Suicide is one of the leading death causes worldwide, mainly among young adults, and Colombia has experienced an increase during the XXI century. The suicide impact has diverged between age groups and locations in Colombia, where young adults have taken higher incidences than the other age groups. The COVID-19 lockdown induced changes in mental health, affecting the previous suicide trends in the country. We conducted a spatiotemporal analysis of suicide attempts in Colombia per age group, adopting Bayesian models that represent 85,526 individual records in 1,121 municipalities from 2018 to 2020 using R-INLA. We found that Colombia exhibited an increase in suicide-attempt incidence from 2018 to 2019, and suddenly, the incidence fell in the first semester of 2020. The fixed effect of the models evidenced the highest risk in overall municipalities per trimester in the age group between 15-19 years old. The spatial random effect per model evidenced municipalities with the highest risk in the age groups between 10 to 59 years, mainly in the states in the Andean region of Colombia, and other states such as Putumayo, Vaupés, Arauca, Córdoba, Amazonas, and Meta. The temporal random effect evidenced a decay in suicide trends from the fourth trimester of 2019 to 2020, except in the age group > 59 years old. Geographically, our study pinpointed specific regions in Colombia, particularly in the central, southwest, and southeast areas, where the incidence of suicide attempts exceeded 100 cases per 100,000 inhabitants. The nuanced breakdown of incidence across different age groups further underscores the importance of tailoring preventive strategies based on age-specific and regional risk factors.

The agricultural sector of Colombia supports the national economy and food security due to the rich lands for cultivation. Although Colombia has a vast hydrological basin, climate change can impact agricultural productivity, generating economic and social adverse effects. For this, we evaluated the impact of some environmental variables on the production of the most sold crops using production, climatic, and hydrological data of the 1121 municipalities from 2007 to 2020. We modeled the production of coffee, rice, palm oil, sugarcane, and corn, adopting a Bayesian spatio-temporal model that involved a set of environmental variables: average temperature, minimum temperature, maximum temperature, evapotranspiration, precipitation, runoff, soil moisture, vapor pressure, radiation, and wind speed. We found that increases in the average temperatures can affect coffee (-0.2% per °C), rice (-3.76% per °C), and sugarcane (-0.19% per °C) production, meanwhile, these increases can boost palm oil (+2.55% per °C) and corn (+1.28% per °C) production in Colombia. This statement implies that the agricultural sector needs to substitute land use, promoting the production of palm oil and corn. Although our results did not find a significant effect of hydrological variables in any crop, suggesting that the abundance of water in Colombia might balance the impact of these variables. The increases in vapor pressure impact all the crops negatively (between -11.2% to -0.43% per kPa), except rice, evidencing that dry air conditions affect agricultural production. Reproduction and self-maintenance are energetically costly activities involved in classic life history trade-offs. However, few studies have measured the responses of wild organisms to simultaneous changes in reproductive and self-maintenance costs, which may have interactive effects. In free-living female Barn Swallows (Hirundo rustica), we simultaneously manipulated reproductive costs (by adding or removing two nestlings) and self-maintenance costs (by attaching a ∼1 g weight in the form of a GPS tag to half of our study birds) and measured mass, immune status, blood glucose, feather growth, and reproductive output (likelihood of a second clutch, number of eggs, and time between clutches). GPS tags allowed us to analyze how movement range size affected response to brood size manipulation. Tagging altered females' immune function as evidenced by an elevated heterophil to lymphocyte (H:L) ratio, but all females were equally likely to lay more eggs. There was no evidence of interactive effects of the tagging and brood size treatment. Range size was highly variable, and birds with large ranges grew feathers more slowly, but analyzing the effect of brood size manipulation while accounting for variation in range size did not result in any physiological response. Our results support the theoretical prediction that short-lived vertebrates do face a trade-off between reproduction and self-maintenance and, when faced with increased costs, tend to preserve investment in reproduction at the expense of parental condition. Background: Scientific research is fundamental to the education of medical students. However, their involvement in research is limited., Aim: To describe the perceptions of medical students about facilitators and constraints to perform undergraduate research., Material and Methods: Medical students attending the Chilean Congress of Medical Students in the Metropolitan Region in 2018, were surveyed. The responses obtained were subjected to a qualitative content analysis and were grouped according to perceptions of facilitators and constraints., Results: The main facilitators reported were linkage with research teachers, personal motivation towards research, and research-oriented curriculum. The main constraints were lack of time for research, lack of access to formal information channels to engage in research, and sub-optimal conditions for research., Conclusions: The main factor promoting research at the undergraduate level was the link with the teacher, mainly through informal channels. The lack of official information provided through formal instances and lack of time hampers the access to research.

Diversity is a key driver of scientific innovation, yet fields in science, technology, engineering, and mathematics (STEM) have struggled to retain diverse communities. Research suggests that fostering a sense of belonging is critical for retaining diversity. Background: The participation of medical students in research generates professional, scientific, and personal benefits for the student., Aim: To evaluate the interest and opportunities for medical students in Chile to participate in scientific research and their perceptions about factors influencing research., Material and Methods: All students attending the 2018 Chilean Congress for Medical Students were invited to answer a 44 questions survey about interest and opportunities to participate in research., Results: The survey was answered by 489 of the 538 students attending the congress. Eighty five percent referred interest in conducting scientific research, but only 47% had the opportunity to actively participate in a research project. The main research area providing opportunities was epidemiology and the main form to access a research project was through direct contact with a medical professor or researcher. Seventy seven percent of respondents had courses of scientific investigation in their medical curriculum and 92% had a scientific society for medical students in their university., Conclusions: Respondents showed a great deal of interest in participating in scientific research. However, there is a gap between this interest and the available opportunities. Medical professors should promote and facilitate the participation of their students in research.

Purpose: To compare the visual and intraocular optical quality outcomes with different designs of the refractive rotationally asymmetric multifocal intraocular lens (MFIOL) (Lentis Mplus; Oculentis GmbH, Berlin, Germany) with or without capsular tension ring (CTR) implantation., Methods: One hundred thirty-five consecutive eyes of 78 patients with cataract (ages 36 to 82 years) were divided into three groups: 43 eyes implanted with the C-Loop haptic design without CTR (C-Loop haptic only group); 47 eyes implanted with the C-Loop haptic design with CTR (C-Loop haptic with CTR group); and 45 eyes implanted with the plate-haptic design (plate-haptic group). Visual acuity, contrast sensitivity, defocus curve, and ocular and intraocular optical quality were evaluated at 3 months postoperatively., Results: Significant differences in the postoperative sphere were found (P = .01), with a more myopic postoperative refraction for the C-Loop haptic only group. No significant differences were detected in photopic and scotopic contrast sensitivity among groups (P ⩾ .05). Significantly better visual acuities were present in the C-Loop haptic with CTR group for the defocus levels of -2.0, -1.5, -1.0, and -0.50 D (P ⩽.03). Statistically significant differences among groups were found in total intraocular root mean square (RMS), high-order intraocular RMS, and intraocular coma-like RMS aberrations (P ⩽.04), with lower values from the plate-haptic group., Conclusions: The plate-haptic design and the C-Loop haptic design with CTR implantation both allow good visual rehabilitation. However, better refractive predictability and intraocular optical quality was obtained with the plate-haptic design without CTR implantation. Background: The interests that motivate medical students to study this career are diverse and they may change during the seven years of study. In Chile, 22 universities offer medicine and the number of graduated students has increased by more than 50% over the last 10 years., Aim: To determine the motivational profile of medical students at admission, and at the end of their career., Subjects and Methods: A voluntary anonymous survey was applied to 275 first and 140 seventh year medical students from one traditional public and two private schools., Results: The main reason for applying to medical school was social interest (68.7%), followed by interest in science and academia. Thirty six percent of students from seventh year would not study medicine again. In the seventh year, the interest in medical care persists in 88% of students, followed by academic interests in 64%. Only 24% had research interests. Fifty nine and 57% of students projected their medical work in private and public hospital settings, respectively. Only 11% projected themselves as doing research. Sixty nine percent of students would like to receive more information about post graduate education., Conclusions: There is a low interest in research and a high percentage of seventh year students that would not apply to medicine again. Medical schools should perform a systematic analysis of students' interests to improve faulty areas.

Purpose: To compare the visual acuity outcomes and ocular optical performance of eyes implanted with a multifocal refractive intraocular lens (IOL) with an inferior segmental near add or a diffractive multifocal IOL., Design: Prospective, comparative, nonrandomized, consecutive case series., Participants: Eighty-three consecutive eyes of 45 patients (age range, 36-82 years) with cataract were divided into 2 groups: group A, 45 eyes implanted with Lentis Mplus LS-312 (Oculentis GmbH, Berlin, Germany); group B, 38 eyes implanted with diffractive IOL Acri.Lisa 366D (Zeiss, Oberkochen, Germany)., Methods: All patients underwent phacoemulsification followed by IOL implantation in the capsular bag. Distance corrected, intermediate, and near with the distance correction visual acuity outcomes and contrast sensitivity, intraocular aberrations, and defocus curve were evaluated postoperatively during a 3-month follow-up., Main Outcome Measures: Uncorrected distance visual acuity (UDVA), corrected distance visual acuity (CDVA), uncorrected near visual acuity (UNVA), corrected distance near and intermediate visual acuity (CDNVA), contrast sensitivity, intraocular aberrations, and defocus curve., Results: A significant improvement in UDVA, CDVA, and UNVA was observed in both groups after surgery (P ≤ 0.04). Significantly better values of UNVA (P<0.01) and CDNVA (P<0.04) were found in group B. In the defocus curve, significantly better visual acuities were present in eyes in group A for intermediate vision levels of defocus (P ≤ 0.04). Significantly higher amounts of postoperative intraocular primary coma and spherical aberrations were found in group A (P<0.01). In addition, significantly better values were observed in photopic contrast sensitivity for high spatial frequencies in group A (P ≤ 0.04)., Conclusions: The Lentis Mplus LS-312 and Acri.Lisa 366D IOLs are able to successfully restore visual function after cataract surgery. The Lentis Mplus LS-312 provided better intermediate vision and contrast sensitivity outcomes than the Acri.Lisa 366D. Purpose: To compare the visual outcomes and intraocular optical quality observed postoperatively in patients implanted with a rotationally asymmetric multifocal intraocular lens (IOL) and an apodized diffractive multifocal IOL., Methods: Seventy-four consecutive eyes of 40 cataract patients (age range: 36 to 79 years) were divided into two groups: zonal refractive group, 39 eyes implanted with a rotationally asymmetric multifocal IOL (Lentis Mplus LS-312 IOL, Oculentis GmbH); and diffractive group, 35 eyes implanted with an apodized diffractive multifocal IOL (ReSTOR SN6AD3, Alcon Laboratories Inc). Distance and near visual acuity outcomes, contrast sensitivity, intraocular optical quality, and defocus curves were evaluated during 3-month follow-up. Calculation of the intraocular aberrations was performed by subtracting corneal aberrations from total ocular aberrations., Results: Uncorrected near visual acuity and distance-corrected near visual acuity were better in the diffractive group than in the zonal refractive group (P=.01), whereas intermediate visual acuity (defocus +1.00 and +1.50 diopters) was better in the zonal refractive group. Photopic contrast sensitivity was significantly better in the zonal refractive group (P=.04). Wavefront aberrations (total, higher order, tilt, primary coma) were significantly higher in the zonal refractive group than in the diffractive group (P=.02)., Conclusions: Both multifocal IOLs are able to successfully restore visual function after cataract surgery. Purpose: To compare the clinical outcomes after cataract surgery with implantation of 2 models of a new rotationally asymmetric multifocal IOL to ascertain which provides better visual performance and patient satisfaction., Setting: Vissum Instituto Oftalmologico de Alicante, Alicante, Spain., Design: Comparative case series., Methods: Consecutive eyes of cataract patients were divided into 2 groups. Group A received a Lentis Mplus LS-312 MF15 IOL (with a near addition [add] power of +1.50 diopters [D]) and Group B, a Lentis Mplus LS-312 MF30 IOL (with a near add power of +3.00 D). Distance, near, and intermediate visual acuity outcomes were evaluated preoperatively and postoperatively during a 6-month follow-up. Postoperative contrast sensitivity, patient satisfaction, ocular aberrations, ocular optical quality, and defocus curves were analyzed., Results: Patients ranged in age from 55 to 83 years. Group A comprised 22 eyes and Group B, 21 eyes. Both groups had a statistically significant improvement in uncorrected and corrected distance and corrected near visual acuities postoperatively (P≤.01). Group B had statistically significantly better uncorrected and distance-corrected near visual acuity (P≤.01) and Group A, significantly better uncorrected intermediate visual acuity (P=.01) at 3 months. There were statistically significant differences between-group differences in several levels of defocus based on defocus curves. There were no significant differences in postoperative contrast sensitivity, patient satisfaction, optical quality, and ocular aberrometry between groups (P≥.07)., Conclusions: Both IOL models restored distance visual function and improved intermediate vision. Purpose: To compare bilateral reading performance within the first 6 months after implantation of 4 intraocular lens (IOL) models., Setting: Vissum-Instituto de Oftalmológico de Alicante, Alicante, Spain, and University Eye Clinic, Paracelsus Medical University, Salzburg, Austria., Design: Comparative case series., Methods: Patients had bilateral phacoemulsification and implantation of Acri.Smart 48S monofocal, Acrysof Restor SN6AD3 apodized multifocal, Acri.LISA 366D diffractive multifocal, or Rezoom refractive multifocal IOLs. Bilateral reading performance with and without near correction was evaluated preoperatively and postoperatively using the Salzburg Reading Desk., Results: The study comprised 304 eyes of 152 patients 51 to 90 years old. All groups had a significant improvement in uncorrected and corrected distance visual acuities postoperatively (P≤.01). The apodized multifocal and diffractive multifocal groups had significantly better uncorrected reading acuity than the monofocal and refractive multifocal groups 1 month and 6 months postoperatively (P<.01). Uncorrected reading speed was significantly worse in the refractive multifocal group than in the monofocal group at 1 month (P<.01). The monofocal group had the greatest uncorrected reading distance at 1 month and 6 months (P<.01)., Conclusion: Multifocal IOLs with a diffractive component provided a comparable reading performance that was significantly better than the one obtained with refractive multifocal and monofocal IOLs., (Copyright © 2011 ASCRS and ESCRS. Purpose: To compare vision-related quality of life using the National Eye Institute Visual Function Questionnaire (NEI VFQ-25) in patients with 1 of 3 types of intraocular lenses (IOLs) and to correlate it with postoperative visual outcomes., Setting: Vissum Corporation-Instituto Oftalmológico de Alicante, Alicante, Spain., Design: Comparative case series., Methods: This study comprised eyes having cataract surgery with bilateral implantation of a monofocal IOL (Group A), apodized multifocal IOL (Group B), or full diffractive multifocal IOL (Group C). Distance and near visual acuities, contrast sensitivity, and quality of life were evaluated preoperatively and postoperatively., Results: The study enrolled 106 eyes (53 patients; age range 49 to 80 years). All groups had significant improvement in uncorrected and corrected distance visual acuities postoperatively (P ≤.05). Near vision outcomes were significantly better in Groups B and C (P ≤.01). Groups B and C had significantly less difficulty in some near tasks, such as reading the newspaper (A-B, P=.02; A-C, P=.02) or reading bills (A-B, P=.04; A-C, P=.004). Group C also had significantly less difficulty driving at night than Group B (P<.01). Near visual acuity and contrast sensitivity were significantly correlated with difficulty in near visual tasks in Groups B and C. Purpose: To evaluate the results obtained with the treatment for interface epithelial ingrowth after laser in situ keratomileusis (LASIK) with neodymium:yytrium-aluminum-garnet (Nd:YAG) laser., Design: Retrospective, interventional, noncomparative, consecutive case series., Methods: Thirty eyes were treated with Nd:YAG laser for clinically significant epithelial ingrowth developed after LASIK. Each impact produced an explosion with evaporation of epithelial cells giving rise to bubbles. The mean intensity of the spots was 0.6 mJ, and they were variable in quantity depending on the size of the epithelial ingrowth area. The patients were followed up for two years., Results: The opacities caused by the epithelial ingrowth disappeared in 80% of the cases with the Nd:YAG laser treatment. In 40% of the cases, two or more sessions of Nd:YAG laser treatment were necessary to eliminate the epithelial ingrowth areas entirely. In 60% of the cases, visual acuity improved by one or more lines of vision and the corneal topography was more regular. After the treatment, symptoms like glare and halos improved in all cases. The intensity of the laser was adjusted in each case and was focused perfectly on the epithelial ingrowth areas. There were no complications., Conclusions: Nd:YAG laser, in our experience, is an easy and effective technique for the treatment of epithelial ingrowth after LASIK.

Purpose: To evaluate the changes in visual performance and ocular optical quality after hyperopic laser in situ keratomileusis (LASIK) using the Esiris excimer laser (Schwind eye-tech-solutions)., Setting: Vissum-Instituto Oftalmológico de Alicante, Alicante, Spain., Methods: Excimer laser surgery was performed with the Esiris laser in 51 eyes of 29 patients with hyperopia or hyperopic astigmatism. The follow-up was 6 months. Changes in visual acuity, refraction, and corneal and ocular wavefront aberrations (6.0 mm pupil) were recorded and analyzed., Results: The mean preoperative sphere was +4.45 diopters (D)+/-1.08 (SD) (range +2.50 to +7.25 D) and the mean preoperative cylinder, -0.55+/-0.36 D (range 0.00 to 1.00 D). Ten eyes (19.61%) had a LASIK enhancement during the follow-up for the correction of the residual refractive error and were excluded from the refractive analysis. Of the 41 remaining eyes, 95.12% had the same or improved distance best spectacle-corrected visual acuity (BSCVA) 6 months after surgery; 90.25% had no change or a gain of lines of near BSCVA. Moreover, 80.50% of eyes were within +/-0.50 D of emmetropia. Regarding corneal aberrations, statistically significant changes were observed in higher-order aberrations (HOAs), coma-like aberrations, and the primary spherical aberration coefficient Z(4,0), which changed from positive to negative values (all P<.01). Regarding total ocular aberrations, statistically significant changes were observed in total and higher-order root-mean-square values (P<.01)., Conclusions: Hyperopic LASIK using the Esiris excimer laser for the correction of 2nd-order aberrations was safe and effective. Ocular and corneal HOAs increased significantly postoperatively.

Purpose: To investigate the refractive and visual outcomes of hyperopic LASIK using Esiris/Schwind technology., Methods: This retrospective non-comparative observational study included 106 eyes (65 patients) operated with the Esiris/Schwind laser for hyperopia. Eyes were divided into two groups: group 1, < or = +3.99 diopters (D) and group 2, > or = +4.0 D (up to +7.0 D). Visual outcome for distance and near uncorrected (UCVA) and best spectacle-corrected visual acuities (BSCVA) (decimal values), cycloplegic refraction, keratometry (K), pachymetry, and complications were evaluated., Results: In group 1, mean UCVA was 0.4 +/- 0.1 preoperatively and 0.8 +/- 0.2 6 months postoperatively. Mean BSCVA was 0.9 +/- 0.1 preoperatively and 0.9 +/- 0.1 6 months postoperatively. Safety index was 1.1 and efficacy index was 0.97. Mean spherical equivalent refraction was +2.33 +/- 0.9 D preoperatively and +0.3 +/- 0.3 D 6 months postoperatively. Mean K reading was 43.7 +/- 1.1 D preoperatively and 45.0 +/- 1.6 D 6 months postoperatively. In group 2, mean UCVA was 0.3 +/- 0.1 preoperatively and 0.8 +/- 0.2 6 months postoperatively. Mean BSCVA was 0.9 +/- 0.1 preoperatively and 0.9 +/- 0.1 6 months postoperatively. Safety index was 0.98 and efficacy index was 0.92. Mean spherical equivalent refraction was +5.1 +/- 0.9 D preoperatively and +0.4 +/- 0.5 D 6 months postoperatively. Mean K reading was 43.1 +/- 1.6 D preoperatively and 46.0 +/- 1.4 D 6 months postoperatively. Forty-six (90.2%) of 51 eyes in group 1 and 47 (85.5%) of 55 eyes in group 2 were within +/- 0.5 D of emmetropia. At 6-month follow-up, 40 (78%) of 51 eyes in group 1 had UCVA of 20/20 compared to 41 (75%) of 51 eyes in group 2. One (2%) of 51 eyes in group 1 and 4 (7.3%) of 55 eyes in group 2 lost < or = 2 lines of BSCVA. No eye lost >2 lines of BSCVA., Conclusions: Hyperopic LASIK with Esiris/Schwind technology is effective and safe in the correction of hyperopia up to + 7.0 D. Although a slight statistical significance was found for low hyperopia, visual and refractive results obtained in high hyperopia were encouraging.

Purpose: To evaluate the safety, efficacy, and predictability of hyperopic LASIK retreatments., Methods: This retrospective, consecutive, non-comparative, observational study included 85 eyes that underwent hyperopic LASIK retreatment with 1-year follow-up. Complete ophthalmic examination included distance uncorrected (UCVA) and best spectacle-corrected visual acuity (BSCVA), cycloplegic refraction, and pachymetry. Patients were divided into two groups according to the preoperative spherical equivalent refraction: group 1, < or = +3.9 diopters (D) and group 2, > or = +4.0 D., Results: The UCVA improved from 0.31 +/- 0.2 to 0.7 +/- 0.2 in group 1 and from 0.2 +/- 0.2 to 0.6 +/- 0.2 in group 2 following retreatment. Mean spherical equivalent refraction improved from +2.8 +/- 0.85 to +0.2 +/- 0.9 in group 1 and from +5.3 +/- 0.9 to +0.3 +/- 1.3 in group 2 after retreatment. In group 1, 32 (72.7%) of 44 eyes had BSCVA > or = 20/25, and in group 2, 24 (58.5%) of 41 eyes had BSCVA > or = 20/25. After primary hyperopic LASIK, 25 (56.8%) of 44 eyes in group 1 and 19 (46.3%) of 41 eyes in group 2 maintained BSCVA or gained > or = 1 lines postoperatively in contrast to 21 (47.7%) of 44 eyes in group 1 and 22 (53.9%) of 41 eyes in group 2 after retreatment. In group 1, 11 (25%) of 44 eyes lost > or = 2 lines of BSCVA after initial hyperopic LASIK compared to 14 (31.8%) of 44 eyes after retreatment. In group 2, 10 (24.4%) of 41 eyes lost > or = 2 lines of BSCVA after initial hyperopic LASIK compared to 12 (29.2%) of 41 eyes after retreatment. After hyperopic LASIK retreatment, 31 (70.5%) of 44 eyes in group 1 and 19 (46.4%) of 41 eyes in group 2 were within +/- 0.5 D of emmetropia. Safety was 0.9 in both groups and efficacy was 0.8 and 0.7 in groups 1 and 2, respectively. Complications included epithelial ingrowth of 1 to 3 mm (30%) and flap edge melting (2%)., Conclusions: Hyperopic LASIK retreatment improved the refractive results of initial hyperopic LASIK surgery with 20% to 30% of eyes gaining > or = 1 lines of BSCVA. The loss of BSCVA was greater after primary hyperopic LASIK than after retreatment.

Purpose: To evaluate uncorrected near visual acuity (NVA), accommodation, corneal aberrations, and the optical quality of the retinal image in presbyopic eyes after photorefractive keratectomy (PRK)., Design: Nonrandomized comparative retrospective study., Participants: Ten post-PRK patients and 10 normal patients., Methods: Twenty eyes (10 right and 10 left after PRK for myopia, minimum of 10 years after the operation; group A) were compared with 20 eyes (10 right and 10 left age- and gender-matched normal controls; group B). All subjects were over 40 years of age., Main Outcome Measures: With best distance correction, NVA was measured at 40 cm (Jaeger, J series), and the range of accommodation (diopters [D]) was measured subjectively (negative relative amplitude and positive relative amplitude). The modulation transfer function (MTF) and corneal aberrations such as comalike, spherical (SAs), and higher order (HOAs) were measured with a Hartmann-Shack aberrometer. From the MTF curves, the spatial frequencies corresponding to contrast values of 0.1 and 0.5 were noted., Results: Mean ages (+/- standard deviations [SDs]) were 46.3 years (4.7) for group A and 47.6 years (4.9) for group B (P>0.05). Near acuity was J1 or better in 12 of 20 post-PRK and 4 of 20 control eyes. Mean accommodations (+/- SDs) were 3.2 D (1.14) for right group A eyes and 2.1 D (0.94) for right group B eyes (P = 0.0152), and 3.4 D (0.99) for left A eyes and 2.3 D (1.02) for left B eyes (P = 0.0168). Total HOA indexes (+/- SDs) were 1.449 (0.409) for right group A eyes and 0.824 (0.241) for right group B eyes (P = 0.008), and 1.464 (0.388) for left A eyes and 1.067 (0.542) for left B eyes (P = 0.0752). Pooling the data from post-PRK and control eyes, a significant correlation was found between near acuity and SA (right eyes, r = -0.535, P = 0.015; left eyes, r = -0.493, P = 0.027). Significant associations were found between accommodation, near acuity, HOA, and comalike aberration for right eyes only. Mean spatial frequencies (+/- SDs) corresponding to contrast values of 0.1 for right and left eyes were 14.96 (5.71) for right group A eyes and 22.02 (6.85) for right group B eyes (P = 0.074), and 15.11 (7.80) for left A eyes and 21.41 (9.00) for left B eyes (P = 0.271). Mean spatial frequencies (+/- SDs) corresponding to contrast values of 0.5 for right and left eyes were 2.86 (0.63) for right group A eyes and 3.21 (0.35) for right group B eyes (P = 0.596), and 2.76 (0.98) for left A eyes and 3.22 (0.27) for left B eyes (P = 0.194)., Conclusions: Compared with normal eyes, in previously myopic eyes treated with first-generation PRK lasers there is a tendency for (1) the optical quality of the retinal image to be reduced at low contrast, (2) the aberrations attributed to the corneal surface to increase, and (3) both measured subjective accommodation and near acuity to be greater than expected. We postulate that the corneal aberrations induced by PRK for myopia may reduce the quality of the retinal image for distance but enhance near acuity by way of a multifocal effect that can delay the onset of age-related near vision symptoms.

Purpose: To evaluate the effect of bitoric ablation (cross cylinder) in 1 eye and ordinary monotoric treatment in the contralateral eye of patients with mixed astigmatism., Setting: Instituto Oftalmológico de Alicante and University of Miguel Hernández, Alicante, Spain., Methods: A comparative consecutive study was initiated in 30 eyes of 15 patients with mixed astigmatism. In each patient, 1 eye was treated with bitoric ablation (Group 1) and the other eye was treated with monotoric ablation (Group 2). The decision to treat with bitoric or monotoric ablation was randomized. All patients were operated on by the same surgeon (J.L.A.) and examined by the same investigators (M.J.A., M.A.H.). All completed a 6-month follow-up. Visual acuity was estimated by transforming the Snellen chart readings into logMAR equivalents. In Group 1, theoretical ablation was also performed on glass slides by making 3 ablation profiles (positive, negative, and bitoric)., Results: There was a statistically significant improvement in the mean uncorrected visual acuity (UCVA), the mean spherical equivalent (SE), and the mean refractive cylinder in Group 1 (P<.0001, P =.04, P<.0001, respectively) and in the mean refractive cylinder in Group 2 (P =.01). Better results were obtained in the mean incremental change in UCVA, best corrected visual acuity, SE, and refractive cylinder in Group 1., Conclusions: Treatments to correct mixed astigmatism vary in ablation profiles and depth. Bitoric ablation appears to be safer, more effective, and more tissue sparing than the standard treatment.

Purpose: To report a case of traumatic corneal rupture that occurred 6 years previous, following combined radial keratotomy (RK) and photorefractive keratectomy (PRK)., Methods: A 28-year-old man experienced a severe direct trauma to the right eye. Upon initial examination, a ruptured globe was diagnosed. Four of the eight radial incisions were ruptured with extrusion of intraocular tissues. The patient was also diagnosed with a fracture of the medial wall of the right orbit. The patient received immediate surgery during which the radial incisions that had been torn were stitched with a suture., Results: Following the postoperative period, the patient had visual acuity of light perception., Conclusion: PRK, carried out on a cornea already treated with RK, may increase the likelihood of rupture in blunt trauma.

Purpose: To compare Orbscan II pachymetry with ultrasonic pachymetry in eyes having corneal refractive surgery., Setting: Instituto Oftalmológico de Alicante, Alicante, Spain., Methods: In this comparative prospective study, 72 eyes (36 patients) were divided into 4 groups: Group 1, 16 eyes with haze after photorefractive keratectomy (PRK); Group 2, 12 eyes with no haze after PRK; Group 3, 24 eyes that had uneventful laser in situ keratomileusis; Group 4, 20 normal eyes. The corneal thickness was measured in all eyes by the Orbscan II (version 3.27.10, Orbtek Inc.) and then by ultrasonic pachymetry (DHC 500, Technology Inc.)., Results: The results between the 2 systems of pachymetry were significant in Group 1 (P < .0001) and not significant in Groups 2, 3, and 4 (P = .056, P = .051, and P = .68, respectively). A statistically significant correlation existed between the grade of haze and the decrease in Orbscan readings (C = -0.821, P < .0001)., Conclusion: The Orbscan II pachymetry measurements correlated with the ultrasound measurements in eyes with clear corneas. Haze after PRK decreased the Orbscan pachymetry.

In cases with previous photorefractive keratectomy (PRK), there is a risk of developing severe haze after laser in situ keratomileusis (LASIK). We report 3 patients (4 eyes) who developed severe haze after LASIK treatment for residual myopia following PRK. Both PRK and LASIK procedures were performed using the VISX 20/20 excimer laser. We report the grade of haze, amount of regression, and visual acuity after the patients were treated with topical steroids. In 2 eyes, the uncorrected visual acuity was 1.0 after 1 year with grade I haze. In the other 2 eyes, there was a residual refractive error, and the best spectacle-corrected visual acuity was 0.7 with grade II haze.

Purpose: To evaluate the results of laser in situ keratomileusis after photorefractive keratectomy., Methods: Eighty eyes of 80 patients with residual myopia after photorefractive keratectomy were reoperated with laser in situ keratomileusis. The study was retrospective. Laser in situ keratomileusis was performed using the automated corneal shaper microkeratome and Chiron Technolas 217-C d1 excimer laser. Data measured after laser in situ keratomileusis included uncorrected visual acuity, best-corrected visual acuity, refraction, haze, pachymetry, and keratometry. The follow-up was at least 12 +/- 1.6 months (range, 12 to 15 months)., Results: After laser in situ keratomileusis the mean spherical equivalent was -0.24 diopters +/- 0.78. (range, -3 to +1.5) at 12 months, and the mean uncorrected visual acuity was 0.76 diopters +/- 0.24 (range, 0.1 to 1). Sixty-five eyes (81.3%) had various degrees of haze after laser in situ keratomileusis. One eye (1.2%) lost 2 lines of best-corrected visual acuity., Conclusions: Laser in situ keratomileusis enhancement may be a good alternative to correct residual myopia and astigmatism after primary photorefractive keratectomy. Corneal haze is a common problem in these eyes, and the treatment after laser in situ keratomileusis enhancement should be the same as the treatment after primary photorefractive keratectomy.

Purpose: To evaluate the effectiveness, predictability, and safety of laser in situ keratomileusis (LASIK) for correcting residual myopia after cataract surgery with intraocular lens implantation., Methods: Twenty-two eyes of 22 patients underwent LASIK for the correction of residual myopia after cataract surgery. LASIK was carried out using the Chiron Automated Corneal Shaper and the NIDEK EC-5000 excimer laser. In all eyes, the follow-up was 12 months., Results: Before LASIK, 1 eye (4.5%) had an uncorrected visual acuity of 0.5 or better; 12 months after LASIK, 10 eyes (45.4%) achieved this level of visual acuity and 0 eyes achieved 1.00 or better. Before LASIK, mean refraction was -2.90 +/- 1.80 D; 12 months after LASIK it decreased significantly to 0.40 +/- 0.60 D (P < .01). In 18 eyes (81.8%) at 12 months after LASIK, spherical equivalent refraction was within +/-1.00 D of emmetropia; 11 eyes (50%) were within 0.50 D. No vision-threatening complications occurred., Conclusion: LASIK with the Automated Corneal Shaper and Nidek EC-5000 excimer laser was an effective, predictable, stable, and safe procedure for correcting residual myopia after cataract surgery. No intraocular lens or cataract incision related complications occurred when LASIK was performed at least 3 months after phacoemulsification.

Purpose: To evaluate the incidence, characteristics, and results of treatment of choroidal neovascularization (CNV) in myopic eyes corrected by photorefractive keratectomy (PRK)., Setting: University Miguel Hernández, Instituto Oftalmológico de Alicante, Alicante, Spain., Methods: This study was of the incidence of CNV in 5936 consecutive eyes that had PRK for the correction of myopia. Mean follow-up was 38.5 months +/- 17.4 (SD)., Results: Extrafoveal CNV developed in the right eye of a 44-year-old woman 26 months after PRK for the correction of -12.00 diopters (D) of myopia. The follow-up after PRK was 38 months. Best corrected visual acuity (BCVA) before PRK was 20/40 (spherical equivalent [SE] -12.00 D). After PRK, BCVA was 20/32 SE -1.75 D). The CNV was treated by direct argon-green laser photocoagulation and did not recur in the subsequent 12 months). After CNV treatment, BCVA was 20/32 (SE -2.25 D)., Conclusion: The incidence of CNV after PRK for myopia was low. Choroidal neovascularization is a possible complication in myopic eyes, and the risk exists before PRK. After PRK, the risk of CNV in myopic patients did not increase.

Purpose: To evaluate the effectiveness, predictability, and safety of photorefractive keratectomy (PRK) for correcting residual myopia and myopic astigmatism after cataract surgery with intraocular lens implantation., Setting: Refractive Surgery and Cornea Unit, Instituto Oftalmológico de Alicante, Alicante, Spain., Methods: Thirty consecutive eyes (30 patients) had PRK for residual myopia after cataract surgery. Surface PRK with a VISX Twenty-Twenty excimer laser was used in all patients. Follow-up was 1 year., Results: Before PRK, no eye had an uncorrected visual acuity (UCVA) of 20/40 or better. Twelve months after PRK, 16 eyes (53.33%) had a UCVA of 20/40 or better. After PRK, best corrected visual acuity (BCVA) improved 1 line or more in 14 eyes (46.66%) over the preoperative values, and 15 eyes (50.00%) had the same BCVA as before PRK. Mean pre-PRK refraction of -5.00 diopters (D) +/- 2.50 (SD) decreased significantly to -0.25 +/- 0.50 D at 12 months (P < .001). At 12 months, the spherical equivalent was within +/- 1.00 D of emmetropia in 27 eyes (90.00%). No vision-threatening complications occurred., Conclusion: Photorefractive keratectomy was an effective, predictable, and safe procedure for correcting residual myopia and myopic astigmatism after cataract surgery.

Objective: To evaluate the effectiveness, predictability, and safety of laser in situ keratomileusis (LASIK) retreatment for correcting residual myopia., Design: Retrospective noncomparative case series., Participants and Intervention: Fifty-nine consecutive eyes (43 patients) underwent LASIK retreatment at 3 or 6 months after the primary LASIK procedure. Lifting the corneal flap and reablating the stromal bed with a VISX 20/20 excimer laser was the procedure used for LASIK enhancement., Main Outcome Measures: The following parameters were studied before and after retreatment: visual acuity, refraction, videokeratography, applanation tonometry, and corneal thickness. Complications after LASIK enhancement also were evaluated. Follow-up was 12 months., Results: Before retreatment, only 3.38% of eyes (2 of 59) had an uncorrected visual acuity of 0.5 (20/40) or better, and after retreatment, this percentage increased to 60% (30 of 50) at 6 months and 61.8% (34 of 55) at 12 months. After reoperation, mean best-corrected visual acuity improved by half a line over the values before retreatment. The preretreatment refraction of -2.92 +/- 1.22 diopters (D) (mean +/- standard deviation) decreased significantly to -0.44 +/- 0.80 D at 6 months and to -0.61 +/- 0.82 D at 12 months (P < 0.001). In 82% of eyes (41 of 50) at 6 months and 81.8% (45 of 55) at 12 months, the spherical equivalent was within 1.00 D of emmetropia. There was a significant regression of effect (0.38 D) between 3 and 12 months (P < 0.01). Postretreatment refraction was related to the original refraction before the primary LASIK, the preretreatment refraction, and the ablation diameter used. Although no vision-threatening complications were found, epithelial ingrowth and flap melting were more common after than before LASIK retreatment, with 31% of eyes at 12 months with epithelial ingrowth and 10.9% with flap melting. However, LASIK enhancement improved decentration and night-vision problems., Conclusions: LASIK retreatment was an effective and predictable procedure for correcting residual myopia. Epithelial ingrowth and flap melting were more frequent after than before LASIK retreatment, whereas decentration and night-vision symptoms improved.

Purpose: To evaluate the results and complications of photorefractive keratectomy (PRK) for myopia., Setting: Alicante Institute of Ophthalmology, University of Alicante, Spain., Methods: This prospective study evaluated the results and complications of the first 3000 cases of PRK performed with a VISX Twenty-Twenty excimer laser. Myopia ranged from -1.0 to -14.0 diopters (D) and astigmatism, from -1.0 to -5.0 D. Follow-up was 2 years., Results: Ten eyes (0.7%) lost two or more lines of best corrected visual acuity 1 year after surgery. Retreatment for undercorrection or regression was done in 7.41% in the low myopia group and 38.69% in the high myopia group. Central islands, which could be detected only on videokeratoscopy, occurred frequently but influenced the refractive and visual outcomes in few cases. Severe haze was present in 17 eyes only after 1 year follow-up. There were no cases of progressive hyperopia. Other complications such as eccentric ablation producing astigmatism (n = 15), delayed re-epithelialization, or recurrent corneal erosion were rare., Conclusion: There were few complications after PRK in 3000 eyes. With proper patient selection, PRK can be considered relatively safe compared with other refractive procedures.

Purpose: To evaluate the visual results of excimer laser photorefractive keratectomy (PRK) to treat pediatric patients with amblyopic myopic anisometropia in whom conventional amblyopia treatments have failed., Setting: Instituto Oftalmológico de Alicante, University of Alicante, Spain., Methods: Six patients aged 5 to 7 years with amblyopic myopic anisometropia were treated by PRK in the more myopic eye using a VISX Twenty-Twenty excimer laser. Minimum preoperative refraction in the amblyopic eyes ranged from -4.00 to -13.00 diopters and best corrected visual acuity (BCVA), from 20/40 to 20/400. All cases completed a minimum of 2 years follow-up of antiamblyopic treatment., Results: After PRK, preoperative BCVA was maintained without optical correction and complementary refraction resulted in increased BCVA in every case. One patient with high myopia developed severe haze. There were no other significant complications., Conclusion: Our results indicate that PRK may be an alternative for the correction of pediatric myopic anisometropia and an important aid in treating amblyopia. When other therapies have failed, using PRK in children of the ages in this study must be carefully evaluated; however, our results suggest a beneficial effect.

For one year we followed 46 eyes that had photoastigmatic refractive keratectomy with an excimer laser to correct simple myopic astigmatism. Mean preoperative cylinder was -2.50 +/- 0.70 diopters (D); mean preoperative best corrected visual acuity was 20/25 +/- 1 Snellen line. Mean postoperative refraction one year after surgery was -0.5 +/- 0.2, and mean uncorrected visual acuity was 20/25 +/- 1 Snellen line. In all eyes, the final axis of the residual cylinder was +/- 5 degrees around the preoperative axis. Uncorrected visual acuity equaled best preoperative acuity in 24 eyes. Eight eyes gained one line of best corrected visual acuity, six gained more than one line, and eight lost one line. No patient lost more than one line of uncorrected visual acuity. No significant complications such as haze, halos, eccentric ablation, or hypercorrection were observed.

To test the effect that the treatment with topical antioxidants may have on corneal infection, we have studied the effect of topically applied antioxidants, such as dimethylthiourea 0.5% (DMTU) and dismutase superoxide 0.2% (SOD), on infectious experimental keratitis caused by Staphylococcus aureus. We have quantified the results of the incubated corneas in ex vivo as well as in in vivo treated with antioxidants by using the luminol amplified chemiluminescence technique (LAC). The evaluation of corneal inflammation was performed calculating the average inflammatory index obtained from the clinical observation of the corneal secretion, corneal edema and ciliary injection. The evolution of the corneal infiltration was evaluated by means of computerized planymetry. The antioxidants used in this study demonstrated a significant reduction of the LAC values when compared with a control group both in the in vivo as well as in ex vivo studies. No significant differences in the clinical evaluation of the average inflammatory index were observed between the study and the control groups. However, a significant increase in the corneal infiltration was registered in the antioxidant treated group (p < 0.001) evaluated by computerized planymetry. Our results indicate that the use of antioxidants as antiinflammatory drugs may have a potential negative influence on the course of infectious keratitis.

To ascertain the effectiveness of topical antioxidant therapy on acute corneal inflammation, we have studied the effectiveness of topical treatment with a saline solution and with antioxidants such as 0.2% superoxide dismutase and 0.5% dimethylthiourea (DMTU) in a controlled experimental study. The evolution of the inflammatory process was evaluated by a multimodel approach, including computer-assisted planimetry of the corneal ulcer and infiltrate, ultrasonic pachymetry, luminol-amplified chemiluminescence and the study of corneal transparency by direct spectral spectrophotometry transmittance. The experimental model was a corneal ulcer created by a 60-second application of 1 N sodium hydroxide. Topical treatment with DMTU was shown to significantly improve all parameters tested, while superoxide dismutase reduced only the corneal ulcers. Antioxidant topical therapy with DMTU was shown to be efficient in reducing the inflammatory reaction that occurs during acute corneal inflammation. This suggests that antioxidant therapy could be considered as a complementary treatment in the pharmacological modulation of acute corneal inflammation.

In this study, we have correlated the relative effectiveness of nonsteroidal anti-inflammatory drugs versus corticosteroids in the topical treatment of acute corneal inflammation. The authors have studied the effect of 1 drop every 6 h of flurbiprofen 0.03%, indomethacin 1%, diclofenac 0.1% and dexamethasone 0.1% in an experimental model of corneal alkali burn. The objective parameters used to evaluate the corneal inflammatory response were Luminol-enhanced chemiluminescence, ultrasonic pachymetry and computer-assisted analysis of the area of the corneal ulceration. Indomethacin 1% showed statistically significant effects on the improvement of all the parameters tested in the study. No such effect was demonstrated for flurbiprofen 0.03% or diclofenac 0.1%. We can conclude that indomethacin 1% is an effective drug in the treatment of experimental acute corneal alkali-burn inflammation.

In order to ascertain the accuracy and specificity of the Potential Acuity Meter (PAM) in the preoperative evaluation of likely postoperative visual acuity in cases of cataract associated with age-related macular degeneration (ARMD), we studied the results in 130 cases, 54 of whom had preoperatively proved or suspected ARMD associated with a cataract. All of them were operated with extracapsular extraction and implantation of an intraocular lens (IOL), and final visual acuity was established when all the patients were visited three months postoperatively. A positive linear correlation was found between normal ocular fundus and PAM prediction (r = 0.77), and in the group of patients with preoperatively confirmed ARMD (r = 0.77). No difference was found between the preoperative PAM estimation and the final visual acuity in both groups. The PAM prediction was less accurate in cases with doubtful preoperative ARMD (r = 0.58) for whom showed PAM the highest score of false negatives in favor of the final visual acuity. Our results show that PAM is a reliable clinical technique for the prediction of visual results after cataract surgery in cases with preoperatively proved or doubtful macular pathology. To the best of our knowledge, this is the first report regarding the sensitivity and specificity of the PAM in diseased or suspicious maculas associated with cataract.

The authors studied the effect of topical 2% 6-aminonicotinamide and 0.1% adenosine on an experimental model of acute corneal inflammation. Luminol-enhanced chemiluminescence (LAC), as an indirect measurement of free-radical release, and computer-assisted planimetry of the corneal ulcer and its infiltrate were performed both ex vivo and in vivo on the fifth day following the induction of experimental alkali burn keratitis. The authors proved that both drugs significantly inhibited LAC both ex vivo and in vivo and that such treatments had also a significant beneficial effect on the evolution of the corneal ulcer and its infiltrate. To the best of the authors' knowledge, this finding had not been previously reported in experimental corneal inflammation and may indicate that treatment with inhibitors of the oxidative metabolism could offer a new approach in the pharmacological modulation of acute corneal inflammation.
