19. Abstract 16273: Natural History of Aortic Growth After Tetralogy of Fallot Repair

Abstract

Introduction:Aortic root dilation has been described in tetralogy of Fallot (TOF) but few studies have examined longitudinal trajectory of aortic size over time. This study aims to evaluate changes in aortic dimension across age in patients with repaired TOF and examine associated clinical characteristics.Methods:Retrospective single-center study of TOF patients derived from a historical research cohort with up to 4 serial echocardiograms until December 2017. Aortic dimensions at the annulus, sinus, and sinotubular junction (STJ) were measured and converted to Pediatric Heart Network Z-scores. Fractional polynomial regression models were fitted to describe aortic size across age after adjusting for sex, race, age at repair, and hypertension. Association of clinical variables with aortic size were examined using multivariate regression models.Results:There were 126 patients (35% female, age 12.0 [range 8.0 - 18.9] years at first echo) with 426 echocardiograms. Age at last echo was 19.4 [range 10.4 - 26.9] years. Aortic annulus size was stable, whereas sinus and STJ size progressively increased across age (Figure 1). Age, male gender, body surface area, and pulmonary atresia were associated with increase in aortic dimension (Table 1). Age at repair, arch anomaly, prior shunt, 22q11 deletion, hypertension, and aortic regurgitation were not significantly associated.Conclusions:In children and young adults with TOF, the aortic annulus, sinus, and STJ are dilated compared to healthy peers. Aortic size is stable at the annulus but there is progressive growth of the aorta, most notably at the STJ. Features associated with increase in aortic dimension with age are identified.

20. Abstract 14367: Diuretic Use is Associated With Worse Transplant-Free Survival in Adult Fontan Patients

Abstract

Background:Single ventricle physiology can lead to heart failure necessitating diuretics, yet diuretic use and its prognostic value has not been fully explored in the adult Fontan patient. We aim to examine diuretic use in this population and its association with adverse outcomes.Methods:Single center retrospective review of Fontan patients ? 18 years from 2005-2017. Diuretic use at most recent outpatient clinic visit was recorded. Outcomes of interest were heart transplantation and death. Comparison of Fontan patients by diuretic use was performed using Chi-square and Wilcoxon rank-sum tests. Kaplan-Meier survival analysis stratified by diuretic use was performed to examine time to transplant or death. Cox Proportional Hazards models examined risk of outcome based on diuretic use.Results:Of 167 Fontan patients (age 30 [range 21 - 60] years, 49% female), 69 (41%) were taking diuretics at their last outpatient visit. Patients on diuretics were older, symptomatic, underwent Fontan surgery at an older age, and had more comorbidities (Table 1). During a median follow-up of 4.3 years (0.1 - 14.8), 16 patients underwent transplant or died (9.6%). Kaplan-Meier survival analysis showed significantly lower transplant-free survival for Fontan patients taking diuretics (Figure 1). Diuretic use was associated with a 17-fold increased risk of transplant or death (HR 17.4, 95% CI 2.3 - 131.9; p=0.006).Conclusion:Diuretic use is seen in older, symptomatic adult Fontan patients with comorbidities and is associated with worse transplant-free survival. Use of diuretics signals the onset of clinical heart failure and is prognostic of poor outcomes.

Abstract

Introduction: Clinic non-attendance is associated with adverse clinical outcomes in adults with congenital heart disease (ACHD). It is not known whether non-attendance is associated with urgent visits to the emergency department (ED) or hospitalizations. Hypothesis: ACHD patients who miss scheduled outpatient visits have higher rates of ED visits and hospitalizations. Methods: Single-center retrospective study of patients scheduled in ACHD clinic from August 2014 - December 2014. For each patient, only the first visit during the study period was included. Visits were categorized as either "attended" or "non-attended", which was defined as a no-show or same-day cancellation. Outcomes of interest were ED visits and hospitalizations until December 2017. Univariate logistic regression analyses explored the association between non-attendance and outcome. Kaplan-Meier analyses stratified by clinic attendance were used to observe time to outcome. Cox proportional hazards models examined the risk of outcome based on clinic attendance. Results: Of 527 clinic visits scheduled, there were 55 non-attended visits (10.4%). There was no significant difference in age, gender, or disease complexity between patients with non-attended or attended visits. Compared to patients who attended clinic, patients with a non-attended visit had a 3-fold increased odds of having > 1 ED visit (95% CI 1.4 - 6.4; p=0.006) and significantly higher rate of ED visits over time (Figure 1A). Patients who did not attend clinic had a 2-fold increased risk of ED visit (HR 2.1, 95% CI 1.2 - 3.6; p=0.008) vs those who attended. There was no difference in hospitalizations between patients with non-attended and attended visits (Figure 1B). Conclusions: Patients who miss scheduled ACHD clinic visits have higher rates of ED visits but not hospitalizations. Non-attendance may serve as a marker of higher ED utilization, and a potential quality improvement target to reduce ED utilization in this growing, resource-intensive population. 22. Do We Need to Monitor for Graves' Disease After a Diagnosis of Pemphigoid Gestationis?

Abstract

Pemphigoid gestationis (PG) is a rare autoimmune bullous disease that occurs during pregnancy or the postpartum period. PG has been associated with an increased risk of Graves' disease possibly due to shared genetic factors and immune system fluctuations during pregnancy. However, the evidence supporting the association between PG and Graves' disease is mixed. Although dermatologists are cautioned to watch for Graves' disease in patients with a history of PG, this guidance is based on a single cohort where most patients were diagnosed with Graves' disease prior to PG onset. Recent data failed to find an association between Graves' disease and PG but did not capture the lifetime risk of Graves' disease in these patients. Abstract

Dermatology dogma has cautioned against the use of orally administered glucocorticoids (OAG) in the treatment of psoriasis, largely due to concerns of life-threatening generalized pustular psoriasis (GPP) and erythrodermic psoriasis (EP). However, studies show that OAG are frequently used for psoriasis, often by dermatologists. Given the widespread use of OAG, we see an urgency in examining the relationship between OAG usage and the development of GPP and EP. This anonymous electronic survey of 50 US dermatologists examines OAG use in the management of psoriasis and the frequency at which dermatologists report seeing associated adverse outcomes of GPP and EP. Overall, 9 out of 50 (18%) respondents occasionally prescribe OAG to patients with psoriasis. Dermatologists who prescribe OAG tended to be younger than those who did not, with two-thirds in clinical practice for 0-10 years. Among all respondents, 16% (8/50) had experienced one or more patients developing GPP/EP in the context of OAG treatment for psoriasis. Our study suggests that OAG for the management of psoriasis is not uncommon among U.S. dermatologists, despite nearly universal awareness of its risks. Our observed low prevalence of GPP and EP emphasizes the need for prospective studies to better characterize OAG’s risk/benefit profile in psoriasis.
