Objectives: Anorexia is one among the most prevalent conditions triggering malnutrition, drastic weight loss and serious health issues in people especially older adults. Habb-e-Hilteet is commonly prescribed Unani pharmacopoeial formulation for anorexia. However, no scientific data is available as to its safety and efficacy. The current study assessed the appetite-inducing effect and safety of Habb-e-Hilteet in Ḍu‘f al-Ishtihā (anorexia) patients. Methods: An open prospective clinical trial was carried out in 95 clinically diagnosed anorexia patients of either gender (19-65 yrs old). The study was approved by Institutional ethic committee and conducted in accordance with the GCP guidelines. Habb-e-Hilteet, 1 pill (500mg) was administered orally in patients twice daily for 14 days. The safety of the formulation was assessed by important pathological and biochemical indices and monitoring adverse events. The efficacy was assessed on the basis of improvement in the Simplified Nutritional Appetite Questionnaire (SNAQ) score. Results: Out of 95 patients recruited, 78 completed the trial and 17 patients lost to follow-up. The mean age of patients was 29.8 yrs, with the majority (59.0%) being female. Background: Saqmunia, a resin obtained from the root of Convolvulus scammonia L. is widely used in Unani system of medicine for the treatment of several body ailments. This important medicinal plant is belonging to Convolvulaceae family, and is commonly grown in Mediterranean regions. Purpose of the review: The core objective of this appraisal is to emphasize the different characteristics of Saqmunia (Convolvulus scammonia L.) viz. botanical description, pharmacological and therapeutic properties, folklore uses, temperament, dose, scientific studies, etc, as mentioned in Unani and botanical literature that will be helpful in the rationale use of this botanical drug. Materials and methods: This review was done after going through ample of literature survey of Unani and botanical texts, and published articles related to Saqmunia (Convolvulus scammonia L.). The botanical names of drugs mentioned in this review have been validated through ‘World Flora’ (http://www.worldfloraonline.org). Results: The Unani physicians prescribe Saqmunia for the treatment of skin diseases, chronic headache, bilious fever, conjunctivitis, jaundice, etc. This drug is also added in many compound Unani formulations such as Habb-i-Kotwali, Safoof-i-Suranjan, Habb-i-Banafsha, Ayarij-i-Kabir, Tiryaq-i-Zahab, Habb-i-Saqmunia, Majun-i-Anjeer, etc which are used for various therapeutic purposes. The phytochemical analysis of the root of Convolvulus scammonia L. has been reported that it contains 8% resin along with beta-methyl-esculetin, dihydroxy cinnamic acid, ipuranol, sucrose, reducing sugar, starch, etc. The resin yields glycosides mainly scamonin which is pharmacologically active. Some scientific reports showed that this important medicinal plant possesses certain potential pharmacological activities. The significant anticancer property of aqueous and alkaline extracts of Convolvulus scammonia L. has been identified in mice. Conclusion: Although, the resin of Convolvulus scammonia L. is extensively used in Unani medicine. But, sufficient studies regarding its pharmacognosy, phytochemistry, pharmacology and toxicology are still lacking. Hence, it is suggested that extensive studies by using modern scientific parameters may be carried out to explore the every aspect of this important botanical drug. Jaundice due to hepatitis viruses is considered to be a most prevalent disease worldwide. In Unani literature, it is referred as 'yaraqān' which is caused by inflammatory diseases of liver, intra or extra-hepatic obstructions, and poisoning due to drugs or animals. Several Unani drugs including Ma'jun Dabid al-Ward, syrup Jigreen, Araq-iMako, Araq-i-Kasni, etc are frequently used for the treatment of jaundice and liver disorders including infective hepatitis. A 16 year old male patient suffering from yellowish discoloration of sclera, skin, mucous membranes and urine, nausea, vomiting, loss of appetite and fever for one week admitted in the male ward of RRIUM, Chennai. The laboratory investigations revealed severe hyperbilirubinemia, raised serum transaminases, alkaline phosphatase and negative HBsAg. On the basis of history, clinical observations and laboratory investigations, the patient was diagnosed as a case of 'yaraqān' (jaundice) which may be due to acute viral hepatitis. A Unani regimen comprising Ma'jun Dabid al-Ward (5 g twice a day), syrup Jigreen (10 ml twice a day), Araq-i-Mako (75 ml twice a day), Araq-iKasni (75 ml twice a day) for 25 days, Sharbat-i-Khaksi (10 ml twice a day for 7 days), and Habb-i-Tursh Mushtahi (1 g twice a day) for 15 days were given by oral route. The fever has subsided after 7 days and the appetite improved after 15 days of the treatment. The yellow discoloration of sclera and mucous membranes started to reduce gradually th from 7 day onwards. At the end of the treatment, the laboratory reports revealed that the total bilirubin, direct bilirubin, indirect bilirubin, SGOT, SGPT and ALP were within normal range. The core objective of this case report is to re-emphasize the effectiveness of Unani medicine particularly in the treatment of jaundice. Thus, this data may be helpful to increase awareness amongst the common people and scientific community for the popularity of Unani medicine in general and for the treatment of jaundice and liver disorders in particular.

Background: Zarawand Mudharaj (Aristolochia rotunda L.) belongs to family Aristolochiaceae. According to Unani scholars, it is a female plant of Aristolochia fontanesii Boiss. & Reut. (Syn. A. longa L.), usually found growing in Mediterranean regions, Central Asia and South Europe. Purpose of the review: The main aim of this review is to highlight various aspects of Zarawand Mudharaj such as temperament, botanical description, pharmacological properties, therapeutic uses etc, as mentioned in Unani literature that will ultimately guide researchers to design various studies for further exploration of this important medicinal plant. Materials and methods: The present review has been carried out through extensive literature survey of various classical Unani and botanical texts, and published papers available on different search engines. The botanical names and their synonyms were validated through ‘The Plant List’ (www.theplantlist.org). Results: In Unani medicine, the rhizome of Zarawand Mudharaj is used for the treatment of several ailments viz. stomach and liver diseases, jaundice, cough, septic wounds, splenomegaly, gout etc. Apart from its use as single drug, it has also been added in many compound preparations viz. Anqarooya-i-Kabir, Habb-i-Rewand, Majun Falasifa, Majun Dabeedul Ward etc. A study on physicochemical standardization of rhizome of Aristolochia rotunda L. has reported the presence of moisture content, total ash, acid insoluble ash, water soluble ash, sulphated ash, alcohol and water-soluble extractive values within normal limits. The same study has also revealed that the hydro-alcoholic extract of rhizome of Aristolochia rotunda L. possesses significant hepatoprotective activity against CCl4 induced liver toxicity in albino Wistar rats. Several toxicity reports have pointed out that aristolochic acid containing herbal drugs including Aristolochia rotunda L. cause renal impairment and other serious adverse reactions. Conclusion: It is concluded that Aristolochia rotunda L. is widely used in Unani system of medicine. However, extensive studies on pharmacognosy, pharmacology, toxicology, phytochemistry and quality control of different parts of this potential medicinal plant have not been carried out so far. Therefore, such studies on modern scientific parameters may be conducted for further exploration so that the benefits may reach the masses. Objectives To validate the concept of abadāl-i-adwiya (drug substitution) by evaluation of physicochemical standardization and hepatoprotective activity of Aristolochia rotunda & its substitute, Curcuma Zedoaria in albino Wistar rats. Methods Physicochemical standardization by estimation of moisture content, ash values and extractive values were carried out using standard methods. Hepatotoxicity was induced in albino Wistar rats using CCl4 1 mL/kg s. c. on alternate day for 14 days. Group I was served as Plain control and Group II as Negative control. Group III was administered silymarin 50 mg/kg p. o. while Group IV received HAE of A. rotunda 89.64 mg/kg p. o., and Group V was administered HAE of C. Zedoaria 45.73 mg/kg p. o. At the end of the study, serum bilirubin, AST (SGOT), ALT (SGPT) and ALP were estimated. The histopathology of liver was also carried out. Results The physicochemical parameters of both test drugs viz. moisture content, total ash, acid insoluble ash and water soluble ash were found within normal limit. The total serum bilirubin, direct bilirubin, AST (SGOT), ALT (SGPT) levels were significantly decreased in Test groups A and B when compared to the Negative and Standard controls. The microscopic examination of liver collected from animals of Group IV and Group V revealed significant recovery from hepatic toxicity compared to the Negative control. Conclusions The study experimentation has revealed that C. Zedoaria may be used as a substitute for A. rotunda in the treatment of liver diseases. However, the outcome has to be further corroborated with phytochemical evaluation and clinical trials of both the drugs. Furthermore, the concept of drug substitute in Unani system of medicine is also validated in the light of above study.

Since antiquity, the Unani system of medicine has been participating in health care system. Usually, four modes of treatment viz. regimenal therapy, dietotherapy, pharmacotherapy and surgery are applied for the treatment of diseases. Regimenal therapy is an important mode in which the morbid matter present in the body is either dispersed/excreted or its unnecessary production is blocked or its flow is restricted and the diseases are cured by natural healer of the body, consequently bring back the humoural stability. Nearly 30 regimens have been mentioned in classical Unani literature. Commonest regimenal procedures such asfasd(venesection/phlebotomy),hijāma(cupping),ta‘līq al-‘alaq(hirudotherapy/leech therapy),ishāl(purgation),qay’(emesis),idrār-i-bawl(diuresis),huqna(enema),ta’rīq(diaphoresis),riyādat(exercise),dalk(massage),hammām(bathing),tadhīn(oiling),natūl(irrigation),sakūb(douching/spraying),inkibāb(steam/vapour application),takmīd(fomentation) etc. are usually applied for the management of various ailments. These regimenal procedures are completely based on holistic approach and are potential but needs to be explored scientifically. This review outlines the therapeutic applications of various regimens of regimenal therapy used in Unani medicine.

Background Diabetes mellitus (DM) is a decisive risk factor for severe illness in coronavirus disease 2019 (COVID-19). India is home to a large number of people with DM, and many of them were infected with COVID-19. It is critical to understand the impact of DM on mortality and other clinical outcomes of COVID-19 infection from this region. Aims The primary objective of our study was to analyze the mortality rate in people with DM infected with COVID-19. The secondary objectives were to assess the effect of various comorbidities on mortality and study the impact of DM on other clinical outcomes. Methods This is a retrospective study of COVID-19 infected patients admitted to a tertiary care hospital in north India in the early phase of the pandemic. Results Of the 1211 cases admitted, 19 were excluded because of incomplete data, and 1192 cases were finally considered for analysis. DM constituted 26.8% of total patients. The overall mortality rate was 6.1%, and the rate was 10.7% in the presence of diabetes (p &lt, 0.01, OR 2.55). In univariate analysis, increased age, chronic kidney disease (CKD), coronary artery disease (CAD), stroke, and cancer were associated with mortality. On multiple logistic regression, the independent predictors of mortality were CAD, CKD, and cancer. Breathlessness and low SpO2 at presentation, extensive involvement in CXR, and elevated ANC/ALC ratio were also significantly associated with mortality. Conclusions The presence of comorbidities such as DM, hypertension, CAD, CKD, and cancer strongly predict the risk of mortality in COVID-19 infection. Early triaging and aggressive therapy of patients with these comorbidities can optimize clinical outcomes.

Migraine is a type of headache characterized by episodic throbbing or pulsating pain usually occurring in one side of the head. According to the prevalence report, it is the third most prevalent disorder worldwide. In Unani Medicine, it is called shaqiqa derived from an Arabic word ‘shaq’ meaning ‘half part’. The condition is difficult to treat by medication only. Unani system of medicine has a theory of imala (diversion of morbid matters) and tanqia-e-mawad (evacuation of morbid matters) by using different modes of ilaj bit tadbeer (regimenal therapy) and pharmacotherapy. A therapy including regimenal therapy and pharmacotherapy has been used in this case for 23 days. Nutool (irrigation therapy) and Dalak (massage therapy) by Rogan-e-Kaddu (medicated oil) were given as regimenal mode and Itrifal Ustukhuddus, Itrifal Kishnizi and Jawarish Shahi were given orally as pharmacotherapy. Signs and symptoms were assessed by Visual Analogue Scale (VAS) and result was assessed as per grading system, such as complete relief, relief, partial relief and no relief. After completion of the treatment and post treatment follow up, complete relief from the sign and symptoms was observed. The study suggested the authenticity of principles of treatment of Unani Medicine.

BACKGROUND AND OBJECTIVE: Tiryaq-i-Afayee, a polyherbal formulation of Unani medicine used in the prevention and treatment of various wabāi amrād (epidemic/ pandemic diseases) since olden days. This review study has been carried out to explore the therapeutic efficacy of this formulation in the light of scientific studies. MATERIALS & METHODS: A total of 74 literature comprising classical Unani texts from the period of 9th – 20th century AD, and published research articles in various reputed journals from 1999 – 2020 AD were reviewed regarding immunomodulatory, antiviral, antiinfluenza, antioxidant, antitussive, antimicrobial effects of Tiryaq-iAfayee, as a whole preparation and its individual ingredients. RESULTS:Tiryaq-i-Afayee contains Sibr Zard (Aloe vera) (2 parts), Murr Makki (Commiphora myrrha) (1 part) and Zafran (Crocus sativus) (1 part). It is prepared in the form of pill, and 2 g is used in alternate day for prevention and management of epidemic diseases. All the three ingredients of this compound formulation belong to plants and individually used for various medicinal purposes. DISCUSSION: A study revealed that Tiryaq-i-Afayee possesses significant immunomodulatory effect in a small group of immuno compromised persons. An ethanol extract of Aloe vera exhibited antiinfluenza effect against influenza A virus due to presence of aloe-emodine (anthraquinone glycoside). Certain studies have reported that Commiphora myrrha possesses significant antioxidant and antimicrobial effects against Staphylococcus aureus and Pseudomonas aeruginosa. Another study has revealed that Saffron extract exhibited promising antiviral activity due to presence of crocin and picrocrocin. CONCLUSION: The aforementioned classical and modern conventional materials have given plentiful information regarding use of Tiryaq-i-Afayee in the prevention of epidemic/ pandemic diseases.

The Unani system of medicine is one of the traditional systems of medicine practised since centuries in many parts of the world including India. At a time when the coronavirus disease 2019 (COVID-19) pandemic is still raging across the globe and there is still no appreciable satisfactory management available with vaccination being the only panacea in the near future, unani medicine chiefly composed of herbal drugs is replete with many classical references for the management of influenza like illness and COVID-19 like epidemics. In Unani medicine, nazla-i-wabāi is referred as influenza for which a decoction containing Behidana (Cydonia oblonga), Unnab (Ziziphus jujuba) and Sapistan (Cordia myxa) are recommended to relieve the clinical features of nazla-i-wabāi and other COVID-19 like epidemics. All the three ingredients of this formulation are also individually used for the treatment of sore throat, cough, septicaemia, fever, dyspnoea, pharyngitis, chest pain etc. Certain scientific studies have validated various pharmacological actions of these drugs as claimed by unani physicians. A concerted rational approach has been attempted to highlight the effect of the unani decoction as to its immune boosting property both as prophylactic and therapeutic use in the treatment of influenza like illness and COVID-19 like epidemics in the light of ancient Unani classics and recent scientific studies.
