2. Comparing Scoring Systems used in Intensive Care Unit Regarding Predictive Roles Mortality

Objective: It is important to use scoring systems for admission to ICU or transfer of patients from ICU to other clinics. In our study we compared performance of APACHE II, SAPS II, MPM0 II and LODS scoring systems to predict mortality rates. Materials and Methods: All patients admitted to anesthesiology and reanimation clinic between 2005-2006 were evaluated retrospectively. The study included 615 patients. Demografic data were collected and APACHE II, SAPS II, MPM0 II, LODS scores calculated. ICU and hospital stay, clinic where the patient was referred from and 6-month mortality were noted. Results: APACHE II, SAPS II, MPM0 II and LODS all found to be significantly associated with mortality prediction in ICU. In ROC analysis area under curve was calculated as 0.812 (95% CI: 0.778-0.845) for APACHE II; 0.841 (95% CI: 0.810-0.872) for SAPSII; 0.797 (95% CI: 0.763-0.832) for MPM0 II; and 0.815 (95% CI: 0.781-0.849) for LODS. ICU mortality was 46%. Scoring systems were not associated with ICU stay when evaluated with Pearson correlation. All the values in ROC systems showing the relationship between scoring systems and 6-month mortality were under 0.5 and these values were not significant. 6-month ICU mortality was 24%. Conclusion: Out of 615 ICU patients, 289 were deceased and 326 were discharged. Although APACHE II, SAPS II, MPM0 II and LODS all were significantly associated with ICU mortality prediction, when compared with the other two scoring systems SAPS II ve LODS seems to be more suitable for ICU mortality prediction due to more feasibility, higher specifity and sensitivity. 3. Evaluation of anthropometric measurements and clinical tests in the diagnosis of difficult airway in patients undergoing head and neck surgery

The aim of this study is to research the incidence of difficult airways and the effectiveness of anthropometric measurements and clinical tests used to predict difficult airways in patients undergoing head and neck surgery.This study was performed on a total of 200 patients over the age of 18 who underwent head and neck surgery between December 2019 and March 2020. The demographic data of the patients in the preoperative period, previous operations/ radiotherapy history applied to the head and neck region, and obstructive sleep apnea syndrome symptoms were recorded. In the physical examination, the jaw structure, mouth opening, jaw movement, and tooth structure of the patients, modified Mallampati classification, head and neck movements, neck circumference, thyromental and sternomental distance, atlantooccipital joint mobility, upper lip bite test, Wilson risk scoring, and Cormack-Lehane classification were evaluated.The difficult laryngoscopy rate was identified as 19%, and the difficult intubation rate as 8%. Operation history related to head and neck (p = 0.002), presence of at least two of the obstructive sleep apnea syndrome symptoms (p = 0.008), Modified Mallampati score (p = 0.009), Wilson risk score (p = 0.004), upper lip bite test (p0.0001) and mouth opening (p = 0.001) were found to be associated with difficult laryngoscopy. Modified Mallampati score (p = 0.002), Wilson risk score (p0.0001), upper lip bite test (p0.0001), mouth opening (p0.0001), sternomental distance (p = 0.003), Atlantooccipital joint mobility (p = 0.001), and Cormack-Lehane classification (p0.0001) were found to be associated with difficult intubation. According to multiple logistic regression analysis, the results obtained for sternomental distance and mouth opening were OR 0.8, 95% CI 0.6-1.1 and OR 0.2, 95% CI 0.1-0.4, respectively.In patients who underwent head and neck surgery, it was observed that the frequency of difficult airway was higher, and particularly the Modified Mallampati score, Wilson risk score, upper lip bite test, and mouth opening were associated with both difficult laryngoscopy and difficult intubation.

It is recommended to heat and humidity COOne hundred patients who underwent total laparoscopic hysterectomy for benign pathology were divided into two groups: Group CD (cold-dry) patients were administered standard COA total of 96 patients were included in the study, taken from the 100 patients. Group HH (n:47) had only higher systolic blood pressure at 75, mean blood pressure at 50 and 55 and a lower heart rate between 15 and 45 min (p:0.049, 0.037, 0.013 respectively). Pain score, morphine consumption, end-tidal COIt has been found that both standard and heated-humidified CONCT04508387.

5. Ventricular fibrillation due to hypokalemia as a consequence of thallium scintigraphy: A case report

Olgu, (44 yaşında, erkek) böbrek transplantlı, koksartroz tanısıyla total kalça protezi planlanarak ortopedi kliniğine yatırıldı. Hastanın preoperatif anestezi değerlendirilmesinde üç gündür devam eden diyare ve son birkaç gündür 20 dakikadan kısa süren eforla ilişkisiz göğüs ağrısı mevcuttu. Kardiyoloji konsültasyonu doğrultusunda hastaya talyumlu miyokard perfüzyon sintigrafisi çekildi. Sintigrafi sonrası hastada hipokalemiye bağlı ani kardiyak arrest gelişti. Hasta orotrakeal entübe edilerek monitorize edildi. Hastanın ritminin ventriküler fibrilasyon olması üzerine defibrile edilerek, ERC kriterlerine göre kardiyak kompresyonlara devam edildi. Dirençli ventriküler fibrilasyonlar ve kan potasyum düzeyinin düşük olması nedeniyle resüsitasyonun 15. dakikasında20 mmol l-1 KCl ve 12 mmol l-1 Magnezyum 5 dakikada infüze edildi. Sinüs ritmi sağlanan hasta kardiyoloji servisinde izlendi.Hipokalemi ventriküler disritmilere neden olan elektrolit bozuklukları arasında yer alır. Preanestezik muayanede diüretik kullanan ve gasrointestinal bozuklukları olan hastalarda hipokalemi gelişebileceği akılda tutulmalıdır. Ayrıca hipokalemisi olan hastaların talyum miyokard sintigrafisi sonrası hipokalemileri daha da derinleşebilir ve bunun sonucunda dirençli ventriküler disritmiler gelişebilir., A 44 year old man who had kidney transplantation was hospitalized to orthopedia clinic for total hip prosthesis due to his coxarthrosis. In his preoperative evaluation, he had diarrhea for three days, and he had a chest pain that lasted less than 20 minutes which was not related with effort. According to cardiology consultation the patient was applied myocard perfussion scintigraphy with thallium. After the scintigraphy, a sudden cardiac arrest occured. He was monitorized after having orotracheal intubation. Since he was in ventricular fibrillation, he was defibrillated. According to the ERC criteria cardiac compressions were continued. Because of resistant ventricular fibrillation and hypokalemia at the 15th minute of cardiac resusitation, 20 mmol l -1 KCl and 12 mmol l -1 magnesium were administered within five minutes. After these applications sinus rythm was achieved and the patient was transfered to the cardiology service and he was followed up there. Hypokalemia exists in the electrolyte imbalances that cause ventricular dysrhythmias. Occurance of hypokalemia in patients with diarrhea and gastrointestinal disorders should be kept in mind during the anesthetic evaluation. In addition, hypokalemia can become deeper as a consequence of thallium scintigraphy that can result with resistant ventricular dysrhythmias.

Histeroskopik işlemler, kolay uygulanabilme özelliği sayesinde jinekolojik vakalarda tanı ve tedavi aracı olarak yaygın olarak kullanılmaktadır. Ancak özellikle operatif histeroskopik girişimler sırasında hiponatremi ve akciğer ödemi gibi hayatı tehdit eden komplikasyonlarla karşılaşılabileceği de unutulmamalıdır. Bu tür komplikasyonların genellikle kısa süreli tedavilerle düzeltilebileceği gibi bazı vakalarda mekanik ventilasyon tedavisine gereksinim olur. Bu olgu sunumunda, operatif histeroskopi sırasında ciddi hiponatreminin eşlik ettiği ve yoğun bakımda mekanik ventilasyon tedavisi gerektiren akciğer ödemi olgusunun tedavi yönetimini ve bu komplikasyonlar ile ilgili önleyici yaklaşımları tartışmayı amaçlıyoruz., Hysteroscopic procedures, due to ease of application properties has been widely used at diagnosis and treatment of gynecological cases. However, it should kept in mind that life-threatening complications such as hyponatremia and pulmonary oedema might occur during the operative hysteroscopic procedure. Such complications are usually treated in a short term period but unfortunately some cases require mechanical ventilation therapy. In this case report, we aimed at presenting the treatment of a patient under histeroscopic procedure with severe hyponatremia accompanied by the phenomenon of pulmonary oedema requiring mechanical ventilation, and discuss the intensive care treatment approaches and management.
