 by 5.5 points (p < 0.001). The most common side effects were hypotension and dizziness. Conclusion: Intravenous fosphenytoin relieves trigeminal neuralgia pain in most patients and provides a window for titrating prophylactic trigeminal neuralgia medications or planning neurosurgery. The decision to administer intravenous fosphenytoin should be taken with support from trigeminal neuralgia experts and involves considerations of co-morbidities and other treatment options for acute exacerbation of trigeminal neuralgia. 28. Is intravenous magnesium sulphate a suitable adjuvant in postoperative pain management? – A critical and systematic review of methodology in randomized controlled trials.

Abstract

A growing worldwide focus on opioid-free anaesthesia entails multimodal analgesic strategies involving non-opioids such as magnesium sulphate (MgSO4). Several systematic reviews have concluded there is beneficial analgesic effect of MgSO4 administration but do not take considerable heterogeneity among the studies into consideration. Medical literature published until June 2021 was searched in PubMed/Medline, Embase, Central and Web of Science: The final search yielded a total of 5,672 articles. We included only randomised controlled trials assessing the effect of intravenous MgSO4 on opioid consumption and acute postoperative pain when compared to either placebo or standardized analgesic treatment. The primary aim was to compare the homogeneity of essential variables and confounders. A post-hoc meta-analysis demonstrated a reduction in both postoperative morphine consumption (−6.12 mg) and pain score (−12.32 VAS points) in favour of the MgSO4-groups. Data for meta-analysis was missing from 19 studies (45%) on morphine consumption and 29 studies (69%) for pain score, the majority of which reports no effect for either morphine consumption or pain score. The calculated heterogeneity among the included studies was considerable for both outcomes; I2=91% for morphine consumption and I2=96% for pain score. Although we found a per se reduction in opioid consumption and pain score, methodological heterogeneity and clinical shortcomings of pre-, intra-, and post anaesthetic data precludes conclusions on clinical importance of intraoperative intravenous MgSO4. Abstract

Background and Objective: Self-injury (SI), also called self-mutilation or cutting, is highly stigmatized emotional disorder. The aim of this study was to estimate the incidence rate of self-injury among male pupils in Tabriz – North West in Iran and to evaluate the associated risk factors. Materials and Methods: In this longitudinal study of all the pupils high school in Tabriz, 1772 (13.7%) were randomly sampled, and assessed in this and subsequent year. A self-administered questionnaire with 48 questions was distributed to high school in February and March 2005. After one year (February and March 2006), another questionnaire with 10-items was distributed to those students to determine the incidence rate of self-injury. The influence of different factors on self-injury was evaluated with a logistic regression model. Results: The mean age of the subjects was 16.3±0.87 (min 15, max 19) years. At the end of follow–up, 4.8% of pupils reported incidence of self-injury. Most type of incidenced self-injury were cutting and burning. The results of logistic regression model indicate that transition in smoking stages (OR=2.81), incidence of alcohol drinking (OR=2.27) and having smoker friend (OR=1.78) were factors associated with incidence of self-injury. Conclusion: This study showed that incidence rate of self-injury in this group of adolescents was 4.8% in one year, which related to age, alcohol and ciggaret smoking.

Abstract

A positive association between renin-angiotensin system, especially AT1 receptor, and oxidative stress in the pathogenesis of hypertension and cardiovascular/renal diseases has been suggested. However, the role of oxidative stress, especially superoxide radicals in renal sodium handling in response to AT1 and AT2 receptors, is not known. Therefore, the present study was designed to investigate the role of NAD(P)H oxidase (NOX), a major superoxide radical producing enzyme, in AT1 and AT2 receptor function on natriuresis/diuresis in Sprague-Dawley rats. The rats under anesthesia were intravenously infused with NOX inhibitor apocynin (3.5 [micro]g x [kg.sup.-1] x [min.sup.-1]), the AT1 receptor antagonist candesartan (100 [micro]g/kg; bolus), and the AT2 receptor agonist CGP-42112A (1 [micro]g x [kg.sup.-1] x [min.sup.-1]) alone and in combinations. Candesartan alone significantly increased urinary flow (UF; [micro]l/30 min) by 53 and urinary Na excretion ([U.sub.Na]V; [micro]mol/min) by 0.4 over basal. Preinfusion of apocynin had no effect on the net increase in UF or [U.sub.Na]V in response to candesartan. On the other hand, apocynin preinfusion caused profound increases in CGP-42112A-induced UF by 72, [U.sub.Na]V by 1.14, and fractional excretion of Na by 7.8. Apocynin and CGP-42112A alone did not cause significant increase in UF or [U.sub.Na]V over the basal. CGP-42112A infusion in the presence of apocynin increased urinary nitrite/nitrates and cGMP over basal. The infusion of candesartan, apocynin, and CGP-42112A alone or in combinations had no effect on the blood pressure or the glomerular filtration rate, suggesting tubular effects on natriuresis/ diuresis. The data suggest that NOX may have an antagonistic role in AT2 receptor-mediated natriuresis/diuresis possibly via neutralizing nitric oxide and thereby influence fluid-Na homeostasis. angiotensin II receptors; apocynin; nitric oxide doi: 10.1152/ajprenal.00310.2010.
