Giriş: Yenidoğan sarılığında kan değişimi, yaşamsal önemde ancak riskleri olan girişimsel bir işlemdir. Kan değişimi gerektirecek hiperbilirubinemi gelişiminin önlenmesi öncelikli yaklaşımdır. Çalışmamızda şiddetli hiperbilirubinemide, kan değişimi risk faktörleri ve ileri dönem nörogelişimsel durumun araştırılması amaçlanmıştır. Gereç ve Yöntem: Total serum bilirubin değeri Amerikan Pediatri Akademisi rehberine göre kan değişimi sınırında veya sınırın üstünde, 35 ve üstü gestasyonel hafta doğumlu 104 yenidoğan çalışmaya dahil edildi. Olguların 12-36 ayındayken ulaşılabilen 65’ine Gelişimi İzleme ve Destekleme Rehberi uygulandı. Kan değişimi uygulanan ve uygulanmayanlar sosyodemografik özellikler, risk faktörleri, etiyoloji, tedavi süreçleri ve ileri dönem nörogelişimsel durum açılarından karşılaştırıldı. Bulgular: Kan değişimi olguların %18,3’ünde uygulandı. Başvuru total serum bilirubin ve bilirubin/albümin oranı yüksekliği kan değişimi riskini arttıran temel değişkenlerdi. Kan değişimi riskini artıran eşik değerler, total serum bilirubinde 26,43 mg/dl ve bilirubin albümin oranında 7,43 mg/g saptandı. Sarılık etiyolojisinde ilk üç sırada; hemolitik hastalık, dehidratasyon ve erken anne sütü sarılığı ile prematürelik saptandı. Etiyolojilerle tedavi şekilleri arasında anlamlı ilişki bulunmadı. Postnatal 96 saatten sonra hastaneye başvuranların istatistiksel anlamlı yüsek bilirubinle (≥25 mg/dl) yattığı görüldü. On iki-otuz altı ayında değerlendirilen 65 olgunun %13,8’inde farklı alanlarda gelişimsel gecikme saptandı. Başvuru zamanı, tedavi şekilleri, total serum bilirubin ve bilirubin albümin oranıyla nörogelişimsel durum arasında anlamlı farklılık saptanmadı. Sonuç: Kan değişimi için başlıca risk faktörleri yüksek total serum bilirubin ve bilirubin albümin oranı bulundu. Hastaneye geç başvuranların daha sıklıkla ciddi hiperbilirubinemiyle yatması, taburculuk öncesi riskli grubun saptanması temel yaklaşımının önemini vurgulamaktadır. Ciddi hiperbilirubinemili olgular gelişimsel gecikmeler için riskli sayılmalı, düzenli izlem ve erken tanıyla müdahale fırsatları kaçırılmamalıdır. Introduction: Though predicting the emerging hyperbiluribinemia is the rational approach, timely exchange transfusion in significant neonatal hyperbilirubinemia is vital. The aim of this study is to evaluate the socio-demographic and clinical characteristics of newborns with significant hyperbilirubinemia, to evaluate risk factors for exchange transfusion and long-term neurodevelopmental status. Materials and Methods: Newborns who were admitted with bilirubin levels above the excahge transfusion thresholds (American Academy of Pediatrics, guideline 2004) total 104 cases were enrolled to the study. A total of 65 cases at 12-36 months were evaluated with Guide for Monitoring Child Development. The clinical and demographic characteristics, risk factors, treatment modalities, etiology of the groups and long-term neurodevelopmental status were compared between the groups with and without exchange transfusion, Results: Exchange transfusion was performed in 19 (18.3%) patients. Main factors that increase the risk of exchange transfusion were bilirubin level and bilirubin/albumin ratio. Cut-off bilirubin level and bilirubin/albumin ratio which increase the risk for exchage transfusion were 26.43mg/dl and 7.43mg/g respectively. The underlying etiologies were hemolytic disease (29.8%), dehydration and breast milk jaundice (28.8%), prematurity (26.9%). Late comers (postnatal >96 hours) were more likely to have bilirubin level ≥25 mg/dl. Development delays were detected in 13.8% of 65 cases. Conclusions: Admission bilirubin level and the ratio of bilirubin to albumin was among the factors that increase the risk of exchange transfusion. Late admission with significant hyperbiluribinemia may be prevented by identification of neonates at risk for developing significant hyperbilirubinemia before discharge or early in follow-up.

Platelets are critically involved in murine patent ductus arteriosus (PDA) closure. To date, the clinical significance of these findings in human preterm infants with PDA is still controversial. We discuss the available study data on the role of platelets for PDA closure in preterm infants: Several mostly retrospective studies have yielded conflicting results on whether thrombocytopenia contributes to failed spontaneous ductal closure. The same applies to investigations on the role of thrombocytopenia as a risk factor for unsuccessful ductus arteriosus closure by pharmacological treatment with cyclooxygenase inhibitors. Nonetheless, recent meta-analyses have concluded that thrombocytopenia constitutes an independent risk factor for both failed spontaneous and pharmacological PDA closure in preterm infants. However, the available investigations differ in regard to patient characteristics, diagnostic strategies, and treatment protocols. Several studies suggest that impaired platelet function rather than platelet number is critically involved in failure of ductus arteriosus closure in the preterm infant. A recent randomized-controlled trial on platelet transfusions in preterm infants with PDA failed to show any benefit for liberal vs. restrictive transfusion thresholds on PDA closure rates. Importantly, liberal transfusions were associated with an increased rate of intraventricular hemorrhage, and thus should be avoided. In conclusion, the available evidence suggests that thrombocytopenia and platelet dysfunction contribute to failure of spontaneous and pharmacological PDA closure in preterm infants. However, these platelet effects on PDA seem to be of only moderate clinical significance. Furthermore, platelet transfusions in thrombocytopenic preterm infants in order to facilitate PDA closure appear to cause more harm than good.

Amaç: Çalışmada, yenidoğan yoğun bakım ünitesine alt solunum yolu enfeksiyonu nedeniyle yatırılan ve viral etken tespit edilen yenidoğanların epidemiyolojik ve klinik özelliklerinin araştırılması amaçlanmıştır.Gereç ve Yöntemler: 1 Ocak 2014-31 Aralık 2015 arasında alt solunum yolu enfeksiyonu nedeniyle yatırılan yenidoğanların özellikleri ve nazofarenjiyal aspirat örneklerinde PCR yöntemiyle saptanan viral etkenler araştırıldı. Konjenital kalp hastalığı, konjenital solunum yolları anomalisi, immün yetmezliği olan hastalar ile nozokomiyal kaynaklı viral enfeksiyonlar çalışmaya dahil edilmedi.Bulgular: Çalışma döneminde yatan toplam 1768 hastanın 81’i (%4.6) akut alt solunum yolu enfeksiyonu tanısı aldı, 56’sında (%69.1) viral etken izole edildi. En sık izole edilen etken respiratuvar sinsityal virüs (RSV)’di (%80.4, n=45), dört hastada (%7.1) RSV ile birlikte ikinci virüs izole edildi, 7 hastada (%12.5) RSV dışı virüs saptandı. Yatışlar en sık Şubat ve Ocak aylarında idi. Viral etken olarak RSV saptanan hasta grubunun hastaneye yatış gün ortancası (21 gün), RSV dışı virüs saptanan hasta grubuna (41 gün) göre anlamlı düşüktü (p=0.011). RSV ve RSV dışı virüslerin etken olduğu hasta gruplarında, hastaneye başvuru bulguları, başvurudaki laboratuvar bulguları, uygulanan solunum desteği açısından fark saptanmadı. Hastanede yatış süresi ortalaması tüm hasta grubunda 7.1±3.6 gün olup, RSV ve RSV koenfeksiyon grubunda, RSV dışı virüs saptanan gruba göre anlamlı olarak uzun idi (7.3±3.8 ve 4.8±3.1 gün, p=0.03). Kaybedilen hasta olmadı.Sonuç: Yoğun bakım ünitesine alt solunum yolu enfeksiyonu nedeniyle yatırılan yenidoğanlarda en sık izole edilen etken RSV’dir. RSV enfeksiyonuna bağlı hastane yatışları en sık Ocak-Şubat aylarında olup, daha erken postnatal günlerde gerçekleşmektedir. Yatış gerektiren yenidoğanlarda epidemiyolojik ve klinik faktörlerin bilinmesi engelleyici yaklaşımların geliştirilmesine olanak sağlayacaktır., Objective: We aimed to define the clinical and epidemiological features of neonates hospitalized in the neonatal care unit with virus-related to lower respiratory tract infections in this study.Material and Methods: The neonates who were hospitalized with lower respiratory tract infection between January 2014 and December 2015 were evaluated for clinical features and the viruses were identified on multiplex reverse transcription polymerase chain reaction using nasal swab samples. Neonates with congenital heart disease, malformation of the respiratory tract, immune deficiency and nosocomial virus infection were excluded. Results: Eighty-one (4.6%) of 1768 neonates were admitted to NICU with lower respiratory tract infection during the study period. In 56 of 81 patients, a virus was identified from the nasal swab sample. Respiratory syncytial virus (RSV) was the most common etiology (80.4%, n=45), four patients had RSV co-infection (7.1%), and seven patients had a virus other than RSV (12.5%). Infants were hospitalized most frequently in January and February. The median postnatal age at admission of RSV-positive neonates was significantly lower than the neonates with causes other than RSV (21 vs. 41 days, p=0.011). The signs at admission, laboratory analyses and the respiratory support of RSV-positive neonates and neonates with causes other than RSV were similar. The mean hospitalization duration of all patients was 7.1±3.6 days. This was longer in RSV-positive neonates than neonates with causes other than RSV (7.3±3.8 and 4.8±3.1 days, p=0.03). All infants were discharged from the hospital. Conclusion: RSV was the most common viral etiology in neonates without underlying diseases who were hospitalized with a lower respiratory tract infection at the intensive care unit. We found that RSV-related lower respiratory tract infection more frequently caused hospitalization in January and February and at a lower postnatal age. It is important to know the epidemiological and clinical factors that lead to hospitalization of neonates in order to develop preventive approaches

Hypertrophic cardiomyopathy (HCM) is an inherited autosomal dominant disorder with variable penetrance. Infants of diabetic mothers, metabolic diseases, antenatal steroid exposure and perinatal exposure to stress should be considered for the differential diagnosis of HCM in neonates and infants. Asymptomatic first-degree family members of patients with HCM must be screened. Here, we aimed to discuss familial approach to HCM in a case of infant of mother with HCM., Ailesel hipertrofik kardiyomiyopati (HKM) değişken penetranslı otozomal dominant kalıtım gösteren bir hastalıktır. Yenidoğan ve infant döneminde saptanan HKM’de ayırıcı tanıda öncelikle, diabetik anne bebeği, metabolik hastalıklar, antenatal steroid kullanımı ve perinatal uzun strese maruziyet düşünülmelidir. HKM olan olguların asemptomatik birinci derece aile bireyleri mutlaka taranmalıdır. Burada, HKM’li anne bebeği sunularak ailesel HKM’ye yaklaşımın vurgulanması amaçlanmıştır.

Approximately 10% of neonates require respiratory support immediately after delivery due to transitional problems or respiratory disorders, and up to 1% of neonates are in need of resuscitation. Respiratory distress is the most frequent cause of neonatal intensive care unit (NICU) admission, and the individual management strategies should be the main task in NICUs for these infants. Regardless of the cause, if not recognized and managed in advance, respiratory distress can escalate to respiratory failure and cardiopulmonary arrest. This chapter explores the evaluation and differential diagnosis of respiratory distress in neonates and presents an update on management strategies according to the protocol of Ankara University Children’s Hospital Neonatal Intensive Care Unit.

The objective of all studies evaluating retinopathy of prematurity (ROP) screening criteria has been to minimize the number of infants screened while missing no patients with ROP who required treatment. Studies supporting the recommendation that screening for ROP could be restricted to infants born at less than 30 weeks of gestation with birth weights of less than 1250 g have been reported recently, especially from Western countries. However, with the lack of local outcome data, application of the new restricted screening guidelines to infants in middle income countries may lead to misguidance. We aimed to investigate ROP screening requirements in infants born at more than 30 weeks of gestation with birth weights above 1250 g in our unit and to compare them with different countries’ screening guidelines to assess the possible risk of restricted guidelines in our unit. Materials and methods: Retinopathy screening results were analyzed in relevance to the birth weight, gestational age, and contributing clinical factors. Infants who had a gestational age of 30 weeks or less and a birth weight less than or equal to 1250 g (Group 1) were compared to those who had a gestational age greater than 30 weeks and a birth weight greater than 1250 g (Group 2). Results: Among 226 live premature infants, 51 (22.5%) of them required laser treatment. No patient developed blindness. The majority of patients who required laser treatment (n = 27, 53%) belonged to Group 2. Conclusion: This study showed that applying developed countries’ guidelines might lead to a misdiagnosis of many patients who require treatment. Restriction criteria in retinopathy screening guidelines should be based on local population studies of the disease, not on results from other regions like high income countries.

Anne sütü, özellikle prematürelerde kazanılmış sitomegalovirüs (CMV) enfeksiyonu için majör kaynaktır ve anne sütünden kazanılan CMV enfeksiyonu seropozitif anne bebeklerinde görülmektedir. Türkiye’de annelerin çoğunluğunun seropozitif olmasına karşın prematüre bebeklerde yaşamı tehdit eden akkiz CMV enfeksiyonu yalnızca vaka sunumları şeklinde çok az olguda bildirilmektedir. Preterm semptomatik anne sütü kaynaklı CMV enfeksiyonunun tedavisi klinik bulguların ağırlığına göre yapılmalıdır. Postnatal 111. gününde menenjit-sepsis tanısı alan, anne sütünden kazanılan CMV enfeksiyonu tanımlanan ancak yaşamı tehdit eden çoklu organ yetmezliği gelişmemesi nedeniyle antiviral tedavi verilmeyen prematüre bir bebek literatür bilgileri eşliğinde sunulmuştur. Preterm bebeklerde etken saptanmayan sepsis kliniği, açıklanamayan trombositopeni, karaciğer enzim yüksekliği ve direkt bilirübinemi varlığında kazanılmış CMV enfeksiyonu akla getirilmelidir. Breast milk is a major source for acquired cytomegalovirus infection especially in premature infants and acquired CMV infection occurs in infants whose mothers were seropositive for CMV. Although most of mothers of premature infants are seropositive in Turkey, acquired life-threatening breast milk acquired CMV infection was reported occasionally. Treatment of preterm with symptomatic breast milk acquired CMV infection should be done according to the severity of clinical signs. In this report, a preterm case with a diagnosis of breast milkacquired CMV meningitis and sepsis without multiorgan failure on the 111th day of life, who did not require antiviral therapy was presented and discussed in the context of the acquired CMV literature. In preterm babies, when there is sepsis with no apparent causes, unexplained thrombocytopenia, elevated liver transaminases and direct hyperbilirubinemia acquired CMV infection should be suspected.

Antenatal ve postnatal ekokardiyografi ile tanınabilen rabdomyomlar sıklıkla tüberosklerozla ilişkilidir. Burada tüberoskleroz aile öyküsü varlığında fetal ekokardiyografi ile kardiyak rabdomyom saptanarak antenatal tüberoskleroz tanısı düşünülen bir yenidoğan olgusu sunulmaktadır., Tuberous sclerosis is an autosomal dominant disorder in which hamartomas occur in several organs. The most common heart tumor in children is rhabdomyoma. Rhabdomyoma is often associated with tuberous sclerosis and can be diagnosed by echocardiography during the antenatal or postnatal period. Herein, we suggest that fetal echocardiography should be performed in cases with a positive family history of tuberous sclerosis. Additionally, tuberous sclerosis should be kept in mind in a case with fetal rhabdomyoma

Konjenital glukoz ve galaktoz malabsorbsiyonu absorbe edilemeyen glukoz ve galaktoz nedeniyle ortaya çıkan kronik osmotik bir ishal durumudur. Burada, yenidoğan döneminde ishal nedeniyle başvuran ve konjenital glukoz ve galaktoz malabsorbsiyonu tanısı alan, takibinde tekrarlayan Candida albicans, Klebsiella pneumoniae ve Enterococcus faecalis etkenleri ile sepsis atakları geçiren fakat altta yatan hücresel ve humoral immün yetmezlik tespit edilmeyen bir olgu sunulmaktadır. Nadir görülen konjenital glukoz ve galaktoz malabsorbsiyonunda, erken tanı ve uygun tedavinin yaşamı tehdit eden komplikasyonların önüne geçebileceği, büyüme ve gelişmenin normal olabileceği bilinmektedir. Konjenital glukoz ve galaktoz malabsorbsiyonu tanısı alıp tekrarlayan fırsatçı mikroorganizmalarla enfeksiyon geçiren literatürde bizim olgumuz dışında yalnızca bir olgu daha tanımlanmaktadır. Congenital glucose-galactose malabsorption is a chronic osmotic diarrhea due to defective absorption of glucose and galactose in the intestine. Here, we present a newborn that was admitted to our hospital for neonatal diarrhea and was diagnosed as congenital glucosegalactose malabsorption. He had recurrent sepsis with Candida albicans, Klebsiella pneumoniae and Enterococcus faecalis during follow-up without having any underlying cellular or humoral immune deficiency.Early diagnosis and appropriate treatment of this rare disease can prevent life-threatening complications, and normal growth and development can be achieved. To our knowledge, the present case will be the second glucose-galactose malabsorption case with recurrent infectious due to opportunistic microorganisms after only one similar case in the literature.

Amaç: Bu çalışmada Kadın Hastalıkları ve Doğum Kliniği (KHDK)’nde doğan veya başka merkezde doğup, Yenidoğan Yoğun Bakım Ünitesi (YYBÜ)’ne yatırılan geç prematüre yenidoğanların [34(0/7) ile 36(6/7) gebelik haftasında doğma] morbiditelerinin araştırılması amaçlanmıştır. Metod: 1 Ocak-31Aralık 2007 tarihleri arasında YYBÜ’de yatırılarak tedavi alan geç prematüre bebeklerde, anne yaşı, paritesi, yardımcı üreme tekniği varlığı, gebelik haftası, doğum şekli, doğum ağırlığı, neonataldönem sorunları ve yatış nedenleri, hastanede yatış süresi ile tekrar hastaneye yatış oranları KHDK ve YYBÜ hasta kayıtları retrospektif olarak incelenerek araştırıldı. Sonuçlar: 1 Ocak-31 Aralık 2007 tarihleri arasında KHDK’nde doğan 1852 bebeğin 1556 (%84)’sı zamanında, 296 (%16)’sı prematüre doğdu. Geç prematüre doğan 174 bebek doğumların %9.4’ünü oluşturmaktaydı. Yenidoğan Yoğun Bakım Ünitesine yatış sayı ve oranları, zamanında doğan bebeklerde ve geç prematürelerde sırasıyla 155 (%10) ve 63 (%36.2) bulundu. Yatırılan toplam 80 geç prematürenin 17’si dış merkezden kabul edilmişti. Geç prematüre bebeklerin annelerinin ortalama yaşı 29.3±5.3’tü ve %37.4’ü primipar idi. Neonatal dönem sorunlarından en sık %37.5 ile indirekt hiperbilirubinemi daha sonra; solunum sıkıntısı %30, hipoglisemi %22.5 bebekte saptandı. Tartışma ve Sonuç: Zamanında doğan bebeklerle karşılaştırıldığında geç prematüre bebeklerde hastaneye yatış oranı yaklaşık 4 kat fazladır. Geç prematüre bebeklerde yatış gerektiren önemli morbiditeler indirekt hiperbilirubinemi ve solunum sıkıntısıdır. Doğum kliniklerinde bu bebekler potansiyel morbiditeleri ve YYBÜ’ne yatış olasılıkları göz ardı edilmeden yakın izlenmelidir., Objective: To investigate the morbidity pattern of late preterm infants (born between 34(0/7) and 36(6/7)) born in Obstetric and Gynecology Department or in an another hospital and admitted to Neonatal Intensive Care Unit (NICU). Methods: Hospital records of maternal age, parity, use of reproductive techniques, gestation week, birth type, birth weight, neonatal problems and reasons for hospitalization, length of stay at hospital, rehospitalization rates of late preterm infants admitted to NICU between the period of January 1 and December 31, 2007 were reviewed retrospectively.Results: At Obstetric and Gynecology Department, 1852 infants were born in the 12-month period; 1556 (84%) were full-term and 296 (16%) were preterm. One hundred seventy four infants were late preterm and late preterm delivery rate was 9.4%. NICU admission rate was 10% and 36.2% for full-term and late preterm infants, respectively. Seventeen of 80 late preterm infants admitted from another hospital. Mean maternal age was 29.3±5.3 and 37.4% of them was primiparae. Hyperbilirubinemia (37.5%), respiratory illness (30%) and hypoglycemia (22.5%) were the most common reasons for hospitalization.Discussion: Late preterm infants had 4 times the risk for hospitalization compared with full-term infants. Hyperbilirubinemia and respiratory illness are the most common morbidities. Late preterm infants who have potentially higher risk for morbidity and hospitalization require closer monitoring

Amaç: Perinatal ve neonatal bakım alanındaki gelişmeler, daha fazla sayıda prematüre veya riskli bebeğin yaşamasına olanak sağlamıştır. Özellikle prematüre bebekler, uzun hastanede yatış süreçleri ve rehospitalizasyon gereksinimleri ile yenidoğan ünitelerinin yatak kapasitelerinin çoğunu işgal etmektedir. Bu çalışmada, yenidoğan yoğun bakım ünitesine (YYBÜ) yatan hastaların gebelik haftası ve doğum ağırlıklarına göre, sağkalım oranları, ortalama hastanede yatış süreleri, rehospitalizasyon oranları ve nedenleri ile YYBÜ’nde toplam yatış gün ve kapasite kullanımı dağılımının araştırılması amaçlanmıştır. Gereç ve Yöntem: Yenidoğan yoğun bakım ünitesinde 1 Ocak 2008 ile 31 Aralık 2008 tarihleri arasında yatırılarak izlenen 544 olgu ve rehospitalize edilen 30 olgunun kayıtları geriye dönük olarak incelendi. Bulgular: Bir yıllık süreçte yatırılan hastaların ortalama hastanede yatış süresi 9.3 gün olarak, toplam sağkalım oranı %96.5, rehospitalizasyon oranı %2.9 olarak hesaplanmıştır. The aim of this study was to investigate the survival rates, LOS, and rehospitalization rates of the patients according to gestation age and birth weight, and to determine the number of bed-days and the utilization of bedscape in neonatal intensive care unit (NICU).Material and method: Hospital records of 544 infants and 30 rehospitalized infants who admitted to NICU between January 1, 2008 to December 31, 2008 were reviewed retrospectively.Results: In one-year period, the mean LOS was 9.3 days, the overall survival rate was 96.5% and the rehospitalization rate was 2.9%.
