Honey is a natural, healthy commodity and is probably among the most complex foods produced by nature. It is the oldest recorded and certainly the only natural sweetener that can be used by humans without any further processing. Nowadays, the increase in honey's value, along with its growing list of healthy attributes, has made the present raw material a prime target for adulteration. In the current study, NMR-based metabolite profiling in combination with chemometrics was applied in the quality control of Greek honeys from northeastern Aegean islands. Moreover, statistical total correlation spectroscopy (STOCSY) was employed for the first time as a dereplication and structural elucidation tool in the honey biomarker identification process. A total of 10 compounds were successfully identified in honey total extracts via 1 H NMR spectroscopy. Compounds such as 5-(hydroxymethyl)furfural, methyl syringate, a mono-substituted glycerol derivative and 3-hydroxy-4-phenyl-2-butanone, among others, were identified as potential biomarkers related to the botanical and geographical origin of the samples. High-Resolution Mass Spectrometry (HRMS) was used as an additional verification tool on the identified compounds.

Sideritis sipylea Boiss. (Fam. Lamiaceae) is an endemic plant of the North Aegean Islands (Greece), commonly known as ironwort. Traditionally, its aerial parts have been used to relieve several ailments, especially gastrointestinal disorders, however, with scant knowledge about the pharmacological basis. In the present study, an endemic S. sipylea Greek species from Lesvos Island has been characterized for phytochemical composition and biological activities, in order to give a possible scientific basis to its traditional use and to highlight a further nutraceutical interest as a source of bioactive phytochemicals and extracts. Three different fractions obtained from a methanolic extract of S. sipylea aerial parts by using ethyl acetate with 10 (S10), 20 (S20), and 50% (S50) methanol as fractionation solvents were phytochemically characterized. Moreover, their antioxidant power and cytoprotective activity in different human cell lines were evaluated. The phytochemical analysis highlighted the presence of flavonoids, iridoids, and phenolic acids in all the tested samples. Particularly, the S10 fraction mainly contained iridoids, while S20 and S50 lavandulifolioside and chlorogenic acid, respectively. The fractions also showed antioxidant properties, S10 and S20 being the most potent. When assessed in human cholangiocytes, they counteracted the cytotoxicity of the tBOOH pro-oxidant agent, by reducing ROS levels and affecting GSH antioxidant system. The present findings highlight a possible interest in S10 and S20 fractions from S. sipylea as sources of bioactive molecules and stimulate further studies in order to characterize their possible application for nutraceutical and pharmaceutical purposes.

Cymbidium is one of the most popular genera in Orchidaceae family, commercialized either as loose flowers or as potted plants in floriculture worldwide. The non-marketable parts are typically discarded (e.g., unsuitable flowers, leaves, pseudobulbs, roots), generating an enormous quantity of unutilized biomass. The above by-products were studied through phytochemical analysis and investigated for their dermo-cosmetic potential. The initial antioxidant, anti-tyrosinase, anti-elastase, and anti-collagenase assays of the total extracts indicated that the pseudobulb and root ethyl acetate extracts were the most potent. Those extracts were then submitted to chromatographic separation leading to the isolation of 16 secondary metabolites (four phenanthrenes, three 1,4-phenanthrenquinones, three dibenzyls, two phenolic acid derivatives, two sterols, one dehydrodiconiferyl alcohol derivative, and one simple phenolic compound), including 6-hydroxy-5,7-dimethoxy-1,4-phenanthrenequinone (cymbisamoquinone), which was identified as a new natural product. In parallel, 48 metabolites were identified by UPLC-HRMS analysis of the extracts. The biological evaluation of the isolated compounds revealed that gigantol and tristin present important anti-tyrosinase activity, while bulbophyllanthrin, 3-hydroxy-2,4,7-trimethoxy-phenanthrene, marylaurencinol A, 5-hydroxy-2-methoxy-1,4-phenanthrenequinone, and ephemeranthroquinone B show dose-dependent anti-collagenase activity. In contrast to isolated metabolites, which may act selectively on specific enzymes, the initial total extracts exhibited inhibitory activity against tyrosinase, elastase, and collagenase enzymes, thus showing better prospects for use in dermo-cosmetic formulations.

Sideritis sipylea Boiss. is an endemic plant of the Mediterranean basin that is distributed in the Greek islands of the North Aegean Sea, i.e., Lesvos, Chios, Samos, and Ikaria, and in the West and Middle peninsula of Turkey. It is considered an endangered species because of its uncontrolled collection from its original habitat. Although the antioxidant, anti-inflammatory and antimicrobial properties have been previously reported, the total chemical profile has not yet been explored. In this context, the chemical profiles of the water/methanol (HA), methanol (ME), and ethyl acetate (EtOAc) extracts were analyzed using ultra-performance liquid chromatography coupled with high-resolution mass spectrometry (UPLC-HRMS). In parallel, analysis by gas chromatography-mass spectrometry (GC-MS) was employed for the dichloromethane extract (DCM) as well as for the essential oil (EO) and the extract obtained by supercritical fluid extraction (SFE). Furthermore, the total phenolic content (TPC) along with the in vitro tyrosinase and elastase enzyme inhibitory activity of different extracts was evaluated, towards the discovery of new active agents for cosmetic formulations. These activities are in accordance with its well-known antioxidant and anti-inflammatory properties, confirming the importance of ethnopharmacological references for S. sipylea in Greece and Turkey.

Greek islands of the North Aegean Region are a group of nine inhabited islands (Lemnos, Agios Efstratios, Lesvos, Chios, Psara, Oinousses, Samos, Ikaria, and Fourni) located in the northern part of the Aegean Sea, close to Asia Minor. Each island of this region can be considered autonomous in terms of culture and biodiversity. With this work we try to evaluate the status of the traditional uses of medicinal plants in this region. Endemic and endangered species such as Sideritis sipylea Boiss., Origanum sipyleum L., Thymus sipyleus Boiss., Pistacia lentiscus L., Verbascum ikaricum Murb., are still used by locals to treat different ailments. Moreover, the use of some species for the treatment of specific diseases has been reported for the first time. We report about 109 wild plants of medicinal importance, from 52 families, listing their uses for therapeutic purposes and galenic preparations provided by local medical doctors and pharmacists. The information we include was derived from literature sources and additionally collected through semi-structured interviews conducted on 200 informants (100 men and 100 women). Additionally, informant consensus factor (FIC) and UV value were calculated for the medicinal plants in the current study in relation with the diseases treated. This research confirms the importance of the medicinal plants and the diffusion of their use in traditional medicine within this region. This ethnopharmacological survey is a fundamental step for the preservation of the local knowledge both for further scientific research and for the protection of endangered and endemic medicinal plants.

Objectives: To investigate the expression of a VH1-69-encoded idiotype, and the phenotypic and functional features of monoclonal B-cells from patients with type II mixed cryoglobulinaemia (MC) secondary to chronic hepatitis B virus (HBV) infection., Methods: B-cell immunophenotype and expression of a VH1-69-encoded idiotype were investigated by flow cytometry. B-cell proliferative responses to stimuli were investigated by the CFSE dilution assay., Results: Two out of five patients with chronic HBV studied had massive monoclonal expansion of VH1-69-expressing B-cells. These cells had the peculiar CD21(low) phenotype and low responsiveness to stimuli typical of the VH1-69-expressing B-cells commonly expanded in MC secondary to hepatitis C virus (HCV) infection. In both patients, anti-HBV therapy led to the regression of MC and of VH1-69+ B-cell expansion., Conclusions: VH1-69-encoded antibodies are known to preferentially recognise a variety of viral proteins including HCV E2, influenza A virus haemagglutinin and HIV gp41/gp120, and may serve as innate first line antiviral defense. Thus, like HCV, HBV may cause MC by protracted antigenic stimulation of VH1-69-expressing B-cells.

We herein report a rare case of a massive upper gastrointestinal (GI) bleeding, caused by high-grade diffuse B-cell lymphoma of the duodenum, secondary to immunoproliferative small intestinal disease (IPSID) and treated with an emergency partial pancreatoduodenectomy. A 42-year-old man was admitted to our hospital because of hematemesis. Upper GI endoscopy was unrevealing because of the copious bleeding. Initially, the patient underwent conservative treatment, thus resulting in the temporary cessation of the bleeding. Later, the hemorrhage massively relapsed. An urgent abdominal ultrasound raised the suspicion of a large, possibly bleeding, neoplasm of the duodenum, which was finally confirmed by abdominal computed tomography. The patient underwent an emergency laparotomy, during which a partial pancreatoduodenectomy was performed (Whipple procedure). Histologically, the tumor was a high-grade B-cell lymphoma of the duodenum. The nearby small intestinal mucosa was suggestive of IPSID. A massive upper GI hemorrhage from a high-grade B-cell non-Hodgkin lymphoma of the duodenum, which develops secondary to IPSID, is a very rare clinical demonstration of this disease. Our case is one of the few reports in the English literature, for which the Whipple procedure has been performed as a curative treatment.
