Amaç: Bağımlı değişkenin sayma/kesikli olma durumuyla birçok araştırmada karşılaşılmaktadır. Her ne kadar Poisson regresyon modeli sayma verilerinin analizinde sıklıkla kullanılsa da ortalama ile varyans eşitliği varsayımı birçok deneysel veri için sağlanmamaktadır.Aşırı/az yayılım olarak tanımlanan bu durumlarda alternatif regresyon modelleriyle analizedilmesi daha uygun olmaktadır. Hem aşırı hem de az yayılım durumu için uygun analiz olanağı veren Conway-Maxwell-Poisson Regresyon modeli ayrıntılı olarak tanıtmaktır. Gereç veYöntemler: Poisson, negatif binom ve COM-Poisson regresyon modelleri önce aşırı yayılımgösteren 2011-2014 yılları arasında Giresun ili Toplum Ruh Sağlığı Merkezine (TRSM) kayıtlıolan hastaların, merkeze gelme sayılarına etki eden sosyo-demografik faktörlerin modellenmesinde, daha sonra az yayılım gösteren öğretmenlerin sosyal medya hesap sayılarına etkieden okul türü, cinsiyet ve yaş grubu faktörlerinin modellenmesinde uygulanmıştır. Her ikiveri seti için değişken seçim yöntemi olarak tüm olası alt kümeler yöntemi kullanılmıştır. Enküçük AIC değerli model en iyi model olarak seçilmiştir. Bulgular: TRSM’ye geliş sayılarınacinsiyet, eğitim düzeyi, medeni durum ile sosyal güvence durumunun, sosyal medya hesabına ise okul türü ve yaş grubunun etkili olduğu saptanmıştır. Sonuç: Her iki veri setinde deCOM-Poisson regresyon en iyi modeli oluşturmuştur. Objective: Counting/discrete dependent variable is encountered in many studies.Although the Poisson regression model is frequently used in the analysis of counting data, theassumption of the equality of mean and variance is not provided for many experimental data.It is more appropriate to analyze with alternative regression models in case of over-underdispersion. The Conway-Maxwell-Poisson Regression model which is suitable for analysis ofboth over-under dispersed data is presenting in detail. Material and Methods: Poisson, negative binomial and COM-Poisson regression models were used to determine the effect ofsocio-demographic factors on the number of visits Giresun Community Mental Health Center(TRSM) between 2011 and 2014. Then, three regression models applied to model school type,gender, and age group factors with the number of social media accounts of the teachers. Allpossible subset regression approach was used as a variable selection method for both data sets.The smallest AIC-valued model was selected as the best model. Results: It was determined thatgender, education level, marital status and social security status were effective on the numberof arrivals to TRSM and the type of school and age group on social media account. Conclusion:COM-Poisson regression was the best model in both datasets.

As technology progresses, automatic analyzers substitute manual procedures and reduce analytical errors. However, as the number of test orders inhospitals and correspondingly as the number of sample accessioning in medicalbiochemistry laboratories increase, preanalytical phase of the total continuumbecomes more inclined to errors. At present, vast majority of errors affecting testresults occur in preanalytical phase (1). Among the tests studied, one of the mostmeasured tests is complete blood count (CBC). Therefore, CBC is one of thetests which are numerically affected by the preanalytical errors (2). Managementof preanalytical phase is difficult due to the need of participation of divisionsother than the laboratory. Knowledge about variables impacting CBC test has agreat importance for preventing preanalytical errors. Developing and complyingwith the instructions related to this issue are included in the responsibilities of thelaboratory management. This reference document is prepared with the purposeof performing a useful, practical and applicable guideline for all medical laboratory staff, nurses, phlebotomists, students and researchers.

INTRODUCTION: Measurement of parathyroid hormone (PTH) is essential in the investigation and management of calcium metabolism disorders. To assess the significance of any assay result when clinical decision making biological variation (BV) of the measurand must be taken into consideration. The aim of the present study is determining the BV parameters for serum PTH. MATERIALS AND METHODS: Blood samples were taken at weekly intervals from 20 healthy subjects for ten weeks in this prospective BV study. Serum "intact PTH" concentrations were measured with electrochemiluminescence method. Biological variation parameters were estimated using the approach proposed by Fraser. RESULTS: The values of within-subject biological variation (CV(I)), between-subject biological variation (CV(G)), analytical variation (CV(A)), reference change value (RCV) and individuality index (II) for serum PTH were 21.1%, 24.9%, 3.8%, 59.4% and 0.8%, respectively. Within-subject biological variation and CV(G) were also determined according to gender separately; 18.5% and 24.0%; 26.2% and 18.6% for male and female, respectively. Calculated desirable precision and bias goals were < 10.6% and < 6.3%, respectively. CONCLUSION: This study may contribute to BV data on serum PTH as it includes a sufficient number of volunteers from both genders over an acceptable period of time. We do not recommend the usage of population-based reference intervals for serum PTH concentrations. Reference change value may be helpful for the evaluation of serial serum PTH results. Nonetheless, evaluation of data according to gender is necessary when setting analytical performance specifications.

Üriner sistem enfeksiyonları (ÜSE) gebelikte en sık görülen enfeksiyonlardandır. Gebeliktemekanik ve hormonal faktörlerin yol açtığı fizyolojik değişiklikler nedeniyle idrar yolu enfeksiyonuriski artar. Ayrıca mesanenin yetersiz boşalması nedeniyle, idrarın bir kısmı burada kalarak bakteriüremesine zemin hazırlar. Bu çalışmada amacımız 1. basamak aile hekimlerine başvurangebelerde ÜSE sıklığını belirlemek ayrıca tam idrar tetkiki (TİT) ve kültür sonuçları arasındakiuyumu saptamaktır.Yöntem: Laboratuvar bilgi sisteminde 2014 yılı süresince Aile Sağlığı Merkezleri’nden (ASM, birincibasamak sağlık merkezleri) Halk Sağlığı Laboratuvarı’na gönderilen, gebelere ait toplam 1295TİT ve idrar kültür istemi saptanmıştır. Laboratuvarımıza kabul edilen ve eş zamanlı kültür ile TİTistemi bulunan 1191 örnek çalışmaya alınmıştır. TİT, Labumad Urised (77 Electronica, Budapest,Hungary) cihazında yapılmıştır. Kültür çalışması konvansiyonel yöntemler ile mikrobiyoloji uzmanıtarafından gerçekleştirilmiştir. Kültür altın standart olarak kabul edilerek TİT-kültür karşılaştırmasıgerçekleştirilmiştir.Bulgular: Kültür analizi sonucu üremesi olan 107 numunenin 41’inde TİT sonucunda pozitif özelliksaptanmıştır. Bunlar bakteri/lökosit varlığı ve nitrit pozitifliğidir. Kültürlerde 92 Escherichia coli,sekiz Klebsiella spp., beş Proteus mirabilis ve iki diğer idrar yolu patojeni saptanmıştır. Örneklerin292’sinde (% 24.5) kontaminasyon saptandığından örnek tekrarı istenmiş,768 örnekte ise üremeolmamıştır.Sonuç: Çalışmamızda TİT’in duyarlılığının düşük özgüllüğünün yüksek olduğu görülmüştür. Bunedenle, gebelerde tek başına idrar yolu enfeksiyonunu dışlamakta TİT yetersiz kalmış ve gebelerdetüm gebelik süreci boyunca mutlaka en az bir kez idrar kültür istemi yapılması yararlı olabilecektir.Ayrıca, kontaminasyon sayılarının yüksek olması, idrar kültür test değerlendirilmesindepreanalitik sürecin önemli bir yer tuttuğunu çalışmamız bir kez daha göstermiştir.

Sürekli verilereait çalışmaların birleştirilmesinde meta-analiz uygulanabilmesi için ortalamave standart sapmaya gerek olmaktadır. Bu çalışmada, yayımlanan çalışmalarınmedyan, aralık ve örneklem hacmi olarak verilmesi durumunda ortalama vestandart sapmanın tahmin edilmesi için geliştirilen dönüşüm yöntemleri elealınmıştır. Uygulama olarak erken evre meme kanserli hastalara en çok uygulananiki tedavi yönteminin tercihinde yaş faktörünün etkisi incelenmiştir. Elealınan dönüşüm yöntemleri kullanılarak meta-analizine dahil edilen çalışmasayısı 9'dan 17'ye çıkarılmış ve örneklem hacmi arttırılarak daha etkinmeta-analizi elde edilmiştir. Analiz sonucunda, yaş faktörünün tedavi yöntemitercihinde önemli olmadığı sonucuna varılmıştır., To perform meta-analysis of continuous data,meta-analyses need the mean and the standart deviation. In this paper, theconversion methods that improved to estimate the mean and the standartdeviation from published studies that report median, range and sample size werediscussed. The impact of age on preferring the most used surgery type for earlystage breast cancer patients was analyzed. Using the conversion methods, thenumber of studies included in the meta-analysis was increased from 9 to 17 andthe sample volume was increased to obtain more efficient meta-analysis. As aresult of the analysis, it was concluded that the age factor was notsignificant in the choice of treatment.

Enerji içeceği kullanımı genç nüfusta özellikle son yıllarda artış göstermektedir. Bu tür içeceklerin sağlığa faydalarının yanı sıra zararları da bulunmaktadır. Bu çalışmanın amacı, Türkiye'deki enerji içeceği kullanım sıklığını belirlemektir. Bu amaçla Türkiye'de enerji içeceği kullanımıyla ilgili yapılan çalışmalar derlenerek klasik ve Bayesci meta-analizi ile analiz edilmiştir. Analiz sonucunda Bayesci yaklaşımın daha kesin sıklık tahmin değeri verdiği saptanmıştır. The consumption of energy drinks has increased in recent years, especially among the young population. In addition to the health benefits of such beverages, they also have damages. The aim of this study is to determine the frequency of energy drink consumption in Turkey. For this purpose, studies on energy drink consumption in Turkey have been compiled and analyzed by classical and Bayesian meta-analysis. As a result of the analysis, the Bayesian approach was found to give a more accurate frequency estimation value.

Amaç: Tıbbi laboratuvar hatalarının %70’inden fazlasıpreanalitik evrede gerçekleşmektedir. Hataların analizi vekayıt altına alınması, durum değerlendirilmesi ve hatalarınönlenebilmesi için gereklidir. Çalışmamızda mikrobiyolojilaboratuvarı preanalitik evre hataları değerlendirilerek altısigma yaklaşımı ile değerlendirilmesi sınıflanmasıamaçlandı.Gereç ve Yöntem: Tıbbi Mikrobiyoloji Laboratuvarı’ndaanaliz için uygun olmadığı saptanan numuneler, hatakategorileri, örneklerin geldiği hastane birimleri ve analizigerçekleştirilen laboratuvar alanları olmak üzere üç anabaşlık altında sınıflandırıldı. Bunun için milyonda hataoranı saptanarak sigmametrik hesaplama yapıldı.Hesaplanan değerler ≥5.0 ise çok iyi, 4.0

Aim: Biological variation (BV) of the HbA1c test can be affected by analysis method, sampling period, and disease activity. The aim of the present study was to examine the impact of CVI, CVA, and II for Tosoh G7 system. Material and Method: 96 samples from 12 healthy volunteers were collected for every two weeks during a two-month period. HbA1c levels of all samples were measured by G7 HPLC (non-porous ion-exchange) Analyser. Biological variation parameters CVI, CVA, CVG, RCV, and II were calculated. Results: The mean of HbA1c was 5.10% (32.3 mmol/mol). CVA was calculated as 2.81%. The CVI and CVG for HbA1c were 1.69% and 7.56%, respectively. RCV and II values were 9.08% and 0.22. Discussion: According to our results, the sampling period and study method were fundamental to determining biological variation parameters. When using the Tosoh G7 system, we observed similar results to other methods, e.g. boronate affinity or immunoturbidimetry. Low individuality index of HbA1 c presented that the analyte had marked individuality and that the population-based reference values had low utility for disease monitoring. Thus, estimation and inclusion of RCV in laboratory reports for HbA1c will guide the clinicians in evaluating the importance of difference between serial results and glycemic status of the patients.

PubMed: 29607822 AIM: The aim of this retrospective study was to analyze 6,613 diagnosed cancer cases in the Black Sea Turkish province of Giresun over a period of 12 years. MATERIAL AND METHOD: Data of this retrospective crosssectional study was collected from 6,613 cancer patients at Prof. Dr. A. Ilhan Özdemir Education and Research Hospital between 2005- 2016. MINITAB 15 package software was used for statistical analysis. Chi-Square test and differences between two ratios significance test were used to calculate the relationship between two nominal (categorical) variables. RESULTS: Total 6,613 cancer patient's (3,759 men, 56.8% and 2,854 women, 43.2%) average age is 66.27±17.17. The most frequent types of cancer found among men included prostate (24.4%), bronchus-lung (12.5%) and colorectal (11.15%) cancer; whilst among for women it was breast (26%), thyroid (14.9%) and skin (10.6%) cancer, respectively. Amaç: Histerektomi materyallerinde adenomyozis sıklığının saptanması amaçlanmıştır. Gereç ve Yöntemler: Çalışmamızda Bakırköy DrSadi Konuk Hastanesi Kadın Hastalıkları ve Doğum Kliniği'nde benign nedenlerle histerektomi yapılan 255 hastanın postoperatif histerektomi materyallerinin histopatolojik değerlendirmelerinde adenomyozis sıklığı ve bu vakaların demografik özellikleri incelendi. Bulgular: Bu çalışmada toplam 255 histerektomi materyali incelendi ve 48 tanesinde (%19,2) adenomyozis varlığı tespit edildi. Sonuç: Adenomyozis tanısı, değişik nedenlerle kliniğe başvuran hastalarda tedavi planlaması esnasında akılda tutulmalıdır Objective: To determine the frequency of adenomyosis in hystrectomy materials.Materials and Methods: We here in analyse the adenomyozis frequency in histopathologic evaluation of 255 hysterectomy materials performed due to bening causes in Bakırköy Dr Sadi Konuk Hospital Obstetric and Gynecology Clinic and demographic properties of these patients are evaluated. Results: 255 hysterectomy materials are evaluated in this study and 48 adenomyosis (%19.2) case are determined. Conclusion: Adenomyosis must be remembered in mind in patients admitted to clinic due to different causes and before managing treatment.

Count data become widely available in many diciplines. The most popular distribution for modeling count data is the Poisson distribution which assume equidispersion (Variance is equal to the mean). Since observed count data often exhibit over or under dispersion, Poisson models become less ideal for modeling. To deal with a wide range of dispersion levels, Quasi Poisson regresion, Negative Binomial regression and lately Conway-Maxwell-Poisson (COM-Poisson) regression used as an alternative regression models. Tez (yüksek lisans) -- Ondokuz Mayıs Üniversitesi, 2005 Libra Kayıt No: 16933 Genelleştirilmiş lineer modeller fen ve mühendislik alanlarında sıkça karşılaşılan kesikli veya sürekli verilerin analizine olanak veren modellerden biridir.Lineer modellerde bağımlı değişken ile bağımsız değişkenler arasındaki ilişkinin doğru biçimde belirlenmesinde kullanılan çeşitli istatistiksel ve tanısal grafik yöntemler bulunmaktadır.Bu çalışmada bu yöntemlerin Genelleştirilmiş Lineer Modellerde de uygulanabilirliliği incelenmiştir. Genelleştirilmiş Lineer Modellerde tanısal grafik yöntemlere bağımlı değişken ile bağımsız değişkenleri ilişkilendirilen bağıntı fonksiyonunun doğru belirlenip belirlenmediğini özellikle bağıntı fonksiyonunda yer alan bağımsız değişkenlerdeki ilişkinin biçimini ortaya çıkarmak için başvurulduğu gösterilmiştir.Bu amaçla uygulanan bazı tanısal grafik yöntemlerin bağımsız değişkenler arasında mevcut olabilecek çoklu birlikte değişim probleminden dolayı doğru sonuçlar vermeyeceği gösterilmiştir.Bu duruma çoklu bağlantı problemi olmayan ve olan iki sayısal veri türetilerek değinilmiştir.Ayrıca 1965-1998 yılları arasındaki grev sayılarını sendikalaşma oranı ücret oranı işyeri ve kapanan işyeri sayıları ile analiz ederek en uygun model seçilmeye çalışılmıştır.
