Introducción: Las lesiones raquídeas son frecuentes en hombres adultos jóvenes. Reconocer la cinemática ayuda a disminuir la tasa de diagnósticos tardíos, principalmente si hay trastornos de la conciencia. Las fracturas vertebrales pueden ser únicas o múltiples, y asociarse con lesiones extravertebrales. Los objetivos de este estudio fueron analizar la distribución de la lesión según el mecanismo de producción, caracterizar el cuadro neurológico, evaluar el patrón de lesión y la asociación con lesiones extravertebrales, y analizar el tratamiento. Materiales y Métodos: Estudio multicéntrico, prospectivo de pacientes con lesiones vertebrales postrauma, que ingresaron entre el 1 de julio de 2018 y el 30 de junio de 2020. Se analizaron los siguientes parámetros: edad, sexo, cinemática, cuadro neurológico, sector afectado, patrón de lesión, lesiones extravertebrales asociadas. Resultados: Se evaluó a 281 pacientes (60% hombres) con 400 lesiones vertebrales y 118 extravertebrales que, en 62 casos, conformaban un cuadro de politraumatismo; 147 con trauma por caída de altura y 98, por accidente de tránsito. El cuadro neurológico más observado fue ASIA E (8 casos), no determinado al ingreso. El sector T2-L5 fue el más afectado, en su mayoría, por lesiones por compresión. Las lesiones extravertebrales más frecuentes fueron el trauma de cráneo y de tórax; hubo un caso de SCIWORA y un óbito temprano. Conclusiones: Los sectores raquídeos más afectados fueron: el toracolumbar, el torácico y el lumbar; las lesiones suelen deberse a caídas de altura y suelen ser únicas, sin cuadro neurológico. El tratamiento se decide según la estabilidad y el cuadro neurológico.

Background: The study evaluated the effects of regular blood donation on serum transferrin and soluble transferrin receptor levels at Wenchi Methodist Hospital. Methods: This was a hospital-based cross-sectional study conducted at the Medical Laboratory Department of the Wenchi Methodist Hospital in the Bono Region of Ghana. A total of eighty-nine (89) venous blood samples from apparently healthy blood donors were analyzed. Introducción: Las fracturas vertebrales múltiples son el resultado de traumatismos de alta energía. Se clasifican en contiguas cuando hay fractura de dos o más cuerpos vertebrales en forma adyacente y no contiguas cuando hay lesiones de varios niveles separadas por, al menos, una vértebra sin lesión. Objetivos: Evaluar la clínica y la cinemática de las fracturas vertebrales múltiples, establecer su distribución, evaluar la asociación con lesiones extravertebrales y detallar las complicaciones. Materiales y Métodos: Estudio multicéntrico, prospectivo y observacional de pacientes con fracturas vertebrales múltiples. Resultados: 66 pacientes (26 mujeres y 40 hombres; promedio de edad 39 años) con 196 lesiones vertebrales, 55 no tenían déficit neurológico. Cinemática: 32 accidentes automovilísticos, 27 caídas de altura, 5 traumas directos y 2 traumas deportivos. Topografía: dos C0-C3, cuatro C3-C7, ocho C7-T1, 61 T1-T10, 91 T10-L2, 28 L2-L5 y una sacro. Combinación: 21 toracolumbar-toracolumbar y 8 torácica-torácica. Treinta y dos fracturas eran contiguas; 19, no contiguas y 15 tenían un patrón mixto. Veintiséis pacientes presentaron 47 lesiones extravertebrales (20 politraumatismos), 12 traumas torácicos. Treinta y seis pacientes recibieron tratamiento conservador y 30 fueron operados. Conclusiones: La presencia de fracturas vertebrales múltiples es frecuente y son consecuencia de accidentes automovilísticos, la mayoría de los pacientes no tiene compromiso neurológico. El sector más afectado fue entre T2 y L5, la combinación más frecuente fue toracolumbar-toracolumbar, seguida de torácica-torácica. Las lesiones extravertebrales más frecuentes fueron traumatismos de cráneo y de tórax.

Art For Children (AFC) adalah sebuah program yang bentuk oleh Taman Budaya Yogyakarta. Tercatat sebanyak 7 cabang seni yang diajarkan di dalamnya, salah satunya ialah teater. Proses kreatif teater pada AFC ini cukup menarik karena tidak menggunakan teks. Penelitian ini mencoba melihat upaya yang dilakukan guru selama proses kreatif teater di kelas AFC. Peran tersebut tidak hanya tentang otoritas guru dalam kelas teater, tetapi juga cara guru memaknai dan melaksanakan perannya pada setiap tahapannya. Metode penelitian yang digunakan ialah kualitatif, dengan metode pengumpulan data wawancara semi terskturktur, dokumen, dan studi pustaka. Hasil penelitian menunjukkan bahwa dalam pelaksanannya, guru membagi dua tahapan yakni training dan rehearsal. Training yang berarti melatih keterampilan dasar anak dengan cara melakukan observasi di lingkungan sekitar, olah tubuh dan olah rasa. Kemudian berlanjut pada tahapan inti proses kreatif yakni rehearsal yang berarti melatih dengan mencapai tujuan tertentu atau dalam hal ini ialah mencipta naskah bersama-sama.

ABSTRACT Background: Pyogenic Spinal Epidural Abscess (PSEA) is difficult to diagnose and can have devastating consequences. Magnetic Resonance Imaging (MRI) has high sensitivity and specificity, which are further increased with the use of contrast. There are several classifications of vertebral infectious processes, with emphasis on spondylodiscitis. Objective: To analyze the morphological parameters and their reproducibility; and to analyze different resonance imaging sequences. Methods: Using an image database, a morphological classification of PSEA was planned, with five parameters: Region (R), indicating the upper and lower limits of the abscess; Location (U), indicating whether the abscess is anterior or posterior within the canal; Compromise (C), meningeal or content of the structures; Association (A), discitis, osteomyelitis or both; and Perivertebral (P), anterior, lateral or posterior extravertebral abscess. The first three parameters give an idea of the volume of the PSEA, while the last two give the related infectious foci. Thirty-five cases were analyzed using Kappa’s coefficient. Results: The global intra- and interobserver reproducibility was Kappa 0.81. The results for each parameter were as follows: R=0.95, U=0.92, C=0.66, A=0.70 and P=0.80. The first three give a notion of volume and the last two relate to the presence of vertebral infectious foci outside the canal. T2 weighted MRI with contrast was found to be the most effective imaging sequence. Conclusion: The morphological classification is simple to use, with excellent reproducibility. The parameters with the highest reproducibility were region and location, with values >0.92. The addition of gadolinium contrast increased the sensitivity of the diagnosis; the use of sagittal and axial images in T2-MRI was the most sensitive imaging sequence. Evidence Level III; Original.

ABSTRACT Objective: Intradiscal vacuum phenomenon (IVP) is a common finding in the study of degenerative disc disease. Discogenic low back pain can be manifested in different ways, including irradiation to the lower limbs. This study aims to acknowledge the complementary studies used to diagnose IVP, determine their sensitivity, and assess the correlation between clinical and imaging findings. Methods: This is a descriptive, observational and prospective study involving clinical and imaging evaluation of 100 patients with IVP, using dynamic and plain radiographs, computed tomography and magnetic resonance imaging. The factors of analysis include sex, age, reason for consultation, visual analogue scale, irradiation and topography of the pain, the existence of sciatica and claudication, smoking status, and body mass index. Results: The overall average age of the patients was 64.6 years, who particularly evidence degenerative pathology. IVP was observed in 99 CT, 85 dynamic radiographs, 80 plain radiographs and 65 nuclear magnetic resonance images (MRI). Conclusion: The most useful studies for diagnosing the vacuum disc phenomenon are plain and dynamic radiographs, tomography, and magnetic resonance. The CT is the most sensitive imaging study for IVP detection, followed by dynamic radiographs obtained during extension. A correlation was observed between older age, overweight, and IVP. This study describes the taxonomic composition and horizontal distribution of Serranidae larvae along the Southeastern Brazilian Bight (SBB, approximately 22°S and 28°S), and investigates how environmental and biological factors influence the larval distribution. Ichthyoplankton samples were collected in 10 oceanographic cruises conducted from 1975 to 1981. Serranidae larvae were identified into 15 species, including 11 genera and four subfamilies. Except for Diplectrum formosum, which was widely distributed throughout the SBB, Hyporthodus spp. and Baldwinella vivanus were distributed in the southern portion of the SBB, while other species were distributed mainly in the northern portion of the SBB, inside the 200 m isobath. Local depth, latitude, distance from the closest island and salinity were factors that influenced significantly on the distribution and abundance of the most abundant taxa D. formosum, Diplectrum spp., Serranus spp. and Mycteroperca spp. Significant progress was made in identifying grouper larvae from the SBB (Epinephelus morio, Epinephelus itajara and Hyporthodus nigritus). ABSTRACT Objective: To present the clinical case and update the bibliography. Methods: A male patient, 24 years of age, sought treatment for right lumbosciatalgia of 3 years of evolution with topography L5 and motor deficit (M4). The radiograph showed a radiopaque lesion between the fourth and fifth lumbar vertebrae, with right pedicle effacement of L4. The tomography identified a lytic lesion, partially surrounded by sclerosis with a central nest of 3 centimeters in diameter located in the right pedicle with involvement of the transverse apophysis and reaction of the intertransverse space (Enneking 3). It was complemented by magnetic resonance and bone scintigraphy. The percutaneous biopsy guided by tomography yielded a diagnosis of osteoblastoma and foci of necrosis. A radical block resection was performed with clear tumor margins and instrumented stabilization. Results: After the surgical treatment, the patient evolved favorably, reversing the motor deficit. The anatomopathological study of the specimen confirms the preoperative diagnosis. Discussion: Intralesional resection may be an option in Enneking stage 2. In Enneking stage 3, a percutaneous diagnostic biopsy may be useful, and block resection is the preferred definitive treatment. Conclusions: The management of spinal osteoblastoma requires an exhaustive clinical-imaging analysis. Block resection with clear margins is preferred in advanced cases for management and to decrease the risk of recurrence. Level of Evidence IV; Case seriesh.

We investigated the minimum expressive intensity that is required to recognize (above chance) static and dynamic facial expressions of happiness, sadness, anger, disgust, fear, and surprise. To this end, we varied the degree of intensity of emotional expressions unfolding from a neutral face, by means of graphics morphing software. The resulting face stimuli (photographs and short videos) were presented in an expression categorization task for 1 s each, and measures of sensitivity or discrimination (A') were collected to establish thresholds. A number of physical, perceptual, categorical, and affective controls were performed. All six basic emotions were reliably recognized above chance level from low intensities, although recognition thresholds varied for different expressions: 20% of intensity, for happiness; 40%, for sadness, surprise, anger, and disgust; and 50%, for fear. The advantage of happy faces may be due to their greater physical change in facial features (as shown by automated facial expression measurement), also at low levels of intensity, relative to neutral faces. Recognition thresholds and the pattern of confusions across expressions were, nevertheless, equivalent for dynamic and static expressions, although dynamic expressions were recognized more accurately and faster. We investigated the visual attention patterns (i.e., where, when, how frequently, and how long viewers look at each face region) for faces with (a) genuine, enjoyment smiles (i.e., a smiling mouth and happy eyes with the Duchenne marker), (b) fake, nonenjoyment smiles (a smiling mouth but nonhappy eyes: neutral, surprised, fearful, sad, disgusted, or angry), or (c) no smile (and nonhappy eyes). Viewers evaluated whether the faces conveyed happiness ("felt happy") or not, while eye movements were monitored. Results indicated, first, that the smiling mouth captured the first fixation more likely and faster than the eyes, regardless of type of eyes. This reveals similar attentional orienting to genuine and fake smiles. Second, the mouth and, especially, the eyes of faces with fake smiles received more fixations and longer dwell times than those of faces with genuine smiles. This reveals attentional engagement, with a processing cost for fake smiles. Finally, when the mouth of faces with fake smiles was fixated earlier than the eyes, the face was likely to be judged as genuinely happy. Emotional scenes were presented peripherally (5.2° away from fixation) or foveally (at fixation) for 150 ms. In affective evaluation tasks viewers judged whether a scene was unpleasant or not, or whether it was pleasant or not. In semantic categorisation tasks viewers judged whether a scene involved animals or humans (superordinate-level task), or whether it portrayed females or males (subordinate-level task). The same stimuli were used for the affective and the semantic task. Results indicated that in peripheral vision affective evaluation was less accurate and slower than animal/human discrimination, and did not show any advantage over gender discrimination. In addition, performance impairment in the peripheral relative to the foveal condition was greater or equivalent for affective than for semantic categorisation. These findings cast doubts on the specialness and the primacy of affective over semantic recognition. The findings are also relevant when considering the role of the subcortical "low route" in emotional processing.

Pictures of emotionally neutral, positive, and negative (threat- or harm-related) scenes were presented for 3 seconds, paired with nonemotional control pictures. The eye fixations of high and low trait anxiety participants were monitored. Intensity of stimulus emotionality was varied, with two levels of perceptual salience for each picture (colour vs. greyscale). Regardless of perceptual salience, high anxiety was associated with preferential attention: (a) towards all types of emotional stimuli in initial orienting, as revealed by a higher probability of first fixation on the emotional picture than on the neutral picture of a pair; (b) towards positive and harm stimuli in a subsequent stage of early engagement, as shown by longer viewing times during the first 500 ms following onset of the pictures; and with (c) attention away from (i.e., avoidance) harm stimuli in a later phase, as indicated by shorter viewing times and lower frequency of fixation during the last 1000 ms of picture exposure. This suggests that the nature of the attentional bias varies as a function of the time course in the processing of emotional pictures.

Eye fixations of participants high or low in trait anxiety were monitored during reading of context sentences predicting threatening or nonthreat events, followed by sentences in which a target word represented the predictable event or an inconsistent event. No effects were found on the target word but on the regions following it. When this word represented a threatening event suggested by the context, high anxiety facilitated reading of the posttarget region. When the target word was inconsistent with a threatening event, high anxiety was associated with interference in the final region. No such effects occurred with nonthreat sentences. This reveals selective prediction of threat in high anxiety. However, rather than being automatic, this bias involves elaboration that takes time to develop.
