Fragestellung: Nur wenige Kliniken für Kinder- und Jugendpsychiatrie (KJP) bieten eine ambulante Behandlung von Jugendlichen mit Substanzkonsumstörungen (SUDs) an. Daher fehlen Angaben, welche Konsummuster, SUDs und begleitenden psychischen Störungen diese Jugendlichen aufweisen. Methodik:N = 201 Patient_innen einer universitären Spezialambulanz (12–19 Jahre alt) wurden bezüglich Konsummustern, SUDs und aktuellen psychischen Störungen mittels Interview untersucht. Es wurden diesbezüglich deskriptive Darstellungen zu Prävalenzzahlen der SUDs, Konsummustern und begleitenden psychischen Störungen aufgeteilt nach Geschlecht und aktuellem Alter erstellt. Ergebnisse: Tabak (88 %) und Cannabis (86 %) waren die verbreitetsten Substanzen. Bei 67 % aller Patient_innen wurde mehr als eine SUD festgestellt. SUDs bezogen sich am häufigsten auf Cannabis (84 %), gefolgt von Tabak (77 %). 72 % aller Patient_innen zeigten eine die SUD begleitende psychische Störung, insbesondere Störungen des Sozialverhaltens (40 %), hyperkinetische Störungen (21 %) und depressive Störungen (18 %). Schlussfolgerungen: Ambulant behandelte jugendliche KJP-Patient_innen mit SUDs präsentieren sich häufig mit begleitenden psychischen Störungen. Angebotene Behandlungsprogramme sollten in der Behandlung von SUDs insbesondere Störungen des Sozialverhaltens, Depressionen und hyperkinetische Störungen berücksichtigen., Objective: Only few clinics offer the outpatient treatment of substance use disorders (SUDs) among adolescents. Therefore, only limited data describe substance use patterns, SUDs, and co-occurring psychiatric disorders characteristic of adolescents who present in such outpatient clinics specialized in the treatment of SUDs. Method: Via interview we collected data from n = 201 patients between 12 and 19 years concerning their substance use, SUDs, and current co-occurring psychiatric disorders. We created descriptive presentation of data regarding use patterns, SUDs, and co-occurring disorders divided by sex and current age. Results: Tobacco (88 %) and cannabis (86 %) were the most frequently used substances. 67 % of all patients presented with more than one SUD, cannabis use disorder being the most prevalent one (84 %). 72 % presented with at least one co-occurring disorder, with conduct disorders (40 %), attention deficit (hyperactivity) disorders (21 %), and depressive disorders (18 %) being the most frequent ones. Conclusions: Adolescent SUD patients often present with co-occurring psychiatric disorders. Institutions for adolescent SUD treatment should also focus on treating co-occurring conduct disorders, depression, and attention deficit disorders.

Background: Older individuals are not only more sensitive to the impact of alcohol but also face significant health risks from alcohol-drug interactions. To alter current drinking patterns, it is crucial to understand the motives for abstention of adults in late middle age. Objectives: We hypothesized that, for adults in late middle age, socio-demographic characteristics and health-related factors predict alcohol abstinence; and that current motives for abstention vary between subgroups of abstainers. Method: Data on adults aged 50-64 years (n = 2,308) came from the German Epidemiological Survey of Substance Abuse (ESA). Logistic regression was used to examine the association between different types of abstinence and socio-demographic and health-related variables. Results: Low income, low education and poor self-rated physical health predicted 12-month abstinence. Men with a chronic disease had a 9.5 % chance to be abstinent, whilst it was 17.7 % for women. Main motives for older lifetime abstainers were 'dislike of taste or smell', 'loss of control' and 'family constraints'. For 12-month abstainers, it was 'loss of control', 'health constraints' and 'dislike of taste or smell'. Conclusion: Poor health in middle-aged drinkers offers an opportunity to recommend reduction or cessation of alcohol use by explaining the negative health effects from alcohol. Future research investigating abstention needs to differentiate between lifetime and 12-month abstainers.

Background: Not much is known about the correlation between gaming problems and substance use across different countries. This paper presents cross-national analyses of different gaming indicators and their relationship to substance use. Methods: Based on data from the 2015 ESPAD study, differences in the relationship between gaming and substance use across 35 countries were analysed using multi-level logistic regression, using substance use as an individual level predictor, economic wealth as a country-level predictor and a combined problem gaming indicator as the outcome. Results: Multi-level logistic regressions revealed significant correlations between individual substance use and gaming problems, which varied across countries and were moderated by economic wealth. Students who used alcohol, tobacco or cannabis and who lived in high-income countries had a smaller risk of scoring positively on a combined problem gaming indicator than students who used alcohol, tobacco or cannabis and who lived in less prosperous countries. Discussion: Different gaming indicators varied substantially across countries, with self-perceived gaming problems being more common in countries with a low prevalence of gaming. Significant cross-level effects demonstrate the need to take the societal context into account when the relationship between problem gaming and substance use is analysed. Prevention measures need to take the fact into account that patterns of substance use among problem gamers vary across countries.

Hintergrund Schätzungen zum E-Zigarettenkonsum in Deutschland variieren erheblich. Die Nutzung von E-Zigaretten zur Tabakentwöhnung wird kritisch diskutiert. Anhand aktueller Daten soll die Verbreitung des Konsums von E-Zigaretten und deren Nutzung in der erwachsenen Allgemeinbevölkerung Deutschlands dargestellt werden. Methoden Als Datenbasis diente der Epidemiologische Suchtsurvey 2015, eine bundesweite Befragung von 18- bis 64-jährigen in Deutschland (n=9,204; Rücklaufquote: 52,2%). Ergebnisse E-Zigaretten waren einem Großteil der Befragten bekannt (85,3%, 43,5 Mio.), wohingegen lediglich 2,9% (1,5 Mio.) E-Zigaretten in den letzten 30 Tagen genutzt hatten. Jüngere Personen (OR=0,95, 95%-KI=(0,93; 0,97)), Männer (OR=1,45, 95%-KI=(1,02; 2,07)) und Raucher (OR=12,53, 95%-KI=(8,71; 18,03)) zeigten ein signifikant höheres Risiko, E-Zigaretten zu konsumieren. Etwa ein Drittel (36,6%) der Raucher und Ex-Raucher konventioneller Zigaretten nutzte E-Zigaretten zur Tabakentwöhnung, wovon ein Fünftel (21,3%) mit dem Rauchen aufhören konnte. Schlussfolgerungen E-Zigarettennutzer scheinen häufiger männlich, jünger und Raucher konventioneller Zigaretten zu sein. Neben Neugierde ist die Veränderung des Rauchverhaltens ein bedeutendes Konsummotiv. Based on current data, the distribution of the consumption of e-cigarettes and their use in the adult general population of Germany will be presented. Methods The 2015 Epidemiological Survey of Substance Abuse, a nationwide survey of 18 to 64 year-old people in Germany (n=9,204, response rate: 52,2%), was used as data basis. Results E-cigarettes were known to most of the respondents (85,3%, 43,5 Mio.), whereas only 2,9% (1,5 Mio.) used e-cigarettes in the last 30 days. Higher risk of consuming e-cigarettes was seen in younger people (OR=0,95, 95%-KI=(0,93; 0,97)), men (OR=1,45, 95%-KI=(1,02; 2,07)) and smokers (OR=12,53, 95%-KI=(8,71; 18,03)). About a third of smokers and ex-smokers of conventional cigarettes (36,6%) who consumed e-cigarettes used these for tobacco cessation of which one fifth (21,3%) was able to quit smoking. Conclusion E-cigarette users seem to be more likely to be male, younger and smokers of conventional cigarettes. In addition to curiosity, the change in smoking behavior is an important motive for consumption. The results indicate that the use of e-cigarettes can contribute to tobacco cessation, the majority of users, however, continue to consume conventional and/or e-cigarettes.

BackgroundThe perception that e-cigarettes are less harmful than traditional tobacco products can influence the consumption of e-cigarettes.ObjectivesThree questions were examined: (1)How do different population groups perceive health risks of e-cigarettes? (2)Do sociodemographic variables explain differences in the risk assessment of e-cigarettes? (3)Does the perception of health risks predict the use of e-cigarettes for smoking cessation?MethodsData came from the 2015 Epidemiological Survey of Substance Abuse (ESA) with asample size of n=9204 participants, aged 18 to 64years (response rate 52.2%). Data were collected by telephone, online, or by written questionnaires. Assessments of risk perception of e-cigarettes and conventional cigarettes (more harmful, just as harmful, less harmful, do not know) were compared. Descriptive statistics and logistic regressions were performed.ResultsIndividuals with lower education rated e-cigarettes as more harmful. Older people and women perceived e-cigarettes as just as harmful. Smokers considered e-cigarettes to be more harmful than or just as harmful as conventional tobacco products. The likelihood of using e-cigarettes for smoking cessation was higher if people thought they were less harmful than conventional cigarettes.ConclusionsOnly one-third of the population knows that e-cigarettes are less harmful to health than conventional cigarettes. The perception of health risks is related to the usage of e-cigarettes for smoking cessation.

Ziel: Ziel der Studie ist die Analyse von Trends des (klinisch relevanten) Konsums von Tabak, Alkohol, Cannabis und anderen illegalen Drogen sowie der Einnahme von Medikamenten zwischen 1995 und 2015. Methodik: Datengrundlage bilden acht Erhebungswellen des Epidemiologischen Suchtsurveys (ESA) aus den Jahren 1995 bis 2015. Die Befragungen erfolgten schriftlich, telefonisch oder online. Eine einheitliche Darstellung zeitlicher Trends ist für den Altersbereich 18 bis 59 Jahre möglich. Ergebnisse: Der Tabakkonsum hat in den letzten 20 Jahren abgenommen. Trenddaten des Alkoholkonsums zeigen sinkende Konsumwerte für Männer und stabile Werte für Frauen. Die Prävalenz des Rauschtrinkens ist bei Frauen seit dem Jahr 2009 angestiegen. Der Cannabiskonsum hat zwischen 2012 und 2015 bei beiden Geschlechtern zugenommen. Der wöchentliche Gebrauch von Schmerzmitteln ist angestiegen, während bei Schlaf-/Beruhigungsmitteln ein Rückgang zu verzeichnen ist. Indikatoren eines klinisch relevanten Konsums zeigen für Cannabis keine Änderungen, für Tabak einen Rückgang und für Medikamente einen Anstieg. Hinweise auf klinisch relevanten Alkoholkonsum sind bei Männern konstant geblieben und bei Frauen angestiegen. Schlussfolgerungen: Die weite Verbreitung insbesondere der legalen Substanzen legt einen dringenden Handlungsbedarf für Prävention und Intervention nahe. Als besondere Zielgruppe wurden Frauen identifiziert, bei denen der klinisch relevante Konsum von Alkohol und Medikamenten zugenommen hat., Aims: The study analyzes trends in the (clinically relevant) use of tobacco, alcohol, cannabis, other illegal drugs as well as pharmaceuticals. Methods: Data from eight waves of the Epidemiological Survey of Substance Abuse (ESA) from the years 1995 to 2015 were used. The data were collected using self-administered questionnaires, telephone interviews, or online questionnaires. A consistent analysis of trends is possible for the age range 18 to 59 years. Results: Tobacco consumption significantly declined over the past 20 years. Alcohol data showed declining consumption in males and stable consumption in females. The prevalence of episodic heavy drinking increased in females since the year 2009. Compared to the year 2012, cannabis consumption increased in both sexes. Weekly use of analgesics also increased, whereas weekly use of sedatives/hypnotics decreased. Indicators of clinically relevant use remained constant for cannabis, declined for tobacco and increased for pharmaceuticals. With regard to alcohol, males showed stable and females increased rates of clinically relevant use. Conclusions: Prevention and intervention measures need to be intensified especially with regard to the legal substances tobacco and alcohol. Trend analyses identified women as a particularly vulnerable subgroup because of an increasing prevalence of clinically relevant use of alcohol and pharmaceuticals.

Ziel: Die Verbreitung des Substanzkonsums in der erwachsenen Allgemeinbevölkerung Deutschlands soll anhand aktueller Daten dargestellt werden. Methodik: Datengrundlage bildete der Epidemiologische Suchtsurvey 2015, eine bundesweite Befragung in der Wohnbevölkerung von 18 bis 64 Jahren (n = 9,204; Rücklaufquote: 52.2 %). Probanden wurden zum Gebrauch von Tabak, Alkohol, illegalen Drogen und Medikamenten befragt. Ergebnisse: In den letzten 30 Tagen hatten 28.7 % der Befragten Tabakprodukte genutzt. Den Konsum von Alkohol in diesem Zeitraum berichteten 72.8 %. Hinweise auf klinisch relevanten Konsum in den letzten 12 Monaten wiesen 28.3 % (Männer) bzw. 9.6 % (Frauen) auf. Die 12-Monats-Prävalenz illegaler Drogen lag bei 6.1 % für Cannabis und bei maximal 1 % für alle anderen Substanzen. Unter Medikamenten hatten Schmerzmittel die höchste Gebrauchsprävalenz (47.1 %) sowie die geringste Prävalenz täglichen Gebrauchs unter Nutzern (8.6 %). Schlussfolgerungen: Die Ergebnisse belegen die nach wie vor hohe Verbreitung des Substanzkonsums in der Allgemeinbevölkerung. Konsumprävalenz sowie Hinweise auf klinisch relevanten Konsum sind in Bezug auf legale Substanzen, die eine hohe Verfügbarkeit charakterisiert, am häufigsten. Method: Data come from the 2015 Epidemiological Survey of Substance Abuse, a national survey conducted among the resident population aged 18 to 64 years. With a response rate of 52.2 %, the sample size was n = 9,204. Participants were asked about their use of alcohol, tobacco, pharmaceuticals, and illicit drugs. Results: Of all respondents, 28.7 % had used tobacco products in the past 30 days, and 72.8 % had drunk alcohol. Indications of clinically relevant use within 12 months prior to the survey were found for 28.3 % of men and 9.6 % of women. Regarding illicit drugs, the 12-month prevalence was 6.1 % for cannabis and at most 1 % for all other substances. Analgesics were the class of pharmaceuticals with the highest prevalence of use (47.1 %), but the lowest prevalence of daily use among consumers of a given pharmaceutical (8.6 %). Conclusions: Results demonstrate that the use of psychoactive substances is still highly prevalent in the general population. Prevalence of use as well as indications for clinically problematic use were highest for licit substances that are highly available. Ziel: Der vorliegende Beitrag beschreibt die Methodik und das Studiendesign des Epidemiologischen Suchtsurveys (ESA) 2015. Methodik: Zielpersonen des ESA waren deutschsprachige und in Privathaushalten lebende Personen im Alter zwischen 18 und 64 Jahren. Die Ziehung der Personenstichprobe erfolgte auf Basis der Einwohnermelderegister disproportional zur Verteilung der Geburtsjahrgänge in einem zweistufigen Zufallsverfahren. Die Befragungen wurden schriftlich, telefonisch und online durchgeführt. Ergebnisse: Insgesamt konnte im ESA 2015 mit n = 9204 Teilnehmern eine Nettoausschöpfung von 52.2 % erreicht werden. Durch die verwendete Gewichtung konnte die Stichprobe hinsichtlich Bundesland, BIK-Gemeindegrößenklasse, Geschlecht, Jahrgangsgruppe und Schulbildung an die Verteilung in der Grundgesamtheit angepasst werden. Mit der Studie nicht erreichte Personen zeigten problematischere Konsummuster als Teilnehmer der Befragung. Schriftlich befragte Personen gaben häufiger Substanzkonsum an als telefonisch und online Befragte. Methods: The sampling frame of the ESA included German-speaking individuals living in private households aged 18 to 64 years. A sample was drawn from population registers using a two-stage probability design oversampling younger birth cohorts. The survey was realized using paper-and-pencil questionnaires, telephone interviews, and online questionnaires. Results: A total of 9204 individuals participated in the survey, resulting in a response rate of 52.2 %. Using redressement weights, the sample could be adjusted to the distribution of the general population according to federal state, size of the community, sex, year of birth, and school education. Survey nonrespondents showed more problematic consumption patterns than respondents. Individuals in the paper-and-pencil mode reported higher rates of substance use compared to those in the telephone and online mode.
