Purpose: To study the outcome of anterior transposition of inferior oblique muscle in the treatment of inferior oblique over action. Study Design: This was a hospital based prospective study done at Department of Ophthalmology Khyber Teaching Hospital during January 2014 to June 2016. Methods: A total of 35 patients were included in the study. Type of strabismus, ocular alignment, pre and post operative degree of Inferior Oblique over action were obtained using specified check list. Results: A total of 35 patients were included in the study 57 eyes were operated. All patients had grade +3 inferior oblique overaction. Pre operative V pattern was present in 22 patients. After surgery it remained in only 5 patients. Overall 22 (62.8%) and 6 (17.4%) patients had preoperative estotropia and exotropia. While 7 (20%) patients had only IOOA Inferior oblique over-action improved in (91.4%). Residual hypertropia occurred in 2 (5%) and postoperative hypotropia was observed in 1 (2.8%) patient. Conclusion: We concluded that anterior transposition of inferior oblique is effective for treatment of grade +3 inferior oblique over-action with minimum side effects. [ABSTRACT FROM AUTHOR]

Introduction : Cardiac events due to coronary artery disease (CAD) are the most common cause of death in all over the world. Myocardial ischemia and its consequent heart failure are considered as common problems and complications of CAD. The purpose of this study was to assess the anti-ischemic effects of fibrates in patients with CAD. Methods : This clinical-trial study was conducted on patients with dyslipidemia and CAD referred to the Baqiyatallah university hospital, Tehran, during 2008-2010. A total of 124 patients randomly divided into two groups: the first group (64 patients) received simvastatin (20-60 mg/day)+fenofibrate (200 mg/day) with lipid-limited diet and exercise and the second group (60 patients) received simvastatin (20-60 mg/day) with diet and exercise for one year and the patients were evaluated at the end of this period. Background: Sympathetic complex of over-activation kidneys is one of the main causes of primary hypertension (HTN). We aimed to assess the efficacy and safety of sympathectomy using 5Fr mariner catheter ablation on patients with refractory hypertension.Methods: In this prospective cohort study, patients who received three or more anti-hypertensive medications with 160mmHg systolic blood pressure (BP) or more were randomly included and divided into 2 groups. Cases in the first group were undergone to renal denervation and the second group was treated by previous antihypertensive medications. Both groups were followed for six month by assessing BP and adverse effects.Results: One hundred and seventeenth patients (54%) out of 212 screened patients were included in the first group (renal denervation) and 95 patients as the second group. Aims and Objectives: We evaluated the incidence of coronary artery disease (CAD) in Sulfur mustard (SM) exposed veterans. We also evaluated the relationship between exposure to SM and angiography findings and compared angiography findings of SM exposed individuals with unexposed ones after two decades from the time of exposure to SM. Materials and Methods: A case-control study was conducted on 200 consecutive patients (100 SM exposed vs. 100 unexposed) undergoing angiographic assessments due to CAD. Results: The coronary angiography findings between two groups were significantly different ( P < 0.001). Ninety two (92%) patients in SM exposed group and 82 (82%) in unexposed group had abnormal findings in their coronary arteries ( P = 0.031). Conclusions: The incidence of CAD and angiographic changes were significantly increased with exposure to SM. Further studies on cardiovascular effects of SM are needed.

ntroduction: Dysphagia is a common chief complain of various diseases with different benign or malignant etiologies. Iran is one of countries with a high incidence rate of esophageal cancer. The aim of this study was to evaluate the common causes of dysphagia for earlier diagnosis and treatment of this disease and reduction of its morbidity and mortality rate. Materials and Methods: In this descriptive study, we analyzed the etiology of dysphagia in 200 patients who were admitted to ENT and thorax surgery wards of Mashhad Ghaem Hospital during 2005-2007. Results: Of 200 patients, 79 patients were female and 121 patients were male. The most prevalent cause of dysphagia in these patients was esophageal SCC and the most common endoscopic presentation was the ulcerative view. Other common etiologic factors were esophageal stenosis, adenocarcinoma, mediastinal tumors, achalasia, lyomyoma, sarcoma and diffuse esophageal spasm, respectively. Conclusion: According to these results, the complaint of dysphagia with or without odinophagia has particular clinical importance, especially in our country with high frequency of esophageal malignancies.

Objective: The aim of this study was to determine the false positive ratio of Tilt Table Test (TTT) result by using TNG spray (Sub–lingual; SL) as compared to TNG pearl in patients referred to military service. Material and Methods: This was a prospective study. It was conducted on 110 cases referred for military service, expressed vasovagal symptoms. We divided the subjects into three groups; first Group (60 cases) used TNG pearl for provoking syncope in TTT, Group 2 (50 cases) and Group 3 (control cases) used TNG spray in the same dose (0.4 mg). Results: In the first step of tilt study, 10%, and 8% of subjects had fainted on not using provoking drug in cases and controls, respectively.
