Objectives The aim of this study was to conduct the translation and cross-cultural adaptation of the original Team Emergency Assessment Measure (TEAM) tool into the Brazilian Portuguese language and investigate the internal consistency, inter-rater reliability, and concurrent validity of this new version (bp-TEAM). Methods Independent medical translators performed forward and backward translations of the TEAM tool between English and Portuguese, creating the bp-TEAM. The authors selected 23 videos from final-year medical students during in-situ emergency simulations. Three independent raters assessed all the videos using the bp-TEAM and provided a score for each of the 12 items of the tool. The authors assessed the internal consistency and the inter-rater reliability of the tool. Results Raters assessed all 23 videos. Internal consistency was assessed among the 11 items of the bp-TEAM from one rater, yielding a Cronbach's alpha of 0.89. inter-item correlation analysis yielded a mean correlation coefficient rho of 0.46. Inter-rater reliability analysis among the three raters yielded an intraclass correlation coefficient of 0.86 (95% CI 0.83‒0.89), p < 0.001. Conclusion The Brazilian Portuguese version of the TEAM tool presented acceptable psychometric properties, similar to the original English version.

As doencas cardiovasculares estao entre as maiores causas de morbidade e mortalidade em todo o mundo e sao responsaveis por um elevado custo para os sistemas de saude. Assim, as medidas de prevencao dessas doencas e o controle de seus fatores de risco tornam-se essenciais. Para isso, temos como alternativa, intervencoes educacionais para a populacao, como maneira de fortalecer o individuo para realizar as mudancas necessarias em seu estilo de vida, e medidas de educacao profissional, para difundir o manejo de emergencias cardiovas-culares, com grande impacto na sobrevida de individuos com esses problemas. Neste texto, procuramos descrever as medidas educacionais mais frequentes e efetivas para essa prevencao.

Objectives To investigate acute stress response in residents playing nurse and physician roles during emergency simulations. Methods Sixteen second-year internal medicine residents participated in teams of four (two playing physician roles and two playing nurse roles). Stress markers were assessed in 24 simulations at baseline (T1) and immediately after the scenario (T2), using heart rate, systolic and diastolic blood pressure, salivary α-amylase, salivary cortisol and salivary interleukin-1β. The State-Trait Anxiety Inventory was applied at T2. Continuous data were summarized for the median (1st-3rd interquartile ranges), and the Mann-Whitney U Test was used to compare the groups. Results The percent variations of the stress markers in the physician and nurse roles, respectively, were the following: heart rate: 70.5% (46.0-136.5) versus 53.0% (29.5-117.0), U=89.00, p=0.35; systolic blood pressure: 3.0% (0.0-10.0) versus 2.0% (-2.0-9.0), U=59.50, p=0.46; diastolic blood pressure: 5.5% (0.0-13.5) versus 0.0% (0.0-11.5), U=91.50, p=0.27; α-amylase: -5.35% (-62.70-73.90) versus 42.3% (12.4-133.8), U=23.00, p=0.08; cortisol: 35.3% (22.2-83.5) versus 42.3% (12.4-133.8), U=64.00, p=0.08); and interleukin-1β: 54.4% (21.9-109.3) versus 112.55% (29.7-263.3), U= 24.00, p=0.277. For the physician and nurse roles, respectively, the average heart rate was 101.5 (92.0-104.0) versus 91.0 (83.0-99.5) beats per minute, U=96.50, p=0.160; and the state anxiety inventory score was 44.0 (40.0-50.0) versus 42.0 (37.50-48.0) points, U= 89.50, p=0.319. Conclusions Different roles during emergency simulations evoked similar participants’ engagement, as indicated by acute stress levels. Role-play strategies can provide high psychological fidelity for simulation-based training, and these results reinforce the potential of role-play methodologies in medical education.

Como qualquer outra medicação, os meios de contraste não estão isentos de riscos, podendo ocasionar reações adversas após o seu uso. Felizmente, as reações adversas relacionadas aos meios de contraste não são comuns e, quando ocorrem, são geralmente leves. Conhecer as possíveis reações relacionadas aos meios de contraste é fundamental para que haja um planejamento, profilaxia e atendimento adequados a cada paciente. Neste capítulo, abordaremos as reações adversas não renais relacionadas aos meios de contraste iodado e à base de gadolínio, inclunindo suas classificações, fisiopatologia, fatores de risco, manifestações clínicas e tratamento.

Purpose Medical simulation is fast becoming a standard of health care training throughout undergraduate, postgraduate and continuing medical education. Our aim was to evaluate if simulated scenarios have a high psychological fidelity and induce stress levels similarly to real emergency medical situations. Materials and Methods Medical residents had their stress levels measured during emergency care (real-life and simulation) in baseline (T1) and immediately post-emergencies (T2). Parameters measuring acute stress were: heart rate, systolic and diastolic blood pressure, salivary α -amylase, salivary interleukin-1 β , and State-Trait Anxiety Inventory score. Results Twenty-eight internal medicine residents participated in 32 emergency situations (16 real-life and 16 simulated emergencies). In the real-life group, all parameters increased significantly ( P β increased significantly after emergencies. The comparison between groups demonstrates that acute stress response (T2 – T1) and State-Trait Anxiety Inventory score (in T2) did not differ between groups. Conclusions Acute stress response did not differ between both groups. Our results indicate that emergency medicine simulation may create a high psychological fidelity environment similarly to what is observed in a real emergency room.

Providing care for simulated emergency patients may induce considerable acute stress in physicians. However, the acute stress provoked in a real-life emergency room (ER) is not well known. Our aim was to assess acute stress responses in residents during real emergency care and investigate the related personal and situational factors. A cross-sectional observational study was carried out at an emergency department of a tertiary teaching hospital. All second-year internal medicine residents were invited to voluntarily participate in this study. Acute stress markers were assessed at baseline (T1), before residents started their ER shift, and immediately after an emergency situation (T2), using heart rate, systolic, and diastolic blood pressure, salivary α-amylase activity, salivary interleukin-1 β, and the State-Trait Anxiety Inventory (STAI-s and STAI-t). Twenty-four residents were assessed during 40 emergency situations. All stress markers presented a statistically significant increase between T1 and T2. IL-1 β presented the highest percent increase (141.0%, p .001), followed by AA (99.0%, p = .002), HR (81.0%, p .001), DBP (8.0%, p .001), and SBP (3.0%, p .001). In the multivariable analysis, time of residency had a negative correlation with HR during the emergency (adjusted R-square = .168; F = 8.69; p = .006), SBP response (adjusted R-square = .210; F = 6.19; p = .005) and DBP response (adjusted R-square = .293; F = 9.09; p = .001). Trait anxiety (STAI-t) was positively correlated with STAI-s (adjusted R-square = .326; F = 19.9; p .001), and number of procedures performed during emergency care had a positive association with HR response (adjusted R-square = .241; F = 5.02; p = .005). In the present study, emergency care provoked substantial acute stress in residents. Resident experience, trait anxiety, and number of emergency procedures were independently associated with acute stress response.

Objective Our main objective was to assess patient and family members' perception of bad news communication in the emergency department (ED) and compare these with physicians' perceptions. Methods This is a cross-sectional study carried out at the ED of a tertiary teaching hospital. To compare physicians' and receivers' (patient and/or family member) perceptions, we created a survey based on the six attributes derived from the SPIKES protocol. The surveys were applied immediately after communication of bad news occurred in the ED. We analyzed agreement among participants using κ-statistics and the χ-test to compare proportions. Results A total of 73 bad news communication encounters were analyzed. The survey respondents were 73 physicians, 69 family members, and four patients. In general, there is a low level of agreement between physicians' and receivers' perceptions of how breaking bad news transpired. The satisfaction level of receivers, in terms of breaking bad news by doctors, presented a mean of 3.7±0.6 points. In contrast, the physicians' perception of the communication was worse (2.9±0.6 points), with P value less than 0.001. Conclusion Doctors and receivers disagree in relation to what transpired throughout bad news communications. Discrepancies were more evident in issues involving emotion, invitation, and privacy. An important agreement between perceptions was found in technical and knowledge-related aspects of the communication.

BACKGROUND: Acquired immunodeficiency syndrome (AIDS) is a pandemic disease commonly associated with respiratory infections, hypoxemia, and death. Noninvasive PEEP has been shown to improve hypoxemia. In this study, we evaluated the physiologic effects of different levels of noninvasive PEEP in hypoxemic AIDS patients. METHODS: Thirty AIDS patients with acute hypoxemic respiratory failure received a randomized sequence of noninvasive PEEP (5, 10, or 15 cm H2O) for 20 min. PEEP was provided through a facial mask with pressure-support ventilation (PSV) of 5 cm H2O and an FIO2 of 1. Patients were allowed to breathe spontaneously for a 20-min washout period in between each PEEP trial. Arterial blood gases and clinical variables were recorded after each PEEP treatment. RESULTS: The results indicate that oxygenation improves linearly with increasing levels of PEEP. However, oxygenation levels were similar regardless of the first PEEP level administered (5, 10, or 15 cm H2O), and only the subgroup that received an initial treatment of the lowest level of PEEP (ie, 5 cm H2O) showed further improvements in oxygenation when higher PEEP levels were subsequently applied. The PaCO2 also increased in response to PEEP elevation, especially with the highest level of PEEP (ie, 15 cm H2O). PSV of 5 cm H2O use was associated with significant and consistent improvements in the subjective sensations of dyspnea and respiratory rate reported by patients treated with any level of PEEP (from 0 to 15 cm H2O). CONCLUSIONS: AIDS patients with hypoxemic respiratory failure improve oxygenation in response to a progressive sequential elevation of PEEP (up to 15 cm H2O). However, corresponding elevations in PaCO2 limit the recommended level of PEEP to 10 cm H2O. At a level of 5 cm H2O, PSV promotes an improvement in the subjective sensation of dyspnea regardless of the PEEP level employed.

Neste trabalho, relata-se os resultados obtidos no primeiro momento de uma pesquisa que objetiva conhecer o perfil e a formação de professores da área da saúde que usam a simulação como estratégia didática no Laboratório de Habilidades (LabHab) da Faculdade de Medicina da Universidade de São Paulo. Com previsão de dois anos, a pesquisa busca contribuir para uma proposta de formação docente que observe as prioridades e a relevância da simulação nas suas atividades didáticas. A ideia é analisar como esse profissional recria seus conhecimentos em grupo para trabalhar a simulação na atuação clínica e observar como ele percebe o alcance dos processos de simulação aprimorados pela tecnologia e pelas propostas de inovação educativa. This article reports the results of the first stage of a study aimed at evaluating the profile and training of professors from the health fields that use simulation as a learning strategy in Laboratory Skills (LabHab) at the University of São Paulo School of Medicine. Scheduled to last two years, the study seeks to contribute to a proposal for teacher training that meets the priorities and relevance of simulation in teaching activities based on an analysis of how medical professors recreate their knowledge to work with simulation in clinical practice and how they perceive the scope of technology-enhanced simulation processes and proposals for educational innovations.

Summary Objectives: Assess the effect of re-expansive respiratory patterns associated to respiratory biofeedback (RBF) on pulmonary function, respiratory muscle strength and habits in individuals with functional mouth breathing (FMB). Methods: Sixty children with FMB were divided into experimental and control groups. The experimental group was submitted to 15 sessions of re-expansive respiratory patterns associated to RBF (biofeedback pletsmovent; MICROHARD 1 V1.0), which provided biofeedback of the thoracic and abdominal movements. The control group was submitted to 15 sessions using biofeedback alone. Spirometry, maximum static respiratory pressure measurements and questions regarding habits (answered by parents/guardians) were carried out before and after therapy. The Student’s t-test for paireddata andnon-parametric tests were employedfor statistical analysis at a5% level of significance. Results: Significant changes were found in forced vital capacity, Tiffeneau index scores, maximum expiratory pressure, maximum inspiratory pressure and habits assessed in FMB with the use of RBF associated to the re-expansive patterns. No significant differences were found comparing the experimental and control groups. Conclusions: The results allow the conclusion that RBF associated to re-expansive patterns improves forced vital capacity, Tiffeneau index scores, respiratory muscle strength and habits in FMB and can therefore be used as a form of therapy for such individuals.

Background Obesity is associated with increased sympathetic activity and higher mortality. Treatment of this condition is often frustrating. Roux-en-Y gastric bypass is the most effective technique nowadays for treatment of obesity. The aim of the present study is to assess the effects of this surgery on the cardiac autonomic activity, including the influence of gender and age, through heart rate variability (HRV) analysis. Methods The study group consisted of 71 obese patients undergoing gastric bypass. Time domain measures of HRV, obtained from 24-h Holter recordings, were evaluated before and 6 months after surgery, and the results were compared. Percentage of interval differences of successive normal sinus beats greater than 50 ms (pNN50) and square root of the mean squared differences of successive normal sinus beat intervals (rMSSD) was used to estimate the short-term components of HRV, related to the parasympathetic activity. Background Patients with cirrhosis have high risk of bacterial infections and cirrhosis decompensation, resulting in admission to emergency department (ED). However, there are no criteria developed in the ED to identify patients with cirrhosis with bacterial infection and with high mortality risk. Study objective The objective of the study is to identify variables from ED arrival associated with bacterial infections and inhospital mortality. Methods This is a retrospective single-center study using a tertiary hospital's database to identify consecutive ED patients with decompensated cirrhosis. Clinical variables and laboratory results were obtained by chart review. Logistic regression models were built to determine variables independently associated with bacterial infection and mortality. Scores using these variables were designed. Results One hundred forty-nine patients were enrolled, most of them males (77.9%) with alcoholic cirrhosis (53%) and advanced liver disease (Child-Pugh C, 47.2%). Bacterial infections were diagnosed in 72 patients (48.3%), and 36 (24.2%) died during hospital stay. Variables independently associated with bacterial infection were lymphocytes less than or equal to 900/mm 3 (odds ratio [OR], 3.85 [95% confidence interval {CI}, 1.47-10]; P = .006) and C-reactive protein greater than 59.4 mg/L (OR, 5.05 [95% CI, 1.93-13.2]; P = .001). Variables independently associated with mortality were creatinine greater than 1.5 mg/dL (OR, 4.35 [95% CI, 1.87-10.1]; P = .001) and international normalized ratio greater than 1.65 (OR, 3.71 [95% CI, 1.6-8.61]; P = .002). Scores designed to predict bacterial infection and mortality (Mortality in Cirrhosis Emergency Department Score) had an area under the receiver operating characteristic curve of 0.82 and 0.801, respectively. The Mortality in Cirrhosis Emergency Department Score performed better than Model for End-Stage Liver Disease score. Conclusions In this cohort of ED patients with decompensated cirrhosis, lymphopenia and elevated C-reactive protein were related to bacterial infections, and elevated creatinine and international normalized ratio were related to mortality. Scores built with these variables should be prospectively validated.

dence of venous lesions after permanent pacemaker insertion is around 45%. However, this incidence has been based on retrospective studies in a small series; moreover, factors predicting the development of these venous injuries have not been clearly defined. The aim of this study was to identify the risk factors for higher incidence of upper extremity deep vein thrombosis after transvenous permanent pacemaker insertion. The study included 229 patients and the criteria were age above 12 years and first permanent transvenous pacemaker implant. Exclusion criteria were pulmonary embolism, lower or upper extremity deep venous thrombosis, previous use of central venous catheters, coagulation disturbances, and malignancy. Age, race, sex, underlying cardiac disease, functional class to heart failure, LVEF, venous access, number, material and caliber of the leads, and previous use of a transvenous temporary pacemaker were considered. Six months after the pacemaker was implanted, 202 patients were submitted to digital subtraction venography ipsilateral to pacemaker implant. The venographies were normal in 73 (36%) patients and abnormal in 129 (64%) patients. Patients with previous use of transvenous temporary leads (P 5 0,0001, OR 5 4,260, confidence limits 5 2,133‐ 8,465) and LVEF # 40% (P 5 0,0378, OR 5 3,437, confidence limits 5 1,064‐ 12,326), had higher incidence of venous lesions. Previous use of a temporary pacemaker and LVEF # 0.40 were considered independent risk factors to a higher incidence of venous stenosis or thrombosis 6 months after permanent pacemaker insertion. Despite the severity of pneumonia in patients with pandemic influenza A infection (H1N1), no validated risk scores associated with H1N1 pneumonia were tested. In this prospective observational study, we analyzed data of consecutive patients in our emergency room, hospitalized because of pneumonia between July and August 2009 in a public hospital in Brazil. The following pneumonia scoring systems were applied: the SMART-COP rule; the Pneumonia Severity Index; and the CURB-65 rule. Of 105 patients with pneumonia, 53 had H1N1 infection. Among them, only 9.5% that had a low risk according to SMART-COP were admitted to ICU, compared with 36.8% of those with the Pneumonia Severity Index score of 1-2 and 49% of those with CURB-65 score of 0-1. The SMART-COP had an accuracy of 83% to predict ICU admission. The SMART-COP rule presented the best performance to indicate ICU admission in patients with H1N1 pneumonia.

Although Aspergillus is widespread, clinically significant disease is rare in immunocompetent patients. We present a case of an otherwise healthy individual who developed cerebral vasculitis and stroke symptoms from Aspergillus, to raise awareness of this entity.

Introduction Triage protocols are only initiated when it is apparent that resource deficits will occur across a broad geographical area despite efforts to expand or acquire additional capacity. Prior to the pandemic the UK Department of Health (DOH) recommended the use of a staged triage plan incorporating Sepsis-related Organ Failure Assessment (SOFA) developed by the Ontario Ministry of Health to assist in the triage of critical care admissions and discharges during an influenza outbreak in the UK. There are data to suggest that had it been used in the recent H1N1 pandemic it may have led to inappropriate limitation of therapy if surge capacity had been overwhelmed. Methods We retrospectively reviewed the performance of the Simple Triage Scoring System (STSS) as an indicator of the utilization of hospital resources in adult patients with confirmed H1N1 admitted to a university teaching hospital. Our aim was to compare it against the staged initial SOFA score process with regards to mortality, need for intensive care admission and requirement for mechanical ventilation and assess its validity. Results Over an 8 month period, 62 patients with confirmed H1N1 were admitted. Forty (65%) had documented comorbidities and 27 (44%) had pneumonic changes on their admission CXR. Nineteen (31%) were admitted to the intensive care unit where 5 (26%) required mechanical ventilation (MV). There were 3 deaths. The STSS group categorization demonstrated a better discriminating accuracy in predicting critical care resource usage with a receiver operating characteristic area under the curve (95% confidence interval) for ICU admission of 0.88 (0.78-0.98) and need for MV of 0.91 (0.83-0.99). This compared to the staged SOFA score of 0.77 (0.65-0.89) and 0.87 (0.72-1.00) respectively. Low mortality rates limited analysis on survival predictions. Conclusions The STSS accurately risk stratified patients in this cohort according to their risk of death and predicted the likelihood of admission to critical care and the requirement for MV. Its single point in time, accuracy and easily collected component variables commend it as an alternative reproducible system to facilitate the triage and treatment of patients in any future influenza pandemic.

To the Editor: First, we would like to thank Dr. Russo for his comments. In his letter, he states that our work confirms the results of previous reports in smaller groups of patients and comments about our selection criteria of obesity patients. Although we really did not include in the exclusion criteria the presence of respiratory disturbances, there was in our group just one patient with history of chronic pneumopathy. The others have no diagnosis of respiratory diseases. We described in our paper that the number of patients with antihypertensive drugs decreased from 25 to six patients 6 months after surgery. In his letter, Russo comments the absence of information about angiotensinconverting enzyme inhibitors (ACE inhibitors) and angiotensin II receptors blockers (ARBs), others drugs (steroid hormones, antidepressants, appetite suppressants), and smoking. In fact, we found in our group ten patients using ACE inhibitors before surgery and just one patient 6 months after surgery. Three patients were using ARBs before and none after. There were no patients using sex steroid hormones or appetite suppressants. No patient was using antidepressants before surgery and just one patient was using after (fluoxetine). The purpose of this study was to develop a more efficient and stable model of ventricular fibrillation (VF) in the isolated rabbit heart, because there is not a satisfactory model with this animal. We also observed the effects of increasing extracellular calcium in the stability and reversibility of the arrhythmia. After suspending the hearts in a classical Langendorff preparation, VF was induced by burst stimulation (current = 2.0 mA, pulse duration = 3 milliseconds, frequency = 50 Hz, voltage = 10 V, duration of stimulation = 5 minutes). The hearts were then divided into 2 groups, A and B. The hearts in group B were perfused with a modified Krebs-Henseleit solution, which contained twice as much calcium as the solution used in the other group. The rate of success with this model was 100% for both groups. The hearts fibrillated up to 30 minutes in group A and more than 40 minutes in group B, longer then all studies ever published in rabbit hearts. Ventricular fibrillation reverted to sinus rhythm in 100% of the hearts of group A when treated with an antifibrillatory drug, whereas no reversion at all was observed in the hearts of group B. We conclude that high extracellular calcium makes the reversion to sinus rhythm more difficult in this model. Our high rate of success and the exceptionally stable and long-lasting VF turn our model very effective for the study of antiarrhythmic interventions in the isolated rabbit heart.

Introdução: o trabalho em equipe constitui um dos principais requisitos no sistema altamente complexo de prestação de cuidados ao paciente crítico pediátrico. Na literatura existe amplo consenso de sua ligação direta à qualidade e segurança da assistência. Objetivos: sintetizar as evidências existentes na literatura sobre o impacto da capacitação para o trabalho em equipe na performance de profissionais que prestam assistência ao paciente crítico pediátrico. Avaliar satisfação dos participantes de capacitação em reanimação cardiorrespiratória em pediatria utilizando simulação de alta fidelidade. Verificar se abordagem específica sobre trabalho em equipe melhora os desempenhos técnico e não técnico. Método: Foi realizada revisão integrativa por meio de busca em bancos de dados da literatura médica de artigos que estudaram o efeito do trabalho em equipe no cuidado do paciente crítico pediátrico. Foi realizada capacitação de 65 alunos de Medicina sobre atendimento de parada cardiorrespiratória em crianças, conforme protocolos da American Heart Association de 2015. O grupo intervenção recebeu, adicionalmente, treinamento sobre trabalho em equipe com versão customizada do protocolo TeamSTEPPS. Houve avaliação teórica e prática antes e após a capacitação, assim como questionário de satisfação. Resultados: na revisão integrativa, os estudos demonstraram a melhora de habilidades não técnicas, autoeficácia e/ou cultura de segurança quando uma intervenção relacionada ao trabalho em equipe foi realizada; além de melhora no desempenho técnico e desfechos clínicos, quando avaliados. A respeito da capacitação realizada, no teste teórico não houve diferença entre os grupos controle e intervenção, tanto pré (p=0,291) quanto pós-teste (p=0,397), mas em ambos houve melhora na nota (p=0,000 e p=0,049). No checklist do teste prático, ambos os grupos apresentaram melhores percentuais de acerto no pós-teste (p=0,000); não houve diferença entre eles no pré-teste (p=0,289), mas, no pós-teste, o grupo intervenção conseguiu melhores escores (p=0,000). Houve um aumento do número de eventos de equipe em ambos os grupos no pós-teste (p=0,000) com maiores valores no grupo intervenção no pré-teste (p=0,017) e no pós-teste (p=0,009). Ambos apresentaram melhora no tempo total gasto para realizar os primeiros passos da reanimação no pós teste (p=0,000), com redução de 33,6% no grupo controle e de 34,2% no grupo intervenção, mas sem diferença entre eles no pré-teste (p=0,994) e pós-teste (p=0,819). Conclusões: as evidências do efeito benéfico do trabalho em equipe na assistência em saúde são crescentes, mas, a despeito disso, as pesquisas na área pediátrica ainda são escassas. A capacitação de trabalho em equipe utilizando protocolo TeamSTEPPS customizado melhorou a atuação em equipe e desempenho técnico. A avaliação dos alunos quanto à capacitação recebida foi favorável. Introduction: teamwork is one of the major requirements in the highly complex system for providing care to pediatric critically ill patients. There is broad consensus in the literature of its direct link to quality and safety of care. Objectives: to summarize the evidence in the literature on the impact of teamwork training on the performance of professionals who provide care to pediatric critically ill patients; to evaluate the satisfaction of people who attended a training on pediatric cardiopulmonary resuscitation using high fidelity simulation; and to check if the specific approach on teamwork improves the technical and non-technical performance. Method: an integrative review was performed by searching for articles that studied the effect of teamwork on the care of pediatric critically ill patients in medical literature databases. Training was given to 65 Medical students on cardiac arrest care in children, according to the 2015 American Heart Association protocols. The intervention group also received training on teamwork with a customized version of the TeamSTEPPS® protocol. There was theoretical and practical evaluation before and after training, as well as a satisfaction questionnaire. Results: in the integrative review, the studies showed improvement in non-technical skills, self-efficacy, and/or safety culture when an intervention related to teamwork was performed; in addition to improvement in technical performance and clinical outcomes, when evaluated. Regarding the training carried out, in the theoretical test, there was no difference between the control and intervention groups, both pre-(p=0.291) and post-test (p=0.397), but in both there was an improvement in the score (p=0.000 and p=0.049). In the practice test checklist, both groups showed better percentages of right answers in the post-test (p=0.000); there was no difference between them in the pre-test (p=0.289), but, in the post-test, the intervention group achieved better scores (p=0.000). There was an increase in the number of team events in both groups in the post-test (p=0.000), with higher values in the intervention group in the pre-test (p=0.017) and in the post-test (p=0.009). Both groups presented improvement in the total time spent to perform the first resuscitation steps in the post-test (p=0.000), with reduction of 33.6% in the control group and 34.2% in the intervention group, but without difference between them in the pre-test (p=0.994) and in the post-test (p=0.819). Conclusions: evidence of the beneficial effect of teamwork on health care is growing, but, despite this, research in the pediatric area is still meager. Teamwork training using the customized TeamSTEPPS® protocol has improved teamwork and technical performance. The evaluation on the part of students regarding the received training was favorable.

A prática da medicina vem mudando rapidamente. Nos últimos 20 anos os profissionais de saúde tem se preocupado cada vez mais com os erros que ocorrem durante o processo de cuidado dos pacientes enquanto trabalham duramente para preveni-los e mitigá-los. Tão importante quanto o erro em si é a maneira como o evento adverso é discutido e a maneira como os profissionais envolvidos no erro são abordados. O objetivo da pesquisa foi aplicar um questionário sobre erro e abordagem do erro. O instrumento foi originalmente publicado em língua inglesa e validado em português pelo método de Brislin. Responderam ao questionário 161 profissionais de saúde (enfermeiros, técnicos de enfermagem, médicos e fisioterapeutas) provenientes de 19 diferentes unidades de terapia intensiva adulto públicas e privadas da cidade de São Paulo (Estado de São Paulo - Brasil).Os resultados revelaram que profissionais do sexo masculino são mais propensos a reconhecer que cometem erros em relação a profissionais do sexo feminino com uma razão de chances de 0,21 (0,07-0,65); que profissionais oriundos de unidades públicas reportam com mais frequência que ameaça de processos {RC 0,23 (0,11 - 0,48)}, ameaça à estabilidade no emprego {RC 0,49 (0,24 - 0,99)} e personalidade de outros membros da equipe {RC 0,22 (0,09 - 0,51)} são motivos para que os erros não sejam discutidos tampouco abordados adequadamente. Estas informações levantam possíveis oportunidades para aprofundar a discussão e o tratamento de eventos adversos em unidades de terapia intensiva The practice of medicine is changing quickly. In the last 20 years, health professionals have increasingly worried about errors that occur during the process of patient care while working hard for its prevention and mitigation.As important as the error itself is the way each adverse event is discussed as well as each professional involved in an error is approached .The goal of the research was applying a survey about error and its approach. The tool was originally published in english and afterwards validated in portuguese by the Brislin method. The survey was answered by 161 health professionals (nurses, nurse technicians, intensive care physicians and physiotherapists) coming from 19 different adult intensive care units both public and private in the city of São Paulo (São Paulo - Brazil). The results revealed that male professionals are more likely to recognize having made an error than female professionals with a odds ratio of 0.21 (0.07-0.65). It also showed that professionals coming from public units report more often that the threat of litigation {RC 0.23 (0.11 - 0.48)}, threat of unemployment {RC 0,49 (0,24 - 0,99) } and other team members personality {RC 0.22} (0.09 - 0.51) are reasons for problems not being discussed or addressed. The informations collected on this survey raise opportunities to improve the study and treatment of adverse events in intensive care units

Introdução: A Regulação Médica, que representa a aplicação de técnicas de logística ao contexto de emergência, é responsável pela disponibilização de recursos apropriados, nas condições apropriadas para pacientes apropriados. Um sistema para Regulação Médica de Urgências e Emergências foi desenvolvido em 2009 e foi implantado na forma de um projeto-piloto. Técnicas nas áreas de processamento de linguagem natural, recuperação de informação e aprendizado de máquina podem ser utilizadas para processar registros clínicos e auxiliar processos de tomada de decisão. Objetivos: No presente trabalho busca-se: (i) comparar diferentes metodologias para representação e extração de informação de documentos em texto livre, tais como solicitações de regulação; (ii) proporcionar suporte à decisão na definição de prioridade de casos, com processamento textual e semântico do resumo clínico dos casos; e (iii) analisar as contribuições dos dados clínicos e prioridade definida durante o processo de regulação para o desfecho do caso. Metodologia: Foram utilizados dados do projeto-piloto, assim como dados relativos ao desfecho do caso de pacientes regulados e admitidos na Unidade de Emergência do HCFMRP-USP. Os dados foram processados com o auxílio de tecnologias de Aprendizado de Máquina, Mineração de Textos e Recuperação de Informação para extrair informações organizadas em atributos a serem utilizados pra permitir suporte à decisão na prioridade do caso. Resultados: Os dados de pedidos de regulação apresentam uma grande quantidade de casos com valores de atributos muito parecidos (algumas vezes idênticos), contudo com classes (prioridades) diferentes, caracterizando uma base de dados com grande quantidade de ruídos, o que dificulta a aplicação de tecnologias como Aprendizado de Máquina. Resultados evidenciam o caráter subjetivo na definição de prioridades, que talvez seja influenciada por outros fatores que não estão presentes no texto do registro clínico do paciente. Resultados de suporte à decisão na definição de prioridade e desfecho do caso indicam que aplicar processamento semântico, mapeando termos para conceitos médicos do UMLS, reduz o problema da dimensionalidade quando comparado a abordagens menos robustas de mineração de textos. A abordagem apoiada por recuperação de informação, permite que sejam classificados apenas pedidos de regulação que sejam mais similares que um limiar (threshold) desejado em relação a algum caso do banco de dados. Desta maneira, esta abordagem pode ser utilizada para reduzir sobrecarga, permitindo que reguladores concentrem sua atenção em casos mais críticos e casos de maior particularidade (não similares a casos históricos). Conclusões: O presente trabalho proporcionou suporte à decisão na priorização de casos em regulações de urgência e emergência, com processamento textual e semântico do resumo clínico dos casos. Definiu-se como proposta para suporte à decisão na priorização de casos um processo composto por três etapas: (i) análise do risco de óbito; (ii) pré-priorização automática de casos de alta similaridade com casos históricos; e (iii) apoio à decisão com base em casos históricos (aprendizagem baseada em exemplos). Introduction: The Medical Coordination, which is the application of logistics techniques to the emergency context, is responsible for providing appropriate resources, in appropriate conditions to appropriate patients. A system for medical coordination of emergency requests was developed in 2009 and was implemented as a pilot project, although some activities related to medical coordination decision making are extremely subjective. Techniques from the areas of natural language processing, information retrieval and machine learning can be used to process clinical records and assist decision-making processes. Objectives: The present study aims to: (i) compare different methodologies for representation and information extraction from free text documents, such as coordination requests; (ii) provide decision support to prioritization of requests, with textual and semantic processing of clinical summaries of the cases; and (iii) analyze the contributions of clinical data and priority defined during the coordination process to the final case outcome. Methodology: Data from the pilot project, as well as data on the case outcome of coordinated patients admitted to the HCFMRP-USP Emergency Unit we used. Data was processed with the aid of Machine Learning, Information Retrival and Text Mining techniques to extract information organized into attributes to be used to enable decision support on the priority of the case. Results: The coordination requests data contain a large number of cases with very similar attribute values (sometimes identical), but with different classes (priorities), characterizing a database with a large amount of noise, making it hard to apply technologies such as Machine Learning. Results denote the subjective aspect in the definition of priorities, which may be influenced by other factors that are not present in the patient\'s clinical record text. Decision support results in prioritization and case outcome indicate that applying semantic processing, mapping terms to UMLS medical concepts, reduces the dimensionality problem when compared to less robust text mining approaches. The approach supported by information retrieval allows to classify only coordination requests that are more similar than a defined threshold to a historical case. Thus, this approach can be used to reduce overhead, allowing coordinators to focus their attention on the most critical cases and cases of greater particularity (not similar to historical cases). Conclusions: This work provided decision support in prioritizing cases of urgency and emergency coordination requests, with textual and semantic processing of clinical summary cases. It was defined as a proposal for decision support in prioritization of requestes a process consisting of three steps: (i) analysis of the risk of death; (ii) automatic pre-prioritization of cases of high similarity with historical cases; and (iii) decision support based on historical cases (examples-based learning).

Introdução: A comunicação de más notícias é uma prática difícil, porém frequente, tornando-se praticamente rotineira no dia-a-dia de médicos. No departamento de emergência, esta categoria de comunicação adquire aspectos bastante particulares. Objetivos: O principal objetivo deste estudo é avaliar a percepção de pacientes e familiares acerca da comunicação de más notícias no departamento de emergência, comparando-as à percepção de médicos residentes. Método: Trata-se de um estudo observacional transversal realizado no departamento de emergência de um hospital universitário terciário. Com o intuito de comparar as percepções de médicos residentes e receptores (pacientes e familiares), foi criado um questionário baseado nas seis recomendações provenientes do protocolo SPIKES (Setting [Ambientação]: questões 1-5; Perception [Percepção]: questão 6; Invitation [Convite]: questão 7; Knowledge [Conhecimento]: questões 8-12; Emotions [Emoções]: questões 13-15; Strategy and Summary [Estratégia e Resumo]: questão 16). Os questionários foram aplicados aos participantes imediatamente após a comunicação de uma má notícia no departamento de emergência. A concordância entre médicos residentes e receptores foi analisada utilizando os testes estatísticos de Kappa e Qui-quadrado. Resultados: Um total de 73 comunicações de más notícias foram analisadas. Os participantes foram 73 médicos residentes, 69 familiares e 4 pacientes. Em geral, houve um baixo nível de concordância entre médicos residentes e receptores acerca de como se deu a comunicação da má notícia no departamento de emergência. O nível de satisfação de pacientes e familiares sobre como os médicos residentes comunicaram estas notícias apresentou média de 3,7 + 0,6 pontos. Em contraste, os médicos residentes demonstraram uma pior percepção do mesmo encontro (2,9 + 0,6 pontos), sendo p < 0.001. Conclusão: Médicos residentes e receptores tendem a discordar em relação a diversos aspectos da comunicação de uma má notícia. As discrepâncias foram mais evidentes em questões envolvendo emoções, convite e privacidade. No entanto, uma importante concordância entre as percepções foi encontrada nas questões técnicas e de conhecimento que surgiram durante a comunicação Introduction: Breaking bad news is a common and routine practice, performed practically every day by physicians. In the Emergency Department, communication acquires unique aspects. Objective: Our main objective was to assess patient and family member\'s perception about bad news communication in the Emergency Department and compare these with physicians\' perceptions. Method: This is a cross-sectional study performed at the Emergency Department of a tertiary teaching hospital. To compare physicians\' and receivers\' (patient and/or family member) perceptions, we created a survey based on the six attributes derived from the SPIKES protocol (Setting: questions 1-5; Perception: question 6; Invitation: question 7; Knowledge: questions 8-12; Emotions: questions 13-15; Strategy and Summary: question 16). The surveys were applied immediately after bad news communication happened in the Emergency Department. We analyzed agreement amongst participants using Kappa statistics and Qui-squared test to compare proportions. Results: A total of 73 bad news communication encounters were analyzed. The survey respondents were 73 physicians, 69 family members and 4 patients. In general, there is a low level of concordance between physicians\' and receivers\' perceptions of how breaking bad news transpired. The satisfaction level of receivers in regards to breaking bad news by doctors presented a mean of 3.7 + 0.6 points. In contrast, the physicians\' perception of the communication was worse (2.9 + 0.6 points), with p < 0.001. Conclusions: Doctors and receivers disagree in relation to what transpired throughout the bad news communication. Discrepancies were more evident in issues involving emotion, invitation and privacy. However, an important agreement between perceptions was found in technical and knowledge related aspects of the communication

INTRODUÇÃO: A síndrome da imunodeficiência adquirida (AIDS) é atualmente uma pandemia, e as doenças pulmonares são a principal causa de morbidade e mortalidade dos pacientes com AIDS. Nesse sentido, as infecções respiratórias são frequente causa de hipoxemia e morte. Os pacientes com AIDS e insuficiência respiratória hipoxêmica frequentemente necessitam de ventilação mecânica invasiva, a qual é independentemente associada com mortalidade. A ventilação não invasiva com pressão positiva refere-se à oferta de assistência ventilatória mecânica sem a necessidade de invasão artificial das vias aéreas, sendo reconhecida por melhorar a oxigenação e a dispneia dos pacientes com insuficiência respiratória hipoxêmica, principalmente se aplicada de forma sequencial e progressiva, e esta pode reduzir a necessidade de ventilação mecânica invasiva nestes pacientes. Tendo em vista as incertezas quanto à resposta da oxigenação a PEEP nos pacientes com AIDS com insuficiência respiratória aguda hipoxêmica e usando o racional da pressurização progressiva das vias aéreas e seu potencial benefício na oxigenação sanguínea, nós fizemos a hipótese de que o incremento sequencial dos níveis de PEEP até 15 cmH2O pode melhorar a oxigenação sanguínea sem afetar o conforto e a hemodinâmica do paciente. O objetivo principal deste estudo foi investigar os efeitos de diferentes sequências de níveis de PEEP aplicado de forma não invasiva sobre as trocas gasosas, a sensação de dispneia e os padrões hemodinâmicos em pacientes com AIDS e insuficiência respiratória aguda hipoxêmica. O objetivo secundário foi avaliar o tempo livre de ventilação mecânica invasiva em 28 dias e a mortalidade hospitalar em 60 dias. MÉTODOS: Foram estudados 30 pacientes adultos com HIV/AIDS e insuficiência respiratória aguda hipoxêmica. Todos os pacientes receberam uma sequência randomizada de PEEP não invasivo (os valores usados foram 5, 10 ou 15 cmH2O) por vinte minutos. A PEEP foi fornecida através de máscara facial com pressão suporte (PSV) de 5 cmH2O e uma FiO2 = 1. Um período de washout de 20 minutos com respiração espontânea foi permitido entre cada PEEP. Variáveis clínicas e uma gasometria arterial foram registradas após cada etapa de PEEP. RESULTADOS: Analisando os 30 pacientes, a oxigenação melhorou linearmente com a elevação da PEEP, contudo, estudando os pacientes conforme a PEEP inicial randomizada, a oxigenação foi similar independentemente da primeira PEEP randomizada (5, 10 ou 15 cmH2O), e somente o subgrupo com PEEP inicial = 5 cmH2O melhorou mais a oxigenação quando PEEPs maiores foram usadas. A PaCO2 também aumentou junto com a elevação da PEEP, especialmente com uma PEEP = 15 cmH2O. O uso de PSV = 5 cmH2O foi associado com significante e consistente melhora da sensação subjetiva de dispnéia e da frequência respiratória com PEEP de 0 a 15 cmH2O. CONCLUSÕES: Os pacientes com SIDA e insuficiência respiratória hipoxêmica melhoram a oxigenação com a elevação progressiva e sequencial da PEEP até 15 cmH2O, contudo a elevação da PaCO2 limita a PEEP até 10 cmH2O. Uma PSV = 5 cmH2O promove uma melhora da sensação subjetiva da dispnéia independentemente do uso de PEEP INTRODUTION: The acquired immunodeficiency syndrome (AIDS) is a pandemic, and lung diseases are the leading cause of morbidity and mortality and are often associated with respiratory infections, hypoxemia and death. The noninvasive ventilation with positive pressure refers to the provision of mechanical ventilatory assistance without the need for artificial airway invasion, being recognized for improving oxygenation and dyspnea in patients with hipoxemic respiratory failure. Patients with AIDS and hypoxemic respiratory failure often require invasive mechanical ventilation, which is independently associated with mortality. Given the uncertainties about response in oxygenation with PEEP in patients with AIDS with acute hypoxemic respiratory failure and using the rational for progressive pressurization of the airway and its potential benefits on blood oxygenation, we made the hypothesis that increased levels of sequential PEEP up to 15 cmH2O may improve blood oxygenation without affecting the comfort and hemodynamics of the patient. The main objective of this study was to investigate the effects of different sequences of PEEP levels on gas exchange, the sensation of dyspnea and hemodynamics in patients with AIDS and acute hypoxemic respiratory failure. The secondary objective was to assess the time free of invasive mechanical ventilation in 28 days and hospital mortality within 60 days. METHODS: We studied 30 adults patients with HIV/AIDS and acute hypoxemic respiratory failure. All patients received a randomized sequence of noninvasive PEEP (the values used were 5,10 or 15 cmH2O) for twenty minutes. PEEP was delivered via face mask with pressure support (PSV) of 5 cmH2O and FiO2 = 1. A washout period of 20 minutes with spontaneous breathing was allowed between each PEEP trial. Clinical variables and arterial blood gases were recorded after each PEEP step. RESULTS: Analyzing the 30 patients, oxygenation improved linearly with increasing PEEP, however studying the patients randomized according to the initial PEEP, oxygenation was similar regardless of the first randomized PEEP (5,10 or 15 cmH2O), and only the subgroup with initial PEEP = 5 cmH2O further improve the oxygenation when high PEEP were used. The PaCO2 also rose beside the PEEP elevation, especially with a PEEP = 15 cmH2O. The use of PSV = 5 cmH2O was associated with significant and consistent improvement of subjective sensation of dyspnea and respiratory rate with a PEEP from 0 to 15 cmH2O. CONCLUSION: AIDS-patients with hypoxemic respiratory failure improve oxygenation with a progressive sequential elevation of PEEP up to 15 cmH2O, however the elevation of PaCO2 limit the PEEP up to 10 cmH2O. A PSV = 5 cmH2O promotes an improvement of subjective sensation of dyspnea independently from the use of PEEP

Para se avaliar o trabalho dos generalistas/ médicos de família, é necessário um adequado sistema de classificação das consultas ou encontros entre profissionais da saúde e pacientes. O sistema atualmente conhecido como Classificação Internacional de Doenças (CID) começou seu desenvolvimento no século XIX e até sua quinta edição era apenas uma classificação de causas de morte. Após a sexta revisão, ela passou a ser um instrumento que envolvia morbidade também; porém, perdeu as características e princípios de um sistema de classificação. A Organização Mundial de Médicos de Família (WONCA) vem desenvolvendo, desde os anos 70, um compacto e robusto sistema de classificação chamado Classificação Internacional de Atenção Primária que se encontra atualmente na segunda versão (CIAP 2), apropriado para ser usado na atenção primária à saúde. Ela é baseada em três componentes principais: queixa do paciente (motivo da consulta), diagnóstico médico (problema) e processo (intervenção). Objetivo: este estudo objetivou avaliar os principais motivos da consulta, problemas e comorbidades nas Unidades Básicas de Saúde de Florianópolis, Santa Catarina, e testar o uso da CIAP2, baseado na avaliação de encontros, porém, inter-relacionando os motivos das consultas expostos pelos pacientes com os problemas encontrados pelos profissionais. Metodologia: um formulário em papel foi desenvolvido com informações gerais dos pacientes (idade, gênero, estado civil e ocupação) e informações sobre a consulta - natureza da consulta (agendada ou não), motivo(s) da consulta (descrito(s) com as palavras dos pacientes) e os problemas correspondentes (estabelecidos pelos profissionais da saúde voluntários), plano (referenciado para especialista ou outro profissional de atenção primária), exames complementares e prescrição de medicamentos. Os 90 generalistas/ médicos de família de Florianópolis que estavam trabalhando na Estratégia Saúde da Família foram convidados. Todos que aceitaram participar deveriam responder o formulário após cada consulta durante uma semana típica de trabalho por estação do ano. Um especialista em CIAP2 codificou todos os formulários preenchidos. Resultados: trinta voluntários aceitaram participar e 26 completaram pelo menos uma semana típica de trabalho. 5698 encontros foram avaliados com regular distribuição ao longo das estações do ano. Foram estabelecidos em média 1,625 motivos da consulta (MC) e 1,475 problemas por consulta. Os 30 problemas mais comuns representaram 50% de todas as consultas, o que é compatível com as referências internacionais disponíveis. Estes problemas mais frequentes pertencem a 13 capítulos diferentes da CIAP 2 (cada capítulo corresponde a um órgão ou sistema) com distribuição homogênea. Os dados apresentaram boa qualidade e nenhuma aberração, como hipertensão em recém nascido ou problema ginecológico em homem, foi encontrada. Foi possível calcular probabilidades pré teste de motivos das consultas para problemas comuns como infecção de vias aéreas superiores (IVAS) e perturbações depressivas bem como a probabilidade de diagnóstico de diferentes problemas para motivos da consulta frequentes como tosse e cefaléia. As comorbidades mais comuns encontradas foram hipertensão e diabetes e hipertensão e dislipidemia. Em apenas 26,6% de todas as consultas nenhum medicamento foi prescrito. Conclusão: o estudo encontrou dados que colaboram no raciocínio clínico, no planejamento do desenvolvimento profissional contínuo e na proteção do paciente contra diagnósticos inapropriados de doenças e suas possíveis intervenções desnecessárias. A CIAP 2 é uma potente ferramenta para ser usada na prática diária não para guiar o processo diagnóstico, mas para colaborar na produção e análise de dados transformando cada unidade básica de saúde em um campo de pesquisa In order to evaluate the work of generalist/ family doctor it is necessary an adequate classification system. The nowadays known as International Classification of Diseases (ICD) started its development in 19`s century and until its fifth edition it was just a cause of death classification. After sixth revision it became a morbidity and mortality tool but lost its classification characteristics and principles. The World Organization of Family Doctors (Wonca) developed after 70`s one compact and strong classification system called International Classification of Primary Care, which is in its second edition (ICPC2), appropriate to be used in primary care settings. It is based on the three main consultation parts: patients complaint, doctor diagnose (problem), and process (intervention). Objective: this study aims to evaluate main complaints, problems, comorbidities in Florianópolis` health centers and test ICPC2 use in an encounter mode but interrelating complaints and problems. Method: one paper form was designed with patient general information (age, gender, civil state, occupation) and information regarding the consultation: nature of consultation (schedule or not), complaints (with patients words) and the correspondent problem (stated by health provider), plan (referred to specialist/ other primary care provider), complementary exams and prescriptions. All 90 generalists/ family doctors of Florianópolis who were working in Family Health Program were invited. Everyone should answer the form after each consultation during one typical work week per season along one year. One ICPC2 specialist coded all filled forms. Results: thirty volunteers accepted to participate and 26 completed at least one typical work week. 5698 encounters were evaluated with regular distribution among seasons. There were 1,625 complaints and 1,475 problems per consultation on average. The 30 most common problems represented 50% of all consultations which is compatible with international data available. These more frequent problems belong to 13 different chapters (each one corresponds to a different organ or body system) with unvarying distribution. Data collected presented a very good quality and no aberration as hypertension in new born or gynecological problems in a male was found. It was possible to calculate prior probability of complaints for common problems as upper respiratory infection acute and depressive disorder as well the probability of different problems for a frequent complaint as cough or headache. The most common commorbidities were hypertension and diabetes and hypertension and lipid disorder. Only in 26.6% of all encounters no medicine was prescribed. Conclusion: the study provided adequate data that help in clínical reasoning, continued professional development plan and patient protection against inappropriate diagnose and its consequent intervention. ICPC2 is a strong tool to be used in daily practice not to guide diagnosis process but to produce and analyze data and transform each health center in a research field.

Objetivos: avaliar o efeito dos padrões ventilatórios reexpansivos associados ao biofeedback respiratório (BFR) sobre a função pulmonar, força muscular respiratória e hábitos em respiradores bucais funcionais. Métodos: foram avaliadas 60 crianças respiradoras bucais funcionais (RBF), divididas em Grupo Experimental e Controle, sendo o Grupo Experimental submetido a 15 sessões de padrões ventilatórios reexpansivos associados ao BFR por meio do biofeedback pletsmovent (MICROHARD® V1.0), o qual proporciona o biofeedback dos movimentos tóraco-abdominais e o Grupo Controle submetido ao mesmo número de sessões realizando apenas o biofeedback. Espirometria, medidas das pressões respiratórias máximas estáticas e questões para avaliação de hábitos, respondidas pelos responsáveis pelas crianças, foram realizadas antes e após a terapia. Teste t de Student para dados pareados e testes não paramétricos, com nível de significância de 5%, foram utilizados para análise. Resultados: Foram observados alterações significativas na capacidade vital forçada (FVC), índice de Tiffeneau (IT), pressão expiratória máxima (PEmáx), pressão inspiratória máxima (PImáx) e nos hábitos avaliados em RBF com o uso do BFR associado aos padrões reexpansivos. Não foram observadas diferenças significantes quando comparados o Grupo Experimental e o Controle. Conclusão: Os resultados permitem concluir que o BFR associado aos padrões reexpansivos melhora a capacidade vital forçada, IT, força da musculatura respiratória e hábitos em RBF, podendo ser, portanto, utilizado como uma das formas de terapia nesses indivíduos Objectives: Assess the effect of re-expansive respiratory patterns associated to respiratory biofeedback (RBF) on pulmonary function, respiratory muscle strength and habits in individuals with functional mouth breathing (FMB). Methods: Sixty children with FMB were divided into experimental and control groups. The experimental group was submitted to 15 sessions of re-expansive respiratory patterns associated to RBF (biofeedback pletsmovent; MICROHARD® V1.0), which provided biofeedback of the thoracic and abdominal movements. The control group was submitted to 15 sessions using biofeedback alone. Spirometry, maximum static respiratory pressure measurements and questions regarding habits (answered by parents/guardians) were carried out before and after therapy. The Student\'s t-test for paired data and non-parametric tests were employed for statistical analysis at a 5% level of significance. Results: Significant changes were found in forced vital capacity, Tiffeneau index scores, maximum expiratory pressure, maximum inspiratory pressure and habits assessed in FMB with the use of RBF associated to the re-expansive patterns. No significant differences were found comparing the experimental and control groups. The results allow the conclusion that RBF associated to re-expansive patterns improves forced vital capacity, Tiffeneau index scores, respiratory muscle strength and habits in FMB and can therefore be used as a form of therapy for such individuals.

INTRODUÇÃO: Informações da literatura associam a obesidade a maior atividade simpática. A gastroplastia com derivação gastrojejunal em Y de Roux, que leva a redução rápida e intensa do peso, pode influenciar o padrão autonômico cardiovascular. O objetivo deste estudo é avaliar os efeitos dessa cirurgia sobre a modulação autonômica do coração, tolerância ortostática e excreção urinária de noradrenalina. METODOS: Trata-se de um estudo observacional longitudinal, realizado na cidade de Maringá - PR, com 71 pacientes, incluídos no período de julho de 2004 a dezembro de 2005, avaliados antes e seis meses após a cirurgia. Foram estudados 42 mulheres e 29 homens, com idade variando de 18 a 66 anos (mediana de 36 anos) e índice de massa corpórea (IMC) variando de 37,1 a 56,2 kg/m2 (mediana de 41,9 kg/m2). Do total, 28 eram hipertensos. Não foram incluídos pacientes com diagnóstico de diabetes melito. Análise da variabilidade da freqüência cardíaca no domínio do tempo através de gravações de Holter 24 horas, teste de inclinação ortostática e dosagem de noradrenalina em urina de 24 horas foram realizadas nas duas fases do estudo. Também foram avaliados glicemia e insulina de jejum, perfil lipídico, proteína C-reativa de alta sensibilidade, fibrinogênio e qualidade de vida através do questionário SF-36. RESULTADOS: A redução média do peso, seis meses após a cirurgia, foi de 25,46% e da circunferência abdominal, de 20,4%. Introdução: Em 13 de Setembro de 2009, a OMS reportou que existiam mais de 296.471 casos confirmados laboratorialmente de infecção pelo p-H1N1 2009. Ainda assim muitas questões permanecem, incluindo o papel de regras de probabilidade clínica e escores de gravidade de pneumonia adquirida na comunidade nestes pacientes. Nós descrevemos as características clínicas e epidemiológicas de pacientes internados por pneumonia adquirida na comunidade com ou sem infecção pelo p-H1N1. Objetivos: Verificar a incidência e características clínicas da pneumonia adquirida na comunidade associada com infecção pelo p-H1N1 2009, comparado as pneumonias adquiridas na comunidade sem infecção pelo p-H1N1 2009 e a aplicação de regras de probabilidade clínica e escores de gravidade de pneumonia. Métodos: Estudo observacional prospectivo avaliando pacientes consecutivos hospitalizados por pneumonia adquirida na comunidade por mais de 24 horas no HC-FMUSP. A infecção pelo p-H1N1 foi confirmada utilizando ensaios realtime PCR (RT-PCR). Os dados coletados incluíam variáveis clínicas e laboratoriais e 3 escores de gravidade de pneumonia: PSI (Pneumonia Severity Index), CURB-65 e o SMART-COP. Resultados: De 12 de julho a 17 de agosto de 2009, um total de 118 pacientes com pneumonia foram hospitalizados e RT-PCR realizado em 105 pacientes, infecção pelo p-H1N1 foi identificada em 53 pacientes. Comparado com os 52 pacientes sem infecção pelo p-H1N1, o grupo p-H1N1 apresentou significativamente mais coriza [razão de chances (RC): 6,09;intervalo de confiança 95% (IC95%): 1,72-21,52) e infiltrado bilateral (RC: 11,08; IC95%: 3,48-35,2).Um modelo clínico baseado em nossos resultados, incluindo infiltrado bilateral, febre, coriza e idade menor que 65 anos, foi capaz de predizer infecção pelo p-H1N1 2009 com sensibilidade de 90,6% e acurácia de 82% e com uma área sobre a curva (AUC) de 0,82. Nós também verificamos que, em pacientes com infecção pelo p-H1N1 2009, apenas 9,52% com escore SMART-COP entre 0-2 foram admitidos em UTI ou evoluíram para óbito intra-hospitalar comparado a 36,84% dos pacientes com escore PSI 1-2 e 51% dos pacientes com escore CURB-65 de 0-1. O prognóstico da pneumonia foi similar nos grupos com ou sem infecção pelo p-H1N1 2009. Conclusões: A pneumonia associada com infecção pelo p-H1N1 2009 possui apresentação clínica diferente de pacientes sem infecção pelo p-H1N1, entretanto, possuem prognóstico similar. Escores tradicionais de gravidade de pneumonia como PSI e CURB-65 tiveram desempenho ruim em pacientes com infecção pelo p-H1N1 e o escore SMART-COP foi o melhor preditor de internação em UTI em pacientes com pneumonia e infecção pelo p-H1N1. Introduction: As of September 13, 2009, the WHO had reported over 296.471 laboratory-confirmed cases of p-H1N1 2009. However many questions remain unanswered, including the role of clinical prediction rules and community-acquired pneumonia severity scores. We describe clinical and epidemiologic characteristics of patients hospitalized for pneumonia at our tertiary hospital with laboratory-confirmed and laboratory-excluded H1N1 infection. Objectives: Verify the incidence and clinical characteristics of community-acquired-pneumonia associated with p-H1N1 2009 infection compared with community-acquired pneumonia without p-H1N1 infection and the applicability of clinical prediction rules and pneumonia severity scores. Methods: We prospectively reviewed medical chart in daily basis to collect data on that patients. H1N1 infection was confirmed in specimens using a real-time reverse transcriptase-polymerase-chain-reaction (RT-PCR) assay. The data collected included clinical and laboratorial variables and three pneumonia severity scores: Pneumonia Severity Index, CURB-65 and the SMART-COP rule. Results: From 12 of July through August 17, 2009, a total of 118 cases of pneumonia were hospitalized, and RT-PCR was performed in 105, indentifying p-H1N1 infection in 53 patients. Compared with the 52 patients without p-H1N1 infection , the p-H1N1 group presented significantly more often with rhinorrhoea (OR 6,09 IC 95 1,72-21,52) and bilateral infiltrates ( OR 11,08 IC 95 3,48-35,2), a clinical model based on our results and using bilateral infiltrates, fever, rhinorrhoea and age less than 65 years was capable of predict p-H1N1 infection with 90,6% sensitivity, 82% accuracy and area under the ROC curve (AUC) being 0.82. We also find that in the patients with pneumonia and p-H1N1 infection, only 9.52% of those with SMART-COP score of 0-2 presented ICU admission/in-hospital mortality, compared with 36.84% of those with PSI score of 1-2 and 51% of those with CURB-65 score of 0-1. The prognosis of pneumonia was similar in the patients with and without p-H1N1 2009 infection. Conclusions: Pneumonia associated with p-H1N1 2009 has different clinical presentation than in pneumonia patients without p-H1N1 infection, but the prognosis is similar. Traditional pneumonia severity scores like PSI and CURB-65 performed poorly in patients with p-H1N1 infection and the SMART-COP rule was the best predictor of ICU admission in pneumonia patients with p-H1N1 infection.

Há mais de 20 anos a Catuama® vem sendo utilizada contra fadiga física e mental, disfunção sexual e astenia muscular. Nos últimos anos, diversos estudos demonstraram efeitos como ação antinociceptiva, antidepressiva, neuroprotetora, vasodilatadora e dilatadora dos corpos cavernosos. Dados do Laboratório de Investigação Médica da Disciplina de Emergências Clínicas da FMUSP (LIM-51) demonstraram que a Catuama® é capaz de reverter e prevenir a fibrilação ventricular (FV) induzida em coração isolado de coelho. Tendo em vista a comprovação de que a Catuama® possui efeito cardíaco significativo, tornou-se necessário investigar mais a fundo outras potenciais propriedades cardioprotetoras. Investigamos então se a Catuama® e a Trichila catigua podem oferecer proteção ao miocárdio submetido a isquemia e reperfusão em coração isolado de ratos quando administrados cronicamente. Ratos Wistar machos e adultos foram submetidos a um tratamento de 14 dias com Catuama®, T. catigua, Água destilada ou Tween 80 por gavagem. Ao término do tratamento, os animais foram anestesiados com pentobarbital e os corações retirados e perfundidos com solução de Krebs-Henseleit (KHB) pela aorta em sistema de Langendorff. Foi mantido fluxo constante de 8mL/minuto, temperatura de 36º C e oxigenação com 95% de oxigênio e 5% de gás carbônico. Os corações foram submetidos a uma isquemia global através de interrupção da perfusão por 30 minutos seguida de 2 horas de reperfusão. Para avaliar o grau de lesão causada pelo protocolo, analisamos os aspectos hemodinâmicos e biomoleculares. Foi possível observar uma melhora significativa em muitos dos parâmetros analisados. Os grupos que receberam os extratos de Catuama® e T. catigua mostraram área de necrose inferior a 16% da área total, enquanto os grupos Tween 80 e Água destilada apresentaram uma necrose superior a 60%. Além disso, a pressão desenvolvida no ventrículo esquerdo também apresentou melhora nos primeiros minutos de reperfusão, alcançando uma recuperação próxima de 70% da pressão préisquemica nos animais tratados com os extratos, enquanto os animais dos grupos Tween 80 e Água destilada apresentaram uma recuperação em torno de 20% da pressão desenvolvida. O mesmo ocorreu com a pressão diastólica dos grupos que receberam os extratos: nos primeiros minutos de reperfusão a pressão diastólica foi reduzida para valores inferiores a 30 mmHg durante a reperfusão, próximos dos pré-isquemicos, enquanto os outros dois grupos mantiveram valores elevados de pressão diastólica durante toda a reperfusão (Água destilada: 69,56+10,05 mmHg; Tween 80: 101,69+19,80 mmHg). Os grupos tratados com os extratos também apresentaram aumento na expressão de proteínas totais e de Catalase. Por outro lado, houve uma diminuição da peroxidação lipídica e de subprodutos do óxido nítrico. A Catuama® e a T. catigua já são amplamente usadas pela população e, devido a sua popularidade e acessibilidade, podem tornar-se aliadas para seres humanos com risco de doença cardíaca isquêmica. For over 20 years Catuama® has been used against physical and mental fatigue, muscular asthenia and sexual dysfunction. In the last years, several studies have shown effects such as antinociceptive action, antidepressant, neuroprotective, vasodilator and effects in erectile-dysfunction. Data from the Medical Research Laboratory in the Department of Clinical Emergency demonstrated that Catuama® is able to reverse and prevent ventricular fibrillation (VF) induced in isolated rabbit hearts. Given the evidence that Catuama® has significant cardiac effects, it became necessary to proceed a deeper investigation on other potential cardioprotective properties. We investigated if Catuama® and Trichilia catigua may offer protection to the myocardium subjected to ischemia and reperfusion in the isolated rat heart. Adult male Wistar rats were treated during 14 days with Catuama®, T. catigua, Distilled Water or Tween 80 by gavage. At the end of the treatment, the animals were anesthetized with pentobarbital and their hearts removed and perfused with Krebs-Henseleit (KHB) through the aorta in a Langendorff system. Perfusion flow was kept constant at 8mL/minute, temperature 36° C; the preparation was aerated with 95% oxygen and 5% carbon dioxide. The hearts were submitted to global ischemia by stopping perfusion for 30 minutes followed by 2 hours of reperfusion. To assess the extension of the injury caused by the protocol, we analyzed the hemodynamic and molecular aspects. It was possible to observe a significant improvement in many parameters. The groups that received the extracts Catuama® and T. catigua showed necrotic area inferior to 16% of the total area, while the groups Tween 80 and Distilled Water showed higher than 60% necrosis. In addition, left ventricular developed pressure also improved in the first minutes of reperfusion, reaching a recovery of about 70% of pre-ischemic values in animals treated with the extracts, while animals in groups Tween 80 and Distilled Water showed a recovery around 20% of the developed pressure. The same occurred with diastolic pressure of the groups that received the extracts: in the first minutes of reperfusion, the diastolic pressure was reduced to below 30 mmHg during reperfusion, close to the pre-ischemic values, while in the other two groups diastolic pressure remained elevated throughout reperfusion (Distilled Water: 69.56 +10.05 mmHg; Tween 80: 101.69 +19.80mmHg). The groups treated with the extracts also showed increased expression of total protein and catalase. On the other hand, there was a decrease in lipid peroxidation products and nitric oxide. Catuama® and T. catigua are already widely used by the population and, due to their popularity and accessibility can become allies to humans at risk for ischemic heart disease.

O Teste do Progresso é uma estratégia para avaliação do ganho de conhecimento longitudinal. Foi implementado no curso de Odontologia da Universidade da Cidade de São Paulo (UNICID), com metodologia tradicional de ensino, com os objetivos de avaliar se o ganho de conhecimento dos alunos durante a graduação é contínuo, se existe perda de conhecimento das disciplinas do curso básico ao longo da graduação, e se houve retenção dos conhecimentos na disciplina de Saúde Coletiva. O teste foi aplicado duas vezes por ano, em 2007 e 2008. As provas foram compostas por 85 questões de múltipla escolha, enviadas pelos responsáveis por cada disciplina do curso. O escore médio foi calculado para cada teste por ano letivo e comparado utilizando-se ANOVA, com teste post hoc de Bonferroni. O fitoterápico CATUAMA, utilizado no Brasil há mais de 20 anos, e seu extrato isolado T catigua apresentam efeito de reversão e prevenção da fibrilação ventricular (FV). FV, em pacientes com disfunção cardíaca, representa uma importante causa de morte súbita no mundo ocidental. Neste contexto, os efeitos de CATUAMA sobre a função cardíaca e os parâmetros hemodinâmicos foram investigados em ratos normais. Várias concentrações (10 a 800 ?g/mL) de CATUAMA (uma mistura dos extratos das plantas Trichilia Catigua, Paullinia Cupana, Ptychopetalum Olacoide e Zingiber Officinale) e de cada um de seus constituintes foram infundidas em preparações de músculos papilares isolados. A tensão desenvolvida (TD), tensão de relaxamento (TR) e as velocidades de aumento e redução da tensão (dT/dtmax e dT/dtmin) foram analisadas. Uma única dose de 200 ?g/mL foi injetada na veia jugular direita in situ. Os índices (dP/dtmax e dP/dtmin) de função ventricular esquerda (VE), as pressões sistólica (PSVE) e diastólica final (PDFVE), de perfusão coronariana (PPC), arteriais sistólica (PAS), diastólica (PAD) e média (PAM) e a frequência cardíaca (FC) foram monitoradas por 10min via cateterização VE. CATUAMA afetou a contratilidade miocárdica na maior dose (dose tóxica), reduzindo 9% a TD e 16% a TR, aumentando 90% a dT/dtmin. CATUAMA também melhorou em até 16% as PAS, PAD, PAM e PPC. Seus extratos isolados apresentaram efeitos diferenciados. T catigua não interferiu nas variáveis. P cupana reduziu a contratilidade e a PDFVE e aumentou os parâmetros hemodinâmicos assim como CATUAMA. P olacoide reduziu a TD, a TR e a PDFVE. Z officinale também reduziu a TD e a TR. CATUAMA e T catigua não trazem prejuízo à função cardíaca. The Brazilian herbal medicine CATUAMA and its extract T catigua have shown reversion and prevention action on ventricular fibrillation (VF). VF, in patients with cardiac dysfunction, is an important cause of sudden death in Western world. In this regard, CATUAMA effects on cardiac function and hemodynamic parameters in normal rats were investigated. Various concentrations (10 to 800?g/mL) of CATUAMA (a mixture of Trichilia Catigua, Paullinia Cupana, Ptychopetalum Olacoide and Zingiber Officinale) and each herbal extract were infused into isolated papillary muscle bath. Developed tension (DT), tension at rest (RT) and velocities of increase and decrease tension (dT/dtmax and dT/dtmin) were analyzed. A single 200?g/mL dose was injected into jugular vein in situ. Left ventricular (LV) function index (dP/dtmax and dP/dtmin), LV systolic and end-diastolic pressures (LVSP and LVEDP), coronary driving pressure (CDP), systolic, diastolic and mean blood pressures (SBP, DBP and MBP) and heart rate (HR) were monitored during 10min by LV catheterization. CATUAMA affected myocardium contractility in the higher and toxic dose, decreasing 9% DT and 16% TR and increasing 90% dT/dtmin. CATUAMA also improved 16% SBP, DBP, MBP and CDP. Its isolated extracts showed different actions. T catigua demonstrated no interference in the variables. P cupana decreased contractility and LVEDP. It improved hemodynamic parameters as CATUAMA. P olacoide decreased DT, TR and LVEDP. Z officinale also decreased DT and TR. CATUAMA and T catigua did not affect cardiac function. These are important results for development of a new drug to prevent and to reverse VF without heart function impairment.

INTRODUÇÃO: Sepse e choque séptico são doenças comuns em pacientes gravemente enfermos, evoluindo muitas vezes com síndrome de disfunção de múltiplos órgãos (SDMO) e morte. Este trabalho investiga a eficácia da administração precoce de dexametasona em evitar a progressão do choque séptico para SDMO e morte e o comportamento da proteína amilóide A sérica (SAA) e da proteína C-reativa (PCR) como marcadores da evolução e gravidade dos pacientes em choque séptico no período pós-operatório. MÉTODOS: Estudo prospectivo, aleatório, duplamente encoberto, realizado na Unidade de Terapia Intensiva pós-operatória do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo, com 29 pacientes que no período pós-operatório evoluíram com choque séptico. Os participantes foram divididos de forma aleatória em dois grupos, de acordo com a solução administrada: dexametasona 0,2 mg/kg (grupo D) ou placebo (grupo P), repetidas a cada 36 horas. Os pacientes foram acompanhados durante sete dias de internação na UTI através do escore SOFA (Sequential Organ Failure Assessment) e dosagens séricas diárias de PCR, SAA e lactato. RESULTADOS: Os pacientes do grupo D, quando comparados aos pacientes do grupo P, permaneceram mais dias sem necessidade do uso do vasopressor (respectivamente 2,9±2,7 e 0,7±0,6, p=0,01) e mais tempo livre de ventilação mecânica (respectivamente 2,6±2,5 e 0,6±0,5, p=0,03). A mortalidade no grupo P foi de 67% (10 em 15) e no grupo D foi de 21% (3 em 14) (Risco Relativo=0,31, IC95% 0,11-0,88). Os valores de PCR e SAA apresentaram forte correlação durante o período de observação (R=0,91/p=0,002). PCR e SAA não tiveram correlação com o escore SOFA (respectivamente R=0,71/p=0,05 e R=0,52/p=0,18), nem com o lactato (p=0,88 e p=0,67). CONCLUSÕES: O tratamento precoce com dexametasona nos pacientes com choque séptico reduziu a mortalidade em 7 dias de acompanhamento. Os níveis séricos de PCR e SAA apresentaram-se elevados nos pacientes em choque séptico e tiveram forte correlação, porém não foram preditores de disfunção orgânica nem de mortalidade. INTRODUCTION: Sepsis and septic shock are a very common condition in critically ill patients, leading to multiple organ dysfunction syndrome (MODS) and death. This study aimed at investigating the efficacy of early administration of dexamethasone in patients with septic shock in order to block the evolution to MODS and death. It also evaluated serum amyloid A protein (SAA) and C-reactive protein (CRP) as evolution and organ dysfunction markers in postoperative septic shock patients. METHODS: Prospective, randomized, double-blind study, developed in a surgical intensive care unit of Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo that involved 29 patients with septic shock. All eligible patients were prospectively randomized to receive either a dose of 0.2 mg/kg of dexamethasone (group D) or placebo (group P), repeated every 36 hours intervals. Patients were monitored over a 7- day period by Sequential Organ Failure Assessment score (SOFA) and daily measurements of CRP, SAA and lactate. RESULTS: Patients treated with dexamethasone had more vasopressor therapy-free days (2.9±2.7 versus 0.7±0.6, p=0.01) and more mechanical ventilation-free days (2.6±2.5 e 0.6±0.5, p=0.03). Mortality in group P was 67% (10 in 15) and in group D was 21% (3 in 14) (Relative Risk=0.31, 95%CI 0.11 to 0.88). CRP and SAA were strongly correlated during the 7 day period of observation (R=0.91/p=0.002). CRP and SAA did not correlate with SOFA (respectively R=0.71/p=0.05 and R=0.52/p=0.18) and lactate (p=0.88 and p=0.67). CONCLUSIONS: Early treatment with dexamethasone reduced 7-day mortality in septic shock patients. CRP and SAA levels were significantly elevated in septic shock and were strongly correlated to each other, but did neither correlate with organ dysfunction nor predict mortality.

INTRODUÇÃO: No Brasil, são poucos os estudos sobre a freqüência das doenças tireoidianas e a maioria limita-se a populações específicas. OBJETIVOS: O objetivo principal deste estudo é correlacionar função tireoidiana subclínica a aspectos ultra-sonográficos em mulheres que realizaram rastreamento para função tireoidiana em local de trabalho. MÉTODOS: Estudo transversal de rastreamento com 135 mulheres de 40 anos ou mais, funcionárias da Universidade de São Paulo. Todas as participantes responderam a questionário específico, foram submetidas à mensuração de medidas antropométricas e da pressão arterial, tiveram uma amostra de sangue colhida para avaliação de função tireoidiana e anticorpos anti-tireoperoxidase, glicemia de jejum, colesterol total, HDL-colesterol, triglicérides e a proteína C ultra-sensível e foram submetidas à ultrasonografia da tireóide. Em uma subamostra do estudo foi realizada punção aspirativa por agulha fina guiada por ultra-sonografia de nódulos tireoidianos que preenchiam critérios de punção. RESULTADOS: As freqüências de hipotireoidismo e hipertireoidismo subclínico foram, respectivamente, 7,4% e 2,2%. A frequência de função tireoidiana normal foi 90,4%. Os valores médios de TSH foram diferentes nestes três grupos (P < 0,0001). Os níveis de anti-TPO foram significativamente diferentes entre os grupos, sendo mais elevados nos dois grupos de doença subclínica (P < 0,0001). O volume médio da tireóide no grupo do hipotireoidismo subclínico foi 8,9 ± 5,2 cm3, seguido pelo grupo do hipertireoidismo subclínico (8,9 ± 1,9 cm3) e pelo grupo das mulheres com função tireoidiana normal (7,7 ± 2,5 cm3) (P= 0,32). Não houve diferenças significativas entre os três grupos quanto à hipoecogenicidade da tireóide (P = 0,64). Entretanto, a ecotextura tireoidiana foi significativamente diferente entre os grupos (P = 0,02). Não foram encontradas diferenças significativas entre os grupos quanto à presença de nódulos císticos, sólidos ou mistos, nem quanto ao número médio de nódulos nas participantes que tinham algum nódulo à ultra-sonografia. Em 14% das mulheres houve indicação de biópsia dos nódulos e em duas foi diagnosticado carcinoma papilífero (1,4% da amostra). Houve uma associação entre ecotextura heterogênea e aumento dos níveis de anti-TPO (P < 0,0001) e uma associação limítrofe entre hipoecogenicidade e presença de anti-TPO (P = 0,051). O volume médio da tireóide no grupo das pacientes com anti-TPO elevado foi de 9,5 ± 4,5 cm3 e no grupo das pacientes com anti-TPO normal foi de 7,5 ± 2,5 cm3 (P = 0,02). A função tireoidiana e os aspectos ultra-sonográficos das mulheres procedentes de zona de bócio endêmico na infância não foram estatisticamente diferentes dos resultados encontrados em mulheres procedentes de zonas iodo-suficientes. CONCLUSÃO: O estudo mostrou associação entre função tireoidianasubclínica e alteração da ecotextura da tireóide em amostra de funcionárias da Universidade de São Paulo submetidas a rastreamento de função tireoidiana em local de trabalho. Níveis elevados de anti-TPO se associaram à alteração da ecotextura e ecogenicidade. Não houve diferenças em relação ao perfil de função tireoidiana ou aos achados ultra-sonográficos nas mulheres quando estratificadas de acordo com a procedência remota. INTRODUCTION: In Brazil, there are few studies on the frequency of thyroid diseases and the majority is limited to specific populations. OBJECTIVE: The goal of this study is to correlate subclinical thyroid function to ultrasound characteristics in a sample of women submitted to screening of thyroid function at worksite. METHODS: Cross-sectional screening study with 135 women aged 40 years or older, all of them civil servants, working at Universidade de São Paulo. All the women answered specific questionnaires, had anthropometric variables and blood pressure measured, blood analyzed for total-cholesterol, LDL-cholesterol, HDL-cholesterol, triglycerides, fasting glucose, thyroid-stimulating hormone (TSH), free-thyroxine (free-T4), antithyreoperoxidase antibodies (anti-TPO), high-sensitive C reactive protein (hsCRP) and they underwent a thyroid ultrasound. An ultrasound-guided fine needle aspiration biopsy of thyroid nodules was performed in a sub-sample of this group. RESULTS: The frequency of subclinical hypothyroidism and subclinical hyperthyroidism were, respectively, 7,4% e 2,2%. The frequency of normal thyroid function was 90,4%. The mean values of TSH were different among the three groups (P < 0,0001). The levels of anti-TPO were significantly different among the groups, with higher levels within the two subclinical disease groups (P < 0,0001). The average thyroid volume within the subclinical hypothyroidism group was 8,9 ± 5,2 cm3, followed by the subclinical hyperthyroidism group with 8,9 ± 1,9 cm3 average volume and lastly by the normal thyroid function group with 7,7 ± 2,5 cm3 average volume (P= 0,32). There were no significant differences among the three groups regarding thyroid hypoechogenicity (P = 0,64). However, thyroid echotexture was significantly different among the groups (P = 0,02). There were no meaningful differences among the groups neither for the presence of cystic lesions, solid or mixed nodules nor for the average number of nodules of any kind within the group of women with thyroid nodules. Fourteen percent of the women had a nodule biopsy indication and 2 were diagnosed with papillary carcinoma (1,4% of the sample). There was an association between heterogeneous echotexture and high levels of anti-TPO (P < 0,0001) and a borderline association between hypoechogenicity and the presence of anti- TPO (P=0,051). The group with the high levels of anti-TPO showed a mean thyroid volume of 9,5 ± 4,5 cm3 and the group with normal levels of anti-TPO showed a mean thyroid volume of 7,5 ± 2,5 cm3 (P = 0,02). The thyroid function and the ultrasonographic aspects of the women from endemic areas in childhood were not statistically different from the results found in women from iodine sufficient areas. CONCLUSION: This study demonstrated the association between subclinical thyroid function and altered thyroid echotexture in a sample of female civil servants working at Universidade de São Paulo submitted to screening of thyroid function at worksite. It was demonstrated that high levels of anti-TPO were associated with thyroid echotexture and echogenicity alterations. There were no differences relating to the thyroid function profile or regarding the ultrasonographic findings among the women when classified according to their origin.

Paraoxonase (PON) é uma família multigene de enzimas, a qual inclui PON1, PON2 e PON3. Investigações há mais de duas décadas vêm permitindo um melhor conhecimento da função dos genes da paraoxonase, em especial da PON1, no metabolismo de inseticidas organofosforados, lípides oxidados e medicamentos. O principal local de síntese da PON1 é o fígado, e no soro encontra-se mais comumentente associada à HDL-C. Exibe dois principais polimorfismos, posição 55 (L/M) e 192 (Q/R) que estão relacionados ao nível sérico e atividade enzimática respectivamente. A freqüência dos alelos do gene PON1 apresenta considerável variabilidade entre diferentes populações. São escassos os estudos sobre a PON2, porém sabe-se que é expressa em vários tecidos, sugerindo, dessa forma, que essa enzima tenha uma ação localizada (intracelular). Dois polimorfismos são os mais estudados no gene PON2, posições 148 (A/G) e 311 (C/S) e têm sido associados à numerosas condições fisiopatológicas como variações no metabolismo e níveis plasmáticos de lipoproteínas e glicose. Este trabalho tem por objetivos caracterizar as freqüências das mutações 192 (Q/R) e 55(L/M) no gene da PON1 e 311(C/S) e 148(A/G) no gene da PON2, bem como analisar a atividade das isoformas da enzima PON1 em uma população brasileira, da cidade de São Paulo, de diferentes origens étnicas. O estudo foi realizado entre 2005 e 2006 com 179 doadores de sangue, classificados etnicamente. Foi coletado sangue para extração do DNA genômico, para posterior determinação dos polimorfismos, através da técnica de PCR e soro para a determinação da atividade basal sérica da enzima paraoxonase. Os genótipos LL (46,4%) e LM (45,2%) na posição 55 (L55M) e QR (49,2%) na posição 192 (Q192R) do gene PON1 são os mais freqüentes na população total. Entre os doadores brancos, os genótipos mais freqüentes foram LM (51,9%) posição 55 e QR (45,6%) na posição 192. No caso dos doadores mulatos, LL (50,0%) e QR (52,8%) são os genótipos mais observados e para os doadores negros, os genótipos LL (69,7%) e QR (50,0%) nas posições 55 e 192 respectivamente. O alelo L é o mais freqüente nos três grupos étnicos, no entanto, em relação a freqüência alélica do polimorfismo da posição 192, o alelo Q predomina entre brancos e mulatos, já para os negros o alelo R é mais freqüente. No gene PON2, os genótipos mais freqüentes na população são AA (54,2%) na posição 148 (A148G) e CS (52,5%) na posição 311 (C311S). Quando comparadas as freqüências genotípicas do gene PON2 no polimorfismo da posição 148 (A/G) entre os três grupos étnicos, o genótipo AA foi o mais freqüente, em brancos (60,7%), mulatos (50,0%) e em negros (46,4%). Já para o polimorfismo da posição 311 (C/S), os doadores brancos têm o genótipo CS (46,8%) e SS (46,8%) como o de maior freqüência e nos doadores mulatos e negros o genótipo mais freqüente é CS com 56,9% e 57,1% respectivamente. Não houve diferença na distribuição alélica dos três grupos étnicos, sendo os alelos A e S os mais freqüentes. Em relação à atividade da enzima, as isoformas resultantes dos genótipos LL (posição 55) e RR (posição 192) apresentaram os valores das medianas significantemente maiores que as demais isoformas, sendo mais eficazes na hidrólise do paraoxon. Paraoxonase (PON) is a multigene family of enzymes that include PON1, PON2 and PON3. Investigations at more than two decade coming to allow the best understanding of the function of the paraoxonase genes, in special of the PON1 in the metabolism organophosphate insecticides, oxidized lipids and drugs. The main place of PON1 synthesis is the liver, and in the serum is currently associated to HDL-C. Show two main polymorphisms, in the position 55 (L/M) and 192 (Q/R) that are relation with serum level and enzymatic activities, respectively. The allele frequency of PON1 genes shows variability in different populations. There is a few studies about PON2, but it is known that is expressed in many tissues, suggesting, the enzyme have a local action (intracellular). Two polymorphisms are the most studied in the PON2 gene, 148 (A/G) and 311 (C/S) positions and they have been associated to a lot of physiologic conditions like metabolisms chances and lipoprotein and glucose plasma level. This work have the objective to characterizer the frequency of 192 (Q/R) and 55 (L/M) mutation in the PON1 gene and 311 (C/S) e 148 (A/G) mutations in the PON2 gene as well as to analyze the enzyme PON isoform activity in the brazilian population of São Paulo City, in different ethnics groups. The study was realized during 2005 to 2006 in 179 blood donor classificated according to the ethnics. It was colleted the blood to DNA genomic extraction after to polymorphisms determination was utilized the PCR technique and the serum to determine the paraoxonase enzyme basal activity. The genotypes LL (46,4%) and LM (45,2%) in the 55 (L55M) positions and QR (49,2%) in the 192 (Q192R) position in the PON1 gene are the most frequently in the total population. Among the white donor, the genotypes most frequently were LM (51,9%) in the 55 position and QR (45,6%) in the 192 position. In mulatoes, LL (50,0%) and QR (52,8%) are the most observed genotypes and black donor, the genotypes LL (69,7%) and QR (50,0%) in the 55 and 192 positions, respectively. The L allele is the most frequently in the three ethnics groups, however, the relation of polymorphism allelic frequency in the position 192, the Q allele to predominate among mutates, and to the negroes the allele R is the most frequency. In the PON 2 gene, the most frequently genotypes are AA (54,2%) in the 148 (A148G) position and CS (52,5%) in the 311 (C311S) position. When they are compared with PON2 genotypes frequency in the polymorphisms of 148 (A/G) position among three etnics groups AA was the most frequently, in the white (60,7%), mulatoes (50,0%) and negroes (46,4%). To the polymorphisms at 311 (C/S) positions is the most frequently in white donors have a genotype CS (46,8%) and SS (46, 8%) and in the mulatoes and negroes donors the most frequency genotypes is CS with 56,9% and 57,1% respectively. There aren?t differences in the allelic distribution in the three ethnics groups, the A and S allelic are the most frequently. In relation to enzyme activity, the product of isoform to LL (55 position) and RR (192 position) genotypes to present the median level are significantly more than another isoforms, they are more efficient in the paraoxon hydrolyzes

Futebol é um esporte de natureza intermitente, consistindo de séries repetidas (6-segundos) de trabalho rápido de alta intensidade e curta duração e de períodos de recuperação de intensidade baixa a moderada (60-segundos). O objetivo deste estudo foi caracterizar a antropometria, o consumo alimentar, o desempenho e as respostas bioquímicas, moleculares e celulares de jogadores de futebol no teste de esforço máximo e determinar se a reposição aguda de glutamina altera essas variáveis. Quatorze jogadores de futebol (idade: 18 e 21 anos), de time profissional, representativos dos atletas dessa modalidade e faixa etária, foram avaliados. A avaliação antropométrica mostrou que esses jogadores apresentavam altura, peso e percentual de gordura, compatíveis com a idade e modalidade. A avaliação do consumo alimentar mostrou um consumo inadequado de macronutrientes e cálcio segundo IDR e ACSM. O desempenho desses atletas no teste foi dentro do esperado para atletas desta modalidade. O exercício causou acidose metabólica; aumento dos níveis plasmáticos de produtos de peroxidação lipídica, glutamina e glutamato, e aumento dos leucócitos circulantes, sem alterações na glicemia, enzimas musculares, ou proteína do choque térmico 27. A suplementação com glutamina peptídeo não proporcionou melhora no desempenho nem alterou as variáveis analisadas, após o exercício, exceto os produtos da peroxidação lipídica, que diminuíram com a suplementação. Frente aos resultados obtidos, concluiu-se que sem uma alimentação adequada para faixa etária, nível de maturação sexual e modalidade esportiva, mesmo com a prescrição correta de treinamento, a suplementação com glutamina não será capaz de proporcionar efeitos positivos no desempenho físico. Soccer is an intermittent sport in nature, consisting of repeated bouts of brief (6-second) maximal/near-maximal work interspersed with relatively short (60-second) moderate/low-intensity recovery periods. The aim of this study was to characterize anthropometry, food habit, performance, as well as biochemical, molecular and cellular responses of fourteen soccer players (age 18-21) to maximum exercise testing and to determine whether glutamine peptide supplementation changes these variables. The anthropometric and body composition assessment showed that the players presented height, weight and amount of body fat compatible with international sport criteria; however, body fat revealed a great variability. The dietary assessment identified an inadequate macronutrient and calcium consumption, compared to RDI e ACSM recommendations. Athlete\'s performance in maximum exercise testing was close to this modality average. The exercise testing induced a metabolic acidosis, with increased blood lactate levels; increased levels of glutamine and glutamate; increase in lipid peroxidation markers and in white blood cell count, while no change was detectable in blood glucose, muscle enzymes or heat shock protein 27 expression. Glutamine supplementation caused no change either in performance or in biochemical, molecular or cellular variables after aerobic exercise, except for lipid peroxidation markers, which decreased with supplementation. Therefore, it was concluded that without a healthy diet, ajusted to age and sport, glutamine supplement is not able to induce positive effects on performance.

A obesidade tem sido associada com a insuficiência cardíaca, e indivíduos obesos apresentam uma forma de cardiomiopatia caracterizada por dilatação ventricular esquerda, e hipertrofia ecêntrica, atribuída à sobrecarga volêmica. A disfunção ventricular tem sido descrita, sendo que a maioria dos estudos descrevem anormalidades da função diastólica do ventrículo esquerdo. O presente estudo foi desenhado para se determinar o efeito da obesidade sobre a função diastólica do ventrículo esquerdo e o impacto da perda de peso induzida pela cirurgia bariátrica sobre esta função.104 pacientes agendados para cirurgia bariátrica foram incluídos, 74 mulheres e 30 homens. Idade média de 38,8 ± 11,44 anos. Oitenta e um pacientes eram obesos mórbidos ( IMC maior ou igual 40 kg/m²) e 23 pacientes eram obesos graves ( IMC entre 35 e 39,99 kg/m²) com pelo menos duas co- morbidades associadas. A análise ecoDopplercardiográfica estrutural e funcional foi realizada previamente e após 6 meses do procedimento cirúrgico. Encontramos que indivíduos obesos tinham aumento da espessura septal do ventrículo esquerdo, do diâmetro atrial esquerdo e da massa ventricular esquerda. Além disso, apresentavam uma incidência maior de disfunção diastólica no pré-operatório. Observamos que a perda de peso induzida pela cirurgia bariátrica causou impacto nas velocidades do fluxo mitral, nas velocidades do fluxo de veia pulmonar e no Doppler tissular, mas não causou impacto nos outros parâmetros ecoDopplercardiográficos de análise da função diastólica. Como conclusões, obtemos que a perda de peso induzida pela cirurgia bariátrica melhora a função diastólica do ventrículo esquerdo em indivíduos obesos. O fato do paciente ser hipertenso, diabético ou ser portador de outras co- morbidades, não esteve relacionado a uma maior incidência de disfunção diastólica. Os parâmetros ecoDopplercardiograficos de análise da função diastólica do ventrículo esquerdo devem ser utilizados em associação para uma melhor avaliação desta função. Obesity has been associated with heart failure, and individuals with severe obesity have been recognized to have a form of cardiomiopathy attributed to chronic volume overload, characterized by left ventricular dilation, and eccentric left ventricular hypertrophy. Impairment of cardiac function has been reported, with most studies reporting abnormal diastolic function.The present study was designed to determinate the effect of obesity on left ventricular diastolic function and the impact of weight loss after bariatric surgery on this function. One hundred and four patients scheduled for bariatric surgery were enrolled, 74 women and 30 men. Baseline mean age was 38.8 ± 11.44 years. Eighty- one patients were morbid obese (B.M.I. maior ou igual 40 kg/m²) and 23 were severe obese (B.M.I from 35 to 39.99 kg/m²), with at least two associated co-morbidities. Structural and functional echoDopplercardiographic analysis were performed before and 6 months after the bariatric surgery. We found that obese individuals had higher septal wall thickness, left atrial diameter, left ventricular mass and a higher incidence of diastolic dysfunction before the surgery than after. It was observed that weight loss had an impact on mitral flow velocities, on pulmonary vein flow velocities, and on tissue Doppler imaging, but had no impact on the other diastolic echocardiographic parameters. Weight loss after bariatric surgery had improved cardiac diastolic function. Being hypertensive, diabetic or having another associated co-morbidity was not related to a higher incidence of diastolic dysfunction. The diastolic echoDopplercardiographic parameters should be used in combination for a better diastolic evaluation.
