Abstract

Interindividual differences in metabolism of the soy isoflavone, daidzein, to equol and O-desmethylangolensin (ODMA) by human gut bacteria, have been associated with altered risk of cancer and other chronic diseases, according to some studies. Differences have been reported in the prevalence of the equol-producer phenotype among populations, with a higher prevalence in soy-consuming Asian populations than in Western populations. To date, prevalence of the daidzein-metabolizing phenotypes in Asians, compared with Caucasians, has not been evaluated in the context of a standardized phenotyping method. We assessed the prevalence of equol- and ODMA-producer phenotypes in 91 Korean American (KA) women and girls living in the Seattle, Washington area and compared this with previous similarly collected prevalence data in Caucasian American (CA) women and girls (n 222). We also compared the dietary habits of the 2 groups. Isoflavonoid concentrations in first-void morning urines, collected after a 3-d soy challenge, were used to establish equol-, and ODMA-producer phenotypes (>44 [micro]g/L). The prevalence of the equol-producer phenotype was higher (51 vs. 36%; P = 0.015) and the ODMA-producer phenotype was lower (84 vs. 92%, P = 0.03) in KA than in CA women and girls. KAs consumed approximately 3 times more soy foods than the CAs, but no significant associations were found between the consumption of soy foods and equol-producer phenotype. Our findings support the reports that, compared with Western populations, Asian populations have a higher equol-producer prevalence. The additional observation that the prevalence of the ODMA-producer phenotype is lower in KAs suggests that daidzein-metabolizing patterns in general may differ between KAs and CAs. Abstract

Data suggest that soy protein, a source of isoflavones, may have favorable effects on cardiovascular risk factors. Women (n = 205), ages 49-65 y, were randomized into this double blind, placebo-controlled trial of 43.5 mg red clover-derived isoflavones/d. A total of 177 women completed the trial. There were no differences between treatments for changes from baseline to 12 mo in total cholesterol, LDL cholesterol, triglycerides, HDL cholesterol, systolic and diastolic blood pressures, fibrinogen, and plasminogen activator inhibitor type 1 (PAI-1) (P [greater than or equal to] 0.1). Interactions between treatment and menopausal status were significant for changes in triglycerides and PAl-1 (P = 0.02 and P = 0.01), and changes were significant among perimenopausal women. In the isoflavone and placebo groups, changes in triglycerides were -0.2 [+ or -] 0.6 and 0.4 [+ or -] 0.6 mmol/L, P = 0.02, and changes in PAl-1 were -3.06 [+ or -] 5.88 and 4.95 + 6.25 IU/L, P = 0.004, respectively. Interactions between apolipoprotein E (apoE) genotype and treatment tended to be significant for changes in total and LDL cholesterol (P = 0.06 and P = 0.05), and differences between treatments were significant in E2/E3 women. In the isoflavone and placebo groups, changes in total cholesterol were -0.61 [+ or -] 0.79 and 0. 18 [+ or -] 0.79 retool/L, P = 0.03, and changes in LDL cholesterol were -0.84 [+ or -] 0.79 and -0.04 [+ or -] 0.69 mmol/L, P = 0.02, respectively. Although there were potentially beneficial changes in triglycerides and PAI-1 among perimenopausal women consuming isoflavones, this study suggests that isoflavones alone are not responsible for the well-documented effects of soy protein on blood lipids. Abstract

Daidzein can be metabolized to equol, dihydrodaidzein (DHD), and O-desmethylangolensin (ODMA) by intestinal bacteria. Only one third to one half of individuals produce equol, and evidence exists to support potential cancer-protective effects of equol production. We investigated the in vitro metabolism of daidzein by fecal bacteria and assessed the effect of several antibiotics on metabolism. Fresh or previously frozen feces from 7 equol producers and 6 nonproducers were incubated with daidzein, with or without antibiotics, for 5 d at 37[degrees]C. With the exception of one previously frozen sample, fecal inoculates from equol producers converted daidzein to equol. Conversion occurred under anaerobic, but not aerobic conditions. Fecal inoculates from equol nonproducers did not produce equol, but some produced ODMA and DHD. Between-subject differences in the effects of antibiotics on daidzein metabolism were apparent. Some antibiotics inhibited the production of equol but had no effect on DHD production. These results suggest that several bacteria may be involved in daidzein metabolism, and that they may differ among subjects. This simple in vitro system can facilitate the study of factors influencing equol production and minimize the need for animal models or human interventions. Furthermore, these analyses can be conducted on fecal samples that have been frozen and stored. KEY WORDS: * antibiotic * bacteria * equol * isoflavone * soy
